Historical Excerpts of the Fluoride Experiment By Rachel Sinacola, DDS, MS Grand Rapids WMDDS Editor Treasurer, Kent County Dental Society*

Local organized dentistry experienced challenges during World War II. Many member dentists were called to serve in the armed forces. West Michigan dentists helped their colleagues maintain their practices and care for their families while they were serving. On May 16, 1944, the members were asked to vote if they would be able to keep the organization intact throughout this challenging time. They overwhelmingly voted yes (Fig. 2). Figure 2

During the same summer, the National Institutes of Health (NIH) proposed that Grand Rapids host the first water fluoridation trial in the country. Grand Rapids was ideal: the state had legal control over the water supply, the city had a stable school population, and the neighboring city of Muskegon would be a comparable experimental control.1 To convince city officials to agree to the trials, they reached out to local dentists for support.

This includes the Kent County Dental Society (KCDS) secretary’s book, which contains minutes from thirty-five years of the organization’s meetings (Fig. 1). The minutes mention the name change from the E. S. Holmes Club to KCDS, the formation of what is now the West Michigan District Dental Society in 1937, and the first water fluoridation trials completed in Grand Rapids. As we approach 2020, the 75th anniversary of water fluoridation, these notes allow us to reflect on the experience of local dentists during this historic public health achievement. 4

A copy of a letter sent on August 1, 1944 from the KCDS to the City of Grand Rapids shows that the dentists agreed with water fluoridation as a promising solution for caries prevention and enthusiastically supported the trials (Fig. 3). It says, “Grand Rapids should be proud to be one of the first WEST MICHIGAN DISTRICT DENTAL SOCIETY | HOLIDAY ISSUE 2019

cities in the country to take this forward step.â€? The following day, the City Commission approved that Grand Rapids agreed to be the first city to add fluoride to the water supply.

U.S. Public Health Service and Michigan State Health Department researchers attended the October 26, 1944 meeting. They provided details on research timelines, fluoride concentration levels, and methods for examining the school children. One KCDS member, Dr. Nelson, thoughtfully proposed that information be sent to all local dentists, including nonmembers, so that everyone would be equipped to answer questions for local patients (Fig. 4). On December 14, 1945, members met the researchers who would later publish the experiment results. Dr. Philip Jay, who studied caries at the University of Michigan, informed the members about fluoride. Dr. Francis Arnold of the NIH offered to answer questions. He planned to work from the junior college building for the next ten to fifteen years. They would begin adding fluoride to the water supply with a concentration of one part per million (Fig. 5).

Occasionally, research updates were documented. For example, on October 3, 1951, members were informed of the optimistic preliminary results. One in three children in Grand Rapids were caries free (Fig. 6). WEST MICHIGAN DISTRICT DENTAL SOCIETY | HOLIDAY ISSUE 2019

The secretary began pasting sheets from a typewriter rather than writing the minutes by hand in the 1950s. Drs. Jay and Arnold presented more positive results at the December 3, 1952 meeting (Fig. 7). Children 5-7 years old showed a 69% reduction of caries! Cities across the country were now adding fluoride to their water supply. In fact, the city of Muskegon withdrew from their role as an experimental control so they could provide fluoride benefits to its citizens.

In 1955, the KCDS passed resolutions regarding the ten-year results of the experiments (Fig. 8). The decrease in caries rates was astonishing! As the first city to fluoridate their water supply, they proudly mailed the results and their resolutions out to local, state, and national organizations. But not everyone shared the enthusiasm of the local dentists and researchers. Figure 8

Arnold et al. published papers with the 10- and 15-year results. While there still remains a small level of criticism, fluoridation is generally regarded as a safe and effective way to reduce dental caries. The achievement, as well as the researchers who published the results, are widely celebrated today. In 1999, the Centers for Disease Control and Prevention named the fluoridation of drinking water one of the Top 10 Greatest Public Health Achievements of the 20th century.2 The secretaryâ€™s book documents the commitment that local dentists maintained to public health, to each other, and to their patients. This WMDDS Bulletin takes us into the year 2020, the 75th anniversary of water fluoridation, and contains features that highlight our pride in this achievement.

