Update on HCGC projects An update on work that is happening collaboratively in our community was provided to participants to help them connect the dots. You can access the update slides used here. Collaborative Shaping Process: Where is a good place to start to reduce variation in the Medical Neighborhood around diabetes and/or depression management?  Care Coordination for diabetes and/or depression Ex: Alignment of referral for continuity of care within the MN once a patient is diagnosed with diabetes and/or depression  Education around diabetes and/or depression Ex: Alignment of literature provided to patient upon diagnosis of diabetes and/or depression  Screening for Depression and/or Diabetes Ex: Alignment of the screening tool utilized by members of the MN to diagnose depression, i.e. PHQ2, PHQ-9, etc.  Self-Management programs for diabetes and/or depression Ex: Alignment of diabetes and/or depression self-management programs in the MN

Key reflections from activity  Screening should be aligned. We just need to get started  Screening tool should be educational for professionals  Self-Management, education, and prevention all go hand in hand.  Should consider doing preventative and proactive screenings. What questions are we asking?  Sometimes we will have to lose money to do the right thing  Standardize the messaging that happens around these conditions  Suggest creating a neutral, centralized repository for community standardized screening and education resources  We have to change from being reactive to becoming more proactive. Best way to do that is to prevent Closing Participants will continue to be engaged in the process of shaping the work of impacting chronic conditions in the Medical Neighborhood. Results of the value survey administered at the conclusion of the session are listed below. Always helpful to hear about national perspective and local collaborative efforts Always valuable to bring diverse organizational representatives together. Beneficial to hear others views and share ideas BH organizations want primary care to screen for depression but don’t always do it themselves Consider integrating service level provider who might bring a different perspective Enjoy networking opportunity Enjoyed the discussions and group activity. This was my first meeting and I really enjoyed it. Group activity and discussion was valuable Helpful to have a review of current work to understand how it all fits together How to reduce variation without impacting innovation Learned some things I did not know before Loved starting with participants sharing their passion. So good to increase positivity and remind us what is really important Variation exercise was great, interesting to participate as a MNLG participant and then today and see the stark differences Very well done, thank you
