Message from Our Director Dear Friends, Reflecting on the tremendous growth that has taken place at the Center for Community Health & Prevention (CCHP) is a humbling experience. Looking back, we are inspired by all the partnerships that we developed with community organizations, both within the CCHP and throughout the Medical Center. While we continue to nurture these existing partnerships, making them “ever better,” we formed new relationships and new initiatives in 2018 that we now celebrate.

Our commitment to community health and engagement is exemplified by our research, education and service programs. Working closely with community organizations and other healthcare systems, we discover the best prevention and treatment protocols, educate community-engaged health professionals and provide critically needed services.

Medicine is in a period of rapid change, and prevention and community engagement have never been more important. Increasingly, we all acknowledge the importance of health behaviors and the social determinants of health in preventing, treating and curing disease. Moving forward, the CCHP is dedicated to supporting the health of Rochester and the surrounding region. The partnerships that we form are critical to developing relevant models and meeting the needs of those we serve where they are, in a way that’s most meaningful to them.

More than 84 million American adults - 1 in 3 - have prediabetes.

Of those with prediabetes, 90% don’t know they have it.* We train facilitators to bring the CDC Diabetes Prevention Program (DPP) into their communities, allowing for greater access to physical activity and nutrition education.

Participants say the DPP is making a difference in their lives.

“People are lobbying for having more fruits and vegetables in their stores and they’re being more active in what’s going on in their neighborhoods and in their communities. It’s definitely making a big impact.”

Linking Interventions for Total Population Health (LIFT Population Health), a $750,000 three-year grant from the New York State Department of Health (NYSDOH), expands community health initiatives for diabetes prevention in Monroe County.

Healthy Living Program Our Partner: Refugees Helping Refugees

Through engaging activity and health education, 55 refugee participants achieved weight loss and added more fruits and vegetables to their diets.

New Relationships

Cancer Services Program of the Finger Lakes Region (CSP-FLR) Our Partners: More than 215 providers in Monroe, Livingston, Wayne, Ontario, Seneca and Yates Counties.

The NYSDOH awarded the Center a $1.8 million grant to expand its Cancer Services Program of Monroe County to the surrounding Livingston, Wayne, Ontario, Seneca and Yates Counties, naming it CSP-FLR. The program expansion enables CSP-FLR to provide free screenings for breast, cervical and colorectal cancers to many more eligible men and women who are uninsured or underinsured.

Guests impacted by the CSP-FLR attend the annual Breast Health Education Dinner in October.

More than 480 men and women received free screenings between April and November 2018

Monroe County Executive Cheryl Dinolfo presented a proclamation at a Colorectal Cancer Awareness Month event hosted by CSP-FLR and the Central Library of Rochester & Monroe County in March.

Promote Health. Prevent Cancer. (PHPC) Our Partners: Wilmot Cancer Institute and recreation centers, libraries and other community organizations in Dansville, Mt. Morris, Webster, Nunda, Wayland and Wellsville.

This 15-county wide initiative empowers participants through evidence-informed cancer prevention and risk-reduction education consisting of an eight-week group program and individual one-hour health talks. Curriculum is based on recommendations from the American Institute for Cancer Research and is supported by Wilmot Cancer Institute.

RocHealthData Website Our Partners: URMC Clinical & Translational Science Institute • MCDPH • Rochester RHIO • Common Ground Health

www.rochealthdata.org The new website allows health researchers and community partners to map local health outcomes and social determinants of health.

With 97 registered users, the website contains extensive information on the region’s health challenges and resources shown in both map and report formats.

Healthy Living Partners Using proven strategies and the science of motivation, our team of physicians, nurse practitioners, registered dietitians, exercise specialists, and counselors offers lifestyle programs proven to prevent chronic disease. These programs are offered to the community through individual and group sessions and the UR Employee Wellness Program.

15 new UR departments engaged in our employee wellness programs.

Collaborations included: • Allergy, Immunology and Rheumatology Division in the Department of Medicine: Physical activity program for patients with Lupus

• Department of Obstetrics & Gynecology: Weight loss program for patients undergoing fertility treatment

Smokers Have a Higher Risk from Surgery.

Research shows that intensive smoking interventions prior to surgery result in 58% fewer complications and 69% fewer wound complications. We initiated a Tobacco Dependence Treatment Program for preoperative patients receiving care through the Department of Surgery.

The program increases a patient’s motivation for short- and long-term tobacco abstinence to achieve the goal of a four-week period of tobacco abstinence prior to the patient’s planned surgery. Appointments are available in-person or through telehealth.

The integrated referral process creates new opportunities with providers and increases the number of patients we serve.

