Skilled Nursing

What makes a good referral to Skilled Nursing?

A good referral to Skilled Nursing would be someone in need of short-term, longterm or hospice nursing care. Some of our nursing communities accept Medicaid though the preferred referral would be either long-term private pay, short-term skilled Medicare or commercial insurance. Short-term should be those that are planning to discharge to either home or personal care.

Encourage those you know to apply for jobs – especially nursing jobs within our network.

Talk about our full continuum of care early and often.

Advise people going in for surgery that we can assist in their recovery.

Anyone! Residents, family members, team members, individuals from the community-at-large, foundations, business partners or vendors: • Who had a good experience in our care and expressed an interest in wanting to give back in appreciation.

• Who have connections to a local foundation or company that supports nonprofit organizations.

• Has mentioned they have a Donor Advised Fund.

• Expressed an interest in helping our team members or a particular program or location.

Spread the word! Let everyone know that Presbyterian SeniorCare Network is a non-profit, charitable organization and that we welcome charitable gifts. The Foundation office handles all charitable giving for the Network.

When asked, confirm that the Foundation raises funds for ALL of our Network locations – and that donors may designate their gift to support the Network community closest to their heart. Dementia360 is a program that is designed to support family caregivers of people living with dementia at home who have the goal of being able to keep them at home for as long as possible.

1.

What makes a good referral for Dementia360?

We accept families at any point in the dementia journey. Families who reach out to the network wanting more information because someone they love is living with dementia are GREAT referrals. They’re not ready to make a move, and our program can support them while that is their goal.

Most of our referrals are people who are later on in the process, when families are struggling; there is a lot we can do to help bring down the stress and make things easier at home. BUT a really great referral is a family that is early in the disease process. These families are scared and sad – they know they want to help their family member but aren’t sure what to do.

Dementia360 supports family caregivers through education about dementia and by helping them build a foundation of good habits and behaviors that will be essential for them and their loved ones to cope with this ever changing disease.

How can each of us can help Dementia360 grow?

Look all around you. Look at your church and the people that you run into on a daily basis.

Get contact information of interested individuals and say, “This is a great program and I’m going to have somebody reach out to you.”

Family caregivers are stressed and sleep deprived, so telling them to contact Dementia360 can feel like “just one more thing to do.”

Instead, get their name and number and ask a Dementia360 Care Coordinator to contact them.

When you’re sharing information about Network services, be sure to mention that we also support people at home. Dementia360 is one of our newest offerings and it’s different because we support the person living with dementia but also the entire family. And that we can help them all live better, even within the context of dementia.

What makes a good referral for Woodside Place?

• We’re looking for people living with dementia who are ready to transition into a specialized, memory care community who meet the move-in criteria. A perfect referral source would be a senior with the potential to transition within 6 months to one year. Ideally, for a smooth transition, the individual would start in the Adult Day Program prior to move-in. The Adult Day Program offers the families, the resident, and the team time to get to know each other prior to the move-in day, making the admission a much smoother transition for all involved.

• Medically, we are looking for a resident who has a: - Dementia diagnosis, mid-moderate stage.

- Can walk safely—benefit from our secured wide open space.

- Require minimal nursing needs.

• Meet our financial criteria: Woodside communities are private pay only. We can accept long-term care benefits, VA benefits, but not Medicaid. Presbyterian SeniorCare Network does offer the benevolent program through the SeniorCare Fund for residents who outlive their assets. Financial approval is needed prior to all move-ins.

How can each of us help Woodside Place grow?

Encourage our families to be proactive and contact us before the crisis.

All of our Woodside Place communities are typically full, operating on an interest list. However, send any and all leads over. Due to the continuum of care, we will have rooms opening and can give families tentative time lines as well as other resources to help manage their at-home needs until a room is available.

We all have resources we can offer the family that do not qualify and would be happy to share those resources to make sure the individual living with dementia and their families receive the services they need.

