Editorial Dear colleagues, friends, referring physicians, patients, partners and readers. On behalf of our team at the Department of Dermatology of the University Hospital Zurich, it is our pleasure to provide you with an update of our recent activities in this biennial report. This biennial report covers 2017 and 2018. During these past two years, we continued to improve the quality of patient care, research and education in the fields of dermatology, venerology and allergology. In November 2018, our chairman, Prof. Dr. Lars E. French, left our department. He followed a call to become the chairman of the world- renowned Department of Dermatology and Allergology at the Ludwig-Maximilians- Universität in Munich. During his 12 years, he has built the Zurich Department to become world class, both in terms of patient care and research. With more than 100 original research papers per year, which in 2017 and then again in 2018 had an impact factor of more than 1,000, our research department ranks among the leading institutions in the world. In the past two years, we also saw other faculty members following calls to lead Departments. Prof. Barbara Ballmer-Weber in 2018 became physician-in-chief ad personam in Allergology at the Cantonal Hospital in St. Gallen. In 2017 PD Dr. Dr. Wolfram Hötzenecker received the call to be physician-in-chief of the Dermatology and Allergology Department at the Kepler University Hospital in Linz. Last but not least, Prof. Dr. Dr. Alexander Navarini followed a call to become physician-in-chief and full professor for Dermatology at the Basel University Hospital. We have to thank all these colleagues for the many years they worked in Zurich and helped to build our Department. Within our own Department, two members were promoted to associate professors ad personam. In January 2017, Prof. Thomas Kündig started as Professor for Immunodermatology, and in June 2017, Prof. Mitchell Levesque started as Professor for Experimental Dermatology. In June 2018, PD Dr. sc. nat. Pål Johansen was promoted to adjunct professor for Allergology and Immmunology. Also, in the past two years, six members received their Venia Legendi: PD Dr. sc. nat. Hans Dietmar Beer, PD Dr. sc. nat. Philipp Bosshard, PD Dr. nat. Emmanuel Contassot, PD Dr. med. Martin, Glatz, PD Dr. med. Simone Goldinger, PD Dr. med. Beda Mühleisen. Cutaneous oncology is an important focus of our research. We have built a Skin Cancer Foundation for promoting both our melanoma research and expanding our extensive Biobank with live primary tumor cell lines that are linked to genomic, epigenomic and clinical data. Our thanks also go to the Bruno-Bloch Foundation, installed by the first chairman of our Department, which keeps supporting our research. In terms of fund raising events, we remain active. After organizing a concert with Anne-Sophie Mutter in 2013 and with soprano Edita Gruberova and the Zurich Chamber Orchestra in 2015, the Bruno Bloch Foundation successfully organized a

performance of the renowned Béjart Ballet Lausanne at Theater 11 in Zurich in 2017. Our department and the Bruno Bloch Foundation are extremely grateful to the above artists and the generous participants and donors for their continued generous support. Thanks to the efforts, especially of Prof. Lars French and Patrick Schaffter (www.brunoblochstiftung.ch), the foundation could double its yearly support to CHF 180,000 per year. Prof. Lars E. Year by year, we could also expand our already considerable third-party research funding, which reached approx. CHF 8 million in 2018. Our focus on cutaneous oncology is being promoted by a university-wide Research Priority Program in translational oncology. From 2012 to 2016, this program was financed with over CHF 6 million in collaboration with 11 outstanding physicians and scientists from the University of Zurich (www.cancer.uzh.ch). Based on an outstanding interim external evaluation, a further four years of funding with over CHF 11 million from 2017 to 2020 have been received. Secondly, several members of the Department of Dermatology in collaboration with clinician scientists in rheumatology, plastic surgery, angiology and pediatrics, as well as scientists and engineers from the University of Zurich and the Swiss Federal Institute of Technology in Zurich (ETHZ) under the direction of Prof. Dr. Sabine Werner (ETHZ) and Prof. L. French, won competitive seed-funding of CHF 1 million for a project called Skintegrity (www.hochschul-medizin.ch). The Skintegrity consortium in Zurich brings together leading experts in the above areas to address the need for more efficient diagnosis and therapy of major skin diseases and of acute and chronic wounds. Our department also keeps expanding its clinical activities. The number of outpatient visits has grown to 75,000 in the year 2018 and to 1,600 inpatients per year. In the past two years, twelve of our residents in training became successfully boardcertified in dermatology and venereology: Dr. Florian Anzengruber, Dr. Regula Brändli, Dr. Marie-Charlotte Brüggen, Dr. Samuel Conrad, Dr. Anna Frauchiger, Dr. Caroline Guillod, Dr. Omar Ali Hassan, Dr. Nicoletta Jaberg, Dr. Fabrice Kaufmann, Dr. Claudia Lang, Dr. Julia-Tatjana Maul, Dr. Selina Thurneysen. Also, eight residents in training became board certified in allergology and clinical immunology: Dr. Eugen Bersuch, Dr. Sabine Bruckert, Dr. Nadja Galliker, Dr. Caroline Guillod, Dr. Susann Hasler, Dr. Antonio Kolios and Dr. Iryna Yastremska. Challenges lay ahead of us and we are preparing ourselves for these. In 2020, our more than one century old Department of Dermatology building will have to make room for a new construction, and all of our research will be moved to the campus at Schlieren. Our outpatient facilities will be relocated to the University Hospital Zurich Airport (UHZ Airport, see www.usz.ch/bau). With this overview of selected milestones and developments for the period 2017 to 2018, we invite you in the name of our entire team to find out more on the following pages.

Assignment

The Department of Dermatology at University Hospital Zurich (UHZ) is devoted to the provision of sustained leadership in research, patient care and education in the fields of dermatology, venereology and allergology. Our nationally and internationally recognized department includes centers of excellence for skin cancer, inflammatory and allergic skin diseases, and dermatologic surgery. Our team of 47 specialist physicians offers high-level patient care in all areas of dermatology, venereology and allergology through our large-scale outpatient care clinic (Dermatologische Poliklinik), our inpatient dermatology service, our daycare unit and a network of tertiary care referral clinics covering a wide range of specialty areas, including melanoma and difficult pigmented lesions, non-melanoma skin cancers, cutaneous lymphoma, bullous and autoimmune skin disease, medical dermatology, psoriasis, hair and scalp disease, occupational and contact dermatitis, dermatologic surgery, dermatopathology and physical therapies (phototherapy, radiotherapy and laser therapy).

Our research activities focus on basic and translational research relevant to skin disease, primarily skin cancer and inflammatory skin diseases. The efforts of our research groups are reflected by regular publication in high-level journals and a high cumulative annual impact factor. Collaboration and networking with talented scientists and research groups at the University of Zurich, ETH, collaborative networks such as the Cancer Network Zurich, and other leading European dermatology centers help to significantly strengthen our research activities. Education is the third principle field of activity for our department. In the past few years, the department has focused its resources on pre- and post-graduate training, and the development and maintenance of DOIT (Dermatology Online with Interactive Technology), our innovative internet-based pre-graduate learning tool, in collaboration with the European Dermatology Forum.

In the Swiss healthcare system, the five university hospitals are responsible for providing virtually all of the country’s inpatient dermatology services. By contrast, outpatient care is available in both the private sector and public settings (university and cantonal hospitals). The UHZ Department of Dermatology is the largest academic dermatology center in Switzerland and provides stateof-the-art inpatient medical care for complex and rare skin problems. The department’s skilled surgeons perform complex surgical and histologically controlled procedures, and the team is able to administer complex chemotherapeutic regimens and intravenous immunotherapy, including antibodies and fusion molecules. We also provide an extensive range of outpatient services, including highly specialized consultations. Our department benefits from a large area referral base for these specialist activities and its own on-site histopathology laboratory.