Strengthened Partnerships Throughout 2018, the Center expanded its program networks to broaden its impact and to improve the well-being of more residents across the region.

Communicable Diseases Surveillance and Prevention (CDSP) The CDSP program includes the NYS Emerging Infections Program (EIP) and Healthcare Associated Infection Prevention Collaboratives.

Giant Year for Flu: The EIP team collected data on 12,213 laboratory-confirmed positive flu cases from Monroe County and the 6 surrounding counties. Within Monroe County, 7,357 cases were identified. Data on hospitalized flu cases are also reported to the CDC to be included in weekly national FluView updates.

Strengthened Partnerships

Reducing Healthcare Associated Infections: The CDSP team assisted area nursing homes in improving their use of antibiotics in order to reduce infections from Clostridioides difficile (C.diff ) and resistant bacteria. Through this work, C. diff infections decreased in participating nursing homes by 42% since 2014.

Dr. Ghinwa Dumyati, director of CDSP, is a leader in reducing healthcare associated infections.

“Dr. Dumyati’s leadership as an infectious disease specialist has assisted facilities in reviewing their antibiotic use data and has also helped educate nursing staff, medical providers, pharmacists and families. I feel the Rochester region is extremely fortunate to have been able to benefit from Dr. Dumyati’s expertise and I look forward to continuing with our collaboration.”

Dr. Dumyati serves on the C. difficile Infection Prevention Task Force at Strong Memorial Hospital, which garnered the University’s. Meliora (‘ever better’) award in April.

Addressing the Opioid Epidemic: A team of physicians and epidemiologists from the CDC visited the Center to conduct an “Epi-Aid,” an urgent public health investigation. With assistance from the Center’s EIP team, they evaluated and defined opportunities to prevent bacterial and fungal infections among people who inject drugs.

Workforce Navigation Teen Heath & Success Partnership (THSP) supports academic success, part-time employment and career goal achievement of at-risk students.

Teen Health & Success Partnership Director Annabelle Kleist, PhD, received a competitive two-year UR Inclusive Climate Leadership Fellowship for her work supporting University supervisors in creating an inclusive climate for low-wage staff.

• 100% of THSP students graduated from high school • 68% of students enrolled in college • 82 at-risk RCSD high school students were employed at UR through the program • Graduates were employed in 18 different departments at the River Campus, URMC, Highland Hospital, Memorial Art Gallery, Highlands at Pittsford and Highland Family Medicine • 13 students and program graduates received promotions

Rochester Health Profession Opportunity Grants (ROC-HPOG) provide opportunities for entry-level or low-income individuals to pursue healthcare training and career advancement within the healthcare field.

Our Partners: Action for a Better Community, Inc. • Ibero-American Action League, Inc. • Starbridge • RochesterWorks! • MCC • RIT • Rochester Refugee Settlement Services • The College at Brockport, Rochester Educational Opportunity Center Denyda works as a pediatric dental assistant at UR Medicine’s Eastman Institute for Oral Health. Through trainings and support, the ROC-HPOG program has helped Denyda pursue her dream of becoming a dental hygienist.

Collaborations with the Clinical & Translational Science Institute (CTSI) CCHP partners with the CTSI to link faculty, students and staff with community partners to address community health concerns.

5 Community Engagement Studios, with 5 researchers & 50 community experts.

62 individuals applied and 41 completed the Community-Based Participatory Research (CBPR) Training Program, garnering 3 pilot grants.

19 biomedical research institutions from across NYS that assist faculty and trainees from the UNYTE Translational Research Network (UNYTE) institutions in increasing translational research in the Upstate New York region. UNYTE has consulted on 10 cross-institutional projects.

New UNYTE members include: Canandaigua Veterans Administration Medical Center (VAMC) and its Rochester VA Outpatient Clinic

The Greater Rochester Practice-Based Research Network (GR-PBRN) welcomed Trillium Health as the newest member of its network of pediatric, family medicine and internal medicine practices interested in collaborative research.

More than 45 practices engaged in 20 studies.

Strengthened Partnerships

We coordinate the development and implementation of the Monroe County Community Health Needs Assessment and Community Health Improvement Plan on behalf of the four hospitals and public health department in the county.

The collaborative team was selected as 1 of 10 best practices nationwide for community health improvement planning.

Theresa Green, PhD, MBA and Nancy Bennett, MD, MS represent the Center on the URMC leadership team to the American Association of Medical Colleges “Building A Systems Approach to Community Health and Health Equity” initiative, 1 of 8 academic medical centers selected.

Our Staff: Represented on 22 community committees and coalitions and 26 national committees and coalitions. Authored 17 scientific papers. Gave 10 professional presentations, including 5 at national events/conferences.
