Although we are past the April peak of COVID-19, we understand that some people may still be wary of going to the hospital or the doctor’s office right now for routine tests and screenings that were put off during the height of the pandemic. But don’t postpone your health. We at Saint Barnabas Medical Center (SBMC) are taking all safety and social distancing precautions and are always here for our patients, whether you’re coming in for an outpatient appointment or require hospitalization. September is Prostate Cancer Awareness Month, and Breast Cancer Awareness Month is coming up in October, so now is the optimal time to get screened for these and various other conditions. Make an appointment with your general practitioner and/or your specialists to make sure you’re up-to-date on important tests. That’s exactly what Joseph Fogelson did. When the 63-year-old with a family history of heart disease felt more tired than usual, his long-time cardiologist ordered some tests. The transesophageal echo confirmed Fogelson’s aorta had gotten larger and he had an aortic root aneurysm. His valve-replacement surgery with Ioannis Loumiotis, M.D., cardiothoracic surgeon at SBMC, was postponed from March when COVID hit until May, but the procedure ultimately saved his life. Turn to page 30 to read more about Fogelson’s story, his surgery and how his recovery is going now. This issue of Morris/Essex Health & Life also introduces our readers to The Maida Geriatrics Institute, a comprehensive geriatrics center of excellence at the Barnabas Health Ambulatory Care Center in Livingston that tends to the physical and medical needs of adults ages 65 and older. The center features a sub-specialty group of family doctors or internists who are specially trained in geriatric medicine. Along with a multidisciplinary staff, these doctors conduct annual visits and also help patients maintain their complex chronic conditions. Page 32 has more info on this exciting new facility, the second such facility in our system. COLLEGE, 2020 STYLE This year, when local campuses roll out the welcome mat, it will be “safety first.” A collegian’s move-in day—parents helping sons and daughters tote suitcases, futons and mini fridges into stuffy dormitories—has long been a rite of passage. But this year, because of the continuing danger of COVID-19, the semester will begin a bit differently for many. Here’s what local campuses have planned: —Caldwell University: The Catholic liberal arts school begins its fall semester Sept. 1, though in-person or remote learning will be determined by the state. If New Jersey is still in phase 2 of reopening, classes will be held virtually with the exception of labs and other clinical courses; if it has moved to phase 3, students will have the option to attend class in person or remotely. A survey will be sent to students about their preferences. The school is planning a condensed semester with classes held Sept. 7, Oct. 12–13 and Nov. 25—days it typically would be closed. Classes will be online following Thanksgiving break. Residence halls and dining areas will have reduced capacity, and outside guests will not be permitted to visit. All students, faculty and staff will be required to wear face masks on campus and follow regulations for social distancing, hand washing and other sanitization practices. —County College of Morris: Classes on the Parsippany campus begin Sept. 9 and will fall into three categories: Hybrid courses combining traditional face-to-face instruction and virtual sessions; online courses without specific meeting times; and remote courses that include virtual meeting times with the entire class. Most classes will take place in the online and remote formats. Students and employees who report to campus should follow safety guidelines such as wearing face masks, washing hands frequently and

refraining from visiting campus if they are ill or have COVID symptoms. —Essex Community College: Regular classes at the Newark school began in an online format on Aug. 31. Weekend classes are scheduled to start virtually on Sept. 5. Academic labs are open with limited capacity to students without access to technology at home. The campus gym remains closed, and all non-essential events are suspended. —Fairleigh Dickinson University: The Florham Campus in Madison and Florham Park is home to nearly 3,500 students. Virtual classes began Aug. 17 and will continue for three weeks, while in-classroom instruction resumes Sept. 8. At that point, teachers and students will follow a hybrid schedule that combines online and in-person learning. All students, staff and faculty will be required to complete and report a self-screening every day of the semester. FDU has also installed hand sanitizers and plexiglass shields throughout campus and reduced the density in living spaces. Common areas in the residence halls and laundry facilities will specify maximum capacities. —Montclair State University: Besides stressing social distancing, the use of personal protective equipment and hand hygiene, university officials require daily self-health monitoring. Classes begin Aug. 25 and are offered in a range of options known as HawkFlex: traditional face-to-face and on-campus, face-to-face with groups while streaming content to other classmates; online classes with in-person class activities; online with classmates; and online independent work. Instruction following Thanksgiving break will be conducted virtually. Students living on campus will see changes in the residence halls after their staggered move-in period in August. All dormitories will have reduced occupancy. —Rutgers University–Newark: Most classes at the urban campus begin Sept. 1 and will be conducted in one of three ways: remotely for asynchronous learning, remotely with interaction and in person, when feasible. In-person classes will be reserved for labs, and all attendees will be required to follow strict health precautions. The university will make approximately 30 percent of its student housing available to students, and all rooms will be singleoccupancy. In addition, residence hall lounges will be closed, and overnight guests will not be allowed. Dining will be available on a takeout and grab-and-go basis. —Seton Hall University: This Catholic institution in South Orange will conduct its fall semester from Aug. 24 through Nov. 24 using a hybrid flexible (HyFlex) plan that employs in-person and virtual instruction. Students will not return to campus after Thanksgiving break. Those moving into residence halls will be asked to quarantine for 14 days prior to their move-in day or submit a copy of a negative COVID-19 test. Dorms will reopen with reduced density. Residents will be asked to sign a pledge accepting their health responsibilities and possible COVID testing in the future.

