As a result, OSHA issued an Emergency Temporary Standard (ETS) for workers in Healthcare. The ETS aims to protect those working in environments where suspected or confirmed COVID-19 patients are treated and require employers to develop and implement effective COVID-19 plans to protect workers against the spread of the virus. Additionally; OSHA released updated guidance for all other industries not covered by ETS. The guidance aims to mitigate and prevent the spread of COVID-19 in the workplace. Important updates include focusing on unvaccinated and otherwise at-risk workers, encouraging vaccinations, and providing links to access the most up-to-date content as it becomes available. And more recently President Joe Biden ordered sweeping new federal vaccine requirements for as many as 100 million Americans — private-sector employees as well as health care workers and federal contractors — in an all-out effort to curb the surging COVID-19 delta variant.

It is my pleasure to bring you the 13th annual edition of the On-Site magazine. We hope this edition serves to inform and inspire occupational safety and health professionals across our state as we cover recent developments in workplace safety, provide in-depth analysis on OSHA standards, and explore the latest trends in occupational health and safety.

It’s exciting to witness our society evolve right before our eyes. The recent public health crisis has not only changed the way we live and work, but it has also elevated the ways we connect. As an alternative to in-person interactions, more of our lives have been thrust into digital spaces as we meet and collaborate over video conferencing platforms such as Zoom, Microsoft Teams, GoToMeeting, etc. In this increasingly digital world, relevant messaging and the tools used to communicate to your workforce will be critical to keeping your workforce engaged. Poor employee engagement can result in increased turnover, decreased productivity, and decreased employee morale. Alternatively, according to Gallup's State of the Workplace Report, a highly engaged business unit can be up to 21% more profitable than their counterparts. As you plan your communication strategy, it’s important to consider the demographic of your workforce and the digital spaces they occupy to ensure your message makes an impact.

The success of worksite safety and health management planning often relies on effective and dynamic leadership. Plan employee outreach around channels that are easily accessible to your workforce. Adopting digital communication channels such as email or text messaging will allow you to deliver updates to your workforce in real-time, and ultimately increase engagement among your workforce.

After you've identified communication channels that best fit the needs of your business, you'll want to plan messaging that helps you get closer to achieving business goals. In this edition of On-site, USF SafetyFlorida’s Keith Brown details management techniques you can use to achieve higher employee involvement, and leadership styles employers can adopt to successfully implement safety plans on page 14.

Mental Health and Tips for Coping during the Coronavirus Pandemic The COVID-19 pandemic has greatly impacted the way we live and work.

Stress affects everyone differently and although subtle, feelings of anxiety, grief, or worry can have significant health impacts if not properly managed.

The USF SafetyFlorida Consultation Program identified this an opportunity to provide attendees with the necessary tools and techniques to safely manage stress through these unprecedented times. On March 25, USF SafetyFlorida Consultation Program partnered with the University of Central Florida’s Dr. Deborah C. Beidel, Ph.D., ABPP , the USF Sunshine ERC and the USF OSHA Training Institute Education Center to provide an in-depth presentation covering risk factor Identification, healthy coping mechanisms, mental health evaluation techniques and readily available resources.

OSHA’s Top 10 Hazards for FY2020 Each year OSHA publishes a list of the top 10 most frequently cited standards following inspections of worksites by federal OSHA. This list is published by OSHA to alert employers to the these commonly cited standard violations. Employers are encouraged to use this list to guide preemptive action toward finding and fixing these hazards.

Health and Safety: Working in Confined Spaces in Construction Many occupations across various industries work in confined spaces and workers are often suffocated, engulfed, experience falls to a lower-levels, and exposed to many other dangers while occupying these spaces. This webinar offered on January 28th, aimed to increase awareness of the hazards involving confined spaces and provide OSHA compliance solutions in the general industry. Small business owners, employers, employees, managers, supervisors, and safety and health personnel in the construction industry were encouraged to join as our experts covered OSHA permitrequired confined space standard requirements, safety and health hazards associated with confined space entry. USF SafetyFlorida’s Gabe Garcia and Luis Silva led 116 attendees through the evaluation, prevention, and abatement of confined space-related hazards through demonstration and OSHA standard reviews. High Powered: Electrical Safety in the Workplace In 2019, there were 166 deaths recorded in the construction industry since 2011 according to the Bureau of Labor Statistics (BLS). This represents a 3.75% increase from the previous year. With electrical hazards on the rise, this training proved essential for attendees. This webinar focused on giving attendees the tools to recognize, respond and prevent electrical hazards such as arc flash, electric shock and electrocution. On March 4th, USF SafetyFlorida’s Keith Brown and Noel McCatty, Jr. led an informative discussion with 225 virtual attendees to provide a general understanding of the latest changes to the NFPA 70E such as approach boundaries, PPE requirements, host employer responsibilities, requirements for qualified employees and OSHA’s enforcement of these standards.

