t’s a fact, after the age of 50 muscle mass starts to decline. How much? Is there a way to minimize the loss? Rate of muscle loss ranges between 1-2% per year after the age of 50 and strength loss 1.7-2.5% per year after 65. If you’re interested in living a long prosperous healthy life and playing with your grandkids and greatgrandkids someday, strength and muscle mass are essential. It’s important to establish a fitness routine that incorporates strength as early as possible. If you’re already in your 50’s and 60’s

it’s not too late. Surprisingly, you don’t have to become a gym rat to slow down the rate of muscle loss. Strength training 2x’s per week for 30 minutes each session has shown to be effective for maintaining strength and muscle mass. Grip Strength A recent study from 2015 shows that a powerful grip correlates to longevity. Grip

strength can predict overall strength, health, and your risk of cardiovascular disease. The stronger your grip, the more likely you are to survive diseases like cancer. For every 11-pound decrease in grip strength, there is a 16 percent higher risk of death from any cause. Preserving Strength - a Balanced Fitness Approach

A full body strength program performing “push, pull, squat,” movements is a great foundation for maintaining muscle mass. These movements stimulate the “big” muscles that burn the most calories and stimulate the most muscle fibers. Other activities such as “walking” are great ways to stay active, BUT walking is not going to preserve precious muscle or improve mobility. can hear him now. Like all good grandfathers, mine, imparting wisdom to young, eager ears over cinnamon rolls and cold cereal at the breakfast table. He was an English teacher in Iowa before he and my grandmother moved to Oregon, where he later became a banker. His brother, a farmer. My grandma, the church pianist. A Morning with My Grandparents And so, it was another morning of juice and coffee splashed with lessons on language, salvation, pigs, and piggy banks. As usual, I asked for a banana on my Cheerios; my grandmother, oddly, sliced it with a spoon. And thinking on it now, they were probably the offbrand O’s. “Take this banana, for instance,” holding the yellow fruit to his ear like Adam West picking up the Bat Phone, “I don’t understand why anyone in their right mind would pay a few extra cents for organic bananas! Who wants bugs in their fruit anyway? Ludicrous.” I sipped my 6

grape juice, imagining worms squirming in an apple core. “Matter of fact, when I was a kid, we didn’t have enough money to buy juice.” My grandmother, the pragmatist, “Don’t drink too much, juice will go straight to your thighs.” Amen. Grown Up Off into the world I went, trying to keep my piggy bank plump and my thighs not. I still skip the juice, but my understanding of economics and health has grown to be much more wellrounded. I also know my grandparents were simply sharing what they thought was best, in hopes that I would have a big, beautiful future. And I have, in part, because of them. And like many grandfathers, mine, adheres to the notions that have served him well over his ninety-five years. kagit Regional Health offers two options to help make receiving care more convenient. MyEClinic and MyChart Video Visits are both available to bring patients the care they need, but there are differences. Try MyEClinic when getting to an Urgent Care isn’t convenient, and opt for a MyChart Video Visit for a scheduled appointment. MyEClinic MyEClinic is available to anyone in Washington without an appointment for a $40 flat rate. With MyEClinic, patients have 24/7 access to a team of board-certified doctors and nurse practitioners for consultation, diagnosis and treatment through computer video chat or over the phone without an appointment. MyEClinic providers treat a variety of minor illnesses and common conditions, such as pink eye, rash, urinary tract infections, sinus symptoms and more. Providers are also able to prescribe medications when appropriate. MyEClinic visits MyEClinic visits typically last about 20 minutes. If you are signed up for MyChart with Skagit Regional Health, you will receive a summary of the visit that is accessible via the patient’s online account. MyEClinic staff can even help patients find a new doctor for future 10

Palliative care is an interdisciplinary medical approach to helping people living with any serious or debilitating illness like cancer, congestive heart failure, lung disease, kidney failure, liver disorder, Parkinson’s disease, Alzheimer’s disease, or other forms of dementia. Palliative care focuses on providing relief from the symptoms and stress with information, support, and resources to improve the overall quality of life for both the patient and their family. Facing a long-term serious illness is not only challenging for the patient but can be very stressful for their family and caregivers. Most people don’t understand the wide range of services available with Palliative care and when to ask for their help.

Patients with terminal illness who engage the Palliative Care team early, generally have an easier journey with better outcomes, especially when transitioning into Hospice Care. ~ PC Cable, RN What is Hospice Care? Hospice teams provide family-centered, holistic, and compassionate end-of-life care for people with terminal illness. They help manage pain and symptoms, provide medication, and supplies, as 12

well as emotional support and respite for caregivers. Hospice care is appropriate when the patient, patient’s family, or patient’s physician determine the goal is comfort and quality of life and no further tests, procedures, or treatments. At WhidbeyHealth, our Hospice Care team works with each patient to determine their goals. Are they looking for quality of life or length of life?

