Reporting on the Three Gs

The recent World conference in Sydney was a much welcome opportunity to spend time in person with many WONCA friends. November WONCA News was a lovely tribute to the event and the camaraderie. Still prominent in my mind are the three words summing up what I want to achieve in the coming two years: Governance, Greening and Growth. An update on each follows:

WONCA has a new Executive Committee and I look forward to working with them. We have met in Sydney and again by zoom, for induction and short meetings where we have made decisions on who will lead our WONCA Standing Committees in 2024 and 2025.

See lists of all members of Standing Committees here The Standing Committees carry on important work on behalf of Council between world conferences, and are a key part of the governance structure of our organization. I express my gratitude to those who have agreed to serve WONCA in this manner.

TASKFORCE ON MEMBERSHIP AND VOTING Those of you who were at WONCA Council in Sydney will know that we have a new Taskforce on Membership and Voting, to be chaired by Canadian colleague, Dr Francine Lemire. The Taskforce is to consider issues relating to Membership and Voting, including relating to countries with more than one WONCA Member Organization. I anticipate that many of you will be consulted as we want input from many different groups of stakeholders. The final report of the Taskforce will be presented to the WONCA 2025 World Council, in Lisbon.

GREENING

UHC

REALISATION OF A DREAM” AT COP28 As I write, there is news from COP28 in Dubai: “COP28 Presidency joined with the World Health Organization to announce a new ‘COP28 UAE Declaration on Climate and Health’ (the Declaration) to accelerate actions to protect people’s health from growing climate impacts… Signed by 124 countries, the Declaration… marks a world first in acknowledging the need for governments to protect communities and prepare healthcare systems to cope with climate-related health impacts such as extreme heat, air pollution and infectious diseases.” Source COP28 Media hub

However, the health declaration does not mention fossil fuels: our Working Party on Planetary Health has led a Call to Secure a Just & Equitable Transition to Sustainable Energy and Safeguard Health to end the proliferation of fossil fuels. I encourage you all to sign this petition if you have not done so already. Download the WONCA Call to Secure a Just & Equitable Transition to Sustainable Energy & Safeguard Health.

The declaration calls for climate action to achieve “benefits for health from deep, rapid, and sustained reductions in greenhouse gas emissions, including from just transitions, lower air pollution, active mobility, and shifts to sustainable healthy diets.”

Dr Maria Neira, who was a keynote speaker at our WONCA Sydney conference, and who is Director of the WHO’s Department of Environment, Climate Change and Health is quoted as saying “This is the realization of a dream for which the global health community has been fighting for years… The climate crisis is a health crisis.”

For WONCA, 2024 brings six regional conferences and a world rural conference. Plenty of opportunity for everyone to again experience the joy of a WONCA conference, so start planning. If you missed Sydney then there is an event close to you in 2024. As your new President, I will be hoping to spend time in all regions over the next two years.

2024 The Gregorian calendar year of 2023 is coming to a close and we look forward to 2024.

Universal Health Coverage (UHC) day was held on December 12. Growth is desperately needed to achieve UHC. WHO says that the world is off track to make significant progress towards universal health coverage by 2030 (Sustainable Development Goals (SDGs) target 3.8). This pattern is global - consistent across all regions and the majority of countries. UHC targets measure the ability of countries to ensure that everyone receives the health care they need, when and where they need it, without facing financial hardship. It covers the full continuum of key services from health promotion to prevention, protection, treatment, rehabilitation and palliative care. Don’t be fooled – even in countries where Universal Health Coverage seems to exist it may not be as “universal” as you presume. I am sure most of us don’t need to think for long to realise UHC is deficient in our own countries. I think of workforce shortages, inadequate funding and many people who cannot get the care they need when they need it in my health system. It doesn’t matter where we are in the world to achieve UHC requires growth in advocacy for strengthening primary care services, resources and funding including of family medicine / general practice. UHC matters. Universal health coverage (UHC) (who.int) I take this opportunity to wish you and your families good wishes for the festive and holiday season that many of us enjoy at this time of year. I am sure we all hope and pray for a peaceful 2024.

