The monthly program features a health talk by a local healthcare provider, a mile-long walk around downtown Winfield, and a stop at a sponsoring business. Downtown Doc Walks (and coordinator Sarah Johnson) were recognized as a Spring 2023 Winfield Chamber Champion highlighting the program's goal to help our communities grow through collaboration and teamwork. For many facilities in rural health, 2023 has been a trying year. William Newton Hospital has been focusing on financial stability to ensure access to care for our community members. Our operations have improved compared to 2022, and we’re continuing to expand to offer a higher level of care.

WNH is one of two critical access hospitals in Kansas operating a cardiac catheterization lab, which is slated to have a record year in 2023. In addition, the hospital launched a new service earlier this year, pulmonary rehabilitation, to treat patients with chronic lung disease. Each of these services helps address major health needs within our region.

Strengthening our community partnerships has been another focus, and will continue to be one of our key initiatives in 2024 as we strive to educate our community about the services we provide. Also in 2024, we will be asking you, our constituents, how we can engage to better serve you and your loved ones. I invite you to join us in these initiatives as we continue to uphold our commitment to providing quality, local healthcare for all.

ABOUT WNH

William Newton Hospital is a 25-bed Critical Access Hospital in Winfield, KS. The non-profit community general hospital is locally managed and financially self-sufficient, relying entirely on patient revenue and philanthropic support. It is not supported by local taxes and does not receive outside health system assistance. The William Newton Hospital network provides general medical and surgical care for inpatient, outpatient, and emergency room patients, and participates in Medicare and Medicaid programs. Just a few of the services offered include 24/7 emergency care, primary care, labor & delivery, radiology, cancer care, orthopedics, cardiology, occupational health, and rehabilitation services such as physical, speech, and occupational therapy.

BREATHING BETTER WITH PULMONARY REHAB

Pulmonary rehab at WNH opened in March 2023 and, as of this publication, has 20 active patients with 11 patients on a waiting list. Since opening in March, we have welcomed a second respiratory therapist, Serenity Morris, and are almost ready to add a third to our growing team. The growth potential is unlimited. As more patients are referred, additional staff will be added. This program is unique from other traditional pulmonary rehabs which only offer group class type sessions. The therapy sessions at WNH are all one-on-one with a respiratory therapist. This individual attention allows for a personalized experience to help patients achieve goals in a more timely manner.

During treatment, the referring provider receives patient results from the initial evaluation and monthly progress of a six-minute stress test and a forced vital lung capacity test that can be compared to prior results. The program length is 18 weeks with 2-3 visits per week. Pulmonary rehab won’t cure lung disease, but it will improve a patient's quality of life and allow them to breathe a little easier. Questions? RYAN'S JOURNEY

Thirty-three-year-old Ryan Smith was the program's very first patient. We caught up with him and respiratory therapist Michelle Jones to learn about Ryan's experience with WNH pulmonary rehab and the impact it has had on his quality of life.

Michelle says, "The first session with Ryan, he terrified me. He got on the treadmill and his lips turned blue. He had his oxygen on and we did a six-minute stress test. I was afraid that we weren't going to make it the entire six minutes." In late summer, towards the end of Ryan's treatment sessions, Michelle reported that he was able to maintain a quick walking pace at a slight incline for up to 30 minutes with his oxygen staying steady. "He is doing amazingly better than when he first started," she beamed.

In March, Ryan was on a waiting list for an immediate lung transplant. Part of the requirements for the transplant was completion of a pulmonary rehab program.

Through his dedication to the rehab program and Michelle's support during all of IT'S A MARATHON. IT'S NOT A SPRINT. IT TAKES TIME AND CONSISTENCY EVERY DAY, MAKING LITTLE IMPROVEMENTS TO HELP ACHIEVE GOALS OF BEING ABLE TO BREATHE EASIER.

Ryan's treatments, the lung transplant can be postponed for up to five years. "It's not immediate like what it was before he started the program. He has worked very hard to achieve that," shares Michelle.

Prior to participating in the rehab program at WNH, Ryan was driving to Wichita regularly for rehab sessions with his lung specialist. The program in Wichita was a group setting as opposed to the one-on-one sessions offered to patients in Winfield.

Ryan, who suffered lung damage in his early teens, says the one-on-one setting has been a great experience for him. "It's been fun, but a lot of learning curves. We've got fun banter. You get to talk and you have someone there just in case something goes wrong. They understand your lung issues and they know when to push and when not to push so you're not over-exerting your lungs and causing more issues."

