AMERICAN INCOME LIFE Insurance Company $4000 Accidental Death and Dismemberment Coverage • Coverage for members of the union and dues paying retirees • Any benefit paid to minors shall be limited to $1000 or in case no beneficiary is listed, the benefit paid to the estate will be limited to $1000. In order to ensure the maximum payout for the benefit be sure to name a beneficiary and that they are 18 years of age or older.

How to submit your photo Thank you for supporting the UFW Foundation and becoming a member. To receive your UFW Foundation membership credential, email membership@ufwfoundation.org or text (951) 6380021 a photo of yourself. You can take the photo with your mobile device. A couple of things to keep in mind: Make sure you take the photo passport style Your face is clearly visible. Have a white background if possible. Stand in a spot that is well lit. Once we receive your photo we can print out your UFW Foundation membership credential and mail it to you.