December 2nd, 2023 AUTHORS: DR ADIL H. HAIDER AND DR UNAB I. KHAN It is with profound sadness that we announce the passing of our founding Chair of Family Medicine, Professor Riaz Qureshi. Dr Riaz Qureshi is widely acknowledged as the “Father of Family Medicine” in Pakistan, and our surrounding region. He graduated from Dow Medical College in 1966 after which he spent nearly two decades in the UK and then the Middle East. He returned to Pakistan hoping to introduce the specialty of Family Medicine (FM) and founded the first section of FM in Pakistan at AKU within the Department of Community Health Sciences. Over the next 10 years he demonstrated the impact of superb clinical training in primary care for the country’s health system. These efforts culminated in the creation of the Department of Family Medicine in 2003 at AKU where he was honored to serve as its founding Chair. The establishment of the first Dept of FM represented yet another milestone for both Pakistan and AKU. Dr Riaz was a visionary whose enthusiasm for FM was infectious. In his gentle, collaborative and inclusive manner, he helped in creating an excitement for FM and its potential. He spearheaded the development of the Royal College of General PractitionersInternational (MRCGP-INT) program and was instrumental in introducing it to Pakistan and South Asia. For this work, he was awarded the Royal College of General Practitioner’s President’s Medal, and he remains the only South Asian who has received this honour.

To date, MRCGP (Int.) has helped in the professional development of thousands of general practitioners in South Asia. He was a sought-out advisor internationally and helped with the creation of FM training programs in Saudi Arabia and the Middle East. He was awarded Fellowship of the RCGP for exceptional contributions to teaching and training of FM in Saudi Arabia and UK. As a mentor and teacher, Dr Riaz's influence spanned nearly three decades, touching the lives of every resident and faculty member who joined the FM department at AKU. Beloved by students at all levels, he earned multiple Outstanding Teacher awards, not just at AKU but also at various institutions across Pakistan and the region. Today, many of his residents and trainees lead FM departments and programs throughout Pakistan. Dr Riaz’s clinical and communication skills are legendary. He truly enjoyed seeing patients and his sessions on hypnosis for treatment of addictions and stressmanagement were highly acclaimed. His holistic approach made him a loved and respected physician for three generations of loyal patients. He also believed in family physicians leading care in various settings. Under his leadership, AKUH initiated the medical centers in Malir and Rashid Minhas, that continue to provide accessible and quality care. He also initiated the department’s occupational health program, through which family physicians and nurses provide care across Pakistan including far flung areas and even petroleum rigs.

Beyond his academic achievements, Dr Riaz was a compassionate individual who was loved dearly by all who knew him. He was always available to advise, mentor, and teach, enriching the lives of faculty, residents, medical staff, nurses, and administrators with his humor, positive outlook, and spontaneous verses of Urdu poetry. Professor Riaz Qureshi is survived by his wife, Dr Zubaida Qureshi, and their three daughters. We extend our deepest condolences to his family. His legacy will live on in the countless lives he touched as a physician, teacher, and mentor, as well as through the work of his students and trainees. He will be profoundly missed.

