Shining a Light on the Opacity of ADA Governance

he American Dental Association (ADA) is governed by its House of Delegates.

In addition to having “legislative and policy-making power,” the House of Delegates possesses the “supreme authoritative power” to determine association policy, revoke charters of state dental associations, amend the Principles of Ethics and Code of Professional Conduct, and levy special assessments among many other powers. Decisions made by the House of Delegates inform state law and regulation related to the practice of dentistry throughout the country and beyond.

Beyond this member-centric existential foundation, “rank and file” members actually have minimal, indirect input and no transparency into the ADA’s governance before decisions are made. To borrow from the acclaimed musical Hamilton, access to critical information on what decisions are being made and how they will be decided is left to those in “the room where it happens.”

Today, only members of a state’s ADA House of Delegates delegation have access to the reports, resolutions, and other information pertinent to the business of the House of Delegates. Dues-paying members have limited ability to learn about the matters coming before the House of Delegates. In addition, the ADA does not disclose the names of the delegates and alternate delegates, meaning the general membership does not even know who is making these important decisions.

According to some proponents of maintaining the status quo, ADA delegates are charged with tapping into the zeitgeist of our membership through some sort of unarticulated stakeholdering process and applying it to current House of Delegates matters.

Delegates do not have the time or resources to do this stakeholdering effectively beyond interacting with dentists they know, which leaves most members out of the process entirely. Assuming that this method is the best way to make decisions on policy with industry-wide ramifications, would it not be optimal to at least make sure the members know what decisions are being made and who is making those decisions?

The ADA, through policies made decades ago and reaffirmed each year by its leadership and the House of Delegates, systematically keeps ADA member dentists who are not involved in its governance in the dark. It is time to openly question if the way the ADA’s governance currently works is truly the way decisions should be made in the third decade of the 21st Century.

Increasingly, members are demanding to know what benefits their membership in the Tripartite (ADA, state and local dental society) provides.

As technology dramatically reduces barriers to engagement, members (and potential members) are also seeking out venues where their voices are both heard and considered.

Organized dentistry spends extensively to help our members’ voices be heard in external advocacy each year; the same commitment should be extended to ensuring our members can more easily contribute to sharing their experiences, knowledge, and beliefs on matters related to the dental profession that come before the House of Delegates each year. Ultimately, ADA policy will be stronger if all members impacted are given an opportunity to provide input.

This year, the ADA’s 11th District, which includes Alaska, Idaho, Montana, Oregon, and Washington, has submitted a resolution that we hope will serve as a catalyst for getting more member engagement into sweeping policy discussions before decisions are made. Fundamentally, we believe this approach will demonstrate the value and importance of ADA membership and help identify blind spots in drafted policies before they are adopted by the House of Delegates.

With this resolution, we hope to see the ADA make House of Delegates materials available to all members and provide more easily navigable ways for members to provide input before the House of Delegates makes decisions. This process could be very similar to public comment processes used in government decision making.

We wholeheartedly believe that final decisionmaking authority on critical matters should be left to a defined decision-making body selected by the states; we believe in representative governance.

However, dentistry is full of intelligent, passionate people who care about the future of the profession.

It would be prudent to seek out their guidance and consider their points of view before making decisions that will impact them. We will be stronger for it if we do. n W

Healing Our Wounded Heroes

M

any of us are feeling the joy and relief of returning to a semblance of normalcy as we emerge from the siege of the COVID-19 global pandemic. Summer travel is up, hotels and flights are in high demand, and it looks like most kids will be back to in-person learning when school starts in the fall. However, for some of us, the effects of isolation, loss, social unrest, political divisiveness, and financial hardship linger.

The practice of dentistry, even in the best of times, is stressful. It requires mastery in clinical and business domains, has unattainable expectations of perfection, and often involves managing painful procedures in patients who are unhappy to be in the chair in the first place.

The profession selects for, and then reinforces, trait compulsivity. Guilt, self-doubt, and an exaggerated sense of responsibility drive excellent clinical performance at the cost of self-care and routine maintenance. Compulsivity among health professionals is socially desirable, yet personally costly. Professional burnout is common among dentists as is early retirement due to disability.

Most dental practices in Washington closed during the first wave of the COVID-19 pandemic and then had slow openings in the months that followed. This resulted in financial hardship and the attendant stress of retaining staff and keeping the practice afloat through some very lean times.

Meanwhile, the need for emergency procedures continued, placing dental professionals at uncertain risk working in the oral cavity with dental aerosols during a time when virus transmission via these routes was unknown.

Though data are lacking in the U.S., dentists in the U.K. experienced high levels of stress, anxiety symptoms, and depression during the height of the pandemic.1 As part of the critical health care infrastructure we need to support he mental health of dentists and the sustainability of the dental workforce now and in the years ahead.

For more than 35 years, the Washington Physicians Health Program (WPHP) has been a resource for health professionals, including dentists, struggling with stress-related illnesses.

Anxiety, depression, and substance use disorders are common reasons for referral to WPHP and, while WPHP is often thought of as a program for addicted health professionals, 60% of referrals are for non-substance related health concerns. WPHP can assist dentists with any health condition.

Unfortunately, dentists are under-represented among WPHP participants. This is due to the

relative isolation of dentist practices compared to physicians, the dependence of practices and staff on the dentist, and misguided fear and loyalty among subordinate staff who may, understandably, be reluctant to provide feedback to the dentist or make a confidential referral to WPHP when concerns arise.

It is important that dentists, and those who care about them, understand that WPHP is a confidential, therapeutic alternative to discipline. Practically speaking, this means that the laws in Washington allow dentists to get help from WPHP without ever being known to DQAC. Though WPHP has a reporting requirement for dentists who do not follow WPHP direction and pose a risk to public safety, WPHP is forced to act on that obligation in only about 5% of cases. In addition, less than half of professionals referred to WPHP are recommended for a monitoring agreement. Most get an initial assessment, support, and referral to additional resources without the need for ongoing involvement with WPHP. This model is designed to effectively assist individuals who can benefit from support upstream of health-related practice impairment while also addressing more severe concerns before practice, patient, and life are at risk.

85% of WPHP graduates describe the program as extremely useful or lifesaving on exit surveys and nearly 90% are working in their field at program completion. One of the silver linings of the COVID-19 pandemic is that it galvanized efforts to support professional well-being and workforce sustainability. It is my hope that dentists and those who care about them will take this opportunity to help themselves or another who might be suffering in silence benefit from WPHP’s services. Whether wounded by the pandemic, the chronic stress of dental practice, or a combination of both, our dentists are heroes who deserve the compassionate support that WPHP provides. Using the Physician Payments Sunshine Act

few years ago, I wrote an article for WSDA News about the inconsistent quality of continuing education, which received positive feedback from readers.

Today, I would like to expand on that idea by addressing an interesting piece of legislation that allows you to be a better consumer of your education.

In 2010, Section 6002 of the Affordable Care Act, called the Physician Payments Sunshine Act, required that all transfers of value from medical/dental product manufacturers to doctors or teaching facilities be reported to the government.

While the intent was to allow more patients to seek out information about their doctors, it hasn’t worked too well. Most patients don’t even know this database exists, and a 2019 study concluded, “Open Payments does not appear to have achieved its goal of increasing patient knowledge of whether their physicians have received money from pharmaceutical and medical device firms.” [1]

Thankfully, the vast majority of doctors and dentists don’t collect enough money to be viewed suspiciously by patients; the median dentist collects a little over $50 per year in payments. However, providers should be aware of the small number of their peers who collect much more.

By digging a little more into Open Payments data (available at openpaymentsdata.cms. gov), you will discover that the mean dentist collects about $500. When the mean and median are so different, it’s because of a small number of very large recipients — those are the ones we need to focus on.

Look up the providers of your continuing education. I attended a class a couple of years ago, during which the speaker showed his disclosures by listing a variety of companies all on one slide. He then moved quickly to the next slide, acknowledging only that these companies covered speaker fees. Out of curiosity, I looked him up on the Open Payments website, and his income from corporate payments exceeded the income of the average U.S. dentist. The speaker covered several topics, and for each topic he spoke

glowingly about a product from a company that was paying him. So, if you plan to attend a lecture, look up your speaker to understand what financial conflicts they might have. I would encourage anyone hiring a speaker for an event to require a disclosure to prevent a corporate shill from showing up at your next seminar.

Look up the specialists to whom you refer your patients. In addition to informing our CE consumption, we can also use this information to see what colleagues who work with our patients are collecting. Are they collecting hefty sums from a company to promote that company’s equipment? When they say, “This brand works really well for me,” is it because that company is paying them? In most cases, no; most dentists just have a few lunch-and-learns on the ledger, but it’s worth taking a few minutes to look it up.

Look up researchers and public employees.

Should professors and deans at public universities be collecting money on the side from manufacturers? If you are writing a paper for publication, do you know what your co-authors are collecting? If you’re in a dental residency — especially one with a research focus — you should know how much and from whom your mentors are collecting. With this information, we are able to hold researchers and public employees accountable.

