Editor’s comment As I listened to a variety of experts in their respective fields at last month’s combined Independents Day (iDay) and NEG roadshow in London, the main thing that struck me as running through all the different strands was how important it was to think about ‘feelings’. As Andy Clark said when discussing retail strategies, “Style is about how you make people feel”; as Lindsey Small said when discussing staff retention: “Run regular team meetings so your staff feel included”; as Dean Waugh suggested when discussing retail design: “Think about how customers feel when they walk into your practice”.

At the risk of sounding overly ‘hygge’, if something doesn’t feel right – whether it be the conversations you are having (or indeed not having), the physical environment you are in, or the service and/or product you are receiving – then the likelihood is you won’t be back and you most certainly won’t be recommending the experience to your friends and family. So, it’s certainly worth thinking holistically as to how your practice makes not just your patients feel – but your staff too.

I’ll be sharing more pearls of wisdom from the iDay/NEG roadshow in next month’s issue for those who were unable to attend. Until then, turn to page 21 for some useful insights from Julian Wiles into how to prescribe the best spectacle lens solution for your presbyopic patients; see and hear about some of the fantastic eyewear ‘works of art’ on show at this year’s Silmo in Joan Grady’s Style Spotlight (pages 24-25); and hear some sage advice from sight loss entrepreneur Daniel Williams on supporting patients facing the tough news that they are no longer able to drive due to vision loss (page 19).

Independent practitioners gathered together last month for the NEG/Proven Track Record (PTR) Independents Day roadshow events, which were held in London, Coventry and Bolton. Running in the afternoon and evening for the first time, the CET meetings attracted delegates from across the UK – keen to find out how they could balance the clinical demands of a busy practice and the commercial demands of a profitable High Street business. With the theme of ‘Balancing eyecare and eyewear’, the roadshow featured lectures from expert business speakers and breakout sessions along with parallel CET workshops and a roundtable discussion giving delegates a chance to share their views too.

Full programme of CET events available

Heidelberg Engineering UK’s 2019 education schedule will see the return of its popular nationwide OCT roadshow series – with delegates learning how to use OCT parameters to assess a patient’s risk of glaucoma. In addition, CET courses will run at its Academy Training Centre, including ‘Hands-on OCT operator’, ‘Glaucoma evaluation’ and ‘OCT operation and interpretation’. The company will also be offering CET at 100% Optical in January when Andrew Tatham, consultant ophthalmologist, presents a CET accredited lecture on using OCT to manage glaucoma patients in the primary care setting. The annual British Contact Lens Association (BCLA) Pioneers Lecture, to be delivered this year by associate professor Jennifer Craig from the New Zealand National Eye Centre at the University of Auckland, will explore new studies and emerging data to shed new light on meibomian gland dysfunction (MGD). The Pioneers Lecture is the highlight of the BCLA Visionaries meeting, being held for the first time in conjunction with the Medical Ocular Surface and Contact Lens Association, on 30 November in London.

Entitled ‘In the blink of an eye: exploring drivers of MGD development’ the lecture will examine a series of age and environmentallycontrolled cross-sectional studies that compare the ocular surface and tear film between the South East Asian eye (where MGD is more common) and the Caucasian eye. Differences noted in childhood, young adulthood and older age are consolidated and hypotheses regarding possible key driving factors that might be contributing to the development of MGD will be discussed. Daniel Williams, founder of Cardiff based Visualise Training and Consultancy and author of this month’s guest Developing Thoughts column (page 19), was named one of the most influential people with a disability in the UK at a reception at the South Bank Centre last month. The Shaw Trust Disability Power 100 List is an annual publication of the 100 most influential disabled people in the UK, and more than 700 nominations were received for the 100 places.

Daniel said: “I’m delighted to be included in the Shaw Trust Disability Power 100 List at such an early stage in my career and accept this accolade on behalf of the two million people in the UK with sight loss. At the beginning, sight loss feels like the end but later you realise it’s just the start of seeing in a different way and having a new outlook on life. Ideas for improving customer service, driving patient loyalty and increasing patient retention were shared at the first Glasklar Hub meeting for independents organised by the brand’s distributor, Positive Impact. The free inaugural education event in London was themed, ‘See you again soon’, and the guest speaker was dispensing optician, Dan Scott. Dan, who worked his way up from lab technician and spent numerous years honing his craft in the family practice and other practices, told delegates: “The fitting appointment is the most valuable appointment you can make,” he told delegates in his presentation, ‘Making spectacle collections memorable experiences’.

Dan explained if done well, that one appointment could result in additional spectacles and accessories sales, improve customer retention and increase referrals.

Due to the success of the event, which included an interactive workshop on patient referrals and a presentation on customer loyalty by PI’s Nick Atkins, PI is looking to run more around the country in 2019.

