They work in partnership with a wide range of organizations and groups to help meet the health needs of the communities. Our CHBs working in their communities create collaborative community health plans, which are an important part of the district’s planning processes and one important way that AVH hears the voice of the community. For more information about the activities of the CHBs, visit us on the web: http://www.avdha. nshealth.ca/community.

YOU can visit the centre to learn how to manage your diabetes by modifying your lifestyle to make it work for you. We offer individual and group sessions. Programs are tailored to the needs of people with diabetes and their family member/support person(s). We work with family physicians, specialists and other team members and community agencies to provide comprehensive care for our area.
