When David Cameron announced that he intends to hold a referendum about the UK’s membership of the EU, he probably just wanted to appease those backwards thinkers Tories party who had been banging on about this subject for years. Nobody I know ever thought that such a referendum would result in Brexit. So, what happened?

A few months ago, when the campaign started in earnest, it was amazing to see how well organized the ‘leave camp’ (LC) was and how many totally diverse interest groups it managed to unite. By contrast, the ‘remain camp’ (RC) seemed to be lethargic, almost as though they did not take the whole thing seriously. Then it became obvious that the LC intended to use highly emotive arguments (e.g. fear of foreigners, Britain can be great again), while the RC mainly tried to rely on dry and often relatively boring facts. And finally, the campaign degraded into an aggressive political warfare of unseen proportions where one side was trying to outdo the other with intrigue, lies and misleading information. Politics had suddenly turned into a mudbattle, and the rules of the jungle were the only ones respected by those involved.

Even at that stage, the prevailing opinion was that the RC would be able to achieve a narrow victory. Personally, I had always thought they would suddenly wake up, spring into action and start putting some true conviction and persuasive arguments into the debate. But such optimism was harshly disappointed. The nightmare of Brexit

has become reality. What will happen next?

As both major political parties are assessing the damage they have inflicted on themselves, and as politicians are frantically jostling for positions, a few things have become fairly clear: • The disgraceful display by many politicians has thoroughly destroyed the trust of the public in politics, and it will take many years to rebuild it.

• The UK has become a much poorer country, and the economic outlook is depressing.

• Most other economies in the EU and beyond have been damaged as well.

• Racism is on the rise.

• The far right senses ‘its hour’.

• Europe is not inclined to do Britain any favors.

• Some national European groups are now thinking of mounting their very own Exit movements.

• The unity of the UK is in great danger.

It seems obvious that such profound changes will also affect the realm of healthcare. At present, the NHS employs a large percentage of foreigners.

This might change for at least two reasons: firstly, after Brexit, many of them might be forced to leave the UK; secondly, some might not feel a t home in a country gripped by mounting xenophobia. This would mean that the NHS, which has been close to breaking point for some time, would no longer be able to provide an adequate service for everyone.

Such developments will be aggravated the financial recession that many experts predict for the UK. Money for healthcare is currently already desperately short. The chancellor has made it quite clear that further austerity measures are unavoidable in a post-Brexit UK. If the NHS budget is cut any further, healthcare in Britain will seriously deteriorate.

Rampant xenophobia might also mean that the many visitors to the UK will no longer be provided for when they fall ill. Under EU rules, we were all able to receive free healthcare wherever we are in Europe. Once Brexit is complete, this option is unlikely to survive.

When it comes to medical research, there can be little doubt about the negative effects Brexit will have. Collaboration of UK scientists with colleagues in Europe will become more difficult. In the past, research funds often came from the EU; after Brexit, UK scientists will no longer be eligible to apply for such funds. If we add to this the possibility of a financial recession, we have a near perfect doomsday scenario for UK medical research.

As I already stated: a nightmare has become reality. I am unable to see anything positive in the UK’s vote to leave the EU. In my view, the decision to hold a referendum was a shortsighted and stupid move, and its results are utterly deplorable.

Die Herausforderung beim Management der autosomal-dominanten polyzystischen Nierenerkrankung (ADPKD) ist groß – für die behandelnden Ärzte wie für die betroffenen Patienten. Bisher konnte die ADPKD nur symptomatisch behandelt werden. Seit Mai 2015 ist mit Tolvaptan (Jinarc®) das erste krankheitsmodifizierende Medikament der ADPKD in Europa zugelassen. Der selektive Vasopressin2-Rezeptor-Antagonist mindert den jährlichen Nierenfunktionsverlust und reduziert das renale Zystenwachstum [1, 2]. Jinarc® ist indiziert bei erwachsenen Patienten mit Niereninsuffizienzstadium 1–3 zu Behandlungsbeginn mit Anzeichen für eine rasche Krankheitsprogression [2].

Als systemische Erkrankung kann sich die ADPKD sowohl renal als auch extrarenal manifestieren. Meist leiden die Patienten unter Hypertonie, Hämaturie und Proteinurie (Abb. 2), oft auch unter Flankenschmerzen, rezidivierenden Harnwegs- und Zysteninfektionen.

Auch renale Zystenbildungen, insbesondere in der Leber, intrakraniale Aneurysmen, Herzklappenveränderungen und Kolondivertikulose können auftreten.

Insgesamt ist die ADPKD individuell sehr unterschiedlich ausgeprägt [5–7].

ADPKD: Vierthäufigste Ursache für chronisches Nierenversagen

Die Erbkrankheit ist die vierthäufigste Ursache für chronisches Nierenversagen [4]. Charakteristisch für ADPKD sind die Bildung und stetige Ausdehnung von flüssigkeitsgefüllten Zysten in den Nieren, was zu einer erheblichen Vergrößerung des gesamten Nierenvolumens führt (Abb. 1). Durch das Zystenwachstum kommt es zu einer fortschreitenden Nierenschädigung, die bei den meisten Betroffenen im Alter zwischen 50 und 60 Jahren in terminales NierenversaPerfusion 3/2016

die Möglichkeiten für eine frühzeitige Diag­nose mittels Ultraschall oder Magnetresonanztomographie (MRT) sehr viel besser. Wichtig ist die sichere Abgrenzung der Systemerkrankung ADPKD, der sogenannten Zystennieren, von den häufig vorkommenden einfachen Nierenzysten. Zur Unter-

Erkrankungsmechanismus: Genmutation führt zur Hochregulation von cAMP Bei ca. 85 % der ADPKD-Patienten liegt eine Mutation des Gens PKD1 und in der Mehrzahl der übrigen Fälle des Gens PKD2 vor [13]. Infolge der Mutation kommt es zu einer Fehlfunktion des Polycystin-Komplexes (Polycystin 1, Polycystin 2) auf den Primärzilien der Tubulusepithelzellen in der Niere. Dadurch kann der mTOR-Signalweg (m-TOR = mammalian target of rapamycin), der den Metabolismus der Zelle aktiviert, nicht mehr gehemmt werden, sodass die Zellproliferation gesteigert wird. Außerdem wird der Vasopressin-2-Rezeptor hochreguliert. Bindet Vasopressin an den V2-Rezeptor, werden die vom zyklischen Adenosinmonophosphat (cAMP) gesteuerten Signalwege in Gang gesetzt, die zur Antidiurese und damit zu Flüssigkeitsansammlungen führen. Damit ist Vasopressin ein entscheidender Faktor für die Progression der ADPKD [13].

Können im Alter von 30 Jahren keine Zysten nachgewiesen werden, ist eine ADPKD praktisch ausgeschlossen [9].

Ein Gentest wird nur in Ausnahmefällen durchgeführt, beispielsweise, wenn aufgrund des klinischen Bildes keine eindeutige Diagnose gestellt werden kann oder wenn bei jüngeren Patienten (<30 Jahre), z.B. wegen einer geplanten Nierenspende, eine ADPKD definitiv ausgeschlossen werden muss.

Besonderer Wert sollte auf eine adäquate Blutdruckkontrolle gelegt werden – einerseits zur kardiovaskulären Prophylaxe, andererseits zur Progressionshemmung der ADPKD [11, 12].

Vor allem junge ADPKD-Patienten mit noch erhaltener Nierenfunktion profitieren von einer aggressiven Blutdruckeinstellung auf Werte von 95/60– 110/75 mmHg in Form von geringerem Zystenwachstum und geringerer Albuminurie. Empfohlen wird die Gabe eines ACE-Hemmers oder AngiotensinRezeptor-Blockers, eine Kombination bringt keinen Benefit [12].

Mit dem selektiven Vasopressin-2-Rezeptor-Antagonisten Tolvaptan (Jinarc®) steht seit August 2015 erstmals eine nierenspezifische und effiziente Behandlungsmöglichkeit bei ADPKD zur Verfügung, mit der sich das Fortschreiten der Krankheit verlangsamen lässt [2].

Wirksamkeit und Sicherheit von Tolvaptan bei ADPKD wurden im TEMPO-Studienprogramm untersucht.

Hauptstudie ist die randomisierte, doppelblinde, placebokontrollierte Phase-III-Studie TEMPO 3/4 mit 1.445 ADPKD-Patienten im Alter von 18–50 Jahren. Einschlusskriterien waren ein per MRT gemessenes Gesamtnierenvolumen (TKV, beide Nieren addiert) von ≥750 ml sowie eine KreatininClearance berechnet nach CockcroftGault von ≥60 ml/min. Die Patienten erhielten im Verhältnis 2:1 randomisiert entweder Tolvaptan (n=961) oder Placebo (n=484).

Als primärer Endpunkt wurde die jährliche Veränderung des Gesamtnierenvolumens (TKV) festgelegt. Kombinierter sekundärer Endpunkt war die © Verlag PERFUSION GmbH

klinische Progression (Verschlechterung der Nierenfunktion, Auftreten oder Verschlechterung behandlungsbedürftiger Nierenschmerzen, Auftreten oder Verschlechterung einer Hypertonie sowie Zunahme der Albuminurie).

Ein weiterer sekundärer Endpunkt war die Veränderung der Nierenfunktion (gemessen anhand des reziproken Serum-Kreatinin-Spiegels, da dieser reziproke Wert in linearer Beziehung zur glomerulären Filtrationsrate steht) [1].

Unter der Behandlung mit Tolvaptan kam es zu einer signifikant geringeren jährliche Vergrößerung des Gesamtnierenvolumens von 2,8 % (95%-KI 2,5–3,1) im Vergleich zu 5,5 % in der Placebo-Gruppe (95%-KI 5,1–6,0; p < 0,001). Tolvaptan verbesserte auch den kombinierten sekundären Endpunkt der Krankheitsprogression signifikant: Es traten weniger ADPKD-bezogene Ereignisse pro 100 Patientenjahren (PJ) auf als unter Placebo (44/100 PJ vs. 50/100 PJ; HR 0,87; p=0,01). Sowohl die Verschlechterung der Nierenfunktion (2/100 PJ vs. 5/100 PJ; p<0,001) als auch behandlungspflichtige Nierenschmerzen (5/100 PJ vs.

7/100 PJ; p=0,007) traten in der Tolvaptan-Gruppe seltener auf als in der Placebo-Gruppe. Kein Unterschied zeigte sich in Bezug auf Hypertonie und Albuminurie. Insgesamt führte Tolvaptan zu einer Verringerung der kombinierten Endpunktereignisse um 13,5 % [1].

Außerdem verringerte sich unter der Tolvaptan-Behandlung der jährliche Nierenfunktionsverlust um 31–33 % je nach Niereninsuffizienzstadium (Abb. 3) und das renale Zystenwachstum reduzierte sich um 49,2 % im Vergleich zu Placebo (absolute Reduktion –2,71 %/Jahr; 95-%KI –3,27 bis –2,15; p<0,0001) (Abb. 4) [1].

Gutes Sicherheitsprofil In der TEMPO-Studie konnte für Tolvaptan eine gut vertretbare Sicherheit nachgewiesen werden [1].

Die Häufigkeit von unerwünschten Begleiterscheinungen war in beiden Studienarmen vergleichbar (Tolvaptan 97,9 % vs. Placebo 97,1 %). Aufgrund Perfusion 3/2016

Tolvaptan: Zielgerichtete Wirkung bei ADPKD Tolvaptan (Jinarc®) blockiert spezifisch die Bindung von Vasopressin an den V2Rezeptor in den Tubulusepithelzellen der distalen Abschnitte des Nephrons. Die Bindungsaffinität von Tolvaptan an diesen Rezeptor ist 1,8-mal höher als die des körpereigenen Vasopressins. Indem Tolvaptan Vasopressin am V2-Rezeptor verdrängt, wird die wird die bei ADPKD überaktive Vasopressin-Signalisierung für eine cAMP-Produktion in der Zelle gehemmt und weniger cAMP synthetisiert. Da die über cAMP gesteuerten Signalwege nun seltener aktiviert werden, kommt es zu einer Verringerung der Zellproliferation, insbesondere der renalen Zystenbildung und des Zystenwachstums. Gleichzeitig wird die Wasserausscheidung verstärkt [2].

Wirkmechanismus: Tolvaptan blockiert die Bindung von Vasopressin an den Vasopressin2-Rezeptor. Jinarc® wird 2-mal täglich oral eingenommen. Die empfohlene Tagesdosierung beträgt initial 60 mg, aufgeteilt in 45 mg morgens nach dem Aufstehen und 15 mg 8 Stunden später. Danach erfolgt – je nach Verträglichkeit – zunächst eine Dosiserhöhung auf 60 mg/30 mg (90 mg Tagesdosis) und schließlich auf 90 mg/30 mg (120 mg Tagesdosis). Die Behandlung wird dann mit der maximalen tolerierten Dosis fortgeführt [2].

des Wirkmechanismus des selektiven Vasopressin-2-Rezeptor-Antagonisten kam es unter Tolvaptan erwartungsgemäß zu einer erhöhten Aquarese (mit Durst, Polyurie, Nykturie, Polydipsie), wohingegen ADPKD-bedingte Ereignisse (z.B. Makrohämaturie, Harnwegsinfekte) seltener auftraten als unter Placebo [1].

Brust- und Kopfschmerzen, Hämaturie sowie eine Erhöhung der Leberenzyme Alanin- und Aspartat-Aminotransferase) waren unter Tolvaptan häufiger, renale Zysteninfektionen und Pyelonephritis hingegen unter Placebo (Abb. 5). und bei 1,2 % in der Placebo-Gruppe auf. Diese Anstiege waren mittels Therapieunterbrechungen von bis zu 8 Wochen in der Regel beherrschbar, wonach die Therapie – nach Normalisierung von ALT/AST – mit reduzierter Tolvaptan-Dosis ohne Relapse fortgeführt werden konnte [1].

Fazit für die Praxis

Bislang mussten sich die Ärzte auf die Behandlung der Symptome der ADPKD beschränken, mit Tolvaptan (Jinarc®) können sie erstmals das Fortschreiten der Erkrankung verlangsamen und die Nierenfunktion erhalten.

Mit der Tolvaptan-Therapie sollte begonnen werden, sobald die Diagnose autosomal-dominante polyzystische Nierenkrankheit Typ I feststeht. Laut Fachinformation müssen die Patienten zu Behandlungsbeginn Anzeichen für eine rasche Krankheitsprogression aufweisen [2]. Klinische Kriterien, die ein hohes Risiko für rasche Progression beinhalten, sind vor allem: • das Gesamtnierenvolumen in Bezug auf Alter und Körpergröße: >750 ml im Alter von 18 Jahren bzw. >1500 ml mit 35 Jahren • die Nierenfunktion: abnehmende eGRF im CKD-Stadium 2 oder Vorliegen von CKD-Stadium 3 (beides bei Patienten unter 50 Jahren) • das erstmalige Auftreten einer behandlungsbedürftigen arteriellen Hypertonie vor dem 35. Lebensjahr • das erste Auftreten von urologischen Komplikationen (Flankenschmerz, Zysteninfektion, Makrohämaturie) ebenfalls vor dem 35.

Lebensjahr • Eintritt der Dialysepflichtigkeit bei Vater oder Mutter vor dem 50. Lebensjahr Die Behandlung mit Tolvaptan muss unter ärztlicher Aufsicht erfolgen. Um das Risiko signifikanter Leberschäden einzudämmen, müssen die Leberenzymwerte in den ersten 18 Behandlungsmonaten monatlich, danach vierteljährlich kontrolliert werden [2].

Entresto® in LeitlinienUpdates aufgenommen Bereits wenige Monate nach seiner Zulassung wurde der Angiotensin-Rezeptor-Neprilysin-Inhibitor Entresto® (Sacubitril/Valsartan) mit dem höchsten Empfehlungsgrad (Klasse 1) in die aktualisierten europäischen und amerikanischen Leitlinien aufgenommen.

Grundlage dafür sind die Ergebnisse der PARADIGM-HF-Studie, in der Entresto® mit einer Senkung der kardiovaskulär bedingten Mortalität sowie einer Reduzierung von Krankenhauseinweisungen aufgrund von Herzinsuffizienz im Vergleich zur bisherigen Standardtherapie ACE-Hemmer überzeugt hatte (siehe Beitrag S. 86ff.).

Die aktuellen Leitlinien der European Society of Cardiology (ESC) zur Behandlung der Herzinsuffizienz empfehlen Entresto® anstelle eines ACE-Hemmers oder AT1-Blockers bei Patienten, die den Einschlusskriterien der PARADIGM-HF-Studie entsprechen.

Wie bisher sollen Patienten zunächst mit ACE-Hemmern und Betablockern behandelt werden, ergänzt um einen Mineralokortikoidantagonisten. Noch weiter gehen die Empfehlungen der neuen amerikanischen Herzinsuffizienz-Leitlinien: Sie stufen Entresto® neben ACE-Hemmern und AT1-Blockern als neue First-Line-Therapie bei chronischer Herzinsuffizienz mit reduzierter Ejektionsfraktion (HFrEF) ein. „Die Aufnahme von Entresto® in die Leitlinien trägt dazu bei, die Behandlungsziele bei Herzinsuffizienz zu erreichen, nämlich eine bessere Lebensqualität mit weniger Krankenhausaufenthalten und eine geringere Sterblichkeit,“ kommentierte Prof. Dr. med. Burkert Pieske, Klinikdirektor und Facharzt für Innere Medizin mit Schwerpunkt Kardiologie, Charité – Universitätsmedizin und Deutsches Herzzentrum, Berlin, und Mitglied der Leitlinien-Kommission der ESC. Faktor-IX-FcFusionsprotein bietet verlängerten Schutz vor Blutungen Ende Mai hat die Europäische Kommission den halbwertszeitverlängerten Faktor IX Alprolix® (rFIXFc) für die Behandlung der Hämophilie B zugelassen ihm den Orphan Drug Status erteilt. Alprolix® ist das einzige rekombinante Faktor-IX-Fc-Fusionsprotein zur Therapie von Hämophilie B in der EU, das eine verlängerte Protektion vor Blutungsepisoden mit weniger prophylaktischen Dosierungsintervallen bieten kann. Das Medikament ist für die Bedarfsbehandlung und Prophylaxe von Menschen mit Hämophilie B in allen Altersgruppen zugelassen. Prophylaktisch kann es mit einer Anfangsdosis alle 7 Tage oder alle 10 Tage verabreicht werden, mit der Möglichkeit, das Dosierungsintervall den individuellen Bedürfnissen anzupassen.

