A

long wait at the pharmacy to fill a new prescription is something you come to expect, but it is often the last thing you want to do after leaving the hospital.

“Many patients go straight home to continue their recovery and don’t get their prescriptions filled right away,” says Tom Sweeney, vice president and chief nursing officer at Washington Adventist Hospital. “They might not think it’s a big deal, but delays in taking prescribed medicines or missing a dose can result in slowed recovery or even a return to the hospital.”

To tackle this issue and to help ease the transition from hospital to home, Washington Adventist Hospital and Walgreens have teamed up to provide patients with bedside delivery of their prescription medications.

Washington Adventist Hospital is the first hospital in Montgomery County working with Walgreens to provide this valuable service.

“When a patient requests the Walgreens bedside delivery service, I meet with them to get all the correct paperwork filled out, I facilitate a consultation with the pharmacist, and I make sure they have their medication in hand when they leave the hospital,” says Gina Williams, Walgreens pharmacy technician at Washington Adventist Hospital. “In addition, a Walgreens pharmacist will place a follow-up call to the patient 48 hours after discharge to answer any questions that may have come up about their new medication.”

The program helps ensure patients have the medication needed to stay on the road to recovery after they leave the hospital.

“Waiting at the pharmacy for my prescriptions to be filled was the last thing I wanted to do after leaving the hospital,” says Jessica Tiggs, a former patient at Washington Adventist Hospital who used the bedside delivery program. “This service was so convenient. Not only did I get all my medications before I went home, but I had a consultation with the pharmacist who was so patient in answering all my questions about my new prescriptions. I found that really helpful.”

Williams says, “Patients often seem surprised when I tell them I can deliver their prescriptions directly to their room.”

Adds Sweeney: “Washington Adventist Hospital and Walgreens are dedicated to providing convenience and accessibility to the communities we serve, and this bedside medication delivery service takes that care to another level.”

Serving Up Healthy Advice The oncology dietitian at Shady Grove Adventist and Washington Adventist Hospitals offers valuable nutrition guidance for cancer patients Nutrition plays an important role

during cancer treatment and recovery. Eating well can help people who have cancer feel better and stay stronger while they battle their illness.

The cancer care teams at Shady Grove Adventist and Washington Adventist Hospitals include an outpatient oncology dietitian to help those who have cancer eat as healthfully as possible and stay well nourished during their cancer journeys.

Cynthia Clark, M.S., R.D., CSO, LDN, the coordinator of oncology nutrition for both hospitals, is one of only seven licensed oncology dietitians in Maryland.

“I work with our cancer patients to find creative solutions to help maximize their nutritional status before, during and after cancer treatments,” she says.

Navigating Through the Cancer Journey Wading through a cancer diagnosis and doctors’ appointments on top of everyday life can take its toll emotionally, physically and financially. Adventist HealthCare’s cancer care navigation team is there to help. Whether it’s arranging for a ride to an appointment, answering questions about diagnoses and care plans, or connecting patients to a variety of resources, the navigation team is an advocate for the local community.

Alternative Pathway for Heart Catheterizations

Each year, more than 1 million cardiac catheterizations to diagnose heart disease are performed in the U.S., and for most of them, doctors enter through the groin to gain access to heart arteries. Today, interventional cardiologists at Washington Adventist and Shady Grove Adventist Hospitals are performing more and more heart catheterizations by entering through the wrist, also known as transradial access. The transradial approach can reduce the risk of complications and improve patient results. Washington Adventist Hospital is one of the leading hospitals in the area offering this approach. “Patients prefer the transradial approach because it enables them to be mobile much faster and they tend to have less overall discomfort,� says David Brill, M.D., medical director of the cardiac catheterization lab at Washington Adventist Hospital and a cardiologist at both hospitals.

A More Compatible Pacemaker

On Feb. 16, 2011, Washington Adventist Hospital became the first in the Mid-Atlantic region to implant a new “MRI-safe” pacemaker. The procedure, performed by James Cockrell, M.D., electrophysiologist, came a week after the U.S. Food and Drug Administration approved the first-of-its-kind pacemaker specifically designed to be compatible with magnetic resonance imaging (MRI) exams.