Ryan has eight young children at home. Before the rehab program, he wasn't able to walk much or play with his kids without toning down their activities. Now, he says, "I can do more and get up with my kids and go outside for a little bit. I can't run marathons, but I can do little activities and it makes a difference every day. When I finish with the program, what am I looking forward to the most is being able to play with my kids without having to stop as much."

One of the first things Michelle picked up on was Ryan's infectious personality and his love for his family, which she says helped him along his journey to breathing easier.

"Ryan is the nicest guy that you would ever want to meet. He has the best attitude.

He's always positive. He works really hard. He is always happy, always smiling, even when he's having problems breathing. He has eight children to be there for."

WNH's pulmonary rehab program is growing and has a waiting list of patients ready to step towards getting their breath back. Michelle explains, "Anyone with shortness of breath, this program can benefit them. If you think you can't, you definitely can.

We start out slow and we work at your level and make tiny improvements every day.

It's a marathon. It's not a sprint. It takes time and consistency every day, making little improvements to help achieve goals of being able to breathe easier."

Ryan graduated from the pulmonary rehab program in August 2023 and is looking forward to spending more active time with his wife and kids. William Newton Hospital is proud to be able to offer a program like pulmonary rehab in Winfield, continuing a commitment to providing big-city medicine in a small-town setting.

IF THIS IS THE ER, WHY DOES IT TAKE SO LONG?

You wake in the middle of the night with horrible chest pain, your child is running a fever, or you fell earlier in the day and you’re concerned about a fracture. Your local doctor is fast asleep, as you wish you were, but you need answers. So, you visit your local emergency room. You arrive, your information is taken, and you are checked in. What comes next, and why do things play out the way they do?

The emergency department (ED or ER) is open 24 hours a day every day, no matter the holiday or the weather. When you have a medical concern, we’re there to try and answer your concerns. Expectations of the ED have changed as people’s perceptions and access to healthcare have changed.

CASSIE STEIN, BSN, RN, EMT However, the base reason for the ED is to rule out emergency conditions.

After checking in with registration, you will be triaged. Certain conditions Emergency Department Manager or concerns (such as stroke symptoms, chest pain, or trauma) get a higher level of priority and attention. This isn’t to say your concern isn’t important, but the ED does not operate on a first-come, first-serve basis. Certain situations are more time-sensitive and life-threatening than others. This is where the triage system comes into play.

A triage system helps physicians prioritize by patients’ chief complaints, vital signs, symptoms provided by the patient, and assessment. Depending on the information gathered, this will assist in how quickly you are seen. Those with more life-threatening conditions will be placed at the front of the line while others who have less critical or serious concerns will be addressed secondarily. At times, you are brought straight back into the ED and placed in a room. Other times you may be triaged and asked to return to the waiting room. The ED staff doesn’t like you to wait to be seen any more than you do. Sometimes, there are multiple critically ill patients already in the back and rooms are limited. The tempo of the ED can change quickly — the ED staff does its best to prepare for the next emergent situation. Understanding and consideration are appreciated if you’re asked to return to the waiting room. If something THE ED DOES NOT OPERATE ON changes, alert registration and you’ll be assessed again for changes.

A FIRST-COME, FIRST-SERVE The nurse and provider take the information collected from your assessment BASIS. CERTAIN SITUATIONS ARE and orders will be placed for staff to complete. Lab work usually takes around an hour THREATENING THAN OTHERS. to return, but some tests can take up to days. Radiology exams may have to wait based on lab results. Once completed, all radiology exams are sent to a radiologist, who will view and send a report back.

This usually takes around an hour but may be longer if they have a large number of exams to view. The ED works in tandem with other departments to complete your assessment and decide on a course of treatment.

While you wait, other patients come and go. Medical staff are working hard to assess, reassess, complete orders, and discharge, admit, or transfer patients. As new patients arrive or a current patient has a change of condition, the medical crew's response can change. A 2022 study found the national average length of stay in the ED is three hours, 28 minutes.

The ED has many working and ever-changing parts. No day, patient, or even shift is ever the same. The staff works diligently to take the best care of you in the most efficient and timely manner possible. Sometimes after all those tests, questions, and medication, the staff doesn’t have an answer. While it's frustrating, our job is to rule out life-threatening emergencies. Follow up with your primary care provider to continue care and answer questions the ED may be unable to.

While our emergency room is open to all, we ask you to help keep it available for those who need it most. Some ailments can be addressed by your primary care provider (PCP) while others require immediate care.

