Who We Are Welcome to the Windsor Essex Community Health Centre.

As a community health centre, WECHC continuously aims to support the vulnerable populations in Windsor and Essex County by offering equitable, safe, quality care that is client centered and close to home.

We operate 6 physical locations throughout Windsor and Essex County. They include: Teen Health Site (Ouellette Ave.), Street Health Site (Pelissier), Diabetes Wellness Site (Lauzon Pkwy), Leamington Site (Princess Street), Sandwich Site (College Ave.), and Pickwick Site (Tecumseh Rd. E.). In addition to these sites we offer numerous services at various locations throughout Windsor and Essex County.

The Erie St. Clair LHIN largely funds our programs and services allowing us to offer them to our clients at no charge.

Community Health Centres (CHCs) are community governed centres providing high quality primary care services with health promotion programs, illness prevention programs and community development initiatives.

As part of this integrated, comprehensive primary health care approach, CHCs support communities and residents to achieve health by addressing “social determinants of health� which includes factors such as income levels, access to shelter/housing, education, language/geographical barriers and other factors that are known to have a direct impact on health outcomes for individuals, families and communities.

CHCs partner with other local agencies and community partners within the health and other community sectors to address the bigger picture of health.

Diabetes Wellness Site Windsor Essex Community Health Centre - Diabetes Wellness Site is a community based diabetes education program providing no charge services at various locations in the city and the county. The Diabetes Wellness team is made up of registered nurses and registered dietitians, most of whom are certified diabetes educators.

Our Clients: • are adults diagnosed with Type 1 or Type 2 Diabetes • are adults at risk of developing Diabetes • are the support person for someone living with Diabetes We offer: Individual Appointments offer you the individualized care you need at a date/time that is convenient for you.

Group classes allow you to learn more about how to manage diabetes. Classes are offered mornings, afternoons and evenings throughout Windsor and Essex County.

Diabetes Support Group meetings are offered in Windsor and Leamington.

ADP Insulin Pump Program supports clients with a new or existing insulin pumps.

Anyone can access service from a Diabetes Wellness team member – no referral is required.

Chronic Disease Self Management Master Your Health and Choices and Changes Windsor Essex Community Health Centre is the Erie St. Clair lead for Chronic Disease Self Management.

Self-management is a key component of improving the management of chronic disease(s), such as diabetes. The ability of individuals to self-manage their condition 365 days per year has a powerful impact on their health and well-being.

Master Your Health is designed for Chronic Conditions, Chronic Pain and Diabetes.

Master Your Health is a six week course that helps to empower individuals with or at risk of developing diseases to assume greater control and responsibility for daily health care decisions.

Choices and Changes is a workshop designed for Health Care Providers.

Choices and Changes is an evidence-based program which is approved and accredited by the College of Family Physicians of Canada. This four hour FREE workshop specifically for healthcare professionals is led by two faculty trained members from the Institute of Healthcare Communication.

PEP Talk Diabetes, Healthy Feet and You - Educational Workshop - A peer-led workshop that reviews how to enjoy an active life, keep your feet healthy and avoid foot complications through good diabetic foot-care practice.

Chronic Disease Management Respiratory Wellness Windsor Essex Community Health Centre -Chronic Disease Management offers specialized programming for individuals living with Chronic Obstructive Pulmonary Disease (COPD). Programs have been designed to combine education and exercise to support clients in managing their COPD. Clients are supported by a multidisciplinary team. The intake assessment will ensure we can meet your needs.

COPD education provides education and support to clients who have been diagnosed with COPD. Caregivers are encouraged to attend. The program is facilitated by a multidisciplinary team.

Active Aging Windsor Essex Community Health Centre, Active Aging Program provides specialized exercise programs and education to people living with chronic disease in Windsor and Essex County.

Some components of the program include: • Circuit exercise programs based on function ability in Windsor and Essex County • Group exercise for clients with chronic diseases in Windsor and Essex County • Therapeutic pool program in Windsor and Essex County • Smoking cessation counselling • Social work support by referral • Registered Dietitian by referral • Speech language pathology • Stretch Out Stress (SOS) • Upper Extremity Program to improve strength and coordination • Minds in Motion (in partnership with the Alzheimer’s Association of Windsor) • Mall walking at Devonshire Mall • Outdoor walking and Nordic walking during the summer months Individuals interested in taking part in the exercise program or in Respiratory Wellness can register themselves or be referred by their health care provider.

Falls Prevention Falls Prevention Program Falls are a major cause of injury and death among seniors. Most falls can be prevented. The Falls Prevention Program works with clients to assess the risk factors related to falls and provides individualized education, recommendations and referrals to community resources. Monthly Clinics are held across Windsor and Essex County. The goal is to improve the safety and independence of seniors by reducing the number of falls and fall-related injuries.

