About VON VON is the longest serving healthcare organization in Canada. We offer more than 75 different home care, personal support, and community services to enhance each client’s quality of life. We provide seniors with the supports needed to remain independent and in their own homes for as long as possible. In the Durham Region we’ve been providing these services for over 100 years.

Our Service Stands Out • VON Durham Region delivers care, 24 hours a day, 7 days a week, all year long.

• VON delivers a variety of nursing care, specialized services, home support services, Community Support and Hospice Palliative Care Services.

• VON offers personally designed care for the needs of our clients, caregivers and families.

Local VON Home Health Care Services VON, through the Durham Region Site provides Registered Nurses and Registered Practical Nurses who provide holistic client-centred care.

Our health care professionals provide:

SELECT Program (Special Extended Living Environment for Challenged Tenants) This program provides professional specialized community based supports and services to adults with developmental disabilities. The goal of this program is to support the development of a personal individual Support Plan based on the principles of person centred planning, self determination and choice to provide services that are part of this plan.

WIN Program (Wellness in Independence) This program provides expert mental health case management and is designed to support clients with chronic mental health issues in our communities. Clients receive individualized ongoing assessments, interventions, treatments, counselling and referrals from mental health nurses.

Local VON Health Promotion VON is committed to taking a leadership role in health and wellness promotion, through the delivery of programs to support good health.

Volunteer Blood Pressure Check Program This service is currently offered during four sessions each month in the Courtice, Oshawa and Whitby areas. Under the guidance and supervision of the program coordinator, adult volunteers are trained to conduct blood pressure checks and support users of this service on a monthly basis in a confidential and professional manner.

The goal of the Volunteer Blood Pressure Check Program is to improve community awareness of the importance of cardiovascular health and assist older adults to monitor their blood pressure on a regular basis.

Participants are provided with health promotion materials as well as links to other health services available in the community.

Local VON Community Support Services Community Support Services are offered through a variety of professional and volunteer programs.

VON, through the Durham Region Site offers:

Seniors Maintaining Active Roles TogetherŽ SMART Exercise Program The VON SMART Group Exercise Program is an exercise program designed for adults 55+ who may require a little extra support due to health issues, mobility restrictions, confidence, chronic disease or other reasons. There is no floor or mat work involved and all exercises are tailored to the participant’s ability level. Developed and implemented by VON Canada, the SMART Program provides exercise opportunities for older adults keeping in mind their personal fitness level and ability.

VON SMART Exercise Programs are led by adult volunteers who receive specialized training, including CPR and First Aid to deliver safe, appropriate, physical exercise designed for successful aging. These exercise groups are offered weekly at 16 locations throughout the Durham Region, seven of which are specialty classes for participants living with Diabettes, Multiple Sclerosis, Mental Health concerns or Stroke recovery.

Weekly classes are delivered in 15 week sessions taking breaks between sessions.

The VON SMART In-Home Exercise Program is offered to those having barriers getting outside of their home due to health or transportation issues. This program offers clients who are isolated and unable to attend a traditional exercise class, the opportunity to receive one on one coaching and motivation during weekly one hour sessions in the comfort of their own home over the course of 15 weeks. The exercises are easy to follow with noted success when done independently or with the assistance of family or friend between weekly sessions with the VON volunteer.

Benefits of VON SMART Programs include…..

• Improve physical health, mobility, independence and energy.

• Increase and maintain strength, coordination, balance and flexibility – which in turn help to prevent falls.

• Manage weight and improve lung and heart function.

• Help to manage chronic conditions such as high blood pressure, cholesterol, stress, arthritis and osteoporosis.

• Provide opportunities to meet new people and socialize.

Along with a $2.00 fee per class, funding for these 15 week programs is supported in part by the Central East Local Health Integration Network combined with local fundraising and charitable donations to VON Durham.

Assisted Living Services for Seniors The VON Assisted Living Services for Seniors provides support services to eligible seniors living within designated geographic areas across the Central East LHIN. Within Durham Region, the Central East LHIN funds VON to provide Assisted Living Services in a geographic hub that includes the communities of Beaverton, Cannington and Sunderland.

Services include personal support assistance such as dressing, personal hygiene, assisting and monitoring medication use and other routine activities of living; homemaking services including shopping, housecleaning and meal preparation; security checks or reassurance services including visits to ensure client health or safety; and care coordination.

Services are flexible to the needs of the client and available 24 hours a day, 7 days a week on both a scheduled and unscheduled basis.