References A response letter from Dr. R. H. Friedrich of the American Dental Association alluded to some of the criticisms the experiment was receiving. Throughout the United States, some were fearful of adding fluoride to the water. Members agreed to ask the local medical society to show support of fluoride and deny that is has deleterious health effects (Fig. 9). 6

The snow is coming down this morning. Beautiful to look upon, but slippery as I drive to work, so I am going very slowly. Having to take it slow, though, has given me time to reflect on several things. Is anyone familiar with the song above? Or the artist? This song, and others of his, were a bit zany and brought smiles and laughter in our house. I am sure there are many songs you know that did the same for you. The holidays are upon us! Celebration and cheer, festivities and gatherings, family and friends. Looking forward, we have several local events to anticipate. An exciting CE presentation in January. Also, the opportunity to give honor to a great person and colleague at our West Michigan Silent Bell banquet in January. Then, of course, there is the MDA Annual Session in Grand Rapids. I hope to see many of you at these events. This coming year dentistry will be celebrating the 75th anniversary of community water fluoridation, one of the most significant public health achievements of the 20th century in the U.S., says the Centers for Disease Control and Prevention.1 Thanks to the efforts of those who have gone before most of us, a major disease has been subdued. Thanks, too, to the near-heroic efforts of our Steel Water Maintenance Committee, who have succeeded in getting the commemorative sculpture much-needed restorative work and shine-up. There are plans to celebrate this anniversary during the Annual Session throughout the coming year with local, state, national, and international recognition. It is my observation that West Michigan does a great job of helping one another. Mindful of the dichotomy this time of year brings – messages abound of joy and happiness, while life challenges remain ever-present in our lives, I hope you all get plenty of opportunity to enjoy your holidays and, perhaps, to give some joy to others. It was good for me to have to take it slow today.

Sharpening Your Practice Tools “ What makes a good design and how can I use it to improve my marketing and website?” Response: John Fetter, Wolfmark Adwerks There are many different elements that need to be considered when creating good design for marketing materials and websites that are both aesthetically pleasing and attract consumers to a business service or product. Here I’m going to touch upon three of them I consider important.

Use of color Different colors can conjure up different emotions, and a whole psychology is built around their use. For instance, red can denote energy and excitement, yellow can denote warmth and optimism, green alludes to growth and fertility, while purple can denote both mystery and sophistication. But beyond that, the combination of colors and the level of contrasts used also help denote certain feelings. Contrasting colors opposite each other on a color wheel, such as orange against blue, can denote a higher level of excitement, while colors close to each other on the color wheel, such as blue with green, can conjure up feelings of comfort and calmness.

Text copy In developing any marketing piece or page within a website, elements of images and copy areas should be considered as shapes that need to relate to each other and can be key to the readability of a page. If the “shape” of a copy area is too large it can distract from the overall marketing effort. Also, lengthy text blocks can be overwhelming, confusing, and can cause a reader to consider it an information overload

and will stop reading. Consider applying the “KISS principle” or “Keeping it simple stupid” when developing your blocks of copy. If you think a lengthy description of a product or service is essential to provide full information, think again. Editing down a text block can create a more readable and aesthetically pleasing page or marketing piece. Also, from my own experience, leaving some tidbit of information that you may think of as essential OUT of the description can be a benefit. It may seem counter intuitive, but leaving a couple questions unanswered can cause a reader or user to do just what you want them to do – call you.

Use of white space A third consideration essential to developing a good spatial relationship of design elements is the use of white space. White space (or negative space) can help frame design elements and also offers a breathing space for the eye. It can also help define a headline or a CTA, or “Call to Action.” Not using enough white space can cause a feeling of disorganization, which could be distracting for your site’s brand. On the flip side, too much white space can appear unprofessional and make it look as though you lack content. With this in mind, there’s a fine line between too little and too much white space. This is one important area that a professional graphic designer can assist with. Other elements of good marketing design are consistency of image and the use of professional images and photography.