THE FAMILY VIOLENCE PROJECT A collaborative effort by: Special Interest Group on Family Violence WONCA Working Party on Women and Family Medicine Young Doctors' Movements We are proud to present The FaV Project, a vital initiative under our Collaborative Fund aimed at tackling the shadow pandemic of Family Violence and Intimate Partner Violence. In the wake of COVID-19, the surge in violence against women has called for urgent action. WONCA’s response? A comprehensive E-Learning Program for Family Doctors worldwide. The FaV Project, part of the WONCA Collaborative Fund, is designed to address this urgent issue through: A comprehensive E-Learning Program on the WONCA platform. DIGITAL HEALTH TRAINING FOR GPS AND FAMILY DOCTORS A collaborative effort by: WONCA Working Party on eHealth Working Party on Research Special Interest Group on Policy Advocacy We are also excited to unveil our latest initiative: Digital Health Training in General Practice/Family Medicine. This project is a concerted effort by WONCA’s Working Party on eHealth, Working Party on Research, and Special Interest Group on Policy Advocacy. As primary care faces increasing demands, the integration of digital health technologies is no longer optional but essential. Our project seeks to assess and enhance the digital health literacy of Family Doctors globally. Through a comprehensive mixed-methods approach, including surveys, consensusbuilding, and focus groups, we aim to: Evaluate current digital health literacy among Family Doctors. Map existing training programs. Identify critical training needs. Provide actionable recommendations for curriculum integration. The digital revolution in healthcare is here, and with your support, we can ensure that Family Doctors are fully equipped to navigate this new landscape with confidence and competence. The Irish College of General Practitioners (ICGP) has hosted a number of very successful international conferences in the past - let us host you so you can join your thinking and energy together with colleagues to create an inspiring conference in Dublin in 2024. WONCA WP ON RURAL PRACTICE

UBUNTU 2024 UBUNTU 2024 will take place in person and virtually in Cape Town, South Africa from the 10th to 13th of September 2024 Ubuntu 2024 is a joint conference between The Network: Towards Unity for Health (TUFH) and Rural WONCA (The Working Party on Rural Practice). It’s bringing together the TUFH 2024 Annual Conference for TUFH, which is celebrating its 45th anniversary in 2024, and the 20th WONCA World Rural Health Conference! This conference is being co-hosted by 5 partners in Cape Town, South Africa: University of the Western Cape; University of Cape Town; Stellenbosch University; Cape Peninsula University of Technology, and the Department of Health and Wellness – Western Cape Government. This joint conference has been named Ubuntu 2024; referring to the meaning of Ubuntu “I am, because you are”. Ubuntu 2024 will be a fully hybrid conference. We strongly recommend attending in person, but for those unable to travel, we will provide the opportunity for virtual participation.

WONCA WORLD CONFERENCE 2025 LISBON NEW VISION FOR PRIMARY HEALTH CARE AND SUSTAINABLE DEVELOPMENT WELCOME MESSAGE Dear Friends and Colleagues, Family Doctors working in teams with other health professionals and involving communities create a unique ecosystem to provide the right care in a sustainable and appropriate way. This goal combined with Hippocratic principles will keep us as the most advanced defence line of Medicine, facing challenge times. SOCHIMEF ORGANIZED THE XXIV CHILEAN CONGRESS OF FAMILY MEDICINE The event, hosted by the Chilean Society of Family Medicine, featured about 50 speakers, including former Health Minister Soledad Barría, Health Reform Coordinator Bernardo Martorell, actress and former Minister of Culture Paulina Urrutia, and other distinguished professionals. From Thursday, November 23, to Saturday, November 25, the Chilean Society of Family Medicine (Sochimef) held the XXIV Chilean Congress of Family Medicine, with over 50 speakers, at the Aula Magna of the University of La Frontera in Temuco, Region of La Araucanía.

Over the three days of the event, five plenary sessions were held, addressing topics such as universalization and community; epidemiology of cardiovascular diseases; as well as roundtable discussions related to the National Dementia Plan, palliative care, sexual diversity, neurodiversity, insulinization, among others.

In this edition, emphasis was placed on the challenges posed by the healthcare reform promoted by President Gabriel Boric's government, especially concerning the universalization of primary care and the role that family medicine should play in the public policies expected to be implemented in the medium and long term. The congress featured presentations by academics, researchers, healthcare professionals, and various political authorities. Notable figures included former Health Minister (2006-2008) Soledad Barría, Health Reform Coordinator Bernardo Martorell, Head of the Division of Prevention and Control of Diseases (Diprece) María Soledad Martínez, former Minister of Culture Paulina Urrutia, the president of the Chilean Society of Family Medicine, Roxana Sepúlveda, and other distinguished professionals. For the committee of the XXIV Chilean Congress of Family Medicine, this event represented an opportunity to "think of universalization as a great challenge. At the same time, we must continue to work to inspire and train more family physicians. Several universities have started their process in recent years, so we must find ways to expand our knowledge and our work. The Chilean Congress of Family Medicine is an opportunity to achieve this."

