Voorwoord Hier zijn we met de vierde editie van ons activiteitenrapport. In dit overzicht vindt u welke (nieuwe) werkgroepen en projecten er in d e e e r s t h e l f t v a n 2 0 2 2 binnen ons netwerk actief waren. We geven enkel een korte samenvatting. NIEUWE WERKGROEPEN
GERIATRIE De geriatrische evaluatie is het werkinstrument van het pluridisciplinaire team van zorgverleners betrokken in de zorg voor geriatrische patiënten. Verdere diagnostiek en zorgplanning worden hierop geënt. Zorg gebaseerd op geriatrische evaluatie en geleverd door een pluridisciplinaire team van zorgverleners verbetert de overleving, functionele en geestelijke toestand van geriatrische patiënten en verhoogt de kans op een terugkeer naar de thuissituatie. Elk zorgprogramma geriatrie gebruikt een of andere vorm van geriatrische evaluatie in de zorg, maar er is momenteel geen standaard voor deze evaluatie beschikbaar. Het doel van de werkgroep geriatrische evaluatie van het VznkuL is om een
standaard
geriatrische
evaluatie
te
ontwikkelen
en
in
de
geriatrische
zorg
te
implementeren. Voor de geriatrische syndromen, die door de evaluatie aan het licht komen, zullen eveneens interventieprotocollen ontwikkeld en uitgerold worden. De introductie van de geriatrische evaluatie en protocollen en de effecten hiervan op relevante uitkomsten voor geriatrische patiënten zullen opgevolgd worden door proces- en uitkomstindicatoren die door de werkgroep bepaald worden. De kick-off meeting heeft plaatsgevonden op 17 januari 2022 met de geriaters verbonden aan de VznkuL ziekenhuizen en AZ Alma. Hieruit ontstond een kerngroep die het voorbereidende werk op zich neemt.

PROSTAATKANKER
Om een objectief beeld te vormen over de functionele en oncologische resultaten van de verschillende prostaatkankerbehandelingen, willen we in verschillende Vlaamse centra real-world data verzamelen door implementatie van elektronische case report forms (eCRFs) en elektronische vragenlijsten voor het bepalen van Patient Reported Outcomes (PROMs) in de routine praktijk. Deze registratie werd al uitgebouwd in UZ Leuven, az groeninge, az Sint-Blasius en AZ Sint-Jan BruggeOostende en staat intussen open voor geïnteresseerde leden van het VznkuL. Het
doel
van
dit
kwaliteitsindicatoren
project te
is
meten
om
de
en
op
geautomatiseerde wijze terug te koppelen aan de
artsen/centra.
Tijdens
de
halfjaarlijkse
meetings worden inzichten en analyses met elkaar gedeeld. De deelnemende urologen krijgen dan ook toegang tot een up-to-date dashboard
met
interactieve
kwaliteitsindicatoren.

LOPENDE WERKGROEPEN
BELEIDSINFORMATIE De leden van de werkgroep beleidsinformatie valideren onze VZN-BI webtool waarin de MZG 2021 werd bijgevoegd. Ze houden tevens de vinger aan de pols bij projecten van andere werkgroepen zoals daar zijn: •

In het najaar richten we een ad hoc werkgroep select*plus op voor de nexuzziekenhuizen die ervaring willen uitwisselen hoe data te exporteren, verwerken en analyseren via deze tool.
VALPREVENTIE De werkgroep valpreventie komt terug op vaste momenten samen onder het voorzitterschap van Andy Swennen om kennis te delen over uiteenlopende thema’s: alternatieve fixatiemiddelen voor 3en 5-puntsfixaties, inhoud functie consulent vallen-delier-agressie-fixatie in UZ Leuven, uitwisselen ideeën week van de valpreventie, het nut van valrisicoscreening, communicatie valincidenten. Jan Yperman stelde hun boodschappenrail voor. Dit is een mooie manier om over o.a. het valrisico van een patiënt te communiceren met medewerkers en bezoekers. De vergadering in mei ging door in de Faculty Club Leuven met een lunch. Tijdens de lunch werd nog verder gediscussieerd over hoe te communiceren rond valrisico. Tevens bekeken we de mogelijkheid om in het najaar externe sprekers uit te nodigen.