Ben Frederick to introduce a bill that will recognize three new dental specialties in Michigan: dental public health, oral and maxillofacial radiology, and dental anesthesiology. n The MDA Teledentistry Workgroup is creating recommendations for the MDA Board to address many of the concerns that the MDA has heard regarding do-it-yourself dentistry and to ensure patient safety and the appropriate use of teledentistry.

This will provide members the opportunity to learn about the dentists representing them and for the delegation to share their stories and some business from the House.

Elizabeth Christopherson, DDS, MS, Grand Rapids Dr. Christopherson was elected to represent WMDDS for a two-year MDA Delegate term in 2019.

What is your favorite part of serving in the House of Delegates? I enjoy interacting with member dentists from around the state and learning how regions can have different concerns on certain topics. Gathering together to share experiences and opinions is how we can learn and grow from each other. Working together benefits not only ourselves, but also the patients and communities we serve. It is also a great way to stay current on issues that are relevant to our profession.

Where is your favorite place in West Michigan? We are water people and love to visit Lake Michigan. We keep our boat on Reeds Lake, so many summer evenings you can find us floating, swimming, and skiing there.

Do you have a favorite holiday tradition? I was raised in an interfaith home, so holidays for us were always a mixture of latkes, lighting the menorah, stockings, and Santa. Faith, family, and of course good food! We are carrying those traditions on in our young family. Whatever you choose to celebrate, I wish you and your family peace, health, and happiness in the coming year!

What motivated you to get involved in organized dentistry? Self-representation is important. We are our own best advocates. I wanted to become involved in a network of colleagues that could be my mentors and perhaps meet others that were looking for one. As a woman, a specialist, business owner, and mother, I feel it is important to be present at the discussions that are important for the future of all of these roles. When member dentists unite we can harness the power of advocacy.

Tell us about your hobbies. When I am not at work, I am often at our daughter’s school – it is so fun to be able see them in their element while helping out. Our family loves to travel, and recently got back from exploring Yellowstone and the Grand Tetons – so beautiful! For the last twelve years I have belonged to a book club and I always have two or three books I am carrying around. My latest challenges have been coaching a t-ball team of 8-year-olds and the Reebok Ragnar Relay, where I ran from Muskegon to Traverse City with a group of pretty awesome ladies. After graduation, she returned to her hometown of Grand Rapids where she is currently in private practice. She previously served as an alternate delegate during the 2008–2010 MDA Annual Session and represented the WMDDS at the 2008 New Dentist Conference. The Mecosta area study club members met this past October at the Blue Cow in Big Rapids. We will reconvene after the holiday season for our next meeting.

On Thursday, October 24, the Kent County and Michigan Oral Health Coalitions were on Capitol Hill meeting with members of Congress and advocating for oral health coverage under Medicare Part B, mandatory pregnancy-related benefits in Medicaid, and increased CDC funding for state oral health programs. Through funding by the DentaQuest Partnership for Oral Health Advancement, oral health advocates from across the nation were brought together by the Oral Health Progress & Equity Network (OPEN). OPEN takes a network-building approach to improving oral health equity. By engaging health equity advocates, community-based organizations, providers, policy makers, and community organizers, OPEN builds alignment and coordinates efforts toward the goal of healthier communities so all can thrive. See the “Dentists Doing Good” section (Page 29) for information on how to get involved!