Die Zulassung von Alprolix® basiert auf den Ergebnissen von 2 Phase-IIIStudien, die die Wirksamkeit, Sicherheit und Pharmakokinetik von Alprolix® bei Hämophilie B gezeigt haben: der Studie B-LONG bei vorbehandelten Erwachsenen und Jugendlichen und der Studie Kids B-LONG bei vorbehandelten Kindern unter 12 Jahren.

Als unerwünschte Arzneimittelwirkungen mit einer Inzidenz von ≥0,5 % wurden Nasopharyngitis, Influenza, Arthralgie, Infektionen der oberen Atemwege, Kopfschmerzen und Hypertonie beobachtet. Die Mehrzahl dieser Ereignisse wurde als nicht oder unwahrscheinlich in einem Zusammenhang mit dem Medikament stehend beurteilt. Wie bei jedem injizierten Protein können allergische Überempfindlichkeitsreaktionen und die Entwicklung von Inhibitoren nach der Verabreichung von rFIXFc auftreten.

Chronische Herzinsuffizienz: Angiotensin-Rezeptor-NeprilysinInhibitor überzeugt durch neues Wirkprinzip

Die chronische Herzinsuffizienz ist mit einer hohen Mortalität assoziiert: Trotz Standardtherapie verstirbt jeder zweite Patient innerhalb von 5 Jahren nach der Diagnose [1]. Neue Hoffnung verspricht der Angiotensin-RezeptorNeprilysin-Inhibitor (ARNI) Entresto®, der im Januar 2016 zugelassen wurde.

Entresto® besteht aus zwei funktionalen Einheiten im molekularen Wirkstoffverhältnis 1:1, dem Neprilysin-Inhibitor Sacubitril und dem AT1-Blocker Valsartan. Damit beeinflusst Entresto® als erster Wirkstoff bei chronischer Herzinsuffizienz auch direkt das natriuretische Peptid-System (NPS), das eine Schutzfunktion auf das Herz entfaltet, und unterdrückt gleichzeitig das bei der Herzinsuffizienz überaktivierte, schädigende Renin-Angiotensin-Aldosteron-System (Abb. 1) [2].

Valsartan, die zweite Wirkkomponente von Entresto®, blockiert den AT1Rezeptor und schwächt dadurch die RAAS-vermittelten Effekte ab. Insgesamt führt dieser innovative Wirkmechanismus u.a. zu Vasorelaxation, Natriurese, Diurese sowie zu einer verminderten Fibrose und Hypertrophie des Herzmuskels (Abb. 2) [4]. Während andere Herzinsuffizienz-Medikamente nur die schädliche Wirkung des überaktiven RAAS blockieren, wird durch die Behandlung mit Entresto® der insuffiziente Herzmuskel doppelt entlastet.

Hochsignifikante Überlegenheit gegenüber Enalapril Die Zulassung des Angiotensin-Rezeptor-Neprilysin-Inhibitors Entresto® basiert auf den Daten der PARADIGMHF-Studie, der mit 8.442 Patienten bislang größten klinischen Studie zur Herzinsuffizienz [4, 5]. Zu den Einschlusskriterien gehörten eine chronische Herzinsuffizienz (NYHA II–IV) mit LVEF ≤40 % (in einer Prüfplanänderung auf ≤35 % gesenkt), ein BNP(bzw. NT-proBNP-)Spiegel von ≥150 (bzw. ≥600 pg/ml) oder ≥100 (bzw. ≥400 pg/ml) und Hospitalisierung wegen Herzinsuffizienz innerhalb der letzten 12 Monate sowie eine ≥4-wöchige stabile Behandlung mit einem

Dualer Wirkmechanismus Bei Herzinsuffizienz schüttet das Herz aufgrund der erhöhten Wandspannung natriuretische Peptide als Schutzmechanismus aus, die die Natriurese und Diurese fördern, schädigende Prozesse am Herzen hemmen und den Blutdruck senken. Die Wirkungen dieser natriuretischen Peptide sind jedoch begrenzt, da sie bereits nach kurzer Zeit vom Enzym Neprilysin wieder abgebaut werden [3].

Studie mit Parallelgruppendesign war die Beurteilung der Wirksamkeit und Sicherheit von Sacubitril/Valsartan im Vergleich zu Enalapril hinsichtlich Morbidität und Mortalität bei Patienten

mit chronischer Herzinsuffizienz und reduzierter Auswurffraktion. Die Studiendauer betrug im Median 27 Monate.

Die Ergebnisse zeigen eine hochsignifikante Überlegenheit für Entresto® gegenüber der Standardtherapie mit dem ACE-Hemmer Enalapril: Der in der Studie definierte primäre Endpunkt (kardiovaskulärer Tod oder erste Hospitalisierung wegen Herzinsuffizienz) konnte um 20 % gesenkt werden. Dabei nahm das kardiovaskulär bedingte Mortalitätsrisiko im Vergleich zu Enalapril unter Entresto® um 20 % ab (Abb. 3), die HI-bedingten Hospitalisierungen reduzierten sich um 21 %. Die Gesamtsterblichkeit sank unter der Behandlung mit Sacubitril/Valsartan im Vergleich zu Enalapril um 16 %. Der Vorteil des ARNI gegenüber dem Vergleichspräparat Enalapril war konsistent über alle Risikogruppen [5].

tan im Vergleich zur Therapie mit dem ACE-Hemmer Enalapril eindeutig und signifikant war. Überzeugendes Sicherheitsprofil In der PARADIGM-HF-Studie erwies sich Sacubitril/Valsartan als sicher und gut verträglich. Die häufigsten Nebenwirkungen, die mit der Einnahme von Entresto® einhergingen, waren Hyperkaliämie, niedriger Blutdruck, eine Verschlechterung der Nierenfunktion sowie Husten (Abb. 4) [5]. Insgesamt kam es unter Entresto® zu weniger Therapieabbrüchen aufgrund eines unerwünschten Ereignisses als unter Enalapril (10,7 vs. 12,3 %). 76 % der Patienten blieben bis zum Ende der Studie bei der Zieldosis von 200 mg 2× täglich Sacubitril/Valsartan [5].

Problemlose Umstellung Wie erste Praxiserfahrungen zeigen, verläuft die Umstellung auf den Angiotensin-Rezeptor-Neprilysin-Inhibitor unkompliziert. Bei vorheriger Behandlung mit einem AT1-Blocker kann direkt mit der Behandlung mit Entresto® begonnen werden, wogegen bei Patienten, die zuvor mit einem ACE-Hemmer behandelt wurden, nach dessen Absetzen eine Auswaschphase von 36 Stunden eingehalten werden muss.

Die empfohlene Anfangsdosis von Entresto® beträgt eine Tablette mit 49 mg/51 mg 2× täglich. Die Anfangsdosis sollte nach 2–4 Wochen, sofern der Patient dies verträgt, verdoppelt werden, bis die Zieldosis von Entresto® 97 mg/103 mg 2× täglich erreicht ist.

Eine reduzierte Anfangsdosis von Entresto® mit 24 mg/26 mg 2× täglich empfiehlt sich bei Patienten, die

• nicht mit einem ACE-Hemmer oder einem AT1-Blocker vorbehandelt wurden, • nur eine niedrige Dosierung eines ACE-Hemmers oder eines AT1-Blockers erhielten, • schwere Nierenfunktionsstörungen (GFR <30 ml/min/1,73 m2) haben oder • eine mittelschwere Leberfunktionsstörung Child-Pugh B oder AST/ ALT-Werte von mehr als dem Zweifachen der Obergrenze des Normalwertes haben.

Bei einer Anfangsdosis von 24 mg/26 mg sollte die Dosis nach 3–4 Wochen auf 49 mg/51 mg zweimal täglich und nach weiteren 3–4 Wochen auf die Zieldosis von 97 mg/103 mg zweimal täglich erhöht werden.

Für den Arzt gilt es bei der Verlaufskontrolle zu beachten, dass unter der Behandlung mit Entresto® der kardiale Biomarker BNP gegenüber NTproBNP an Aussagekraft verliert, da der Abbau von BNP durch den ARNI gehemmt wird und nicht mehr mit dem

durch die Krankheit hervorgerufenen Wert korreliert.

In Europa haben die meisten Menschen unter 80 Jahren mit koronarer Herzkrankheit (KHK) trotz lipidsenkender Therapie erhöhte LDL-Werte von über 70 mg/dl und mehr als jeder Vierte leidet an einem Diabetes mellitus.

Dies zeigen die Ergebnisse der Studie EUROASPIRE* IV [1]. Die meist verschriebenen lipidsenkenden Medikamente sind Statine, doch zeigen die Ergebnisse, dass nur sehr wenige Patienten mit ihnen leitliniengerechte Zielwerte erreichen. Als Ergänzung wird die Kombination Statin plus Ezetimib empfohlen [2].

EUROASPIRE IV zeigt Grenzen der Statin-Therapie auf Die europaweite Studie EUROASPIRE IV untersuchte in 78 Zentren in 24 europäischen Staaten die Krankenhausdaten von über 16.000 Patienten und wertete Interviews von fast 8.000 Patienten aus [1]. Alle Patienten hatten eine diagnostizierte KHK, waren unter 80 Jahre alt (Durchschnittsalter 62,5±9,6) und hatten bereits einen koronaren Bypass, eine perkutane Koronarintervention (PCI) oder ein akutes Koronarsyndrom (ACS). Mithilfe der Krankenhausdaten wurden diese Patienten identifiziert, und dann nach mindestens 6 Monaten nachuntersucht und interviewt.

Aufgrund der Einschlusskriterien hatten die erfassten Hypercholesterin­ ämie-Patienten ein sehr hohes kardiovaskuläres Risiko. Gerade für diese Patienten ist es besonders wichtig, den leitliniengerechten LDL-Zielwert von unter 70 mg/dl zu erreichen, damit die Progression von Plaques aufgehalten werden kann [3]. Doch fast alle ermittelten LDL-Werte lagen darüber.

der Vierte (26,8 %) Diabetes, was einen weiteren, entscheidenden Risikofaktor darstellt [4].

Basierend auf den Ergebnissen der Vorgängerstudien EUROASPIRE I bis EUROASPIRE III zeigt EUROASPIRE IV, dass seit dem Jahr 1995 der größte proportionale Anstieg in der Verschreibung von lipidsenkenden Medikamenten stattgefunden hat. So erhielten fast alle Patienten (86,6 %) in der EUROASPIRE IV lipidsenkende Medikamente. Mit 85,7 % waren dies größtenteils Statine. Doch zeigen die Ergebnisse auch, dass diese Therapie meist nicht ausreicht, um die Patienten auf den Zielwert einzustellen [1]. So führt eine Verdoppelung der Statindosis im Schnitt nur zu einer zusätzlichen LDL-Senkung von 6 % [5] und ist mit einer überproportionalen Zunahme des Nebenwirkungsrisikos assoziiert [6].

Starke Alternative: Kombinationstherapie mit Statin plus Ezetimib Als Alternative für viele Patienten bietet sich die Kombination eines Statins mit dem Cholesterinresorptionshemmer Ezetimib an. Der Cholesterinspiegel wird durch das Prinzip der dualen Hemmung gleich von zwei Seiten gesenkt: Das Statin bremst die hepatische Cholesterin-Synthese, während Ezetimib die intestinale Cholesterin-Resorption vermindert [7]. In Studien konnte bereits belegt werden, dass durch die Zugabe von Ezetimib zu einer Statin-

therapie eine zusätzliche Senkung des LDL-Cholesterins erzielt werden kann [8, 9]. Selbst in relativ niedrigen LDLBereichen (Ausgangswerte von im Durchschnitt 93,8 mg/dl) konnte durch die Kombination aus Simvastatin und Ezetimib in der IMPROVE-IT**-Studie bei kardiovaskulären Hochrisikopatienten eine stärkere LDL-Senkung erzielt werden als durch Simvastatin allein [10].

Außerdem zeigte die tiefere Senkung auch eine signifikante Wirkung auf die kardiovaskuläre Ereignisrate. So verringerte sich durch die zusätzliche Reduktion der LDL-Werte von etwa 70 mg/dl auf 54 mg/dl das relative Risiko für schwere kardiovaskuläre Ereignisse (CV-Tod, Myokardinfarkt, Schlaganfall, dringende Koronarrevaskularisation, instabile Angina pectoris) um 6,4 % unter der Kombinationstherapie gegenüber der Statin-Monotherapie (p=0,016). Unter den 18.000 in der IMPROVE-IT-Studie untersuchten Hypercholesterinämie-Patienten waren 27 % Patienten mit Diabetes [10].

KONGRESSE tin plus Ezetimib) gefunden: Sie sind jetzt auch angezeigt zur Risikoreduktion kardiovaskulärer Ereignisse bei Patienten mit koronarer Herzkrankheit (KHK) und akutem Koronarsyndrom in der Vorgeschichte, unabhängig von einer Vorbehandlung mit einem Statin [11, 12].

Beide Präparate sind zudem begleitend zu einer Diät indiziert bei Patienten mit primärer (heterozygoter familiärer und nicht familiärer) Hypercholesterinämie oder gemischter Hyperlipidämie, bei denen eine Therapie mit einem Statin alleine nicht ausreicht oder die bereits mit einem Statin und Ezetimib behandelt werden. Außerdem können Tioblis® und Goltor® bei Patienten mit homozygoter familiärer Hypercholesterinämie begleitend zu einer Diät zur Senkung von LDL- und Gesamtcholesterin eingesetzt werden. Goltor® ist zusätzlich für Jugendliche ab 10 Jahren zugelassen. Die Behandlung ist hier durch einen Spezialisten einzuleiten.

Typ-2-Diabetes:

Mit oraler TripleTherapie hohe HbA1cWerte kontrollieren

Neue Daten haben gezeigt, dass eine konsequente Blutzuckersenkung im Frühstadium des Typ-2-Diabetes das Fortschreiten der Erkrankung verlangsamen kann. Aktuelle Empfehlungen der American Association of Clinical Endocrinologists (AACE) und des American College of Endocrinology (ACE) sehen daher schon zur initialen Behandlung eine Dreifachkombination vor, sofern die HbA1c-Werte zum Zeitpunkt der Erstdiagnose hoch sind. Auf einem Symposium von AstraZeneca im Rahmen der DDG-Jahrestagung 2016 wurden Daten zur Dreifachkombination aus Metformin, Saxagliptin und Dapagliflozin präsentiert, die eine bessere glykämische Kontrolle im Vergleich mit einer Metformin-basierten Zweifachkombination aufzeigen.

Zur Therapie des Typ-2-Diabetes steht heute eine Auswahl an Antidiabetika mit verschiedenartigen, teilweise sich ergänzenden Wirkmechanismen zur Verfügung. „So ist es möglich, neben der Senkung des Blutglukosespiegels über pleiotrope Effekte das Gesamtbild der Erkrankung zu adressieren“, erläuterte Professor Diethelm Tschöpe, Bad Oeynhausen, und ergänzte: „Eine besondere Rolle spielen hierbei die SGLT-2-Hemmer, die neben einer effektiven Reduktion der Blutzuckerspiegel eine Gewichtsreduktion begünstigen und zu einer Blutdrucksenkung beitragen.“ Dies steht im Einklang mit Studien, die einen positiven Einfluss der SGLT-2-Hemmer auf das kardiovaskuläre Outcome zeigen. Der insulinunabhängige Wirkmechanismus macht SGLT-2-Hemmer darüber hinaus besonders interessant für die Kombinationstherapie, so Tschöpe.

Optimale Einstellung von Anfang an Für Patienten, die schon bei Beginn der Behandlung hohe HbA1c-Werte aufweisen, ist der initiale Einsatz einer Dreifachkombination der sequenziellen Therapieeskalation überlegen, wie PD Dr. Jens Aberle, Hamburg, anhand der EDICT-Studie demonstrierte: Eine unmittelbar nach Diagnosestellung begonnene Triple-Therapie führte zu einer deutlich nachhaltigeren HbA1cSenkung als eine Metformin-Monotherapie, die schrittweise um Sulfonylharnstoff und Insulin ergänzt wurde.

Aberle stellte außerdem die Ergebnisse einer Studie vor, die eine orale Dreifachkombination aus Metformin, dem DPP-4-Hemmer Saxagliptin und dem SGLT-2-Hemmer Dapagliflozin mit zwei Zweifachkombinationen – Metformin mit Saxagliptin oder Dapagliflozin – verglich. Nach 24 Wochen war der HbA1c-Wert unter der oralen Triple-Kombination gegenüber dem Ausgangswert um durchschnittlich 1,47 % gesunken. Für die duale Kombination von Metformin mit Dapagliflozin lag diese Differenz bei –1,20 %, für Saxagliptin plus Metformin bei –0,88 %.

Nach Ergänzung einer bestehenden Zweifachkombination aus Metformin und Dapagliflozin um Saxagliptin ließ sich gegenüber Placebo eine weitere HbA1c-Wert-Reduktion (Δ HbA1c –0,35 %) erzielen und mehr Patienten erreichten den HbA1c-Zielbereich von <7 %. Noch eindrucksvoller war der therapeutische Zugewinn, wenn eine Kombination aus Saxagliptin und Metformin um Dapagliflozin erweiterte: Hier lag der HbA1c-Wert nach 24 Wochen um 0,72 % unter dem Wert der um Placebo ergänzten Zweierkombination.

Das Verträglichkeitsprofil der TripleKombination unterschied sich nicht von dem der Zweifachkombinationen.