UNIVERSALIZATION On April 3 of the past year, in the commune of Renca, the process of universalizing Primary Health Care began. This meant that from now on, in the seven designated pioneer communes (Renca, Canela, La Cruz, Alhué, Coltauco, Linares, and Perquenco), users not affiliated with the National Health Fund (FONASA) could enroll and receive care at primary care centers. The implementation of this public policy in Chile posed a significant challenge, whose realization had to be based on a broad dialogue among all stakeholders.

WONCA WP ON MENTAL HEALTH

MUSIC & MENTAL HEALTH: UNVEILING PHASE TWO The Wonca Working Party on Mental Health (WWPMH) started The Music and Mental Health Project (MMHP) in 2020 due to the challenges imposed by the pandemic. In 2021, four distinctive productions marked the beginning of this creative journey. These included: 1) a live event; 2) a teaser featuring the participation of family physicians from around the globe; 3) a collaborative composition titled "Tone of Mind," crafted between Brazil and India; and 4) a music video. The details of this experience have been documented in WONCA News. MMHP is currently in its second phase, dedicated to the production of a documentary showcasing the experiences of groups that have already been mapped. Australian filmmaker Tristan Pemberton is taking the helm, overseeing both the direction and editing of this project. Within the documentary, four distinct experiences, each situated in a different country, will be explored to understand the connection between music and mental health. MMHP's objective is to delve into the daily lives of these groups, capturing not only their unique musical expressions but also the implications of the relationship between music and mental well-being. The documentary will feature firsthand accounts and insights, as all of them will be filmed by the groups themselves. The experiences are: • Harmony Gets Crazy (Brazil): This band, comprised of psychiatric patients and health professionals, originated from music therapy workshops at the Rio de Janeiro Psychiatric Center 22 years ago. Currently in the process of recording their fifth album featuring original songs, the band serves as a symbol of Brazilian psychiatric reform. Alfredo de Oliveira Neto, leader of MMHP and Vice-Chair of WWPMH Iberoamerica, is an active member of this musical collective.

• The Desert Stars (Australia): Hailing from the Tjuntjuntjara Community in the remote Great Victoria Desert region of Western Australia, The Desert Stars claim the title of "the most remote rock band in the world." Their journey is documented in "Gravel Road," a film directed by Pemberton. • Know Your Rhythm (India): Arnab Chowdhury, dedicated to music therapy, collaborates with autistic children and health professionals experiencing burnout in Pondicherry. • Delisile Kubheka (South Africa): A healthcare professional and accomplished opera singer. The film is set to be finished in 2024, and our aim is to premiere it at the Wonca World Conference in Lisbon in 2025. We're also considering a musical presentation highlighting the four experiences in Portuguese lands. On a related note, MMHP has already created an extended trailer, showcased via QR code at the 2023 Wonca World Conference in Sydney. This preview has set the stage for the film's official debut in Lisbon. We received encouraging responses from amazing family doctors: "Congratulations on your work on music and mental health. I loved the video." (Michael Kidd) “Great film! Great work!” (Iona Heath) We hope MMHP helps to expand the visibility of the musical projects and sensitize mental health professionals and managers, as well as patients and their families, to the transformative potential of music as a tool for care in psychosocial suffering. WP ON WOMEN AND FAMILY MEDICINE