Outpatient unit (polyclinic) and specialist care clinics Prof. Th. M. Kündig and his team of dermatologists and specialist nurses treat more than 22,000 patients on referral as part of more than 70,000 consultations a year. In contrast to general outpatient clinics, patients benefit from the expertise of international specialists in the fields of dermatology and venereology, and more specifically in autoimmune disorders, psoriasis, atopic dermatitis, hidradenitis, acne and other severe dermatologic diseases, hair, hyperhidrosis, pediatric dermatology, urticaria and more. The site is home to a weekly interdisciplinary wound clinic, which is organized in collaboration with the departments of vascular surgery, angiology, orthopedics and plastic surgery, and is intended for patients with chronic difficult-to-heal wounds. In our daycare unit, infusion therapies are administered in an outpatient setting. Most of our patients suffer from metastatic melanoma and are treated primarily using recently developed immunotherapies. These are highly effective treatment modalities. Further diseases treated at the daycare unit include inflammatory and autoimmune diseases, and cutaneous lymphoma. By relocating to a single room (C19), we have improved our monitoring capabilities, as it means one nurse and one physician can now be on site. The new, rapid point-ofcare laboratory analysis system for blood samples facili-

tates treatment of a shorter duration. Due to the rapidly increasing number of immunotherapies that are available, our capacity utilization has increased from 41% to 123%. We offset the resulting shortage of space and personnel by optimizing various processes, and are continuing to work on other improvements with the aim of making each patient’s stay at our daycare unit as comfortable as possible.

Skin cancer center – part of the Comprehensive Cancer Center Zurich Our department’s certified (DKG: German Cancer Society) skin cancer center is headed by Prof. R. Dummer, who directs a team of expert skin cancer specialists (Prof. R. Braun, PD, Dr. E. Guenova and Dr. J. Mangana). The center comprises the following clinics: – The pigmented lesion & skin cancer prevention clinic: this clinic is headed by Prof. R. Braun and makes use of modern high-tech equipment (confocal microscopy, skin surface video-microscopy, dermatoscopy, fullbody imaging) for the early detection of melanoma. – The skin cancer clinic is headed by Prof. R. Dummer and offers highly professionalized treatment and follow-up services for patients with different types of skin cancer (melanoma and non-melanoma skin cancer) as well as access to the most innovative clinical trials. – The cutaneous lymphoma clinic is headed by PD Dr. Dr. E. Guenova and PD Dr. K. Kerl (until September 2018). With the incidence of melanoma and other skin cancers growing worldwide and new therapies prolonging the survival of patients at advanced stages of the respective disease, complex medical care has become critical. The most effective care for complicated melanoma and skin cancer is provided in specialized referral centers such as the Skin Cancer Center, which is part of UHZ’s Cancer Center Zurich (www.cancercenter.ch). Our aims are to provide optimized melanoma therapy, record the best patient-reported treatment outcomes and carry out successful clinical and translational research, all of which require a dedicated interdisciplinary team. In today’s rapidly evolving scientific environment, modern skin cancer management is a typical example of precision medicine. Networking and cooperative projects involving basic research at local, national and interna-

tional level are crucial here. The inclusion of well-established local basic research groups and cooperative projects – including the investigation of patient samples collected in the biobank – is especially sought after. Clinical research with clinical trials sponsored by pharmaceutical companies and initiated by investigators is essential for creating a critical understanding of clinical science. Patients are regularly informed about these projects and actively support these efforts from time to time.

Dermatologic surgery and Mohs micrographic surgery Our dermatosurgery unit focuses on the surgical treatment of skin cancer through the application of Mohs micrographic surgery and “slow Mohs” following the “Tuebingen cake” technique. The staff team consists of Prof. J. Hafner, PD Dr. S. Läuchli, Dr. M. Barysch, Dr. S. Nobbe, Dr. S. Bruckert, Dr. M. Kunz, A. Reiser, A. Zambernardi, S. Kostadinova and other members. Together, they treat over 4,000 skin lesions (primarily malignant) each year. The vast majority can be operated on under tumescent local anesthesia. Frail and elderly persons are able to stay as inpatients for brief periods of hospitalization, making this center particularly suitable for geriatric dermatologic surgery. Specialized wound care is administered to a smaller number of patients, with a variety of debridement techniques and grafts being employed. Electrochemotherapy, CO2 laser surgery and other specialized techniques can also be provided.

Clinic for immunocompromised patients The clinic for immunocompromised patients is intended primarily for patients taking immunosuppressive medication following an organ or stem cell transplant, as well as patients taking continuous immunosuppressive medication to combat inflammatory diseases. The ultimate aim is to prevent skin cancer, as the risk of these patients developing non-melanoma skin cancer (NMSC) is at least 65 times higher than that of immunocompetent individuals. In addition, we provide information on prevention by means of protective measures (clothing, sunscreen, etc.). The clinic benefits from extremely close cooperation with the UHZ Transplantation Center, whose coordinators regularly refer patients to the clinic. With patients who have undergone an organ transplant now living longer, the number of patients is on the increase. In 2018, more than 1,900 patients were seen as part of more than 3,000 consultations during the clinic’s specialized consultation hour. We are also affiliated with international organizations such as SCOPE (Skin Care in Organ Transplant Patients Europe; scopenetwork.org) and ITSCC (International Transplant Skin Cancer Collaborative).

Physical treatment unit – including radiotherapy, phototherapy and laser therapy

Weekday inpatient ward

Headed by Dr. L. Imhof, this unit is equipped with the latest physical therapy technologies, including UVA1 devices, a full range of lasers, radiofrequency equipment and an intense pulsed light (IPL) platform. Two modern X-ray devices for the superficial radiotherapy of skin cancer and selected refractory inflammatory diseases were recently installed. Over 3,000 treatments are performed each year. The Department of Dermatology has a longstanding interest in the application of laser technology. The laser therapy and aesthetic dermatology unit attracts patients from all over the world seeking treatment for aesthetic and skin conditions. We perform a full range of laser procedures, such as permanent hair removal, tattoo removal, laser treatment for vascular disorders, both fractional and traditional resurfacing, CO2 and erbium treatments, and skin tightening procedures. We also provide numerous other treatments, such as injections with botulinum toxin, soft tissue fillers and chemical peels. Dr. L. Imhof, the director of the laser unit, pursues interdisciplinary collaboration with plastic surgeons, angiologists and gynecologists.

Headed by Dr. M. Nägeli, our weekday inpatient ward has a total of six beds for the provision of rapid, complex inpatient workups and short inpatient therapies. This unit also provides extracorporeal photopheresis (four photopheresis machines (as of January 2017) and another four beds) – a form of immunomodulatory treatment – to stem cell transplant recipients suffering from graft-versus-host disease, lung transplant recipients suffering from organ rejection, and cutaneous lymphoma (Sézary Syndrome) patients. The same floor is home to the bath therapy room, which is intended for the provision of complex dermatological therapies.

Our inpatient clinic The inpatient clinic provides 36 beds for the treatment of particularly severe and extensive forms of skin disease. It is especially suited to patients with refractory atopic dermatitis, psoriasis, complex autoimmune diseases, as well as skin cancer patients suffering from metastatic melanoma or cutaneous lymphoma and frail patients undergoing major skin cancer surgery for non-melanoma skin cancer. The medical staff (Prof. L. French, Prof. R. Dummer, Prof T. Kündig, Prof. J. Hafner, Dr. M. Nägeli, Dr. N. Galliker, Dr. N. Gräni and Dr. J.T. Maul, together with a team of staff physicians and residents) and the team of specialized nurses (S. Beeler, A. Andreoletti and more) apply their extensive knowledge and experience in order to provide the best possible care for severe skin diseases. The high level of therapeutic support, commitment and friendly atmosphere are highly valued by the patients.