Electrophysiologists at Washington Adventist and Shady Grove Adventist Hospitals offer this device to patients. There is an estimated 50 to 75 percent chance that a patient with an implanted heart device will need an MRI. “Providing pacemaker patients with a device that is MRI-safe means better and safer detection and treatment of serious medical conditions, such as vascular disease, stroke, cancer and orthopedic injuries,” says Sung Lee, M.D., medical director of electrophysiology at Washington Adventist Hospital. “It’s revolutionary for our patients.”

When other treatments have failed to relieve persistent angina, or chest pain, doctors at Shady Grove Adventist Hospital offer patients a nonsurgical treatment option called enhanced external counterpulsation, or EECP. “During EECP, pressure cuffs are placed on the legs and buttocks,” says Dennis Friedman, M.D., medical director of cardiovascular services and cardiac rehabilitation at Shady Grove Adventist Hospital. “The cuffs are timed to inflate and deflate with each heart beat, sending blood to the heart and improving blood flow and circulation.”

This contributes to the relief of angina symptoms. Shady Grove Adventist Hospital is the only hospital-based EECP program in Montgomery County.

EECP is not invasive, does not require a hospital stay and allows patients to return to their normal routines each day after treatment.

Valve Surgery a Cut Above the Rest

New technology and advancements in medicine allow heart surgeons to repair or replace a diseased heart valve through smaller incisions.

“During minimally invasive valve surgery, the patient receives the same operation as they would with a traditional, ‘open’ surgery,” says Paul Massimiano, M.D., heart surgeon and program director of cardiac surgery at Washington Adventist Hospital. “The difference is in the approach and tools used. Incisions are very small, only 5 to 6 centimeters long.”

For patients, this means a quicker recovery with less pain and a faster return to normal activity. Patients who have minimally invasive valve surgery can sometimes go home in as little as a day or two.

t started as tingling in his shoulder and continued as discomfort in his chest. Then his heart began racing.

At just 41 years old, Parag Parikh of Germantown was suffering a heart attack.

Despite a family history of heart disease, Parikh wasn’t sure he was having an attack. “I called 9-1-1 to be safe,” he says.

Within minutes, paramedics had performed an electrocardiogram (EKG) and sent the results directly to physicians at Shady Grove Adventist Hospital, thanks to LifeNet—a communications system newly installed in all Montgomery County ambulances that sends critical medical information to physicians while patients are en route to Shady Grove Adventist Hospital and Washington Adventist Hospital.

“This means better, faster treatment. The sooner a heart attack patient is treated, the better the chance for survival and a quick recovery,” says Michael Castine, M.D., Parikh’s interventional cardiologist. When EMS transmitted Parikh’s EKG, the hospital was able to activate the cardiac catheterization laboratory before he arrived.

Shortly after his arrival at Shady Grove Adventist Hospital, Parikh experienced cardiac arrest but was quickly resuscitated and rushed to the catheterization lab, where Dr. Castine performed an emergency percutaneous coronary intervention, or angioplasty, to relieve a blockage in one of his heart arteries.

When Parikh woke up in the Intensive Care Unit, he learned “the nurses were nice enough to call my parents, who were 8,000 miles away in India, and notify them. Because of that, I knew I was in good care— and so did my family,” he says.

For Parikh, his heart attack was a wake-up call to improve his health.

The cardiac rehabilitation team at Shady Grove Adventist Hospital helped him focus on heart-healthy exercise and lifestyle.

“My cardiologist checks in on my rehab routine and the exercise physiologists monitor my heart rate and take my blood pressure,” he explains.

Cardiac rehab following a heart attack has been shown to reduce heart disease risk factors and improve quality of life, emotional stability and the ability to carry out daily activities. The Cardiac Rehabilitation Centers at Shady Grove Adventist Hospital in Rockville and Washington Adventist Hospital in Takoma Park are nationally certified exercise and education programs by the American Association of Cardiovascular and Pulmonary Rehabilitation.

“There is a multidisciplinary team dedicated to rehabilitating patients after a cardiac event,” says Dennis Friedman, M.D., medical director of cardiovascular services and cardiac rehabilitation at Shady Grove Adventist Hospital. “Cardiologists, registered nurses, physiologists, dietitians and certified diabetes educators work together to improve the patient’s quality of life and to prevent the progression of heart disease.”

As he works to improve his health and prevent another heart attack, Parikh has also met with a cardiac rehab dietitian, plans to keep exercising and may join some of the free education programs offered by the Cardiac Rehab Center.

“Cardiac rehab, this entire experience,” Parikh says, “is helping me regain my confidence, physically and emotionally.”