I think it can be so cathartic to know that you’re not the only one going through something. It’s sometimes easy to forget that dentists are so much more than their practice life and those things really do affect how we practice. For many of our members, the WSDA publications are one of the major ways that they interact with the association. Being a part of the editorial board gives you a chance to help shape what the WSDA looks like to many of our statewide members. I have also really appreciated that nearly no topic is off-limits as a potential article. If it’s a pain point for dentists, the magazine is a place we can highlight that (such as the shortage of dental assistants in the state) and talk about what can actually be done to address it.”

Recently Implemented Laws & Regulations Specialty Representation DQAC has approved amendments to WAC 246-817-420 to clarify dental specialty areas of practice and requirements for those who wish to represent themselves as a specialist.

General dentists may still provide specialty services but may not advertise or represent themselves as a specialist. Effective July 3, 2021, the rule offers two options to obtain specialty designation. The first option is completion of a CODA post-doctoral education program at least 2 years in length that is approved by the National Commission on Recognition of Dental Specialties and Certifying Boards. The second path to obtain specialty designation is by completing a CODA advanced educational program or program of other accreditors recognized by the United States Department of Education that is at least 2 years in length. Additionally, effective July 1, 2022, the rule requires a licensed dentist in a group practice with two or more dentists to identify themselves as either a general dentist or a specialist. If the dentist is a specialist, then the provider must include the area of specialty, and upon patient request the dentist must provide qualifications of specialty to the patient.

Examination Content On May 9, 2021, amendments to the examination content rule became effective.

The rule change adds the Joint Commission on National Dental Examination’s Dental

Licensure Objective Structured Clinical Examination (DLOSCE) to the list of allowable practical/clinical examinations as well as allows applicants to pass the components of the practical examination using a maximum of two testing agencies, with each agency administering practical examination sections in coordination with one another.

Dental Infection Control Rules On Jan. 23, 2021, DQAC adopted new dental infection control rules. The rules add several new requirements for dental providers, including the sterilization of single-use items when appropriate, requirements for high volume evacuation, and an hour of annual infection prevention training. The rules also call for a delayed implementation of the sterilization of slow-speed hand piece motors and periodic testing of water delivery systems. PMP Rule for Medicaid Providers In an effort to address opioid abuse in the United States, in 2018, the Substance UseDisorder Prevention that Promotes Opioid Recovery and Treatment (SUPPORT) Act was signed into law. Part of this new law, section 5042, mandates that all state Medicaid programs require Medicaid providers to check a prescription drug monitoring program (PMP) prior to prescribing a controlled substance (Schedule II through Schedule IV), not just opioids, to a Medicaid

patient. The Health Care Authority (HCA) has filed a permanent rule regarding the SUPPORT Act that will become effective Oct. 1, 2021 and will impact all Apple Health (Medicaid) providers who prescribe controlled substances.

Rules Under Consideration

Continuing Education DQAC will be moving forward with rulemaking to modify the current continuing education rule to allow dentists to receive full continuing education credit, in place of half credit, for watching live or recorded interactive webinars.

The Commission will address what qualifies as an interactive webinar as well as other likely questions at future meetings.

Dental Anesthesia Updated Rules DQAC is considering several changes to our state’s dental anesthesia rules. Specifically, DQAC is considering modifying on-site inspections and emergency protocols, continuing education requirements, the minimum number of qualified dental personnel that must be in the operating room when administering anesthesia, the addition of a pediatric sedation endorsement, and several other updates to minimal, moderate, and deep sedation requirements.

WSDA strongly encourages interested members to attend DQAC’s Dental Anesthesia Committee meetings as well as submit comments on the proposed rules to DQAC’s Program Manager Jennifer Santiago at Jennifer.Santiago@doh. wa.gov.

an interpretive statement which states that the administration of the COVID-19 vaccine is within a licensed dentist’s scope of practice during the declared state of emergency when the treating dentist has appropriate, verifiable training and experience that includes a hands-on component. The interpretive statement also outlines the different trainings and requirements a dentist should follow when ordering and administering COVID-19 vaccinations. COVID-19 Vaccine Delegation DQAC is considering permanent adoption of the emergency rule currently in place that permits a supervising dentist to delegate the administration of a COVID-19 vaccination to a licensed dental hygienist under the dentist’s close supervision, provided the licensed dental hygienist has demonstrated skills necessary to perform the task competently.

For the purpose of administering a COVID-19 vaccine, a dentist’s approval of the vaccination protocol and screening meets the requirement to diagnose the condition to be treated and personal authorization of the procedure as required by close supervision under WAC 246-817-510(1).

Novel Coronavirus Disease 2019 Screening This rule, which allows a supervising dentist to delegate the administration of a test to screen for COVID-19 to a registered dental assistant and licensed expanded function dental auxiliary under the dentist’s close supervision, remains in effect. The rule also allows a dentist to delegate administration of a COVID-19 screening test to a licensed hygienist under general supervision. In order for a dentist to delegate to any of the aforementioned dental professions, they must have demonstrated the skills necessary to perform this task competently. On June 25, 2021, following a public hearing, DQAC voted to approve the permanent adoption of this rule language.

Electronic Prescribing Requirement

Upcoming DQAC Business Meeting Dates

Do you prescribe controlled substances? As of Jan. 1, 2022, you are required to electronically transmit prescriptions for controlled substances Schedules II-V and refills for Schedules III-V or file a waiver with the Department of Health (department).

Substitute Senate Bill 5380, passed during the 2019 legislative session, mandated all controlled substances be electronically prescribed beginning Jan. 1, 2021. The law directed the department to develop a waiver process in which practitioners may attest to the need for a waiver if they have an economic hardship, technological limitation, or other exceptional circumstance that prevents them from complying with the mandate.

The Pharmacy Quality Assurance Commission and the department adopted rules to meet the legislative directive by providing practitioners criteria to attest to the need for a waiver from the electronic prescribing mandate using the department’s process. For more information or questions on the e-prescribing requirement and the secretary of health’s waiver, please contact PharmacyRules@doh.wa.gov.

Because the COVID-19 pandemic has taken an unprecedented toll on Washington’s health care system over the last several months, the secretary of health is exercising his authority under RCW 69.50.312(3) to grant a waiver to all health care providers authorized to prescribe controlled substances and subject to RCW 69.50.312(1) from complying with the e-prescribing requirements of RCW 69.50.312(1).

This secretary’s waiver will expire on Jan. 1, 2022. At that time, all health care providers prescribing controlled substances must either comply with RCW 69.50.312(1) or meet at least one of the exemption criteria in RCW 69.50.312(2) and file a waiver with the department.

Save the date for Dental Action Day, WSDA’s largest annual advocacy event, scheduled for Tuesday, Feb. 1, 2022, in Olympia.

BRACKEN KILLPACK RECOGNIZED AS A YOUNG LEADER IN PSBJ “40 UNDER 40” WSDA Executive Director Bracken Killpack has been named to the Puget Sound Business Journal’s “40 Under 40” list of young local business and community leaders for 2021. Killpack was honored for his commitment to bringing high-quality oral health care to more Washingtonians. In particular, the paper recognized his role in helping WSDA plot a new course in its relations with Delta Dental of Washington and brokering a landmark settlement that gives dentists more say in Delta policies and governance. The impact of the settlement is being seen in new WSDA-Delta cooperative initiatives, such as supporting the transfer and expansion of the Shoreline Community College Dental Hygiene Program to the University of Washington School of Dentistry, and other efforts to jointly address an acute dental hygienist shortage that constrains access to preventive care in Western Washington. His willingness to partner with other organizations has also led to the creation of WSDA-supported dental residencies, in which fully trained and licensed dentists provide up to $700,000 each, per year, in free or reduced fee care to underserved patients.

In addition to his ongoing efforts to enhance access to care, Killpack was recognized for his leadership over the past year in coordinating dentistry’s response to the COVID-19 pandemic so that dentists could continue to care for patients and serve the community. Specific contributions included working with dentists to donate excess PPE to front-line responders early in the crisis, coordinating with the state to secure and distribute replacement PPE statewide to enable the safe reopening of dental offices, and responding to the governor’s call for health care providers to encourage their patients to become vaccinated with a statewide public information campaign that included dentist-authored pro-vaccine columns in newspapers in Seattle, Spokane, Tri-Cities, Yakima, and other markets.

“This recognition is well deserved,” said WSDA President Dr. Denny Bradshaw. “I know I speak for everyone at WSDA when I say that we are very fortunate and very proud to have Bracken representing us in our dealings with the governor, legislature, local officials, other business leaders, social service providers, and others.”

WSDA President-Elect Candidate Dr. John Gibbons It has been a privilege to serve on the WSDA’s Board of Directors for the last four years. What a humbling experience to serve with these great volunteer leaders from across the state. This year I am a candidate for the WSDA position of President Elect. Prior to serving on the WSDA Board I had the opportunity to be the President of the Washington State Academy of Pediatric Dentistry (WSAPD), the WSAPD’s Public Policy Advocate, President of the Western Society of Pediatric Dentistry and the Western District Trustee of the American Academy of Pediatric Dentistry. Each of these positions have allowed me to gain an understanding of the important work that organized dentistry does for our members and the patients we serve.

The importance of both the WSDA and the ADA were highlighted this last year with their efforts related to the COVID 19 pandemic.

Those efforts allowed our profession to reopen on time and stay open even through multiple reoccurring surges. Although we can see a light at the end of the tunnel we still have work to do to assure that our members

are able to return their offices to full capacity.