Dabei war das Hypoglykämierisiko sowohl unter der Dreifach-, als auch unter den beiden Zweifachkombinationen niedrig.

Eine Triple-Therapie bietet sich demnach vor allem bei Patienten an, die zum Zeitpunkt der Diagnose hohe HbA1c-Werte aufweisen.

Moderne NOAKTherapie mit Edoxaban Seit Juni 2015 ist der einmal täglich einzunehmende Faktor-Xa-Inhibitor Edoxaban (Lixiana®) aus der Substanzklasse der Nicht-VKA oralen Antikoagulanzien (NOAK) in der EU zugelassen. Er ist indiziert zur Prophylaxe von Schlaganfällen und systemischen embolischen Ereignissen (SEE) bei Patienten mit nicht valvulärem Vorhofflimmern (nvVHF) sowie zur Behandlung und Rezidivprophylaxe von tiefen Venenthrombosen (TVT) und Lungenembolien (LE).

Im Rahmen eines Pressegesprächs von Daiichi Sankyo in Pfaffenhofen stellte Professor Stephan Willems, Hamburg, die zentralen Studiendaten von Edoxaban vor und berichtete anhand von drei Patientenbeispielen von seinen Erfahrungen mit Edoxaban im Behandlungsalltag.

Edoxaban in der Schlaganfallprophylaxe Die Wirksamkeit und Sicherheit von Edoxaban zur Schlaganfallprophylaxe bei Patienten mit nvVHF wurde in der Phase-III-Studie ENGAGE AF-TIMI 48 untersucht. Die jährliche Inzidenz für Schlaganfälle und SEE lag unter Edoxaban einmal täglich 60 mg (inkl.

30 mg Dosisreduktion) bei 1,18 % versus 1,50 % unter Warfarin (HR=0,79; 97,5%-KI=0,63–0,99; p<0,001 für Nicht-Unterlegenheit). Gleichzeitig war die Rate schwerer Blutungen signifikant um 20 % reduziert (2,75 % vs. 3,43 % pro Jahr; HR=0,80; 95%-KI=0,71– 0,91; p<0,001). Edoxaban ist damit in der Prophylaxe von Schlaganfällen und SEE bei Patienten mit nvVHF gegenüber Warfarin vergleichbar wirksam und sicherer.

teralen Antikoagulans über mindestens 5 Tage). „Die Einmalgabe ist bei vielen Patienten ein wichtiges Kriterium für die Umstellung von einem Vitamin-KAntagonisten auf Edoxaban, so auch bei meiner nvVHF-Patientin Doris K.“, erklärte Willems (Tab. 1). Nach einem Schlaganfall 2011 wurde die inzwischen 78-Jährige mit Phenprocoumon antikoaguliert. Ein Schlaganfall zählt bereits 2 Punkte auf der Skala des CHA2DS2-VASc-Score, einer klinischen Risikoanalyse für das Auftreten eines Schlaganfalls bei Menschen mit Vorhofflimmern. Bei 2 Punkten ist gemäß der aktuellen ESC-Leitlinie eine Antikoagulation bereits zwingend erforderlich. Alter, Geschlecht und Hypertonus erhöhen den CHA2DS2VASc-Score von Doris K. um weitere

4 Punkte und damit auch das Schlaganfall-Rezidivrisiko erheblich. „Die Therapie mit Phenprocoumon ist jedoch problematisch: Der INR-Wert ist häufig instabil, das heißt, er liegt dann außerhalb des therapeutischen Zielbereichs von 2,0–3,0.“ Das einfache Therapiemanagement der einmal täglichen Gabe von Edoxaban sieht Willems als Vorteil: „Wir gehen davon aus, dass dies die Compliance positiv beeinflusst.“ Darüber hinaus belegt eine Subgruppenanalyse der Studie ENGAGE AF-TIMI 48 die Wirksamkeit und Sicherheit speziell für die Gruppe der älteren Patienten: Aufgrund des höheren Risikos für Schlaganfall und insbesondere auch Blutungen war der absolute Nutzen von Edoxaban bei älteren Patienten in allen Behandlungsgruppen © Verlag PERFUSION GmbH

tendenziell größer. Auch dies trägt laut Willems zur Therapieentscheidung bei älteren Patienten wie Doris K. bei.

Anpassung der Edoxaban-Therapie an das individuelle Risikoprofil des Patienten Bei bestimmten klinischen Faktoren wird die Edoxaban-Dosis von 60 auf 30 mg einmal täglich reduziert. Dazu zählen ein geringes Körpergewicht (≤60 kg), eine Komedikation mit den P-Glykoprotein-Inhibitoren Ciclosporin, Dronedaron, Erythromycin oder Ketoconazol sowie eine eingeschränkte Nierenfunktion (KreatininClearance 15–50 ml/min). Dies ist vor allem bei der Behandlung älterer und multimorbider Patienten relevant, wie das Beispiel von Helmut P. zeigt (Tab. 2). Der 83-Jährige leidet unter par­ oxysmalem Vorhofflimmern. Als Komorbiditäten kommen eine koronare Herzkrankheit, ein Hypertonus, eine Hyperlipoproteinämie und eine eingeschränkte Nierenfunktion mit entsprechenden Begleitmedikationen dazu.

Sein CHA2DS2-VASc-Score liegt bei 4. „Edoxaban bietet die Möglichkeit, die Therapie bei Patienten wie Helmut P. individuell an das klinische Profil anzupassen“, so Willems. Initiiert wurde die Therapie bei Helmut P. mit der Standarddosierung von 60 mg Edoxaban einmal täglich. Im Verlauf erhöhte sich sein Kreatinin-Wert und die Kreatinin-Clearance sank auf 46 ml/min. „Gemäß der Zulassung war deshalb eine Halbierung der Edoxaban-Dosis auf 30 mg einmal täglich indiziert“, erklärte der Kardiologe. „Um die Sicherheit der Therapie zu gewährleisten, führen wir in einem solchen Fall eine engmaschige Kontrolle der KreatininWerte durch.“ In der Studie ENGAGE AF-TIMI 48 waren Patienten, die Edoxaban in der reduzierten Dosis von 30 mg erhielten, im Vergleich zum entsprechenden Patientenkollektiv der Warfarin-Gruppe vergleichbar gut vor Schlaganfällen geschützt wie Patienten ohne Dosisreduktion. Edoxaban in der VTE-Therapie Die Wirksamkeit und Sicherheit von Edoxaban in der Behandlung und Prophylaxe venöser Thromboembolien (VTE) wurde in der Phase-III-Studie Hokusai-VTE nachgewiesen. Die Besonderheit von Hokusai-VTE war das praxisnahe Studiendesign. Es wurde ein breites Patientenspektrum einge-

schlossen und die Antikoagulation konnte vom Arzt – je nach klinischem Befund – flexibel für 3–12 Monate durchgeführt werden. Die Auswertung erfolgte unabhängig von der tatsächlichen Therapiedauer immer nach 12 Monaten. Das zentrale Ergebnis: Mit einer numerisch geringeren Inzidenz an rezidivierenden symptomatischen VTE (TVT und LE, tödlich und nicht tödlich) war Edoxaban gegenüber Warfarin nicht unterlegen (3,2 % vs.

3,5 %; HR=0,89; 95%-KI=0,70–1,13; p<0,001 für Nicht-Unterlegenheit).

Langzeittherapie nach initialer Heparin-Gabe Diese Studiendaten lassen sich laut Willems auch in die klinische Praxis übersetzen. Dies veranschaulichte er am Beispiel der Patientin Margit S., 38 Jahre (Tab. 3). Die Ärztin klagte nach einem Langstreckenflug über Abgeschlagenheit, Luftnot und einen leichten Brustschmerz. Darüber

hinaus war ihr linker Unterschenkel angeschwollen. Mittels D-DimerTest, Thorax-Computertomografie und EKG wurde die Diagnose „tiefe Venenthrombose mit beidseitiger Lungenembolie“ gestellt. „Auch auf Wunsch der Patientin, die selbst Ärztin ist, wurde eine Heparin-Initialtherapie gewählt.“ Die betroffene Patientin erhielt initial für 5 Tage 2× täglich 60 mg Enoxaparin und wurde im Anschluss daran auf Edoxaban in der Standarddosierung von 60 mg 1× täglich eingestellt. „Mit dieser Strategie machen wir in der klinischen Praxis gute Erfahrungen“, so Willems abschließend.

Heart Failure 2016: Neue Studiendaten zu Ivabradin Die anlässlich des Heart-Failure-Kongresses 2016 in Florenz vorgestellte neue ESC-Leitlinie zur Herzinsuffizienz bestätigt die Empfehlung, Betablocker bei Patienten mit chronischer systolischer Herzinsuffizienz und hoher Herzfrequenz (HF) frühzeitig mit Ivabradin zu kombinieren, um eine signifikante Reduktion der kardiovaskulären Mortalität sowie Herzinsuffizienz-bedingter Hospitalisierungen zu erreichen. Im Fokus standen überdies Daten einer neuen Analyse zur SHIfTStudie, die eine substanzielle Reduzierung früher Rehospitalisierungen nach der ersten Herzinsuffizienz-bedingten Hospitalisierung durch Ivabradin aufzeigten.

ESC-Leitlinie empfiehlt Ivabradin als Standardtherapie In der aktualisierten ESC-Leitlinie 2016 werden bei chronischer systolischer Herzinsuffizienz unverändert ACE-Hemmer, Betablocker, MR-Antagonisten und bei einer LVEF ≤35 % und weiteren Symptomen ARNI (statt ACE-Hemmer) sowie bei einer HF ≥70/min Ivabradin als Standardtherapien empfohlen. Vor dem Hintergrund der suboptimalen Implementierung der früheren Leitlinie im Praxisalltag wird explizit betont, alle empfohlenen Therapien konsequent zu kombinieren, um eine effektive Reduktion der kardiovaskulären Mortalität und Morbidität zu erreichen, wie Professor Piotr Poni­ kowski, Wroclaw (Polen), berichtete.

fessor Christoph Maack, Homburg/ Saar, erläuterte, reduzierte Ivabradin signifikant sowohl den primären kombinierten Endpunkt aus kardiovaskulärem Tod und Hospitalisierung aufgrund von Herzinsuffizienz um 18 % (p<0,0001) als auch die sekundären Endpunkte Hospitalisierung bzw. Tod aufgrund Herzinsuffizienz um je 26 % (p<0,0001 bzw. p=0,014).

Einen besonders großen Handlungsbedarf hinsichtlich einer besseren Umsetzung der ESC-Leitlinie machten die Experten bei der frühen Therapieoptimierung nach Stabilisierung einer akut entgleisten Herzinsuffizienz im Krankenhaus aus. Denn jede Hospitalisierung infolge akuter Herzinsuffizienz sowie eine zu hohe HF bei Entlassung erhöhen insbesondere in der vulnerablen Phase 30–90 Tage nach dem stationären Aufenthalt das Risiko für erneute Einweisungen, aber auch die Sterblichkeit. Daher sollten solche Patienten direkt nach ihrer Stabilisierung im Krankenhaus bei trotz Betablocker-Therapie hoher HF unverzüglich auf Ivabradin eingestellt werden, betonte Prof. Dr. Michel Komajda, Paris (Frankreich), unter Verweis auf eine Analyse der SHIfT-Studie. Darin wurden jene 1.158 Patienten mit ≥1 Herzinsuffizienz-bedingten Hospitalisierung erfasst, von denen 28 % binnen 3 Monaten erneut Perfusion 3/2016

hospitalisiert werden mussten. Nach 1 und 3 Monaten führte Ivabradin zu einer signifikanten Reduktion sämtlicher Rehospitalisierungen (IRR 0,70 bzw. 0,79; je p<0,05) (Abb. 1). Für die Reduktion kardiovaskulär bzw. Herzinsuffizienz-assoziierter Einweisungen zeigte sich bereits nach einem Monat ein starker Trend zugunsten der Iva­ bradin-Therapie (IRR 0,66 bzw. 0,67; p=0,05 bzw. p=0,13).

RELIf-CHF-Studie: BetablockerDosis nicht ausschlaggebend Dass für die mit Ivabradin zu erreichenden Therapieeffekte die Höhe der Betablocker-Dosis in der täglichen Praxis nur eine untergeordnete Rolle spielt, legen ebenfalls aktuell auf dem Heart Failure 2016 vorgestellte 1-Jahres-Daten der von Professor Christian Zugck, Straubing, präsentierten nicht interventionellen RELIf-CHF-Studie mit 767 überwiegend mit Betablocker vorbehandelten Herzinsuffizienz-Patienten nahe. Im Vergleich zum 12-Monatszeitraum vor der Einstellung auf Ivabradin kam es in den 3 Subgruppen mit <50, 50–99 und ≥100 % der Betablocker-Zieldosis nach der Etablierung der Kombinationstherapie zu einer signifikanten Abnahme der Herzinsuffi-

zienz-assoziierten Hospitalisierungen von 19 auf 5, 31 auf 6 und 29 auf 3 % (alle p<0,001). Parallel dazu nahm in allen Subgruppen der Anteil von Patienten mit Zeichen einer Dekompensation nach 12 Monaten signifikant ab (alle p<0,001). Zugleich sank auch der Anteil von Patienten in NYHA-Klasse III um 25, 33 und 31 %, während jener in NYHA-Klasse I anstieg (alle p<0,0001). Diese symptomatischen Verbesserungen spiegelten sich zudem in einem Anstieg der Lebensqualität im EQ-5D-Index um 20, 26 und 19 % wider (alle p<0,0001). Sowohl hinsichtlich der Effektivität als auch der überwiegend sehr guten Verträglichkeit dieser Kombinationsstrategie wurde keine signifikante Beeinflussung des mit Ivabradin erreichten Therapienutzens durch die Höhe der BetablockerDosis festgestellt.

Insulintherapie bei Typ-2-Diabetes: Frühzeitig mit Basalinsulin beginnen „Hypoglykämien gehören zu den größten Barrieren für den Beginn einer Insulintherapie bei Typ-2-Diabetes. Es ist somit notwendig, Insuline mit einem möglichst niedrigen Unterzuckerungsrisiko zu verwenden“, erklärte Dr. Stefan Gölz, Esslingen, auf einer Fachpressekonferenz in Frankfurt und ergänzte: „Insulin glargin U300 (300 E/ ml, Toujeo®) zeigt bei Menschen mit Typ-2-Diabetes eine vergleichbare Blutzuckereinstellung mit einem geringeren Hypoglykämierisiko als Insulin glargin U100 (100 E/ml, Lantus®).

digt ist. Für den frühzeitigen Einsatz von Insulin sprechen eine Reihe von Argumenten: Insulin ist die physiologische Antwort auf erhöhte Blutzuckerwerte, lässt sich individuell bis zum gewünschten Zielwert titrieren, ermöglicht so eine normnahe Blutzuckereinstellung und entlastet die Betazelle. Trotzdem wird in Deutschland eine Insulintherapie spät begonnen: Im Mittel sind die Patienten zu Beginn bereits mehr als 8 Jahre an Typ-2-Diabetes erkrankt mit einem HbA1c-Wert von 8,4 %. Als mögliche Gründe für das zögerliche Vorgehen bei der Intensivierung der antihyperglykämischen Therapie nannte Gölz neben Sorgen vor Einschränkungen im Alltag bzw. einer Gewichtszunahme insbesondere die Angst vor Hypoglykämien.

Einfach zu verordnen

Mit Insulin glargin U300 das Hypoglykämierisiko senken

Boehringer Ingelheim veröffentlichte kürzlich die Ergebnisse einer internationalen Umfrage, an der 900 Patienten mit Vorhofflimmern (VHF) teilgenommen hatten. Die Ergebnisse zeigen: 9 von 10 Befragten sorgen sich um ihr Schlaganfallrisiko. Zur Verringerung dieses Risikos werden vorbeugend orale Antikoagulanzien (OAK) verordnet.

Die Mehrheit der Patienten fragt dabei aktiv nach Informationen zur Therapie und möchte in die Wahl des OAK miteinbezogen werden. Diese und andere Resultate der internationalen Umfrage wurden auf der zweiten Konferenz der European Stroke Organisation vorgestellt, die vom 10. bis 12. Mai in Barcelona stattfand.

Besonders einfach ist der Einstieg in die Insulintherapie mit einer basalunterstützen oralen Therapie (BOT) umzusetzen: Das Basalinsulin wird einmal täglich zusätzlich zu oralen Antidiabetika verabreicht. „Die Auswahl des Basalinsulins kann dazu beitragen, dass mehr Menschen mit Diabetes der Balanceakt zwischen guter Blutzuckereinstellung und Vermeidung von Hypoglykämien gelingt“, betonte Gölz. Die Therapie mit Insulin glargin U300 ist bei Menschen mit Typ-2-Diabetes bei vergleichbarer Blutzuckereinstellung mit einem geringeren Hypoglykämierisiko assoziiert als mit Insulin glargin U100 – insbesondere in der sensiblen Titrationsphase (Woche 1–8). Insulin glargin U300 verfügt darüber hinaus im Vergleich zu Insulin glargin U100 über eine stabilere und länger anhaltende Wirkung. Die Patienten können mit einer größeren Flexibilität davon profitieren: Zu Beginn der Therapie ist es möglich, den Injektionszeitpunkt frei zu wählen, im weiteren Verlauf erfolgt die Injektion jeden Tag in einem Zeitfenster von plus/minus 3 Stunden.

Und: Unter Insulin glargin U300 fällt die Gewichtszunahme bei Patienten mit Typ-2-Diabetes etwas geringer aus als unter U100.

„Für mich als niedergelassener Arzt ist es wichtig, dass durch die Rabattverträge, die mit allen Gesetzlichen Krankenversicherungen (GKV) geschlossen wurden, die Erstattungsfähigkeit von Insulin glargin U300 gegeben ist und weil Insulin glargin U300 und U100 jeweils den gleichen Wirkstoff enthalten, kein AMONG-Verfahren notwendig ist“, unterstrich Gölz abschließend.

