1 he year 1879 was one marked in Little Rock by gradual resolution of the turmoil which accompanied reconstruction fol­ lowing the Civil War. It was also the year that saw the birth of formal medical education in the state of Arkansas.

Under a charter of the Arkansas Industrial University in Fayetteville, an enterprising group of physicians, led by P. O. Hooper, moved to establish a medical branch in Little Rock. The Sperindio Hotel on W. 2nd St. was purchased in August, and on October 7, 1879 Hooper and the other seven co-founders opened the doors of the state’s first medical school. Off to a meager start with only twenty-three students, the school produced it’s first graduate. Tom M. Pinson, in March of 1880.

Lacking any requirements for admission until 1885, the school proved to be a financial success for it’s founders but public senti­ ment in the largely rural, poorly educated state may have been ' best expressed in an 1882 House of Representatives’ resolution, “ . . . that medicine and surgery are humbugs, and that all medical colleges should be declared nuisances and injurious to the health of the Commonwealth.” Despite such sentiments, 109 graduates were produced in the first ten years and by 1890 the great demand for medical education forced construction of a new $15,000, steam heated facility at 2nd and Sherman.

Early medical training was largely didactic with formal clinical training at a minimum until the opening of the Logan H. Roots Memorial Hospital in 1898, and St. Vincent Infirmary in 1900. In addition, Isaac Folsom of Lonoke Co. endowed the school with a gift of $20,000 upon his death in September, 1892, to be used to establish a clinic bearing his name.

With this apparently secure foundation, the newly renamed University of Arkansas Medical Department entered the twen­ tieth century with seeming complacency. That was soon to change however, with the formation of the College of Physicians and Surgeons in July of 1906. Competition between the two schools prompted the Medical Department to upgrade the laboratory facil­ ities and faculty, extend the academic year to eight months, and finally, in 1910, require a high school diploma for admission.

Given the fact that in 1899 there were only 6 four year public high schools in the entire state, it is understandable that the enrollment fell sharply. With a loss of tuition revenues, and the implementa­ tion of costly improvements, it became painfully apparent that additional funding must be found.

In 1910, the Flexner Report was issued by the Carnegie Founda­ tion which showed that over half of all U.S. medical schools, including both Little Rock schools, were below acceptable stand­ ards. The editorial outcry which followed, increased the impetus for merger of the two schools, and on July 5, 1911, after much legislative debate, the University of Arkansas Board of Trustees officially assumed direction of the Medical Department. Despite this adoption by the state, the conditions in the school remained essentially unchanged. As a result, in March, 1912, the senior students made headlines in the Arkansas Gazette by presenting a list of grievances accusing the school of poor facilities and in­ adequate education. The furor that followed led to the resignation of Dean Thomas H. Lenow, and his replacement by Dr. Morgan Smith.

In an effort to improve the situation. Dean Smith obtained permission to move pre-clinical instruction to the old State Capitol Building on W. Markham thus making the 2nd and Sherman site available for clinical training. Full time pre-clinical faculty was hired, the curriculum was upgraded, and admission requirements were increased in an effort to raise the quality of the institution.

With these increased standards came a drastic decrease in enroll­ ment at a time when increased operating costs were causing dire financial problems for the school.

I he lack of available clinical training had become particu­ larly acute thus prompting Dean Smith to appeal to the legislature for funds to build a state operated charity teaching hospital. On February 8, 1917, such a bill was approved, but with the U.S.’s involvement in World War I came a virtual standstill in the growth of the school. With the school in shambles after the war, and no hope for additional clinical facilities. Dean Smith was forced to suspend clinical training in 1919. This drastic measure diverted needed funds for pre-clinical education but forced third and fourth year medical students to complete their education elsewhere.

Clinical education was re-instituted in 1922 with the availability of St. Vincent Infirmary, St. Luke’s and Baptist Hospital as training centers. With it came increased enrollment and school spirit as reflected by organization of a school band, and publica­ tion of the Cadiiceiis in 1924. Dean Smith continued to struggle for funding but legislative sentiment was not with medical education and in May 1927 he resigned.

With the depression of 1929 and the Medical Department’s unpopular stand in favor of teaching the theory of evolution.

