As our nation works to expand access to health care, Adventist HealthCare is constantly working locally to continue its mission of improving the health of people and communities. Our work extends beyond the walls of our hospitals—Shady Grove Adventist in the Gaithersburg/Rockville area and Washington Adventist in Takoma Park. In fact, last year, our health and wellness services touched more than 70,000 residents through health screenings and education programs.

While there is not a cure for breast cancer, early detection through breast selfexams and mammograms can save lives. For the past 18 years, quality cancer care programs at Shady Grove Adventist and Washington Adventist Hospitals Gaurov Dayal, M.D. have been working to save the lives of women in our community by providing free mammograms. Now, a nearly $1 million, four-year grant from Susan G. Komen for the Cure is allowing us to expand the Navigate to Health: Rapid Referral Program, which is a great example of how we are working to expand access to breast screenings and care for women. You can read more about this important program on page 10.

Adventist HealthCare’s community-based programs deliver health programs through senior centers around the area. Whether we are in Rockville, Damascus, Takoma Park or Silver Spring, our goal is to not only provide quality care to the aging members of the community in our facilities, but to also help them access health screening, information and services in places that they visit weekly or even daily.

Programs like these are a vital part of our mission and are ingrained in our daily work. Watching Out for Heat exhaustion and Heatstroke Keep your family healthy when temperatures rise by knowing the signs of heat exhaustion and heatstroke.

Heat exHaustion Heat exhaustion results from the body overheating after excessive physical activity or exposure to high temperatures (with high humidity levels). Infants, children younger than 4 and adults older than 65 are at a higher risk for heat exhaustion. Scott Freedman, M.D., medical director of the pediatric emergency department at Shady Grove Adventist Hospital, notes the following symptoms: ◗◗ Heavy sweating ◗◗ Weak, rapid pulse ◗◗ Faintness ◗◗ Muscle cramps ◗◗ Nausea ◗◗ Headache “If you notice any of these symptoms, particularly in young children or the elderly, stop activity and move to a cooler place immediately,” Dr. Freedman says. “Provide the person with water to drink, and if the symptoms continue or worsen over the next 60 minutes, contact a doctor or seek care at the closest emergency room.”

Jamie Futterman, R.D., L.D., CDe, outpatient dietitian at Washington adventist Hospital, offers tips to provide simple, wholesome snacks when your family is on the go.

“Nutritious snacks act as ‘mini meals,’ supplying the nutrients lacking in other meals,” Futterman says. “Look for snacks that offer a combination of high fiber, carbohydrates, healthy protein and minimal fat.”

Futterman recommends the following snacks, packed with nutrition and a bit of fun. Some are nonperishable and will keep for a few hours or days. Others may require an insulated lunch bag with a reusable ice pack to stay cool: ◗◗ Whole-grain rice cake made into a “face” spread with cream cheese or peanut butter, and then topped with fruit (blueberries for the eyes, a strawberry for the mouth, a grape for the nose) ◗◗ ants on a log: celery with almond butter or cream cheese and raisins (to look like ants) ◗◗ Fruit kebabs with vanilla yogurt for dipping (for young kids, use round-edged plastic chopsticks) ◗◗ applesauce cups (no added sugar) or fruit cups (packed in juice) paired with a piece of cheese

Heatstroke When the body’s temperature rises above 104 degrees, a potentially fatal condition called heatstroke may be the cause. When temperatures are high, engaging in strenuous activities and extended exposure to heat can cause heatstroke.

Young children and the elderly are at high risk.

Experts suggest seeking immediate medical care if the following symptoms appear: ◗◗ High body temperature (104 degrees or higher), the main indicator ◗◗ Lack of sweating ◗◗ Racing heart and strong pulse ◗◗ Neurological symptoms including seizures and loss of consciousness ◗◗ Muscle cramps or weakness Heatstroke is a serious and life-threatening condition that requires immediate medical attention. It can happen in an instant. A loved one loses the ability to move one side or starts slurring speech.