Patienten mit Vorhof­ flimmern wollen in Therapieentscheidungen einbezogen werden

Hoher Stellenwert von Kommunikation und Information „Erstmals liegen uns fundierte Daten zur Sichtweise und zu den Therapiepräferenzen von Patienten mit Vorhofflimmern aus vielen Ländern vor“, kommentierte die Studienleiterin, Dr. Deirdre Lane, Universität Birmingham. „Eine zentrale Erkenntnis ist, dass viele VHF-Patienten in die Entscheidung über ihr OAK miteinbezogen werden wollen. Dieser Wunsch spiegelt die Empfehlungen der aktuellen Leitlinien wider, die zur Berücksichtigung der

Patientenpräferenz bei der Wahl des Antikoagulans raten.“ Die Umfrage verdeutlicht, welchen hohen Stellenwert die Kommunikation zwischen Patienten und ihrem behandelnden Arzt hat: Mehr als 3 Viertel (76 %) der VHF-Patienten gaben an, dass der Arzt ihre wichtigste Informationsquelle zur Therapie mit einem OAK sei. Insgesamt konsultierten Patienten durchschnittlich 2–3 Informationsquellen. Das Patientenwissen zu Ursachen, Anzeichen und Symptomen eines Schlaganfalls war dagegen sehr unterschiedlich: Nur etwa die Hälfte (48 %) verfügte über gute bis mäßige Kenntnisse über die Symptome, Ursachen und Risikofaktoren eines Schlaganfalls.

Dabei beeinflusst das Wissen um den Schlaganfall die Therapiepräferenzen. Patienten mit guten bis mäßigen Kenntnissen: • ordneten die Schlaganfallvorsorge als wichtigen Faktor bei der OAKWahl höher ein (65 % bzw. 54 %; durchschnittlich 47 %) • zogen es häufiger vor, in die Entscheidung über die OAK-Therapie mit einbezogen zu werden (73 % bzw. 62 %; durchschnittlich 56 %). „Schlaganfälle durch Vorhofflimmern zeigen besonders schwere Folgen.

Sie können Behinderungen nach sich ziehen, die eine lebenslange Betreuung erfordern oder gar zum Tod führen”, erklärte Jon Barrick, Präsident der „Stroke Alliance for Europe“. Die Non-Profit Organisation repräsentiert Patientengruppen aus ganz Europa und setzt sich für eine verbesserte Versorgung von Schlaganfall-Patienten ein. „Es ist entscheidend, Patienten mit Vorhofflimmern zu motivieren, aktiv an der Behandlung ihrer Erkrankung teilzunehmen. Das ist besonders wichtig, da die chronische Herzrhythmusstörung eine langfristige Behandlung erfordert. Ein offenes Gespräch mit dem Arzt zum Gesundheitszustand, dem Umgang mit der Erkrankung und den Behandlungsmöglichkeiten zur Verringerung des Schlaganfallrisikos ist dabei unerlässlich.”

A possible in vitro setup to measure the potential of anti-inflammatory compounds is the simultaneous challenge of murine macrophages with LPS and the molecule of interest, and the determination of the released NO in the supernatants using the Griess reagent. This colorimetric assay is challenging when applied to cell culture supernatants. From our experience, the density of cells and the times of incubation are crucial. We therefore systematically investigated the effect of the (pre)-incubation time with molecules of interest and the time and duration of the LPS challenge on the responsiveness of murine macrophages.

Methods: Murine RAW264.7 macrophages, a cell-line well established for measuring the effects of compounds on NO formation, were (pre)-incubated with the compounds of interest for Perfusion 3/2016

various times and the cells were then challenged with LPS. In case of antiinflammatory molecules, a reduced NO production compared to the controls treated with only LPS is detected. After incubation, the supernatants were centrifuged and mixed with 130 µl deionized water and 20 µl Griess reagent.

After 30 min incubation in the dark, the extinction was measured at 544 nm.

Results: In a systematic way, the incubation times were adjusted, starting with varying pre-incubation periods with known anti-inflammatory compounds (i.e. the long-chain metabolites (LCMs) of tocopherols, α-13’-OH and α-13’-COOH). A total incubation time of 24 h was split into pre-incubation and LPS challenge (4 h LCMs + 20 h LCMs and LPS, 6 h + 18 h, 8 h + 16 h and 10 h + 14 h). The combination of 4 h pre-incubation and 20 h LPS revealed the strongest and most reliable induction of NO in the LPS control and resulted also in the largest inhibition of the NO release. In the next step, the period of LPS challenge was set to 20 h, but pre-incubation times were varied (4 h + 20 h, 6 h + 20 h, 8 h + 20 h). Also in this test matrix, the combination of 4 h pre-incubation and 20 h LPS revealed best results. The last optimization was made by changing the mode of application of the anti-inflammatory compounds to the cells when LPS was given. In the first experiments, LPS was just added into the pre-incubation medium, then a pulse chase or a complete change of incubation medium

were tested. The anti-inflammatory potential of the LCMs was best when a complete change of incubation medium was used. In our optimized setup for the determination of the anti-inflammatory capacity of compounds we use now a pre-incubation time of 4 h, followed by a second treatment with new incubation medium and a simultaneous stimulation with LPS for 20 h.

In our hands, this gives the strongest and most reliable LPS response and the best substance effects for RAW264.7 cells and the tested LCMs. Conclusion: It is valuable to optimize long-established methods to reveal the best results possible. Some simple modifications to the standard protocol allowed us to measure NO production in a more reliable and routine way.

tigate the uptake of triglyceride-loaded low density lipoproteins (LDL) by macrophages and the distribution and storage of the triglyceride provided by the lipoproteins. Especially modified lipoproteins, e.g. oxidized LDL (oxLDL), are of interest since they play a key role in the early stages of atherosclerotic plaque development.

Methods: LDL was isolated from human blood plasma, oxidized with CuSO4 and subsequently loaded with deuterated tripalmitate. Macrophages were differentiated from THP-1 monocytes and incubated with 50 µg/ mL oxLDL for 10 to 72 hours. After incubation, the cells were fixed with formalin. Raman measurements were performed with an excitation wavelength of 785 nm and Raman images were recorded at a spatial resolution of 1 μm using a 60x/NA=1 water immersion objective.

Results: Raman spectra of LDL and oxLDL revealed β-carotene as a reliably detectable component inside the investigated lipoproteins. We chose deuterated tripalmitate, since deuterium provides a specific signal in Raman spectra. Therefore tripalmitate can be distinguished from naturally occurring lipids already inside the cells. In this study we show that β-carotene is getting stored in lipid droplets alongside deuterated tripalmitate. Small inclusions inside the cells were found which show a high lipid content, especially tripalmitate, at shorter incubation times which decreased for longer incubations. Simultaneously, the content of tripalmitate in lipid droplets increased over time.

Conclusions: Due to distinct β-caro­ tene signals in Raman spectra, the uptake of low density lipoproteins by macrophages can be traced. Since β-carotene is normally not present in THP-1 macrophages, it can be used as a marker for the successful uptake of oxLDL particles by the cells.

Emerging role of post-translational modifications in atherosclerosis Prathibha Reddy Gajjala University Hospital of the RWTH Aachen, Germany Post-translational modifications (PTMs) of proteins and peptides have recently gained much attention, as they are involved in the pathogenesis of cardiovascular disease especially during inflammation. Post-translational protein modifications such as carbamylation, glycation and oxidation are described, starting with their definitions, mechanisms and clinical relevance in the setting of atherosclerosis. The methods currently used for the identification and, in particular, quantification of PTMs are described and potential treatment options in the context of PTMs are reviewed. We foresee that advancements in mass spectrometrybased methods leading to the identification of novel disease markers and/or pathophysiologically relevant factors will certainly boost the clinical utility in sample analyses and would be an attractive clinical option in the near future.

In former studies we have shown that AMPKα1 stimulates autophagy in response to pharmacological activators and is critical for vascular endothelial growth factor (VEGF)-triggered angiogenesis. This study was aimed at investigating the stimulation of the AMPK/

autophagy pathway by VEGF and its involvement in angiogenesis.

Methods: The role of AMPK and autophagy in human umbilical vein endothelial cells (HUVEC) was studied using downregulation of AMPKα1 and/or AMPKα2 or of the autophagy initiators ULK1 and Beclin1 by specific siRNAs. In addition, the autophagy inhibitor Bafilomycin A1 was used. Cytokine production, oxidative stress and intracellular ATP content were measured using Multiplex FACS measurement, protein carbonylation in Western blot and luciferase-based ATP assay, respectively. Endothelial functions such as proliferation, survival and angiogenesis were studied with the help of cell cycle analysis and spheroid assays.

Results: HUVEC showed a pronounced basal autophagic flux. Inhibition of this flux led to an increase of cytokine release and ROS production suggesting a homeostatic function of autophagy due to protection from inflammatory and oxidative stress.

Intracellular ATP levels were not affected by autophagy inhibition. ULK1/ Beclin1-siRNA clearly reduced VEGFstimulated angiogenesis without affecting VEGF-induced cellular responses such as activation of angiogenic signalling proteins, proliferation and survival. However, basal proliferation and survival were impaired. Inhibition of autophagy by bafilomycin A1, which was not accompanied by increased cell death, also reduced VEGF-induced angiogenesis pointing to a specific angiogenic role of autophagy in regulating angiogenesis in addition to its homeostatic function. VEGF stimulated ULK1 phosphorylation thus activating autophagy. This pathway was triggered

by AMPKα1, and thus might be one of the pathways mediating the proangiogenic function of AMPKα1.

Conclusion: Autophagy under basal conditions is important for the suppression of inflammatory and oxidative stress in endothelial cells. This process is also stimulated by VEGF via AMPKα1 and is critical for VEGF© Verlag PERFUSION GmbH

Specific recruitment of immune cells to endothelium from different organs Sonja Ebenhöch, Frederic Emschermann, Harald F. Langer Dept. of Internal Medicine III, University Hospital Tübingen, Germany Aims: We investigated differences in adhesion of immune cells elicited by tissue specificity of endothelial cells.

Methods: Primary microvascular endothelial cells were isolated from murine heart and lung. Between proliferation steps, cells were purified twice by using anti-CD-102 magnetic beads.

They were used for experiments at passage 5 after checking for high purity using flow cytometry analysis to detect endothelial markers PECAM-1, ICAM-2 and VE-Cadherin. Blood brain barrier endothelial cells were isolated from the cerebral cortex by percoll gradient centrifugation and tested by immunofluorescence for von-Willebrand-Factor and absence of smooth muscle actin. Viability and their capability to build up a tight barrier was measured by trans-endothelial-electric-resistance (TEER) over several days. Endothelial monolayers were stimulated with TNF-α and cell adhesion assays were carried out either with freshly isolated splenocytes, bone marrow derived dendritic cells or macrophages from wildtype or C3a receptor knock out mice.

Results: After stimulation of endothelial cells with TNF-α, splenocytes adhere in significantly higher numbers on cells from lung and brain than from heart, whereas there are tendencies for macrophages to adhere preferably on heart and brain, but not on lung endothelium. lung and heart endothelial cells. The effect is abolished by pretreatment of dendritic cells with the anaphylatoxin C3a.

Conclusion: Site specificity of dendritic cells to endothelium from different organs can be influenced by C3a signal on its receptor.

Characterization of neuropilin-1 in human blood vessels Lilli Mey1, Sabine Pankuweit2, Anja Schwarz1, Lisa-Marie Schütz1, Ralf Kinscherf1, Nadia Al-Fakhri3 1 Institute for Anatomy and Cell Biology, Philipps University Marburg, Germany 2 Dept. of Cardiology, Philipps University Marburg, Germany 3 Center of Transfusion Medicine and Hemotherapy, Philipps University Marburg, Germany Aim: Neuropilin-1 (NP-1), a co-receptor of vascular endothelial growth factor (VEGF), is present in endothelial and smooth muscle cells, can enhance tumor growth and VEGF induced protection from apoptosis. VEGF can stimulate or inhibit the progression of arteriosclerosis. Fine-tuned regulatory mechanisms seem to influence the VEGF effect. VEGF binds to different main and alternative receptors, primarily VEGF-R1, VEGF-R2 and NP1. NP-1 can be cleaved from the cell membrane or is secreted and circulates as “soluble” form (sNP-1) in the blood stream. So far, NP-1 was mainly investigated in terms of arteriogenesis and angiogenesis, but less regarding vascular disease and arteriosclerosis. Therefore, we intended to identify NP-1 in human arteries and arteriosclerotic vessels showing a correlation between arteriosclerosis and NP-1.

Methods: 8 specimens of human saphenous vein, 8 specimens of internal mammary artery and 5 specimens of arteriosclerotic vessels were embedded in Tissue-Tek, fresh frozen and sliced into 6µm thick sections. Immunohistochemical and immunofluorescent NP-1 (Invitrogen) staining was performed

with Fast-Red or Alexa Fluor488. Human umbilical vein cells (HUVEC) were incubated for 24 h/48 h with 50–100 µg/ml oxidized LDL (oxLDL) and native LDL (nLDL). Protein was collected and analysed via Western blot and measured with automated densitometry.

Results: In human arteries we could detect NP-1 on the endothelium and in smooth muscle cells. Arteriosclerotic vessels showed a massive expression of NP-1 in the plaque. On the contrary, venous vessels showed no expression. After HUVECs were incubated for 48 h with oxLDL, there was an increase in NP-1 expression, in comparison to nLDL treated cells.

Conclusion: Assuming that NP-1 is oxLDL-triggered and present in arteries, plus is highly expressed in the neointima, a correlation between NP-1 and inflammatory processes in the pathogenesis of arteriosclerosis is obvious. Because of the absence of NP-1 in veins we suppose, that NP-1 has no important function in the venous system. Further experiments need to clarify the role of NP-1 in context of arteriosclerosis.

lial metabolism may represent a new therapeutic strategy for cardiovascular diseases. The current study aims at investigating the impact of inflammatory cytokines on glucose metabolism in endothelial cells.

Methods: The study was performed in primary human umbilical vein endothelial cells stimulated with combined tumor necrosis factor α (TNF-α), interleukin-1β (IL-1β) or lipopolysaccharide (LPS) or with IL-1β alone.

Different metabolic parameters were determined: glucose consumption and lactate content with colorimetric assays, ATP with a luminescence assay, glutathione (GSH) with a colorimetric assay, glucose uptake with radiolabeled glucose. In addition, metabolic flux through pentose phosphate pathway (PPP) was measured as 14CO2 production from C1-labelled glucose while glucose oxidation via the tricarboxylic acid (TCA) cycle and respiratory chain was detected as 14CO2 generated from C6-labelled glucose. Furthermore, incorporation of labelled glucose into total RNA was determined and HIF1α and metabolic enzymes were analysed in wWestern blots.

Results: A combination of tumour necrosis factor α (TNF-α), interleukin 1β (IL-1β) and LPS or IL-1β alone induced a remarkable enhancement of glucose consumption. This response was mediated by HIF1α, which was accumulated upon cytokine treatment in a MEK/ MAPK-sensitive manner. Inhibition of HIF1α accumulation by pharmacological or genetic means (HIF1α-siRNA) prevented cytokine-induced glucose consumption. Glucose uptake was only moderately increased. Cytokines triggered an increase of glycolysis, which was monitored by intra- and extracellular lactate levels. In addition, the PPP was stimulated in response to proinflammatory cytokines while glucose oxidation was not altered. The increased flux through the PPP was not due to an increased protein expression of its key enzyme, glucose-6-phosphate dehydrogenase. RNA when endothelial cells were incubated with labelled glucose and stimulated with proinflammatory agents suggesting that increased glucose metabolism feeds translational processes.

ATP levels were not altered in response to cytokines and GSH levels recovered quickly after an initial drop.

Conclusion: Endothelial cells are able to differentially regulate their glucose metabolizing pathways in the context of inflammation in order to adapt to their specific metabolic needs. The observed alterations suggest that increased glycolysis and PPP maintain energy production and redox control and provide biosynthetic precursors and may thus play an important role in the inflammatory response of endothelial cells.

Hypochlorite-modified LDL induces redistribution of free cholesterol in endothelial cells – implications for eNOS localisation and trafficking Dana Kürsten, Christina Valkova, Siegfried Krause, Regine Heller Institute for Molecular Cell Biology, Center for Molecular Biomedicine, University Hospital Jena, Germany Aim: Oxidized LDL (oxLDL) is known to promote endothelial dysfunction and atherosclerotic disease.

Among the oxidants potentially leading to oxidative modification of LDL, hypochloric acid (HOCl) generated by the myeloperoxidase/H2O2/halide system of activated monocytes may play a key role since HOCl-specific modifications of proteins (3-dityrosine and chlorotyrosine) as well as catalytically active myeloperoxidase were detected in atherosclerotic lesions. Previous results from our group have shown that NaOCl-LDL generated in vitro induced mislocalisation of endothelial NO synthase (eNOS) in endothelial cells, which was paralleled by a reduction of agonist-stimulated NO release.

The current study was aimed at investigating the mechanisms underlying this

observation thereby characterising this new pathway of developing endothelial dysfunction.

Methods: Experiments were performed in human umbilical vein endothelial cells. The localisation of eNOS in different subcellular compartments was detected by Western blot analyses and immunofluorescence studies using antibodies against eNOS, eNOS-interacting proteins (NOSTRIN, dynamin-2, actin), specific markers of intracellular organelles as well as lysotrackers and fluorophores reacting with free cholesterol or filamentous actin (F-actin).

Results: NaOCl-LDL led to a loss of eNOS from the plasma membrane and from the cis-Golgi apparatus, the compartments of highest eNOS activity. Redistributed eNOS was accumulated in the endoplasmic reticulum, in LAMP-1 positive late endosomes/ lysosomes, and in the trans-Golgi network, respectively. The mislocalisation of eNOS was closely related to NaOClLDL-mediated changes in the cellular distribution of free cholesterol. NaOCl-LDL caused a distinct depletion of cholesterol from the plasma membrane comparable to what was seen after short-term incubation of cells with methyl-β-cyclodextrin (MβCD), a cholesterol-binding compound, which caused eNOS disappearance from the plasma membrane. Furthermore, NaOCl-LDL was internalized into early endosomes and lysosomes where it was hydrolysed as indicated by the high amount of free cholesterol in these organelles. As a consequence, cholesterol may be released from lysosomes and accumulate in the endoplasmic reticulum (ER) leading to disturbed vesicular protein transport between ER and Golgi apparatus and subsequent eNOS trapping in non-functional compartments.

