D

oes staying physically fit keep our brain in shape? Or, does having a fit brain promote physical function? In the past, research has been focused on the positive relationship of physical activity contributing to enhanced cognitive function, assuming that increased physical activity in older adults will lead to a healthier brain. A University of Geneva research team in Switzerland did a long term study on which comes first: More physical activity or a healthier mental state. The research team evaluated data from over 105,000 adults, ages 50–90, over a 12-year period. The result? People with healthier brains are more likely to be physically active. Therefore, cognitive capacities influence physical activity levels, more than vice versa. Author Boris Cheval, PhD, a sports psychologist researcher at University of Geneva, stated, “This study backs up our theory that the brain has to make a real effort to get out of a sedentary lifestyle and that by working on cognitive capacities, physical activity will follow.” Another way to think about the data is that if people maintain physical activity over a lifetime, they will simultaneously be supporting healthy cognitive resources. hen you think of a special occasion, you might envision Christmas dinner, an important anniversary, or a milestone birthday. And surrounding those occasions is probably some really good food and even better people. Those big celebrations are a great excuse to gather and enjoy delicious food, but in times where we’re finding joy in the little things, it’s worth noting that every meal is a chance to create a special occasion. Setting Your Table No matter the occasion, setting the table can bring a whole meal together in a way that feels celebration-like. ealth issues of a colorectal nature— those dealing with the colon, rectum and anus—aren’t ones people like to openly discuss, for obvious reasons. They’re sensitive, sometimes embarrassing issues, but that doesn’t mean you should avoid addressing them. Expert Surgeons Dr. Allison Robinson is a colorectal surgical expert at PeaceHealth St. Joseph Medical Center in Bellingham. (Photo credit: Mark Turner) At PeaceHealth St. Joseph Medical Center in Bellingham, expert surgeons can help with both the surgical and non-surgical treatment of colorectal issues. After completing both general and colorectal surgical residencies, colorectal surgeons become board-certified with oral and written examinations by the American boards of Surgery and Colon and Rectal Surgery. Afterward, they receive additional fellowships and advanced surgical training. Dr. Allison Robinson, a retired Navy Medical Corp captain with colorectal surgery board-certification since 2005, recently joined PeaceHealth in Bellingham after more than a decade in California. She has special interest in treating inflammatory bowel disease (IBD) and rectal cancers, and is highly-trained in roboticassisted surgery. “I’m someone who’s genuinely interested in all these problems,” she says of colorectal health. “In this specialty, you really have to talk intently with your patients, examine your patients, and come up with a treatment plan with your patients. That’s what this job is all about.” PeaceHealth St. Joe’s, she says, is especially 12

well-equipped to treat colorectal issues without referring patients to out-of-town locations, and she can provide a full spectrum of care. Dr. Robinson is happy to work with a patient’s gastrointestinal doctor for treatment of inflammatory bowel disease (IBD). Robotic-Assisted Surgeries With robotic-assisted surgeries, Dr. Robinson operates using a vision monitor with threedimensional, magnified images. She guides robotic arms with surgical instruments, mimicking her hands movements but doing so with a greater range of motion and efficiency than would occur in traditional or laparoscopic surgeries. Dr. Robinson says colon surgeries can be done robotically, as well as hernias and lower rectal cancer surgeries. Robotic-assisted surgery is especially helpful for the latter, because it provides the surgeon with superior vision and precision compared to other surgical methods. Whether a colorectal surgery is done with or without robotic assistance, minimally invasive procedures typically provide patients with less post-operative pain, fewer complications, and faster recovery times. Address Health Issues And whether your colorectal health issue is ulcerative colitis, Crohn’s disease, fecal incontinence, rectal prolapse, diverticulitis, or just a hemorrhoid, Dr. Robinson says there’s no shame in addressing it. you’ve gotten past that psychological hurdle of not wanting to be there. A lot of people don’t want to admit to colorectal problems. They don’t want to deal with it, and they don’t want someone else to deal with it.” In fact, seeing a medical expert about a thing like hemorrhoids might even save your life. “Not everything is a hemorrhoid,” she says. “I’ve had plenty of anal cancers and rectal cancers where people thought it was their hemorrhoid.” Rectal bleeding, she adds, should never be ignored. While it could be caused by a benign issue, it could also be evidence of cancer. The only way to know, she says, is to be examined. And when it comes to cancer, colorectal cancer screenings are recommended often for anyone with a personal or family history of such cancers, or evidence of polyps. MyChart Video Visits Skagit Regional Health Launches New Telemedicine Program