Every 40 seconds someone suffers from a stroke, which is also known as a “brain attack.” It is the third leading cause of death in the U.S. and the leading cause of adult disability.

During a stroke, a blood clot blocks an artery or a blood vessel breaks, interrupting blood flow to an area of the brain. As a result, brain cells can die and brain damage can occur.

When a stroke occurs, time is critical to saving brain cells.

“If you suspect that a loved one is having a stroke, stay where you are and dial 9-1-1 immediately,” says Margaret Nash, R.N., stroke program coordinator at Shady Grove Adventist Hospital.

David Srour, M.D., chief of emergency medicine at Shady Grove Adventist Hospital, explains: “Most strokes are caused by a clot that forms in a narrowed blood vessel or breaks off from the heart or somewhere in the body, cutting off blood and oxygen to the brain. Or a weakened blood vessel could rupture, causing bleeding in the brain.”

As a designated primary stroke center, Shady Grove Adventist Hospital has a stroke team on-call 24 hours a day.

stroke nurse responder, the bedside nurse and the radiologist respond within minutes.

“Throughout our hospital, we provide a system of quality care to stroke patients,” Dr. Srour says. “Our dedicated stroke team rapidly identifies stroke patients and begins treatment quickly to improve outcomes and minimize the chances of long-term disability.”

Within minutes of arriving at the hospital, a patient’s CT (computed tomography) scan is read, labs results are available and—if the patient is a candidate—a clot-busting therapy called tissue plasminogen activator, or tPA, is administered. Stroke patients can receive tPA three hours from the start of symptoms to reduce long-term disability for the most common type of stroke.

Once patients are admitted to the hospital, the coordinated care continues. Nash meets with each patient admitted to Shady Grove Adventist Hospital’s dedicated stroke unit, which is staffed with specialized nurses, speech therapists, physical therapists and the nurse diabetes educator.

In addition to knowing the signs of a stroke (see “Act FAST”), experts advise understanding risk factors such as high blood pressure, cholesterol, diabetes, smoking and irregular heartbeat.

“Lifestyle risk factors can be managed with weight loss, a healthy diet and medications,” Dr. Srour says. “This is also especially important for people with risk factors they cannot change, such as family history.”

Act FAST Symptoms of stroke include numbness of the face, arms or legs, especially on one side; loss of balance; trouble speaking or walking; difficulty seeing in one or both eyes; and severe headache.

Pioneering blood therapy helps mom deliver a healthy baby

“I am a blessed woman,” she says. “Shady Grove Adventist Hospital gave me something I never thought I would have based on my medical history—a family.”

Her youngest, Christopher, was delivered in November after Shady Grove Adventist Hospital became the first in Maryland to use a treatment called Thrombate III to counteract the hereditary condition antithrombin III deficiency.

“It’s a rare blood deficiency, where a patient could be at 75 percent lifetime risk of clotting,”

explains Sheri Hamersley, M.D., an obstetrician at Shady Grove Adventist who specializes in maternal fetal medicine. “These risks are increased during pregnancy, delivery or any surgical procedure.”

Obstetrician Michelle Spector, M.D., delivered Christopher and Dr. Hamersley co-managed the high-risk case.

Thrombate III therapy involves providing a specific preparation of intravenous medication to the patient.

“It was exciting, because it was the first time we gave a factor-specific treatment to prevent a blood clot, as opposed to just a blood thinner in general,” Dr. Spector says.

Faison-Ricks took blood thinners during her pregnancy with her older son, Dominick, but she had to be taken off the medicine 48 hours before she delivered him in 2008. “He was a cesarean,” Faison-Ricks says. “There was a fear that if I had that surgery, my blood would clot.”

With her second son, Faison-Ricks and Dr. Hamersley petitioned to get Maryland’s first-ever obstetrical Thrombate III therapy approved by insurance. “It was very reassuring that with the treatment,” Faison-Ricks says, “I wasn’t going to have the clotting problem with Christopher.”