Accordingly, long-term incubation of HUVEC with MβCD known to sequester free cholesterol from lysosomes and leading to its enrichment in the ER induced a redistribution of eNOS similar to the one observed after NaOCl-LDL treatment. NaOCl-LDL also triggered a © Verlag PERFUSION GmbH

reorganisation of the actin cytoskeleton as well as a decrease of interactions between eNOS, the actin cytoskeleton, the syndapin NOSTRIN and the GTPase dynamin-2. These alterations may be closely related to changes of membrane properties as a consequence of altered cholesterol redistribution.

Conclusion: The data underline the importance of subcellular targeting of eNOS for efficient NO production and characterize HOCl-modified LDL as a pathophysiologically relevant agent that affects eNOS activity by interfering with eNOS transport and localisation.

Endothelial specific α1AMPK preserves endothelial function during chronic angiotensin II treatment Lan Phuong Tran, Swenja Kröller-Schön, Thomas Jansen, Matthias Oelze, Andreas Daiber, Thomas Münzel, Eberhard Schulz Center for Cardiology, University Medical Center of the Johannes Gutenberg University Mainz, Germany Background: The AMP activated protein kinase (AMPK) is ubiquitously expressed in mammalian cells and known for maintaining endothelial function by promoting angiogenesis, inhibition of endothelial cell apoptosis and activation of eNOS. The global knockout of α1AMPK, the vascular isoform, leads to increased endothelial dysfunction and vascular ROS during chronic angiotensin II treatment. However, the role of endothelial-specific α1AMPK has not been elucidated yet. Therefore, we studied the particular function of endothelial-specific α1AMPK in angiotensin II induced oxidative stress.

Methods and results: α1AMPKflox/flox and TekCre+ mice were used to generate an endothelial-specific α1AMPK knock-out mouse strain. Effective gene deletion was proved by immunohistochemistry of intact aortic rings and depressed mRNA and protein levels of α1AMPK in mouse lung endothelial cells (MLEC). II infusion (0,5 mg/kg/d, 7 days) led to significantly worsened endothelial dysfunction in endothelial specific α1AMPK knockout animals compared to TekCre+ mice. Furthermore ROS production measured by amplex red assay, DHE and mitoSOX staining of aortic cryosections was increased. Incubation of intact aortic rings ex vivo with a ROS scavenger (PEG SOD) improved endothelial dysfunction induced by angiotensin II significantly, which suggests a causal role of oxidative stress in endothelial damage. Besides, the angiotensin II-induced upregulation of cytoprotective heme oxygenase 2 in TekCre+ mice was depressed in endo­ thelial α1AMPK-knockout-mice. FACS analysis of aortic tissue showed significantly increased infiltration of inflammatory cells to the vessel wall. Moreover, transcription and expression of NOX2 were enhanced in aortic tissue.

Conclusion: Endothelial specific α1AMPK prevents endothelial dysfunction by defending oxidative stress and preserving the barrier function of the endothelial cell layer during angiotensin II-induced hypertension.

Endothelial cells play a key role in angiogenesis. It is known that also platelets have the capacity to modulate

angiogenesis. In a model of growth factor-induced angiogenesis we were able to show in vivo that platelets exert an antiangiogenic effect. In vitro, we were able to show that the complement receptor C5aR expressed on platelets inhibits endothelial migration and 2-D and 3-D tube formation. In a model of hindlimb ischemia, we could demonstrate that C5aR-deficient mice display a phenotype of enhanced angiogenesis. Furthermore, the angiogenesis inhibiting effect of the C5a receptor in C5aR-deficient mice was partially reversible by administration of wildtype platelets. This effect is inhibited by pharmacological platelet depletion or inhibition of the C5a receptor. In vivo, systemic platelet depletion in C5a receptor knockout mice lowers their level of angiogenesis towards the level of wildtype mice. The C5a receptors on platelets exert their effect via a secretory mechanism as confirmed by endothelial cell studies. In a PKC-dependent fashion, platelet factor 4 is released from platelets upon stimulation with C5a. In conclusion, upon stimulation with complement components platelets seem to exert an antiangiogenic effect via secretion of the antiangiogenic agent PF4.

Aims: The complement anaphylatoxin C5a functions through its two functional receptors, the C5aR1 (CD88) and C5a receptor-like 2 (C5L2, GPR77).

Several studies have shown the protective effect of C5aR inhibition in myocardial infarction (MI). However, the underlying mechanisms of C5a/ C5aR-mediated action remain elusive.

The role of C5L2 is completely unexplored. Therefore, we analyzed the role of these receptors in cardiac remodeling after MI in C5aR1- and C5L2-deficient mice.

Methods: MI was induced by temporary ligation of the left anterior descending coronary artery. Echocardiography, histology, quantitative immunohistochemistry and RT-PCR, in vitro angiogenesis and myofibroblast transdifferentiation assays were performed.

Results: A significantly reduced recruitment of neutrophils was detected 1 day after MI and monocyte infiltration 1 week after MI in C5aR1-deficient mice compared to wild-type (WT) mice. Four weeks after MI, the infarct size was reduced by 58 % in C5aR1-deficient mice in comparison to WT-mice but not in C5L2-deficient mice. Cardiac ejection fraction was improved in C5aR1-deficient mice accompanied by a reduced left ventricular diastolic volume compared to WT-mice. The reduction in infarct size was associated with a reduced neutrophil and monocyte infiltration as well as with a decreased content of collagen and myofibroblasts. Furthermore, an increased amount of pro-angiogenic CD31+ endothelial cells was detected in infarcted areas, which was confirmed by an improved tube formation in vitro. In addition, while the number of apoptotic cells was significantly reduced, the amount of proliferating cells was increased. Notably, C5a mRNA Perfusion 3/2016

and protein expression were elevated in C5aR1- and C5L2-deficient mice in infarcted cardiac tissue one week after MI when compared to WT-mice whereas transforming growth factor-beta1 mRNA and protein expression were reduced. In the presence of C5a, fibroblast-to-myofibroblast transition was reduced whereas neutralizing of C5a with an antibody promoted the transdifferentiation in vitro.

Conclusion: C5aR plays a crucial role in endogenous remodeling processes after MI whereas the gene deletion protects from cardiomyocyte injury and positively affects cardiac remodeling by contributing to the inhibition of inflammatory cell recruitment, apoptosis, myofibroblast transdifferentiation and promoting the neovascularization and proliferation. In contrast, C5L2 showed no effect.

Non-invasive up-regulation of heart preconditioning program in vivo with minimal side effects Adelina Curaj, Sakine Simseekyilmaz, Mareike Staudt, Andreas Götzenich, Andreea Urs, Mircea Leabu, Elisa A. Liehn Institute for Molecular Cardiovascular Research, University Hospital of the RWTH Aachen, Germany Despite considerable progress over the last period, acute myocardial infarction continues to remain the major cause of morbidity and mortality worldwide.

Phosphatidylserine (PS) is a phospholipid component of the inner leaflet of cell membranes and seems to be a messenger for the cellular death and an important destruction signal for the macrophages. On the other hand, PS has demonstrated some usefulness in treating cognitive impairment but also speeded up recovery, prevented muscle soreness, improved well-being, and might possess ergogenic properties. In our studies, we also found an up-regulation of PS in cxcr4-deficient hearts, as an adaptation to the poor blood supply present in these mice. The best known mechanism, which is described

to protect the heart in the absence of oxygen, is the ischemic preconditioning. Ischemic preconditioning is an experimental technique gaining resistance against the loss of blood supply in many tissues. By a short (>5 min) and repeated (2 or 3 times) impairment of the blood supply, the tissue or the organ becomes robustly protected from a predicted severe ischemic insult. In this regard, we believe that PS can play an important role in myocardial preconditioning and protection of the heart from ischemia. The effect of PS was tested in vitro on isolated cardiomyocytes undergoing hypoxia. After 3 hours of preincubation with PS, the cardiomyocytes increased significantly their protection, indicating no stress signals as analysed by Alamar blue staining after 5 hours of ischemia. Using a mouse model of myocardial infarction, we tested the effect of oral administration of PS on myocardial function and biology. The pre-treatment with PS one week before myocardial infarction induction showed a significantly preserved heart function and reduced infarction size. Starting the administration after myocardial infarction induction also reduced infarciton size, however, the hearts remained with a significantly diastolic dysfunction. No differences in apoptosis were detected, as measured by TUNEL staining in infarcted myocardium. Inflammation was also reduced, due to probably reduced tissue damages after PS administration. mRNA extraction from isolated cardiomyocytes after hypoxia and from myocardium after myocardial infarction showed a significant up-regulation of main players of preconditioning program, such as protein kinase C type ε, cyclooxygenase-2, aldose reductase, Mn superoxide dismutase.

As already known, PS side effects are rare and include only mild gastrointestinal discomfort. When taken together with other blood thinner drugs (such as warfarin, aspirin, pentoxifylline, clopidogrel, ticlopidine, garlic and vitamin E), thinning the blood was also reported. jor changes in lipid composition, lipid were extracted from heart, kidney, liver, lung after PS administration and fatty acids were analysed. We did not detect any significant changes in fatty acids or lipid composition after one week of PS administration in any analysed organs.

Moreover, the blood analysis showed normal and unchanged parameters in all groups. We demonstrated that PS can be used to activate preconditioning program of the heart, to assure a significant protection from ischemia in vitro and in vivo. We strongly believe that PS can be used in the treatment of heart failure to protect the cardiomyocytes and to adapt them to chronical hypoxic conditions, preserving their function.

Compared with other methods used in present, as remote ischemia or opioide administration, PS administration seems to be easy to perform with minimal side effects, no professional supervision being neccesary. Since PS is already in clinical use for some cognitive diseases, we believe that our findings would be of a high relevance for the cardiovascular community with immediate clinical implication.

ures, to reduce scar formation and to preserve heart function in patients suffering MI. In the present study, the role of CCR3 in myocardial ischemia/re­ perfusion was established.

Methods and results: One week after ischemia/reperfusion induction in a mouse model (coronary ligation model), the functional and morphological parameters of the heart were analyzed. Isolated-heart Langendorff perfusion showed no significant differences in heart function, infarction size and post infarction angiogenesis after pharmacological blockade of CCR3.

Apoptotic or proliferation signals as well as collagen synthesis were not affected in CCR3 antagonist-treated mice. Notably, CCR3 inhibition was accompanied by massive neutrophil infiltration, while leaving the presence of other immune cell subsets in the heart unaffected.

Conclusion: Since neutrophils represent one of the most widely explored therapeutic targets in the treatment of cardiac disease, this study may open new perspective for a better understanding of the physiology and homeostasis of neutrophils and point out new directions for intervention in acute MI.

in peripheral blood mononuclear cells (PBMCs) as determinants of human atherosclerosis is unknown. miR-34a is highly induced in diabetic mice and mice fed a high-fat diet. Moreover, miR-34a is involved in endothelial cell function and is upregulated in mouse and human atherosclerotic plaques.

The aim of the present study was to evaluate the clinical value of PBMC levels of miR-34a as determinant for human subclinical atherosclerosis and for incident coronary artery disease (CAD).

Methods: Expression levels of miR34a were measured by TaqMan qPCR in total RNA derived from PBMCs of 49 consecutive subjects, i.e. 19 subjects without clinical evidence of CAD, 15 patients with stable CAD and 15 patients with acute myocardial infarction (AMI).

Results: PBMC miR-34a levels were associated with ageing (r=0.442, p=0.001), common carotid and femoral intima-media thickness (r=0.475, p=0.003; r=0.351, p=0.033), the number of carotid (r=0.529, p=0.001) and femoral (r=0.341, p=0.039) atherosclerotic plaques, as well as with the maximum height of carotid plaques (r=0.406, p=0.014). Further, increased PBMC miR-34a levels were observed in patients with type 2 diabetes mellitus, arterial hypertension or patients with a prior myocardial infarction (p<0.05 for all). Patients with stable CAD or AMI presented with increased PBMC miR-34a levels compared to subjects without clinically overt cardiovascular disease (p<0.05).

Of interest, PBMC miR-34a levels positively correlated with the number of diseased CAD vessels (r=0.367, p=0.05), while they were inversely associated with the creatinine levels (r= –0.347, p=0.018).

Conclusion: Taken together, PBMC levels of miR-34a are associated with extent of subclinical atherosclerosis and incident CAD, supporting a proinflammatory role of immune cell miR-34a in the pathogenesis and development of atherosclerosis. Future © Verlag PERFUSION GmbH

CXCL4 has been shown to promote atherogenesis by various mechanisms.

However, data on CXCL4 plasma levels in patients with coronary artery disease are largely inconclusive. Computed coronary artery angiography (CCTA) represents an excellent tool to quantify and characterize coronary atherosclerotic plaques. We hypothesized that increased CXCL4 plasma levels may be associated with features of plaque instability resulting in adverse cardiovascular events. Specifically, we sought to determine whether CXCL4 levels are correlated with specific features of coronary artery disease including (1) plaque volume, (2) calcium score, (3) degree of stenosis, or (4) vascular remodeling.

Methods and results: CXCL4 plasma levels were measured by ELISA in 217 patients undergoing CCTA for suspected CAD (mean age 64.2±9.4 years, 107 (49.3 %) male). Mean CXCL4 plasma levels were 12.5±4.6 ng/mL. There was no significant correlation between CXCL4 levels and any clinical or demographic parameters including cardiovascular risk factors. plasma levels did not differ between patient with or without coronary artery disease (CAD: 12.5±4.5 ng/ml, no CAD: 12.5±4.8 ng/ml). Neither univariate, nor multivariate analysis showed an association between CXCL4 levels and plaque volume, total calcium score, degree of stenosis, or vascular remodeling. Subgroup analysis of patients with CAD as confirmed by CCTA did not show any association of CXCL4 levels with the extent of CAD. Conclusion: While CXCL4 may be present and active within the arterial wall, local increase of CXCL4 may not translate into systemically elevated CXCL4 levels. Further studies will have to test whether CXCL4 may still represent a suitable therapeutic target in human atherosclerosis.

Menopause and oxidized low-density lipoprotein Felix Kork, Vera Jankowski, Joachim Jankowski Institute for Molecular Cardiovascular Research, University Hospital of the RWTH Aachen, Germany Aims: Oxidized low-density lipoprotein (oxLDL) leads to atherosclerosis and cardiovascular disease, the most frequent causes of death worldwide.

After menopause, lipid and lipoprotein metabolism changes and women are at greater risk of cardiovascular disease compared to fertile women. The aim of this study was to determine the pre­ valence of serum oxLDL in postmenopausal women and to identify possible associations of clinical and laboratory features with oxLDL in these patients.

Methods: After clinical examination and completing a clinical questionnaire, an ultrasound examination of both carotid arteries was conducted and blood was drawn from 533 postmenopausal women. oxLDL concentration was determined using proton NMR spectroscopy.

Results: Oxidized LDL was detected in 12.4 % (95% confidence interval 9.7– 15.5) of postmenopausal women with a median of 0.18 mg/dl (interquartile

range 0.10–0.43). Although intima-media thickness did not differ, postmenopausal women with serous oxLDL had more often atherosclerotic plaques compared to women without oxLDL (6/66 vs. 0/467; p<0.01). Higher concentrations of high-density lipoprotein, impaired glucose intolerance, and DBP were independently associated with the occurrence of oxLDL. If oxLDL was present, higher high-density lipoprotein and glucose intolerance were associated with higher concentrations of oxLDL. In contrast, higher blood urea concentrations were associated with lower concentrations of oxLDL. Conclusion: This study presents the prevalence and concentration of oxLDL in postmenopausal women and demonstrates that oxLDL concentration can be quantified by proton NMR spectroscopy in large patient samples. The data suggest that oxLDL may be a biomarker for incipient atherosclerotic changes in postmenopausal women. In contrary to the association of dyslipoproteinemia and diabetes, higher blood urea concentrations were associated with lower concentrations of oxLDL.

The preloadreducing organic nitrate pentaerythrityl tetranitrate (PETN) is the only organic nitrate in clinical use that is devoid of development of oxidative stress, endothelial dysfunction and nitrate tolerance under chronic therapy. PETN therapy showed beneficial effects in models of diabetes, hypertension, atherosclerosis and ischemic heart failure, potentially due to pleiotropic antioxidant properties. The role of PETN on pulmonary artery pressure, pulmonary and thoracic endothelial function as well as oxidative stress in PAH has not yet been defined.

Methods and results: The alkaloid monocrotaline (MCT) was injected (i.v.) to induce PAH in Wistar rats. Animals were treated with low (30 mg/kg; MCT30), middle (40 mg/kg; MCT40) or high (60 mg/kg; MCT60) dose of MCT for two, four and six weeks.

Endothelium-dependent relaxation was impaired dependent on MCT dose and treatment duration in pulmonary arteries as well as in aorta. MCT induced pulmonary morphological changes and dose-dependent protein tyrosine nitration. Pulmonary arterial pressure (PAP), heart/body and lung/body weight ratio were increased in MCT40 rats (4 weeks) and reduced by oral PETN (10 mg/kg, 3.5 weeks) therapy.

PETN therapy improved endotheliumdependent relaxation in pulmonary arteries of rats treated with MCT40.

Oxidative stress in the vascular wall (dihydroethidium staining), heart (NADPH oxidase activity and 3-nitrotyrosine levels) and whole-blood (xanthine oxidase activity and oxidative burst) was increased in MCT40-treated rats and normalized by PETN therapy. Expression (mRNA) of the antioxidant gene heme oxygenase-1 (HO-1) was increased in MCT group and further increased by PETN therapy. Endothelin signaling was aggravated in pulmonary hypertensive rats, since mRNA levels of endothelin-converting-enzyme-1 (ECE-1), endothelin-1 and ET-1a receptor were increased, which was normalized by PETN. Conclusion: In summary, MCT-induced PAH impaired vascular function (aorta and pulmonary arteries) and increased oxidative stress, which was accompanied by induction of endothelin-1 signaling, whereas PETN therapy attenuated these adverse effects. Thus, PETN therapy improved pulmonary hypertension in accordance to its known pleiotropic antioxidant properties, beneficial effects on vascular function and cardiac preload reducing ability.