Three departments in focus

a. Nursing care The UHZ Nursing Practice Model provides a conceptual frame for nursing care at UHZ and articulates nursing’s contribution to quality care. At its core is the provision of relational, person-centered care to individuals and families. In 2018, the nursing services provided at the dermatological clinic focused extensively on working environments as well as patient and family-centered care. As part of these efforts, various joint occasions were organized on a cross-discipline basis as a means of strengthening the working environment through partnership-based cooperation and promoting cultural development at our clinic. Two central themes here were the type of care provided and the promotion of patients’ self-management strategies. The aim of reference care is to establish a professional relationship with patients and their relatives. Caregivers are assigned to a patient and their family and

accompany them throughout their stay at the hospital. They form an interface between the various disciplines and professions, and are informed about the process, enabling them to support patients with optimum effect. Information sheets on various therapies and procedures have been prepared to provide support. This means that patients always have the opportunity to read information on their therapies when they need it. This creates a sense of security for the patients in an unfamiliar environment. They are also encouraged to commit to self-management and learn how to deal with their illness. Two brief training concepts – inhalation and anaphylaxis – for promoting the self-management of patients with allergies were developed based on evidence and integrated into the day-to-day work of the outpatient ward. All members of the inter-professional team were trained in correct inhalation techniques and the use of the autoinjector. This increased the professional competence and consulting quality of the nursing staff, promoted safe, correct inhalation and injection techniques, and boosted recognition of faulty applications and safe guidance toward patients. This makes it possible for the allergy ward’s inter-professional team to train patients in a structured, efficient and high-quality manner. According to initial experience, even people who have had an adrenaline autoinjector or inhaler for years greatly appreciate training of this kind and can benefit from it.

Laboratories: routine laboratories, dermatopathology and research laboratories Routine laboratories The department manages fully equipped routine laboratories for dermatopathology, allergology, molecular diagnostics, mycology and serology/venereology. Together, they are accredited according to ISO 15189:2012. In the last 12 years, the number of orders has more than doubled overall (see graph).

Dermatopathology The dermatopathology team examines around 30,000 skin samples per year. We offer high-quality diagnostics on formalin-fixed and native tissues (such as frozen sections), which can be supplemented as required by specific immunopathological investigations (immunohistochemistry, immunofluorescence and innovative special stains). Furthermore, we offer molecular biology examinations (PCR) for pathogens or lymphoma diagnostics. The UHZ Department of Dermatology has an internationally renowned biobank with a focus on skin tumors, which allows it to conduct additional molecular biology exams as required, and to combine the latest research with the dermatopathology findings. The establishment of the MelArray gene panel in particular has made it possible to offer personalized treatment to melanoma patients. Our close cooperation with colleagues from the Institute of Pathology enables us to discuss difficult cases from an interdisciplinary perspective on a weekly basis and to offer a wide range of additional molecular biological investigations. Our dermatopathology unit is a training center accredited by the international and European committees for dermatopathology (ICDP and UEMS). We attach great importance to the training of our future dermatologists and regularly hold dermatopathology courses for residents. Providing a personal and efficient service for our customers is extremely important to us, and we are always seeking to establish new partnerships and collaborations.

Other routine laboratories Aside from dermatopathology, the clinic is equipped with laboratories devoted to allergology, molecular diagnostics, mycology and syphilis serology. These labs are run by the laboratory head, PD Dr. P. Bosshard (together with Prof. P. Schmid-Grendelmeier in the allergology laboratory), and together analyze 15,000 patient samples per year. The allergology lab was among the first to use the ISAC (Immuno Solid-phase Allergen Chip) test in Switzerland, which allows for the simultaneous determination of

allergen-specific IgE against 112 allergenic components originating from more than 50 sources. The lymphocyte transformation test (LTT) and the basophil activation test (BAT) are both in routine use for diagnosing drug hypersensitivity and other allergic reactions. Instances of syphilis have increased dramatically in the last 20 years (see figure). In the serology lab, several new cases are detected each week. Additionally, a multiplex PCR for genital ulcer disease has been developed that allows for the simultaneous detection of T. pallidum (syphilis), herpes type 1 and 2, and H. ducreyi (chancre). Other tests in the molecular diagnostics lab include the T-cell receptor clonality (TcR) and the immunoglobulin heavy chain (IgH) assays for the diagnosis of lymphoproliferative disorders. The mycology lab is renowned as a reference laboratory, in particular for dermatophyte infections. Thanks to the long-standing experience of the staff and the head of laboratory, this facility is able to provide courses and teaching sessions for various stakeholders, e.g. resident doctors, pharmacists and lab technicians.

Research laboratories The continued expansion of our research activities encompassing around 65 researchers has led to an expansion of our research space. We now operate research laboratories on the top floor of our department at Gloriastrasse 31, as well as in two research buildings at the new biomedical campus in Schlieren (Wagistrasse 12 and 14). The latter contain state-of-the-art equipment for performing competitive research in the fields of biochemistry, molecular biology, cell biology, immunology and allergy. For animal experiments, we have access to a modern animal house at UHZ and in Schlieren. Our research labs are certified according to ISO standards and comply with GMP guidelines for patient samples.

c. The allergy unit Diagnosis and management of the full range of allergic diseases

phylactic shock or severe cutaneous or systemic reactions such as DRESS, Stevens-Johnson syndrome and TEN). We provide a full range of skin tests, state-of-the-art in vitro diagnostics including cellular tests (BAT, modified LTT and cytotoxic assays) and, if necessary, a variety of provocation tests and drug deactivation protocols for vaccines, biopharmaceuticals, cytostatic drugs and so on.

Allergic diseases are increasingly common and account for around one third of our patients. We cover the full range of allergies, attending to allergy-related skin diseases such atopic dermatitis, urticaria and contact dermatitis. We also cover very common inhalant allergies such as rhinoconjunctivitis and allergic asthma due to pollen, mites or pets, as well as increasingly prevalent allergies and intolerances to food, e.g. nuts, celery and seafood and food components such as histamine, gluten and lactose. Hymenoptera-related allergies, (e.g. allergic reactions to bees or wasps) can be attended to and, if necessary, treated with highly efficient and fast-acting ultra-rush immunotherapy regimens. We offer the full range of diagnostic tools, including molecular-based allergy diagnosis, all kinds of provocation tests and a large variety of treatments with conventional drugs, biopharmaceuticals and immunotherapy. To fulfill the needs of our patients as effectively as possible, we maintain very close interdisciplinary ties with Clinical Immunology, ENT, Gastroenterology, Hematology, Pulmonology and departments providing innovative immunotherapies.

Patient education is one of our priorities. We adopt an interprofessional approach in order to provide and teach individual patient education for anaphylaxis, asthma and atopic dermatitis. In addition, we maintain ties with special clinics that cover rarer diseases such as mastocytosis and hereditary angioedema.

Drug hypersensitivity – a major challenge

As there is an increasing need for well-trained personnel, we offer training opportunities to young doctors to become board-certified specialists in allergies and immunology and training opportunities for allied health professionals.

For mastocytosis, an interdisciplinary board together with hematology, rheumatology, gastroenterology and psychosomatic experts has been initiated by Dr. Claudia Lang. It allows us to treat our mastocytosis patients in accordance with the latest international guidelines and treatment protocols.

Research

One ongoing aim is to improve the quality of our services for patients. We are committed to the highest level of research in dermatology. Our achievements in this field – as judged by the cumulative impact factors for 2017 and 2018 (1156 and 1089) – position us among the world’s leading departments. Our research activities focus primarily on basic, patient-oriented translational and clinical research in dermato-oncology, where we concentrate on the pathogenesis, early diagnosis and targeted treatment of skin cancer, particularly of melanoma, cutaneous lymphoma and non-melanoma skin cancers. Immunodermatology is also a research focus, in particular innate immunity and T-cell biology in inflammatory skin diseases, notably psoriasis, neutrophilic dermatoses, cutaneous drug eruptions and severe allergic adverse drug reactions. Allergy research is centered on atopic dermatitis, drug and food allergy as well as immunotherapy.

