The company’s initial objective to fill a void in the custom fit AFO market has inspired the procurement of eleven US product patents since its inception. The company also carries patents in several European countries. A Comparison Between the PRAFO® Ankle/Foot Orthosis and Recumbent Ankle Positioning Splints By design, the plastic and Kodel® type orthotic preparatory splints were intended for the recumbent patient population. In order for these splints to be used for ambulation, a base must be provided on which to stand or walk. For this to be accomplished it is necessary to apply a cast shoe over the splint, attach a base with Velcro® or attach a base with an aluminum or plastic knurled torque knob. This type of splinting also has very limited (to say the least) ability to be adjusted to increase or decrease dorsiflexion assist or to be made to match any type of equinus contracture. In comparison, the PRAFO® orthosis has evolved into an AFO system that, by design, is not strictly a recumbent ankle-positioning splint. It is, in fact, intended to be used for ambulation in addition to having the capabilities of being used in the recumbent patient population. Case in point, many orthotic lower extremity orthoses are used on the recumbent patient population to protect fractures and offset unwanted orthopedic anomalies, and most of these can also be used for ambulation. The definition for the PRAFO® orthosis is a single posterior upright modular AFO system that can be used to offset drop foot in both the ambulatory and the recumbent patient population. It can also be used to offset and/or correct certain ankle contractures, and it is an AFO that, when providing the functions previously outlined, will not cause pressure on the heel and other areas of the ankle/foot complex both in a recumbent position or standing and walking position. This AFO can also be used in the custom fabrication of a variety of AFO’s and KAFO’s by those qualified and capable of designing and fabricating custom AFO’s and KAFO’s. This modular AFO system has the following component description: • 3/16” injection molded polypropylene calf and foot segments with toe extension. • 11/2” x 1/8” aluminum, 11/2” x 3/32” spring steel or 11/2” x 3/16” heavy duty aluminum heel/foot connecting upright attached to the calf and foot segments with four nickel plated brass PTB type retainers and stainless steel screws. This heel/foot connecting upright allows the PRAFO® orthosis to be adjusted and modified to suit a variety of orthotic objectives. • A 5/8” wide aluminum control bar attached to the calf segment with brass PTB type retainer and stainless steel screw to offset rotation of the extremity. Refer to adjustability above. • A 1/2” RUVA® and 1/8” polypropylene walking base is attached to and made part of the AFO with four PTB nickel plated brass retainers and four stainless steel screws (8/32” x 1 /2”) for a secure and adequate walking foundation. • A soft interface made of Kodel® is available or the practitioner may utilize other materials of his or her choice to provide the necessary padding and/or closures that are found in all custom and prefabricated AFO systems.

Contact Sensor

®

For the patient population that cannot tell when a heel suspension orthoses is properly fitting or when such patients experience bouts of uncontrollable extensor thrusts. The Contact Sensor ensures that the orthoses does what it was intended to do, along with preventing unwanted contact with bony prominences or other sensitive areas.

Consistent follow-up and possible reapplication of any orthosis is critical in the proper care of the patient. Therefore, once the Contact Sensor indicates an adjustment is needed the medical professional or patient will need to follow the proper fitting instructions for their existing orthosis.

To ensure that the AFO is properly secured to the patient’s lower extremity the Contact Sensor will provide early warning before any discomfort to the patient is noticed.

The Contact Sensor comes complete with a 9V battery, 6-foot power cord and self-adhesive trimable sensor strip. The sensor box can be easily attached to the foot of the patient’s bed or a wheelchair.

Once the patient’s extremity comes into contact with the sensor strip the red warning light will illuminate stating an adjustment will need to be made. The warning light will remain on until contact is removed from the sensor strip and for an additional 15 seconds before it resets itself.

NOTE: The Contact Sensor can be applied to any heel suspension device or AFO given a proper initial evaluation by a qualified medical professional.

Its custom-contoured aluminum heel connector bar helps to control dorsi/plantar flexion by allowing infinite, measurable adjustments. Its ergonomically designed foot plate allows its Kodel® liner to be changed easily without tools. Its integral, securely fastened walking base provides a shoe-type surface for ambulatory patients. Its calf and foot segments can be custom-modified to accommodate patient physiology. Its anti-rotation bar is made of malleable aluminum and adjusts to control unwanted extremity rotation.

The adjustable posterior upright design provides the practitioner a more precise and simplified method of setting the ankle/foot complex in the sagittal plane. The posterior approach requires controlling one upright joint, which lessens the chances of mechanical failure while also eliminating any pressure to the medial/lateral aspects of the malleoli that can be attributed with two joint systems. The APU® PRAFO® orthosis is also available for the inclusion in custom made thermoplastic AFO and KAFO systems.

The EV PRAFO® orthosis not only offers positive heel suspension, but also accommodates and helps control varus and valgus conditions of the ankle/foot complex. The calibrated settings on the bar allow for a more accurate documentation of the patients’ progress. The adjustable anti-rotation bar helps to control unwanted extremity rotation. The EV PRAFO® orthosis, with its securely fastened walking base, can be used on the ambulatory as well as the recumbent patient. The EV PRAFO® orthosis bar not only works well in our standard AFO’s, but also is extremely useful when used as the distal segment in our custom made KAFOs.

The sophisticated 650DDA/PS (Dynamic Dorsi-Assist) PRAFO® orthosis is designed to dynamically stretch the gastroc muscles, offsetting equinus contracturing. The use of a series of posterior articulations allows for a virtual unrestricted range of plantar or dorsiflexion. The DDA PRAFO® orthosis also provides nearly unlimited inversion or eversion of the ankle/foot complex. Designed by Anatomical Concepts, Inc., the Tri-Planar PRAFO® orthosis combines the sagittal and coronal plane adjustment features of the Dual Action PRAFO® orthosis with the transverse plane adjustment. The sagittal plane adjustment, featured on the APU® and Dual Action PRAFO®, adjusts for the plantarflexion/ dorsiflexion of the foot/ankle complex. The coronal plane adjustment, featured in both the EV and Dual Action, adjusts for varus/valgus of foot/ankle complex. The orthosis is secured and fully adjustable to accommodate all anatomical parameters. The transverse plane adjustment, new to the PRAFO® orthoses line, adjusts for internal/external rotation of the foot/ankle complex.

This orthosis was designed for osteoarthritic patients and provides finite adjustability to optimize off-loading tolerance. Auto Aligning Polycentric Joints