PIJN Ook de werkgroep pijn komt na de coronacrisis terug meer samen. Ze bespraken hun b e n c h m a r k met de gegevens tot en met 2021, zochten naar een vereenvoudiging voor het algoritme pijnbevraging in het elektronisch patiëntendossier. Het OLV Aalst-Asse-Ninove startte in het najaar met het verder optimaliseren van het p i j n b e l e i d o p s p o e d . Liesbeth Van Uytvanghe deelde hun ervaring op de werkgroep. 21 juni komen ze terug samen voor hun jaarlijkse lunch in de Faculty Club te Leuven waar de resultaten van de benchmark met gegevens van de eerste maanden van 2022 zullen bekeken worden. Hun belangrijkste indicator is het aantal herevaluaties binnen het uur na een pijnbehandeling wegens een hoge pijnscore (>3) en hoeveel van deze patiënten toch nog een hoge pijnscore aangeven.
DECUBITUS
Met een kerngroep werd onze indicatorenfiche aangepast zodat deze meer in overeenstemming is met de registratie van decubitus voor het Vlaams Instituut voor Kwaliteit van zorg. Zo dienen onze leden geen twee aparte registraties in te dienen. In het najaar zullen onze extra indicatoren rond preventie bijgewerkt worden aan de nieuwste richtlijnen. Op 2 mei werd hun werk ter goedkeuring voorgelegd aan de voltallige werkgroep. We starten met onze nieuwe invoertool voor de registratie in juni, gelijk met deze van VIKZ. Op de externe audit voor decubitus van VIKZ kwamen dermate bezorgdheden van onze leden, dat we

b e l a n g e n v e r d e d i g i n g dienden op te starten. Dit heeft ertoe geleid dat VIKZ de externe audit uitstelt naar het najaar en ondertussen met ons in overleg gaat om deze meer haalbaar te maken voor de leden. We willen met hen tevens een algemener kader ontwikkelen rond externe audits dat ook bruikbaar is voor andere indicatoren.

BEROERTEZORG De klinische werkgroep beroertezorg besprak in januari de stand van zaken van het B I C 4 S T R O K E p r o j e c t van het Netwerk Klinische Paden van KU Leuven, waar veel van onze leden aan participeren. We bespraken tevens de eerste resultaten van het t r o m b e c t o m i e r e g i s t e r en de stand van zaken van de piloot voor het uitrollen van het multicenter z o r g p r o g r a m m a b e r o e r t e z o r g met az groeninge. Dit wordt als een positieve samenwerking ervaren. Het doel is dit ook voor onze andere geïnteresseerde leden uit te rollen. Charlotte Lens, PhD, ondersteunt onze leden hierbij.

KWALITEITSINDICATOREN De sinds vorig jaar opgerichte werkgroep kwaliteitsindicatoren deelde b e s t p r a c t i c e s rond de interne communicatie over kwaliteit. Het Jan Yperman Ziekenhuis stelde hun boodschappenrail voor en Olik Sense werd voorgesteld door het OLV Aalst-Asse-Ninove. Men hen zullen we een aantal i n d i c a t o r e n verder uitwerken die als overzicht kunnen gepresenteerd worden op directieniveau en aan de raden van bestuur. De volgende vergaderingen zullen we een indicator verder uitdiepen. We blijven hierbij niet enkel staan bij de definitie en interpretatie van de resultaten, maar zullen tevens handvaten aanreiken om – indien er een rooksignaal is – hoe dit intern in het ziekenhuis aan te pakken. Hierbij wordt nagegaan of we de resultaten van de V l a a m s e P a t i ë n t e n P e i l i n g binnen VznkuL met extra dimensies kunnen vergelijken.

Zij zijn onze kampioen wat betreft het gebruik maken van de blogfunctie op ons intranet. Hierbij kan een lid van het platform op een gestructureerde manier een korte vraag stellen aan de collega’s in de zin van Hoe doen jullie dat? Hoe gaan jullie om met…? De anderen kunnen dan via de ontvangen notificatie klikken op ‘Reageren’ om hun ervaringen, procedures en dergelijke te delen.

