Move...a lot
The most basic key to achieve functional longevity is human movement. We were created to move and we should be moving naturally every day. Natural movements are those things we do in order to accomplish a task or to provide enjoyment. Lifting, bending, stooping, walking, running, carrying, pushing, pulling, jumping, skipping, hopping, squatting, throwing, catching, crawling, climbing, rolling, somersaulting, etc. These are all natural movements that we have been doing since birth. Functional Exercise Exercise is really just a specific type of physical activity, but it is one that yields many additional benefits beyond just being physically active. Unfortunately, exercise is poorly understood by the typical adult, especially in the context of aging. Exercises Are Not All Created Equal Not all exercises are created equal. An exercise program designed for functional longevity looks VERY different 6

than the kind of program the vast majority of people engage in. Most people follow programs that increase muscle size and strength. On the surface, this sounds good to combat aging, since the aging process tends to reduce both. However, research has made it pretty clear that, although these types of programs unequivocally improve muscle size and strength (even in 90 or 100 year olds), they do NOT necessarily improve physical function. Latham and colleagues (2004) reviewed the best training studies available. They concluded that strength gains are not equal to similar functional improvements in older adults who exercise regularly. And those who become the strongest do not necessarily become the most functional. Physical Function The reason that strength gains and functional improvement are not equal is because physical function is much more complicated than we realize. There are many other factors which contribute to physical function other than just how big and strong the muscles are. These factors include: motor control, coordination, muscle power, proprioception, sensory integration, balance, stability, mobility, agility and more. Eating for Energy is Easy! By Nichole Vander Meulen

T

here is a lot of information swirling around out there about what’s healthy, what isn’t, and how to meet your personal nutritional needs: fad diets, more fat, less sugar, no carbs, all protein, fasting, cleanses, and the list goes on. The overload of health information is confusing, and can put you off-course on your way to feeling healthy. And the more we learn about nutrition, the more apparent it becomes that to eat (and be) healthy we need to get back to basics: what can you eat that will give you energy? What can you eat that makes you feel more alive? And what can you eat to be the best you, at any age? The answer is surprisingly simple: real, whole foods. Experts in the health community believe that eating more whole foods is the best way to improve health and lower the rate of disease, including heart disease, cancer, and diabetes. Real, whole foods are foods that are as close to their natural form as possible: think vegetables, fruit, whole grains, nuts, and legumes. Whole foods are great because they retain more nutrients than processed foods and don’t include many of the added calories, fats, sugars, and mystery ingredients you can find in processed and fast foods. Whole foods are easy to spot, but when you’re feeling uncertain, just remember that you don’t always have to make the best choice, just make the better choice! For example, you can pick a chicken thigh instead of a chicken nugget, a loaded baked potato instead 8

of potato chips, or a smoothie full of berries instead of a fruit-flavored juice. Eating healthy still needs to be flavorful and delicious. Another great way to eat more whole foods is simply to add more plants to your plate—no need to take out what you love. Add a fruit to your breakfast, a leafy green and some carrots to lunch, and a vegetable side to your dinner. Next add whatever else you want to feel energized. It’s your food, and you should enjoy it! Food co-ops and farmers markets are a great place to find whole foods. Food co-ops in particular are a great resource. They have taken the guesswork out of grocery shopping by considering every product in regard to antibiotics, artificial preservatives, and GMO status. Their careful sourcing ensures that whatever you put in your basket is a good choice—raised sustainably and ethically and doesn’t contain strange, non-food ingredients. It’s never too early—or too late—to eat for the life you want to live. Start with your next shopping trip and head home with a few more vegetables than before. Regaining Mobility and Independence: Mary’s Story By Lauren Fritzen