The aim of our research activities is to: – promote high-quality research with clinical relevance in the defined fields of focus. – promote intensive networking and collaboration with other institutes and departments at the University of Zurich and ETH, and through active participation in networks such as Cancer Network Zurich and Life Science Zurich. – support fellowships abroad in excellent research institutions and maximize competitive research funding, including from the Swiss National Science Foundation (SNF professorships). – create endowed professorships in our department for inflammatory skin disease, skin cancer and allergy, with the aim of recruiting and retaining leaders in these research fields.

SSS Weekend (Science, Sun and Snow)

Approximately 40 clinical trials on new therapeutic approaches for skin disease are ongoing, corresponding to around 15% of all studies performed at UHZ. In 2018, our department published more than 140 original research articles and approximately 44 reviews in peer-reviewed journals.

Our 22nd and 23rd Science, Sun and Snow (SSS) weekends took place in 2017 and 2018. Approximately 50 researchers participated in this event, which was held in the Ameron Hotel and Arabella Hotel in Davos. Over the course of one and a half days, our young researchers presented their work and updated the attendees on their progress, which also led to intense discussions. The event also included a ski day, dinner and a fondue evening, creating the perfect atmosphere for scientific exchange.

a. Three research groups in focus In this report, we would like to look at three of our 14 research groups in more detail as a means of demonstrating the scope of research in our department.

1. “Tools for precision medicine in melanoma” Prof. Mitchell Levesque, PhD Recently, melanoma therapies have experienced dramatic success in the clinic, with more than half of all patients receiving some benefit. With access to one of the largest live-cell melanoma biobanks in the world, we have a unique resource through which to study the mechanisms of how gene expression and cellular diversity contribute to the ability of melanoma tumors to respond to targetedor immunotherapies. For instance, we – together with other groups – have contributed to the development of a “phenotype switching” model of melanoma progression, in which cancer cells are able to alternate between different fates and cellular behaviors (Figure 1). cyte into a melanoma cell. Most of these initial melanoma cells are melanocytic in nature (green), but a phenotype switch as the result of micro-environmental stresses (such as hypoxia, tumor lymphocyte infiltration, etc.) or therapeutic intervention can induce mesenchymal features (red). The role of that switch in tumor progression and therapeutic resistance is the main focus of our research. Some projects in our lab are aiming to acquire a better understanding of how this switching behavior occurs in tumors in different contexts (e.g. by using single-cell sequencing approaches or proteomics techniques such as CyTOF). Meanwhile, other projects aim to target the novel features of cells that switch between these fates. By screening thousands of small compounds, we may be able to identify drugs that can specifically kill cells in each of these melanoma fates. We hope some of these novel drugs could one day lead to better tumor control in the clinic.

A vaccination that protects against cancer is a longstanding dream in medicine. This dream has now become a reality thanks to the vaccination against human papillomavirus infections and therefore protection against the cervical cancer that they can trigger. However, serious success has thus far eluded us when it comes to protection against other types of cancer. This is because the immune system can process vaccines in two ways and cancer vaccines are processed in the wrong way.

Gene information encoded by DNA is transcribed into messenger RNA (mRNA) that are translated into protein. The mRNA can be synthesized in vitro (“ivt mRNA”) and transfected in cells in order to express the desired protein. Because ivt mRNAs are transiently present in the cytosol, their use is safe and highly effective.

Every immune response begins with specialized antigen-presenting cells (APCs) absorbing the antigen. Depending on whether the antigen floats inside the defense cells in an endosome or freely in the cytosol, it is degraded in different ways and identified on the surface of the APC. By default, antigens enter the endosome and from there ultimately reach the MHC2 complex, which plays a key role in the immune system, especially in the interactions for the production of antibodies. However, when an antigen enters the cytosol, it binds to the MHC1 complex, which is primarily associated with the maturation of cytotoxic T-cells (CTLs). CTLs are more effective than antibodies in the fight against tumor cells. The aim of our research project is therefore to transfer the vaccines from the endosomes into the cytosol of the APCs. In experiments with mice, we injected the vaccine together with a light-sensitive substance under the skin. When the mice were irradiated with intense light, the light-sensitive substance caused the endosomes inside the APCs to burst. This resulted in the vaccine entering the cytosol, and the immune response led to an increased formation of cancer cell-killing CTLs.

The mRNA platform at URPP – Translational Cancer Research – provides optimized (“AptaMessengerTM”; Figure below) and purified ivt mRNAs as well as optimized formulations for the desired site/duration of expression. Through this, fundamental and pre-clinical investigations aiming at studying or utilizing a protein activity in vitro or in vivo can be undertaken in all research groups of the dermatology department as well as at collaborating institutions in Switzerland.

This photochemical internalization (PCI) works well in vitro and in mice. In cooperation with an industrial partner, the first experiments on humans are now underway. If the results from animal experiments are confirmed, we will have taken medicine another step closer toward fulfilling our dream.

Main Fields of Research Our research activities are focused on food allergy, in particular pollen-related food allergy. We are especially involved in the identification and molecular characterization of important cross-reactive allergens, and the development of diagnostic and therapeutic methods based on recombinant allergens. Allergen-based or component resolved diagnosis in food allergy is a major topic of our research group. Another important aspect of our research activity is risk assessment in food allergy with regard to genetic modified foods/novel foods but also threshold doses (minimal allergen doses to induce an allergic reaction) of plant and animal food allergens.

Main Fields of Research The group is interested in the molecular mechanisms underlying inflammation and cancer development in human skin. The focus is on the roles of inflammasomes expressed by epidermal keratinocytes in these processes. Studies are based on patient-derived primary cells, such as keratinocytes, skin cancer cells and (cancer-associated) fibroblasts. The dermal-epidermal crosstalk of wildtype cells and those modified by the CRISPR/Cas9 approach is analyzed in a three-dimensional skin model. State-of-the-art proteomics approaches are used to identify novel regulatory pathways.

Main Fields of Research Our main interest is non-invasive diagnosis of skin cancer, especially malignant melanoma. We use established techniques such as dermoscopy as well as new techniques in vivo laser scanning microscopy. We also have a strong interest in artificial intelligence. Due to different international research collaborations, our patients benefit directly from the latest technological developments. Detecting skin cancer, especially melanoma, at early stages offers a unique opportunity because, if diagnosed early enough, skin cancer can be cured.

Main Fields of Research Cutaneous inflammation is a very common condition in dermatology, and numerous insults from the environment or endogenous danger signals can lead to inflammation. The aim of our research is to better understand the molecular events leading to inflammatory responses in the skin. We are particularly interested in the mechanisms involved in i) the immune responses involved in cutaneous adverse drug reactions such as Acute Generalized Exanthematous Pustulosis; ii) the regulation of keratinocyte inflammasomes in response to danger signals; iii) the role of CARD14 variants in psoriasis. We found that i) the inflammatory cytokine IL-36 is overexpressed in AGEP patients’ myeloid cells and keratinocytes in response to drug exposure; ii) the p38 MAP-kinase positively regulates the NLRP1 inflammasome in primary keratinocytes; and iii) the presence of gain-of-function mutations in CARD14 is sufficient to induce a psoriasiform phenotype in mice. These new findings highlight the key role of deregulated innate immunity in the skin and keratinocytes in pathological conditions.