Both of Faison-Ricks’ sons spent time in Shady Grove Adventist Hospital’s Level IIIB Neonatal Intensive Care Unit, where she says they received excellent, compassionate care. Today, Dominick, 2 years old, and Christopher, 6 months, are thriving. Perhaps most important, Dr. Hamersley notes, is the fact that a possible health factor for both boys has been identified and can be monitored in the future.

“Since antithrombin III deficiency is inherited, you are not only prolonging the patient’s life with the diagnosis, you could be diagnosing family members,” Dr. Hamersley says. “You are decreasing their lifetime risk of clotting and dying at an early age.”

Adds Dr. Hamersley, “This innovative therapy allows us to approximate the right recipe to make sure the blood is not too thick and not too thin.”

Dr. Hamersley is also the physician who diagnosed Faison-Ricks’ antithrombin III deficiency.

After the diagnosis, Faison-Ricks traveled from her home in Charles County, Md., to receive care from Dr. Hamersley. “I didn’t mind driving those extra miles because I was confident Dr. Hamersley could help ensure a successful, healthy pregnancy,” Faison-Ricks says.

With her second son, she also began meeting with Dr. Spector for prenatal care. “Despite all her risk factors, she was always positive,” Dr. Spector says. “She would say, ‘I’m going to do whatever I need to do to have a healthy baby.’”

Over the past eight years, Faison-Ricks and her husband, Donald, experienced a heartbreaking string of pregnancy losses.

“We kept asking ourselves, ‘Why does this keep happening?’ There had to be a reason,” Faison-Ricks says. “I never knew why until Dr. Hamersley ran those blood tests.”

“My wife and I were nervous,” Kearns says, “but we educated ourselves about the procedure, which helped ease our fears. We trusted Dr. Massimiano, and the quicker I could get back to normal life, and work, the better.”

Kearns was surprised at just how fast he recovered. These days he’s working out on the treadmill to rebuild his stamina and hopes to take up hiking again with his wife.

“Not that I’d ever wish to have heart surgery again,” Kearns says, “but if I had to, I wouldn’t change a thing.”

juggling meetings, clients and court cases, John Kearns of Silver Spring couldn’t fathom taking time off for heart surgery. “My friend had heart surgery and it took him months to recover,” he says. “I wasn’t sure how I was going to manage that.”

Luckily, Kearns was a candidate for minimally invasive valve surgery at Washington Adventist Hospital—an option that had him home in one day and back to work a few weeks later.

“It all started with a heart murmur,” Kearns says. “I was told in the past it was nothing unusual, nothing to worry about, but now my cardiologist said it was loud. Too loud.”

After a thorough evaluation, including an echocardiogram and cardiac catheterization, doctors discovered Kearns had severe mitral valve regurgitation—a condition where the heart’s mitral valve doesn’t close tightly, allowing blood to flow backward into the heart.

“I never would have guessed my heart wasn’t working right,” he says. “I had been experiencing some fatigue and a little weight gain, but at 52, I wrote that off as signs of aging.”

According to experts, his fatigue, along with the heart murmur, were symptoms of his leaky valve. Other symptoms can include shortness of breath, heart palpitations and swollen feet or ankles.

Paul Massimiano, M.D., heart surgeon and program director of cardiac surgery at Washington Adventist Hospital, determined that the minimally invasive repair would be the best option for Kearns. Dr. Massimiano and his team of heart surgeons helped pioneer the development of minimally invasive valve surgery, and joined Washington Adventist Hospital last year.

With a minimally invasive approach, incisions are smaller, which means a quicker, less painful recovery and a faster return to normal activity.