This year the AOP has joined forces with the British Academy of Audiology as part of a new initiative to enhance the Audiology Hub, developing a wide-ranging CET programme of sessions. The programme is tailored to bring a holistic approach to integrating the service into practice; sessions offer practical guidance on how best to communicate with patients with hearing impairments. The British and Irish Orthoptic Society is also running a series of seminars dedicated to orthoptists.

Marchon Eyewear took home two trophies at the annual Silmo d’Or awards held at the Musee des Arts Forains in Paris last month. Marni Eyewear won the Fashion Trend Optic Frame category for a new style (ME2623), a rectangular design in a distinctive mix of contrasting colour blocked acetates. The style is created through a handmade process that bonds the two front layers together.

Voting has begun for the Association of Optometrists’ (AOP) annual awards, which will take place during 100% Optical in January. Eleven categories are now open to

AOP chief executive, Henrietta Alderman, said: “All our finalists have demonstrated exceptional passion and professionalism to get this far. We are delighted that in 2019 the awards will also recognise the essential contributions of locum optometrists, and the unique role of optical practices incorporating hearing care.” Voting closes via the AOP website on 12 December.

20 Optometry Scotland New terms for General Ophthalmic Services (GOS) came into force in Scotland on 1 October, following negotiations between the Scottish government and Optometry Scotland (OS). The updates build on the recommendations made in the Community Eyecare Services Review report published in April 2017, more than 10 years after the introduction of free NHS-funded eye examinations in Scotland.

The new arrangements give optometrists and ophthalmic medical practitioners more freedom to make decisions based on individual patient needs, moving away from a ‘tick box’ approach and offering more person-centred care, said OS. The organisation has also negotiated a fee of £38 for the newly introduced enhanced supplementary eye examination (ESEE) where

a practitioner considers dilation clinically necessary. This is higher than the SEE fee of £24.50 where the patient is not dilated.

“This ESEE fee now recognises the significant time needed by community eyecare professionals to deliver a dilated supplementary examination,” OS added.

23 British Contact Lens Association A “seismic shift” in the way practices do business should be embraced rather than feared – with new technology offering practitioners the opportunity to be eyecare pioneers. That was the key message to emerge from BCLA Asia – a two-day conference held in Singapore, which saw more than 500 delegates take part in a series of hands-on workshops and hear presentations from experts from across the world.

The conference, titled ‘Surviving practice change’, examined the nature of a rapidly changing industry and how business owners can adapt to overcome the commercial challenges that presents. BCLA chief Collaborative education event in Singapore executive, Cheryl Donnelly, said: “Technology is advancing fast, both in terms of instrumentation, techniques and products available for eyecare professionals to enhance the life of our patients. It is, and will, represent a seismic shift in day-to-day professional life. BCLA Asia was a platform to provide the tools for practitioners to embrace this change to deliver the best eyecare for patients in times of commercial challenge and best position themselves for future success.”

AN INDEPENDENT VIEW Reaching out to students The future life blood of the independent sector can be found in universities across the length and breadth of the country. The sad fact, however, is that the overwhelming majority of students do not know that independent practice is a real and practical career option.

The multiples are so very good at getting to students at an increasingly early stage of their studies, that the independent sector has a huge challenge to gain space of mind.

The AIO has for several years staged evening events at many universities to try to get the independent message across. Many of these events have been well attended; it is surprising what some free beer and pizza can achieve!

Those presenting on behalf of the AIO have found huge interest in the independent sector from the students, but until now there has been nowhere for them to go to get practical help and advice to either set up or acquire a practice. The AIO Practice Support Network (PSN) has been created exactly for that purpose.

The challenge now is to reach the students, and to make them aware of the PSN as the route into independent practice. The AIO is working with an expert in this field on a strategy that is designed to engage with students from their first year at university, through their preregistration stage and indeed post-graduation into the workplace.

This will be a major logistical exercise and the AIO will be seeking assistance from across its membership base – and beyond – for help in delivering the strategy and, in particular, engaging directly with the students at specially organised events.

Developing thoughts In our guest slot this month, Daniel Williams explains how best to support patients who can no longer drive because of poor vision

Driving loss support For many of us, learning to drive is one of life’s rites of passage. It represents a stepping-stone into adulthood, and a passport to independence we all too quickly take for granted. How it might feel to have that autonomy taken away from us is something to which we barely give a thought; yet for many people in the UK with low vision it will become an all too painful reality.

Last year in the UK, the DVLA revoked the licences of more than 9,000 drivers because of poor vision – an increase of more than 2,000 since 2012. These numbers are set to rise as Britain grapples with the impact of its ageing population and diabetes.

FIRST PORT OF CALL The responsibility for informing patients their eyesight no longer conforms to the requirements for holding a UK driving licence often falls to optometrists. In this situation, many understand their role as simply to refer these patients to ophthalmologists within larger hospital units. But what more can be done to facilitate this often emotional and sometimes traumatic period of adjustment?

