Tips for Your Medical Appointment

Going to see your doctor is important for staying healthy! Make sure to keep up with your yearly check-ups for your whole family. Here are some tips to help make your visit go well. Before Your Appointment:

• Write Down Your Questions: Think of any questions you want to ask your doctor about your health, symptoms, or treatment.

• List Your Medications: Write down all the medicines you take, including what they are for and how much you take. This helps your doctor give you the best care.

• Keep Your Appointments: Missing appointments can create delays in your care. If you can’t make your appointment, let the office know as soon as possible so they can help you reschedule.

• Schedule your YCCO WellRide if Needed: If you need a ride, contact YCCO WellRide and they can provide transportation.

At Your Appointment:

• Arrive On Time: Try to get there early to fill out forms.

• Ask Questions: If you don't understand something, it's okay to ask your doctor to explain it.

• Take Notes: Write down important information you get from your doctor so you can remember it later.

After Your Appointment:

• Follow Your Doctor's Instructions: Make sure you stick to any advice or prescriptions your doctor gives you.

• Schedule Follow-ups: If you need to come back again, set up your next appointment before you leave. Also, add a reminder to your calendar so you don't forget when your appointment is approaching.

• Use YCCO WellRide if Needed: If you need a ride for future visits, remember to book it beforehand!

Resources to Help You Reach Your Health Goals

YCCO WellRide: Transportation for Your Appointments

If you need a ride to your doctor, YCCO WellRide can help! This service gives free rides to non-emergency medical visits for YCCO Members. Call 844-256-5720 or TTY 711.

Hours: 7:30 a.m.–6 p.m., Monday–Friday.

Here are some important things about YCCO WellRide:

• They provide rides 24/7 all year long.

• You can schedule rides up to 90 days in advance.

• If you have an urgent need after hours, you can still reach customer service.

Interpretation Help Available:

You have the right to get free language assistance and interpretation support at all your doctor visits. Oregon law requires healthcare offices to have interpreters for people who prefer to speak a language other than English or who are Deaf or Hard of Hearing. If you need help, call our customer service team at 855-722-8205 or TTY 711.

Care Coordinators Are Here for You! Your health is very important, and we want to help you! If you have many healthcare appointments or need help understanding your medicines or benefits, ask for a Care Coordinator.

Care Coordinators can help you manage:

• Diabetes

• Heart failure

• Asthma

• Depression

• High blood pressure

• Mental health services

• Substance abuse

• And other conditions

Our friendly Care Coordinators are ready to help you reach your health goals. They can also assist with non-medical needs like housing and transportation. Contact our customer service team for help from a Care Coordinator! Call 855-722-8205 or TTY 711.

Did You Hear About Our Quit for Life® Program?

This program helps you quit smoking and includes:

• Tips to manage your triggers and make quitting easier

• 24/7 support through phone, text, or chat

• An online program that tracks your progress

• Group coaching for support and discussions

• Free nicotine replacement like patches or gum, for eligible Members

YCCO Health-Related Social Needs (HRSN) Services Are Here to Help You!

Did you know that YCCO provides special health services called Health-Related Social Needs (HRSN)? These services help you and your family during very hot or cold weather, keeping a home, and getting good food.

What Are HRSN Services?

HRSN Services help OHP Members who might be having a tough time staying healthy, because of social or economic issues.

How Can You Get Help?

With YCCO, you can access different HRSN services that fit your needs. Here’s what is available:

Climate Devices: We provide air conditioners, heaters, air filters, portable power supplies, and mini fridges to keep your medication cool during extreme weather.

Housing Services: We provide assistance with rent assistance, utilities set up and costs, tenancy services, medically-necessary home accessibility modification, storage fees, and more.

Nutrition Services: Get access to meals made for your health and learn about nutrition to improve your well-being.

Who Qualifies for These Services?

To qualify, you must be an OHP Member with a qualifying health condition and meet specific criteria. Additionally, you or a loved one must be going through or have gone through one of these life changes:

• Recently incarcerated within the last year.

• Discharged from an eligible behavioral health facility in the past year.

• Involved with the Oregon child welfare system, either now or in the past.

• Changing from Medicaid to dual eligibility (Medicaid and Medicare) in the next three months or so in the past nine months.

• At risk of becoming unhoused.

Not all OHP Members will qualify for assistance. Certain health, living, and financial conditions must be met to qualify for assistance.

Health-Related Services (HRS) Flex Funds

If you don’t qualify for HRSN services, you might still be able to get help through the health-related services flex funding program. Call customer service at 855-722-8205 or TTY 711 to learn more.

Join Us in Making a Difference: Community Volunteers Needed!

Be a Part of the Community Advisory Council

(CAC)

The Community Advisory Council (CAC) is recruiting members! This group is made up of community members and OHP Members who want to improve health and wellness in our area.

Get Involved:

The CAC meets once a month to talk about programs and plans that affect OHP Members. Members come from different backgrounds and help connect community members with the YCCO Board of Directors. Your input is very important!

The CAC supports important work at YCCO, including but not limited to: making funding recommendations to the Board of Directors, oversight of our Community Health Assessment (CHA), and Community Health Improvement Plan (CHIP).

We invite you to join the CAC! Your participation can help create a healthier future for everyone.

We offer stipends, dinner, childcare, interpreters, virtual meetings, and accommodations for special needs so everyone can join.

Calling All Parents: Join Our Parent Leadership Council!

Are you passionate about early childhood education? Join our Parent Leadership Council (PLC)!

Our council is made up of up to nine parents of children aged 0-8 who want to make a difference. Our main goals are:

1. Providing Feedback: Share your thoughts to help improve the Yamhill Community Care Early Learning Hub.

2. Community Building: Connect with other parents through fun activities and strengthen our community!

This is a great opportunity to share your ideas, make friends, and help shape early education in our area.

Come join us for a fun and exciting free Community Baby Shower hosted by the Yamhill Community Care Early Learning Hub! It's for new and expecting parents in Yamhill County!

Who: New and expecting parents (siblings and partners are welcome too!)

What: A free Community Baby Shower

When: September 18, from 4:00 PM to 7:00 PM

Where: Sue Buel Elementary School 1985 SE Davis St. McMinnville, OR 97128

Why: No registration required; come and go event. This event will provide a resource fair, baby shower prizes, raffle prizes, free goodie bags, food, and non-alcoholic beverages.