WHAT YOU NEED TO KNOW ABOUT LOCAL MEDICARE COVERAGE

OPEN ENROLLMENT FOR MEDICARE COVERAGE IS OCT. 15 – DEC. 7 William Newton Hospital (WNH) finds Original Medicare works very well for our patients, but we understand you may want to consider other coverage options. WNH accepts all insurances; however, higher out-of-pocket costs may result when not in-network. When choosing a plan, keep in mind: IS MY HOSPITAL, CLINIC, & PHYSICIAN IN-NETWORK? Providers often bill for services differently. Just because your doctor is in-network, doesn’t mean all services you receive at that facility are in-network. The hospital itself accepts all insurances, but higher out-of-pocket costs may result when not in-network. The WNH rural health clinics DO NOT accept Medicare Advantage (Part C) HMO plans. Original Medicare (Parts A & B) is accepted by all WNH facilities, providers, and clinics. Make sure the individual healthcare providers you know and trust are in the network you are considering. Please visit our website for a list of other in-network plans at each of our facilities, or call us before you purchase a Medicare Advantage plan.

AM I ENROLLING IN A SUPPLEMENT OR REPLACEMENT? There is confusion surrounding different types of Medicare plans. If you’re looking for a supplement plan (Part D), make sure you are not enrolling in a replacement plan (Part C). Medicare Advantage plans (Part C) cancel and replace your Original Medicare plan (Parts A & B).

AM I CONSIDERING COSTS BEYOND THE PREMIUM? Often, beneficiaries face disappointment when they pay more than they budgeted using additional benefits such as dental, vision, and hearing care. Even though a plan offers coverage, patients may be met with high out-of-pocket copays and low maximum benefit amounts. With some plans, once you exceed a certain amount of coverage, you are responsible for 100% of your costs. Also consider the travel costs you incur when having to go outside of the community to find in-network providers.

WNH recommends scheduling a free session through your local Council on Aging to visit with a certified Senior Health Insurance Counseling for Kansas (SHICK) health counselor. To help you choose the plan that’s the best fit for you, the counselor will evaluate your health needs and medical budget.

PETERS: I walk the dogs, go to the gym at the Winfield Recreation Commission, read books, and take at least one full week of vacation every year.

WEHNER: I like to spend time with my daughters and my grandchildren when I'm able. I usually spend time with them on the weekends as they live and work in Wichita. I would like to take up golf for exercise as my youngest daughter is an avid golfer.

PETERS: We go out to eat in town or watch a movie at home.

WEHNER: I am usually at home resting from the long week. I am usually catching up on my charting from work so I can have a carefree weekend.

Sometimes I will go out to eat. My boyfriend, Tom, will come over and we will walk the dogs.

WHAT PROFESSION OTHER THAN YOUR OWN WOULD YOU LIKE TO ATTEMPT? MORRISSEY: I have a personal training and nutrition coaching business. I also teach Osteopathic Manipulative Medicine at the new DO school in Wichita, so I stay busy. I am a YouTube contractor ;) So, I guess I would like to try house remodeling and perhaps real estate.

COLLABORATION: THE BACKBONE OF WNH The foundation's section of HouseCalls focuses on collaboration, which is something that William Newton Hospital believes in as well. It is the collaborative nature of our partners, donors, stakeholders, and community members that make local, rural healthcare work.

An example specifically linked to our new surgery center is the fact that – for about 15 years now – William Newton Hospital ANNIKA MORRIS has had a great partnership with KU Med, WNHF Executive Director where CRNA students have an opportunity to shadow our CRNAs in the new surgery center. We also have many aspiring physicians and nursing school students that learn about the rural healthcare setting while working alongside Dr. Morrissey, Dr. Bloedel, and several other WNH healthcare professionals. It is our hope that their hands-on experience will positively encourage them in their medical journeys.

SECOND CENTURY CAMPAIGN | SURGERY CENTER EXPANSION

Many of you reading this have given to phase 1 of our long-term strategic plan, which was the H. L. Snyder Medical Foundation Surgery Center. This project, aimed at further supporting our goal of providing comprehensive surgical care close to home, was completed in early 2020, just before the COVID-19 pandemic hit our area. Now, fast-forward three years, and our team is regularly adding new procedures in response to the availability of state-of-the-art technology and operating rooms. The surgery team is projected to perform 2,378 surgeries this year in the new center. Since its opening, the center has performed 8,300 procedures. As donors, your gifts impact the patient experience at WNH, and we thank you for your role in helping us provide the best care.

One of our most recent fundraising projects is a great example of our donors' commitment to local healthcare and their dedication to supporting the future of William Newton Hospital. Through our Arbor Day fundraiser this spring, donors came together to raise money to purchase new trees for our new parking lot, which will help provide shade for future generations while creating a welcoming, refreshing environment as you enter the hospital campus.