DIRECTEURS
De verpleegkundig directeurs polsen op elke vergadering naar de vooruitgang van Magnet4Europe. Ze krijgen tevens een overzicht van de werkzaamheden van de werkgroepen die aan hen rapporteren (decubitus, pijn, valpreventie). Ze gaven ons tevens de opdracht onze procedure belangenverdediging te starten omdat er teveel bezorgdheden waren bij de externe audit voor deelname aan de indicatoren decubitus van VIKZ (zie verder). Hoe omgaan met arbeidsmigratie kwam eveneens aan bod. Ze gaan de patient / care giver ratio behandelen op de volgende vergaderingen.

Op 23 mei 2022 bezocht de werkgroep de Cleanrooms Akwadraat van AZ Turnhout.

HEUP Op de vergadering in maart met diner werd de interactieve app met de benchmarks geëvalueerd door de deelnemende centra. Met de minimale ziekenhuisgegevens (MZG) wordt gekeken naar comorbiditeiten, complicaties, heringrepen, verblijfsduur, tijd tot het operatiekwartier en leeftijd. De app laat toe een aantal correlaties in kaart te brengen. Aangezien we van al onze leden MZG hebben tot en met 2020, zullen we deze app ter beschikking maken voor al onze heupchirurgen.
REVALIDATIE
De werkgroep bekijkt de mogelijkheid om via het elektronisch patiëntendossier de rapportering aan de Vlaamse overheid te automatiseren. De twee multiple sclerose centra gaven een overzicht van de kwaliteitsindicatoren die ze zullen gaan vergelijken om van elkaar te leren. Op de volgende vergadering krijgen de andere revalidatiecentra de mogelijkheid hun indicatoren voor te stellen. Een consensus zal gezocht worden zodat we dit op het niveau van het VznkuL in een webtool kunnen aanbieden. Ook de therapieplanning in het elektronisch patiëntendossier zal aan bod komen.
ZORGPROGRAMMA COMITÉ Dr. Jos Vandekerhof wordt de nieuwe voorzitter van het zorgprogramma comité in opvolging van prof. dr. Stefaan Nijs. Op de vergadering eind juni zal Tiro.health hun data verwerkingsmodel voorstellen, samen met de apps die ze voor verschillende klinische werkgroepen ontwikkeld hebben. Charlotte Lens, PhD, zal een overzicht geven van de verschillende projecten binnen de werkgroep beroertezorg die ze begeleidt: indicatoren CVA, trombectomieregister, BIC4Stroke, zorgprogramma werking met als piloot az groeninge. We bekijken tevens het voorstel tot hervorming van het zorgprogramma niet traumatische aandoeningen van het axiaal skelet.

Op haar vergadering in de Faculty Club te Leuven kwam het Project ‘kostprijs per pathologie’ met de eerste oefening voor het operatiekwartier aan bod. Tevens kregen de leden meer uitleg over het overheidsproject ‘herijking nomenclatuur’ waarbij VznkuL met haar leden zal meewerken aan het in kaart brengen van de praktijkkosten van de medische prestaties.

Wim Tambeur lichtte de prioriteiten toe voor data ondersteuning in aansluiting met het overheidsbeleid. Management Informatie en Rapportering van UZ Leuven had in opdracht van VznkuL het effect van COVID-19 op de activiteiten en outcome van onze leden in kaart gebracht. Dirk De Ridder, onze strategisch directeur, stelde deze voor.

VAN
BESTUUR
Zonder foto’s – met dank aan Dries Renglé – had u misschien gedacht dat dit een grap was: we tekenden met onze raad van bestuur onze toekomstige strategie uit via legoblokjes (Lego® Serious Play®). Dit onder de deskundige begeleiding van Marc Sonnaert van Inthrface. Het resultaat zal de komende maanden verder uitgewerkt worden.