USF SafetyFlorida hosts 2nd Annual SHARP Safety and Health Conference The Safety and Health Achievement Recognition Program (SHARP) is a prestigious honor awarded to small business employers who operate an exemplary safety and health program beyond the standard requirements.

During the conference, you will hear from several recent recipients on the model they used and the many benefits that come from being an award recipient. The conference will be the convergence of inspired people, industry leaders, as well as safety and health professionals from all walks, along with multiple training opportunities on subjects that are currently affecting workers in the General Industry.

he OSHA Strategic Partnership Program (OSPP) provides a unique opportunity for employers, workers, professional or trade associations, labor organizations, and other interested stakeholders to come together in accomplishing an overall goal of improving workplace safety and health practices by establishing specific goals and performance measures and eliminating serious hazards.

Currently, there are 83 active partnerships protecting approximately 705K workers nationwide, with USF as signatories on two (2) of the partnerships and several more in the pipeline.

Obrascón Huarte Lain (OHL) North America initiated the strategic partnership with OSHA and USF to demonstrate their commitment to workplace safety through jobsites, to clients, and more importantly their sub-contractors. OHL specializes in heavy and civil engineering and is currently working on the South Corridor Rapid Transit project, which will extend a rapid bus transit system along 20 miles of exclusive transit right of way between Kendall and Florida City, FL. OHL North America is the general contractor for this project, has approximately 300 employees and 20 subcontractors. The project includes the construction of 14 bus transit stations, a park-and-ride garage, various safety improvements, and landscaping improvements. The project broke ground in July 2021 and is scheduled for completion in October 2023.

The partnership specifically aims to reduce the total number of injuries and illness cases by 10 percent for the entire project, effectively mitigate workplace hazards with safety and health management systems, increase the number of employees and supervisors who are provided effective

he USF OSHA Training Institute Education Center (OTIEC) is a non-profit organization authorized by the Occupational Safety and Health Administration (OSHA) to deliver occupational safety and health training to private, public and federal workers in all industries (construction, general, maritime, and disaster site).

The mission of USF OTIEC is to use policies and procedures set by the U.S. Occupational Safety and Health Administration (OSHA) to assure safe and healthful working conditions for the workers of Region IV (comprising of Alabama, Georgia, Florida, Kentucky, Mississippi, North Carolina, South Carolina, and Tennessee) via training and education.

The OTIEC offers approximately 300 classes annually and trains on average 7,0 0 0 workers throughout the United State s. T he Education Center is currently offering many classes via video conference. Exemplary Safety and Health Equates to Prestigious SHARP Certification What is SHARP?

T

he Safety and Health Achievement Recognition Program (SHARP) recognizes small employers who operate an exemplary safety and health program. Acceptance into SHARP by OSHA is an achievement of status that will single you out among your business peers as a model for your worksite safety and health and will reap rewards for your business. Upon receiving SHARP recognition, your worksite will be deferred from OSHA programmed inspections during the period that your SHARP certification is valid.

How can your company become SHARP? Companies interested in pursuing SHARP status must first request a comprehensive consultation visit that involves a complete hazard identification survey from USF SafetyFlorida.

1: Other qualifying requirements for the SHARP program include: • Involve employees in the consultation process • Correct all hazards identified by the consultant • Implement and maintain an effective safety and health program • Agree to call and notify USF SafetyFlorida prior to making any changes in the working conditions or introducing new hazards into the workplace • Have fewer than 250 employees on-site and fewer than 500 employees corporate-wide

SHARP Renewal If a company wishes to renew its SHARP certification, the application process must begin during the last six months of its exemption period. The company must also undergo a comprehensive visit by a USF SafetyFlorida consultant to ensure its SHMS has improved or maintained. Furthermore, the employer must meet all SHARP eligibility requirements, agree to conduct self-evaluations and provide a written report based on the elements of OSHA 1989 Safety and Health Program Management Guidelines and OSHA’s required injury and illness logs.