Going forward it is important that the WSDA stays positioned to continue to do what is needed to help us transition back to normal.

If given the opportunity I would be honored to continue to serve the members of WSDA and promise I will always give all of my energy for what is best for you.

WSDA Board of Directors Candidates Dr. Chris Dorow I want to thank the members of the WSDA House of Delegates for allowing me to serve you over the past 4 years and ask that you consider reelecting me to another term on the WSDA Board of Directors. Wow what a time to have been on the board.

I came on to the board with the question of would we engage Delta Dental WA in a lawsuit which we did, and ultimately settled to the benefit of Dentistry’s input into the direction of the state’s biggest dental insurance provider. The association made some difficult cuts to become a leaner and more efficient organization. The legislative agenda saw amazing success not the least of which was educating legislators on the concerns of two tiered dental care in the form of dental

therapists, and despite what seemed to be an insurmountable political circumstance, our efforts overcame that challenge yet again. We engaged in great opportunities like the RETRO program. Then there was this COVID-19 thing that happened……WOW!!!!!

Through all this time I’ve seen how important it is to have a board with nimble thinkers with a diverse representation. I am a dentist from one of the smallest components in the state and practice in the small rural town of Othello WA. I’m the only private healthcare provider left in my community and sadly my experience isn’t unique to rural Eastern Washington. As such I feel I provide some unique prospective and experience on a host of issues challenging Dentistry’s future.

As to the challenges yet to come, some are obvious, and some thing will likely surprise us. Workforce continues to be a major challenge and we’ll need to be active in our legislative agenda as well as promoting innovative opportunities to address these concerns. Regulatory bodies have become an ever invasive presence in dentistry and we have to remain vigilant in promoting sound oversight as opposed to repressive and unnecessary regulation. We need to keep having a strong legislative presence so that our message gets heard and allows reasonable leaders to make reasonable decisions as they have over the past 11 years on the dental therapist issue. Finally we need a group of nimble and diverse leaders who can quickly

access and decisively address the next great opportunity or challenge for the good of our association.

My name is Chris Dorow and I’d appreciate you considering me to be part of that leadership again and re-elect me to your WSDA Board of Directors.

Dr. Lisa Egbert Washington dentists face low third-party payer reimbursement, a shortage of qualified auxiliaries, high student debt, and everincreasing regulation. I am running for the WSDA Board to advocate for our profession in a transparent and fiscally responsible manner.

For the last five years, I served as an officer for Seattle-King County Dental Society, one of the largest ADA components. My presidential term was anything but typical, beginning with having no Society staff and ending with the mandatory shutdown of dental practices.

Committed to SKCDS members, I performed the functions of both President and Executive Director while reassembling the staff.

A proven leader, I made difficult decisions for the betterment of the membership during a time of unprecedented challenges. Under my guidance, the Executive Council received ADA governance and leadership training.

SKCDS created a new strategic plan, mission statement, and investment policy. The bylaws received a necessary update. A diversity and inclusion initiative was developed and programs for members expanded, including state-mandated continuing education. I am proud that member benefits increased annually, without raising dues.

As SKCDS Treasurer and serving on boards for UW Bothell and Cascadia College, I gained experience in governance, budgeting, lobbying, and creating community partnerships. My higher education background is well matched to workforce expansion advocacy.

I have the passion, experience, and time to be an effective WSDA Board member and am committed to working through any challenges that arise. I pledge to remain transparent, fiscally responsible, and above all, member-centric. The opportunity to serve is deeply appreciated.

Dr. Joseph de Jesus Dental legislative, regulatory, 3rd party benefit, and workforce issues are and will continue to be significant challenges for our profession. Additionally, graduate indebtedness is affecting the present and future of our profession and Association(s).

A knowledgeable and enthusiastic WSDA Board of Directors is important to coordinate long-term efforts to address these issues, and also to act at critical times, this Covid-19 year, to advocate for the practice of dentistry and the importance of maintaining dental care delivery in our state. Over the past decade I have been fortunate and drawn to serve within state and local organized dentistry and have continuously worked to attain a robust knowledge of our profession’s issues.

In service on the WSDA Board, it has been especially gratifying to work to continue our Association’s direct support of members, whether it be hand-delivering PPE, guiding responses to legislative and regulatory challenges/opportunities to dental practice, growth of our non-dues revenue to maintain fiscal health to encourage membership, and/ or resolution of the Delta lawsuit. Even with the intensity and difficulties of this pandemic Zoom Board year, I have enjoyed serving on the WSDA Board with our talented Board members and Staff. I would like to continue to apply my efforts and energy to serving on the Board, providing a unique perspective and liaison capability from Thurston-Mason Counties, within walking distance of the Capitol dome.

Dr. Christy Kirchner I am a mom of two young boys, a wife and a solo practitioner who has owned and operated her own business for over 20 yrs. I understand the challenges that face dentistry and like most of you, I am affected by these challenges on a daily basis. I am currently on the task force to address the workforce shortage in our state. I want to continue to work with the WSDA to find lasting long term solutions to the current workforce shortage. I would be

honored to serve another three years on the Board of Directors and continue my work to improve organized dentistry in Washington State.

Dr. Mark Koday I have served on the WSDA Board of Directors for the past 3 years. I am also a member of the Board’s Committee on Regulatory Affairs helping to fight onerous regulations detrimental to our profession. Currently, I chair its 2021 Legislative Task Force that monitors and develops strategies to fashion and/or defeat proposed legislation affecting dentistry. While I come from the world of community health and feel a viable public sector is needed for those populations living in poverty, I believe that a strong, healthy private sector is absolutely critical to dentistry.

Our profession is facing great pressures and it undergoing major changes. Change itself is a normal process and nothing to fear but we must be vigilant in our analysis of those changes and vigorously challenge any we find are detrimental to our ability to care for our patients. I believe supporting our members is the primary responsibility of the WSDA Board. I believe a strong profession is critical for the oral health of our patients and communities. It has been a true honor to serve on the Board and hope you’ll allow me to serve another term.

Dr. Liselotte (Lilo) MannionBlack As a dentist and small business owner, I have been the benefactor of the work and advocacy of those before me who have fought for, and led, our organization through the many challenges our industry has faced. Our mission statement to “empower dentists to advocate for and provide optimal oral healthcare”, is only possible when our systems and institutions share the same vision.

It is my belief that as a member of the Board of Directors, I would be able to contribute

to our mission in my ability to see the whole picture and the integration of systems, tempered with thoughtful analysis. The face of our industry and even the delivery, has changed over the last decades, and we as an organization need to remain nimble to help meet the challenges that our colleagues encounter.

On a personal side, my husband Eric and I are the parents of two adult sons and two black labs. You would likely find me hiking our local trails, working in my garden, or sampling the latest Tri-City wines.

Dr. Blake McKinley Dentistry affords many opportunities. Whether owning and operating your own business in a manner that suits your personality, providing care in a community service setting, or teaching in the university setting.

The WSDA provides the opportunity to participate in a collaborative effort to maintain a strong and viable dental health care model.

The WSDA accomplishes this through the PNDC, DentPAC, WOHF, Budget & Finance Committee, and tireless WSDA staff support, all under the direction of the Board of Directors.

However, there are forces out there that for which vigilance must be maintained in order to safeguard the delivery of dental health to Washington State citizens. The COVID-19 pandemic demonstrated how important strong WSDA leadership is; which helped get dentists back online after being shut down by the state government while at the same time bring an end to the lawsuit with DDWA in a mutually beneficial manner.

I have been fortunate to participate at the society and state levels. For the Spokane District Dental Society, a variety of committee positions, and leadership chairs, including president, and until recently as an Executive Council Alternate. Currently, I am a Delegate to the House and I serve on the SDDS Budget & Finance Committee. At the state level I have served on the PNDC Committee, DentPAC Board, and Budget & Finance Committee. I am currently on the WSDA BOD. Through these positions I have developed a good foundation for the operations of the WSDA. I would appreciate your continued support for the WSDA BOD.

Dr. Kim Nordberg My father was the model for my dental career choice.

He had a “servant heart” and served his clients faithfully for over 50 years as a barber. I admired the rich long term relationships with many lifetime customers. Dentistry has allowed me the same wonderful opportunity to develop long term relationships serving. In addition Dentistry has allowed me to serve the poor in Guatemala at the same time mentoring USC, UCLA, Loma Linda, and UofP dental students these 18 years. I dearly love these diverse avenues!

My primary motivation to serve on our WSDA board has been to help upright our economic and workforce problems in Washington.

We continue in a downward economic spiral over the last 10 years operating in a 60% of 2011 reimbursement model, led in WA by WDS. I am proud of the heroic fight we at WSDA waged and negotiated a “seat at the WDS table” but it is only a beginning!

WDS’s unilateral and unfair EOB contract policies continue to penalize and punish dentists that are trying to do conservative, ethical and quality care.

We must double our efforts to demand that WDS actually becomes an advocate for their dentist “partner/stakeholders” and refuse to sell and write EOB policies that downgrade and disallow procedures our patients need.

It is also a painful reality we are in a severe workforce shortage crisis that also hurts our economic survival and ability to keep our doors open.

During my last 3 years on our workforce taskforce committee we have explored strategies to train assistants and expand hygienist numbers.

I am personally most excited about private hygiene program expansions.