SHOP FOR A SENIOR Getting groceries is a challenge these days for some older folks and disabled individuals—and potentially a danger. That’s why stores have designated certain hours for senior citizens and disabled shoppers. But Morris County and the nonprofit group Jersey Cares want to do more. The county is seeking volunteers to shop in grocery stores and other markets on behalf of people vulnerable to COVID-19, keeping them out of harm’s way. Volunteers will be screened by Jersey Cares. To receive assistance, participants must be age 60 or older or be disabled and over 18. “While we developed the program in response to a critical COVID-19 need, we realized this need has always been there,” Michele Epifani, vice president of programs for Jersey Cares, has said. NEWARK’S OWN SEACOOK Spending time out on the ocean can make anyone hungry, particularly when the catch of the day is just outside the porthole. But as Tom Hanks learned in Castaway, crustaceans should be cooked to be enjoyed. The sailors aboard the USS Germantown, a Whidbey Island-class dock landing ship, appreciate Newark’s Levy Peterson, whose kitchen handiwork keeps everyone going. The culinary specialist second class recently prepared lobster tails for the crew’s dinner (pictured) as they approached the Philippine Sea as part of a peacekeeping mission in the IndoPacific region.

PUPPY LOVE

Dog: Pinkerton (aka Pink/Pinkie), a 5-year-old pitbull/chihuahua/boxer (and many more!) Owners: Chelsea Arnott and Clay Clements of Whippany When Arnott and Clements decided three years ago to bring a pooch into their home, they visited the Mt. Pleasant Animal Shelter in East Hanover. Pinkerton didn’t give the best first impression—he growled from his kennel at Clements, says Arnott. But after learning the pup had been mistreated in the past, the couple wanted to give him a chance. “The second he was brought into the room, his personality changed,” she recalls. “He hopped up onto the couch, rolled onto his back and gave us kisses!” Today, Pinkerton joins his owners on walks around the neighborhood, excursions to Loantaka Park and strolls at the Morristown Green. While Pinkerton stays active, Arnott says his demeanor is more laid-back—he enjoys catching up on zzz’s and cuddling with his parents. That’s probably how he developed a love for blankets. “Pink waits for us to tuck him into his dog bed,” Arnott says. Up next? He’s set to be the ring bearer at his owners’ wedding this month! FARMERS’ MARKETS ARE BACK! Whether the DIY garden you started during quarantine is giving you fresh veggies or just feeding the woodland creatures in your yard, we’re all grateful that farmers’ markets have returned. Now that the curve has flattened a bit in the Garden State, farmers here have the go-ahead to sell their products outdoors. Shoppers will find much of the same fresh produce, baked goods and other locally grown and made products as in previous years. What’s different will be the new safety measures in place to guard against the coronavirus. Vendors and volunteers are required to wear face coverings and gloves, while customers will be asked to wear masks, practice social distancing and refrain from handling products if possible. If you want to squeeze a tomato, ask a vendor first! KIDS ARE STILL CURIOUS Imagine a museum with three exclamation points in its name. Imagine That!!!, a New Jersey children’s museum in Florham Park, provides educational and safe fun for young ones of all ages and their parents. And, yes, it has reopened its indoor facility. If your children picked up drawing skills during the quarantine, bring them to the center’s arts and crafts area. Kid who loves to climb big rigs? There’s a fire truck and gym area for your little adventurer. Other designated areas cater to youngsters who dance, play music, build with blocks and more. If you’re concerned about COVID-19,

the staff takes several measures to keep you and your children safe. For example, tickets will be sold in advance only (no walk-ups), admission will be kept to 25 percent of capacity and rules regarding social distancing and sanitization will be observed. Those who are high risk should “wait a few months” before visiting, museum officials say. SENDING A MESSAGE

