Managing Diabetes So It Doesn’t Manage You each year, Shady Grove adventist and Washington adventist Hospitals discharge approximately 8,000 diabetic patients—close to 50 percent of the total for Montgomery County. Now, the two hospitals have developed a new comprehensive program to help these patients manage their illness.

“Our goal is to provide accessible education through which diabetics can gain the skills needed to successfully manage their condition and avoid complications,” says andrea Ford, bSN, R.N., certified diabetes educator at Shady Grove adventist Hospital. “We will strive to assist patients in achieving optimal health, improving their quality of life and decrease health care costs.” this new initiative allows physicians to follow a patient’s prognosis more effectively by developing a personalized plan with set goals and continual follow-up assistance. Services offered at both hospitals include individual assessments, management classes on proper nutrition and exercise, blood glucose monitoring and medication management. eaglebank has donated $50,000 to help start the program, which is in collaboration with the american association of Diabetes educators.

New system turns minutes into lifesaving moments

cancer patients in upper Montgomery county will soon have access to a new state-of-the-art facility for radiation oncology services. this spring, as part of Shady Grove Adventist Hospital’s ongoing commitment to meet the health needs of upper Montgomery county and to provide comprehensive cancer care services, the hospital is opening a new radiation oncology treatment center in Germantown. radiation oncology requires treatments daily for six weeks, making it vital to provide convenient access for patients. the new facility will be located on Seneca Meadows parkway, just off route 118 in Germantown. “the new center will be equipped with the latest approved, state-of-theart equipment, and will afford people in the upcounty expanded access to quality cancer treatment, close to home,” says don Bridges, M.d., who, along with cathy Salem, M.d., and Anu Singh, M.d., will staff the new site in Germantown in addition to their current rockville office on Gude drive.

Each year, there are approximately 4,000 new cancer cases in Montgomery county. radiation oncology is an integral part of treatment for more than 60 percent of people facing a wide range of cancers, including breast, prostate and some types of lung.

a new communications system for ambulances in Montgomery and Prince George’s counties will help people experiencing a heart attack get faster diagnosis and treatment when they arrive at the hospital. the software system, called LifeNet, allows emergency responders to send electrocardiogram (eKG) results from the ambulance straight to hospital staff to view before the patient arrives. Shady Grove adventist and Washington adventist Hospitals, along with several other hospitals, helped purchase the system for Montgomery County. “this system allows us to more accurately understand what kind of cardiac emergency a patient is experiencing and activate our medical teams for that patient’s immediate treatment upon arrival,” says Drew White, M.D., medical director of the emergency department at Washington adventist Hospital. “there are enormous benefits to early detection of a heart attack,” says Dennis Friedman, M.D., medical director of the cardiac catheterization lab at Shady Grove adventist Hospital. “Rapid diagnosis and treatment means a better chance of survival and a quicker recovery.”

Feet and hips, knees and ankles New surgical technology helps patients return to an active lifestyle in no time Three months after

a total right hip replacement at Shady Grove Adventist Hospital, avid runner and outdoorsman Paul “Rusty” Neal embarked on a backpacking trip without any problems.

“That’s the power of the Hana Table,” says orthopedic surgeon Sridhar Durbhakula, M.D. Shady Grove Adventist Hospital is the first in Montgomery County to use the specially designed Hana Table, which looks more like exercise equipment than a surgical tool.

“It is the only table that allows you to drop the leg and rotate the hip at the natural interval between the muscles for an anterior [front] approach,” Dr. Durbhakula explains. “That means less cutting and less trauma.”

It also means a shorter hospital stay—two to four days, compared with three to 10 days with conventional surgery.

“Some patients can walk the night of their surgery,” Dr. Durbhakula says.

“Within two weeks, many patients are back to work and driving.”

Neal, 55, of Germantown, noticed results almost immediately. “Before the surgery, my leg was at center,” he says. “I could not rotate more than five degrees. The day after the surgery, I rotated my hip and made my foot turn outward. I said to my wife, ‘Look at my foot! That doesn’t hurt!’”

A few weeks after surgery, Neal only needed his cane on long walks.

Physical therapy is helping him build up muscle to keep up with the greater range of motion his new hip allows.

It was a very good experience,” Neal says.

It’s an experience afforded to all patients at The Joint Center at Shady Grove Adventist Hospital. It begins with the joint replacement program— “boot camp”—which is a class before surgery to help patients understand what to expect during and after the procedure, from managing pain to putting on socks.

Certified orthopedic nurses also help with postoperative inpatient support and care plans for recovery at home.

The Hana Table is one more tool in Shady Grove Adventist Hospital’s state-of-the-art surgical suite that enhances the latest advancements in knee and hip replacements provided in its Joint Center.

“Age is not a criterion for the Hana Table,” Dr. Durbhakula says. “If a patient needs relief from arthritis or hip pain, the anterior approach can provide that relief with a quicker recovery. I am seeing amazing results.”

Needing Knee Replacement Wolf met with Zohair Alam, M.D., an orthopedic surgeon at Washington Adventist Hospital in Takoma Park. “The patient’s motivation to be active is a very important factor I consider when deciding if surgery is the right option,” Dr. Alam says. “When I met Elaine, it was clear her knee pain was holding her back from living the life she wanted to live.”