Main Fields of Research Our research group is focused on immunotherapy of allergies and cancer. In the area of allergies, we aim to improve and shorten current regimens for immunotherapy or desensitization, which today require three to five years of allergen administration. We found that direct administration of allergens into a lymph node made it possible to shorten immunotherapy from 60 to merely three injections. Also, we developed epicutaneous immunotherapy, where allergens are applied by use of a skin patch, shortening a course of treatment to merely six patch applications. Using virus-like particles, we aim to further reduce the number of allergen applications to one or two. We are also using virus-like particles for anti-cytokine vaccination in horses and dogs as well as in cats to render them hypoallergenic. In the area of cancer research, we focus on the role of cytokines in tumor cachexia and we are working on RNA-based cancer vaccines.

Clinical research is a major focus at academic centers such as ours. It is essential to translate new basic research results into beneficial treatment regimens. Clinical trials today are performed within an increasingly regulated network that combines clinical, ethical, legal and economic aspects. Respect for the patient’s life and well-being and the integrity of all personnel involved (principles of good clinical practice) are absolutely critical. A network of clinical trial specialists is necessary so that clinical trials can be integrated into the treatment strategy for the respective disease. The Department of Dermatology and the skin cancer unit have a long, successful record in this context. We are a major research center for the development of targeted therapies and immunotherapies for skin cancers and immune-mediated skin diseases. Our center’s clinical trials unit has extensive experience in early and late phases of clinical development (phase I-IV). The unit is certified by SAKK to perform phase I trials and offers a post-graduate study in pharmaceutical medicine. The unit also has its own trial pharmacy. Three dedicated trial pharmacists are part of the team and have established pharmaceutical management processes that have further elevated the level of quality for our trial patients.

We focus on the development of investigator initiated trials (IITs) for the treatment of orphan diseases of the skin and for sub-groups of skin cancer patients with high medical requirements, such as those with brain metastases.

Our clinical trial unit is now one of the leading clinical research centers in Europe Recognized as a SAKK Phase I Center on September 12, 2016, the UHZ dermatological clinical trial unit was officially approved by the Swiss Association for Clinical Cancer Research (SAKK) to conduct SAKK phase I/first in human (FIH) studies.

Research collaboration – Skintegrity and university research priority project, translational cancer research (URPP)

SKINTEGRITY – A flagship project on skin In autumn 2016, a new, large-scale interdisciplinary project under the University Medicine Zurich partnership was initiated with the aim of establishing our Zurich location as one of the world’s leading centers for skin research. The project is headed by S. Werner, Professor of Cell Biology at ETH, L. Sommer, Professor of Anatomy at the University of Zurich, and Prof. R. Dummer, Department of Dermatology, University Hospital Zurich, who replaced Prof. L. French from the Department of Dermatology at the University of Zurich (now Department of Dermatology at the University of Munich). The increasing relevance of skin diseases for the general population, such as skin cancer, chronic skin wounds and scleroderma, serves as the basis for this project. Through unique collaboration between highly specialized clinicians and scientists from the University of Zurich and ETH, the best expertise in biology, medicine, mechanics and engineering can be used to conduct rapid, targeted research with the aim of improving the treatment of severe skin diseases. We focus on the development of innovative diagnosis and treatment of difficult or non-curable skin diseases, including chronic wounds, large burn wounds, scleroderma, lymphedema and epithelial skin cancers.

Program “Annual Event 2018”

University research priority project, translational cancer research (URPP) Since the launch of the university research priority project (URPP) for translational cancer research in 2012, the Department of Dermatology has served as a key contributing partner and has greatly benefited from the interaction and financial support provided. The University of Zurich has committed more than CHF 6 million over a period of eight years to finance this URPP, with its primary aim being to bridge the gap between fundamental and clinical cancer research at the university. The URPP was directed by Prof. L. French until October 2018 (Successor: Prof. R. Dummer), supported by vice director Prof. K. Basler and scientific coordinator Prof. M. van den Broek. The funding for URPP translational cancer research clearly underlines the commitment of our university with regard to promoting innovative cancer research based on high-quality science and translating it to the clinic. URPP translational cancer research is unique in that it unites an exceptional team of internationally recognized scientists and physician scientists in the field of oncology and cancer research, and is committed to bridging the gap in communication and collaboration between physicians and scientists and thereby bringing cancer research a step closer to application in the clinic. URPP translational cancer research focuses on the following four sub-projects: 1. Oncogenic signal transduction pathways as targets for personalized tumor therapy 2. Interaction between cancer and the human immune system 3. Tumor biopsy and live tumor cell biobank linked with clinical outcome data

In 2015, the performance of the URPP translational cancer research project was evaluated by three external experts commissioned by the university. Based on the very positive evaluation, productivity and outlook, the University of Zurich decided to continue the project in 2016 and increased financing by an additional CHF 11 million for the next four-year period (2017–2020). The project facilitates innovation thanks to its dedicated post-doctoral fellows who liaise between clinical and basic labs and the high-level mentoring provided by the principal investigators in clinical and basic science. As a consequence of the research strategy set out above, an increasing number of interesting publications have been prepared. One example is the collaborative work between the groups led by Prof. Sommer (Institute of Anatomy, University of Zurich), Prof. Levesque and Prof. Dummer (both at the Department of Dermatology, UHZ) focusing on the control of melanoma growth and metastasis through the silencing of distinct tumor suppressors by the epigenetic modifier EZH2 (Zingg D, Debbache J, Peña-Hernández R, Antunes AT, Schaefer SM, Cheng PF, Zimmerli D, Haeusel J, Calçada RR, Tuncer E, Zhang Y, Bossart R, Wong KK, Basler K, Dummer R, Santoro R, Levesque MP, Sommer L.; Cancer Cell. 2018).

Research collaborations – foundations and associations

Bruno Bloch Foundation

One highlight on the Bruno Bloch Foundation calendar is its annual Memorial Lectures, which take place in February and are attended by more than 250 faculty members and students at UHZ. In 2018, Prof. Thomas Bieber, Chair of the Department of Dermatology and Allergy at the University of Bonn, held the Bruno Bloch Memorial Lecture. The Bruno Bloch Foundation organized a charity recital with the exceptional contribution of Anne-Sophie Mutter for the first time in 2013. The funds collected served to finance research for skin diseases and skin cancer within the Department of Dermatology at UHZ. Important donors supported this charity evening and made a substantial contribution toward this research. Due to its success, the Bruno Bloch Foundation organized two other charity evenings – one in November 2015 with the participation of soprano Edita Gruberova and the Zurich Chamber Orchestra (ZKO) and one in May 2017 with the exceptional participation of the Béjart Ballet Lausanne. The next charity evening will be organized in 2020.

In 2015, the Board of Directors of the Foundation decided to grant two fellowships each year to young dermatologists (residents) from the Department of Dermatology at UHZ for further training abroad. Michael Kunz, MD, was awarded a fellowship in 2017 and joined the team led by Prof. John Paoli at Sahlgrenska Hospital in Gothenburg/ Sweden (research on skin cancer), and collaborated with several institutes in the USA, in particular with the MD Anderson Cancer Center in Houston (one of the largest cancer centers in the world). Julia-Tatjana Maul, MD, received the fellowship in 2018 and collaborated with the Skin Inflammation & Psoriasis International Network in South America. From 2016, selected guests have been invited by the Bruno Bloch Foundation to attend its breakfast round table. At this event, a representative of the medical staff at the UHZ’s Department of Dermatology presents a specific topic of general interest in the field of dermatology to the audience. In 2017, Prof. Alex Navarini held a presentation titled “The Next Generation of Dermatology”. The Bruno Bloch Foundation also organizes the yearly Zurich dermatology training days. In autumn 2018, donators were invited by the Foundation to visit the Museum of Wax Moulages, which is managed by the University of Zurich and University Hospital Zurich (see also 14. Museum of Wax Moulages). Association for Research on Cancer of the Skin (Verein für Hautkrebsforschung)