Smith’s successor. Dr. Frank Vinsonhaler, faced the impossible task of obtaining money from a hostile legislature. Instead, he and the U. of A. in Fayetteville turned to F.D.R.’s Public Works Administration and in 1934 the Medical Department received $500,000 for construction of a new Medical School education building. The Bentley Estate adjacent to the City Hospital at 12th and McAlmont was purchased, and by November of 1934 con­ struction had begun.

The completion of the new building, great increases in student enrollment, and the addition of such outstanding professors as W. C. Langston, and D. A. Rhinehart to the faculty, prompted the administration to apply to the A.M. A. Council on Medical Educa­ tion foi full accreditation. Despite the improvements, the applica­ tion was rejected in 1936 and because of a persistent shortage of available charity hospital beds, the A.M.A. dropped the Medical Department from its list of approved medical schools in 1939.

Although the decision was later repealed, the school was placed on probation until 1944.

The public and legislative outrage that followed, prompted a faculty and administrative reorganization, and eventually led to a contract with City Hospital to increase available hospital beds.

The quality of the faculty was increased with a subsequent boom in pre-clinical research, and the admission of the school’s first graduate students in 1943. In addition to increased admission and academic standards, students faced an accelerated curriculum, and R.O.T.C. training when the education of physicians became a national priority after the Japanese attack on Pearl Harbor.

With the end of the war came increased student enrollment, and unprecedented expansion of clinical faculty. It became apparent also that the old City Hospital would be inadequate to meet the clinical training needs of the future. Sponsored by Gov. Ben T. Laney, the “ Laney Plan” to move the medical school to a new site on the grounds of the State Hospital for Nervous Diseases was formally adopted in September of 1947. Given the impressive improvements in facilities, academics, and faculty, it is not sur­ prising that full accreditation was granted by the A.M.A. in 1958.

Growth followed growth on the new campus To house the students near the isolated west Little Rock loca­ tion, dormitory and student union facilities were completed in 1959. The building, completed with federal funds, was named in honor of Dr. Jeff Banks, Professor of Anatomy, whose untimely death had earlier shocked the campus. With a gift of $600,000 from Col. and Mrs. T. H. Barton and various other contributions, the school applied for and received federal matching funds for construction of a medical research facility. By November of 1961 the T. H. Barton Institute for Medical Research was ready to add to an already dynamic research program.

Because of the school’s stand during the integration crisis at Little Rock in 1957 and the Act 10 controversy which followed, the Medical Center faced considerable hostility in the legislature during the late 50’s. In order to help solve some of the school’s financial problems. Storm Whaley was appointed Vice President for Health Sciences. A masterful administrator, Mr. Whaley worked tirelessly to solve the financial woes of the campus. Un­ fortunately, administrative frictions developed which resulted in the departure of the Dean of the College of Medicine, F. Douglas Lawrason.

Lawrason’s successor as Dean, Dr. Winston K. Shorey, was a man perfectly suited to the task facing him. Dr. Shorey, an open minded, even tempered, decisive gentleman, moved quickly to secure the badly needed support of the legislature, the state medi­ cal profession, and the general public. This combined support helped to make Dr. Shorey’s tenure as Dean a decade and a half of impressive growth and achievement.

The I960’s saw a doubling of full-time and voluntary faculty despire the departure of Dr. Richard Ebert and the deaths of Drs. Masaki Hara, Willis Brown and Ted Panos. Research activity accelerated dramatically, enrollment rose, and post-graduate training opportunities nearly doubled. In addition, the Child Study Center was completed in 1969 to assist children whose school and home adjustment was impaired because of minimal brain dysfunc­ tion, inner conflict, or emotional problems.

With a new decade came important changes in campus leader­ ship. In the Spring of 1970, Vice President Storm Whaley resigned to accept a position with the N.l.H. and was succeeded by Dr. James L. Dennis as Vice President for Health Sciences.

Whhen Dr. Dennis took office, he was faced with the prob­ lem of severe health care deficits in many of Arkansas' rural communities. With his concept of a “ University Without Walls” , he tried to make the campus more responsive to the health care needs of the state. In addition to increasing enrollment in all the health related colleges, his efforts to increase Family Practice residency training programs, and creation of the Area Health Education Centers have helped bridge the medical campus and the rural communities where health care needs are so acute.