O

n a winter day in 2017, “Mary H.” came home complaining to her husband of a terrible headache. As her condition quickly deteriorated, she was rushed to PeaceHealth St. Joseph’s Medical Center in Bellingham. It didn’t take long to discover the cause, hemorrhagic stroke. Mary underwent emergency surgery. Over the next few weeks, she recovered in the hospital. Although recovery went well, the stroke resulted in some paralysis on her left side and trouble speaking and swallowing. As her condition improved, Mary was moved to a rehab facility for intensive physical and occupational therapy. A few weeks later she was home. With continued outpatient physical therapy, she reached most of the goals of increasing strength and mobility. Although she made excellent progress, her left ankle was not cooperating. The stroke had left her lower leg muscles very tight, and she lacked the range of motion for a normal gait. Her need for a wheelchair continued. Her physical therapist 12

referred her to Cascade Prosthetics & Orthotics to be evaluated for an ankle-foot brace. Mary met with Cindy Diener, one of the University of Washington-trained practitioners at Cascade’s Ferndale clinic. They discussed her mobility issues and her therapy goals. A good working relationship was established immediately. “My initial assessment of Mary was how motivated and engaged she was in her own rehab. She was working with the physical therapists and doing all the exercises they had given her as a home therapy program. Mary was completely determined to do whatever it took to get back what she had lost after the stroke,” says Cindy. A 2007 study published in the Journal of the American Medical Association suggests almost 45 million Americans get “physicals” every year. While health insurance plans cover these preventive examinations (and usually appropriate lab testing), there is one unique patient population for whom “physicals” are not covered by insurance: Medicare patients. You read that correctly; if you are 65 years old or older, and have a Medicare health insurance plan, you technically cannot receive a “physical” and have it covered by the insurance plan. Routine lab testing for preventive care needs are also not usually covered by Medicare. There is a reason for this; studies are demonstrating that a routine “physical” is in fact, not helpful for promoting health. What is helpful however, is a targeted doctor’s visit that looks at specific aspects of your health for preventive needs; and this is what is termed a “Medicare Wellness Visit.” Medicare Wellness Visits are absolutely covered by Medicare, but do not necessarily include laboratory testing or a physical examination. The structure of the visit revolves around reviewing medical history, chronic medical conditions (such as high blood pressure, diabetes, etc.), medication review, and preventive 14

health care needs (e.g., colon cancer screening, breast cancer screening). We also take the opportunity to discuss advanced care planning, such as living wills and what a patient’s desires are for end of life planning (should the need arise). These are all critical aspects of one’s health, but depart from what one is used to earlier in life with the good old-fashioned “physical” at the doctor’s office. So, when you are thinking of calling your doctor’s office for a “physical,” and you are a Medicare-covered patient, ask instead for a “Medicare Wellness Exam.” Your doctor will sit down with you (and any loved ones you want present) to offer you a comprehensive review of your overall health, and a conversation on planning for any health needs in the future. Be mindful of increased costs to you if you decide you would like any labs drawn. These will likely go against your deductible with your particular Medicare insurance plan. Medicare Wellness Visits might sound a bit onerous and non-traditional. But they really are a means of cementing and leveraging the therapeutic relationship you have with your doctor to achieve your health care goals. Man on the Moon!

50 Years Past, 50 Years Forward by Kirk Blankenship “That’s one small step for “a” man, one giant leap for mankind.” These were the first words spoken by Neil Armstrong when he made that first historic step onto our moon’s surface on July 20, 1969, already a half century ago! Walter Cronkite, “Man on the Moon!” Since I was only two years of age, I don’t recall the event directly. But I do remember hearing stories from my dad and grandpa about this historic exploration. And I remember seeing TV footage with Walter Cronkite anchoring the Apollo 11 expedition and moon walk. Over 650 million viewers around earth’s globe watched the live footage with awe. As Apollo 11 with both Neil Armstrong and Buzz Aldrin made their lunar touchdown, Cronkite said emotionally, “Man on the moon! Oh… boy!” A little later, Cronkite continued, “There he is. There’s a foot coming down the step. Armstrong is on the moon. Neil Armstrong, 38 year old American standing on the surface of the moon on this July 20th nineteen hundred