The West Michigan Dental Foundation is planning ahead for 2020. With the generous contributions from 2019, we gave over $22,850 to local organizations who applied for community grants related to oral health and dental needs, and we were able to grant funds to help 16 dental, dental hygiene, and dental assisting students from the West Michigan area. Our annual appeal letter will be sent out soon. We look forward to the continued contributions for these efforts in 2020. With these donations, we are able to continue to support the WMDF’s mission and efforts in supporting our local dental community. We are also looking ahead to our next annual golf outing, which is our largest fundraiser. The event will be held May 15, 2020, at the Meadows. Mark your calendars now, we look forward to seeing you there! Information will be sent out regarding sponsorships and sign-up in the near future. October 24th marked the last of the 2019 New Dentist Forum events. The continuing education course was presented by Brian Lafrenier, a CPA from Beane Garter, and Greg McGlaun from Proveer Practice Management. The topic was on practice transitions and optimization. The information presented sparked some good conversations, one of those including Delta Dental’s stronghold on the industry. One Bourbon was the venue and this whole event was made possible by Jeremy Prins from Consumer’s Credit Union. The next event will be Thursday, February 6th at Wheelhouse. Dr. Anthony Guinn will be speaking on endodontics for the general practitioner. Have a fantastic holiday season and see you again in February. 75 Years of Water Fluoridation

A look at the history of how fluoride was found to be an effective addition to our water supply in the fight against dental decay and the role that Grand Rapids played in a 15-year long NIH study.

As 2020 approaches, this marks 75 years since the inception of water fluoridation in Grand Rapids. In 1945, a trial began to assess the benefit of adding fluoride to the city’s water supply in an attempt to reduce the incidence of dental decay. While we know what a success this small addition has made to the oral health of millions, getting to this point required many years of research. The story began more than 110 years ago, with Dr. Lawrence McKay and renowned caries classifier, Dr. G. V. Black. Having recently graduated dental school in 1901, Dr. Lawrence McKay accepted an associate position in Colorado Springs. While working in Colorado Springs he noted many patients had brown staining accompanied by enamel pitting. In 1908, McKay, along with a few colleagues, formed a committee to study the “Colorado Brown Stain.” Approximately 3,000 school children were studied and about 87% were found to have some degree of brown stain or pitted enamel.1 Additionally, it was noted that those affected by the brown stain exhibited a lower incidence of dental decay. Dr. McKay recruited dental researcher G. V. Black to come to Colorado to further investigate the causation. In 1916, a paper was published in the Dental Cosmos, coauthored by McKay and Black.2 By this time, it was known that a component within the water was the cause of the “Brown Stain.” The most common methods of testing water could not identify any anomalies until Dr. H. V. Churchill got involved. By this time McKay had traveled to many cities throughout the US with similar reports of Brown Staining – one of the cities on the list was Bauxite, Arkansas. Dr. Churchill, a chemist in Bauxite, conducted a photospectrographic analysis of the water in 1931. He discovered high levels of fluorides and sent his findings to McKay.3 Churchill advised McKay to collect 22

Throughout the 1930s, Dr. H. T. Dean, the head of dental hygiene at the NIH, was involved and enlisted chemists to find a way to accurately measure the levels of fluoride in water. By the late 1930s, fluoride could be measured down to 0.1 parts per million and it was known that concentrations less than 1.0ppm would not cause fluorosis.3 In 1944, the Michigan Department of Public Health, along with the City Commission of Grand Rapids, agreed to add fluoride to the public water supply to assess if low levels of fluoride would help to reduce tooth decay without comprising esthetics or, more importantly, the health of the population.

Beginning in 1945, a 15-year project began to monitor the rate of dental decay in the schoolchildren of Grand Rapids. The study design was as follows: in 1944, all the public school children were examined. In each of the following years, the number of children in each age category to be examined was increased. The DMF index was used to assess the changes before and after fluoridation. Dr. David Scott oversaw the study locally and was the only scientist involved in all 15 years of testing. In 1955, eleven years after the addition of fluoride to the water, a study was published in the American Journal of Public Health documenting the findings. As witnessed by these associated figures, recreated from this article, the overall reduction in DMF teeth ranged from 34-58%.4 The earlier the exposure to the fluoridated water, the greater the impacts on the DMF index. For children born after the addition of fluoride to their water, the incidence of caries was reduced by 60-65%.4