SHARP Deferral If a business meets most but not all of OSHA’s SHARP eligibility requirements, a USF SafetyFlorida consultant may recommend a business for an inspection deferral up to 18 months, provided the company has: • Completed a hazard identification survey with a USF SafetyFlorida consultant and corrected all identified hazards; • Implemented, demonstrated and maintained a SHMS that, at a minimum, addresses the major elements of OSHA’s 1989 Safety and Health Program Management Guidelines; • Implemented an action plan with established abatement dates that completes the necessary items that prevented the company from reaching SHARP status during the original survey.

A Focus on Workplace Safety and Health Helps Flotech, Inc. Achieve SHARP Recognition

F

lotech began as Gulf Atlantic Valve in 1961 with a small dedicated crew repairing valves in Florida for marine and industrial customers in Jacksonville, FL. The company now sells valves, heat exchangers, and condensers for power plants, paper mills, wastewater treatment plants and provides valve repair, tube repairs, safety valve certification, actuation, and control valve repair both nationally and internationally.

With management at Flotech searching for ways to continually improve their safety program and provide the safest work environment for their employees, they were eventually referred to the USF SafetyFlorida Consultation Program by their local safety council, Northeast Florida Safety Council.

With the help of a USF SafetyFlorida consultant, they were able to establish a safety committee that included members of management, supervisors, and line employees. The committee meets monthly to discuss safety concerns and develops plans for corrective activities for any hazards that may arise.

Since its inception, the company has seen a reduction in days away, restricted, and transferred (DART) rate, an improved workplace safety and health culture, and an increase in employee involvement. Workplace safety is now at the forefront of each project and discussed with all parties involved to ensure the safest work practices are being followed.

A

sk any successful business owner involved in high-risk industries their advice on keeping incident levels low, and their answer will most likely be employers and employees working together to keep a job site safe. This employer-employee safety commitment is the reason USF SafetyFlorida Consultation Program developed the Sunshine State Safety Recognition Award.

Created in 2006, the Sunshine State Safety Recognition Award commends employers and employees in all industries who proactively and routinely engage in job safety. Approximately 40 firms earn this honor annually, and it recognizes companies that have implemented good safety initiatives to better protect employees on the job. According to Keith Brown, a USF SafetyFlorida safety and health supervisor, “Commitment to safety is best demonstrated by teamwork between workers and managers in resolving workplace safety and health issues. Teamwork includes maintaining open and effective means of communication to not only identify problems, but also to achieve solutions.”

To qualify, a business should request and receive a complete on-site consultation survey and employ at least one person at the worksite to be evaluated. Additionally, the business should have no willful, repeat or willful-repeat citations within 24 months of the consultation and not serious citations related to injury or exposure within 12 months of the consultation.

During the consultation, a safety consultant will perform comprehensive on-site evaluation of all written programs. The consultant will also speak with workers and conduct an evaluation of the work area. The safety consultant will review: • • • • •

If hazards are identified during the visit, an abatement of those specific hazards must take place before recognition is granted. Once initial and final consultations are complete, a safety consultant may decide to formally recommend the firm for a Sunshine State Safety Recognition Award, which is approved by USF SafetyFlorida management. A formal letter of commendation is sent to the company, recognizing the employers’ and employees’ efforts in implementing and maintaining an effective injury and illness prevention program.

The Importance of Hearing Protection: Cutting Through the Noise

N

oise is defined as unwanted sound which is determined by the perception of the listener.

Therefore, since all noise is sound, but not all sound is considered noise, we can then conclude that “noise is in the ear of the beholder.” Sound is a disturbance that can travel through any elastic medium such as water, air, wood or steel, in order to be perceived by the ear as sound.

Occupational noise is sound perceived in any work environment. Depending on its intensity, it can cause damage to one’s hearing. Noise is one of the most common of all the occupational hazards in the United States where more than 9 million workers are exposed to daily average A-weighted noise levels of 85 decibels (85 dBA). The US (United States) Department of Labor has estimated that half a million workers are exposed to a daily average noise level of 100 dBA or greater, and more than 800,000 workers are exposed to levels between 95 and 100 dBA in the manufacturing industry alone.

Noise exposure at these levels can prove hazardous to one’s hearing and can produce other adverse effects.

Noise is measured in decibels (dB), named after Alexander Graham Bell and the notation is implied any time a “sound level” or “sound pressure level” is mentioned. Decibels are measured on a logarithmic scale which means that a small change in the number of decibels results in a huge change in the amount of noise and the potential damage to a person's hearing. The method to measure noise will depend upon the measurement goal, whether it be to assess risk of hearing impairment, need for and appropriate types of engineering controls, “noise loud” for compatibility with the type of job to be performed, and background level necessary for communication and safety.