and sixty nine.” Then radio sound waves reached earth from Neil Armstrong, “That’s one small step for “a” man, one giant leap for mankind.” Cronkite continued, “Well for thousands of years now, it’s been man’s dream to walk on the moon. Right now, after seeing it happen, knowing that it happened, it still seems like a dream.” Where Were You? Famous events have a way of making a lasting impression on our mind. Many of you who are from the Greatest Generation, Silent Generation, or Baby Boom Generation may remember watching the live footage of the lunar landing and witnessing that first “small step” and “giant leap.” You likely remember exactly where you were on that historic day. How did this event make you feel at the time? America has born thousands of inventors, men and women, who have changed history. Explorers have changed the world by setting out on adventures across land, and sea, and even into space. Through these explorations and through the internet, the world has become a smaller and more tangible place. Questions to Ask and Discuss How did the lunar landing change history? How did the invention and development of space travel improve life on planet earth? Grannie Gets Moving (Continued from Page 10) “I Can’t...No Way!” Once parked in the lot he put one foot on his bike pedal and then threw his other leg expertly over the frame. He looked at me expectantly. I said, “Honey, I can’t throw my leg over this man’s bike! I will break my hip! Do you want me to break my hip right here in the parking lot?” “You won’t break your hip,” he said. “Try it.” “I can’t!” I protested loudly. “No way!” Finally, after looking around to see if anyone was watching, I laid my bike down on the ground and cautiously stretched one leg over a tire. Then I pulled up the bike under me. A truly nerdy thing to do, but necessary.

“You can figure out gears,” he said reassuringly. Worried, I listened carefully to his instructions: how to climb a hill and how to stop. Could I remember? The gears were wonderful. I slowly extended my riding to eight miles—and then to 14. I began to love the fresh air and the challenges of the trail. Other cyclists were starting to nod their heads at me, almost as if I were a real cyclist! Two Years Later Two years later, a surprising thought entered my mind. A small town lay on the trail, but it was 26 miles away--52 miles total. It seemed impossible, like riding to the moon! Could I do it?

My husband smiled. “OK, we’re off,” he said. He was right. I hadn’t forgotten how to ride a bike. But I was horrified to see I could only ride one mile! “OK, dear,” I said, huffing and puffing up a tiny hill, “I’m going to have to stop. Time to go home. Thanks for the wonderful bike ride.” “Now?” he said. “We were just getting started!” But we packed up and went home. After a few times, I rode the trail by myself. “I can do this!” I thought. “It’s not fun, but I can do this!” Resigned, I hung in there three times a week. Gradually, I lengthened my rides to six miles. I Don’t Do Gears! One day, my husband took me to a bike shop. I felt intimidated by the lean young cyclists in the store, but I left with a nice new jacket, helmet, and bike gloves. But that wasn’t all. My husband had ordered a nine-speed custom-built bike with the gears on one handle. “I don’t do gears!” I said to him in a panic. “I can’t figure out gears! This is too difficult!” 22

I decided to attempt it one day while my husband was gone. I carried water and put healthy snacks and my phone into a lightweight backpack. I was off! My enthusiasm carried me the 26 miles in three hours to the small town. I had stopped along the way to rest a few times and admire the scenery. Dropping onto grass, I tiredly munched lunch. Affordable Housing: A subsidized apartment rental program for people who are disabled or elderly, including Section 8, HUD, public housing, and tax credit buildings. Independent/Retirement: Private apartments rented to seniors. Some offer three meals per day. Activity programs and housekeeping may be provided, but residents are capable of independent living. Private pay only. Retirement Communities are maintenance free to seniors. Housing with Care Assistance Adult Family Home (Adult Care Home): A residence owned or rented by a licensed provider. Room and board, trained caregivers 24 hours a day, and assisted care services for 2 to 6 adults are provided in a home environment.* Licensed Nurses on staff are not required, but may be available. How do I protect my BRAIN? By Dr. Kevin Clay

W

e as a nation are facing increased rates of Alzheimer’s and strokes, and both of these medical conditions drastically affect the brain and alter one’s life and the families as well. Alzheimer’s disease is one of the most rapidly increasing modern medical illnesses, with Diabetes disease coming in second. The startling fact is that those with Diabetes have a two-fold increase in risk of Alzheimer’s Dementia and an increased risk for strokes as well. Strokes are a “Brain Attack,” similar to a heart attack, while Alzheimer’s disease is a “Mind Attack.’’ Strokes are usually caused by the blockage of a small blood vessel in the brain. Alzheimer’s disease is the gradual blockage of nerve tracks caused by inflammation, and creates “Amyloid Plaques.” This disease was first described about 100 years ago by a German housewife. Most Health is Controlled by Choices Medical Science is now recognizing that only 10% of our health is controlled by medical providers and hospitals. Genes have 20-30% impact, leavings lifestyle choices as having 6070% impact on our health. Several recent books have been published that show the significant impact of positive lifestyle choice and the effects on the brain. These lifestyle changes may be more powerful than prescription medications. Let’s look at some of the recent books in the last decade to answer our question, beginning 44