Since the 1950s, a majority of community water supplies in the US have added fluoride to the water. According to the CDC, as of 2014, 66% of the U.S. population receives fluoridated water.5 That is over 211 million people in the U.S. that stand to benefit from the work of Dr. McKay. The World Health Organization recommends a fluoride level of 0.5 to 1.5mg/L. This is seen to be a relatively low-cost way of preventing disease. In the United States, this costs just over $1 per person per year. Steel Water is a 33-ft. sculpture by Cyril Lixenberg positioned along the Grand River. It stands to commemorate the addition of fluoride to the cityâ€™s water supply. It is without doubt that many doctors and researchers spent many decades investigating the use of fluoride. As we approach 75 years since its inception into our community, it seemed very fitting to celebrate their work and the benefit it has served our families and our patients.

Adult Dental Services Program (ADSP) The WMDDS and WMDF created the Adult Dental Services Program to provide access to volunteer dentists for those with limited income who are just above the eligibility thresholds for government assistance. Eligibility requirements have also been broadened from historical norms, including extension to the five counties within the West Michigan District Dental Society. Volunteer dentists provide personalized treatment planning and deliver dental care in the efficient atmosphere of their practice location. Eligible patients who are accepted into the program agree to attendance and co-pay criteria. Applicants are directed to Cherry Health Dental Clinic, who has agreed to employ the ADSP Dental Coordinator and staff dentists, for initial intake processing and diagnostic collection. Private grants managed by the WMDF are used to run the initiative, including lab fees for indirect procedures. This collaborative effort provides approximately $13 of philanthropic dental care for every grant dollar. KCOHC The Kent County Oral Health Coalition works to improve oral health and access to care for Kent County community members. The Coalition is housed at Health Net of West Michigan, an organization that unites health systems, providers, and individuals to improve the health outcomes of Kent Countyâ€™s most vulnerable citizens. The coalition would love more members of the local dental community to get involved in the organization. OUR COMMUNITY / DENTISTS BEYOND THE CHAIR In this special edition of Beyond the Chair, Dr. Lauryne Vanderhoof interviews Dr. James Wieland about his role in honoring Grand Rapids, the first city to fluoridate their water supply.

Chairman, Steel Water Maintenance Committee, WMDF When did you first get involved in commemorating water fluoridation in Grand Rapids? In January 1985, a ceremony was held in Grand Rapids to honor the scientists who performed the original Grand Rapids study from 19451953, the city of Grand Rapids, and the members of our community who assisted with the study. This invigorated the local community to plan a special celebration for the upcoming 50th anniversary of water fluoridation. Mayor Gerald Helmholdt appointed a blue ribbon committee to plan the celebration, including Dr. Kim Erickson and myself as co-chairs, Dr. Gerald VanderWall, and Dr. Charles Caldwell.

What did the original sculpture look like? Dr. James L. Wieland is a native of Grand Rapids who practiced for over 40 years in our community. He served two 4-year terms on the State of Michigan Board of Dentistry. He is the chairman of the Steel Water Maintenance Task Force. Steel Water commemorates the City of Grand Rapidsâ€™ historic role as the first city in the world to fluoridate its water to help prevent dental caries. Water fluoridation is considered one of the top public health measures of the 20th century and has improved the oral health of millions of people around the world. Steel Water is located on a plaza on the east side of the Grand River just south of the Grand Rapids JW Marriott Hotel.

The original sculpture was installed in 1995 in the location where Steel Water now stands. Steve Pierpont, a member of the Grand Rapids City Planning Department, designed a narrative sculpture to tell the story of water fluoridation from its start in Grand Rapids to its extension around the world. It comprised of six white marble pillars in an open octagonal shape with a granite base inset into the ground. The ascending pillars each represented a decade and the increasing number of people benefiting from water fluoridation. The last pillar had a built-in drinking fountain so that anyone could sample the fluoridated water. Quotes from surgeon generals about the benefits of water fluoridation were highlighted on the outside of the sculpture.