Die Expression von Komplementrezeptoren korreliert mit der Expression von Plättchenaktivie­ rungsmarkern. Außerdem moduliert oxidiertes Low-Density-Lipoprotein (oxLDL) die Funktion von Blutplättchen. In der vorliegenden Studie untersuchen wir den Zusammenhang zwischen den Komplementaktivie­ rungsmarkern C3a und C5a und oxLDL. 207 Patienten mit koronarer Herzkrankheit (KHK) wurden in die Studie eingeschlossen. Mittels ELISA wurden die Spiegel von oxLDL, C3a und C5a bestimmt. Zusätzlich wurde plättchengebundenes oxLDL mittels Durchflusszytometrie (FACS) quantifiziert.

Ergebnisse: Die C5a-Spiegel waren zwischen der Troponin-negativen Gruppe (stabile und instabile AP) und der Troponin-positiven Gruppe (NSTEMI und STEMI) nicht signifikant verschieden (62.735±32.370 ng/ml versus 65.820±40.327 ng/ml). Die Anaphylatoxine C5a und C3a zeigten unter­ einander eine signifikante Korrelation (r=0,252; p<0,001). Zwischen C3a im Serum und plättchengebundenem oxLDL konnten wir allerdings keine statistisch signifikante Korrelation feststellen. Auch oxLDL im Plasma korrelierte weder mit C3a noch mit C5a.

Allerdings beobachteten wir eine signifikante schwach negative Korrelation zwischen C5a und plättchengebundenem oxLDL (r= –0,146, p=0,037).

Schlussfolgerungen: Zusammenfassend haben wir eine negative Korrelation zwischen plättchengebundenem oxLDL und dem Anaphylatoxin C5a in einer KHK-Patientenkohorte beschrieben. Diese Studie unterstreicht die Bedeutung des Zusammenhangs zwischen oxidierten Lipiden und Komponenten des Komplementsystems in der Atherogenese.

Given that patients with type 2 diabetes have more epicardial fatty tissue (EFT) compared to patients without diabetes and as EFT may be involved in the pathogenesis of coronary heart disease, we hypo­ thesized that there is a relationship between EFT and features of coronary plaque vulnerability in patients with type 2 diabetes.

stable coronary artery disease in order to assess plaque composition including features of plaque vulnerability.

Furthermore, EFT was quantified using cardiac magnetic resonance tomography.

Results: The mean thickness of the fibrous caps (mean FCT) overlying the necrotic lipid core of all target lesions was 128±30 µm. EFT was related to mean FCT of the target lesions on linear regression analysis (r=0.375, p=0.01). Furthermore, EFT predicted a thin mean FCT (mean FCT<128 µm) on univariable logistic regression analysis (OR 3.956, 95% CI: 1.331–11.760, p=0.013). Multivariable logistic regression analysis using adjustments for (1) BMI and waist circumference (OR 4.822, 95%CI: 1.305–17.823, p=0.022) and (2) additional adjustments for mean arterial pressure and HbA1c (OR 4.065, 95%CI 1.056–15.641, p=0.041) demonstrated that EFT is an independent predictor for a thin mean FCT. Conclusion: EFT is related to the mean FCT overlying the necrotic lipid core of the culprit lesions in patients with type 2 diabetes and stable coronary heart disease. Future studies are necessary to investigate if there is a causal role between EFT and features of coronary plaque vulnerability in patients with type 2 diabetes.

Micro- and macroalbuminuria are strong risk factors for progression of nephropathy in patients with hypertension or type 2 diabetes patients. Early detection of progression to micro- and macroalbuminuria may facilitate prevention and treatment of renal diseases.

We aimed to develop plasma proteomics classifiers to predict the development of micro- or macroalbuminuria in hypertension and type 2 diabetes. Patients with hypertension (n=125) and patients with type 2 diabetes (n=82) were selected for this case-control study from the PREVEND cohort and the Steno Diabetes Center. Cases transitioned from normo- to microalbumin­ uria or micro- to macroalbuminuria.

Controls, matched for age, gender, and baseline albuminuria stage, did not transition. Follow-up was 3.0±0.9 years. Plasma proteomics profiles were measured by LC-electrospray-trap mass-spectrometry. Classifiers were developed and cross-validated for prediction of transition in albuminuria stage. Improvement in risk prediction was tested on top of a reference model of baseline albuminuria, eGFR, and renin-angiotensin-aldosterone system intervention. In hypertensive patients, the classifier improved risk prediction for transition in albuminuria stage on top of the reference model (C-index from 0.69 to 0.78; p<0.01). In type 2 diabetes, the classifier improved risk prediction for transition from microto macroalbuminuria (C-index from 0.73 to 0.80; p=0.04). In both diseases, the identified peptides were linked to pathways recognized to contribute to nephropathy, including fibrosis, inflammation, angiogenesis, and mineral metabolism. dict the transition in albuminuria stage beyond established renal risk markers in hypertension or type 2 diabetes. External validation is needed to assess reproducibility.

Macrophage activation is characterized by the different expression of marker genes, for example, increased surface expression of CCR7. This chemokine receptor is involved in homing of MФ and other immune cells into lymph nodes. CCR7 is dose- and time-dependently induced by the phorbolester phorbol-12-myristate-13-acetate (PMA) in THP-1 MФ. Until now, no signaling pathways have been identified which explain this regulation.

Aim: Unveiling the signaling pathways involved in PMA-induced upregulation of CCR7 in mature human MФ and their regulatory origin is our objective.

Methods and results: We differentiated mature but naïve THP-1 MФ with low doses of 10 ng/ml and high doses of 100 ng/ml PMA. positive feedback signaling loop inducing CCR7 via PKC and an extracellular mediator released by activated MФ.

We assumed the involvement of prostaglandin E2 (PGE2). Indeed release of PGE2 was increased in response to PMA as measured by ELISA. However, blocking of PGE2 production by indomethacin did not influence the upregulation of CCR7. Likewise, other lipid mediators of the arachidonic acid cascade were determined not to be involved. Chemical inhibition of the 5-lipoxygenase, 12-/15-lipoxygenase pathways and cPLA2 did not affect CCR7 expression as confirmed by RTqPCR. Applying specific pharmacologic inhibitors we performed systematic investigations of potentially involved signaling pathways. We confirmed the involvement of ERK, JNK, PKC, PI3K and GSK-3 in PMA-induced upregulation of CCR7. Our investigations revealed that the supernatant of MФ stimulated with 100 ng/ml for 48 hours provokes the upregulation of CCR7 in naïve MФ, which remained when the supernatant was heated to 65°C. We therefore suggest that the autocrine mediator in question is not of protein origin. However, further studies, such as comprehensible metabolomics, are required for completely unraveling the extracellular mediator of PMA-induced MΦ activation pathway.

Conclusion: PMA-induced up-regulation of CCR7 involves several signaling proteins as well as MФ phage-derived autocrine mediators, which orchestrate classical MФ activation. They are likely no proteins but may arise from other classes of the metabolome.

Aims: Nuts contain high amounts of monounsaturated fatty acids (MUFAs) and polyunsaturated fatty acids (PUFAs), which are considered to be responsible for the several healthpromoting effects of nuts. But nuts are mainly consumed roasted and the roasting process is not only responsible for the special taste but may also influence health-promoting and healthharming effects.

Methods: Ground raw and roasted nuts were digested using a standardized in vitro model of the human digestive system till the end of the small intestine.

Hydrophobic substances were extracted using lipophilic solvents to simulate resorption and the extracts were saponified with potassium hydroxide to affirm hydrolysis of the triacylgycerids.

Fatty acid distribution was measured by gas chromatography. After coupling the fatty acids of the nut extracts to a mixture of Krebs Ringer bicarbonate buffer and bovine serum albumin, the extracts were examined for their antiinflammatory capacity using lipopolysaccharide (LPS)-stimulated murine RAW264.7 macrophages. The LPSinduced expression of pro-inflammatory proteins was measured by Western blotting and the effects on the inflammatory response were assessed by measuring formation of nitric oxide. In comparison, we used a synthetic mixture of commercially available fatty acids that showed a fatty acid distribution similar to that of the nut extracts.

Results: The roasting process had no influence on the distribution and quantity of fatty acids. Oleic acid (C18:1 ω9) is the main fatty acid in nut oil, followed by linoleic acid (C18:2 ω6) and palmitic acid (C16:0). Induction of cyclooxygenase 2 and inducible nitric oxide synthase by LPS and the formation of nitric oxide was significantly reduced by all examined nut extracts with no or only slight differences between mixed roasting conditions. The fatty acid mixture blocked the LPSinduced inflammatory response in a similar manner.

Conclusion: Regardless of roasting,

nut lipids but also a fatty-acid-mixture inhibit the LPS-induced formation of inflammatory mediators. In this way nuts can help to prevent inflammation and contribute to a healthy nutrition.

Role of GDF-15 on autophagy in human MФ after oxLDL-exposition Kathrin Ackermann, Anja Schwarz, Gabriel A. Increased GDF-15 and oxLDL (oxidized low-density lipoprotein) serum levels are suggested as independent risk factors for cardiovascular diseases. Behind this, there is an increased interest in the role of autophagy in vascular biology. Therefore, we aimed to investigate a possible role of GDF-15 on autophagy in macrophages (MФ) after oxLDL exposition.

Methods and results: Treatment of human U937 PMA-differentiated macrophages (MФ) with copper (Cu++)oxLDL increased the GDF-15 mRNA (1.25-fold; p<0.014) expression and the protein level (1.7-fold; p<0.05).

Silencing of GDF15 in THP-1 MФ (sGDF15-MФ) resulted in a 75% (p<0.01) and a 40% (p< 0.001) decrease of the GDF-15 mRNA expression and protein level, respectively. sGDF15-MФ were then incubated with or without 50 µg/ml oxLDL and 20 ng/ µl recombinant GDF-15 (rGDF-15).

“AllStars Negative Control siRNA”, without effect on mammal genes, was used as negative control (nsGDF15MФ) for sGDF15-MФ. After 4 hours oxLDL exposition of MФ, we show by using Oilred O staining, a significant (p≤0.05) 22 % increased lipid uptake in nsGDF15-MФ, whereas in sGDF-15MФ lipid uptake was 25 % (p=0.085) decreased in comparison to nsGDF15MФ. protein) was 38 % (p<0.05) decreased in sGDF15-MФ, whereas oxLDL treatment in combination with rGDF-15 increased the p62 protein level up to 1.5-fold (p<0.05) in nsGDF15-MФ.

Additionally, p62 protein level was 112 % (p<0.05) and 127 % (p<0.005) increased in oxLDL-treated sGDF15MФ with or without rGDF15. Important for the autophagosome membrane nucleation, ATG5 protein level was 3–6-fold (p<0.05) increased after oxLDL and rGDF-15 treatment of nsGDF15-MФ or sGDF15-MФ. Interestingly sGDF15-MФ showed a 31 % (p=0.057) decreased ATG5 protein level after oxLDL/rGDF-15 treatment in comparison to nsGDF-15-MФ.

Conclusion: Our results suggest a regulatory role of GDF-15 in the auto­ phagy of MФ after oxLDL exposition, which may have an impact on atherosclerotic processes.

Here, we investigated the number and phenotype of circulating DCs precursors in a patient collective undergoing PMVR to uncover the potential crosstalk of HF with immune regulation.

ing myeloid DCs (mDC), plasmacytoid DCs (pDC) and their surface expression of co-stimulatory molecules CD40, CD86, HLA-DR, as well as adhesion molecule CD11a in healthy control subjects (n=11) and MR patients at the time of PMVR (n=53) and at a median follow-up of 5.8 (range:4 to 10) months. Levels of plasma C-reactive protein (CRP), change in left ventricular end-diastolic diameter (LVEDD) and a 6-minute walk test were assessed prior to PMVR and during follow-up.

Results: Compared to controls (mDC 10.38+1.07/µl and pDC 5.44+0.69/µl, respectively), patients with MR had significantly decreased numbers of circulating mDC precursors (7.05+0.47/ µl, p<0.01), whereas there was no significant difference regarding pDC precursors (5.10+0.4/µl). While a significant up-regulation of mDC surface expression of CD11a (mean fluorescence intensity (MFI) 160, p<0.001) was detected compared to healthy controls (MFI: 131), no significant differences for CD40, CD86 and HLA-DR were found. During follow-up, PMVR treatment restored reduction of circulating mDC precursors (8.93+0.54/µl, p<0.001). Furthermore, we observed an increase in circulating pDC precursors (7.04+0.65/µl, p<0.01). Simultaneously, circulating mDC and pDC surface expression of CD11a significantly decreased after PMVR. The increase of circulating DCs precursor numbers after PMVR was paralleled by a reduction of CRP levels, decreased LVEDD and an improvement of six-minute walk test distance.

Conclusion: Our findings suggest that the change of circulating DC precursors may be involved in the pathophysiology of MR relevant HF.

Flow cytometric analysis of DC precursors in the blood of patients with resistant hypertension undergoing renal denervation Frederic Emschermann, Harald F. Langer Dept. of Internal Medicine III, University Hospital of the Eberhard Karls University Tübingen, Germany Aims: To analyze circulating mDC and pDC precursors regarding their frequency and expression of HLA-DR, CD40, CD86 and CD11a in patients suffering from resistant hypertension and before renal denervation therapy and during follow-up.

Methods: Blood samples were taken from 50 patients at the arterial femoral access at the beginning of the renal denervation procedure or drawn at 6/12 months follow-up visits. Following ery­throcyte lysis, samples were stained with fluorochrome-conjugated antibodies and analyzed using flow cyto­metry.

CD14/19 were used as a marker to exclude B-cells and monocytes, CD1c+ defined mDC precursors, CD303+ pDC precursors to measure precursor frequency. Precursors were then analyzed regarding their surface expression of HLA-DR, CD40, CD86 and CD11a.

Results: Compared to healthy controls, patients with therapy-resistant arterial hypertension have a reduced number of circulating mDCs as well as pDCs, although this difference only reached statistical significance for circulating pDCs. Surprisingly, the number of pDCs increased significantly 12 months after renal denervation indicating an effect of renal sympathetic nerves on the levels of circulating pDCs in patients. Moreover, in both mDCs and pDCs the expression of MHCII decreased 12 months after renal denervation. Similarly, expression of LFA-1 was significantly reduced in mDCs and pDCs. Whereas costimulatory molecule CD86 was unchanged after renal denervation, CD40 levels decreased after 12 months only in the case of pDCs. Preliminary data have shown a pro-atherogenic gene expression in circulating peripheral blood mononuclear cells (PBMCs) with OSAS, however, the separate contribution of OSAS (hypoxia-reoxygenation stress) and obesity, i.e. mostly overlapping conditions, is presently unknown.

Moreover, the reversibility of PBMC activation through continuous positive airway pressure (CPAP) therapy is unclear.

Methods: In a cross-sectional design we therefore studied pro-inflammatory (IL-1β, COX-2, IL-6, TNF-α) and hypoxia-inducible (VEGFa, GDF-15) PBMC gene expression in patients with OSAS (n=15, apnea-hypopnea-index [AHI] >15/h) stratified for BMI≥30 kgm-2 (n=10, AHI 54.1±6.9/h) and BMI<30 kgm-2 (n=5, AHI 40.4±19.4/h) subjects as compared to BMI- and age-matched healthy controls (CON, n=13). polysomnography (two consecutive nights), measurement of intima-media thickness (IMT), and post-absorptive venous blood tests for HbA1c, glucose and insulin. PBMC gene expression was analyzed by real-time RT-PCR and normalized to β-glucuronidase, a hypoxia-stable reference gene.

Results: OSAS with hypoxia <90 % SaO2 of 69.2±20.8 min independently led to a 3–5-fold upregulation of IL1β, COX-2, and VEGFa in relation to CON (p<0.01). According to ANOVA, the impact of obesity was limited to a significant interaction with OSAS. This OSAS-related PBMC activation was significantly reversed upon 3–5 months of CPAP (p<0.05, VEGF: p<0.08). Marginal or no such effects were observed with IL-6, TNF-α or GDF-15. There was a significant positive correlation between IL-1β and AHI (r=0.48, p<0.05), IMT (r=0.35, p<0.05) and HbA1c (r=0.42, p<0.01).

Conclusion: OSAS is independently of obesity associated with pro-inflammatory, pro-atherogenic PBMC gene expression (IL-1β, COX-2, and VEGFa) which is reversible by CPAP. GDF-15 as another hypoxia-sensitive, atherosclerosis-relevant gene was unaffected by OSAS or obesity. Future studies may require control for the significant interaction of BMI.

Monocytes isolated from patients with sustained peripheral ischemia show up-regulation of hypoxiainducible factor 1s subunit HIF1alpha, after vascular reperfusion therapy with Prostavasin® Lukas Heger, Mark Kerber, Christoph Weis, Max Mauler, Moritz Mohr, Daniela Stallmann, Christoph Bode, Christoph Hehrlein, Ingo Ahrens Cardiology and Angiology I, University Heart Center, University of Freiburg, Germany Background: The transcription factor HIF1s activity is modulated by the oxygen level-depending expression of its subunit HIF1α and accounts for the activation of target genes optimizing the

utilization of O2 in oxidative stress situations. Patients suffering from secondary Raynaud syndrome due to connective tissue diseases are often afflicted with ulceration and ischemic gangrene giving notice of said oxidative stress situations. Mononuclear cells contribute to the pathogenesis of peripheral ischemia induced by Raynaud syndrome. We hypothesize that HIF1α expression in monocytes may serve as a marker and thus part of the pathway following reperfusion therapy.