WITH KOMEN Grant allows hospitals to expand access to breast cancer screenings and care

For the past 18 years, thousands of women in Montgomery and Prince George’s counties have received free mammograms through the breast screening programs at Shady Grove Adventist Hospital and Washington Adventist Hospital. Now, a nearly $1 million grant from Susan G. Komen for the Cure is helping the Navigate to Health: Rapid Referral Program expand comprehensive breast care services to medically underserved, low-income, minority women.

The unique program, which has already been reaching women through partnerships with local clinics such as the Mercy Clinic in Gaithersburg and the Spanish Catholic Center in Silver Spring, will now expand to the Mary’s Center in Langley Park, Mobile Medical Care Clinic in Silver Spring and Germantown, the Muslim Community Clinic in Silver Spring, and Greater Baden Medical Services in Prince George’s County.

Navigate to Health: Rapid Referral is designed to decrease the time of referral, screening, diagnosis and treatment of breast cancer. Specifically, the grant will increase the number of mammograms and provide education services to local, underserved women. The program features an enhanced follow-up process for women who have had positive results from screenings.

“We don’t just want to screen women; we want to teach them about their breast health and make sure that those with positive results receive treatment and care,” explains Judy Lichty, regional director of health and wellness for Adventist HealthCare. “We know that with breast cancer, early detection and rapid treatment can make a significant difference for the patient.”

Work to develop the Navigate to Health: Rapid Referral Program started nearly two years ago in partnership with community health clinics and the Montgomery County Primary Care Coalition, which are critical sources of health services for underserved populations in the region. The

By 2014, the Navigate to Health program is expected to serve nearly 2,000 local women.

The nearly $1 million grant given to Shady Grove Adventist Hospital and Washington Adventist Hospital is one of only two large grants given by Komen to help address barriers to care.

ashington Adventist Hospital has served the greater Washington area for more than 100 years, and it continues to provide the communities of eastern Montgomery County and western Prince George’s County with high-quality, compassionate health care.

To meet the area’s growing health and wellness needs, Washington Adventist Hospital is seeking state approval to relocate its hospital within its current service area, near White Oak. Adventist HealthCare will also develop a Village of Education, Health and Well-being on the Washington Adventist Hospital Takoma Park campus.

The White Oak site, off Plum Orchard Drive, will allow for a modern health care facility. Patients will receive care in private rooms, with more space for clinical care, and state-of-the-art equipment and technology. The 48-acre campus will be accessible by public transportation and is near major interconnecting roads, enhancing access to ambulances, patients, visitors and employees.

This hospital campus will bring 1,000 new permanent jobs to Montgomery and Prince George’s counties, and more than 275 additional construction jobs will be created during the building of the hospital and campus.

The Food and Drug Administration (FDA) in White Oak, Washington Adventist Hospital and the Percontee Life Sciences Village property on Cherry Hill Road play a major role in the East County Science Master Plan. “We have an extraordinary opportunity to become the region’s major gateway for biotech and life sciences,” says Joyce Portela, president of Washington Adventist Hospital.

Ahead of the move, Washington Adventist Hospital and the FDA already have an agreement in place that allows collaboration on health research and medical innovation.

“By sharing resources and talents, the two organizations can open up new areas of medical research, discovery, funding and cooperation that are critically important for protecting and promoting public health in the region and beyond,” says Jere Stocks, senior vice president of Adventist HealthCare and chair of Next Century Health.

Washington Adventist Hospital and Adventist HealthCare are also committed to ensuring there is appropriate access to needed primary health care services in Takoma Park once the hospital relocates. The current campus will be redeveloped into a Village of Education, Health and Well-being. The health care offerings will include primary care/urgent care services, inpatient and outpatient rehabilitation services, and a behavioral health clinic, to name a few. Washington Adventist Hospital is also working with Washington Adventist University to plan for expanded educational programs for the region.

Washington Adventist Hospital’s proposed new site already has major traffic and land-use approvals in place, respecting the environment and blending effectively into the surrounding community while employing principles of smart growth. The Maryland Health Care Commission is currently reviewing the project; a decision is expected later this year.