Among the methods to measure noise in the workplace, the two basic procedures are the exposure of each worker, and the measuring of noise levels in various areas to create a noise

map for the determination of risk area.

To assess the risk of hearing loss from specific noise, the exposures should be measured in the vicinity of the worker’s ear. = When assessing the relative hazard of workers’ exposures, subtractions should not be made for the attenuation provided by hearing protection devices. The selection of noise exposure criteria depends on the goal to be attained, such as the prevention of hearing loss or the prevention of stress and fatigue.

There are a variety of sounds in the environment ranging from faint sounds to louder sounds such as music, yelling, and industrial noise.

The human hearing range is called the audible range, which will depend on the pitch of the sound measured in Hertz (Hz), and loudness of the sound measured in Decibles (dB).

Instruments for noise measuring include sound level meters, noise dosimeters and auxiliary equipment. Sound level meters (SLM) have built-in frequency weighting devices, which are filters that allow most frequencies to pass while discriminating against others. On the other hand, the noise dose meter or “dosimeter” is a form of integrating sound level meter that can be worn in the shirt pocket or attached to the worker’s clothing. Data from the noise dosimeter may be computerized and printed out.

It is very important to make sure that noise measuring instruments are always properly calibrated, before and after each day’s use.

The human ear is more sensitive to high frequency sounds than to low frequency sounds.

The A-weighted sound level measurement is thought to provide a rating of industrial noise that indicates the injurious effect such noise has on human hearing and has been adopted

by OSHA in its noise standard.

The OSHA Noise Standard 29 CFR 1910.95 sets a legal limit on noise exposure in the workplace to 90 dBA based on a worker’s time weighted average over an 8-hour day. The standard also

The noise dose is based on both the sound exposure level and how long it lasts (duration) so for each increase of 5 decibels in noise level the duration of the exposure should be cut in half, referred to as exchange rate.

The 29 CFR 1910.95 standard applies to general industry workers and require employers to protect their employees from the effects of occupational noise. This standard specifically states that; “When employees are subject to sound exceeding those listed in Table G-16, feasible administrative or engineering controls shall be utilized.” In addition, the standard mandates that; “The employer shall administer a continuing, effective hearing conservation program whenever employee noise exposure equals or, exceed an 8-hour time weighted average sound level (TWA (Time Weighted Average)) of 85 decibels measured on a scale (slow response) or, equivalently, a dose of fifty percent.”

The implementation of a Hearing Conservation Program requires employers to monitor noise exposure levels in a way that accurately identifies employees exposed to noise at or above 85 decibels (dB) average over 8 working hours, or an 8-hour TWA monitoring. The important elements of the program include baseline audiograms, annual audiograms, training and follow-up procedures.

The construction industry also has an applicable noise standard that protect the construction workers from the noise hazards. Under the construction noise standard 29 CFR 1926.52(a) it is require that; “Protection against the effects of noise exposure shall be provided when the sound levels exceed those shown in table D-2

of the section when measured on A-scale of a standard sound level meter at slow response.”.

Much of the industrial noise can be controlled through simple solutions if those administering the project have a good understanding of the principles of noise control and proper use of acoustic material. Industrial hygienist, safety professionals, facility engineers, and others can make progress in reducing equipment noise levels and worker noise exposures by combining their knowledge of acoustics with an understanding of the manufacturing equipment and/or process. Reducing excessive noise can be carried out by treating the source, the sound transmission path, the receiver, or any combination of these options. Once the source has been found, the hierarchy of controls for noise can be applied when choosing an engineering control, administrative control, or a form of personal protective equipment (PPE) to reduce the noise level, if the noise exposure is too high.

H e a r i n g p rote c ti o n d ev i c e s ( H PD s) a re considered the last option for controlling noise exposures. HPDs are generally used during the time it takes to implement engineering or administrative controls, or when such controls are not possible. Unless great care is taken in establishing a hearing conservation program, workers will often receive very little benefit from HPDs. The best hearing protector, when fitted correctly, is one that is accepted by the worker and worn properly. If the worker exposure is above 85 dBA (8-hour TWA), hearing protection must be made available, along with the other requirements in the hearing protection program.

The loss of hearing is the most well-known adverse effect of noise, and the most serious.