We Nordbergs are a dental family of 2 dentists, 2 orthodontists, and 1 hygienist.

We love our profession as it allows us to serve.

Dr. William Petersen I have been retired from dentistry for the last two years. I have been very active in both the state and my local dental society for then last nine years. I served as President of the SDDS, Delegate to the House for six years, Chairman of the Budget and Finance for the WSDA, and am currently on the Executive Committee of the SDDS. I foresee many challenges for dentistry in the coming years: among which are ownership and management of corporate dental practices, supervision of dental therapists, decline in membership of the WSDA, cost of dental education, and universal health (dental) care. I believe that it is only a matter of a few years before we will have national health care followed by national dental care. I believe that the solo dental practice will be replaced by large group clinics. It will be imperative that a dentist has control over all dental treatment. The great challenge will be how can dentists meet the demand for all the dental treatment that will be demanded by all those individuals who have neglected dental treatment. I foresee the need for dental therapists at this time. We must ensure that they are working under the supervision of a dentist. I feel that having experienced many changes that were very controversial over my many years of practice that I have the insight that would be beneficial in formulating solutions to these challenges. My biggest strength is that I am great at thinking outside the box. I am asking for your vote and want to thank you in advance for it.

Dr. Daniel Tremblay I am running for WSDA Board of Directors because I have a passion and ability to actively listen to the needs of our members, increase participation by new dentists, and work towards effective changes for our profession.

There are a number of issues facing dentistry, but one of the most important is safeguarding against the unlicensed practice of dentistry by non-dentist third parties and preventing any non-dentist from interfering with a

licensed dentist’s independent clinical judgement. With the WSDA/SKCDS, I have advocated for legislation aimed at protecting the doctor-patient relationship and increasing access to quality, affordable dental care in our state.

Moreover, I will continue to work for transparency and effective changes to insurance companies to ensure that their members are heard and patient care, not profit, is their top priority.

Moving forward, we need to investigate ways to proactively introduce measures that address the workforce shortage in dentistry and increasing student debt load, recognizing its potential influence on graduates and their job choices. By doing so, we can work to preserve the integrity of our profession by advocating against changes that would hinder dentists ability to provide quality care to all individuals.

I am confident that my leadership skills, as well as firsthand experience with the challenges young dentists face will allow me to excel in this position and bring a fresh perspective to the Board role, while working to serve all Washington dentists.

Delegate to the ADA Candidates Dr. Christopher Delecki In 1996, I was elected and served two years as the at-large USPHS Delegate to the ADA House (HOD). For the past three years I have been a WSDA Delegate to the ADA HOD as the President Elect, President, and Immediate Past President. With this experience and background of knowing how the ADA HOD functions, I am confident that I will be a valued and contributing member of the ADA House of Delegates.

My educational and professional experiences also make me an exceptional candidate who will bring an outstanding and varied skill set to the ADA House.

As a practicing dentist, with significant proficiencies in community and public health dentistry, I can provide a unique perspective on many of the issues and challenges dentistry faces today and in the

future. I have also been involved in dental education at all levels of the dental team development. I’ve been an affiliate faculty member at the University of Washington, School of Dentistry and mentor for dental students and residents. For over 20 years I have taught both dental assisting and dental hygiene students. Finally, I am member of the ADA’s Council on Advocacy for Access and Prevention (CAAP). As a member of this ADA council, I have gained and maintain a unique understanding of the ADA’s strategic plan, fiscal operations and its members’ concern.

These many facets of my knowledge and experience position me well in representing WSDA members at the national level as a member of the ADA House of Delegates.

I enjoy getting to be the voice for my peers and for my profession. As a member of the delegation these past two years, we have gotten together to debate issues and bring forth and champion causes that are important to us, our colleagues, and our patients. I hope to have the opportunity to continue this work by serving in this capacity. Thank you very much for your time and consideration.

Dr. B. J. Larson I want express my gratitude for your trust in allowing me to serve you and our wonderful profession through so many varied positions over these many years.

It seems as if this past year or so has been put on hold by the virus. I look forward to meeting again soon with our colleagues from across this country and the world of dentistry just as we used to. Albeit with a few more precautions and a whole lot wiser!

Dr. Bryan Edgar My delegate experience began in 1995 at the AGD HOD & AADB General Assembly and 1997 at the ADA HOD. I served as the Parliamentarian for the AADB for 12 years.

During my 6 years as Speaker of AGD I have learned the various nuances of parliamentary debate at a level where it has become 2nd nature. Over the years I have learned this skill at a high level to get things done and help our caucus members become more effective. My goal is to aid in maintaining the strength of our strong caucus and pass this skill on to others. Thank you for the opportunity.

The ADA House of Delegates is an in person, face to face type of governing body and purposeful effectiveness is diminished over the internet. The Zoom house was interesting and certainly better than no meeting at all.

However in my opinion its best attribute was convenience. Very little was accomplished.

Dr. Emily Hobart I would like to officially submit my name for consideration for the position of delegate for the House of Delegates of the American Dental Association. I was appointed as an alternate delegate for the 2020 house, but I ended up getting “promoted” to a full delegate. It was interesting to participate in the first virtual house and to vote via my cell phone, although I still feel like I learned a lot. This year I was appointed as an alternate delegate for the 2021 house and I am looking forward to participating live and in-person in Las Vegas. Although this will be my first live house for the American Dental Association, I served two years as a delegate for the American Student Dental Association and three years as a delegate for the Washington State Dental Association, so I have experience.

I ask that you grant me another term as ADA Delegate so I can attempt to get some of the changes, that I think would benefit us all, through the labyrinth that is our national organization. Hopefully with Linda Edgar as President Elect that will be an easier endeavor.

The time for constructive change could be at hand, I humbly ask for your vote so that I can assist our talented Delegation in making that happen!

Dr. Cynthia Pauley I have many goals in running for ADA delegate and the following is a brief overview of a few of them. If anyone wishes to further this conversation, I am always available by phone, text or email for any and all questions. I take this position very seriously as the delegate position is a representative one, which will always mean my voice is a collective of all the members I represent.

My number one goal is to make sure I help the ADA stay focused on the needs of its membership. Having served as an ADA delegate, it is my opinion that you have a voice at the ADA only if you use it. I spoke up and I am proud to have been part of getting the ADA resolution passed to help fund states across the country in their fight against unfair insurance practices. We must continue to let the ADA know this is a fight they must continue to partake in and it is definitely not something to sit on the sidelines for.

My number two goal is to utilize our voice for strong advocacy. We are uniquely positioned at the ADA to speak for dentistry in meaningful and powerful ways. The ADA continues to represent a majority of dentists and this gives us the ability to speak with authority for dentistry in this country.

We must not let our message be diluted or minimized. The ADA exists for one reason and one reason only, and that is to represent and advocate for its member dentists. The most important and prioritized item on any agenda must be the needs of those member dentists.

My third driving goal will be workforce issues. Locally and across the country,

the dental field is experiencing workforce shortages at every level. Besides the intense stress the individual dentist experiences working through this shortage, it greatly impacts the ability to deliver care to our patients. We need ADA policies and strategies to help change that narrative on local and national platforms to address these shortages.

As a former officer of WSDA, I am familiar with the unique needs of Washington State and the political waters we must navigate to reach solutions.

As a member of the faculty of UW SOD, I see first-hand the difficulties and obstacles facing our students and recent graduates. In my position as the clinical director for RIDE, our mission is to educate dentists to serve in rural and underserved areas. The program has been successful and is serving as a national model.

As a member of the UW SOD Admissions Committee, I am working with the committee to ensure our admissions process is fair and inclusive. The opportunity to have these discussions on a national level is paramount to developing our profession for the future.

2021 Legislative Session Summary

W

ashington state’s 105day legislative session adjourned, on time, on Sunday, April 25, 2021.

Occurring in odd-numbered years, the 105day session is the first half of the biennium, during which legislators must pass the state’s biennial budget.

Due to the pandemic, this session was conducted virtually, which significantly altered advocacy efforts. In an attempt to focus the workload of a virtual session, majority democrats instructed their members to introduce no more than 6 bills and advised that bills would be more likely to advance if they addressed COVID-19 challenges, improved racial equity, advanced economic recovery, addressed climate change, or increased revenue. While this was an admirable goal, there were still several pieces of legislation introduced this session that deviated from the aforementioned categories.

With the introduction of 811 bills and the passage of 337 bills, WSDA tracked several bills regarding dental-specific issues, health care, COVID-19 relief, taxes, small business issues, employer/employee matters, and more.

Vaccine Volunteers: Dentistry’s Shot to Further Protect Our Communities When volunteers from a broad selection of health care fields were needed to support Washington state’s COVID-19 vaccine rollout, dentists and their teams stepped up to help protect their communities through vaccination.

The Best of Times & the Worst of Times: Reflections on an Unprecedented Presidency

L

ooking back over my last two years as WSDA president, I have mixed emotions. As the old saying goes, “It was the best of times, it was the worst of times.” That pretty much sums it up — pen drop!

Thinking back to September 2019, as I started what I thought would be one year as your elected leader, I was excited and somewhat nervous. So many great WSDA leaders had served before me, and I genuinely wondered if I could measure up to their examples. I worried about whether I could balance the time requirements of this position with the demands of my practice and my personal life.