Methods: Mononuclear cells from patients with connective tissue diseases and digital ulcerations due to secondary Raynaud syndrome as well as cells from healthy volunteers were isolated by “ficoll”-gradient centrifugation. Afterwards the monocytes were purified using magnetic bead-based separation technology depleting all CD14 negative cells. A plasmid-based quantitative real-time PCR was established for HIF1α and HIF2. The monocyte RNA was extracted using the TRIzol® reagent protocol and transcribed into complementary DNA. The cDNAs HIF1α/HIF2 expression profile was illustrated by recording the fluorescence emission during the cycling process in a Roche light cycler. The results were put in relation to the expression of a housekeeping gene (beta-actin/GAPDH).

Results: We recruited a total of 20 patients suffering from a connective tissue disease and secondary Raynaud syndrome, 11 women and 9 men, with an average age of 55 years. HIF1α gene expression was significantly up-regulated in monocytes of tested patients following an i.v. Prostavasin® treatment period of 14 days (two-tailed paired T test Prostavasin®, p=0.021). Interestingly, the HIF1α level before treatment was at a comparable level to the one of healthy controls. HIF2 showed no significant changes, neither before nor after therapy (two-tailed P-test, p=0.4).

Discussion: HIF1α expression in monocytes isolated from patients with Raynaud syndrome due to a connective tissue disease does not differ from expression levels in healthy controls.

However, following a 2-week treatment period with Prostavasin®, HIF1α expression in monocytes was significantly increased alongside the clinical improvement. HIF1α expression in monocytes may therefore be a downstream effect of Prostavasin® treatment and could potentially evolve as a valuable target for the treatment of digital ulcera due to prolonged ischemia.

S-Adenosylhomocysteine – a novel non-traditional cardiovascular risk factor in CKD Insa E. Emrich, Adam M. Zawada, Sarah Seiler, Rima Obeid, Jürgen Geisel, Danilo Fliser, Gunnar H. Heine Laboratory of Nephrology, Saarland University Hospital, Homburg/Saar, Germany Background: Patients suffering from chronic kidney disease exhibit high cardiovascular morbidity and mortality.

It has been postulated that a disturbed C1-metabolism, which comprises homocysteine, S-adenosylhomocysteine (SAH) and S-adenosylmethionine (SAM), may contribute to this high cardiovascular risk burden. Recent studies revealed that SAH accumulates in CKD and is associated with prevalent cardiovascular events. Against this background we aimed to investigate the prognostic value of SAH in chronic kidney disease.

Methods: Plasma homocysteine and SAH concentrations were assessed among 297 CARE FOR HOMe participants suffering from CKD (K/DOQI stages G1–G5). SAH was analysed using a Waters 2795 alliance HT, coupled to a Quattro micro API tandem mass spectrometer. tration rate (eGFR), using the MDRD equation. Predefined endpoints of our studies were any cardiovascular event, cardiovascular mortality or all-cause mortality.

Results: Among the 297 CKD patients, mean eGFR was 44±19 ml/min/1.73 m², and mean plasma SAH and homocysteine were 50.1±30.1 µmol/l and 18.4±7.2 nmol/l, respectively. eGFR correlated more strongly with plasma SAH (r=0.497) than with homocysteine (r=0.424). Patients with history of prevalent cardiovascular events had significantly higher plasma SAH levels (p=0.007) than patients without prevalent cardiovascular events. During a follow-up period of 2.5±0.7 years 33 patients experienced the predefined cardiovascular endpoint. Kaplan Meier analysis showed that patients with the highest plasma SAH levels had a significantly higher event rate (log.rank test; p=0.004).

Discussion: In CKD, plasma SAH independently predicts cardiovascular events. Further studies are needed to identify strategies to lower SAH, after B vitamins failed to reduce SAH levels.

Impaired capillarisation and in-vivo microcirculation of skeletal muscle with obstructive sleep apnea: a novel link to insulin resistance?

diabetes through unknown, obesity-independent mechanisms. Impaired skeletal muscle microcirculation has been suggested as a novel factor of insulin resistance but has not previously been addressed with OSAS. Methods: We therefore studied capillary histomorphology and angiogenic signals in vastus lateralis muscle (Vlat) biopsies combined with in-vivo contrast-enhanced ultrasound (CEUS) of Vlat microcirculation in 20 obese (BMI>30 kg m-2) and 8 nonobese (BMI<30 kg m-2) OSAS patients (apnea-hypopnea-index, AHI>15/h) as compared to 12 obese and 12 nonobese, age-matched healthy controls (CON, OSAS excluded). OSAS patients had significantly higher (though still normal) HbA1c, post-absorptive plasma insulin, and HOMA-IR index than CON. Results: Despite massive hypoxic stress (57.7±13.4 min <90 % O2-saturation) with OSAS, Vlat VEGFa, VEGF-R, or Notch-1 mRNA expression were not increased. In non-obese CON, capillary density (CD) and fiber contacts (CC) were 297±17 mm-2 and 5.09±0.21, respectively, however, they were significantly reduced in both, nonobese OSAS patients (226±12 mm-2 and 3.15±0.32) and obese CON (248±20 mm-2 and 4.15±0.34) with no further impairment in obese OSAS patients (242±15 mm-2 and 3.85±0.19).

CD correlated negatively with AHI (r= –0.47, p<0.01) and other hypoxic stress parameters. Moreover, CC and VEGFa were negatively related to insulin (r= –0.42, p<0.01) and HOMA-IR (r= –0.51, p<0.01), respectively. In line with CD, CEUS revealed a significant reduction of (post-exercise) microvascular flow to 32–61 % of non-obese CON with OSAS alone, obesity alone, or their combination. This was attributable to both, lower microvascular volume and flow velocity.

Conclusion: Despite severe hypoxic stress no skeletal muscle neo-angiogenesis is obvious with OSAS. In contrast, capillarisation and microvascular flow reserve are massively impaired © Verlag PERFUSION GmbH

through OSAS with or without associated obesity. Such impaired nutritive microcirculation may represent a novel link between OSAS and insulin resistance, as previously suggested for obesity alone.

This process called VSMC phenotypic switching can typically be found in patients with chronic kidney disease (CKD). Hitherto, data on the role of epigenetic regulation during vascular calcification are lacking. Here we have identified the histone methyltransferase G9a as a selective modulator of smooth muscle cell transdifferentiation and calcification.

Methods and results: To analyze the role of epigenetic mechanisms during vascular calcification, we treated VSMC with CaPO4 leading to smooth muscle cell calcification and crystal formation detected by von Kossa-staining in vitro. Interestingly, this was paralleled by an upregulation of the histone lysine methyltransferase G9a (1.6 fold, p<0.05), controlling gene silencing by histone h3 lysine 9 dimethylation. To analyze functional relevance, we stimulated CaPO4 treated VSMC mit UNC0638, a specific inhibitor of G9a, leading to a significant decrease of VSMC calcification. mediated knockdown of G9a was associated with reduced expression of VSMC calcification markers including ALPL, Runx2, BMP-2 and osteocalcin while the anti-calcification marker Matrix-Gla-Protein was upregulated (1.5 fold, p<0.05). To understand the underlying mechanisms, we next asked whether UNC0638 affects VSMC differentiation. Importantly, UNC0638 treatment significantly attenuated TNFα-induced downregulation of the SMC marker gene SM22α, suggesting that UNC0638 reduces SMC dedifferentiation. This effect was detectable up to 72 hours after the initial treatment and associated with a strong and equally sustained reduction of the respressive histone mark (h3k9me2) at the SM22α promoter indicating sustained preservation of the SMC-differentiation status.

Conclusion: In summary, our data suggest that epigenetic mechanisms might play an important role in osteogenic transdifferentiation of VSMCs and vascular calcification. Pharmacologic modulation of histone methylation might be a promising therapeutic approach for the treatment and/or prevention of vascular calcification in patients with chronic kidney disease.

In this regard, the role of membrane-associated Toll-like receptors (TLR) in atherosclerosis and metabolic disorders has been already well characterized. However, the impact of the cytoplasmic nucleotide-binding oligomerization domain-containing

protein (Nod)-like receptors (NLR) is far less clarified. Concerning the family member NOD1, different experimental setups with conflictive and incomplete outcomes exist. In the present study we generated Nod1/low density lipoprotein receptor double knockout mice (Nod1‒/‒; Ldlr‒/‒) which were subsequently analyzed in regards to experimental atherosclerosis and glucose tolerance.

Methods: Nod1 gene expression from various murine tissues was studied by qRT-PCR and Western blot. Additionally pro-inflammatory cytokine induction upon Nod1 stimulation was measured in murine macrophages (ELISA, RAW 264.7). Furthermore, Nod1‒/‒; Ldlr‒/‒ mice and Nod1+/+; Ldlr‒/‒ control mice were fed with a high fat, high cholesterol diet for 12 weeks and subsequently analyzed concerning atherosclerotic plaque burden and glucose tolerance.

Results: Nod1 mRNA profiling in C57BL/6 mice revealed a robust expression in most organs including heart, adipose tissue and bone marrow (qRT-PCR). Among vascular cells, we detected the highest Nod1 expression in endothelial cells (Western blot). Additionally, we observed Nod1-dependent induction of the pro-inflammatory cytokines IL-6 and TNF-α in vitro upon stimulation with Tri-DAP, a synthetic Nod1-ligand (ELISA, p<0.05, n=3). Quantification of Oil Red O stained aortic roots revealed significant less lipid deposition in Nod1‒/‒; Ldlr‒/‒ mice (p<0.05, n=5–8). Additionally, less expression of the scavenger-receptors SRA-1 and LOX-1 within the aortic arch (qRT-PCR, p<0.05, n=5/ group) was observed. Weight gain in both groups during the 12 week feeding period was similar. Nod1‒/‒; Ldlr‒/‒ showed impaired glucose tolerance after 6 weeks of diet (p<0.05, p<0.01, n=13/group) which was normalized after 12 weeks.

mice. The lesions displayed a higher number of macrophages, whereas the smooth muscle cell and collagen content were reduced. This was associated with a significant reduction in reendothelialization and endothelial cell proliferation in injured Cxcr4EC-KO ApoE–/– carotids compared to Cxcr4EC-WT ApoE–/– controls. Furthermore, stimulation of human aortic endothelial cells with CXCL12 significantly enhanced their wound-healing capacity in an in vitro scratch assay, an effect that could be reversed with the CXCR4 antagonist AMD3100. Also, flow cytometric analysis showed a reduced mobilization of Sca1+Flk1+Cd31+ endothelial progenitor cells and of Lin-Sca1+ smooth muscle progenitors in Cxcr4EC-KO ApoE–/– mice following vascular injury. No differences could be detected in plasma concentrations of CXCL12, VEGF, S1P or FLT3-ligand, all cytokines with an established role in progenitor cell mobilization. Nonetheless, double immunofluorescence revealed a significant reduction in local endothelial CXCL12 staining in injured carotids from Cxcr4EC-KOApoE–/– mice. This may contribute to reduced reendothelialization through reduced recruitment and incorporation of endothelial progenitor cells at the site of injury.

Conclusion: Endothelial Cxcr4 is crucial for efficient reendothelialization following vascular injury through endothelial wound-healing and proliferation, and through the mobilization of Sca1+Flk1+Cd31+ endothelial progenitor cells.

Aims: PACAP (pituitary adenylate cyclase-activating polypeptide) belongs to a superfamily of structurally related peptides, that includes vasoactive intestinal peptide, glucagon, glucagon-like peptide, secretin and growth hormonereleasing hormone. PACAP is widely distributed in the central and peripheral nervous systems and interacts with specific receptors (PAC1, VPAC1, VPAC2). PACAP stimulates the sympathetic activity, modulates immunological processes and inhibits the proliferation of smooth muscle cells. The aim of the present study was to investigate whether PACAP is involved in the development and progression of atherosclerosis.

Methods: PACAP–/–/ApoE–/– and PACAP+/+/ApoE–/– mice were used for the study. 10 weeks-old mice were fed for 20 weeks with a “normal chow” (NC) or with a cholesterol-enriched “western diet” (CED). After 30 weeks, the mice were weighed and blood samples were taken to determine cholesterol and triglyceride serum levels. Thereafter, the animals were sacrificed and the brachiocephalic trunk (TB) was excised and shock-frozen. The lumen stenosis of the TB was assessed on cryo cross-sections (7 µm).

eration (Ki67), autophagy (APG5L/ ATG5) and necroptosis (RIP3).

Results: After NC, PACAP–/–/ApoE–/– mice were reduced in weight by 14.5 % (p<0.05) whereas total cholesterol and triglyceride serum levels were not different from the PACAP+/+/ApoE–/– control group. PACAP–/–/ApoE–/– mice showed an 11.6-fold (p<0.01) increase in lumen stenosis compared with the control. In atherosclerotic lesions of PACAP–/–/ApoE–/– mice, the density of macrophages was decreased by 71.4 % (p<0.01) and that of Ki67 immunoreactive cells by 64.4 % (p<0.05) as compared to control. In contrast, there was an increase in the proportional areas immunostained for IL-1-β (4.75fold, p<0.05), APG5L/ATG5 (8.3-fold, p<0.001) and RIP3 (54.5-fold, p<0.01) in comparison to control. Not any significant differences were observed between atherosclerotic lesions of PACAP–/–/ApoE–/– mice and PACAP+/+/ ApoE–/– mice regarding the proportional areas of α-actin, IL-6 and COX2 immunoreactivity as well as the total cell density. After 20 weeks CED, PACAP–/–/ApoE–/– mice exhibited an increase in serum triglycerides (1.4fold, p<0.01), while serum cholesterol levels and weight were similar to the control. We did not observe any significant differences between atherosclerotic lesions of PACAP–/–/ApoE–/– mice and PACAP+/+/ApoE–/– mice regarding the average lumen stenosis, the total cell density and the rate of cell proliferation, as well as the proportional areas of macrophages, IL-1-β, IL-6, COX-2, α-actin, APG5L/ATG5 and RIP3.

Conclusion: Our present data demonstrate that PACAP deficiency in ApoE–/– mice aggravates the development and progression of atherosclerotic plaques under NC, but not after 20 weeks CED. We conclude that the proatherogenic impact of PACAP deficiency in ApoE–/– mice under NC is at maximum, explaining that CED has no additive proatherogenic effect. Bonaterra, Lilli Mey, Hans Schwarzbach, Eberhard Weihe, Ralf Kinscherf Dept. of Medical Cell Biology, Institute of Anatomy and Cell Biology, Philipps University Marburg, Germany Aims: The neuropeptides vasoactive intestinal polypeptide (VIP) and pituitary adenylate cyclase activating peptide (PACAP) have pleiotropic vasoactive and immunoregulatory functions throughout the nervous and neuroendocrine systems. VIP acts at VPAC1 and VPAC2 but not at PAC1 while PACAP acts at PAC1, VPAC1 and VPAC2.

Given the crucial roles of PACAP and the PAC1 receptor in stress and immune regulation and the well-known contribution of stress and inflammation in the pathogenesis of atherosclerosis it was the aim of our study to explore, whether PAC1 deficiency influences the development and progression of atherosclerosis in the ApoE–/– mouse model of atherogenesis. and Methods: PAC1–/–/ApoE–/– PAC1+/+/ApoE–/– mice were used for the study. 10 weeks old mice were fed for 20 weeks with a “normal chow” (NC) or with a cholesterol-enriched “western diet” (CED). After a total of 30 weeks we determined animal weights and total cholesterol and triglyceride serum levels in the different experimental groups. The brachiocephalic trunks (BT) were dissected and snap-frozen.

Cryo cross-sections (7 μm) of BT were processed do determine BT lumen stenosis and for immunohistomorphomet-

rical analysis of macrophages (CD68), inflammatory cytokines (IL-1b, IL-6, COX-2), smooth muscle cells (α-actin, SMCs), proliferation (Ki67), autophagy (APG5L/ATG5) and necroptosis (RIP3) in the BT wall.

Results: PAC1–/– was found to have no effect on the average body weight, and the total cholesterol and trigly­ ceride serum levels. After NC only small plaques were found in the BT of PAC1–/–/ApoE–/– as well as of PAC1+/+/ ApoE–/– mice. Nevertheless, PAC1–/–/ ApoE–/– mice exhibited a significant (p<0.05) 33-fold increase of RIP3 necroptotic cells, a significant (p<0.05) 42 % decrease of the proportional area of macrophages, a significant (p<0.05) 5,1-fold increase of IL-1b+ proportional areas and a significantly (p<0.05) decreased proportional area of proliferating cells (36 %).

After 20 weeks of CED, in the BT of PAC1–/–/ApoE–/– we observed a significant (p<0.001) 59 % decrease of the lumen stenosis compared to the PAC1+/+/ ApoE–/– mice. In contrast, the proportional areas of IL-1b+, macrophages and proliferating cells was similar in BT lesions of PAC1–/–/ApoE–/– and PAC1+/+/ApoE–/– mice after 20 weeks of CED. Neither after NC nor CED proportional APG5L/ATG5+, a-actin+, COX2+ and IL-6+ areas were affected

by PAC1 deficiency.

Conclusion: PAC1 deficiency in ApoE–/– mice has a remarkable antiatherogenic effect on CED-feeding induced atherosclerotic lumen stenosis without altering cholesterol or trigly­ ceride serum levels. Mechanisms underlying the atheroprotection caused

by PAC1 deficiency in CED fed ApoE–/– mice may include inhibition of cell proliferation concomitant with enhanced necroptosis in the atherosclerotic lesions. Therefore, the development of highly selective PAC1 antagonists as a new class of atheroprotective therapeutics is proposed.

However, the relationship between circulating anxA5 levels and atherosclerotic lesion extension as well as plaque composition in high risk patients with type 2 diabetes is presently unclear.

Objective: To characterize the relationship between circulating anxA5 levels with atherosclerosis burden and plaque composition in patients with type 2 diabetes.

Methods: Intima-media thickness (IMT) has been determined in 96 patients with type 2 diabetes and stable coronary artery disease. Furthermore, intracoronary optical coherence tomography (OCT) has been performed in 106 lesions to determine coronary plaque composition.

