Message from Yamhill Early Learning Hub Director We are happy to share the first edition of our Yamhill Early Learning Hub Annual Report, which reflects the partnerships, programs, and people that made our work possible in the past year. This Annual Report highlights the collective effort of our community to ensure every child in our region has the opportunity to learn, grow, and thrive from the very start.

The stories and outcomes shared on these pages represent more than numbers or initiatives; they reflect the dedication of the families, educators, providers, and community partners who come together every day to build brighter futures for young children. Whether itʼs through family engagement events, access to early learning programs, or connections to essential supports like housing, food, and health care, each effort moves us closer to a shared goal: a community where every child and family is supported and valued.

The Yamhill Early Learning Hub serves as a connector by bringing together voices from across sectors, identifying gaps, and building bridges so that services work better for families. This collaborative model is what makes our region strong. By listening to families and partners, we continue to evolve, ensuring that our programs reflect the real needs and strengths of our community.

About Us

Yamhill Community Care (YCCO) is a non-profit coordinated care organization dedicated to managing the healthcare for Medicaid members, covered under the Oregon Health Plan, in Yamhill County, as well as parts of Washington and Polk Counties. Our mission is to improve the quality of life of the communities we serve by coordinating effective care.

Beyond healthcare, YCCO is uniquely positioned as the only coordinated care organization in Oregon to also serve as an Early Learning Hub. The Yamhill Early Learning Hub supports families and children with essential resources and programs for early childhood development. Together, weʼre building a unified healthy community that celebrates physical, mental, emotional, spiritual, and social well-being.

Through the Yamhill Early Learning Hub, partners in early childhood education, K12 education, the medical community, social services, and business work together with parents to build up protective factors in our community — parental resilience, social connections, concrete support in times of need, knowledge of parenting and child development, and social and emotional skills of children — to give your child the best possible start in life.

Supporting Families, Strengthening Systems At Yamhill Early Learning Hub, we have built systems that bring together schools, service providers, and families to ensure every child and caregiver has access to the support they need.

These efforts are designed to reduce barriers, improve coordination, and create a more responsive network of care. We are not only delivering services more effectively, we are also empowering families to help shape the programs and policies that affect them most.

FamilyCore: Building Strong Foundations for Families with Children Ages 0-5 FamilyCore is a trusted community services network connecting families with resources they need to thrive. Whether itʼs buying diapers, finding parenting support, or building strong relationships, FamilyCore creates customized family plans to meet challenges head-on and to nurture strong families with children ages 0-5.

Requests are reviewed when no other help is available or when full costs canʼt be covered, no more than $400 per request.

Supporting Families, Strengthening Systems Yamhill Community System of Care (SOC): Collaborative Service Model System of Care (SOC) is a collaborative service model that addresses the diverse needs of children, youth, and families. It integrates mental health and essential services into a coordinated network to adapt to their changing behavioral health requirements. SOC fosters partnerships with families and youth, empowering them to navigate various systems and prioritize their preferences for optimal support.

All requests require children under 18 in the household or parents working on reunification. Hereʼs the total for ages 0-5: 66 children in fulfilled requests.

Housing and utilities, transportation, positive skill development including family activities such as pool passes or zoo tickets, or items to help parents get jobs.

Supporting Families, Strengthening Systems Local Family Receives New Beds with SIT Support This year, a Head Start worker discovered that two children, ages 4 and 7, lacked beds, impacting their ability to stay awake and engaged in class. The parents were also without a bed after their mattress was discarded due to a bed bug infestation. In response, our team quickly arranged financial assistance to purchase two twin beds and necessary bedding for the children. We also partnered with a local community organization to provide a new mattress for the parents. This support not only enhanced the children's sleep environment but also strengthened the family's overall well-being.

Supporting Families, Strengthening Systems Community-Driven Leadership for Early Learning YCCO is proud to be the only coordinated care organization in Oregon that also serves as an Early Learning Hub. The Hub connects health, education, and family services to give every child the strongest possible start. At the center of this work are two community-led councils that guide our efforts and ensure family voice is heard at every level.