For more information call 519-997-2823 ext. 417 OT Home Safety & Grab Bar Program An Occupational Therapist meets with clients for an in-home safety assessment to identify falls risks, home hazards, and/or equipment needs. If grab bars are recommended, the Occupational Therapist will help to have them properly placed and professionally installed at no cost to individuals who qualify. The Occupational Therapist also provides education, other recommendations and referrals to community resources/supports. The goal of this program is to assist seniors to stay safe and independent in their own homes.

Street Health Site Windsor Essex Community Health Centre - Street Health serves the homeless and those at risk of homelessness within Windsor and Essex County. Street Health fosters an environment that assists clients with achieving their expected goals by responding to their needs, preferences and values. Street Health strives to provide access to health care related services, supportive services, education and advocacy for our clients. We adhere to a harm reduction model in all our programs and work to address the social determinants of health.

The individuals and families that we work with have lives characterized by extreme poverty, chronic unemployment, unsecure housing, poor nutrition, high stress and loneliness; they also have more frequent and serious illnesses.

Hepatitis C Treatment & Support Windsor Essex Community Health Centre Hepatitis C Treatment & Support team is a specialized inter-professional team that works collaboratively with healthcare, and social service providers. The goal of the team is to establish support and treatment and to ensure that clients affected with or at risk of acquiring Hepatitis C have access to quality specialized care. Our team helps prevent the spread of the Hepatitis C virus and promotes overall health by providing timely education, testing, support and treatment.

Sandwich Site Primary Care Team Services Windsor Essex Community Health Centre - Sandwich Site has a primary care team that consists of Physicians and Nurse Practitioners and is supported by Registered Nurses, Registered Practical Nurses, Respiratory Therapist, Chiropodist, Addiction Support Workers, a Registered Dietitian and Medical Receptionists.

The health care teams work in an inter-professional collaborative environment that includes other health care professionals.

Our Sandwich Site provides laboratory diagnostic services with daily lab courier for registered clients, ECG, spirometry and home visits for clients unable to leave their home. We offer consultants such as a chiropodist, respiratory therapist, diabetes educators, addiction support workers and social work through self referral.

We welcome clients who have difficulty seeking help or are without a family doctor. There are no geographical boundaries or catchment area.

Our Sandwich Site also provides support and transitional care for people with development and physical disabilities in partnership with the John McGivney Children’s Centre.

Counselling The Sandwich Site offers free, confidential general counselling to individuals aged five and up. Counselling is limited to 12 sessions and is a collaborated process that is focused on increasing the wellbeing of our clients. Couples counselling is also available at our site.

Registered Dietitian Services Nutrition assessment and counselling services are provided, free of charge by a Registered Dietitian for vulnerable individuals in the community. Individual counselling is provided for chronic disease management, adapting to the Canadian Food supply, gastro-intestinal disorders and other nutrition concerns. Counselling is available for all ages.

To find out if the dietitian can see you, or for additional information, please leave a message at 519-258-6002 ext. 733. Primary care provider referrals are also accepted by fax at 519-258-7896. Individuals will be contacted to complete a brief intake by phone.

Health Promotion The Windsor Essex Community Health Centre Health Promotors and Community Health Workers assist the vulnerable populations of Windsor and Essex County.

Together the inter-professional team members and health promotion team follow a wholistic approach towards client care with the social determinants of health in mind.

Focusing on Education, Health Promotion and Disease Prevention our team travels throughout Windsor and Essex County engaging children, youth, adults and seniors on how to live a healthier lifestyle.

Overall, Health Promotion involves a number of different approaches.

It can be accomplished through the five action areas of Health Promotion: 1. Building health public policy 2. Creating supportive environments 3. Strengthening community action 4. Developing personal skills 5. FEED - Food Education Exercise and Diversity Fit Kids Summer Camp – A program for children and youth ages 4-14 who attend various summer camps throughout Windsor and Essex County. The program teaches kids the importance of healthy eating, portion sizes, and physical activity, through fun interactive activities.

Let’s Get Cooking With Food – A program designed for all ages! This program can be offered as a series or as a onetime event and teaches participants how to make a healthy meal/meals or snacks through interactive food demonstrations. All participants leave with a sample of the finished product and the recipe.

Out Reach – We respond to community requests for presentations around nutrition related topics. Please visit our website (www.wechc.org) and submit your request through the Speaker’s Bureau.

Food Activity Behavior (FAB) This 8 week program is designed to assist with reaching or maintaining a healthy body weight by providing a basic foundation of healthy eating and exercise. This program includes interactive physical activity and meal planning components in addition to providing tools to manage emotional eating and stress.

weMOVE This adult exercise classes designed to promote and improve endurance, strength, balance and flexibility. Classes include warm up, cardio, strength and endurance training and cool down. Classes are offered twice a week. Call for times and locations.

