1 Register online at activecare.engagementrx.com, then complete the Health Assessment to earn 20 points

Complete the Annual Wellness Exam and record the date and physician information and upload the completed Wellness Exam Form in the Incentives & Rewards tile in ActiveCare. Once you submit the information you will be advanced $300 of the incentive on the last payroll of the following month. OR participate in an onsite screening at your work location in order to receive the incentive in December. Completion of either option earns 30 points.

Complete the Tobacco Attestation based on your tobacco use status. Nicotine users, complete the "Attestation for Tobacco Users" and the Digital Course-Quit Smoking for Good to earn 20 points; non-nicotine users, complete the "Attestation for Non-Tobacco Users" and earn 20 points.

Choose and complete a course from your Plan for Wellness to earn 30 points. Courses will be assigned based on your Health Assessment responses.

Spouses enrolled in the medical plan are also able to par cipate and receive the same rewards as the employee.

1 Go to activecare.engagementrx.com and register your account.

2 Access Google Play or the Apple App Store and search “ALAVEDA.” Click “Get” or “Install” to download the app.

Once the download is complete, click “Open” to open the app or navigate to your homepage and click on the Alaveda App icon.

Click “Already Have a Login.” Enter your existing username and password and click the orange “Sign In” button. activecare.engagementrx.com
