Diabetes Today: The Crisis Diabetes is an often invisible disease that affects every facet of a person’s well-being. It does not discriminate based on age, gender, or race. And the toll it takes can be devastating. In fact, more than 30 million people in the United States are living with diabetes, and 25% of them don’t know they have it. As the incidence of diabetes and its associated risk factors like obesity continue to grow, so too does the need for expert care and intervention before and after diabetes develops. Philadelphia is ground zero for this epidemic. Of America’s ten largest cities, Philadelphia is the most obese and most diabetic. If left untreated, these conditions can lead to debilitating complications such as heart disease, stroke, blindness, kidney failure, nerve damage, leg amputations, and a higher risk of premature death. Despite the prevalence of the disease, there is currently no comprehensive diabetes

With a dedicated team of endocrinologists, educators, nurses, and staff, our team works closely with every patient to ensure they receive the right care at the right time to achieve the best possible quality of life.

Comprehensive and Coordinated Team Diabetes and weight management is a daily challenge affecting every facet of a patient’s life. These issues are only compounded when patients see providers who may not know the best course of action to accommodate their diabetic needs, such as emergency room visits or urgent care clinics. With a nine county service area comprising 14 hospitals, we have the opportunity to provide a formalized and coordinated approach in the care of diabetes patients including: • Effectively transferring inpatients to the outpatient setting • Transitioning juvenile diabetic patients to adult care • Educating providers in different care settings to deliver the most appropriate care in emergency situations • Providing personalized, patient-centered care that you need, where you need it, when it counts

Evidence-Based, Forward-Thinking Medicine At any given time, our endocrinology team is collaborating across the enterprise on more than 20 different clinical trials around the management of diabetes and its many comorbidities. Our dedicated team gets to know each and every patient personally, developing a tailored care plan that minimizes the disease’s effect on their lives through meaningful lifestyle changes, self-monitoring, and medication. With a comprehensive catalog of outpatient and inpatient services, the latest tools and technologies, and a robust clinical trial program—the Institute for Diabetes and Weight Management stands at the forefront of diabetes care. TYPE 1 DIABETES Jefferson is the regional choice for type 1 diabetes care, with a 40% type 1 patient population—twice that of the national average. We have unparalleled experience in addressing the complex health needs of this unique patient base. It is our ultimate vision to establish the region’s first Center for Type 1 Diabetes. Among the Center’s chief priorities will be establishing a clinic devoted to addressing the transition of patients from pediatrics to adult care, a critical step in helping our patients learn how to properly manage their diabetes. PATH-BREAKING RESEARCH When it comes to next-generation diabetes care, research is key. And Jefferson is home to some of the nation’s foremost experts who are uncovering breakthroughs in the lab and rapidly translating their discoveries into novel therapies in the clinic. Jefferson researcher Tung Chan, PhD, has identified an innovative way to regulate glucose levels using Akt proteins. This discovery has opened a new frontier in treating diabetes, and has far-reaching potential for other disease areas. Dr. Chan’s work is just a sample of the bold translational research conducted every day at the Institute. Philanthropic support of these efforts—whether through seed funding, programmatic support, or endowment giving, to name a few options—is paramount to staying on the leading edge of diabetes care.

COMMUNITY EDUCATION & OUTREACH Diabetic patients must be equipped with day-to-day knowledge of nutrition, exercise, monitoring, and medication. Cheryl Marco, RD, LDN, CDE, a diabetes education instuctor in Jefferson’s Comprehensive Weight Management Center, is teaching patients about behavior change, nutrition, physical activity, physiology, and long-term skill development. Patients also have access to insulin management classes— an invaluable resource for the many patients living with diabetes. Despite ample patient education options, far too many diabetic patients in Philadelphia never receive the knowledge, resources, and support they need to live a high quality of life. With philanthropy, we can expand outreach efforts, create targeted programs, and help more patients take advantage of Jefferson’s educational offerings. In doing so, we will markedly improve the health, wellness, and happiness of countless Philadelphians.

WEIGHT MANAGEMENT Weight loss through dietary modification has been shown to prevent type 2 diabetes, improve glycemic control and hypertension, and lower the risk heart disease and stroke, among countless other benefits. The Comprehensive Weight Management Center offers the full range of nutritional and physical coaching, intensive behavioral counseling, medication therapy, and surgery to help every patient achieve their goals and optimize their well-being. Philanthropy provides the resources necessary to expand the breadth and scope of our offerings and increase awareness of and participation in our weight management programs.