In an effort to fuse the Medical Center into an organized, united campus. Dr. Dennis proposed an administrative reorganization of the newly renamed University of Arkansas for Medical Sciences with his assumption of the position of Chancellor. The plan called for the creation of four autonomous colleges under the supervision of the Chancellor, and in 1975 it was adopted by the Board of Trustees.

The following year, Dean Winston Shorey retired after 13 years as Dean of the College of Medicine, and assumed the directorship of the AHEC program until his untimely death in January of 1976.

His successor. Dr. Thomas A. Bruce took office in the summer of 1974 and has in the five years since then contributed to faculty expansion, increased student enrollment, and efforts to afford more student and faculty input into the governance system of the campus.

With the increasing enrollment in all the colleges came a need for even more educational facilities on the campus. Begun in the fall of 1974, Education Building Two was completed in September of 1977. To upgrade clinic facilities, a new ambulatory teaching center will be opened by Summer 1979. Combined with renova­ tion of University Hospital and construction of a new administra­ tive wing, all these projects have given the campus a truly con­ temporary appearance, and a firm foundation on which medical education in the state of Arkansas can enter its second century of achievement.

The University of Arkansas first became involved in the training of health professionals in 1879, when the Medical Department of Arkansas Industrial University was formed. Since its beginnings as a school for train­ ing only doctors, it has grown into a multifaceted ac­ ademic center educating physicians, nurses, pharma­ cists, and the many support personnel needed for to­ day’s health care.

Located on 26 acres in west Little Rock, the Univer­ sity of Arkansas for Medical Sciences serves as the center of medical education in the state of Arkansas.

With a combined enrollment of nearly 1500 students, the Colleges of Medicine, Nursing, Pharmacy, Health Related Professions and the Graduate Division share the educational, clinical, and research facilities on the campus.

Although University Hospital and its outpatient clinics remain the primary clinical facilities for all UAMS students, the more than 300 interns and resi­ dents now work state-wide, through cooperative agreements with other hospitals in metropolitan Little Rock and six regional Area Health Education Centers in other parts of the state.

INTRODUCTION An Area Health Education Center (AHEC) Program for the State of Arkansas was originally proposed in January, 1973, by the Governor’s Committee on Primary Health Care as one of six major components of a State Plan for Primary Health Care. The proposed AHEC plan was subsequently adopted, authorized and funded by the State Legislature in Eebruary, 1973. The program was assigned legislatively to the University of Arkansas for Medi­ cal Sciences and the responsibility for the program was delegated specifically to the Vice-Chancellor for Health Sciences.

As originally conceived, the AHEC Program was to focus prin­ cipally on the state's primary health care needs, particularly in the medically underserved areas. The program was to include efforts to retain more of the graduates of the state’s College of Medicine for practice in Arkansas; the training of more primary care physi­

cians. particularly family practitioners, for the state; and to im­ prove the geographic distribution of these physicians over the state. The program was also to address the needs of the medically underserved areas of the state for other healthcare professionals and disciplines, including nurses, clinical nurse practitioners, physicians’ assistants, and other personnel needed for improved primary health care. The plan called for the demonstration of the team approach to the delivery of primary health care services, and the utilization of this in the training of the various health and health related professions students. It was envisioned that there would eventually be six Area Health Education Centers over the state and that each would attempt to recruit and provide a portion of the training of individuals from within the area, who would ultimately return to their home communities for careers in the practice of primary health care.

Area Health Education Centers

AHEC Fayetteville

The Northwest Arkansas AHEC at Fayetteville was established in November, 1974, with the appointment of Lee B. Parker, Jr., M.D as AHEC Director. In February, 1975, James K. Patrick, M.D., was appointed Director of the Family Practice Residency Program which is based in the AHEC at Fayetteville. This program, which began in July of 1975, incorporates the facilities of Washington Regional Medical Center, Fayetteville V.A. Hospital, Fayetteville City Hospital, and Springdale Memorial Hospital. Cumulatively, these hospitals represeni a total of 692 beds with an average census of 559, and 82 physicians who serve on the staff of the program.

AHEC Texarkana The Area Health Education Center—Southwest was established at Texarkana, Ark. on August 1, 1975 with the appointment of James B. Kittrell, M.D. as Director. The AHEC-Texarkana headquarters is located in facilities provided by St. Michael Hospital, Texarkana, Ark. The facilities of this hospital, as well as those of Wadley Hospital in Texarkana, Texas, are utilized for the AHEC educational progranr. Since July of 1976, elective clerkships have been offered to senior medical students at AHEC-Texarkana, with future plans including development of a Family Practice Residency Program.