Heard first on stage in Hamilton, the statement “This is not a moment; it’s a movement” is now one of the rallying calls of the Black Lives Matter human rights campaign. As part of the movement, in June hundreds of artists took to the street in Newark—Halsey Street, in particular—to peacefully express themselves and paint a tribute to the initiative as well as to the lives lost to police violence. Using yellow traffic paint, artists from Rutgers University–Newark, New Arts Justice, Project for Empty Space and Newark Arts wrote “ALL BLACK LIVES MATTER” in 25-foot-long capital letters to create a message that spans one city block. Another phrase, “ABOLISH WHITE SUPREMACY,” was painted in a similar color adjacent to the Essex County Historic Courthouse on Martin Luther King Jr. Boulevard. “It’s a tribute to all Black lives that have been needlessly lost,” says Newark Ironbound resident Mitchell Bonanno. “And it’s also a reminder that this movement won’t end until all forms of racism end.” MSXHEALTHANDLIFE.COM

DANGERS EVERY PARENT NEEDS TO KNOW

Though the season officially ends this month, summer fun will continue. For moms and dads, that means keeping their guards up against these four hazards. More dangers to worry about? That’s the last thing you need in this pandemic summer of 2020. But health experts know that every year brings preventable tragedies, and you don’t want your family—distracted, perhaps, by virus fears or a wounded economy—to be touched by one. Bad things can happen suddenly. Here are four nightmare scenarios with tips from the Centers of Disease Control and Prevention (CDC) on how to avoid them. BABIES LEFT IN HOT CARS Parents think they’d never forgetfully leave an infant or baby in an automobile in the summer heat. But statistics show that this potentially fatal situation happens to families from all walks of life. Even with windows left open, cars can act as an oven. According to the CDC, temperatures inside a vehicle can rise almost 20 degrees Fahrenheit within 10 minutes. To avoid accidentally leaving a child behind, try setting reminders on your phone or placing something in the back seat that you have to take out such as your wallet or phone. And remember: No errand, no matter how quick you think it’s going to be, justifies leaving your baby alone in your vehicle.


YOUNG CHILDREN AND BICYCLE ACCIDENTS You need to train your kids and remind them regularly: To keep safe while riding bicycles, they must pay attention to their surroundings. Also, if they’re riding at night, they need to wear bright or reflective clothing and make sure their bike has a working headlight. But the CDC says the No. 1 most effective safety measure is to wear a helmet. “All bicyclists, regardless of age, can help protect themselves by wearing properly fitted bicycle helmets every time they ride,” the CDC notes. Bicycle shops like Diamond Cycle Montclair and Cycle Craft in Parsippany can help parents determine an appropriate helmet size for their kids. The absolute rule: No helmet, no ride.

TEENS TEXTING AND DRIVING Cell phones are a huge distraction for teenage drivers, so start telling your children about the dangers of texting and driving before they first get behind the wheel. The CDC advises that teens turn off their phone or put it on silent when they get in the car. Do your part: Don’t text or call your teen when you know they’re driving. If your teen needs to use the smartphone for directions, make sure they program it and set up the phone before they start driving. To take it a step further, there are also downloadable “safe driving” apps such as DriveMode that prohibit texts and calls from going through while one is driving. Again, no matter how fully in control a young driver may feel or how urgent it may seem to quickly scan that latest text, a no-exceptions policy is imperative. In traffic or on the highway, disaster can result from a moment’s inattention.

KIDS OF ALL AGES DROWNING It makes sense that water accidents peak in the summertime. The CDC recommends that kids of all ages require supervision while swimming, as drownings are the leading cause of injury or death for children ages 1 to 14. “Because drowning happens quickly and quietly, adults watching kids in or near water should avoid distractions like playing cards, reading books, talking on the phone and using alcohol or drugs,” the CDC says. It also suggests parents and guardians know the basics of swimming (floating and moving through the water), as well as CPR. Backyard swimming pools should be surrounded on all sides by a fence with a self-latching gate, and life jackets should be worn in and around oceans, lakes and other natural bodies of water as well as around pools.

