Faculty Tools – Choose Testing Center Referral Form

Fill in all required information

Browse for your digital copy of the exam

Note any disability accommodations

Note Resources or Materials allowed during testing or special instructions

Browse for a class roster or add students individually (max 5)

Click Submit Once Finished