But I love a good challenge and I was ready for whatever would come my way.

Or so I thought.

As I started visiting component societies and attending the extra meetings, I kept a time tracker and was so proud of myself for keeping up with my responsibilities. The WSDA Board was working on our next 3-year strategic plan, helping students at the UW dental school, supporting bills making it easier for hygienists to come to Washington and making it harder for giant corporations to operate here in ways that don’t always reflect well on our profession. These were all important issues for the future of dentistry.

Then it all changed.

I remember driving to Olympia for Dental Action Day in late January 2020 with Kainoa Trotter, WSDA’s assistant executive director.

We were talking about the first COVID patient having just been diagnosed in Washington.

It sounded scary, but I was sure it would be a minor bump in the road. Damn, was I wrong!

On March 19, 2020, dentistry’s “worst of times” moment hit, when we followed the guidance of the Department of Health and closed our doors to all but emergency care while public health experts tried to figure out what was safe and what wasn’t. I thought I had been busy with my presidential responsibilities before, but COVID took things to a whole new level. Your leadership team held virtual meetings 5-6 days a week, plus countless hours spent in between, figuring out what WSDA could do to help our members.

Responding to the COVID-19 pandemic was an immense challenge, and we did our very

best. For some, it wasn’t enough. However, for many, we provided crucial guidance and communication as we all moved through a pandemic environment for which no one had all the answers. I am very proud of the work your WSDA volunteer leaders and staff did and continue to do today in responding to the virus — thankfully on a less intense scale.

One silver lining in that very dark cloud is that the challenge of the pandemic led us toward our “best of times.” We were able to break down a long-standing barrier and actually talk with Delta Dental of Washington leaders — not about our differences or our lawsuit, but about working together to help dentists and patients. This was a monumental shift in our relationship which led to months of negotiating a settlement to our lawsuit and the creation of a Memorandum of Understanding between our organizations.

That agreement has paved the way for WSDA and DDWA to work together in addressing governance reforms, the dental workforce shortage, and potential legislation to help dentists and patients in the future.

There will be much more to come on these collaborations.

So, while it has been a difficult journey, it has been a rewarding one, as well. I’m not at all sorry that I was here, at this time, to work on these issues. I know I will look back on my term as your president with very few regrets.

Thank you to everyone who reached out with your calls, emails, and notes of support. They meant a lot at the time and continue to mean a lot to me now. Thanks, too, to those of you who posed questions and challenged answers along the way. That’s how we all get better.

Come September 11, 2021, I will hand the gavel over to your next leader, Dr. Ashley Ulmer. She is very smart and committed to serving our profession — both those working in it today and those who are to come. I am 100 percent confident that, under her leadership and those who follow her, WSDA will continue to be a force for making the delivery of optimal oral health care an obtainable goal for everyone in Washington.

here’s an old saying in volunteer organizations: When you have a big job to be done, look for someone who’s already busy.

“My dad is my hero,” Ulmer says. “He grew up here, too, so his roots in the community are deep. He does a lot of work for people who need help; not just going to fundraisers but giving people jobs and mentoring them. I’ve learned a lot from him.”

That’s the approach WSDA members have taken in electing Dr. Ashley Ulmer to serve as the next president of the Washington State Dental Association.

In addition to leading a successful dental practice in Spokane and taking on leadership roles within WSDA, she volunteers with Spokane’s Inland Dental Expanded Access (IDEA) Clinic, helps out with her family’s business, is active in her church, avidly gardens, and is even an award-winning powerlifter!

father runs a commercial roofing company and serves as a lay pastor at their church.

Both brothers work with him in the business and Ulmer chips in to help with some of the bookkeeping.

HOME IS WHERE HER HEART IS Despite her many interests and commitments, Ulmer considers herself a homebody. She and her husband Erik Gilbertson, an electrical contractor, live in a comfortable bungalow in a modest North Spokane neighborhood, across the street from the house in which she grew up.

Her brother Justin lives in that house now, with her parents, Ron and Rhonda, occupying an apartment over the garage. Another brother Auston lives just down the street, so it seems like a family compound, a feeling reinforced when one of the brothers swings by on a riding mower to cut her lawn. Her

Given such close family ties, it didn’t come as much of a surprise when it was time to choose a college and she opted to remain nearby, attending Eastern Washington University.

While at Eastern, she was considering some kind of medical career, but fate intervened with the need to have a couple cavities filled.

While in her dentist’s office, something “just clicked,” she says. “The mix of the profession and the business activity of the office was so alluring.”

She spent a year going into that office to observe and learn as much as she could about the profession before heading over the mountains to attend the University of Washington School of Dentistry. After graduating from dental school in 2003, she spent two years in residencies with the Veteran’s Administration in San Antonio and Seattle.

“Working at the VA was a very powerful opportunity,” she recalls. “There was an unending population of patients and almost no limits on resources, so we could really concentrate on the care we were providing. I learned a lot.”

SAVED BY WSDA Returning to Spokane in 2005, she bought a solo practice. But things were rocky at first.

The practice had plenty of patients on the books, but that didn’t mean they were active or coming in for regular check-ups or other care.

Looking back on those early days, Ulmer says, “It was a tumultuous time, because I didn’t realize that I was starting from a deficit position. I slowly learned the business aspects of running a successful practice.

I was funneling all my money back into the practice, and it took time to make it successful.”

Along the way, Ulmer got involved with her local Spokane District Dental Society and with WSDA. “WSDA saved me,” she says. “I was working on a practice that wasn’t yet thriving and I felt isolated. It was so good to come together with others for camaraderie, mentorship, and

Today, Ulmer’s “Smiles by Dr. Ashley” family dentistry practice is thriving. It’s a highly personal practice, with no associate

dentists, and a relatively small staff of two hygienists, two assistants, and one frontoffice employee. The structure provides the flexibility to accommodate Ulmer’s volunteer pursuits, including her commitment to WSDA. “I’ve crafted my practice to work around my WSDA involvement and allow me the time to stay involved,” she says. “The knowledge you receive from the members and the info you receive from the association, especially if you take the time to be involved, makes you a better dentist.”

Her approach to time management reinforces what Ulmer thinks makes a successful dental practice.

“To have a successful practice, you first need to have your own definition of success.

You need to know yourself and build your practice model in a way that makes you happy to go to work. Whether you want the business management responsibility or want to just focus on care, for instance. If you don’t feel good about how you’re spending your time, you’re not going to be treating your patients well. Even if you’re making lots of money, it won’t be satisfying.”

MOVING INTO LEADERSHIP Eventually, Ulmer decided that she and her practice were in a position where she could begin serving in WSDA leadership: “I felt strongly that it was my chance to give back for the way WSDA took care of me.”

After serving on the board for about a decade, she began what she calls “an organic progression” through the officer chairs. Now, stepping into the presidency, Ulmer brings a combination of gender, geography and age to the position which hasn’t been represented in two decades. Not only is she a woman from Eastern Washington, but she will also be one of the youngest WSDA presidents ever.

“Ashley has an amazing gift of seeing people for who they are and listening hard to what they are saying and where they are coming from with honesty and integrity,” Pauley says. “I’m proud to have this incredible woman representing us around the state and the country in the coming year.”

She adds that WSDA has created a work group on diversity, equity and inclusion to gather ideas on what can be done operationally and ideologically to make WSDA an even more welcoming environment for a diverse membership serving a diverse public.

“During the pandemic, I really appreciated the diversity of WSDA’s communications with the board and the membership. We couldn’t have in-person meetings, but the organization did a good job in maintaining meaningful outreach,” she says. “Now that we can meet face-to-face again, we have a great opportunity, provided we have clear intentions behind every communication and action we take.”

Cindy Pauley, who knows something about what it takes to successfully lead WSDA, has no doubt that Ulmer will make the most of the opportunity.

Dr. Nathan Russell, who has been friends with Ulmer since they studied human anatomy together in their first year of dental school, agrees.

“She loves life and people, and is a wellrounded, good-intentioned individual,” Russell says. “She listens and is extremely good at consensus building.”

In keeping with her emphasis on good communication and consensus building, Ulmer recently sent every WSDA board member a copy of High Conflict, a New York Times bestseller that focuses on effective communication. She sees strong communication as a key in the coming year.

It’s no accident that she has read the book, as she commits herself to reading at least one book on leadership or a health-related subject each quarter — a goal that’s routinely exceeded.

A CHANGING PROFESSION Even with the stresses of the COVID-19 pandemic beginning to ease, Ulmer will be stepping into the WSDA presidency at a time

“At both the ADA and WSDA, we’re trying to make our associations look more like the profession and more like the patients we serve. We want to get anyone who is interested in an issue to step forward and get involved. We want to hear different voices from different backgrounds and different types of practices. Everyone has something to add to make the profession

when the profession faces serious challenges.

careers in dentistry are all about.”

At the top of that list is staffing.

Another challenge she sees is supporting recent dental school graduates just starting their careers.

“Finding and keeping good, quality staff is a major challenge. It’s a lot of work ensuring that they’re trained, that they have positive attitudes, and that they recognize their role in the team,” she explains. “The supply of qualified candidates is way down.”