CARDIAC CARE POST-COVID

Despite his surgery being delayed two months due to COVID-19, Joseph Fogelson’s aortic valve replacement surgery at Saint Barnabas Medical Center ultimately saved his life. Joseph Fogelson has been seeing a cardiologist since 2003. He has a long history of heart disease in his family: Both his mother and uncle died suddenly from aortic aneurysms. In early 2020, Fogelson, 63, had his annual echocardiogram. “I noticed I was getting more tired than usual,” he says. A follow-up transesophageal echo (TEE) showed that Fogelson’s aorta had gotten larger and confirmed that he had an aortic root aneurysm at the origin of the aorta in the heart. He had a life-saving surgery scheduled for March 30, 2020 with his surgeon, Ioannis Loumiotis, M.D., cardiothoracic surgeon at Saint Barnabas Medical Center and Newark Beth Israel Medical Center, but then COVID-19 hit. When COVID-19 started to consume the hospitals, all non-emergent procedures across the state were postponed, Fogelson’s surgery included. “I understood that my surgery needed to be postponed, but it made me very nervous,” he says. “I knew that without this surgery, there was a chance that I could die.” His surgeon agreed that the situation was timesensitive. “Given the size of his aneurysm and his family history, Joe needed a surgery called valve-sparing root replacement to prevent potential complications like rupture or dissection,” he says. During the peak of the pandemic, Fogelson stayed at home with his wife, daughter and dog. Besides going out to get essentials at the grocery store and taking his dog on short walks, he did not leave his house. “Because of Joe’s already

dilated aorta and family history, he was at a higher risk of complications,” says Dr. Loumiotis. “High blood pressure and smoking are the two most important, modifiable risk factors that can cause an aneurysm to rupture. Joe does not smoke, and we were able to control his blood pressure while he awaited his surgery.” The state allowed surgeries to resume as cases of COVID-19 in New Jersey continued to decline, and so Fogelson’s cardiac surgery took place almost two months later than planned on May 22. This milestone surgery was one of the first ever valve-sparing root replacement surgeries performed in the RWJBarnabas Health system. “I was confident that Saint Barnabas Medical Center had taken every precaution to keep me safe while I was in surgery and in recovery,” the patient says. “This is a technically difficult surgery where we remove the dilated portion of the aorta around the aortic valve, keeping the patient’s own valve in place. We then replace the aorta with a prosthetic graft and reattach the aortic valve within the graft,” Dr. Loumiotis says. “This surgery is so unique because we repair the patient’s aneurysm while still preserving their own aortic valve. It allows the patient to continue living their life without needing blood thinners or any future surgeries.” One pandemic, cardiac surgery and a seven-day stay in the hospital later, Fogelson is continuing to recover at home while doing cardiac rehabilitation. “Everyone from the nurses, technicians and doctors was terrific,” he says.

SALUTE TO SENIORS The James and Sharon Maida Geriatrics Institute at Saint Barnabas Medical Center is up and running—and it’s a one stop shop tending to older adults’ medical and physical needs.

After COVID-19, Saint Barnabas Medical Center (SBMC) debuted the new James and Sharon Maida Geriatrics Institute at the SBMC campus in Livingston. And this brand-new center provides integrated care in one location for those over the age of 65. A multidisciplinary group of physicians and support staff are specially trained to work with the geriatric community and their wide range of physical, mental, medical and social issues, and they work in tandem with you or your loved one’s primary care physician to offer the best possible care. “We are a subspecialty group of family doctors or internists that have additional training after we take or including our fellowship in geriatric medicine,” says Theresa Redling, D.O., FACP, medical director of the James and Sharon Maida Geriatrics Institute at Saint Barnabas Medical Center and one of three geriatricians at the center. “We see patients over 65, [but] the vast majority of people are 85 and above. We also see some of our centenarians who remain independent into their hundreds.” Working with Dr. Redling, Manisha Grover, M.D. and Emily Bahler, D.O., the Maida Geriatrics Institute also has two geriatric nurse practitioners on staff, a psychiatrist that offers electroconvulsive therapy (ECT) and support staff that includes a social worker, geriatrics pharmacy support, support groups and outpatient care such as the administering of intravenous fluids or IV antibiotics.