Creation of a new position: skin cancer nurse

Founded in 2004, the Association for Research on Cancer of the Skin (Verein für Hautkrebsforschung; VHKF) aims to coordinate and stimulate skin cancer research in Switzerland by supporting the research of rare and severe forms of skin cancer (e.g. melanoma, cutaneous lymphoma, etc.). The board consists of Prof. R. Dummer (President), Prof. R. Braun, Prof. L. French and C. Frey-Blanc (treasurer), all of whom are based in Zurich, along with Prof. D. Hohl (Vice-President; based in Lausanne), Prof. R. Hunger (Bern), Prof. R. Kaufmann (Frankfurt) and Prof. W. Kempf (Zurich). The current goal is to generate a substantial contribution for clinical and experimental skin cancer research in Zurich and other institutions in Switzerland. Recently, the association created an assistant professorship position at the University of Zurich. Prof. M. Levesque, an internationally renowned melanoma researcher, will hold the position until he receives a full professorship with tenure track at the university. His research activities involve developing new tools for understanding next generation sequencing data, and performing functional tests of novel melanoma mutations in the laboratory. In this context, he is using the zebrafish as a model organism in order to improve the understanding of the biology of cancer genes. In his new role, Prof. Levesque also manages the dermato-oncology biobank at UHZ, which provides a diverse collection of live cancer cells to collaborators for experimental research purposes. These are fundamental research activities that form the basis of understanding of melanoma progression, thereby enabling improvements to be made in melanoma therapy in the future. In the past year, Prof. Levesque has also established a molecular tumor board for melanoma with the help of the ETH Nexus platform and an innovation grant from UHZ. This board will provide patients who have exhausted other therapy options with new possibilities based on the genomic sequence of their tumors and the availability of new targeted pathway inhibitors in the clinic. In addition, Prof. Levesque has obtained funding from the Skintegrity flagship project at ETH to continue to expand the dermatology biobank and make it more accessible to researchers across Zurich. Finally, collaborations with Roche and Bristol-Myers Squibb have led to further funding being provided in order to identify novel predictive biomarkers for melanoma immunotherapy treatment. These efforts have resulted in a publication that describes novel markers of circulating macrophages in patients receiving anti-PD1 inhibitors that were shown to have a predictive

Although significant progress has been made in the past few years thanks to the variety of treatment strategies in metastatic melanoma, physicians and patients may have overlapping or divergent expectations regarding the outcome of cancer therapies and the quality of the medical services provided. Despite this, virtually no communication takes place between physicians and patients in this regard.

The role of nursing in systemic therapy should be mentioned here. As of October 2016, a specialist skin cancer nurse position – financed by the Association for Skin Cancer Research (Verein für Hautkrebsforschung) – has been integrated into the treatment processes and is responsible, in particular, for regular contact with patients regarding adverse events (AE), which can occur at any time during treatment. Many patients find it easier to phone a nurse than a doctor; nurses can therefore serve as an approachable liaison between the patient and the treatment team. For this position, the nurse must be familiar with the specific AE spectrum and the timing of AEs during therapy.

The Christine Kühne Center for Allergy Research and Education (CK-CARE) is dedicated to the research and education of allergic diseases with a specific focus on atopic dermatitis. Founded in Davos in 2009, it has led to the creation of a network of research groups in Davos, Bonn, Munich, St. Gallen and the UHZ’s allergy unit (see www.ckcare.ch). In the intervening decade, this foundation has become part of a larger initiative with a broad focus on various allergic diseases, with the Allergy Campus Davos serving as the nucleus.

Kantonsspital Thurgau AG: dermatological outpatient clinic at Kantonsspital Frauenfeld

A dermatological outpatient clinic at the Kantonsspital Frauenfeld (cantonal hospital; KSF) has been in operation since 2011, and works in close collaboration with the UHZ Department of Dermatology. The clinic is run by two senior dermatologists from the Department of Dermatology who divide their hours between UHZ and KSF. The team of physicians receives support from a senior dermatology resident from UHZ who is sent to Frauenfeld for a sixmonth rotation. Dr. St. Nobbe currently serves as head of the clinic. Other senior dermatologists who have held positions at the clinic include Dr. L. Wiederkehr (May 2016– June 2017), Dr. P. Kraenzlin (June 2017–August 2018) and Dr. A. Frauchiger (since September 2018).

Kantonsspital Winterthur

Kantonsspital Winterthur has overseen a dermatology and allergy outpatient clinic for several years now. Chaired by Dr. Marianne Lerch, its services are provided in close collaboration with the Department of Dermatology at UHZ. Dr. Lerch teaches students at UHZ, while a senior physician position is shared between the two institutions (currently filled by Dr. Carla Murer).

Kantonsspital Graubünden, Chur

A dermatology service based on a cooperation model with UHZ has been provided at Kantonsspital Graubünden in Chur since 2017. This service is overseen by Dr. Carina Butzmann, a FMH-certified specialist in dermatology and venereology. She works at both institutes, which facilitates a close professional exchange with the UHZ’s Department of Dermatology. Dr. Butzmann’s expertise covers a wide range of dermatological diseases, including pediatric skin diseases. The service is in high demand and works to meet the requirements of referring doctors and other departments at the hospital. Dr. Butzmann has also established a CME circle for local dermatologists, during which talks are provided by external experts and experiences of own cases are exchanged.

The clinic covers a broad range of outpatient dermatology services and has a radiotherapy unit. The majority of patients are referred by practicing physicians. In addition, the clinic is responsible for dermatological inpatient consultations for all hospitals in the canton of Thurgau. Special consultation hours are held on a monthly basis with professors from UHZ.

UHZ works very closely with Hochgebirgsklinik Davos and CK-CARE, both of which are members of the Health and Life Science Campus in Davos. The Kühne Foundation was responsible for establishing an Allergy Competence Center with new research and educational facilities. There are special programs for patients with allergic and inflammatory skin diseases and food intolerances (especially histamine intolerance). Special programs involving a high level of patient care paired with specific educational programs and nutritional and psychosomatic support result in a unique hospitalization experience in a special setting with

fully renovated private rooms. In addition to medical care, collaboration includes training and teaching exchanges for medical doctors, students and allied health programs.

In close connection with the Department of Dermatology at UHZ, Dr. Elisabeth Hohenstein Jaccard runs a two-day dermatology service at Zuger Kantonsspital for outpatients who have been referred and internal consultancies. Prof. Peter Schmid-Grendelmeier oversees this service on the part of UHZ and, together with PD Dr. Martin Glatz, serves as an external consultant for allergic diseases at Zuger Kantonsspital.

Kantonsspital St. Gallen; Department for Dermatology, Venereology and Allergology

The development of the new Department for Dermatology, Venereology and Allergology at Kantonsspital St. Gallen continued to gain momentum in 2018. The allergology department in particular received a significant boost thanks to the appointment of Prof. Barbara Ballmer-Weber as head physician ad personam. On May 4, 2018 – as part of a symposium to mark the opening of the clinic – we presented the new clinic building in House 20 to approximately 200 participating Swiss dermatologists. In the first year alone, the number of outpatients with the clinic’s new B-training status has increased by more than 20% to approximately 19,000 visits. In September 2018,

250 national and international skin lymphoma specialists attended our international congress as part of the EORTC-CLTF (Cutaneous Lymphoma Task Force) in St. Gallen (co-organized with PD Dr. Emmanuella Guenova, UHZ, and Prof. Werner Kempf, Zurich). The clinic presented Swiss dermatology as an innovative specialist field with an excellent supporting network.