TUMOREN
De installatie van de interactieve app ontwikkeld door Tiro.health werd feestelijk gevierd. Ook urologen die nog niet deelnemen, waren aanwezig. Zij zullen ook participeren aan de klinische studie oppervlakkige blaastumoren. Dit is de structurele agenda van deze werkgroep: • • • • •

ONCOLOGIE
De richtlijn maligne pleuraal mesothelioom werd door prof. dr. Kristiaan Nackaerts voorgesteld met de annotaties om deze aan de Vlaamse situatie aan te passen. Prof. dr. Christophe Dooms, voorzitter van de werkgroep, gaf een overzicht van de patient reported outcomes. Tiro.health lichtte op hun beurt toe hoe zij de werkgroep zouden kunnen ondersteunen om hun indicatoren te visualiseren. Wilt u meer informatie over deze klinische werkgroep? We laten ze binnenkort uitgebreider aan het woord in ons jaarlijks magazine ‘Kwaliteit voor u’.

ZIEKENHUISNOODPLANNING Op 25 mei kwam de werkgroep ziekenhuisnoodplanning een halve dag samen met als thema’s: •

VLERICK
Samen met het Leuvens Instituut voor Gezondheidszorgbeleid gingen we een partnership aan met Vlerick Business School. Edoardo Pena zal in zijn doctoraat voor ons nagaan of het AI model dat in Indië op meer dan 3000 patiëntendossiers werd losgelaten ook met onze gegevens kan nagaan wat nodig is om de kwaliteit van de zorg en de patiëntentevredenheid te voorspellen (Using Machine Learning to Predict Net Promoter Score to Improve Patient Experience). Hij ging in april van start onder begeleiding van Kristof Stouthuysen en Dirk De Ridder.
RONDE
VAN
VLAANDEREN
Het directiecomité ging terug met de Ronde van Vlaanderen van start. We bezochten AZ Diest reeds in het najaar van 2021. In het voorjaar 2022 werden az groeninge, het locoregionale klinische netwerk Plexus, Noorderhart – Mariaziekenhuis, Sint-Franciscusziekenhuis, Sint-Trudo Ziekenhuis, Jessa Ziekenhuis, Jan Yperman Ziekenhuis, AZ Sint-Lucas en az Vesalius bezocht. We kregen hieruit constructieve en interessante input om onze werking te optimaliseren in het steeds wijzigende gezondheidszorglandschap. De andere leden komen in het najaar aan de beurt.

collaborating to improve quality of care at all hospital levels. To achieve this goal, the VznkuL built a collaborative learning community where peers work together to share data and knowledge in over 30 working groups. Quality indicators
(QI)
are
defined,
validated,
measured, reported, analysed and discussed. Based on the results quality improvement projects are initiated. In VznkuL we showed that it is helpful to differentiate between QI for internal and external transparency. A mix of QI for
monitor
hospital-wide
processes.
In
VznkuL for instance hospital mortality and readmission ratios are reported. Pathology specific QI aimed at direct quality improvement actions are measured in clinical working groups. These QI can be adapted quickly to varying clinical circumstances and needs. Benchmarks of both types of QI are reported in a nonanonymous way within the network. The next step is to move towards public reporting for a selected set of QI.
The separation between these two kinds of QI is not absolute, as some pathology specific clinical QI can move towards the accountability group after thorough validation. In VznkuL, we support the idea of using dedicated QI for improvement, to allow care givers to define error prone steps with potential

furthermore believe that after exhaustive validation QI for accountability should be used for
internal

transparent
reporting.