The Early Learning Council (ELC) is made up of parents, educators, medical professionals, and social service providers. The Early Learning Council (ELC) is chartered by the YCCO Board of Directors to advise and oversee the Yamhill Early Learning Hub work. The Council meets monthly on the second Thursday at 4 p.m. in person or via Zoom at the YCCO office. Meetings are open to the public. The Parent Leadership Council (PLC) is made up of up to ten parents of children aged 0 to 8, who bring valuable life experiences to the group. Their objectives include providing feedback on the initiatives of the Yamhill Early Learning Hub and the Early Learning Council, cultivating leadership and advocacy skills, and fostering community through shared experiences. Over the past year, the PLC has participated in advocacy training and letter-writing sessions focused on legislative matters. They also established the "Little Library" project, which places libraries filled with bilingual books throughout the community. Additionally, the PLC has begun subcontracting with the McMinnville Playschool. Looking ahead, the PLC aims to become more involved in focus groups and take the lead in organizing community discussions to improve and enhance the Yamhill Early Learning Hub. School Readiness & Kindergarten Transition The Yamhill Early Learning Hub ensures families can access early childhood education and the tools they need to give their children the best start.

Child Success Act: Investing in the First 1,000 Days The Yamhill Early Learning Hub was recognized as one of seven pilot sites for the Oregon Health and Education Collaborativeʼs (OHEC) Upstream Initiative. This initiative focuses on developing a Child Success Model (CSM) aimed at fostering healthy child development during the critical first 1,000 days, from conception through age two. This was written into Senate Bill 1167 also known as the Child Success Act. As a pilot site, Yamhill Early Learning Hub collaborated with local organizations dedicated to creating a future where every child has access to the necessary resources.

“We firmly believe in the importance of early childhood development, and this project enable us to collaborate with other leaders and organizations to develop effective strategies that will benefit children and families across Oregon,ˮ said Seamus McCarthy, YCCO CEO. The CSM design work draws upon FamilyCore, YCCOʼs community services network that facilitates connections for families with local advocates to access essential support systems. The aim is to create a replicable, statewide framework for early childhood success that will guide future legislation and funding priorities. While the initiative was not funded in this legislative cycle, the work developed is vital for improving early childhood outcomes across the state.

School Readiness & Kindergarten Transition Expanding Access to Free Preschool Programs in Yamhill County & Surrounding Areas The Yamhill Early Learning Hub continues to support and promote free preschool options for eligible families across the county. These programs provide critical early learning opportunities, setting children up for success in school and beyond.

Our region offers a wide variety of high-quality preschool programs tailored to meet familiesʼ needs, from school district-based classrooms to specialized programs for migrant families and Tribal communities.

Together with our community partners, we have coordinated a streamlined preschool enrollment process, making it easier for families to find and access quality early learning programs close to home.

School Readiness & Kindergarten Transition Supporting a Smooth Transition to Kindergarten Our Yamhill Early Learning Hub is proud to support families in navigating the transition to kindergarten. We work closely with local schools and community partners to make it easier for families to identify the right school for their child and ensure they have the information and support they need to register on time.

In partnership with school districts across Yamhill County, we help simplify the enrollment process by providing outreach, guidance, and reminders leading up to registration events.

Our goal is to reduce barriers and help every child begin their kindergarten journey with confidence.

Our Kindergarten Enrollment Partners: We collaborate with the following school districts to promote kindergarten registration and connect families to local resources: • Dayton School District • McMinnville School District • Newberg-Dundee Public Schools

• Sheridan School District • Willamina School District • Yamhill Carlton School District

Each district hosts its own kindergarten registration events in the spring, where families can meet school staff, gather information, and get help with enrollment, even if they donʼt yet have all required documents or are unsure which school their child will attend.

In addition, 1,000 books and bags were distributed to elementary schools all over the county for their respective kindergarten registration days.

We believe that building strong connections between families and schools from the very beginning is a key step in creating lasting educational success.

School Readiness & Kindergarten Transition Together for Tomorrow: Kindergarten Partnership Innovation Together for Tomorrow is a program funded by the Yamhill Early Learning Hub and led by the Child Care Resource & Referral (CCR&R) to strengthen kindergarten transitions across Yamhill County. Through the Kindergarten Partnership & Innovation project, the Hub collaborates with districts like McMinnville, Dayton, Willamina, and Sheridan to align expectations, share readiness tools, and engage families early.

Educators and providers emphasized the importance of shared systems, earlier interventions, and culturally responsive support—especially for multilingual and rural families. Despite ongoing barriers like Attendance Summary delayed evaluations and limited cross-system communication, the Hubʼs partnerships are building stronger bridges between early learning programs and K–12 schools, ensuring every child has a smoother, more equitable start to kindergarten.

School Readiness & Kindergarten Transition Investing in Early Literacy As part of our commitment to improving lifelong outcomes for young children, the Yamhill Early Learning Hub proudly announced its participation in Oregonʼs Early Literacy Success Initiative. This opportunity allowed local organizations to apply for grants ranging from $500 to $2,500 to support innovative early literacy projects in our region.

