Wanda is a Baltimore resident and one of UMMC Midtown Campus’ patients who has directly benefited from the “Connected Care” model. Together with our outstanding care providers, Wanda is BUILDING UP HER HEALTH. Here’s how:

Primary Care

Wanda receives her primary care from Dr. Jeffrey Gerbino at Midtown Health Center. During a visit, she was experiencing pain in her leg and had an ulcer that would not heal. Dr. Gerbino referred Wanda to the Wound Healing Center to meet with the appropriate specialists.

Connected Specialty Care

While at the Wound Healing Center, a member of the Vascular Center team met with her to discuss her leg pain. An infection was identified in her leg, and she was admitted to the hospital due to lack of blood flow.

Vascular Center Team

In March 2021, Dr. Gitlitz performed Wanda’s first bypass. After surgery, Dr. Rafael, director of the Wound Healing Center, followed up with her to reassess her leg’s blood flow. It was determined through collaborative efforts that she would need another bypass, which was performed by vascular surgeon, Dr. Chaparala, in October 2021.

Center for Diabetes and Endocrinology

Wanda also has diabetes and is seen by Dr. Melhem in the Center for Diabetes and Endocrinology. Throughout Wanda’s vascular surgeries, Dr. Melhem was able to connect with her expanding care team to provide insights, receive updates, and coordinate her overall care.