Ulmer points to a number of WSDA initiatives helping to address this challenge, including the partnership on workforce issues with Delta Dental, efforts to support the Shoreline Community College hygiene program at the UW School of Dentistry, legislative proposals to expand scope for dental auxiliaries, and the work of a new WSDA task force examining what it will take to train more dental hygienists and dental assistants.

“We need to be doing more outreach,” she argues. “Starting at the junior high level, we need young people to understand what

graduated less than 20 years ago. They leave with so much debt that they have to make money right now to start paying off those loans, and we’re seeing more practice models as a result.”

Staffing and educational expenses are just two of the major challenges facing dentistry today and in the future. Technological innovation continues to accelerate, dealing with reimbursement rates that don’t keep up with rising costs, and an increasing emphasis on total body care all will bring further changes.

ALWAYS LEARNING Looking back at where she’s been and thinking about where she’s going comes naturally to Ulmer. She considers herself a student of history, both in her reading and in her entertainment preferences. Historically based Masterpiece Theatre series like “Poldark” and “Downton Abbey” are among her favorite programs.

She also gleans important life lessons from her personal history. “When you’re younger, you may do things primarily because they’re fun,” Ulmer remembers. “But then you look back and realize what you learned along the way.” As an example, she feels that working with youth in her church helped her learn to value the mentorship role and to be a better mentor to others.

She says she also learns from her patients by listening closely to them.

to see Medicaid patients, but over time she concluded that serving that population — one all too often overlooked — is important. So now, in addition to providing some care to teen patients covered by Medicaid within her practice, she also volunteers at the IDEA Clinic.

Sponsored by the Spokane District Dental Society in conjunction with the Yakima Valley Farm Workers Clinic and Riverstone Family Health, the IDEA Clinic serves adult Medicaid patients. A group of local dentists take volunteer rotations to provide a combination of general and specialty care.

As usual, Ulmer ended up taking a leadership role, according to Dr. Nick Velis, another clinic volunteer.

“She worked as our foundation’s clinical director when we were in need of someone to help provide care for these underserved patients. She would take patients into her office if they needed to be seen outside of her days working at the clinic,” he recalls.

Ulmer found the work extremely satisfying, even if clinic patients can be a tough population to serve, sometimes proving difficult to contact and often less-than-reliable about showing up for appointments. The hoops that have to be jumped through to

secure state benefits are another challenge.

But the IDEA Clinic remains a success and continues to attract patients from throughout Eastern Washington and sometimes beyond.

Seeing Ulmer contribute to the success of the IDEA Clinic comes as no surprise to Dr. Amy Winston, who also has known her since she was in dental school.

“Ashley lives a life of service and seeks opportunities to impact the lives of the most vulnerable patients in our state. She’s spent countless hours volunteering her time both in dental and non-dental capacities. When Providence Sacred Heart expressed interest in starting a dental residency, Ashley was the first person I called. She served on the steering committee, planned and participated in fundraisers, and lobbied lawmakers to secure funding. I can honestly say that without her involvement, this program would not exist,” Winston says.

Explaining her multiple volunteer contributions, Ulmer says simply, “The state system isn’t perfect, but it’s what we have to work with.”

Maybe improving that system is the next big challenge this busy person will take on when her WSDA term is over. n W

Ulmer is also quick to apply lessons learned in one setting to the challenges found in another.

She served for several years on the board of directors for Spokane’s DETOX services, which provides emergency care, overnight holding, and other services for those battling addiction issues. It was a volunteer assignment she calls “totally incidental,” but she walked away with a better understanding of local, state, and federal funding streams for social and public health services, and she has been able to apply that knowledge in her work with the Spokane IDEA Clinic.

The first adjustment for WSDA’s first Retro program year will be calculated in February 2022 by L&I. The chart below is a visual representation of a series of program years starting with 2020-21 and the timing of each adjustment for each program year.

Retro is a voluntary program through the Department of Labor and Industries (L&I) where Washington state-fund employers can earn a refund of some of their workers’ compensation premium they pay each year.

Refunds are determined by comparing the collective group standard workers’

Insurance You Need When Purchasing a Practice or Refinancing

A

t this time of year, dental practices are being purchased, startup practices are opening, and many offices are remodeling or just refinancing their business loans.

Whether obtaining a loan to purchase a practice or refinancing an existing loan, your lender will require insurance for collateralization. Most lending institutions require several types of insurance coverage to be in place to protect their interest should you become disabled, die, or experience physical damage to your office. The best (and least expensive) way to satisfy the bank is by purchasing Business Loan Protection, Term Life Insurance and a Business Owners Policy.

WDIA highly recommends that you begin the insurance application process prior to or early on in procuring your loan. The underwriting for these policies takes time, and you would want the policies approved before your loan is finalized.

s WSDA and Delta Dental of Washington continue to grow their relationship and work towards common goals, the two organizations, along with the School Nurse Organization of Washington (SNOW), are partnering together to further grow The Tooth Fairy Experience — a free statewide program designed to make dental health education fun for kids and reduce cavities.

The award-winning interactive program, presented free by Delta Dental, is designed to teach K-2 students how to care for their smiles, to reduce the incidence of tooth decay, and to increase the number of children who regularly visit the dentist.

Nearly 30,000 Kids Reached in the First Year In its first year, the program reached nearly 30,000 children statewide via in-person and virtual classroom and community activations.

The program is presented in schools by teachers (using free downloadable materials) or by Delta Dental’s diverse team of tooth fairies who visit classrooms (both virtually and in-person) to share messages about caring for your smile, the importance of baby teeth, and the role nutrition and healthy lifestyles play in good oral health. In addition, the team

of tooth fairies travels the state delivering the presentations to children at libraries, museums, community centers, after-school programs, and other public places where kids gather. The program utilizes custom children’s storybooks, videos, dental health puppets and science experiments to engage children in caring for their smiles.

Cause for Collaboration The important role of dentists in addressing airway breathing disorders

QUICK BITES • Dentists play an essential role in assessing patients’ risk for sleeprelated breathing disorders and referring patients accordingly.

• Sleep-related breathing disorders (SRBD) are disorders characterized by disruptions in normal breathing patterns, including snoring and obstructive sleep apnea (OSA).

It’s no surprise that many people have found it tough to get a good night’s sleep in the last year.

The COVID-19 pandemic has changed people’s sleep patterns around the world, as quantified by the 2021 Phillips Global Sleep Survey. Almost half of the survey’s 13,000 respondents in 13 countries said the COVID-19 pandemic negatively impacted their stress.

This, in turn, took a toll on sleep, with 70 percent of respondents reporting one or more new sleep challenges since the beginning of the pandemic.

This “coronasomnia” is presenting itself in a number of different ways in dental offices, including bruxism and other sleep-related breathing disorders (SRBDs). This puts dentists in the unique position of being able to identify potential health problems in their patients beyond the oral cavity.

Because many patients see their dentists more frequently (at least twice a year) than their primary care physicians — and spend more one-on-one time with them — dentists are in a unique position of being able to identify or address the impact of SBRDs and other chronic diseases on oral and overall health outcomes.

In fact, just this July, the Journal of the American Dental Association (JADA) published a risk assessment tool to help dentists screen for psychological conditions, pain disorders, sleep disorders, and orofacial

symptoms when assessing patients’ risk of developing temporomandibular disorder pain.

And in 2017, the ADA House of Delegates adopted a statement on the role of dentistry in the care of patients with sleep-related breathing disorders. “Dentists can and do play an essential role in the multidisciplinary care of patients with certain sleep-related breathing disorders and are well-positioned to identify patients at greater risk of SRBD.”

EXPANDED ROLE IN CARE — NOT IN SCOPE Dr. Steve Carstensen, a Bellevue dentist who specializes in sleep-related breathing disorders, agrees that dentists can and should play a significant role in recognizing whether a patient is at risk for a medical problem.

“We have more ability to change the direction of community health than any other part of the workforce,” said Carstensen, who is a diplomate with the American Board of Dental Sleep Medicine and a consultant to the American Dental Association for sleep-related breathing disorders.

Carstensen is clearly an advocate for the profession but notes that while dentists are well-positioned to identify medical issues

in their patients, they must take care not to exceed their scope of practice.

Every licensed dentist is legally supported in placing oral appliances for treating SRBDs, but the lack of training in most dental education programs requires dentists to acquire additional education to include oral appliance therapy (OAT) within their scope of practice, wrote Carstensen in an April 2020 journal article on dentists and SRBDs. “Dentists are not licensed to treat patients for SRBDs if those patients have never been diagnosed by a medical doctor,” Carstensen wrote. Collaborating with medical colleagues, the “airway aware” dentist can be a crucial part of the treatment of the large number of at-risk adults, he noted.

“Since there isn’t a single entity in medicine that can treat sleep apnea, it requires collaboration — surgeons, doctors, allergists — many parts of medicine are required.”

“By definition, dentists don’t diagnose medical conditions. Our scope of practice does not allow us to diagnose sleep apnea. But the powerful role we play is that we recognize people at risk for these conditions and help them find their way into the diagnostic pathway,” he said.

“The diagnostic process remains the bottleneck in sleep apnea treatment. The number of people who can be guided into that is dependent on health care professionals who

can recognize people at risk,” he said. “The fact that there are 193,000 dentists across the country who can look for those signs and symptoms is what can impact community health.”

Carstensen adds that there are discrepancies from state to state over who can send patients home with diagnostic equipment for SRBDs.