Independent seniors who: • Don’t want to be a burden to their families

• Own a long-term care policy

• Want to live in their homes and, if they had their choice, they would die in their homes

• Have difficulty with care giving

• Don’t really want to move to an Assisted Living or a nursing home

• Can live on one floor • Want to stay with an ailing spouse • Need management care now

• Age 85 and up are a possibility

• Have the means to self-insure

• Ages 75 to 84 are a good possibility

• Live in multiple places

• Ages between 65 and 74 are a great prospect

Longwood is a CARF accredited continuing care retirement community (CCRC), also known now as a life plan community. To live at Longwood, residents must be able to live independently as determined by a medical approval process.

1.

What makes a good referral for Longwood at Oakmont Independent Living?

• The best referral would be a senior in their 70’s with a high net worth. But our minimum age is 62 and we have no top age range – they just have to be able to live independently. Also, many folks that qualify financially are in the medium net worth range.

• A senior who understands all the benefits of a congregate community where they will have a terrific, active lifestyle with subsequent quality care as they move on. All with a written promise that we’re never going to ask them to leave, even if they run out of money, so they must financially qualify upfront.

If they want a life care plan, that qualification is more stringent than if they want a fee for service plan; but we have both.

• A senior who is ready to plan for the future and is not totally attached to their home.

• A senior who is single, possibly widowed, who is lonely and would really benefit from the socialization and programs that we offer.

What makes a good referral for Independent Living at Shenango on the Green?

A good referral has enough assets to pay a one-time entrance fee and enough monthly income to pay the monthly maintenance required to become a resident.

Good referrals are active seniors who are capable of living independently. They are pleased to be a part of a retirement community that has a continuum of care all under one roof, although they do not currently require direct care. They are able to navigate through their day independently, schedule their own appointments, manage their own health and finances. They are open to being a contributing member of an active community.

What can we do to help Independent Living at Shenango on the Green grow?

Direct prospects to Shenago on the Green’s informative website.

More seniors and children of seniors are using the internet to choose the right retirement community. Web leads are answered in a short amount of time and are the beginning to personal relationship building required to move a potential to a resident.

We continue to plan engaging events that are a draw to prospects to visit our community. There is no replacement for a personal tour and the experience of seeing all the positive amenities of our community.

2. Continued Word of mouth referrals. This can be as simple as sharing a Facebook post or sharing about our community in conversation with friends or neighbors that may live in our area. Some people may tour one of our other campuses but find they are too big or not a proper fit for one reason or another. Why not mention Shenango on the Green as another option?

Professional referrals are also a great option. These professionals that we meet may have a client, friend, or family member that lives in the New Wilmington area.

They may also know about Westminster College which is right across the street and a huge draw for potential residents due to the cultural and sport experiences, as well as a beautiful campus for walking.

What makes a good referral to Personal Care?

• Someone who is able to qualify both financially and functionally.

• Personal care is a private pay model. Financial assessments are based on information provided on the application and are decided on a case-by-case basis. We offer Benevolent Care for residents who outlive their assets.

• Someone who has the ability to ambulate with or without an assistive device or has the ability to self-propel in a wheelchair.

• Someone who has the ability to transfer independently with supervision or with assistance.

• Someone who has the ability to perform ADL’s independently with supervision or with assistance.

• Someone who may need assistance with meal preparation, housekeeping and medication management.

What makes a good referral for Home Health?

A good referral for home health is an individual in need of intermittent skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Social Work, or Home Health Aide Services. Services must be ordered by a physician who will oversee the plan of care. Additionally, new patients must have a face-to-face visit with a physician or qualifying practitioner about the home health service they need either 90 days before, or within 30 days after beginning a Home health program.

Home health referrals are facilitated when the agency receives the patient’s demographic information including insurance information, physician orders and a copy of the face-to-face encounter.

We are able to accept Medicare, Highmark Medicare products such as Security Blue and Freedom Blue, UPMC insurances, as well as Aetna and Advantra.

The priority for Presbyterian Senior Care Home Health is to provide continuity of care for individuals who are touched by our network of services. Home health services relies on our internal network partners for referrals.

Individuals participating in discharge planning from skilled nursing can speak to the advantage of continuity of care when referring to Presbyterian SeniorCare Home Health.

Caregivers for our personal care and assisted living communities can refer residents who are newly transitioning to their communities or have a change in status that would benefit from home health services.