SEEING THE LARGER PICTURE Firstly, as eyecare professionals you need to understand and be empathetic to just what a wrench the loss of a licence can be for drivers. Many will worry about becoming a burden and dependent on others, and this fear will often be compounded by fears of further deterioration in their sight. For others, who perhaps live in remote areas or who depend upon a car for work, there will be fears about the very real impacts on their lives and livelihoods that not being able to drive may have.

BEING AWARE OF REFERRAL SERVICES Beyond demonstrating a sensitivity to your patient’s concerns, you need to be fully

informed about appropriate referral services, so that you can seamlessly connect them to local and national sight loss organisations such as the RNIB, Sightline, local sensory impairment teams, local sight loss charities, eye clinic liaison officers (ECLOs) and the Macular Society. These organisations will have trained advisors able to talk patients through the practical adjustments they may need to make, and to help them with the emotional issues they need to process.

PRACTICAL SUPPORT AND ADVICE Part of the sight loss organisation’s job will be to provide practical mobility advice to minimise the impact of no longer being able to drive. This may be as simple and straightforward as how to access public transport in the local area, or ride-sharing apps such as Uber. It may also involve providing information on the financial assistance available to cover additional transportation costs. They will provide advice and guidance for these patients surrounding things like Access to Work, welfare benefits, emotional support, peer support groups, etc.

THINKING OUTSIDE OF THE BOX They will also be able to offer lots of practical support on low vision issues less directly related to physical mobility, but which may be of equal importance after a low vision

diagnosis. Many patients, for example, will not be aware of additional support they may require and be entitled to. This may include vision rehabilitation workers or modifications to living spaces that enable them to continue to live independent lives.

EMOTIONAL SUPPORT An ECLO’s role is to offer so much more than simply practical advice, of course. Advisors will be trained to provide the emotional support that many patients experiencing low vision, and fearful of the consequences of further deterioration in their sight, will require. Many organisations will have sensory teams specialising in a combination of practical support and will be able to offer the sorts of holistic rehabilitation services that are most beneficial to patients. This combination of informed assistance and emotional support allows patients to regain confidence along with their independence.

In a perfect world…

“Why do we compensate?” asks Julian Wiles, managing director of Performance Lenses, as he explains how to avoid the incidence of varifocal non-tolerances

Frustrating, isn’t it? You send us this: R -7.00 -1.25 x 85 L -6.50 -1.50 x 87 And we send your lenses back with these weird numbers: R -6.856 -1.121 x 88 L -6.378 -1.445 x 89 The difference is because the lenses have been compensated for back vertex diameter (BVD), wrap and tilt. We have put our full computing power into designing a lens that best matches the prescription in the patient’s ‘as worn’ position. It’s important to remember we are talking about changing the lens design. We are not changing the patient’s prescription. Here’s why…

EVERYTHING IS DIFFERENT An accurate refraction is still crucial. That hasn’t changed, nor will it. Up to the year 2000, the only measurements you needed to take were the monocular PD and the fitting heights. Today, progressive lenses and the machines that produce them are not the same as they once were. They have got better and so has the computing power.

Everything is different, everything that is except the accuracy of the refraction. After 2000, measuring accurately for BVD, tilt, wrap (and depth and width of eye shape) became critical (Figure 1).

Regardless of whether you use a trial frame or a phoropter, this routine has to remain.

This is how you continually produce consistent, accurate prescriptions. For the consulting room, the lens set must be ‘neutral’: BVD, tilt and wrap, which are fixed in the vertical and horizontal planes, and very unlike eyewear. This is exactly as it should be. Optometrists must be able to get the same perfect, accurate results, time and time again, wherever they work.

In a perfect world, you would be able to dispense spectacles that match your refraction set-up. Now, what happens when your perfect world meets the patient’s real world? People now change (some might

say, spoil) your accurate refraction simply by choosing a frame. Your perfect world has now changed. And so has the dynamic.

Your patient’s real world is different from your perfect world. Patients are more concerned with fashion, cost and value for money. Aren’t you the same when you buy something? The patient’s frame choice changes everything. There are mathematically provable formulas. The power of your perfect lens is perceived (by the human brain) to be different because of the BVD, the tilt and the wrap. The wearer’s perceived power changes.

Different BVDs will result in different perceived powers. Tilt induces power changes in the 90 meridian and wrap induces power changes in the 180 meridian.

Lens base curve selection (at the lab) is important because for each prescribed power there is an ideal base curve. Base curves slightly outside, or significantly outside, the ideal will diminish off axis visual acuity. Figure 2 overleaf shows the difference pantoscopic tilt can make to the resulting lens performance.

If their reality is that they can’t ‘see clearly’ who do you think will receive the complaint?