This collaborative project, spearheaded by the WNHF staff, Joan Cales, Greg & Julie Thompson, and Dr. Sapna Shah-Haque, is one example of the impact philanthropy can have both in the halls of our buildings and out in the community.

SAVING LIVES, ONE HEARTBEAT AT A TIME

The Beats Go On Community Wellness Initiative, co-chaired by Paula Radcliff and Melanie Burnett, is our spin on the national Million Hearts program. Million Hearts began as a way to address health disparities across the nation and help prevent one million strokes and heart attacks within five years. In initiating the Beats program, we will have the tools and resources available to identify ways to help our communities and potentially have some of our friends, co-workers, and family members be a part of those one million lives saved. With a focus on cardiac education, trauma awareness, and community health, we are excited to begin promoting these programs in our local communities: + Project ADAM is being instituted in schools to educate students and staff on sudden cardiac death. + Stop the Bleed will be taught in area businesses to assist in trauma cases and active/accidental shooter situations. + Our physicians and other local healthcare providers are participating in monthly Downtown Doc Walks to aid in public education and get to know the community outside of the office. + In partnership with USD 465, Emergency Preparedness Buckets are being placed in all schools in the Winfield district and surrounding communities with tools and resources used in an emergency situation. + In addition to these educational initiatives, our team will also be part of area events, community celebrations, fairs, and rodeos to get to know people in our surrounding rural areas. In 2022, $26,000 was given to the Beats Go On initiative. Since then, the Beats have been part of nearly 30 events and have provided funding for several of those. Events include CPR training, outreach/educational opportunities in partnership with the Moline Library and local doctors, area parades, Rotary presentations, the Radcliff Lecture Series on Rural Health, Downtown Doc Walks, and more.

A HEALTHY HEART STARTS WITH YOU Heart-healthy living involves understanding your risk, making healthy choices, and taking steps to reduce your chances of getting heart disease, including coronary heart disease, the most common type. By taking preventive measures, you can lower your risk of developing heart disease that could lead to a heart attack.

PREVENTIVE MEASURES:

Eat healthy, get active, maintain a healthy weight, quit smoking and stay away from second hand smoke, learn how to control your cholesterol and blood pressure, drink alcohol only in moderation, and find healthy ways to manage stress.

HOW TO CONTROL YOUR CHOLESTEROL AND BLOOD PRESSURE:

• Cut extra pounds by increasing your activity • Limit your sodium(salt) intake and eat fewer processed foods • Avoid caffeine • Increase your daily activity with 20-30 minutes of moderate activity (like walking, swimming, cycling, jogging, or dancing) • Work on reducing stress and improving your sleep • Increase your intake of soluble fiber by including whole grain breads, oats, kidney beans, chickpeas, and barley in your daily meals • Use low fat options for your dairy choices

SIGNS OF HIGH BLOOD PRESSURE:

High blood pressure is known as the silent killer! Many people have no symptoms but over time if untreated can cause health conditions such as heart disease and stroke. Once blood pressure reaches a certain level you could experience blurry/double vision, headache, heart palpitations, fatigue, lightheadedness or fainting, shortness of breath, nausea, and/or vomiting.

SIGNS OF A HEART ATTACK:

Chest pain (pressure, tightness, pain, squeezing or aching), pain or discomfort that spreads to the shoulder, arm, back, neck, jaw, teeth cold sweat, nausea, heartburn or indigestion, and/or lightheadedness/ dizziness. Women can also experience a general feeling of unwellness and describe the chest pain as pressure or tightness and not the severe pain often felt by men.

PODIATRIC MINIMALLY INVASIVE SURGERY:

FASTER RECOVERY. SMALLER SCARS. LESS MEDS.

Foot and ankle surgery has changed over time. With advancements in Minimally Invasive Surgery (MIS), a podiatric surgery recovery time that used to take months can now be shortened to weeks. MIS also offers smaller incisions (about 75% smaller than regular open incisions) and a reduced need for postoperative opioids.

WNH’s new procedure room is dedicated to these types of procedures. This means patients can bypass the pre/post-op area, and most patients experience reduced wait times and fewer delays for elective procedures.

MIS procedures can be done with only local anesthesia (a.k.a., you're not being 'put to sleep'), which minimizes risks for the patient, and patients are able to recover from the surgery with only a small incision(s) and few stitches.

Among the many procedures that may qualify for MIS are bunion surgery, hammertoe repair, big toe arthritis, and metatarsalgia. Utilizing a MIS option for procedures like these can shorten recovery time, reduce post-operative pain, and reduce long-term scarring and stiffness. While MIS is a great option for most patients, not all patients are good candidates.

It is important to consult with a podiatrist to determine the best treatment option for your specific case.