Most orthopedic experts consider replacement the best method of handling advanced arthritis in the knee or hip. Before having joint replacement surgery at Washington Adventist Hospital, patients attend the hospital’s total joint replacement class where they meet other people considering surgery, learn what to expect during their hospital stay and gain tips for a quick recovery.

“Patients are surprised when I tell them that the day after surgery, a physical therapist will get them up and walking,” Dr. Alam says.

The majority of patients having knee replacement surgery stay in the hospital three or four days. During that time, physical and occupational therapists work with patients to create a rehabilitation program to meet their needs and get them back to health quickly.

Bright Future After Surgery Dr. Alam performed total knee replacement surgery on Wolf in June. She was surprised how soon after surgery the arthritis pain in her knee was gone. “Dr. Alam and his team did a miraculous job,” she says. “Everyone was so encouraging every step of the way. They didn’t treat me like ‘just another knee.’ They treated me like a wife, mother and grandmother who just wanted a normal, pain-free life.”

With her new knee, Wolf feels the future is bright. “I’d love to do a walking tour of Italy, and of course there will be trips with my granddaughters and lots of soccer games.”

She’ll be the one cheering from the sidelines.

After more than five years of constant pain in her right knee, Elaine Wolf decided it was time to consider orthopedic surgery.

“My knee wasn’t responding to medication or therapy,” she says. “I was at a place where I knew if I didn’t do something, I wouldn’t be able to get around on my own much longer.”

At 68, Wolf, a retired nurse who lives in Colesville, Md., wasn’t ready to give up her active life. Her love of travel and desire to volunteer factored into her decision to choose knee replacement surgery. But her greatest motivation was the desire to keep up with her two granddaughters.

“I would go to their soccer games and have to watch from the top of the hill because I couldn’t make it down to the field,” Wolf says. “I felt cheated.

While he is one race shy of his lifelong goal to run in five Marine Corps Marathons—“I’ve been advised it might not be wise to run that fifth”—Neal says that after years of pain, he would be happy to jog a couple of miles.

Today, Neal is using his experience to create a nonprofit organization to make the outdoors accessible to everyone. He has already worked with a wheelchair manufacturer to develop an outdoor wheelchair that the U.S. Forest Service approved for use in all national parks.

Interventional radiology uses image guidance to combat pain without surgery

“It makes a big difference,” Dr. Erkmen says. “It really improves people’s quality of life.”

Less Invasive, Faster Healing Deborah Williams of Mount Airy, Md., agrees. She underwent a uterine artery embolization (UAE) with Dr. Erkmen to relieve painful fibroids, which are noncancerous tumors that can develop in the uterus. As many as one in five women will develop fibroids before reaching menopause.

“I am a very active person, but before my procedure, I didn’t feel like leaving the house sometimes. It was wreaking havoc on my life,” Williams says.

“Now, I can go running, hiking, kayaking—whatever I like, whenever I like.”

Dr. Erkmen says fibroids are the No. 1 reason women consider a hysterectomy, to alleviate heavy bleeding, cramping and other symptoms. “But instead of a six-week recovery time with a hysterectomy, it’s just one week with a UAE,” she says. “It’s a great option for women who don’t want surgery.”

Williams tried prescription medications and other alternatives before consulting with Dr. Erkmen: “She really listened and wanted to help me,” Williams says. “It felt very personal.”

feel like a juggling act to keep work, families and social lives all in the air. For some women, dealing with a personal health issue such as osteoporosis, reproductive pain or infertility—and a lengthy recovery time if surgery is necessary—is enough to throw everything off balance.

Thankfully, interventional radiology procedures are available that may provide relief for these types of medical conditions, with shorter recovery times.

“We’re the minimally invasive specialists, the alternative to open surgery,” explains Zahide Erkmen, M.D., an interventional radiologist at Shady Grove Adventist Hospital.

Interventional radiologists use image guidance such as X-rays, ultrasounds or CT scans to make the smallest incisions possible for targeted treatments.

These include inserting a special tube for delivering chemotherapy, performing angioplasty to improve blood flow and inserting catheter-delivered stents to clear clogged arteries.

Dr. Erkmen has been performing minimally invasive UAE for 13 years at Shady Grove Adventist Hospital, allowing women to receive this specialized treatment and relief close to home, in their own community.

She is also able to help some women who have compression fractures because of osteoporosis. “Compression factures are very debilitating, causing extreme pain,” she says. “People are usually bedridden, then go through a downward spiral.”

In these cases, Dr. Erkmen can perform vertebroplasty and kyphoplasty, where she injects a special type of medical cement into the broken vertebra.

This stabilizes the fracture and helps with the pain, allowing patients to get out of bed and resume a more normal life.

“It’s very gratifying when one of my patients tells me what a difference the procedure I’ve performed makes in her life,” Dr. Erkmen says.

Heart Care Goes SMaLL SCaLe lmost nothing can be scarier than hearing your doctor say you need heart surgery. New technology and advancements, however, allow surgeons to repair a patient’s heart through a smaller incision—a procedure known as minimally invasive heart surgery.

“Heart surgery today isn’t the same as heart surgery a decade ago,” says Paul Massimiano, M.D., a heart surgeon and medical director of cardiac surgery at Washington Adventist Hospital in Takoma Park.

When a minimally invasive approach is used, patients receive the same operation as they would with a traditional, “open” surgery. The only difference is in the approach and tools used. Patients, who once had to endure a long incision scar down the center of their chests and a painful, lengthy recovery, now have a small, 5- to 6-centimeter chest incision.