Introduction Inguinal hernias are the most common abdominal wall defect with a 3 to 27% lifetime risk in women and men respectively.(1) Hence it is not surprising inguinal hernia surgery makes for one of the most frequently performed interventions. In Belgium national health insurance data from 2016-2018 report an average of 24510 patients yearly undergoing surgery for this condition, costing the community >9 million euros/year.(2) Despite being abundantly performed, only limited high-quality real-world data are available on the outcome in general and the problem of invalidating chronic pain after hernia repair in particular. The latter can occur in up to 5% of patients, whom are severely affected on a daily basis with a profound impact on quality of life and important personal and socioeconomical consequences.(3, 4) Independent analysis by Deloitte in 2018 reinforced the importance of registries like FLIPR, as they offer invaluable reliable information in a cost effective way while providing evidence to decrease treatment related costs, morbidity and mortality.(5) The FLanders Inguinal and femoral hernia repair Prospective Registry (FLIPR) was launched as the first prospective population-based cohort of groin hernia patients in the Vlaams Ziekenhuisnetwerk KU Leuven and focuses primarily on the incidence, evolution and risk factors for chronic postoperative pain. It registers a multitude of surgical treatment data and patient reported outcome measures (PROMs) allowing for data analysis of primary (chronic pain) and secondary endpoints (surgical site infection, recurrence, sexual function, patient satisfaction, quality of life, anxiety & depression, …). Methods Data collection (ClinicalTrials.gov Identifier: NCT04623580) started from January 1, 2018 onwards after obtaining ethical approval in compliance with the principles of the Helsinki Declaration, Good Clinical Practice, General Data Protection Regulation (GDPR) and all applicable regulatory requirements in each participating hospital. Male and non-pregnant female patients, >18 years old, with primary or recurrent groin (femoral or inguinal) hernias can be included on a voluntary basis after an informed consent is obtained in an elective or acute setting. All pre- and post-operative PROM data are collected using a secure mobile application (https://nexuzhealth.com/nl/) or web-based email link. Patients complete a set of questionnaires pre-operatively and at regular post-operative intervals: day 1-7, week 4, 12, 26, 52 and further annually up to 5 years after surgery. (Fig.1) The questionnaires include the Numeric Rating Scale [NRS](6) to assess for groin or scrotal pain at rest and during daily activities on a scale from 0 to 10, the Carolinas Comfort Scale™ [CCS](7) to assess for quality of life after hernia repair, a 10-point Likert scale to assess patient satisfaction after the hernia repair, the Inguinal Pain Questionnaire [IPQ] to assess pain and its impact on daily life activities(8), Hospital Anxiety and Depression Scale (HADS)(9), Pain Catastrophizing Scale (PCS), Pain related Sexual Function (PSF)(10), Post-Herniorrhaphy Dysejaculation Scale (PHDS) in male patients(11), neuropathic pain questions (DN4)(12) and Post-Inguinal-repair-Questionnaire by telephone (PINQ phone) for scoring recurrence(13).

Surgical details are entered immediately postoperatively via a standardized digital surgical template form. All patient and surgery data are saved automatically in the Electronic Health Record (EHR) of the patient, which is identical in all participating hospitals. (Fig. 2)

Results Data registration is currently supported by 31 surgeons from 14 hospitals in the northern, flemish speaking part of Belgium, encompassing 77% of the Vlaams Ziekenhuisnetwerk KU Leuven. (Fig. 3)

Since January 2018, 2407 patients have been enrolled. 22.8 % of them have provided outcome data more than two years after their index operation. An online dashboard tool with key performance indicators is available to all participating surgeons for quality control and to allow for benchmarking. (Fig. 4)

At regular intervals group meetings with all participating surgeons are organized where interim data analysis is discussed, feedback is provided and discussions can be held with a view to optimizing patient outcomes. Participation in the registry is also rewarded by a certificate of participation and group authorship in publications. In addition we encourage patients to provide feedback with limited free text possibility in the questionnaires.