Eligible seniors include those at high risk of requiring institutional care without this flexible and client-centred style of enhanced support. The VON Assisted Living Services allow these individuals to remain in their own homes with the services and support they need to maintain their health and safety.

Local VON Hospice Services Bereavement Services

Support for Adults This program is based on a peer support model, which means that volunteers who are bereaved and further along in their grief journey, provide support to clients who are also bereaved. Volunteers provide one-on-one support and facilitate bereavement groups such as: • Spousal Loss • Parent Loss • Mixed Loss (Drop-In)

Support for Children & Teens VON Durham Hospice Services offers a Children/Teen’s Bereavement Program facilitated by volunteers who support participants by creating a safe space to share their stories, explore questions and feelings, and learn healthy coping strategies. A Caregiver Support Group is available concurrently with the Children/Teen’s Bereavement Program, providing caregivers with information about how to best support their children.

Palliative Care Services Our Palliative Care Services provide emotional, social and spiritual support to individuals and families facing a life limiting diagnosis and advanced illness. We offer one-on-one visits, support for clients and/or caregivers, a Caregivers Drop-In Group, as well as a Day Hospice Program. Our staff and trained volunteers work in partnership with community health care professionals and other service providers in an effort to enhance the quality of client care.

VON Palliative Care Coordinators assess clients using a holistic model based on physical, spiritual, emotional and social needs. Clients are then matched with a trained volunteer who provides ongoing support.

Our VON Palliative Care volunteers are rigorously screened and complete the “Fundamentals of Hospice Palliative Care” training program. They consist of men and women of all ages and come from all walks of life.

Palliative Care Community Team The Palliative Care Community Team (PCCT) is a centralized service that links patients and families to palliative care services across Durham Region. The PCCT includes a Patient Navigator and a Supportive Care Counsellor. Working in partnership with community partners, our Palliative Care Community Team provides clients and their families with holistic health care including emotional, social and spiritual support. The PCCT is available for anyone diagnosed with a life-threatening illness at any time between an initial diagnosis through bereavement. The PCCT is currently available in the Municipality of Clarington and the Townships of Brock, Scugog and Uxbridge and is looking to expand throughout Durham Region in the coming years.

Day Hospice This program offers social interaction, peer support and companionship in a group setting for clients who can manage their own care and are cognitively alert. Day Hospice provides caregivers with some respite time as well as an opportunity for them to gather and have a space where they can share and receive support.

Information and Education VON Hospice Services provide education and consultation regarding death, dying, grief and bereavement to both individuals and groups in the community.

Lending Library Our VON Durham library has an extensive collection of books, videos and reference materials on a wide range of subjects including: hospice care, caregiver support, pain and symptom management, complementary therapies, grief and bereavement as well as resources targeted at specific groups such as children/youth, and LGBTQ. Interdisciplinary Palliative Care Education These courses are designed to increase hospice palliative care expertise at all levels of care and scope of practice. They encompass all facets of palliative care to varying degrees including: disease management, physical, psychological and spiritual issues, end of life care, grief and loss and selfcare. Certificates are issued upon successful completion of courses.

• Fundamentals of Hospice Palliative Care This 8-week long course emphasizes the importance of demonstrating sensitivity, understanding and respect for all in order to improve the experience of those who are at end of life. This training is appropriate for health care professionals, volunteers or community members wishing to increase their awareness of hospice palliative care. • Enhanced Fundamentals This curriculum focuses on enhancing knowledge and skills in hospice palliative care and is available for Registered Nurses (RNs), Nurse Practitioners (NPs) and Registered Practical Nurses (RPNs) who have completed the Fundamentals training. • Comprehensive Advanced Hospice Palliative Care Education Program for Nurses (CAPCE) RNs and RPNs with passion and experience in palliative care are encouraged to continue their education with this course designed to develop nurses as leaders and experts in hospice palliative care. Classes are held over several months and include homework/applications and individual mentoring. Prerequisites include Fundamentals of Hospice Palliative Care Certification, Enhanced Fundamentals, internet access and written employer support.

Palliative Pain and Symptom Management Consultation VON Durham employs a Palliative Pain and Symptom Management Consultant to provide consultation, education, mentorship and linkages to palliative care resources for healthcare providers across the continuum of care throughout Durham Region.

Volunteer With VON A Community Team Volunteers are central to VON’s Mission, working in partnership with VON staff to develop, implement and deliver services that meet community needs. We have a role for You!

