Hair loss affects millions of people across the world and can dent self-confidence and cause enormous psychological stress that negatively impacts our lives. As many as 40 – 50% of men will experience hair loss of some degree by the time they reach their 40s, but it can manifest itself from as early as your late teens. The condition is slightly less common in women, but still prevalent. In many cases with women, it is caused by hormonal changes in the body, such as those experienced during menopause or childbirth, although genetics may also play a part. 1 WHAT IS HAIR LOSS AND WHO DOES IT AFFECT? Hair loss is an incredibly common concern and can affect both men and women. On average, the scalp has 100,000 hairs that cycle through periods of growing, resting, falling out, and regenerating. There are many types of hair loss conditions including Telogen Effluvium, Hypertrichosis, Folliculitis Decalvans and Cicatricial Alopecia to name but a few. The most common condition, however, is androgenetic alopecia, also referred to as male pattern balding or female pattern balding. Unlike some other types of hair loss that are triggered by external factors such as illness or stress, thinning caused by androgenetic alopecia is a permanent, chronic condition determined by family genetics.

2 WHAT DO YOU NEED TO KNOW ABOUT THE HAIR CYCLE? Most people on average lose between 50 to 100 hairs a day. Hair loss/growth occurs in cycles. Around 85% to 90% of your hairs will usually be in the growth or “Anagen” phase at any one time. Up to 5% can be in the transition or “Catagen” phase. And around 10% can be in the resting/shedding or “Telogen” phase. In a healthy hair cycle, the latter phase means they are making way for new ones to come through. Hair cycles move slowly. The impact on your hair fall from severe stress is typically delayed. Therefore, it may some weeks or even several months may have passed before shedding manifests. That may make it even more challenging to identify.

3 WHAT IS TELOGEN EFFLUVIUM?
Telogen Effluvium (TE) is a temporary condition in which external factors disrupt the hair cycle. Itcan lead to as many as 50% of hairs entering the shedding phase at once, which results in rapid hair fall. Triggers can include severe emotional stress, illness or high fever, physical shock (causedby an accident or surgery, for example), sudden weight loss, hormonal changes and vitamin deficiencies caused by diet. TE is the second most common type of hair loss. Unlike the most common variety, the geneticcondition known as androgenetic alopecia, or “pattern baldness”, TE hair fall does not follow any distinguishing pattern as such. It occurs all over the scalp and often falls out in patches. But ifthere is one comforting prospect to all this: the hair cycle usually corrects itself over time and hair regrows.

HAVE YOU HEARD OF THE COVID-19 HAIR LOSS EFFECT?
According to dermatologists in the UK and US, there has been an upsurge in patients reporting excessive hair shedding since the pandemic hit its peak. The positive news is that the condition may be temporary, and COVID-19 could be the culprit. If increasedhair fall is worrying you, then let’s look at what might be happening and what you cando about it. Understandably, the COVID-19 pandemic is being heavily linked to a rise in TE cases. In some instances, it may be an actual symptom of the disease itself. Hair loss itselfis not a common symptom of COVID-19 but it is a known symptom now, thanks to research. It generally afflicts people who have been hit hard by the illness and have been undergoing a slow recovery. If you’ve had COVID-19 and are experiencing hair loss, it’s very possible that it is a direct consequence. Your GP should be able to provide further information. The more common link between TE and COVID-19 is situational and psychological. With job losses and financial worries, threatsto our health, and enforced periods of self-isolation, emotional anxieties have beenvast and wide-reaching. You may be worriedabout a vulnerable relative or your own health, for example, or you may be struggling to cope mentally with the social restrictions that have been imposed since mid-March.

It’s a toughtime. Naturally, all these factors can put enormous strain on a person’s mental wellbeing, and this in turn is a common onset of TE. The rise in cases of hair loss since April/May 2020 has been such that even the national press has picked up on the trend. You certainly don’t need to have had COVID-19 for your health to be compromised. However, hair loss and hair growth remain complicated. Other factors could be at play, too, which may not even be TE at all. Firstly, there is “seasonal shedding”. For reasons not entirely known, spring seems to be a period in which hair enters a heightened shedding phase for many. For others,seasonal shedding may occur more prominently elsewhere during the year. But we all experience it. Secondly, if you’re not shampooing or styling your hair as regularly as you were prior to lockdown,then it’s inevitable you will notice more hairs when washing and rinsing. This is even truer when conditioner is applied, which de-tangles and smooths hair. Remember that the average person loses up to 100 hairs per day and the bulk of these fall out when we wash our hair, so they can certainly accumulate.

TRANSPLANTS One of the biggest conundrums faced by people with hair loss is the number of potions and treatments on the market – or in development – all promising a miracle cure. Transplants have become a popular and effective solution, however, despite some great results, they do not always yield the same hair density that you may once had, and don't account for future hair loss, meaning that the surgical procedure may need to be undertaken multiple times. The first surgical hair transplant took place in the 1950s, but the treatment has come a long way since then. The surgical hair transplant industry has exploded over the last 10-15 years, with advances in medical technology, transplant techniques have greatly improved since the 1980s and 1990s. Many celebrities including Robbie Williams and Wayne Rooney have confessed to having such procedures. It appears that, as male grooming and image maintenance has become increasingly popular, hair transplants have become less of a taboo. In simple terms, a hair transplant involves a surgeon taking healthy hair follicles from the back of the scalp – where the hairs aren’t susceptible to hair loss – and replanting them in the thinning areas at the front of the scalp. Eventually, these follicles grow thick, new hair and patients see results between one year and 18 months after the treatment.

There are two common types of hair transplant: The Follicular Unit Transplantation (FUT) method – or “strip” method – involves the surgeon removing a large strip of skin from the back of the head. He or she then extracts hair grafts, which contain groups of hairs, to be distributed into the problem areas. The beauty of this method is that it is faster, cheaper, and allows for larger sessions. However, it does result in some scarring at the back of the head from where the strip was taken. The other popular method is Follicular Unit Extraction (FUE). In this instance, the doctor extracts hairs individually from the back of the head and replants them in the treated areas one-by-one. It is excellent for procedures where total precision is required, for example, if you want to shape the hairline to perfection, or for filling in a small area. However, it is not usually suitable for larger sections. Hair transplants should work for everybody providing results consistently, regardless of the patient; unlike medications, where some people respond better than others.