Advances In Vision Correction Northwest Eye Surgeons

W

e all know our bodies change as we age, and so do our eyes. Cataracts occur when the lens in the eye becomes cloudy and less flexible, causing blurry vision, trouble driving at night or difficulty reading and doing hobbies. Surgery is the next step when glasses can no longer correct the problem. Cataract surgery replaces the clouded lens with a new lens, and also offers the opportunity to correct other vision problems. New technologies in cataract surgery can customize vision correction in aging eyes, giving patients options to see how they want to see, and can reduce dependence on glasses. Light Adjustable Lens™ (LAL) by RxSight® For the first time ever, this technology gives surgeons the ability to customize and optimize a lens that has already been implanted. This adjustable lens is made of a light-sensitive silicone material. Doctors refine vision after surgery by briefly exposing the lens to ultraviolet (UV) light. While the eye is healing, patients do need to wear special UV-blocking glasses but only until the doctor makes the last adjustment that “locks” the lens at the desired vision target. This final exposure to UV light causes the molecules in the silicone material to polymerize, or join together in a chain, into a permanent shape. The manufacturer refers to a study of 600 subjects. According to this study, those who received the LAL, followed by light adjustments, were twice as likely to achieve 20/20 vision at 6 months 16

without glasses, compared to those who received a standard monofocal lens. Optiwave Refractive Analysis (ORA) This system uses three cameras on the viewing microscope and sophisticated computer software to take real-time measurements of the eye during surgery. Before Aaron Kuzin, MD surgery, doctors calculate measurements for the lens replacement. ORA provides real-time data during surgery that further guides surgeons in calculating the proper lens strength and surgical measurements to ensure the patient’s desired vision. All Laser Eye Surgery Options for Vision Correction: Laser eye surgery is no longer limited to the familiar refractive procedure known as LASIK. Eye surgeons now may use lasers in cataract surgery to provide more precision, faster healing time, and faster recovery of vision. Femtosecond lasers incorporate advanced imaging to make a 3D image of the eye, create perfect incisions, and break up the cataract in preparation for removal. Doctors may use other types of lasers, like the YAG laser, following cataract surgery if needed. Glossary: Medicare and Medicaid/COPES Medicare: A federal program that provides health insurance coverage for seniors in the U.S. who have paid into the benefit for a certain number of periods. There are different parts of Medicare that individuals can purchase and individuals must choose which plan type they would like. Typically, individuals become eligible at the age of 65, but individuals under the age of 65 with certain disabilities may be eligible. Medicaid/COPES: A state program that provides comprehensive care management and helps pay for personal care, home delivered meals, home health care, adult day services, respite, transportation and other services for older adults in their homes. COPES may also help pay for care in senior housing and care options such as assisted living, memory care, adult family homes, and skilled nursing homes. However, not all care homes are licensed for Medicaid/ COPES. The ones that do accept Medicaid/COPES usually require a minimum of two years private pay before allowing Medicaid/COPES funding.

Glossary: Home Support Agencies

Home Care: Supportive care provided in the home by a professional agency. Certified Caregivers provide assistance with daily care needs and companionship. Services: Home Care agencies provide non-medical services to people with functional limitations, enabling them to maintain their highest level of independence and to age in place in their own homes. Care may include assistance with dressing, bathing, medication administration, meal preparation, household chores, errands, and more. Hours/Days: Scheduled help 2 to 24 hours per day, up to 7 days per week, or as respite care. See Home Care Directory on Page 30 Home Health: In-home services provided include Skilled Nursing Care and various therapies prescribed by a physician. This includes physical, occupational, respiratory and speech therapies, often used for seniors who are recovering after a hospital and/or nursing home stay. See Home Health Directory on Page 31 Home Modifications - Safety & Accessibility: Regular attention to Home Safety and Accessibility is known to decrease fall risks and increase the chances of aging in your own home. Home Analysis for potential improvements should be done by a Home Contractor who is a Certified Aging-In-Place Specialist (CAPS Certified). This person will look at many factors, including: rugs, electrical cords, placement of furniture, and functional supports around the home.

any of us plan to live in our homes for a long time. It’s important to make sure yours is safe for today and tomorrow. It’s never too early to start. Take the time to look around to see if there are places that can reduce danger. Walk through your home, come in from the outdoors and look for areas that can be improved. Safety saves lives, both young and old. Studies show that over 75% of homeowners over the age of 50 want to stay in their homes after retirement. Creating a safe environment is essential for success. One of the top reasons Seniors go to the emergency room is because of unnecessary falls. These accidents can be prevented with safety improvements to the home. Inexpensive fixes for everyday dangers Here are a few inexpensive and quick enhancements to make your home a safer place to live. Tripping Hazards Stacks of books, piles of paper, pet toys and loose cords on the floor create tripping hazards. Small rugs, especially in the kitchen and bathroom are accidents waiting to happen. Getting things off the floor or organizing them in baskets that can be put away can help