Discover the Joy of being a VON Volunteer Here is a sample of the exciting volunteer opportunities that may be available to you.

SMART Group Program SMART Exercise Program volunteers receive extensive training through the VON SMART Program Coordinator in order to be qualified Seniors’ Fitness Instructors. The Program Coordinator at VON mentors volunteers during this process until they are comfortable in leading the weekly exercise groups on their own. VON provides all the training at no charge to the volunteer. Other volunteers play a supporting role in the SMART Program by welcoming participants and helping to keep groups running smoothly.

SMART In-Home Program SMART In-Home Volunteers receive a one-day training session in carefully designed exercises to assist older housebound adults to maintain or increase their level of fitness. Volunteers then visit a participant’s home on a weekly basis to lead them through this series of exercises and encourage them toward their goal of improved mobility. The SMART Program Coordinator mentors the volunteers and VON provides all the training at no cost to the volunteer.

Palliative and Bereavement Client Support Hospice volunteers must complete 30 hours of Fundamentals of Hospice Palliative Care and/ or Bereavement training prior to being matched with a client. Through one-to-one matches and facilitated group discussions, volunteers provide support to clients living with a life-threatening illness, caregivers and individuals who are bereaved. One-to-one matches are based on common personality traits, life experiences and availability. Bereavement clients and volunteers are matched based on similar loss. If you are interested in becoming a bereavement volunteer then you must have experienced your own significant loss as our bereavement support is based on a peer-support model.

Volunteers within VON hospice services provide peer-based support both in our offices and in the community.

Volunteer Blood Pressure Checks Volunteers receive specialized training from VON to conduct blood pressure checks in convenient locations such as community centres, local pharmacies or senior’s housing. Individuals with an interest in helping to improve community awareness of the importance of cardiovascular health and to assist older adults to monitor their blood pressure on a regular basis would be well-suited to this role.

Community Corporation Provide leadership at the local site level of VON. People interested in this level of volunteering choose to share their expertise and knowledge to enhance organizational objectives and support the goals of advocacy, strategic development and fundraising to enable VON charitable programs to be delivered locally. The Board and Committee members generally meet in the evenings on a monthly basis.

Administrative Support Administrative volunteers assist during regular business hours at the VON office or other off site locations. Volunteers assist with filing, photocopying, faxing, bookkeeping, data entry etc.

Community Awareness Volunteers help to promote VON Durham programs and services at local health and information fairs and public events. Volunteers represent VON Durham in the community at large and are important ambassadors for VON. Special Events Throughout the year, VON Durham holds several fundraising events in the community. A Wine and Cheese event in the fall, Beach Ball Gala in late winter and Hike for Hospice services in the spring are just three of the events where volunteers lend their support. Volunteer assistance is always welcome to make these and other events a success.

How to Become a Service Volunteer with VON It is as easy as calling the VON office and expressing your interest in becoming a VON volunteer. VON will ask that you: • Complete a confidential application form.

• Attend an interview with the Volunteer Coordinator and successfully complete our screening process.

• Participate in training and orientation appropriate to your Volunteer position.

After completing these steps, you will become involved with a program that suits your interests, skills and experience.

How to Become a Community Corporation Volunteer with VON • Contact the VON office and express your interest in joining the Community Board.

• Submit a written resume.

• Attend an interview with the Community Board Chair, Board Development representative and VON staff representative.

• Participate in Board and/or Committee Member training and orientation.

When you become a VON Volunteer, we will: • Match your strengths, interests and available time to a volunteer role that’s right for you.

• Give you a clear picture of the position requirements and the training to do it well.

• Support you in your volunteer role through regular contact.

• Provide ongoing educational opportunities to assist you in your role, to introduce you to new opportunities and to allow you to connect with other VON volunteers.

• Give you the care and appreciation you deserve.

Community Corporation VON Durham Region Community Corporation is the VON equivalent to a volunteer Board of Directors or Foundation; responsible for raising funds to support local programs and services. They also participate in a variety of activities promoting community based health care, and advocate on behalf of seniors and caregivers in the region.

Provincial Governments are responsible for healthcare in Canada; and to address the ever-increasing need, fundraised dollars are also required to help cover costs.

The money raised by our VON Durham Community Corporation comes from a variety of sources and is used to: provide client subsidies for those unable to pay, purchase specialized medical equipment, and to provide ongoing training and education to our professional staff and volunteers.

In 2018 we need to raise $215,000 from the community. Please help us reach our goal by making a donation or organizing a special event.