Medical education and training

Medical education is essential for guaranteeing a high level of professionalism among physicians and healthcare professionals. Our continued commitment to education is reflected by: 1. Student education at the University of Zurich’s medical faculty. 2. Specialist training for resident physicians qualifying in dermatology and venereology, and allergology, clinical immunology and phlebology. 3. Ongoing medical education for private practitioners – primarily dermatologists and allergologists, as well as general practitioners and (to a lesser extent) other healthcare professionals.

Student education Senior physicians at our department provide an introductory dermatology course for third-year medical students, a yearly core course in dermatology (as part of an eightday block) for fourth-year medical students, and a casebased thematic lecture series and a practical training course in dermatology in the sixth year of the medical training curriculum at the University of Zurich. ETH Zurich has offered a curriculum for a Bachelor’s degree in medicine since 2017. As part of this program, several members of our department are involved in teaching introductory modules in dermatology, immunology and allergology. These ex-cathedra and bedside teaching activities are complemented by web-based learning modules involving an online platform (Dermatology Online with Interactive Technology; DOIT), which was developed by our department under the direction of former chairman Prof. Gunter Burg in close collaboration with several other dermatology departments (Basel, Bern, Lausanne, Jena, Stadtspital Triemli in Zurich and the European Dermatology Forum; see also www.cyberderm.net). This platform has been further improved under the direction of Prof. Peter Itin with the support of the European Dermatology Forum and is currently available in seven languages with two further languages scheduled. In 2009, the DOIT platform was chosen as the best internet-based teaching platform and won the Medida Prize (worth EUR 50,000).

In addition to a host of lectures and courses, a new course on translational allergy was launched with great success in 2017. Held at Hochgebrigsklinik Davos, this course has become highly popular in the meantime and is rated as “very useful” by the participants.

Resident physicians The Department of Dermatology at UHZ is a training site for specialization in dermatology/venereology, according to the training program approved by the Swiss Institute of Medical Education (SIME-SIWF). Four of the requisite five years of dermatology training can be completed on site. Resident candidates must fulfill the following requirements: – Two years of medical training at a Swiss hospital: either two years of internal medicine or one year of internal medicine and one year of surgery. – Finalized or pre-finalized MD thesis. Our staff provides opportunities to conduct scientific work alongside an MD thesis. Three training positions are also available that cover part of the board certification in allergology and clinical immunology and angiology. During their residency training in our department, residents benefit from the most comprehensive program in Switzerland, comprising a rotation plan of 20 units and specialist clinics covering all subspecialties of dermatology and allergology, including dermato-oncology, autoimmune skin disease, trichology (hair disease), dermatosurgery, dermatopathology, physical skin therapy (radiotherapy, UV therapy, laser therapy), phlebology and others. In addition, three residency positions are available at the allergy unit for board certification in allergology and clinical immunology. These positions count for the full three years that are required in a specialist clinic to obtain the FMH qualification. The quality of the training and teaching receives a very high rating among residents.

Zürcher Dermatologische Fortbildungstage 2017 / 2018 The Zürcher Dermatologische Fortbildungstage (ZDFT) were launched in 2011 and immediately proved to be highly successful, as reflected in the feedback and number of participants (primarily practicing dermatologists). At last count, this figure stood at more than 240 physicians, thus confirming that this event is one of the most important dermatology congresses in the Germanspeaking region of Switzerland. The three-day intensive event for ongoing medical training offers a mix of plenary lectures and hands-on workshops at the Department of Dermatology, and a half-day session titled “What’s new?”. Each congress centers on an annual theme, which is addressed by a number of international speakers. In 2017, the theme was “(Rare) Skin Tumors and Tumor Syndromes”, while in 2018 it was “Vascular Disorders”. Both subjects proved to be engaging and enriching, including among practitioners in dermatology. For the 2018 edition, four workshops were held for medical practitioner assistants (MPA) and were overseen by our nursing/MTRA and physician team.

The event in 2017 marked the first time that all presentations were made available on the website free of charge. They could be accessed by participants and other interested parties for a period lasting approximately one year, thus providing the opportunity to continue to explore the subjects presented long after the end of the congress. Interesting, interactive exchanges of this kind between our team and our speakers may motivate us to expand this format for medical education to other locations in future.

41st Annual Meeting of the Arbeitsgemeinschaft für Dermatologische Forschung, March 8–10, 2018, Zurich From March 8 to 10, 2018, the 41st Annual Meeting of the Arbeitsgemeinschaft Dermatologische Forschung (ADF) took place in Zurich. This marked the first time in 17 years that this important German research conference has been held in Switzerland. It was very well attended with more than 400 participants, while the local symposium on “Precision Medicine in Dermato-Oncology” led by PD Dr. Emmanuella Guenova and Prof. Reinhard Dummer attracted a great deal of interest in particular.

Other selected meetings SGDV Fostering course in dermatology/phlebology, March 2018 Phlebology and wound management are two traditional mainstays of dermatology and venereology. Tumescent local anesthesia, ambulatory phlebectomy and endovenous laser thermal ablation are all milestone procedures that have originated from the field of dermatology, as have innovations in compression therapy, shave therapy for venous ulcers, and further research in wound healing. Despite these breakthroughs, the importance of phlebology within dermatology has declined in recent decades. Now is a prime opportunity to introduce the latest generation of dermatology residents to this attractive subspecialty. For this occasion, the Department of Dermatology at UHZ put on its first fostering course on phlebology and wound management on March 23 and 24, 2018, under the auspices of the Swiss Society of Dermatology and Venereology. The course consists of five theoretical and corresponding practical modules that cover the entire field.

The forty participants – primarily from Switzerland with a few representatives from other EU countries – were highly motivated, with the course receiving excellent feedback and ratings as a result. SGDV 10th & 11th International Dermatoscopy Course, September 21, 2017 and September 27, 2018

Basic course in laser technology (Organization: Laurence Imhof, MD), December 2018

The annual 10th and 11th SGDV International Dermatoscopy Course (chair: Prof. R. Braun) took place in Zurich at UHZ in September 2017 and 2018. Attended by more than 220 participants, the course featured an array of international speakers, including Prof. Dr. Ashfaq Marghoob (New York, USA), Prof. Dr. Luc Thomas (Lyon, France) and Prof. Dr. Aimilios Lallas (Thessaloniki/Greece).

On December 7, 2018, the first basic course in laser technology (FMCH credits) was held at the Careum Auditorium in Zurich (chair: Dr. Laurence Imhof). The aim of the course was to prepare the participants (physicians) for the FMCH basic course in laser technology exam.

Contributors from Switzerland included Dr. Oliver Gaide (CHUV/Lausanne) and Prof. Robert Hunger (Bern). All speakers helped to ensure interesting, enriching and, above all, high-quality dialogue at international level.

The 32 participants greatly appreciated the highquality, boutique-style course, personal coaching and plenary sessions as part of different workshops with the following external experts: Dr Hans-Joachim Laubach (Geneva), Dr. Jean-Luc Levy (Lausanne), Prof. Leonardo Marini (Trieste/I), Prof. Reinhard Dummer and Dr. Laurence Imhof.

Allergy Training Course for Dermatology Residents in Training 2017 and 2018 For the second time, we organized a two-day interactive allergy course for residents in training together with CKCARE. The course included practical tasks such as skin prick tests and anaphylaxis management. Case-based discussions and workshops combined with short online teaching videos were used to familiarize the residents with allergic conditions such as inhalant, food and hymenoptera allergies as well as allergy-related skin diseases such as atopic dermatitis, contact dermatitis, urticaria and mastocytosis.