S

kagit Regional Health providers and clinics are offering new ways to keep up with patient needs for care by launching a new telemedicine program. These MyChart Video Visits mean appointments are available from the comfort and safety of a patient’s home. MyChart Video Visits is a new service at Skagit Regional Health, launched to provide patients with options as the COVID-19 pandemic arrived in the region. The service offers clinical visits with the expert healthcare providers patients know and trust. Improved Access to Healthcare “MyChart Video Visits are a form of telemedicine that improves access to healthcare and also provides an element of safety for patient distancing,” said Rheumatologist Kevin Welk, MD. “Video visits provide the convenience and comfort of having medical care within a person’s own home.” With MyChart Video Visits, patients can meet with their primary care provider and many of Skagit Regional Health’s specialists over video chat, from nearly anywhere. Patients can use a smartphone, tablet, laptop or desktop computer. They just need stable internet connectivity and a web camera that is enabled during the visit. “It’s a great way to visit your doctor without leaving work,” said Jennifer Benson, MD, a Family Medicine provider in Mount Vernon. “You could schedule a visit on a lunch break or between meetings. It might be great for someone who doesn’t have transportation right now or if you’re home with kids and can’t get a babysitter.” Challenges of Travel Many patients travel a great distance to meet

with a specialist and may have to navigate ferry schedules or other challenges. “We get patients who drive 20, 30, sometimes 40 miles to meet with a specialist,” said Electrophysiologist Ramy Hanna, MD, who practices at Skagit Regional Health-Cardiology. “For those folks, this has really transformed their way of thinking about doctor visits. They can have basically all of the same benefits of coming into the clinic without ever leaving their living room or their home office.” Efficient Visits “For patients who are already established, I can provide almost the same, if not the same level of care as if they were in person,” added

wasn’t expecting the phone call. In one moment, the caregiving responsibility was over. Just like that, she was gone. It all started six years before, when dementia ravaged her brain. It had been coming for some time, we just didn’t understand the signs; in reality, denial hadn’t allowed us to see the clues even when they were obvious. By the time we realized how far the disease had progressed,

we were behind the eight ball. How do We Protect Her? The attorney advised us to document her lapses, but our chances for guardianship were slim, “Unless she is a proven threat to herself or to others, the State will not step in.” “You mean to tell me overdosing on blood thinners because you forgot you’ve already taken your medication is not a proven threat? How do we protect her?” Tough Love Tough love is an easy discussion topic, in theory, but who could imagine tough love would entail involuntary commitment to a state mental hospital? I participated in the gathering of information to prove her inability to make sound decisions before the court. I distracted her while my husband discussed the illness with psychologists and lawyers in hushed whispers. I sheltered my children from the mental decline of their grandmother. I watched the emotional turmoil take its toll. Never Truly Prepared My husband had been chosen as her Durable Power of Attorney. Like many of us, his mother placed her future care in his capable hands years before when her mind was clear and free of paranoia. As he wrestled with the conflicting choices between protecting her autonomy and her long-term well-being, I learned we are never truly prepared. Who (Continued on Next Page)

Senior Get-Away Surprise Linda Jinkens, Freelance Writer Just before the Covid-19 shutdown, my husband and I were given the gift of a onenight stay at an expensive mountain lodge. Our daughter insisted it would be restful and rustic. Excited, we left early, figuring it would take three hours to get there.

Three hours went by. “Maybe we should head home after this,” I said to Ron. “It’s getting late. Check in time is no later than 7 pm.”

After Lunch

“OK, OK, you don’t have to get cranky,” I said. “This is not my fault, you know!”

Later, after lunch, we got back in our car. The ignition clicked, but wouldn’t start. “Great, dead battery,” Ron said. “No worry, dear.” I pulled out our insurance card and called our agent. She kindly said, “I will call a tow truck and get back to you.” Later, she called back. “I’m sorry, but there are no tow trucks available! There’s been a fivecar accident on the freeway, and every tow truck is being used.” “What? No tow trucks?” said Ron, incredulous. “You’re kidding!” Two hours went by. Finally, a tow truck driver swung into view. He jumped our car, and off we went to the repair shop. The manager said he did not have our battery type in stock and needed to order it. We would have to wait. “I can’t believe this!” said Ron, very annoyed. “They don’t have our kind of battery? It’s just a standard battery!” 30

“We’re not going home!” Ron said testily. “Not when we’ve come this far!”

Finally, the battery installed, we drove off. “Ron,” I said, “you won’t believe this! They must have disconnected the radio while putting in the battery!” “I’d believe anything at this point!” Ron said, grumpy. The Lodge We drove through miles of stop and go traffic and then endless forest. Finally, we arrived at the lodge parking lot at 8:30 pm. Frazzled and exhausted, we stumbled to the desk. “They may not have a room for us,” I warned Ron. “They better!” he said grimly. “There’s not another hotel for miles!” “Your room is available,” the Frenchaccented clerk said, “but the dining room is shut for the evening.” “What? No dinner? I don’t believe this!” Ron’s frown had become permanent. We slowly