Grant proposals aligned with the vision and mission of the Yamhill Early Learning Hub and serve one or more of the Hubʼs priority populations, which include: • Latinx and/or Spanish-speaking families • Geographically isolated families (West Valley) • Families parenting children with disabilities • Families experiencing houselessness • Families facing mental health challenges • Families engaged with Child Protective Services The goal of this initiative is to boost early literacy for children ages 0–6, reduce literacy disparities among historically underserved communities, and empower parents and caregivers with the tools they need to support early literacy development.

Over 8,500 children in Yamhill County received children's books.

Investing in Educators, Workface Development Strengthening Local Childcare YCCO announced a major investment of $733,333 to support the Yamhill County Childcare Task Force and its mission to improve childcare access, quality, and infrastructure across the region. This funding, approved by the YCCO Board of Directors through the Community Prevention and Wellness (CPW) Fund, reflected our deep commitment to building a healthier, more resilient community from the ground up.

In partnership with Mid-Willamette Valley Community Action Child Care Resource & Referral (CCR&R), YCCO helped launch and expand key initiatives, including: • Workforce development and training programs.

• Expansion grants for new and existing providers, including home-based care.

• Childcare apprenticeship opportunities.

• Targeted support for Spanish-speaking early educators.

Investing in Educators, Workface Development Partnership Committed to Reducing Barriers in Accessing High-Quality Early Childhook Education and Care through Primeros Pasos Initiative Capaces Leadership Institute, a nonprofit serving Latino communities in Oregonʼs Willamette Valley, has partnered with Yamhill Early Learning Hub, MWVCAA CCR&R, Marion Polk Early Learning Hub, OCDC, and Willamette Valley Wine Foundation, to address challenges in early childhood education through Primeros Pasos Initiative.

Primeros Pasos aims to improve access and quality of culturally and linguistically specific early care for Latino families in Marion, Polk, and Yamhill counties. They identified the severe shortage of available programs for preschoolers and established a consortium of regional organizations to facilitate coordination and community-driven solutions. With ambitious plans to serve an additional 1,500 children, enroll 300 new teachers, and create a culturally specific curriculum, Primeros Pasos has already secured significant state funding to support its goals.

Our training sessions are tailored to equip professionals with the knowledge and skills necessary to enhance their practices and better serve the families they work with. Through a variety of workshops and events, we aim to create a collaborative environment where participants can learn from experts, share insights, and implement effective strategies. Building Connection Through Community Events At the Yamhill Early Learning Hub, we believe that strong communities are built through connection, support, and shared experiences. We continued our tradition of bringing families, partners, and community members together through joyful, welcoming events designed to celebrate and uplift our youngest children and the people who care for them.

Community Baby Showers: Welcoming Families with Care Our Community Baby Showers are more than events. They're moments of connection, reassurance, and celebration. We host two free community baby showers annually for expecting or new parents across Yamhill County.

These gatherings provided a warm, inclusive space where families can connect with community resources, receive essential baby supplies, and build relationships with early childhood partners.

With the support of local organizations, health providers, and early learning professionals, parents leave feeling supported, seen, and prepared for the journey ahead.

We partner with various local nonprofits and agencies to support local families with over 24 agencies who participate. At our 2025 Baby Showers, we were able to partner with A Family Place to offer incentives for participating in Mothers & Babies Support Group, a program that helps to address postpartum depression. This was due to a grant from the Barbara Emily Knudson Charitable Foundation. We also partnered with Public Health to have parents who won free car seats meet with their car seat safety technician to ensure proper installation.

Building Connection Through Community Events “Before joining my first Mothers and Babies group, I felt completely lost. While I was already receiving support through weekly therapy and monthly home visits from a nurse—both of which were helpful—I still felt alone, unseen, and overwhelmed.

Joining that group changed everything. For the first time, I met other moms going through the same struggles. I no longer felt alone. I felt hopeful. Every mom I met and every lesson I learned gave me the understanding that I am normal. I am enough. These thoughts and feelings are not just mine—theyʼre part of the journey many moms silently endure. The group gave me the tools, encouragement, and connection I needed to begin healing. Building Connection Through Community Events Summer Picnic Series: Play, Learn, and Grow Together

Our Summer Picnic Series brings families and partners together for a season of fun, learning, and connection. Held in local parks across the county, these outdoor events feature free meals, games, activities, and opportunities for families to connect with service providers in an informal, family-friendly setting.

Each picnic is a celebration of childhood, with bubbles, music, games, and hands-on activities designed to nurture healthy development. We work together with early learning programs and health organizations to ensure every family feels welcome and informed.