Discussion The etiology of chronic pain post groin hernia repair remains poorly understood and is believed to be multifactorial involving both the neuropathic and nociceptive pathway. It is likely related to pronounced scar or meshoma formation, peripheral nerve trauma and central sensitization at a later stage. Other suggested risk factors include female gender, genetic predisposition, young age, reoperation, social status, psychological resilience...(14-16) Unfortunately, long-term successful treatment has proven to be challenging requiring a multidisciplinary approach with long-term commitment of surgeon, pain team, radiologist, psychologist and other specialists.(16, 17). Registries are an invaluable source of prospective observational data and complementary to RCT and meta-analyses, which are still considered the gold standard of evidence-based medicine.(18, 19) Since 1992 a number of well-designed registries (Swedish Hernia Registry, Danish Ventral Hernia Database, Herniamed, Club Hernie, EHS Registry (formerly EuraHS), EVEREG, ACHQC (formerly AHSQC)…) have been established. (Table 1) They have proven to be instrumental in answering clinical questions and offer an insight in daily practice routines, due to a mix of high external validity and relevant (patient-reported) outcome measures.(20) Unfortunately registries also come with a number of inherent risks such as the potential for weaknesses in the data collection due to reporting bias and/or interobserver variability. Furthermore selection bias is an important risk as most registries only report on a fraction of operated hernias due to voluntary patient inclusion. Also, the data sets need to be appropriately validated for the investigated parameters. It is therefore important to realise that registries can be affected by several potential confounders. (21) With the expansion of artificial intelligence and machine learning techniques, these big data sets will further reveal their true capacity of harboring a wealth of useful health related patient information and offering an alternative viewpoint to patient care in a dynamic world. To the best of our knowledge, FLIPR is the first prospective registry focusing primarily on the incidence, evolution and risk factors for chronic postoperative pain after groin hernia repair including PROM data by means of a standardized reporting and use of validated questionnaires. These completed questionnaires form an integral part of the patient EHR, allowing for source data verification minimizing the risk for reporting bias and interobserver variability. Interim analysis has so far been able to offer us a snapshot of groin hernia surgery in Flanders revealing a dominance of the laparoscopic approach. Further in-depth analysis will provide invaluable information regarding chronic pain comparing a posterior vs. anterior access. With the more than 2-year follow-up rate being seemingly low at 22.8%, a large number of patients in this young registry are yet to reach this follow-up moment. It is however a focus to keep all stakeholders engaged on the long-term. In order to keep patients motivated to register their PROMs data up to 5 years postoperatively, we are working on different aspects: sending reminders for responding to questionnaires, accelerated implementation of the mobile app for improving interaction with patients, personal mail or phone contact by the CTA, a short videoclip with patient testimonies for use on websites and in the waiting rooms of the participating hospitals, a two-level survey in order to relieve asymptomatic patients postoperatively from a number of questions… We also want to involve patient organizations and share with interested patients an overview of aggregated outcome data of the complete cohort versus their own individual data. We strongly believe all these initiatives will largely improve the interaction between the patient and the rest of the team for the sake of better longterm patient compliance.

A very important point of interest with direct relevance to patient and surgeon engagement is the medicolegal aspect related to patient information, patient privacy and patient consent. On the one hand, we understand and support that all local, national and European regulations need to be followed strictly, but on the other hand these rules should serve and not inhibit patients and doctors/researchers from participating in registries that evaluate the quality of the standard of care without the use of any specific Investigational Medicinal Product or outside a clinical trial setting. Moreover, the need for such real-world data will also become more important and necessary for the industry to comply with the new EU Medical Device Regulation 2017/745 on the post-marketing surveillance of their medical device products, in place since May 2021. Therefore, one could consider to explore in the near future whether under certain circumstances, more generic strategies related to GDPR and use of pseudonymised or anonymous patient data could be embedded largely (and preferably electronically) in a process of general information exchange, shared decision making and consenting between patients and the medical team, and this at the first contact with the health care provider. This strategy should ideally be uniform all over the EU and be as user-friendly as possible. For instance, the introduction of implicit or presumed consent under strict conditions, as is the case in the Belgian law for organ donation, could be investigated.

Conclusion FLIPR is the first prospective registry in Flanders using PROMs after groin hernia surgery with a strong emphasis on patient reported chronic pain, satisfaction and quality of life. The progressive uptake and continued participation of our loyal group of surgeons and patients is encouraging. Nonetheless, a permanent interaction with patients and surgeons is crucial to maintain enthusiasm of all partners and keep the register sustainable on the long-term. Medicolegal aspects related to patient information, patient privacy and patient consent should not be a limiting factor for surgeons or patients to engage in registers evaluating the standard of (surgical) care.