eliminate future accidents. Make sure that cords for the tv, computers and floor lamps are out of traffic areas. Don’t use small rugs if possible. Keep Your Home Safe with Good Lighting Reading and hobbies need good lighting. Floor lamps near chairs and work spaces, with cords out of the way, help make reading and tasks easier. Using under cabinet lighting in the kitchen increases visibility for cooking tasks. There are inexpensive and attractive battery operated lights that stick onto walls and cabinets for optimal visibility. Power Up A lot of people don’t know that our household batteries and filters need to be checked regularly. I like to check the batteries in my smoke alarms every 6 months. For example, things like filters for the furnace, dryer and kitchen appliances should be checked according to the manufactures’ schedule. Checking the bulbs on your outdoor lighting can help you be prepared for any visitors. Keep Your Home Safe with Good Communication Make sure that you can contact someone in case of an emergency. These days, technology offers many solutions. Smartphones can be set to respond to a voice command and call a responder. You can ask your iPhone to contact someone by saying “Siri, call Susie”. If you have Amazon’s Echo products, ask Alexa to call a friend on your contact list. (Continued on Page 35)

Reducing Your Risk of Dementia/Memory Loss Teepa Snow, Positive Approach to Care

A

re you worried about getting dementia? Many people do not realize that dementia is not always genetically linked. In fact, the majority of dementia is not hereditary. There are certainly types of dementia that have a genetic component, such as Early-Onset Alzheimer’s and Frontotemporal dementia, and some have a stronger genetic connection than others. However, even for most of the genetically-linked dementias, having a parent with the disease does not guarantee that you will develop it. To find out how to reduce that risk as much as possible, read on.

Exercise to Reduce Risk of Dementia Exercise significantly decreases your risk of getting dementia. Why? When you do aerobic or cardiovascular exercise, the oxygen and blood flow to your brain is increased. This increased flow helps keep your brain’s blood vessels healthy, 44

which in turn keeps your brain cells healthy. Also, exercise helps stimulate the connections between nerve cells in the brain and encourages new brain cells and connections to be made. It is also thought that exercise helps slow the buildup of unhelpful proteins, free radicals, and inflammation in the brain that can lead to cell death and a slowing in brain connections. How much should you exercise? Experts recommend 150 minutes a week of aerobic exercise for ideal benefits. If that seems overwhelming, start with less and work your way up to it.

Food Choices The food choices you make play a large role in dementia risk reduction. Current research shows that diets rich in fruits, vegetables, olive oil, fish, legumes, nuts, and whole grains reduce the risk of developing dementia. Diets high in saturated fat, empty carbohydrates, sugar, red meat, or processed foods increase your risk. One direct reason for this is that increased body weight is correlated with an increased dementia risk. In addition, healthy diets result in less inflammation in the body and brain, which also decreases risk. Some foods actually contain compounds that are believed to protect the brain from decline. Beverage choices matter, too. Consuming larger quantities of alcohol greatly increases your risk. Caffeine in coffee and tea is currently believed to be beneficial to cognitive function, while regular and diet sodas are linked to a decline in function. Drinking plenty of water is always a good idea, as well. Sleep as a Factor of Risk of Dementia Sleep is another key factor to dementia risk reduction. Sleeping six hours or less a night in middle age is linked to an increased risk of dementia. When you sleep, your brain cells shrink slightly, allowing harmful protein accumulations to be flushed away. If you’re not sleeping enough, then proteins and other toxins can build up in your brain, and can eventually lead to cognitive decline. Stress Level Factors This is often easier said than done, but reducing your stress level can decrease your dementia risk. Stress causes the release of the hormone cortisol, which has been linked to decreased cognitive function. Researchers are 46

Smoking Contributes to Risk of Dementia Smoking is a significant dementia risk factor. Many toxins in smoke cause cell damage and inflammation, which can have a harmful effect on your brain. Smoking also causes problems with your blood vessels, including brain blood vessels, which can lead to dementia development. Quitting smoking reduces your dementia risk substantially. Other Medical Conditions High blood pressure, diabetes, stroke, hearing loss, depression, and obesity are all significant risk factors for dementia. It is essential to visit your physician regularly and have any medical conditions treated and monitored. Take medications as prescribed and follow medical recommendations. Focus on What You Can Control Worrying about developing dementia does nothing to decrease your risk. In fact, stressing about it can actually increase your risk. Instead, focus on the dementia risk factors that you can control. Getting regular physical exercise, maintaining social connections, challenging your brain by occasionally learning something new, eating a healthy diet, getting the sleep you need, reducing your stress, quitting smoking, and ensuring that your health is regularly monitored are all part of reducing one’s risk of developing cognitive decline. If it seems like too much, choose one and take baby steps toward your goal. Remember, every little bit helps when it comes to reducing your risk of developing dementia.