Events for the public and patients We regularly organize evening events on topics such as psoriasis, neurodermatitis, rosacea, acne and acne inversa, and provide information on skin cancer during National Swiss Cancer Day in May. Um bessere Therapien zu entwickeln, geht Allergologe Peter Schmid-Grendelmeier auch unkonventionelle Wege. So plante er etwa schon den Einsatz von Hakenwürmern, einem Parasiten. Stefan Müller Kaum tritt der Professor vor seine Bürotür, wird er von einer Patientin angesprochen. Sie hat sich in den Gängen der Dermatologischen Klinik des Universitätsspital Zürich verirrt und erwartet vom erstbesten Weisskittel Hilfe. «Vor zehn Jahren wurde hier der Portier aus Spargründen abgeschafft, seitdem finden sich die Patienten nur noch schlecht zurecht», erklärt Peter Schmid-Grendelmeier, Leiter der Allergiestation, mit einem Augenzwinkern. Mit seiner dunklen Hornbrille, dem Schalk in den Augen und seiner unprätentiösen, umgänglichen Art entspricht Peter Schmid-Grendelmeier, der bei unserem Treffen in einem kleinen, wenig repräsentativen Büro sitzt, keineswegs dem Bild eines abgehobenen Professors. Der Dermatologe hat sich mit der Erforschung von Neurodermitis und Allergien in der internationalen Fachwelt einen Namen gemacht. So wies er kürzlich in einer wegweisenden Studie nach, wie ein verbreiteter Pilz auf der Hautoberfläche die Entzündung bei Neurodermitis fördert und mit einer simplen Pilzbehandlung in Form von Tabletten die Beschwerden deutlich gelindert werden können. «Der Hefepilz kann mit der Haut und dem Immunsystem interagieren, weil bei Neurodermitis die Barrierefunktion der Haut beeinträchtigt ist», erklärt Schmid-Grendelmeier.

Aus wenig Mitteln das Optimale zu machen, lernte er in Afrika. Allergiestation mit seinen rund 30 Mitarbeitenden, die er seit dreizehn Jahren innehat. Seine Erfahrungen in Afrika empfindet er heute als gute Vorbereitung für das Führen eines Betriebes – «aus wenig Mitteln das Optimale herauszuholen», konstatiert er. Auch die klinische Lehre, die Schulung von Patienten oder Hausärzten, liegt ihm sehr am Herzen.

Mehr Dreck statt Hygiene

Wurm im rohen Fisch

Besonders interessiert ihn aber auch, wie Parasiten Einfluss auf das allergische Geschehen nehmen. «Diese Vorliebe trug mir den humoristischen Spitznamen Professor Sushi ein», bemerkt Schmid-Grendelmeier schmunzelnd. Den «Ehrentitel» erwarb er sich an einem Kongress in Deutschland. Er berichtete dort von einem Rega-Arzt, der wiederholt nach Südostasien gereist ist und an einer Sushi-Allergie leidet, ausgelöst durch den Heringwurm, der im rohen Fisch vorkommt. Nach einer Wurmtherapie und dem Meiden des Wurms lebt der Arzt beschwerdefrei. In diesem Fall löste der Parasit eine Allergie aus. Umgekehrt weiss man aber auch vor allem aus Drittweltländern, dass das Immunsystem von Menschen mit Parasitenbefall gedämpft ist und somit auch die allergischen Reaktionen. Vor rund drei Jahren wollte er dieser Wechselwirkung genauer auf den Grund gehen. Er konzipierte eine Studie mit Hakenwürmern. Von diesem Parasiten ist aus der empirischen Forschung bekannt, dass er das Immunsystem unterdrückt und zugleich relativ harmlos ist. Das Unappetitliche an dieser Studie ist jedoch, dass die Wurmeier über die Haut dem Körper zugeführt werden müssten. Die Ethikkommission verhinderte die Durchführung der Studie. «Weil der Wurm im Körper nicht kontrollierbar und die Sicherheit dadurch nicht gewährleistet ist», erklärt Peter Schmid-Grendelmeier. Der «Wirkstoff» des Wurms, der mit dem Immunsystem interagiert, müsste isoliert und steril hergestellt werden können. Bis dahin ist es allerdings noch ein weiter Weg, den Schmid-Grendelmeier trotzdem weiterverfolgt. «Eigentlich wollte ich Tropenarzt werden», erinnert er sich, den andere Kulturen und Länder schon immer faszinierten. 1997 hatte er dann jedoch die

delmeier, dessen Studie zur Teledermatologie als Pionierarbeit gilt. Heute pflegt er weiterhin regen Kontakt mit Fachkollegen in Tansania, wo Allergien genauso verbreitet sind. Einmal pro Jahr besucht er das Land. Mit seinem Afrika-Fieber vermochte er schon hiesige Berufskollegen anzustecken, die ihn mittlerweile auch regelmässig begleiten, auch auf seine beliebten Safaris. Was fasziniert Peter Schmid-Grendelmeier eigentlich an der Allergologie? «Im Lauf der Zeit merkte ich immer mehr, wie sehr das Fachgebiet auf mich als vielseitig Interessierten zugeschnitten ist», sagt er. Allergologie sei das ideale Bindeglied zu Fächern wie Umwelt, Tiere, Essen oder anderen medizinischen Bereichen. Als begnadeter Kommunikator passt ihm auch die Leitungsaufgabe in der

Seit seinem Einstieg in die Allergologie hat sich in diesem Fachgebiet viel geändert. Allergien nehmen in allen Industriestaaten von Jahr zu Jahr zu. Einstige Gewissheiten sind obsolet geworden, wie zum Beispiel: Meiden, was man nicht verträgt. Heute gilt eher das Gegenteil. So lautet die Empfehlung, Babynahrung soll im ersten Lebensjahr möglichst vielfältig sein. Dies soll das Risiko senken, an einer Neurodermitis oder an Asthma zu erkranken. Moderne Prävention lautet heute: mehr Dreck statt Hygiene. Eine wirksame Behandlung steht mit der Desensibilisierung zur Verfügung. Gezielt werden dabei Allergene in sehr geringen Dosen unter die Haut gespritzt, um das Immunsystem zu trainieren, sodass eine Gewöhnung eintritt und eine allergische Reaktion unterbleibt. Weil diese Therapie sehr aufwendig ist, arbeitet man daran, sie zu optimieren, zum Beispiel mit der Gabe der Allergene unter die Zunge mittels Tabletten, was bei Heuschnupfen schon recht erfolgreich ist. Zunehmend verbreitet sind aber auch Nahrungsmittelallergien oder -unverträglichkeiten. Museum of Wax Moulages

Each year, more than 300 medical students attend basic dermatology and venereology courses at the Museum of Wax Moulages in Zurich. They then revisit the collection later in order to refresh their knowledge and prepare for their final exams. For many universities of applied sciences, a visit to the museum has become part of the curriculum for students in healthcare. The museum, which contains three-dimensional life-sized and lifelike exhibits of skin diseases alongside the history of the patient in a contemporary context, also lends itself to interdisciplinary studies, e.g. the UHZ CAS MedLaw program. Interest in the public museum is growing all the time, and special events are regularly held that focus on new and different aspects of the museum’s collection of historical teaching aids and research documents. During the Long Night of Museums in 2017, well-known local artists used the more than 800 visitors as the basis for drawing and painting minor disfigurements as a means of spotlighting the artistic aspects of the wax moulages and encouraging discussion surrounding how we perceive “flaws”. The 2018 edition was even more successful, attracting more than 900 people to the museum’s exhibition on exotic diseases. The museum is a popular destination for meetings and events for many different societies and other groups. One such society is the Bruno Bloch Foundation, whose donators visited the museum in 2018 and subsequently decided to support the research performed at the Museum of Wax Moulages in the form of a grant. Michael Geiges (curator) and Sabina Carraro (restorer) are active with projects concerning academic teaching and medical historical research, and place special emphasis on the improvement of conservation and restoration. In collaboration with the Institute of Evolutionary Medicine at UZH, the team was able to produce a repaired duplicate of a broken historical moulage using modern 3D technology and the traditional manner of moulage production without requiring the original wax object to be touched.