with the “Blue Zones”. “Blue Zones” Most all the world is living longer, and the “Blue Zones” (areas where people live 8-10 years longer with good health) can teach us some valuable lessons. There are five Blue Zone areas in the world: Ikaria, Greece; Sardinia, Italy; Okinawa, Japan; Nicoya Peninsula, Costa Rica; and Loma Linda, California. In each of these communities we find a theme of healthy whole food diets with a strong plant slant. Meat is enjoyed for holidays and special occasions, but not daily. Active lifestyles and strong social connections are also common. “How Not to Die” Dr. Greggor, in “How not to Die,” recommends several diet and lifestyle suggestions that can reduce your risk of stroke. What are his recommendations? Eat a high fiber diet that has vegetables and fruits (especially citrus), beans, whole grains and nuts, and simple whole foods. The ideal diet limits or avoids meat and dairy, as well as excess sugar. The highly processed foods that have a long label are not the best choice. One needs to limit their salt intake to less than 2-3 grams of salt daily (about a teaspoon). Make exercise a regular routine for a minimum of 5 days a week and 30 minutes per day for a weekly total of 150 minutes. Fall-Winter, 2019-2020

How do I Protect My BRAIN? (Continued from Page 44) “Reversing Alzheimer’s Disease” Dr. Bredesen, in “Reversing Alzheimer’s Disease,” has done a lot of research at his Buck’s Institute in the San Francisco area. He says that the one food which causes the most inflammation is sugar. He recommends we avoid processed sugar. He also proposes that the cause of Alzheimer’s disease is from nutritional deficiencies and poor diet that lead to inflammation in the brain. He developed the “ReCode” program that includes nutritional supplements to augment his low fat, grain free, mild meat, and fish consumption diet. His book presents several cases of people who have followed the program, reversing Alzheimer’s disease. He also recommends regular exercise and brain exercises. The worst thing we can do is to sit at home and watch TV.

• E=Exercise: at least 30 minutes daily to improve blood flow to your brain • U=Unstressed: Learn how to manage stress. Too much stress is very destructive on the brain. Under stress we tend to make poor decisions, which impact our ability to make good lifestyle choices. • R=Restore or Rest: We need to have about 7-8 hours of sleep each night. During sleep the brain reformats the day and recharges! And finally… • O=Optimize your brain: Keep using your brain by studying and learning new things. Be socially engaged in your community, family, and group settings, such as church or social functions. In summary, the three authors and research from “Blue Zones” are all recommending similar diet and lifestyle solutions to protect our brains from strokes or Alzheimer’s disease.

“The Alzheimer’s Solution”

Optimal Diet Recommendations:

Dr. Sherazai who wrote “The Alzheimer’s Solution,” has done a significant amount of research around Alzheimer’s disease. He has an Alzheimer’s clinic at Loma Linda University. In his book, he supports the findings of Dr. Bredeson that Alzheimer’s is an inflammatory disease, which is often the result of poor diet and lifestyle choices. In 1900, the average American had less than 10 pounds of sugar in their diet. By 2010, the FDA reported the average American’s sugar intake as 190 pounds! Soda pop in 1900 equaled Zero, an by 2010 equaled 56 gallons annually! He has a simple mnemonic to follow to protect your brain: NEURO • N=Nutrition: Whole foods, plant slant diet with mainly vegetables and fruits, whole grains and nuts, legumes (beans). Avoid sugar and processed foods. He recommends limited fish or small amounts of meat, or no meat at all.

The optimal diet recommended is whole foods with a strong plant slant, lots of vegetables and fruits, nuts, whole grains, and legumes. And AVOID refined sugar. Meat intake: Little or none, moderate amounts of wild fish. Daily physical activity is recommended. And finally, the authors encourage us to never stop learning and enjoying life to the fullest!