In Washington state, the answer to that question is in the hands of the Dental Quality Assurance Commission, or DQAC, which regulates the practice of dentistry.

As Carstensen notes, dentists use tools all the time to identify medical problems — blood pressure cuffs, x-rays. It’s what dentists then do with that information that matters.

“If I take a simple x-ray and I see something that doesn’t look right, my responsibility is to find an expert to determine what that is. Many dentists and pediatric dentists use medical devices and if something unusual shows up, we’re doctors. We have a responsibility to point that out. In my view, home sleep apnea test is the same thing. The danger is what happens if the numbers come back indicating sleep apnea — we can show test results out of the healthy range, but not provide a diagnosis. So, you have to define very carefully how the information is used.”

Treating sleep-related breathing disorders is the most common non-surgical way dentists get into treating medical problems, said Carstensen.

“Since there isn’t a single entity in medicine that can treat sleep apnea, it requires collaboration — surgeons, doctors, allergists — many parts of medicine are required,” he added.

BILLING AND INSURANCE Recognizing possible signs of a SRBD is one thing. Billing for the service and working with insurance is another. Often, the billing and insurance concerns are where dentists may need to seek additional assistance when working with patients on these kinds of medical concerns.

For instance, none of Dr. Carstensen’s SRBD treatments are covered by dental insurance because sleep apnea is considered a medical problem.

“Dentists need to submit through medical to get coverage,” explained Carstensen. “It’s not a complete barrier anymore. We have the same choices we always do with health care — people can choose to pay, and we can choose to work with insurance.”

PEDIATRIC BREATHING ISSUES The next frontier for airway-aware dentists: pediatric breathing issues. Carstensen said it’s much easier to address breathing issues when patients are younger than with an adult.

“Today, I may see a 55-year-old patient with high blood pressure who has chronic sleep problems. But that same patient was once a five-year-old, and their jaw structure may have been limiting their ability to breathe,” he explained. “It’s easier to address that in a five-year-old than in a 55-year-old. No health care provider is more capable of doing that than a dentist. We can impact habits, growth patterns and breathing issues from infancy,” he said.

Carstensen leads the ADA’s Children’s Airway Initiative and formed a task force that has developed a new screening tool for use in dental offices, pediatric offices and school health rooms worldwide. The tool is in validation phase now and the task force hopes to have it ready for adoption by the ADA House of Delegates in 2022. He is also collaborating with the ADA on a brochure about children’s airways, which details the importance of sleep, signs that parents should look for, and how dentists can help identify disordered breathing problems.

ADA Supports Health Enterprise Zones Act The ADA is supporting legislation to improve health disparities in minority and underserved communities.

In a July 22 letter to Rep. Anthony Brown, D-Md., ADA President Daniel J. Klemmedson, D.D.S., M.D., and Executive Director Kathleen T. O’Loughlin, D.M.D., thanked the lawmaker for introducing HR 4510, the Health Enterprise Zones Act. “We offer our strong support for this legislation as we believe that it is vitally important to address health equity issues such as barriers to oral health care so that all Americans have the opportunity to become healthy,” Drs. Klemmedson and O’Loughlin wrote.

“Dentistry is essential health care and integral to a person’s overall systemic health,” Drs. Klemmedson and O’Loughlin said.

If enacted, the ADA believes the bill would help attract more dentists to underserved communities through tax incentives, grants and student loan repayment options which would help more underserved Americans obtain access to dental care.

“Attracting more dentists to these underserved communities would also help expand oral health education and prevention and establish a permanent dental home for patients to receive both routine and complex care,” Drs. Klemmedson and O’Loughlin added.

Retroactive Denial Laws a Priority for ADA Advocacy People don’t become dentists to spend their time navigating the insurance claims process.

They do it to take care of people.

Unfortunately, the reality of dealing with insurance carriers and their varying claims policies can create headaches not only for dentists and their front-office staff, but for patients as well.

“It can be particularly frustrating when it comes to insurers’ retroactive denials,” said

no longer be protected from some of the federal antitrust laws.

Insurers typically limit dentists’ window of time to file clean claims, meaning any claim filed after their arbitrary deadline can result in no payment to the dentist. And, with retroactive denials, dental insurers have the ability to review claims after payment has been delivered to the dentist. Even in those instances when the insurer made an error, they can request a refund from the dental office years after the claim was originally paid.

In a July 20 letter to both agencies, Sens. Patrick Leahy, D-Vt., and Steve Daines, R-Mont., said “public information is lacking” on what steps the FTC or DOJ have taken to extend antitrust enforcement to the health insurance industry.

The ADA and state dental associations recognize the pain points these types of retroactive denials cause, so they have been working together in state legislatures to enact laws that restrict the timeframe in which insurers are allowed to request such a refund.

“The ADA is confident that increased competition among health insurance and dental plans will lead to improved coverage for patients and more attractive terms for dentists who participate,” Dr. Klemmedson said. “It’s a win for everyone.”

“While direct communication with their contracted insurer can be a method for dentists to solve day-to-day concerns, it can take passage of laws to truly rectify problems,” Dr. Watson said.

Retroactive denial laws simply require that when insurers make a payment error, they must execute their refund demands within a certain amount of time. Over half of states — counting Nevada and North Dakota added to the list this year — have such a law and they mirror the insurers’ time limit for dentists to file a claim, typically 12-18 months. However, the laws are not so rigid that they don’t account for instances of fraud or abuse or complicated claim adjudication.

ADA President Daniel J. Klemmedson thanked the senators for sending the letter and said the Association hopes it will lead the agencies to investigate possible anticompetitive practices and activities of health care insurers.

Senators Press FTC, DOJ on Antitrust Reform Two senators are asking the Federal Trade Commission and Department of Justice for an update on their efforts to combat anticompetitive conduct in the health insurance industry following the repeal of the McCarran-Ferguson antitrust exemption.

ummer term is always an exciting time for the School of Dentistry.

We have the rising fourth-year students entering their final year with us, focusing on perfecting their clinical abilities and thinking about the next phase in their careers. We also have the new firstyear students joining us and starting their immersion in a curriculum that is quite different from their undergraduate education.

However, this year’s summer term has been particularly exciting in that we have the Shoreline Community College dental hygiene students joining us. Preparations for this partnership have been moving along for over a year with facilities renovations and logistical planning. In addition, this collaboration has helped to reenergize our philanthropic activities, and we have been successful in attracting funding from both corporate and individual donors to support this program. I think this reflects a keen understanding in the dental community about the substantial shortages in the dental hygiene workforce and a sincere appreciation for the excellence of the Shoreline program. It also recognizes the unique opportunities we have when dental hygiene students and dental students can learn together. Certainly, having the second-year dental hygiene students practice expanded functions with fourth-year dental students provides them with experiences that reflect what they can expect in general dental practice.

Our patients benefit greatly, and our students graduate with a more comprehensive view on having the oral health care team practicing at the top of their licenses. It was wise to reduce the dental hygiene class size for this inaugural year while fine-tuning the logistics and details of this new arrangement, but it is also exciting to look forward to larger classes to better address the needs in our state.

it was the safest approach for dental student clinics where there are quite a few high-speed hand pieces spinning at the same time. We confidently removed that requirement when the Seattle/King County vaccination prevalence passed 70 percent and UW implemented a vaccination requirement for faculty, staff, and students. That was a major pivot point for us, and the patient census recovered rapidly. As I hear from many in private practice, our patients often delayed dental care because of COVID concerns. With increased vaccination rates, people felt confident in scheduling their overdue appointments and returning to our care.

A final bit of good news for our school is that, despite a pandemic that decimated our clinical production for a while, we managed to close fiscal year 2021 in a revenue-positive position.

Mind you, it was not spectacularly positive, but it was much better than early projections based upon clinical revenue. Several things that eventually worked in our favor led to this excellent outcome. We were able to provide sufficient documentation to support two separate payments from CARES Act funding, and that helped to cover part of our increased costs related to COVID. In addition, UW received substantial funding from the Higher Education Emergency Relief Fund and shared some of that with us to help make up a portion of lost revenues. All of that helped immensely in achieving a positive close to the fiscal year, even with increased expenses and decreased revenues. We are now back on the encouraging fiscal trajectory that existed pre-pandemic.

What a difference a year and a half makes. A year ago, we were biting our nails, thinking about creative ways to keep moving forward while protecting the safety of our patients, faculty, staff, and students. We were meeting via Zoom and adjusting schedules and calendars to reflect the demands of a virus. Support the Shoreline Community College Dental Hygiene Program at UW

ast year, WSDA and Delta Dental of Washington (DDWA) committed to working together to find solutions to critical issues facing the dental profession. One of the most pressing issues — well-known by dentists across the state — is the dental hygienist workforce shortage.

In a joint effort to gather workforce data from multiple stakeholder groups, WSDA and the Washington Dental Hygienists’ Association developed surveys for dentists, dental hygienists, and dental assistants that were administered by the Washington State Department of Health in late 2020. The results, analyzed by DDWA and WSDA, found that: • Dental hygienist positions in Washington are open an average of 4 months before being filled.