AHEC Jonesboro The Jonesboro AHEC, established in December 1974, and directed by Robert S. Cohen, M.D., offers elective programs for senior medical students in almost 20 specialty fields. The facilities in Jonesboro include St. Bernard’s Hospital, a 240 bed general hospital, and Craighead County Memorial Hospital, which are served by 62 physicians. The trade area of over 300,000 persons also benefits from a mental health center which has both in and out-patient services.

AHEC Fort Smith The Fort Smith AHEC, under the directorship of John R. Williams, M.D., provides electives in most of the major specialty and subspecialty areas. The excellence of this experience is due to the quality of the medical community, some 165 in number. Sparks Regional Medical Center, which is currently expanding to 681 beds, and St. Edward Mercy Center, which has recently increased its capacity to 33S beds, demonstrate the progressive nature of the area. The Family Practice Residency Program, under the leadership of Dr. David Busby now in its fourth year with 14 residents, six second-year and eight first-year.

AHEC Pine Bluff AHEC-Pine Bluff was established in September 1973, and since April 1974, has had successful clinical rotations for UAMSC medical students and internal medicine residents. Donald L. Miller, M.D., is director, with offices located in Jefferson Hospital. The hospital isa 315 bed facility, with a medical staff 83 physicians. Medical and audiovisual libraries ere located in Jefferson Hospital and staffed b) UAMSC trained librarians. The AHEC-Pine Bluff Family Practice Residency Program, directed by James A. Lindsey, M.D., first accepted residents in July of 1977, and now has four, one second-year and three first-year. The Medical Social Work Education Program isdirectedb) Carolyn N. Sanders, ACSW, and offers one year of field training for graduate social work students in conjunction with UALR.

AHEC El Dorado Established in November of 1973, the El Dorado AHEC program affords participating students and residents the opportunity to observe and participate in the health sciences as practiced in rural and suburban communities of South Arkansas. Currently, emphasis is being placed on electives for senior medical students and nurse practitioner students, preceptorship programs for junior medical students, and continuing education for physicians and nurses. Jacob P. Ellis, M.D., is the director of this AHEC whose future plans include the development of a Family Practice Training Program.

Each of these training programs has a Director and a complete College of Medicine appointed teaching faculty in all the major medical specialties and subspecialties from the local practicing community, and are affiliated with the local private hospital(s) in the community.

The overall goal of the residency program is to teach future family practitioners to become skilled, competent physicians con­ caiBif cerned about the total patient, his family, and community. An­ other important goal of the program is to achieve a better distribu­ tion of primary care physicians throughout the state, but espe­ cially for practice in less populated areas of Arkansas.

In order to achieve these goals the residency training programs emphasize instruction in internal medicine, pediatrics, obstet­ rics-gynecology, surgery, emergency room, behavioral science and psychiatry, with additional experience in a variety of sub­ specialties. Also, each program has a fully staffed and equipped ^ 0 model Family Practice Center which is the focus of activity for li(W outpatient training during the three year program.

Each of the AHEC Family Practice Residency Programs has received federal training grant awards to expand their programs. ijisif* The grants were effective July I, 1977, and each is assured for a period of three years, with provision for renewal upon successful application for a period of two additional years. As a result of these federal grants, during the next four years the three AHEC Family Practice Residency Programs will be able to more than double the total number of residents completing the three years of family practice training. Similar residency programs are being planned for the AHECs at El Dorado, Jonesboro and Texarkana. i.eHI* During the present academic year, 1978-79, a total of 31 family icalsl* practice residents are in full-time training. Sixteen in the first year, sin* 11 in the second, and 3 in the third year.

FUTURE DIRECTIONS Progress toward providing clinical training for students of the other health professions and allied health disciplines has been very limited to date. Early in 1978, preliminary discussions began with representatives of the College of Nursing and the College of Pharmacy, which have now led to the planning of formal programs of clinical training for these students in the AHECs. Tentative plans include the provision of opportunities in the AHECs for teaching and training of these students in the delivery of ongoing, first level care, utilizing a multidisciplinary team or group practice relating to a state system of regionalized secondary and central­ ized tertiary medical care.