Results: AnxA5 plasma levels of patients with increased IMT were significantly higher (3.49±2.19 ng/ml) compared to patients with normal IMT (2.24±1.67 ng/ml, p=0.002). Furthermore, anxA5 predicted thickened IMT on uni- (OR 1.445 [1.106–1.889], p=0.007) and multivariable (OR 1.643 [1.166–2.314], p=0.005) binary logistic regression analysis when adjusted for multiple cardiovascular risk factors and biomarkers. Furthermore, receiver-operating-characteristic analysis demonstrated that anxA5 predicted thickened IMT with low-moderate diagnostic efficiency (AUC 0.700 [0.592-0.808], optimal cut-off value 1.907 ng/ml, sensitivity 72.4 %, specificity 65.8 %, positive predictive value 76.4 %, negative Perfusion 3/2016

predictive value 61.0 % at the optimal cut-off). How­ever, no association was found between circulating anxA5 levels and coronary plaque composition as assessed by OCT including the presence of lipid, calcified, fibrous plaque or the minimal thickness of the fibrous cap overlying the necrotic lipid core (p=ns).

Conclusion: Circulating anxA5 plasma levels are related to carotid IMT but not coronary plaque composition in highrisk patients with type 2 diabetes.

Adaptive immunity and co-stimulatory signals play a pivotal role during all stages of the disease. Recently, regulatory T cells (Treg) were attributed an anti-inflammatory and anti-atherogenic role. The interaction of CD27 with CD70 which are members of the tumor necrosis factor (receptor) super family modulates T cell proliferation, differentiation, and activation at the sites of antigen priming and effector function. Furthermore, CD27-CD70 interactions are important for the development of thymus-derived Tregs. Thus, we hypothesized an increased atherosclerotic burden and an

exacerbation of disease upon CD27 and CD70 deficiency.

Methods and results: Human carotid atherosclerotic plaques, histologically classified as ruptured, display a higher CD27 and CD70 expression compared to stable carotid atherosclerotic plaques.

Atherosclerotic lesions of hyperlipidemic ApoE–/– mice contained lymphocyte-shaped cells positive for CD27 whereas macrophages and smooth muscle cells did not show CD27 expression. On the other hand, CD70 was expressed predominantly by macrophages whereas lesional lymphocytes did not express CD70. Flow cytometry of aortic suspensions of hyperlipidemic ApoE–/– mice confirmed the exclusive expression patterns of CD27 and CD70. In the present study we investigated the effect of hematopoietic and systemic CD27 and CD70 deficiency in ApoE–/– mice in early and advanced stages of atherosclerosis. Cd27–/– mice and Cd70–/– were crossed with ApoE–/– mice to obtain Cd27–/–ApoE–/– or Cd70–/–ApoE–/– mice and littermate controls, respectively. Transplantation of Cd27–/–ApoE–/– and Cd70–/–ApoE–/– bone marrow into lethally-irradiated ApoE–/– mice consuming an atherogenic diet profoundly increased plaque area, necrotic core size, and number of plaque macrophages compared to mice receiving littermate control bone marrow. Accordingly, Cd27–/–ApoE–/– and Cd70–/–ApoE–/– mice fed with chow-diet displayed accelerated progression of plaques characterized by larger lesion size, lower cellularity and more advanced plaques at the age of 18 weeks compared to littermate controls. At a more advanced stage of atherosclerosis (28 weeks) lesion size and phenotype did not differ between the two groups.

Systemic and hematopoietic CD27 deficiency reduced the frequency of Tregs in the aorta, blood, and lymphoid organs as assayed by flow cytometry.

thymic Cd27–/–ApoE–/– Tregs displayed increased apoptosis and fewer markers of proliferation in vivo.

Deficiency of CD70 on the other hand only mildly affected Treg abundance.

However, bone-marrow derived macrophages from Cd70–/–ApoE–/– mice displayed impaired inflammatory capacity (e.g. ROS and NO production), increased both M1 and M2 markers, and were rendered metabolically inactive and prone to apoptosis compared to controls. Moreover, CD70-deficient macrophages expressed diminished levels of scavenger receptors and ABC-transporters, impairing uptake of oxidized low-density lipoprotein (oxLDL) and cholesterol efflux.

Conclusios: In sum, we demonstrate that CD27 co-stimulation in murine models of atherosclerosis increases Treg abundance whereas CD70 improves macrophage function both limiting lesion development and inflammation, particularly during early stages of atherosclerosis. Thus, our study suggests that promotion of CD27 and CD70 function may mitigate atherosclerosis.

cholesterolemic ApoE–/– mice. The aim of the present study was to investigate the role of GDF-15 on possible muscle dysfunctions using GDF-15-deficient (GDF-15–/–) mice in a mouse model for experimental atherosclerosis (ApoE–/–).

Methods: 10 weeks old GDF-15+/+/ ApoE+/+, GDF-15–/–/ApoE+/+, GDF-15+/+/ ApoE–/– and GDF-15–/–/ ApoE–/– mice were used. After 12 or 20 weeks cholesterol-enriched diet (CED) the gastrocnemius muscles were dissected and shock-frozen. RNA and proteins were isolated, relevant genes for apoptosis, angiogenesis and inflammation were analysed by quantitative real-time RTPCR and proteins were analysed for pro-/anti-inflammatory and angiogenic cytokines using a Proteome Profiler™ Antibody Array Kit. Results: After 12 weeks CED, quantitative Real-Time RT-PCR showed in gastrocnemius muscle of GDF-15–/–/ ApoE+/+ mice a 21- and 6-fold (p≤0.05) IL-6 and IL-1β expression, whereas after 20 weeks CED IL-6 and IL-1β expression decreased significantly 69 % and 98 % in comparison with WT mice. Pro-apoptotic Bax expression was 31-fold increased (p≤0.05) in GDF-15–/–/ApoE+/+ mice after 12 weeks CED compared with WT. Moreover, we found in GDF-15–/–/ApoE–/– mice a 86 % (p≤0.05) decrease of Bax expression compared with GDF-15+/+/ ApoE–/– mice after 20 weeks CED. Expression of pro-angiogenic Notch1 was 97 % decreased (p≤0.05) in GDF-15–/–/ ApoE–/– compared with GDF-15+/+/ ApoE–/– mice after 20 weeks CED. Expression of atrophy marker Fbxo32 was 23-fold increased (p≤0.05) in GDF-15–/–/ApoE+/+ mice after 12 weeks CED compared with WT mice.

Furthermore, protein expression array analysis revealed after 20 weeks CED an increase of pro-inflammatory cytokines (e.g. IL-1β, IL-6 and IL-17) and pro-inflammatory and pro-angiogenic chemokines (e.g. CCL2, CCL11, CXCL1) in gastrocnemius muscle of GDF-15–/–/ApoE+/+ mice compared with WT but a decrease of these cytokines and chemokines in GDF-15–/–/

ApoE–/– compared with GDF-15+/+/ ApoE–/– mice.

Conclusion: The lack of GDF-15 seems to modulate the expression of pro-inflammatory, angiogenic, apoptotic and atrophy genes/proteins dependent from the duration of CED application.

position of the transplanted aortic segment were analyzed after 4 weeks of atherogenic diet.

Results: Deficiency in CX3CR1 in vascular cells of the transplant aortic segment resulted in reduced formation of atherosclerotic plaques associated with reduced numbers of SMCs, compared to transplanted control mice.

Moreover, reduced frequencies of proliferating and apoptotic cells were observed in CX3CR1–/– aortic segments.

In contrast, transplantation of ApoE–/– segments into CX3CR1–/– ApoE–/– recipients did change neither lesion size nor numbers of SMCs and changes in the rate of proliferation and apoptosis, indicating that effects on transplant intimal lesions were primarily due to vessel wall resident cells.

We confirmed the cross-talk between SMC and monocytes augments the inflammatory response, since antiapoptotic and proliferative effects of fractalkines on SMCs also reduce the macrophage numbers in transplanted CX3CR1–/– aortic segments.

Conclusion: These results indicate that vascular CX3CR1 plays the key role in the neointima development in the aortic transplanted grafts, but also stabilized the plaque. This finding might have a strong implication in vascular pathology and transplant accelerated arteriosclerosis, since balance of SMCs and macrophages critically determines both plaque stability and vessel stenosis.

Atherosclerotic lesions that critically narrow the artery can necessitate an angioplasty and stent implantation.

Long-term therapeutic effects, however, are limited by excessive arterial remodeling. We here employed a miniaturized nitinol-stent coated with starshaped polyethylenglycole (star-PEG), and evaluated its bio-functionalization with RGD and CXCL1 for improving in-stent stenosis after implantation into carotid arteries of mice. Nitinol foils or stents (bare metal) were coated with star-PEG, and bio-functionalized with RGD, or RGD/CXCL1. Cell adhesion to star-PEG-coated nitinol foils was unaltered or reduced, whereas biofunctionalization with RGD but foremost RGD/CXCL1 increased adhesion of early angiogenic outgrowth cells (EOCs) and endothelial cells but not smooth muscle cells when compared with bare metal foils. Stimulation of cells with RGD/CXCL1 furthermore increased the proliferation of EOCs. In vivo, bio-functionalization with RGD/ CXCL1 significantly reduced neointima formation and thrombus formation, and increased re-endothelialization in ApoE–/– carotid arteries compared with bare-metal nitinol stents, star-

PEG-coated stents, and stents biofunctionalized with RGD only. Biofunctionalization of star-PEG-coated nitinol-stents with RGD/CXCL1 reduced in-stent neointima formation.

By supporting the adhesion and proliferation of endothelial progenitor cells, RGD/CXCL1 coating of stents may help to accelerate endothelial repair after stent implantation, and thus may harbor the potential to limit the complication of in-stent restenosis in clinical approaches.

Triggering heart failure after antiinflammatory treatment with goldinduced autologous cytokines Franziska Cordes1, Sakine Simsekyilmaz1, Adelina Baleanu-Curaj1, Ulrich Schneider2, Elisa Liehn1 1 Institute for Molecular Cardiovascular Research (IMCAR), University Hospital of the RWTH Aachen, Germany 2 Arthro Nova Clinic, Ringsee, Germany Background and Objective: Myocardial infarction (MI) as one of the most frequent causes of morbidity and mortality in western countries is governed by chemokines and cytokines in a complex inflammatory response.

The mechanisms of the inflammatory reaction, playing a great role in the outcome of heart function after MI, are far from being understood. Currently, gold-induced autologous cytokines (GOLDIC) are used in the treatment of inflammatory musculoskeletal disorders, such as osteoarthritis in humans and can modulate the inflammatory reaction. Therefore GOLDIC could also have an important role in ventricular remodeling after MI. We investigated the development of heart failure after myocardial infarction in mice by modulating parts of the inflammation by GOLDIC treatment.

The serum was generated by incubation of whole blood from wild type mice with specific gold particles for 24 hours at 37°C hence leading to a significant change in the composition of serum cytokines, which was proven by ELISA of the GOLDIC serum. 4 weeks after MI we performed echocardiography to measure left ventricular function and histological analyzes to investigate neoangiogenesis and scare formation.

Results: Echocardiography measurements showed significantly decreased ejection fraction (EF) (39.2±1.7 % vs. control 47.5±1.6 %, p<0.05) as well as fractional shortening (FS) (20.2±1.2 % vs. control 24.2±1.1 %, p<0.05) in the GOLDIC-treated group in comparison to the control group. Ventricular mass, heart rate and cardiac output were also diminished in the treated group. This demonstrated a reduced left ventricular heart function, which we did not expect before. The reduced function shown in echocardiography is supported by histological analyzes of the left ventricular infarction volume (36.4±2.9 % vs. control 22.6±2.8 %, p<0.05), which is significantly higher than in the control group. Also, the number of myofibroblasts (SMA positive cells) is significantly increased. This leads to a higher content of collagen in the scar of the heart in the GOLDIC-treated group and could also play a role in the reduced global heart function.

Conclusion: In conclusion, GOLDIC

treatment impaired the ventricular remodeling and worsening heart function, which unfortunately could not be stabilized by increased angiogenesis.

These effects, which could take influence on existing heart failures, should be taken into account when systemic therapies with GOLDIC serum are performed.

Strategies for biofunctionalization of the surfaces for small diameter vascular grafts Sandra Kraemer1, T. Breuer1, S. Cremers1, Mareike Staudt2, Elisa A. Epoxide represents an efficient activation strategy to covalently couple different endothelial progenitor cells (EPCs)-recruiting and antithrombogenic proteins to polycarbonate urethane (PCU) surfaces. Since this activation strategy itself had an impact on EPC and thrombocyte recruitment, we have investigated new strategies in terms of coupling efficiency, cytotoxicity and

cell recruitment. Two strategies to couple antithrombogenic, EPC-recruiting and anti-inflammatory proteins (RGD, Gro-α, RNase) to PCU-surfaces were compared: (1) activation by epoxide (epichlorohydrin) and (2) coupling with 1-ethyl-3-(3-dimethylaminopropyl) carbodiimide (EDC) and N-hydroxy succinimide (NHS) chemistry. Coating efficiency was assessed with an antibody-based detection method. To investigate cytotoxicity of the different coating strategies, cells were seeded on the surface and viability was measured via MTT assay after 5 days. Effect on EPC and thrombocyte adhesion was investigated in a self-designed chamber under laminar flow conditions. The coupling efficiency is comparable between both activation strategies but activation with epoxide generally affects cell viability. In addition, epoxide alone leads to an increase of thrombocyte and EPC adhesion in vitro. In comparison, activation with EDC/NHS alone does not increase unspecific cell recruitment. Coupling of RGD and RNase in a 1:1 ratio leads to a twofold increase of EPC adhesion and at the same time does not activate thrombocytes. Thus, PCU surface activation via EDC/NHS chemistry efficiently couples proteins to the surface and is less cell toxic than epoxide-based coupling. Coating with RGD/RNase (1:1) shows the most promising EPC-recruiting and thrombocyte repelling properties in vitro.

In dem für die neue Indikation eingereichten Dossier zu Ticagrelor hat AstraZeneca dargelegt, dass die Behandlung mit Ticagrelor 60 mg, auf eine Zielpopulation von 100.000 Patienten gerechnet, 1.000 Leben in 3 Jahren rettet. Es zeigte sich eine statistisch signifikante und klinisch relevante Senkung der Gesamtmortalität (20 %), der kardiovaskulären Mortalität (29 %) und der Mortalität durch koronare Herzkrankheit (28 %) bei Hochrisikopatienten nach einem Herzinfarkt. Auch das Risiko für weitere schwerwiegende kardiovaskuläre Folgeereignisse wurde statistisch signifikant (Herzinfarkte: Risikoreduktion von 17 %) reduziert. Diese Ergebnisse resultieren aus dem Datenzuschnitt, der die Zulassungspopulation gemäß Fachinformation abbildet.

Effektive Therapieoption für Hochrisikopatienten „Wir freuen uns, dass wir neben der Therapie von ACS-Patienten mit Ticagrelor 90 mg nun auch für diese Hochrisikopatienten eine neue und effektive Therapieoption zur Verfügung stellen können. Mit der IQWiGBewertung knüpfen wir an die positive Beurteilung von Ticagrelor im Rahmen des ersten AMNOG-Bewertungsverfahrens an“, sagt Dr. Julia Büchner, Vice President Pricing and Market Access bei AstraZeneca Deutschland.

Auf Basis der PEGASUS-TIMI 54 Studienergebnisse hatte die Europäische Kommission im Februar 2016 die Zulassung für Ticagrelor 60 mg zweimal täglich in Kombination mit ASS zur Behandlung von Patienten mit Herzinfarkt in der Vorgeschichte und einem hohen Risiko für die Entwicklung eines atherothrombotischen Ereignisses erteilt. Die endgültige Bewertung des Zusatznutzens nimmt der Gemeinsame Bundesausschuss (G-BA) im September diesen Jahres vor.

Augen, kürzl. aufgetretene intrakranielle Blutungen, bekannte o. vermutete Ösophagusvarizen, arteriovenöse Fehlbildungen, vaskuläre Aneurysmen o. größere intraspinale o. intrazerebrale vaskuläre Anomalien. Gleichzeitige Anwendung anderer Antikoagulanzien z.B. unfraktionierte Heparine, niedermol. Heparine, Heparinderivate, orale Antikoagulanzien außer bei Umstellung der Antikoagulation von o. auf Apixaban o. unfraktioniertes Heparin in Dosen, um die Durchgängigkeit e. zentralvenösen o. arteriellen Katheters zu erhalten. Nebenwirkungen: Häufig: Anämie; Blutungen am Auge (einschließlich Bindehautblutung); Blutungen, Hämatome; Epistaxis; Übelkeit; Gastrointestinale Blutung; Rektalblutung, Zahnfleischblutung; Hämaturie; Kontusion. Gelegentlich: Thrombozytopenie; Überempfindlichkeitsreaktionen, allergisches Ödem, anaphylaktische Reaktion; Pruritus; Gehirnblutung; Hypotonie (einschließlich Blutdruckabfall während des Eingriffs); Intraabdominalblutung; Hämoptyse; Hämorrhoidalblutung, Blutung im Mundraum, Hämatochezie; Erhöhung der Transaminasen, erhöhte Aspartat-Aminotransferase, erhöhte γ-Glutamyltransferase, abnormale Leberfunktionstests, erhöhte Blutwerte für alkalische Phosphatase, erhöhte Blutwerte für Bilirubin; Hautauschlag; Abnormale vaginale Blutung, urogenitale Blutung; Blutung an der Applikationsstelle; Okkultes Blut positiv; Postoperative Blutung (einschließlich postoperatives Hämatom, Wundblutung, Hämatom an Gefäßpunktionsstelle und Blutung an der Kathetereinstichstelle), Wundsekretion, Blutungen an der Inzisionsstelle (einschließlich Hämatom an der Inzisionsstelle), intraoperative Blutung; Traumatische Blutung, Blutung nach einem Eingriff, Blutung an einer Inzisionsstelle.