What happened to the original fluoride sculpture? The JW Marriott Hotel was slated to be built adjacent to the site of the original sculpture. The project manager for the hotel construction did not feel that the original sculpture, often a target of graffiti and vandalism, complimented the new hotelâ€™s design. The city proposed moving it to the Sixth Street Park, but this did not appeal to the local dental community. Eventually, all parties agreed to commission an artist to create a fine art replacement for the original marble sculpture. The replacement would have to be created and placed before the hotel plaza could be constructed; it was a race against the clock. The marble sculpture was moved to a city garage until it was acquired by Delta Dental, who provided the lead gift for the original project. According to a representative from Delta Dental Plan of Michigan, it is stored at their Data Center with no immediate plans for it to be restored or displayed.

Who designed the new sculpture, Steel Water? A committee was formed under the auspices of the WMDDS to raise money for a new sculpture. Under the guidance of Henry Matthews, former Director of Galleries and Collections at Grand Valley State University, the committee studied portfolios from 15-20 international artists. Three finalists were chosen and given a stipend to present a site-specific sculpture. The winner was artist Cyril Lixenberg of Amsterdam, the Netherlands, with his design, Steel Water. Mr. Lixenberg, a charming and humorous man, lived in Grand Rapids while Steel Water was fabricated by Grand Rapids businesses. It was placed in June, 2007 and dedicated on September 13, 2007 with Mr. Lixenberg and other dignitaries in attendance.

Was it challenging to install the sculpture? Being 33 feet tall and containing five tons of steel, Steel Water needed two cranes and a cherry picker to be placed. A local painter was hired to paint the sculpture. The final coat of paint needed to be applied on site. He had to manually apply the final coat rather than use WEST MICHIGAN DISTRICT DENTAL SOCIETY | HOLIDAY ISSUE 2019

spray equipment because the location is a bit of a wind tunnel. This unfortunately gave the sculpture an orange peel-like texture rather than the intended satin finish. The recent restoration has given the sculpture the satin finish that was originally intended.

Tell us about the current maintenance project that was just completed. Steel Water weathered over the years and the paint began to oxidize, bubble, and rust. A professional art conservator, Doyle Horning, was hired to help restore the sculpture and establish procedures to preserve Steel Water for the future. The restoration was completed in October, 2019, just in time for the 75th anniversary of water fluoridation on January 25, 2020 and the 2020 MDA Annual Meeting.

How will Steel Water be maintained in the future? Steel Water will be examined three to four times each year so that issues that need attention are addressed early. We anticipate this preventative approach will maintain the current restoration for twenty to thirty years. Mr. Horning will help us establish guidelines to streamline future restorations, which will be approved by the City Commission of Grand Rapids, the Grand Rapids Parks and Recreation Department, WMDDS, and WMDF. Funding for future restorations will require approval from both the WMDDS and WMDF as well as supervision by the Grand Rapids Parks and Recreation Department. 33

Guidelines for Prescribing Antibiotics for Dental Infections The ADA published guidelines for prescribing antibiotics for dental infections in a November 2019 article in JADA by Lockhart et al. Key Points: n The guideline recommends against using antibiotics for most pulpal and periapical conditions and instead recommends only the use of dental treatment and, if needed, over-the-counter pain relievers such as acetaminophen and ibuprofen. n Instead of prescribing antibiotics, dentists should prioritize dental treatments such as pulpotomy, pulpectomy, nonsurgical root canal treatment, or incision and drainage for symptomatic irreversible pulpitis, symptomatic apical periodontitis, and localized acute apical abscess in adult patients who are not severely immunocompromised. n If a patient’s condition progresses to systemic involvement, showing signs of fever or malaise, then dentists should prescribe antibiotics.