Morning Glory Moments Program • Free Post Natal Program offered to young parents with children ages 0-6 • Parents take part in a parent information/ education session with a Registered Nurse • Each week a different topic is discussed

o For example: nutrition, breastfeeding, sibling rivalry, and behavior • Offers free play time and child-centred crafts for children with a Community Health Worker • Promotes parent-child activities: preparing a healthy snack, story time, singing, games and outside activities.

Leamington Site Windsor Essex Community Health Centre - Leamington Site has a primary health care team that consists of Nurse Practitioners and is supported by Registered Nurses, Registered Practical Nurses, a Registered Dietitian and Medical Receptionists. Pickwick Site Windsor Essex Community Health Centre – Pickwick site integrates a variety of services and programs provided by WECHC in the east end of Windsor.

WECHC has a physician and Nurse Practitioner on site and work in an interprofessional collaborative environment that includes other health care professionals.

Teen Health Site Windsor Essex Community Health Centre - Teen Health provides primary care and counselling services to youth ages 12 – 24 years.

Medical Services The physicians, nurse practitioners and nurses specialize in the health of young people ages 12-24 years.

Our staff encourages clients to ask questions while providing the latest information about medical and emotional concerns.

We offer services in the following areas: • Annual Health Exams • General health concerns such as sore throat, flu, etc.

• Information and prescriptions for birth control • Diagnosis and treatment of sexually transmitted infections (STIs) • Pelvic examinations • Pregnancy tests, counselling regarding pregnancy and prenatal care • Depression and anxiety • Eating Disorders • Smoking Cessation • Acne and skin care • Sports injuries and nutrition • Sexuality and puberty information • Individual Nutrition Counselling Clinical nutrition services are provided, free of charge by a registered dietitian for youth ages 12 - 24 years.

Prenatal Support Program The “Special Additions” program is a support group held in a warm and friendly atmosphere and designed especially for adolescents and young adults. Our prenatal instructor gives participants the information and help they need to have a healthy and happy pregnancy, labour and delivery.

Tuesdays from 6:15pm – 8:00pm Duration: 8 weeks Frequency: Groups are run three times/year

Counselling Teen Health offers free, confidential individual counselling for youth ages 12-24 in the following areas: • Family Conflict • Grief • Anxiety • Eating Disorders • Unplanned Pregnancy • Other general teen concerns

• Relationship problems with friends • Depression • Substance Abuse • Self-Harm • Suicide Thoughts

Youth must consent to receive counselling services.

Mosaics (Mood) Group A therapeutic treatment group for youth ages 17-24 years who are experiencing depression and/or anxiety with negative coping behaviours.

Junior Wellness Group This drop in group is for youth ages 12-16 years. Sessions are offered weekly, where participants learn strategies to manage their stress.

Community Building Program (Substance Abuse) This program provides individual psychotherapy and service coordination for adolescents and their families who are experiencing substance abuse issues and are supported by the Windsor-Essex Children’s Aid Society (WECAS).

Out Patient WECHC Addictions Programs The Substance Abuse Treatment Program offers psychotherapy to young people who are concerned about their alcohol or drug use. The program also serves the needs of those clients seeking therapeutic support in caring for those in their lives who have addiction issues. The program offers assessment, individual psychotherapy and a referral to community and/or provincial resources as needed.

Youth Outpatient Addictions Program Our Youth Outpatient Addictions Program is for those individuals who are seeking support to assist with the challenges of change and recovering in choosing to reduce or discontinue substance abuse.

Individual treatment plans aim to focus on the needs of the individual as a whole.

Our approach is to address the underlying issues contributing to the addiction by way of a best practice approach. This may include the following: standardized addiction assessment, internal collaboration targeted psychotherapy and community referrals. A self-referral is required.

Youth Support Program Our Support Program is for those individuals seeking support to assist them with the negative effects of substance abuse stemming from a current or past relationship (i.e. parent, family member, partner, spouse, friend, etc.). A self-referral is required.

Parent 1-1 Our Parent 1-1 is a one time, one hour confidential support consultation session with a therapist specializing in addictions treatment. This program is aimed at addressing issues of concern and encouraging the next steps for the family towards healing and recovery. A self-referral is required.

Being part of our team is a challenging and rewarding opportunity where we can make a difference within our community. It is for this reason we strive to attract and retain the best qualified individuals who are committed to providing the best care and service.

Volunteering WECHC volunteers donated over 3,300 hours this past year. Our volunteers assist us in various ways. They: • Work with children – post natal program. (i.e. reading books) • Work with seniors and people with disabilities in the Active Aging program – assisting clients while performing exercises, providing support to clients in the pool program, provide one on one support to clients who participate in balance exercises • Perform administrative duties The WECHC Board of Directors is a volunteer governance board consisting of members who encompass a breadth of knowledge and experience.

Student Placements WECHC continually provides learning opportunities for students from various college and university programs. We provide placements at all of our sites from various programs and fields of study.

Student placements are arranged by the school or educational institution.
