We are very pleased to be able to offer zero increase to all premiums for the 2020 plan year, and no plan design changes! We appreciate your hard work and dedication to Ward Kraft and are looking forward to a healthy and prosperous 2020! Warm Regards, Roger Kraft

FLEXIBLE SPENDING ACCOUNTS FSAs through NueSynergy.
MAXIMUM CONTRIBUTION AMOUNTS FOR EMPLOYEES
WHY AN FSA?
An FSA allows you to save up to 30% on your eligible healthcare and/or dependent care expenses every year by using pre-tax dollars. Offering an FSA as part of our employee benefits program provides valuable benefits to our employees:
EMPLOYEE BENEFITS • •
• •
Reduces income tax (Federal, State, and FICA): pre-tax payroll contributions result in a lower taxable salary. Saves on the cost of eligible healthcare and/ or dependent care expenses: using pre-tax dollars spells out a savings of nearly 30%. Offers immediate access to elected healthcare FSA funds. Covers common types of expenses: medical, dental, ortho, vision, prescription drugs, day care and more.
HOW IT WORKS
An employee who participates in a flexible spending account must place a certain dollar amount into the FSA each year. This “election” amount is automatically deducted from the employee’s check (for that amount divided by the number of payroll periods). For every dollar you put into these accounts, the more money you save by paying less in taxes. As you incur eligible expenses, you simply use your provided NueSynergy debit card, or submit a request for reimbursement to NueSynergy, up to the amount of your annual contribution.

Even though you pay these expenses, they don’t count toward the out–of– pocket limit. This plan uses a provider network. You will pay less if you use a provider in the plan’s network. You will pay the most if you use an out-of-network provider, and you might receive a bill from a provider for the difference between the provider’s charge and what your plan pays (balance billing). Be aware, your network provider might use an out-of-network provider for some services (such as lab work). Check with your provider before you get services.
Premiums, balance-billing charges, Pre-Certification penalties, and health care this plan doesn’t cover.
What is not included in the out-of-pocket limit?
What is the out-of-pocket limit for this plan?
You don’t have to meet deductibles for specific services.
Yes. In-Network preventive care and services covered at “No charge”.
Generally, you must pay all of the costs from providers up to the deductible amount before this plan begins to pay. If you have other family members on the plan, each family member must meet their own individual deductible until the total amount of deductible expenses paid by all family members meets the overall family deductible.
Why This Matters:
This plan covers some items and services even if you haven’t yet met the deductible amount. But a copayment or coinsurance may apply. For example, this plan covers certain preventive services without cost sharing and before you meet your deductible. See a list of covered preventive services at https://www.healthcare.gov/coverage/preventive-care-benefits/.
Answers $4,500 Individual / $9,000 Family for In-Network $6,000 Individual / $12,000 Family for Out-of-Network The deductible will be reduced to $500 Individual / $1,000 Family if TopCare provider is utilized Deductible is embedded.

This is not a cost estimator. Treatments shown are just examples of how this plan might cover medical care. Your actual costs will be different depending on the actual care you receive, the prices your providers charge, and many other factors. Focus on the cost sharing amounts (deductibles, copayments and coinsurance) and excluded services under the plan. Use this information to compare the portion of costs you might pay under different health plans. Please note these coverage examples are based on self-only coverage.