Other detrimental ef fects include tinnitus (ringing in the ears), interference with speech communication and with the perception of warning signals, disruption of job performance, annoyance, and extra-auditory effects. In the United States, hearing loss is the third most common chronic physical condition after high blood pressure and arthritis, nevertheless, it is important to know that there are regulations, guidelines, and recommendations to protect those workers that face occupational noise hazards daily.

Management Leadership

M

anagement Leadership” is a n e s se ntial e le me nt of ever y successful workplace s afet y a n d h e a l th p l a n. I t provides a foundation and creates a road map for developing safety management systems. Combined with other attributes, management leadership ensures the success of the organization through the prevention of work-related injuries and illnesses. In October of 2016, the Occupational Safety and Health Administration (OSHA), published its “Recommended Practices for Safety and Health Programs” in which it listed seven interrelated core elements with “Management Leadership” being at the very top of the list, followed by “Worker Participation”.

Management must manifest its commitment by creating and clearly communicating a worksite policy on safe and healthful work and working conditions, so that all personnel understand the priority of safety and health protection in relation to other organizational values. Likewise, management, with input from employees, must establish and clearly communicate goals and objectives related to workplace safety, so that all members of the organization understand the desired results and the means by which they are to be achieved.

Top management also must literally be seen actively supporting the program so that all will understand management is serious in its

commitment to employee safety. Management must encourage employee involvement through soliciting ideas and suggestions on how to improve workplace safety and health as well as achieving the establishment’s goals and objectives.

Management needs to ensure every member of the organization recognizes their individual responsibilities and ensure that adequate authority and resources are provided so those responsibilities can be performed.

What about worker participation? Just as the success of the safety and health management system depends on management’s leadership, it can also be said that same success hinges on employee involvement. Employees must recognize and exercise their own personal responsibilities as well as those assigned by management to ensure their safety and the safety of their coworkers. Employees must serve as an integral part of controlling hazards within the establishment and its operations, by recognizing, reporting, and even correcting hazards within their capabilities. Employees must also avail themselves of training provided by the employer, and become engaged in those training opportunities.

While management leadership and employee invol ve me nt make s the manage me nt of workplace safety and health a team effort, unless management sets the pace and direction, and workers participate, the safety plan will go nowhere.

Are You a Competent Person?

N

ot a very nice question, is it? It’s not an attack on your competency, rather OSHA’s definition, which could be considered open to interpretation by some.

Competent Person – one who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has the authorization to take prompt corrective measures to eliminate them.

What did I mean by that statement? On many occasions in the field, I have been asked to define what a competent person is. I of course, refer to the definition and in many cases, get the “deer in the headlights” stare back at me. Let’s break this down. OSHA does not require official training in the field to qualify someone as competent, but without experience, knowledge and/ or training, how can anyone “identify existing and

predictable hazards”? If you don’t know what’s wrong, how can you correct it? Additionally, if management has not given you the authority to “take prompt corrective measures to eliminate potential or identified hazards”, then how can you be considered competent by this definition?

Ultimately, the responsibility of selecting a competent person falls on the business owners and/or executives within the organization. They are delegating their authority to the assigned employee, to ensure the process is being completed properly, safely and by the standard.

How can a competent person be evaluated?

Very easily, by their actions and their answers to questions regarding the subject in which they are considered competent. For example, if I were evaluating a scaffolding competent person, I would first look at the scaffolding and determine if there are any deficiencies. If many were found, then it is doubtful that the individual could be considered competent.

But before making a final determination, I would ask some questions: • How often do you inspect?

• Do you see any issues?

• It rained earlier today, have you inspected it since?

• Are you able to “red tag” or remove workers from the scaffold if you identify an issue?

• How long have you been the competent person?

• How long have you worked with this type of scaffolding?

Being a competent person for various situations goes beyond just these basics. The entire standard should be reviewed. The scaffold standard, for example, refers to competent person throughout the standard, such as training of employees by a competent person, determining if it is safe to use components from different manufacturers or of dissimilar metals, supervising the erection, alteration, and dismantling process, monitoring the weather conditions to determine the safe use of the scaffold, and determining the feasibility and safety of fall protection for employees while erecting or dismantling scaffolds.

Having a competent person is not only recommended for scaffolding. In fact, there are various standards that require a competent person be present for different processes, such as forklift training, ladder inspections, silica, confined space, cranes, rigging inspections, steel erection, fall protection, excavations, and demolition. Often, the phrase “criteria for a competent person depend on the situation” is used in preambles and referred to in letters of interpretation. The basic definition is the same, but the situation can vary.

