PERSONAL OUTCOME MEASURES Personal Outcome Supporters! Employee Recognition Awards

Angela Narcisse is a valued Site Manager at Anchor Avenue. She has consistently trained her staff to embrace the COMPASS and CQL ideals into day to day operations and activities at her site. She has been a steadfast model in how to incorporate Personal Outcome Measures (POM) interview content and goals into activities for each person that enables them to meet the desired outcomes evidenced through their interview. The challenge of translating POM outcomes into related “goals” in the structure of a volunteers day can be daunting, but Angela has consistently found a way to do just that. For example, one of the volunteers expressed a keen interest in going to a beach, and enjoying the ‘sea” environment. Knowing this, Angela than arranged for a nice trip to a beach, to collect shells which were later used in a very successful fundraiser project. In this manner, the person’s dream of visiting the beach was honored, and was became an integral part of services.

In another instance, a person (JH) expressed his interest in sports during his POM interview. Anyone who knows JH knows he is an avid sports fan! So what did Angela do? She arranged for JH to go to a local Sport Memorabilia store, to make a purchase, which is also a part one of his goals. JH could not have been happier! He returned with a wealth of magazines and proceeded to laminate pictures from some of the magazines to decorate his work area. Of course, many of the magazines were also taken home. Angela has made POM’s “come to life” for people. Of equal importance is Angela’s success at training her staff in these endeavors, and imparting the values inherent within them. Angela sustains a very high standard of service; safety and values, and her staff learn that only the best is acceptable when it comes to the supports they provide. Angela is an exemplary employee and we are so fortunate to have her.

Richard Lundgren, Storyteller by Susan Gill, POM Trainer Once upon a time, Richard Lundgren worked for a “Head Start” program working with children. He recalls that he loved doing this, and in a recent personal outcome interview, he reiterated this vision to the people who give him support. Richard attends the Farmingdale I day habilitation site. After his POM interview, Richard joined some people from the Freeport Vocational Center to visit the Summit Lane Elementary School and read to the children. “He was so excited about this visit” Kris Thulke, assistant manger told me. “Richard likes to talk about the days when he did this kind of thing more often and he couldn’t believe he was getting the chance to do it again.” Four classes of kindergarten and first graders joined Richard and his coworkers to hear stories about animals and tired children. Richard was in charge of holding the books and showing the various illustrations to the students while someone else read the words. Richard knew exactly when to hold up the picture and was adept at every child seeing the illustrations clearly. The school was so happy with the enthusiasm and professionalism of Richard and his group that they have asked them to return when school opens again in the Fall. Because Richard made this dream known during his personal outcome interview and a diligent and productive staff made the connection, he now can say that he has started doing what really matters most to him in his life.

Memorial Tree Planting by Marie Ball On June 7, 2014, friends, relatives and family members joined everyone at the Morris Rd, Bethpage IRA to plant a tree in memory of James Calvacca. Jimmy lived in the Bethpage IRA for over 30 years until his passing last year. Everyone participated in planting a beautiful Weeping Cherry tree in the front yard of the home. The Calvacca/Ball family presented a granite stone that reads, " In Loving Memory of James M. Calvacca 4/29/51 - 1/5/13 - And In Honor Of AHRC Staff for Exceptional Care". Jimmy's brother in law, Dave Ball, sang a song to commemorate the day. Following the tree planting, balloons were released by Jimmy's housemates. Then, everyone enjoyed lunch together. This was a wonderful way to commemorate Jimmy and his living the life he wanted. 18
