About Us The Northern Virginia Fall Prevention Coalition (NVFPC) is a 501c3, not-for-profit organization dedicated to reducing the growing number of falls and fall-related injuries among older adults throughout Virginia. The Coalition was formed in November 2008 to promote a new era of collaboration between members, including health care providers, community service providers, government agencies, and local business owners who serve the aging population every day. The NVFPC conducts free fall prevention seminars and distributes thousands of pieces of fall prevention

literature throughout the year to heighten public awareness to this issue, and to promote the effectiveness of prevention measures. The NVFPC follows the Centers for Disease Control & Prevention (CDC) Preventing Falls program, a nationwide initiative that advocates the creation of community based organizations to educate the public about fall prevention. 4 things YOU can do to prevent falls 1. Begin a regular exercise program Exercise is one of the most important ways to lower your chances of falling. It makes you stronger and helps you feel better. Exercises that improve balance and coordination (like Yoga or Tai Chi) are the most helpful. Lack of exercise leads to weakness and increases your chances of falling. Ask your doctor or health care provider about the best type of exercise program for you. 2. Have your health care provider review your medicines Have your doctor or pharmacist review all the medicines you take, even over-the-counter medicines. As you get older, the way medicines work in your body can change. Some medicines, or combinations of medicines, can make you sleepy or dizzy and can cause you to fall. 3. Have your vision checked Have your eyes checked by an eye doctor at least once a year. You may be wearing the wrong glasses or have a condition like glaucoma or cataracts that limits your vision. Poor vision can increase your chances of falling.

4. Make your home safer About half of all falls happen at home. To make your home safer: • Remove things you can trip over (like papers, books, clothes, and shoes) from stairs and places where you walk. • Remove small throw rugs or use double-sided tape to keep the rugs from slipping. • Keep items you use often in cabinets you can reach easily without using a step stool. Have grab bars put in next to your toilet and in the tub or shower. • Use non-slip mats in the bathtub and on shower floors. • Improve the lighting in your home. As you get older, you need brighter lights to see well. Hang light-weight curtains or shades to reduce glare. • Have handrails and lights put in on all staircases. • Wear shoes both inside and outside the house. Avoid going barefoot or wearing slippers.

Tips for Exercising • Wear comfortable clothes and shoes • Drink water before and after exercise • Do exercises slowly and gently • If you feel pain, STOP that exercise and discuss it with your doctor before resuming • If you feel breathless or dizzy, STOP and rest

Falls are the leading causes of injury deaths in this age group. Approximately 50% of those hospitalized for fall-related hip fractures cannot return home or live independently after their injuries. (Centers for Disease Control, 2009.) Many report, “it happened so quick,” it was just a moment. Balance is an integral part of every day activities most people take for granted, such as getting dressed, walking to the car, and reaching into a cabinet. Just about everything we do throughout the day requires some element of balance control. Most people don’t even think about it…. Until there is a problem. Suddenly even the simplest of tasks become difficult, and in some cases terrifying. What can you do to make sure this doesn’t happen to you? Your risk for falls depends on several factors, the first of which is medication management. A common side effect of many medications is drowsiness and/or light-headedness. Interactions between medications, even over-the counter medications may produce these symptoms as well. Be sure to have an annual check of your medications by your doctor or pharmacist to ensure you are not at risk for side effects that may increase your risk for falls. Additionally, effective balance control relies on information from three balance sensory systems; the eyes, the inner ear (Vestibular System), and the muscles/joints of the body. The brain receives this information and coordinates the body’s movements necessary to maintain balance. Participation in a balance focused exercise program prescribed to you by a balance specialist will help your body to capitalize on its stronger balance sensory systems, and work to train their less effective ones. You should also have an annual vision check to ensure accurate visual input. Furthermore, participation in a daily exercise/walking program will help to ensure you maintain adequate range of motion and strength in your legs to ensure that your body can respond efficiently to any challenges to balance you may encounter. Finally, ensure your home environment is not placing you at an increased risk for falls. Remove any loose items

from the floor such as clothes/shoes or throw rugs that you can easily trip over. Install grab bars in bathrooms when indicated, and make sure your hallways and stairwells have adequate lighting. As our body matures there are many changes to our eyes and vision which can put us at increased risk for falls. These changes can be as a result of disease or just normal age related changes. Some of the changes can be corrected easily, while others require lifestyle or environmental modifications. However, it is extremely important not to ignore any of the changes because many times falls can be a life changing incident. The following table presents many of the common age related vision changes which can occur and what can be done about them. It is not meant to act as a diagnostic instrument, nor take the place of regular visits to your eye doctor. The minimum eye exam frequency for people over the age 50 is once a year. Condition