Expert Care Surgery improves quality of life for Landover Hills woman

H

aving managed congestive heart failure and heart valve disease with medication for more than a decade, 45-year-old Denise Wooten of Landover Hills had a life-changing event persuade her it was time to consider heart surgery.

“My brother passed away from congestive heart failure,” Wooten says.

“I saw a photo of him in the hospital the day before he passed and he didn’t even look like himself. It broke my heart, and I told myself I couldn’t let that be me. It was a turning point.”

Sridhar Chatrathi, M.D., interventional cardiologist at Washington Adventist Hospital, has been seeing Wooten for the past 12 years.

“We talked about surgery several years ago, but Denise wasn’t ready,” Dr. Chatrathi says. “We continued treating her conditions with

medication, but more recently, after running some tests, I told her now was the time to seriously consider surgery. She agreed.”

Paul Massimiano, M.D., heart surgeon and program director of cardiac surgery at Washington Adventist Hospital, then got involved in Wooten’s care. Because of her complicated heart condition, Dr. Massimiano and his team performed double valve surgery as well as a maze procedure for atrial fibrillation.

“Denise did great, and I expect her quality of life to greatly improve,” Dr. Massimiano says.

Wooten adds: “I knew Dr. Chatrathi would send me to the best. I trusted my heart care to Washington Adventist Hospital, and it was the best decision I could have made.”

For Wooten, the surgery was worth the quality of life she has gained in recent months. “I wish I’d had surgery sooner,” says Wooten, who is back to work and enjoying her newfound endurance. “My brother didn’t have surgery. If he had lived closer, I could have introduced him to my doctors. They would have helped him make the right decision.”

Wound Care Centers Offer

Specialized Treatment Do you or a loved one

have a wound that simply will not heal?

About 5 million Americans have chronic wounds, including those that result from diabetes or other conditions, according to the Center for Wound Healing and Hyperbaric Medicine.

Although many wounds heal on their own, some require specialized treatment by clinical experts. The Wound Care Centers at Shady Grove Adventist Hospital and Washington Adventist Hospital can help.

These centers use the latest medical advances to treat chronic wounds.

Additionally, the centers also assess factors, such as nutrition, medications and medical conditions, that may affect a person’s ability to heal.

Physicians create a specialized treatment plan based on the patient’s medical history and health. The plan includes progress checks at the Wound Care Centers.

“In addition to treatment, patients receive detailed instructions on home care, bandage changes and how to protect the wound from further injury,” says Tuesday Cook, M.D., medical director of the Washington Adventist Hospital Center for Advanced Wound Care and Hyperbaric Medicine. “We also discuss your treatment and progress with your doctor.”

Medical Management Adventist HealthCare helps physicians embrace electronic medical records

he challenges of trying to decipher a doctor’s handwriting or remembering which tests you had at your last appointment could soon be a thing of the past as more physicians enter the world of online medical management. In the health care field, electronic medical record (EMR) technology is making it easier for physicians and patients to keep track of their health and medical history.

“No matter where a physician provides care—in an office setting or within the hospital environment—it is important to maintain a continuum of care for patients,” explains Gaurov Dayal, M.D., chief medical officer for Adventist HealthCare. “EMR technology can help coordinate care between the doctor’s office, the patient’s home, and outpatient and inpatient settings that meets a wide range of health needs.”

In response to 2009 health care reform law, community physicians are being encouraged to implement EMRs in their practices over the next several years. Under the new law, physicians are eligible to receive thousands of dollars in federal incentives provided by the Centers for Medicare and Medicaid Services (CMS) by reaching a set of CMS measures used to show how quality of care has been improved for patients. To help Adventist HealthCare’s more than 1,700 community physicians reach this goal, the organization offers the innovative ACES (Ambulatory Care EMR Solution) Program.

ACES is designed to enhance the quality of patient care and help medical offices perform more efficiently. Electronic records can help physician offices schedule appointments, type patient notes, order tests, fill prescriptions and handle

“Since I am now online, as I see patients I am able to print out and give them the latest information about their condition, which would have taken a lot more time before,” notes Sally Jo Belcher, M.D., a Shady Grove Adventist Hospital family medicine physician.

“I can print visit summaries with recommendations for medications and treatments that patients would otherwise have just had to remember, or have written down themselves. For example, I recently printed out a list of high-potassium foods for a patient who needed to supplement her potassium.”