If you are lucky, they’ll tell you. And you will re-refract and discover you are right. If you are unlucky, they will tell their friends and not tell you. They might even vote with their feet.

We didn’t change the prescription, we changed the lens design. The human brain will still see your original, accurate refraction.

R -7.00 -1.25 x 85 L -6.50 -1.50 x 87 We ‘see’ with our brain. The compensated powers have one effect only: to make the wearer’s experiences of the new lenses as good as they can be, moving the image point so that when used with the eye the image falls on the retina giving great vision.

CHOOSING THE RIGHT FREEFORM So now you understand why, here’s the how. Here is a proven quick and simple way to choose the right freeform varifocal lens design for your patients, every time.

Today there is no longer a simple ‘go to lens’. Once upon a time, you trained with a lens that had four or five fixed base curves, a fixed inset, a fixed corridor length and a fixed design so it made perfect sense to find the ‘one that worked best’. Today it is quite possible that lens is no longer available.

You had to change. Your patients have.

The proven, quick and simple rule is to let the numbers decide. Because today you have to take all the measurements: monocular PDs, heights, BVD, tilt and wrap angle along with the frames A&B measurements. After all, you are producing a bespoke item.

You now have to choose the lens that best suits the patient, their prescription and their needs. It says so in your competencies.

Here’s the proven way on how to choose the perfect freeform lens for your patient.

Minus powers: the near-sighted patient can read, thread a needle, do close work without any difficulty. But they need help with their distance vision. So you recommend a design that favours the distance. They’ll love you for it.

Plus powers: the far-sighted patient can see the road signs, they can see the birds in

the tree. But they need help with reading or threading the needle. Eyecare Dual with Camber Technology really helps here.

People come to you with a variety of needs.

New wearer: maybe your patient has a low add or a low prescription (below 1.50?).

Maybe they are already wearing bifocals, trifocals, or occupational lenses which are no longer suitable. You recommend a progressive lens, with a soft design, with a low amount of astigmatism in the nasal and temporal areas. This will help them adapt to their new lenses more easily.

Balanced lens: maybe you want to switch from the high-priced branded product/discontinued varifocal to your own ‘house brand’? A ‘balanced’ lens will be a good place to start. You can choose hard or soft designs if you wish. A ‘short’ corridor.

You can recommend PPLs that ‘work’ all the way down to 11mm.

Mobile devices/occupational lenses: very specific needs for near and intermediate demands both in single vision and progressive lenses. A ‘Smart Add’ will help with those near tasks. We hold our heads differently when reading or viewing a mobile device, so we need a different solution. Smart Add technology gives you the full add power sooner and much wider than conventional add designs.

Sport: tell us what sport the lenses are needed for and we will recommend a suitable design. Trilogy, Transitions, Transitions XTRActive, NuPolar or DriveWear.

No problem.

Driving: is a specialist daytime task with additional demands for night driving. Night myopia is a real challenge, but both need a sympathetic design. InMotion has a 70 per cent wider distance zone and is 45 per cent wider in the intermediate with 15 per cent

High Adds (up to +4.50D)/high plus: Eyecare Dual with Camber Steady Technology induces a little bit of minus into the nasal and temporal areas of distortion that all progressive lens wearers will experience.

This has the effect of reducing ‘swim and sway’ to an absolute minimum, for faster adaption. It will still take a day or so to adapt, but the clinical trials yield spectacular results. The number one complaint for varifocal non-tolerances is a poor reading area. Thanks to the unique Camber variable base curve and the Eyecare Dual with Camber Steady Technology software, you can offer your patients the best possible reading area for many, many years.

Then you need to consider the availability of NuPolar, Transitions, DriveWear and XTRActive lenses. You also need to be able to glaze any ophthalmic frame (rimless, sports, inline metals, semi-rimless), so make sure your lens of choice includes Trilogy made with Trivex. It makes sense to make sure that your ‘house brand’ can do all that. It makes sense to have an owner/director of the lens supplier on speed dial, just in case you need information fast.

And remember, you have two huge progressive lens markets to aim for. Firstly, the existing, experienced wearers (+1.75 to +2.50), who will live longer and demand more and whose reading area will diminish with time. It makes sense to maximise the reading zone. Done correctly you will look after these patients, their families and friends, for 30 to 40 years or more.

Secondly, the new wearer whose experiences and demands have been fashioned by the huge tech advances we have seen recently, but with which they have grown up. It’s normal for them. They don’t need to know what we did. They won’t want to know why things can’t be done the way they want it.

They’ll want you to say ‘yes’ and they’ll pay the price, if it’s right for them, not for you.

SUMMARY The main point to remember is to embrace compensated lenses. Engage with your patients by explaining the measurements, asking them lifestyle questions and work with them to choose suitable frames. A combination of these strategies means your patients will end up with clear comfortable, vision set in a comfortable frame that they enjoy wearing.