Active 55 Plus: These communities offer features geared towards the 55+ crowd. Features vary, but may include a clubhouse, swimming pools, exercise facilities, walking trails, golfing, security, and maintenance. Housing types differ and may include luxury apartments for rent, or homeownership types such as condos or homes in gated communities. Affordable Housing: A subsidized apartment rental program for people who are disabled or elderly, including Section 8, HUD, public housing, and tax credit buildings. Independent/Retirement: Private apartments rented to seniors. Some offer three meals per day. Activity programs and housekeeping may be provided, but residents are capable of independent living. Private pay only. Retirement Communities are maintenance free to seniors. Glossary: Housing & Care

Housing with Care Assistance Adult Family Home (Adult Care Home): A residence owned or rented by a licensed provider. Room and board, trained caregivers 24 hours a day, and assisted care services for 2 to 6 adults are provided in a home environment.* Licensed Nurses on staff are not required, but may be available. In Honor of Veterans - Lest We Forget Jerry Stewart, One Moment In America As we prepare for another Veterans Day in our America, it is important to be reminded lest we forget: U.S. President Harry Truman said,

“America was not built on fear, but on courage, on imagination, and an unbeatable determination to do the job at hand.” Going all the way back to the beginning of our new nation, America has been blessed with brave and courageous men and women who have been willing to give their all, their “last full measure of devotion” for the America they love. Nathan Hale, on a special mission for General George Washington in 1776, was captured and hung by the British Army. His last words before dying were, “I regret that I have but one life to lose for my country.” Captain James Lawrence, commander of the American ship, the Chesapeake, was fatally wounded early in the battle while manning his post from the cockpit. He gave orders to his last breath, “Keep the guns going! Fight her til she strikes or sinks! Don’t give up the ship!” This is the same unbelievable courage we see in our brave men and women serving today. They are willing to give their very lives for their country - and many do. Interviews With Veterans Over the last 20 years, I’ve had the great opportunity to interview over one hundred war veterans. I’ve talked with those who fought in World War II, the Korean War, the Vietnam War, and the Middle East War. I’ve also talked with foot soldiers that fought in Europe, 38

including one man who was captured at the Battle of the Bulge and spent 4 months in a German concentration camp. Interviews also included men who were captured by the Japanese and forced to walk in the brutal Bataan Death March. I’ve talked with Veterans who have worked in various occupations within the Armed Forces. These include a Marine Ranger, PT boat gunner, an airplane mechanic, a jet fighter pilot, and helicopter crew chief, and a tank gunner. These are all brave Americans from every possible circumstance. I find two things to be consistently true: • First, each of them believes that whatever they did in the war wasn’t anything special they were just doing their job. To put their life on the line for our country was their job and their responsibility, and each was so very proud to serve. • Second, each brave war veteran I have spoken with always wants to give the credit to someone else. Each of them is hesitant to take any credit for themselves. That is amazing. But that’s the kind of people America is made of. We can be so thankful for the heroes, those who ride in cars during parades, those who are lavished with awards and medals and praise. We can thank God for them. But wars are not won by a few decorated heroes. For every one decorated hero, there are literally millions of brave American men and women who have fought to give us the freedom we have today - and we are so very thankful. climbed the stairs. “Wow, this is a tiny room,” I said, “but rustic!” The small bed was made from a décor of sticks. The mattress felt like a granite slab. I searched the room for a coffee pot. “Ron, there’s no coffee pot in here!” He was lying on the bed, eyes closed. “You can get it in the morning.” I was shocked. “No coffee pot! I have to have a couple cups in the morning before I get going!” I walked downstairs. The surprised clerk said, “We removed all of the coffee pots years ago.” I walked back upstairs. “Ron, there are no coffee pots in any of the rooms!” Ron was snoring. The Night I tossed and turned on the bed. No air conditioning. I opened the window. Noisy chatter assailed my ears. Groups of people looked up. “Good grief, Ron!” I said. “We are

situated right over the entrance to the lodge!” Ron snorted and fell back asleep. The Morning We paid for an expensive breakfast in the morning and walked outside. “How about visiting the gift shop?” I asked. “Gift shop! Forget it!” he said loudly. “Tell the desk we are leaving! I’ll put our stuff in the car!” “You don’t have to rant,” I said. “I’m not ranting!” he yelled. The young man with the French accent handed me a huge bill. I stared. “But it’s all paid for! We don’t owe anything! Paid for!” He looked puzzled. I told him this was a gift we had been given. “It’s paid for! Paid for!” I headed out to the car. The clerk trailed behind, pleading in French. I kept turning around. “Paid for! Paid for!” “Ron,” I said, getting to the car, “he thinks we are trying to leave without paying! This is so embarrassing! People are staring!” Ron threw the last piece of luggage into the trunk. “Well, just pay him! This is crazy! We will straighten it out later!” I handed the man our card. He rushed off and came back, receipt in hand, now smiling. We weren’t. Turns out our daughter forgot to pay for the room.