Additional expansions of the AHEC program include the estab­ lishment of: I) Family Practice Residency Programs in El Dorado, Jonesboro, and Texarkana AHECs; 2) sub-area or SAHEC cen­ ters; 3) outlying satellite clinics; 4) health education programs for the public; and 5) increased AHEC involvement in regionalized continuing education programs for physicians and other health professionals.

SAHEC centers would be strategically located over the state in intermediate sized communities in order to provide another level of clinical experience and training for health profession students and primary care residents. Each would relate to and be a part of one of the six AHECs. The satellite clinics would be established in small, isolated, rural communities, to be staffed by clinical nurse practitioners or physician assistants and other appropriate ancillary personnel.

The satellite centers would relate closely to the AHEC or SAHEC in which each was located and would become model programs of front line rural health care and education, and would serve as training sites for health profession students particularly future primary care practitioners. The purpose would not necessarily be to encourage physicians or others to enter practice in these com­ munities, but rather to orient them to the concepts and methods of extended health care through the utilization of new primary health care practices. *

Office of Community Medical Affairs

The College of Medicine has received a contract with the Department of Health, Education, and Welfare j for a project to be conducted jointly with the Arkansas Department of Health to provide assistance to rurab communities which are medically underserved and are seeking to recruit physicians.

Titled, “ Consultation and Assistance for Medically Underserved Rural Areas” , the project will have a] staff of four field representatives who will provide direct assistance to communities in such areas as evaluating j the community’s health manpower and assistance in the placement of appropriate health manpower resources.

The staff of this project will have available a wealth of definitive information concerning a large number of j Arkansas communities, and housestaff interested in practice opportunities in Arkansas are invited to contact] Bill North, Director, Office of Community Medical Affairs at (501) 661-5620.

Additional activities of the Office of Community Medical Affairs include: 1. “ Another Arkansas Practice Opportunity" exhibit. This is a permanent display in our University Hospital which gives pertinent information regarding communities throughout the state that are attempting to recruit physicians. We feature a new community I approximately every three weeks.

2. "The Physicians’ Opportunity Fair." This is an annual event in which we invite communities who are seeking to recruit physicians to I come to our campus for one day in order to afford them an opportunity to meet with medical students and residents. We provide each j community with a booth in which they may display materials which tell about their community and that community’s need for] physicians.

3. "Arkansas Practice Opportunities Familiarization Program." This project attempts to present to junior and senior medical students! information concerning the state of Arkansas, to involve the students in a successful physician retention program, to involve the! communities in a program of recruiting and retention and to arrange for meetings between medical students and communities seeking tol recruit physicians. Four times during each academic year we take a limited number of junior and senior medical students and their] spouses on a weekend bus trip (8 a.m. Saturday to 5 p.m. Sunday) to visit four communities in the state in order to accomplish the] purposes outlined above. We began this program in October, 1976, so we are now in our third year. There is no cost to student or spouse] — no commitment on the part of the student — great food and entertainment.

On March 15, 1979, an era will come to an end at UAMS with the retirement of Dr. James L. Dennis as Chancellor.

Since his arrival at UAMS in 1970 as Vice Presi­ dent for Health Sciences, Dr. Dennis has provided outstanding leadership and foresight in the develop­ ment of this campus. With his concept of a “ Medical Center Without Walls” , development of the Area Health Education Centers, expansion of student en­ rollment, and increases in Family Practice training and practice opportunities, he has led UAMSC in the effort to provide training for health professionals to meet the health care needs of the people of Arkansas.

With the reorganization of the U. of A. Medical Center into the University of Arkansas for Medical 40

Sciences in 1975, and completion of the Education Building II in 1978, Chancellor Dennis has helped produce dramatic changes in the inner workings and outward appearance of the campus . . . all for the better. The latest fruits of his many efforts include remodeling of the University Hospital and comple­ tion of the new Ambulatory Teaching Center.

Dr. Dennis, an outstanding leader, accomplished organist, and superb clinician, will be missed by all those at UAMSC that have had the privilege of knowing him, and especially by those of us who have benefited from his efforts to raise the quality of med­ ical education in the state of Arkansas to a level equaled by only a handful, and surpassed by none.

(20.000 sq. ft.), which will include sixteen private room s, a six-bed intensive care unit, and an obstetrical suite.