In addition to annual wellness visits—“outpatient visits prevent inpatient hospitalizations,” Dr. Bahler notes—the Institute staff also helps patients manage chronic conditions that are common as people age, including coronary disease, cancer screening, chronic lung disease, Alzheimer’s disease, stroke, diabetes and chronic kidney disease. “We manage people with multiple chronic medical conditions and we identify accute problems that may present more subtly in the elderly,” Dr. Redling says. The Maida Institute specializes in the “4 Ms” of geriatric care. They are: • What Matters: i.e. your individual goals and care so the doctors can align that with the appropriate treatment. • Mobility: “We look closely at functional status frequently,” Dr. Redling says. “Falls can be disastrous for older adults, and maintaining mobility maintains independence.” • Medication Review and Management, which includes medication reconciliation, where physicians look at how all of a patient’s medications interact with each other and if all are still necessary. “Most people over 65 are on three or more medications and as we get older, some people are on 10, 12 or more medications,” Dr. Redling adds. • Mentation: Geriatricians examine patients’ cognitive impairment—either short term memory loss or something more serious

The Maida Institute is a one stop shop, as it features on-site radiology, audiology, nutritional support, physical and occupation therapy and even a retail pharmacy. Instagram has fallen in love with the avocado, the fruit (a berry, technically) that tops the many artisan toasts and overflowing burrito bowls on users’ feeds. But there’s more to it than a pretty picture. Avos are loaded with heart-friendly monosaturated fatty acids and can lower cholesterol and triglyceride levels. INGREDIENTS:

n 1½ cups low-fat milk* n 2 ripe, fresh avocados, halved, pitted and peeled n ½ cup sugar n 1 Tbsp. lemon or lime juice n 1 cup low-fat coconut milk* n mint sprig, chopped (optional) n vanilla bean, chopped (optional)

DIRECTIONS:

Purée milk, avocados, sugar and juice in a blender. Blend in the remaining liquid of your choice, along with additional flavorings such as mint or vanilla. Follow the instructions on your ice cream maker to freeze and finish.

INGREDIENTS:

n 1 can (12 fl. oz.) fat-free evaporated milk n 1 cup semi-sweet or milk chocolate morsels n ¾ cup water n ½ tsp. ground cinnamon n 1 ripe, fresh avocado, halved, pitted, peeled and diced n 8 5-oz. paper cups n 8 wooden craft sticks

DIRECTIONS:

Heat evaporated milk, chocolate morsels, water and cinnamon in a small saucepan over medium heat until simmering. Stir until smooth, then cool for 10 minutes. Place avocado and chocolate mixture into a blender. Blend on high until very smooth. Divide into eight 5-oz. paper cups. Cover each cup with aluminum foil and insert one stick through the center of each foil cup. Freeze for four hours until firm. Tip: Frozen treat molds may be used. Yield may vary.

RICH AND CREAMY AVOCADO FLAN Yields 8 servings

INGREDIENTS:

n 1½ cups sugar, divided n ¼ cup water n 2 cups whole milk n 1 ripe, fresh avocado, halved, pitted and peeled n 4 large eggs n 1 tsp. vanilla extract n ½ tsp. salt

DIRECTIONS:

Preheat oven to 350°F. Heat 1 cup sugar and water in small heavy saucepan over low heat, stirring until sugar dissolves. Increase heat to medium-high. Wipe the inside of the pan with a wet cloth to remove sugar crystals. Cook, without stirring, about 10 minutes until golden brown. Divide caramel between eight custard cups and swirl to coat bottoms. Place cups in 9x13-inch baking pan. In blender, combine milk, avocado, eggs, vanilla and salt and purée until smooth. Divide mixture between custard cups. Carefully fill baking pan with enough hot water to 1-inch depth. Bake for 30 to 35 minutes or until just set in center. Remove to wire rack to cool slightly. Refrigerate for several hours or overnight. To serve, run small knife around edge of the baking dish. Turn over and shake gently to release.

INGREDIENTS ■ sea salt, for rimming glasses ■ ¼ cup freshly squeezed lime juice, plus lime wedges for rimming glasses and garnish ■ 1½ cups cubed watermelon, chilled ■ 1/3 cup silver (white) tequila ■ ¼ cup triple sec (or Cointreau) ■ 1 cup ice

DIRECTIONS ■ Place sea salt on a small plate. Run lime wedge around the rims of glass and then place glass upside down on plate to coat rim in salt. Place glass in the freezer to chill. ■ Add watermelon, lime juice, tequila and triple sec to blender and blend to combine. Add ice and blend for about a minute, until smooth. ■ Pour margaritas into glass and garnish with lime wedge.