MBDDS Gifts of Gold Program

This past year, Mount Baker District Dental Society — led by the efforts of Dr. Ronald Dahl — launched Gifts of Gold, a program of charitable giving through the collection and refinement of gold from tooth extractions donated with patient permission. Several MBDDS members participated in the first round, which raised $9,462.40 that was donated to the Boys & Girls Clubs of Whatcom County. This fall, MBDDS plans to expand the program to also benefit the Boys & Girls Clubs of Skagit County.

Dr. David Carl Dilts

Dr. Norbert Horst Hertl

Dave was born in Colfax, WA. His first years were in Garfield, WA. The family moved to Waitsburg, WA after his dad purchased The Waitsburg Times. He attended Waitsburg grade school, and WHS where he played basketball, and was active in Boy Scouts where he achieved the highest honor of Eagle Scout.

Norbert Horst Hertl was born on May 26, 1941 and passed away on June 3, 2021 and is under the care of Neptune Society.

He attended WSC for a year after which he enlisted in the U.S. Air Force in 1952. He spent 3 years in Waco, Texas. His next tour was Wiesbaden, Germany.

He married Beth Ann Brinker on April 24, 1955. They were in Germany a year before his honorable discharge from the service in 1956 in New York City. They drove a 1956 Volkswagon across country with their Scottish Terrier “Fritz”. They moved to Eugene, OR where he attended the University of Oregon for two years in Pre-Dental School. When the Ducks played the Cougs he never had a loser. They moved to Portland for four years of Dental School at the U of O. Their son was born in April, 1962, and Dave graduated in June. They moved to Walla Walla, where Dave built their first home and started his Dental Practice in the Drumheller Building. He designed a dental building to house four practices. It was built on South Second and opened with four active dental practices in 1971.

He loved Priest Lake and built their first cabin at Priest Lake in 1967. He designed and built their second cabin on Paradise Point years later. He designed and built their second home in Walla Walla and few years later retired to the golf course.

He was past President of Exchange Club and Prospect Point PTA. Member of local Elks.

He was active in both local Dental Society and Washington State Dental Society and a member of the Aircraft Owners and Pilots Association.

He is survived by his wife of 67 years, Beth Ann Dilts; his son, Daniel Carl Dilts and wife Teresa, (daughter, Dana Ann preceded him in death); granddaughter, Elizabeth Marjorie (Elie) Johnson; grandson, Berton Daniel Palmer (Danny) Dilts and wife Savannah, greatgranddaughter, Hazel Grace Dilts; brother, Rodney Dilts and his son, Scott Randall Dilts of Kingman, AZ. As well as by numerous nieces and nephews and four survivors of the original Tuesday-Friday.

enthusiasm to spare he carted his three kids up to the mountains when young, teaching them how to carry their own gear and gracefully fly down the ski runs as he did well into his later years before his knees begged retirement. Sun Valley Idaho, Mt. Bachelor Oregon, and Crystal Mountain Washington were his favorite places to schuss down the slopes as he loved to say.

Dr. Richard Calvin Knoll Kind, generous, adventurous, Dr. Richard Calvin Knoll lived a full and content life centered around his family, his dental practice and cherished lifelong friends.

He passed away peacefully in his sleep of natural causes, surrounded by loved ones at the age of 89.

Preceded in death by his parents Frederick and Elsie Knoll, Uncle Raymond, Aunt Emily, brother Lee Knoll, nephew Alan, Rich is survived by his wife Sharon Virginia, daughters Wendy (Dan Herforth), Lori (Ken Harrenstien) son Richard C. Knoll Jr. (Carolanne Kane), nephews, nieces and 7 grandchildren.

He quietly persevered through life’s highs and lows, always imbued with a sense of gratitude and dignity. He will be especially remembered for his enduring dedication to Dentistry, community service, love of downhill skiing, the outdoors, hot fudge sundaes, British sports cars, and the utmost care with which he took raising a family as a husband to Sharon and father to Wendy, Lori and Rich Jr. Rich was born in Reading, Pennsylvania along with his brother Lee to Dr. Frederick W. and Elsie Knoll. His father, a Physician of Family Medicine tragically died when Rich was a toddler and the boys grew up under the guidance and love of their indomitable mother Elsie and their Uncle Raymond & Aunt Emily Edes. Rich attended Franklin and Marshall College for his undergraduate degree followed by Dental School at the University of Pennsylvania. Rich then joined the Army, stationed in Fairbanks, Alaska. Upon completion of his Army duties he settled in the Pacific Northwest and started his career as a dentist, first interning at Pierce County Hospital and later joining the office of Dr. Bert Goodman in Tacoma, Washington. He met his beautiful wife, Sharon V. Nourse in the local Bank where she worked. They married and settled in Fircrest, Wa. where Rich established his private dental practice in an office building designed and built by local friend and architect Walt Woodmeyer. His office staff through the years were always professional, with good humor and kindness at the ready.

Rich was active his entire life. Competitive swimmer, lifeguard and camp counselor in youth, he enjoyed home gardening and boating on Lake Chelan and Puget Sound. With

Dr. Randall “Randy” Stephens Randy joined our heavenly father on December 14th after a brief illness of cancer. He was brave, loving, faithful, and concerned for others as he endured an aggressive disease. Randy grew up in Midland, Michigan, and in the middle of 7th grade his family moved to Salinas, California when his father started managing a new plant. After high school, Randy attended Whitworth College in Spokane Washington and went to dental school at USC in LA. After practicing dentistry in California, he moved back to Spokane where he worked for most of his career. Randy loved his profession and was very supportive of other dentists and active in his dental community. He considered his patients friends, and had true friendships with his employees. Randy was a joyful person who loved to smile and laugh.

His passions were: his wife and family, USC football, all sports particularly tennis and bike riding, morning walks or runs, hot stock tips, his friendships that went back to junior high, high school and college, his Christian faith, art and music, travel and experiencing other cultures, the outdoors, growing a vegetable garden, and his true enjoyment of other people. His family that will miss him dearly include: his wife Jesse, his 3 children Nick, Andrea and Anthony, his father Bob, several brother and sister in-laws/ nieces and nephews, 1 uncle and 2 aunts, and around 55 first cousins. Those in his family that went to heaven before him include: his brother Ron, his mother Rae, several aunts, uncles and cousins, and grandparents. Randy has 4 generations of his family buried in Spokane.

The COVID-19 Pandemic: A Year of Living Dangerously

he COVID-19 pandemic was a year of living dangerously. Actually, it was several months longer than a year. Dental professionals went back to work in the spring of 2020. We, it seems, are essential workers — a much used and abused title. And it was a risk, when placed in context, in the spring of 2020.

We know much more now than we did then, when we donned our custom-fitted and uncomfortable N-95 masks, made costly structural changes, isolated and controlled our aerosols, wiped down exposed surfaces, bought and used literally gallons of hand sanitizer, and fundamentally changed the way we run our offices and clinics. So, let us consider the changes we have made and the costs we have incurred in the last year. We will be paying these bills for years to come, in one way or another.

Infection control measures, now routine, are the easiest to live with. We know how to do it and the increased cost of running a compliant office have been accepted and factored into the system. On balance, we and our patients are safe. Dentistry has an enviable record with regard to infection control over the years. Navigating the COVID crisis is yet one more example of our ability to make dental care safe. Many of us remember similar crises in the past. Reference here Hepatitis B, then HIV, then Hepatitis C. I was much more afraid of those viruses than COVID-19. However, it can be argued that the COVID pandemic has had a greater impact on our profession.

Many dentists have gone to limiting patient visits. Having fewer patients who call before they walk in and get their temperature taken and hands sanitized minimizes the risk of transmitting the virus. Instead of seeing ten or twenty patients a day, we might see half that many and increase the length of their appointments to get more dentistry done while those patients are in the office. Find a problem at recall and fix it the same day rather than schedule another appointment.

The risks and the cost of infection control are thus minimized.

We can provide certain types of care over the phone or through a HIPAA-compliant online service. This also minimizes traffic through our offices and clinics and is convenient for

patients. The limitations are obvious. Historytaking, informed consent, post-operative checks, and the business side of dentistry, for example, can all be done well online. Delivery of most dental care, however, still must be done one-on-one in very close proximity to our patients, precisely in the anatomic area that poses the most risk. We have done it safely and effectively.

It is fair to say, however, that we are all waiting for a variant of COVID-19 or a completely new virus to shut us down again.

The old way of doing things is in the rear view mirror. As we move ahead, we take with us a certain anticipation and a nagging fear that it will all happen again. How long until the next pandemic? What sort of economic impact will it have? What preparations must I make to get my family and my practice through the next one? Will we be able to hire staff to help us?

We have yet to appreciate the real cost of the COVID-19 pandemic There are economic costs, certainly, and they continue to mount as the pandemic drags on. There are tremendous medical costs as well. I recently heard from a good friend about one of his patients who was afraid to get routine dental examinations in 2020. That patient has now been diagnosed with a large carcinoma of the tongue that would no doubt have been detected during a routine dental recall examination. Then, there are the social costs.

Our children, especially our teenagers, were — and are in many cases — vulnerable to the effects of the isolation that accompanied the lockdown. These costs are impossible to quantify. They are real, nonetheless.

This year is better than last year for sure. The dental profession stepped up and successfully dealt with yet another nasty viral illness.

There is no doubt we have learned a lot about many things. Could it be that we are stronger, and more resilient, as we enter the new normal? I think I will go with that. n W