Aerial Lifts Personal Fall Protection Requirement

hen job tasks take place in hard-to-reach areas or unfinished environments, aerial lifts are often the answer.

Aerial lifts have replaced ladders and scaffolding on many job sites due to their mobility and flexibility. They may be made of metal or fiberglass reinforced plastic and can be powered or manually operated, usually equipped with vertical rotating capabilities. OSHA’s Vehicle-Mounted Elevating and Rotating Work Platforms Standard (29 CFR 1910.67), utilizes the American National Standard Institute Standard (ANSI A92.2-1969) in classifying aerial devices. It goes on to further define a vehicle as “any carrier that is not manually propelled”. These include but are not limited to: • • • • •

Extendable boom platforms Aerial ladders, Articulating (jointed) boom platforms, Vertical towers, and Any combination of the above.

“A review of both general industry and the construction standards support this interpretation by stating: ‘Employees shall always stand firmly on the floor of the basket and shall not sit or climb on the edge of the basket or use planks, ladders or other devices for a work position.’”

To maintain compliance with this standard, employers are encouraged to require employees operating aerial lifts: • Ensure that access gates or openings are closed • Stand firmly on the floor of the bucket or lift platform.

• Not climb on or lean over guardrails or handrails.

• Not use planks, ladders, or other devices as a working position.

• Use a body harness or a restraining belt with a lanyard attached to the boom or bucket.

• Do not belt-off to adjacent structures or poles while in the bucket.

To better understand the intent of the standard, we must look at 29 CFR 1910.67 for General Industry, more specifically 1910.67(c)(2)(v): “A personal fall, arrest or travel restraint system that meets the requirements in subpart I of this part shall be worn and attached to the boom or basket when working from an aerial lift.”

Workers must wear a personal fall arrest or travel restraint system that meets the requirements of 29 CFR 1910 Subpart I and attach it to the boom or basket when working from an aerial lift such as a cherry picker. In a Letter of Interpretation dated March 22, 1976, OSHA clarified the use of the word “from” does not imply that employees are exempt from wearing fall protection when working on an aerial lift. ach day, our consultants are out in the field visiting small employers in high-hazard industries. From a hazard standpoint, we see it all. On this page are photos of hazards we have identified this year. More importantly, our consultants assisted these companies with correcting the hazards so their employees can work safely.

This switchboard exposed employees to a temporary flexible cord being wired directly with live conductors without a cover or grounding connection. Switchboards that have any exposed live parts should be located in permanently dry locations and accessible only to qualified persons. Panelboards should be mounted in cabinets, cutout boxes, or enclosures designed for the purpose and dead front. Employees in the work area should be trained on the proper use of temporary flexible cords and the hazards related to live electrical equipment.

Performing preventative maintenance on portable air compressors while running can be dangerous as it exposes operators to risk of severe amputation injury. Establishing lockout/tagout procedures compliant to OSHA standards are necessary to prevent serious injury.

This homemade spray booth constructed of plywood, a combustible material, creates a very dangerous workspace as an accelerated fire can result in burns, smoke-related injuries, and traumatic injuries and/or an explosion. Spray booths should be substantially constructed of steel, concrete, masonry or aluminum, rigidly supported and designed to sweep air currents toward exhaust outlets.

This alloy steel chain sling was discovered on a job site with an unapproved fastener. Makeshift links or fasteners formed from bolts or rods, or other such attachments, should not be used on alloy steel chain slings. Loads can fall due to sling failure and can result in sprains, strains, fractures, and/or crush personnel. Employers should establish a system of regular in-house inspections of all work practices, equipment and processes as well as train employees on alloy steel chain sling inspection requirements, proper use and maintenance to prevent these hazards.

A tripod and winch that was not designed for movement of humans was used during permit required confined space (permit space) entry / rescue.

Malfunction or collapse of a tripod and winch during entry/exit of the worker accessing the permit space can result in serious injury or death.

Regularly check all permit space equipment to ensure it is the correct equipment for the task and document this review/inspection procedure.

The scaffolds pictured were erected on 2×2 boards which can be unstable and should not be used to support scaffolds or platform units.

Footings should be level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement. Supported scaffold poles, legs, posts, frames, and uprights should be on base plates, mudsills or other adequate firm foundation.

This defective fire extinguisher was found at a worksite. Fire extinguishers should be regularly inspected when in service and thereafter at approximately 30-day intervals. A small fire can quickly get out of control and failure to manage it can lead to equipment damage, smoke inhalation, burns and even death.
