The first amount of the covered bill is the insured person's accountability before the insurance starts paying. A new deductible applies if the insurer renews it after twelve months or if the insurer needs to have the policy continuously for more than 12 months.

Within the United Healthcare PPO Network and Internationally Outside USA: Once the insured person meets the deductible, the plan pays 100% up to the policy maximum.