ACTIVITIES AT WONCA SYDNEY 2023 WWPWFM PRE-CONFERENCE: The preconference was held on the 25th of Oct 2023 at international conventional centre in Sydney, Australia from 9am to 14 hours. It was a hybrid meeting with 30 in-person attendees and 10 online attendees from various parts of the world. The theme of the pre-conference was “Wonca Woman: Working as Women and Working for Women. The chair welcomed everyone and presented the overview of the working party. Two workshops were conducted: “Global perceptive of women’s health: What can WWPWFM do?” by Prof. Amanda Howe and team and “Cognitive Impairment in Violence Survivors: PTSD and beyond" by Prof. Jinan Usta and Prof. Lucy Candib. There were robust discussions around these two topics. It was followed by a talk from Prof. Anna Stavdal on the leadership role in Wonca and “Reflection of the early days of WWPWFM” by Prof. Marilyn McMurchie. It was an honour to have Prof. Marilyn McMurchie in our Preconference as she was one of the leaders that developed this working party. The preconference was a successful event where women could connect with new and old generations and develop new ideas for the betterment of the women in the world. COLLABORATION WITH YOUNG DOCTOR MOVEMENT (YDM): During the pre-conference, we had a session in the afternoon with the female doctors from the young doctor’s movement. A workshop on “Professional career development and opportunities: Thinking ahead in the early years” was conducted by Prof. Amanda Barnard and team. More than 60 participants attended this workshop and there was robust discussion around the topic. The summary conclusion was that it was important to talk about career development and opportunities from the early days of a doctor’s professional life.

We had many females who were interested to join our working party and they have given their details to us. MAIN CONFERENCE 26TH-29TH OCTOBER 2023 AT ICC, SYDNEY, AUSTRALIA: We had about 40 abstracts accepted and various presenters had a good audience and the sessions were fruitful. AWARDS RECEIVED BY OUR DEAR MEMBERS: Awards were given to some of our dear friends on the 28th of Oct 2023. This was a green day and so we were dressed in green. THE PRACTICAL SKILL WORKSHOP: RESPONDING TO FAMILY VIOLENCE. This workshop took place on the 27th of October 203. About 70 participants attended this workshop. This workshop was conducted by WWPWFM with leadership of Prof. Amanda Barnard and in collaboration with the SIG. Family Violence group. There were great small group discussions and tips from various countries.

WONCA COLLABORATION ON CLIMATE CHANGE AND HEALTH AT COP28

Our colleague Dr Mayara Floss has been working as a member of the Health and Climate Change Capacity Building Subcommittee as a member of CIMF and of Planetary Health WP of Planetary Health. Early this year, they proposed a side event at the WHO pavilion at COP called: Integrating Health into Climate Change Responses - The Role of Health Professionals, on 9th December and I, Paola, a family doctor from Uruguay, was the representative from WONCA. This year the COP took place in Dubai from 30 November to 12th December. The aim was to address the question of how health professionals can be a powerful lever to bring a health focus in conversations and climate dialogues in non-health sectors. Referents formed it from India, the US and Latin America, academia mostly. Unlucky other representatives could not attend. It was a challenge to transmit in 3 minutes the point of view from PHC and Family Doctors regarding the relation with other sectors: transport, and food production among others and how we can add approaches to the problem and achieve better results. I also moderated another panel at the WHO pavilion: Health loss and damage in the Latin America and Caribbean region: a multi-sector approach on 11th December.

It represented a great personal experience. We worked with some colleagues from the WP previously to prepare the speech and feel their support, and a sense of “team”. I had the opportunity to contact colleagues and other health professionals who were working on this topic and meet in person with whom I have worked in the Latin America Network on Climate and Health, and attend the plenaries, and side events. There were many expressions of social movements in which we, as family doctors, shared the same target: to care for planetary health. The Conference of Parties of UNFCCC aims to achieve agreements among countries to reduce carbon emissions that ensure the global temperature will be less than 1,5º than the industrial period. It may ensure a safe environment for life on Earth and of course people’s health. As many countries, civil organizations worldwide and the IPCC have said since years. The answer for this climate crises is to reduce to zero fossil fuel production and consumption.