Note from the Chairman

Another year has passed and the Northern Virginia Fall Prevention Coalition’s (NVFPC) message “Falls are NOT an inevitable part of aging, falls are preventable!” continues to gain momentum. This 501c3 grass roots organization formed in November of 2008, and as of this publication spans 9 Virginia counties. Our unique approach to addressing falls suffered by our elder Virginians has fostered a new environment of cooperation between organizations serving elder Virginians and their families. Regional hospitals, physicians, nonprofit organizations, Area Agencies on Aging, rehabilitation centers, home care/home health care agencies and senior living communities all stand together in addressing this growing health concern. The following statistics provided by the Division of Injury Prevention highlight the importance of our work. Between 2004 and 2008: • 1,613 Virginians over the age of 65 died as a result of a fall. • There were over 60,000 hospitalizations as a result of a fall. • Hospital costs alone were 1.6 billion dollars. We carried this report to the 2011 Virginia General Assembly in Richmond and the positive response we received indicates further support for this initiative by our

elected officials in the coming year. The NVFPC’s mission is to give every Virginian the opportunity to learn about fall prevention initiatives in their community, and what they can do to prevent a fall. Planning is underway for the NVFPC to provide; fall risk assessment screenings, exercise programs, vision screenings, medication reviews, and home safety assessments in the community. The NVFPC membership has stepped up to provide these crucial services at a time when government is cutting back on services. This is an exciting time as a forum, facilitated by the Virginia Department of Health, Division of Injury Prevention, will be held in Charlottesville to discuss the formation of a statewide coalition with other like minded groups from across the state. We applaud their efforts to hold this important forum. Reducing Falls With Those Who Have Dementia Nancy Dezan, Executive Director, Alzheimer’s Family Day Center Joan is a tiny, 75 year old woman with Alzheimer’s disease who enjoys sitting quietly in her big, overstuffed chair. Hearing laughter outside her room, Joan moves to stand up and join the revelry. In doing so, however, she forgets to uncross her legs and Joan falls to the floor, injuring her arm. An hour later, Joan has no memory of the fall and no clue as to why she is experiencing pain. Incidents like these are all too common among those with dementia. In fact, individuals with Alzheimer’s disease experience more than twice the rate of falls compared to others in their age group. Once the fall has occurred, these individuals are also more likely to suffer a serious injury and have more difficulty recovering. Why? The brain damage that occurs with Alzheimer’s and related diseases can cause a multitude of factors that contribute to the risk of falling. Even in the earliest stages

of the disease, individuals complain of difficulty with balance and coordination. They may frequently bump into furniture or have the feeling of falling when climbing a staircase. Visual perception is compromised, and the individual may not recognize cracks in the sidewalk or a small pet in their path. Many of the medications used to reduce anxiety or calm agitation, have side effects that cause poor balance, restlessness and rigidity. The common dementia symptoms of poor judgment combined with impulsive behavior can also lead to risky behavior and falling. For the caregiver, prevention is the key. The most successful intervention is to determine the specific cause of the fall. For example, if the fall is the result of sleep medication, the caregiver should focus on changing medication or finding alternate ways to enhance sleep rather than limiting the individual’s daytime mobility due to fear of falling. In fact, regular exercise is one of the best ways to work toward reducing falls with individuals who have dementia. Physical activity helps to reduce agitation and anxiety, enhance cognitive function, develop strength and balance, and improve sleep. Even mild exercise may be one of the best strategies to begin to reduce falls and injuries with those who have dementia.

In 2010, Loudoun County Department of Fire, Rescue and Emergency Services responded to 1,767 calls for residents who had injuries from a fall. Although each call for service is different, the one thing that the majority of these emergencies had in common is the fact that most of them were preventable. Our role as EMS providers is to stabilize the patient on the scene to the best of our ability and safely transport them to the nearest medical facility for treatment. We have a variety of tools to treat fall injuries from basic splinting and back boarding to advanced pain management options as directed by our local protocols and med control. In addition to providing emergency care, our other role as EMS providers is to educate the public as often as possible to the dangers of falls in the home. This can be done through formal sem-

inars and workshops as well as one on one conversation with residents when we are in their homes. No matter what the setting, fire and rescue realizes the impact an injury from a fall can have on someone’s life and we are committed to helping our community in any way we can.

2010 Fall Prevention Summit Inova Loudoun Hospital hosted the NVFPC’s 2nd annual Fall Prevention Summit on October 14th, 2010. Our distinguished lineup of expert speakers engaged the audience throughout the day with presentations on fall related issues including healthy aging, dementia and Alzheimer’s, and traumatic brain injury. Summit attendees included, elected state officials, state and local government agencies, the local Commissions on Aging, clinicians, private healthcare providers, NPO’s, and private citizens.