Despite the results of the negotiation were not as good as necessary to guarantee we are going to zero-emission, there were some positive aspects to highlight: it was the first Health Day, many minister attendees, and the representative of health professionals increased. The next COP is going to be in another oil country, but in 2025 will take place in Brazil, so we have an important mission there: to advocate until the era of fossil fuels is behind us, ensuring a healthier, more sustainable future for all. A RENEWED CALL: UPDATES ON THE 30 BY 2030 CAMPAIGN The ’30 by 2030’ campaign calls for development partners explicitly to assign 30% of their health funds towards building and improving primary care systems, to deliver integrated, comprehensive primary care services by 2030. By doing this, countries will have a far greater chance of achieving Universal Health Coverage, which is our global goal. In addition to the call to development partners, the 30by2030 campaign recommends analysis and awareness of the long-term impact of continued vertical programmes on health systems. It has been three years since the 30 by 2030 campaign was first launched with an article in the WHO Bulletin (read here). in which both the Past President Dr Donald Li and now Immediate Past President Dr Anna Stavdal collaborated. In November 2020, Dr Donald Li, WONCA Past President, delivered a message urging individuals engaged in development partnerships to be mindful of the 30 by 2030 campaign. Whether serving as advisors, consultants, grant recipients, or working within Ministries of Health seeking funding, Dr. Li emphasized the importance of awareness and adherence to the campaign's principles. Dr Li's message underscored the need for a collective commitment and encouraged stakeholders to leverage their influence and energy in a concerted effort to contribute. to the realization of the campaign's objectives.

WONCA'S IMMEDIATE PAST PRESIDENT AND WHO LIAISON REFLECTS ON 30 BY 2030 Your signature matters! In 2020 WONCA and partners launched the “30 by 2030 Campaign”, as a follow up of the Astana meeting in 2018. 16 DAYS OF ACTIVISM AGAINST

GENDER-BASED VIOLENCE The 16 Days of Activism against GenderBased Violence is an annual international campaign that kicks off on 25 November, the International Day for the Elimination of Violence against Women, and runs until 10 December, Human Rights Day. The campaign was started by activists at the inauguration of the Women’s Global Leadership Institute in 1991. It is used as an organizing strategy by individuals and organizations around the world to call for the prevention and elimination of violence against women and girls. In support of this civil society initiative, the United Nations Secretary-General launched in 2008 the campaign UNITE by 2030 to End Violence against Women, which runs parallel to the 16 Days of Activism. Every year, the UNITE Campaign focuses on a specific theme and this year’s is “UNITE! Invest to prevent violence against women and girls”. The campaign calls on citizens to show how much they care about ending violence against women and girls by sharing the actions they are taking to create a world free from violence towards women. This year's campaign also calls on governments worldwide to share how they are investing in gender-based violence prevention.

Like every year, we also reaffirm our commitment to promoting a world free from violence and ensuring the well-being of women and girls everywhere. A CALL FROM THE WONCA WP ON PLANETARY HEALTH We the family doctors, doctors and health professionals of the world call on world leaders to take urgent action to safeguard the health of global populations from the climate crisis. Climate change is already having widespread impacts on human health. As frontline health workers we are increasingly responding to health emergencies triggered by the climate crisis. Yet in the face of increasing harm and suffering, new fossil fuel resources continue to be developed and greenhouse gas emissions continue to rise. If we are to have any chance of limiting warming to 1.5°C and halting the escalation of the climate health emergency, we must end the proliferation of fossil fuels.

The World Health Organization has called for a Fossil Fuel Non Proliferation Treaty and an end to fossil fuel subsidies. We therefore join these calls, and urge all Governments to adopt the following as urgent health interventions: • End expansion of any new fossil fuel infrastructure and production. • Phase out existing production and use of fossil fuels. • Remove fossil fuel subsidies and invest in renewable energy. • Fast track a just transition that addresses the needs of individuals, communities and countries to move away from fossil fuel energy systems towards more diverse, resilient and inclusive economies powered by renewable energy.
