CHNA IMPLEMENATION PLAN

Support, advocate and lead efforts to realize a healthy, diverse, inclusive, and welcoming community that offers all residents access to quality health care and that attracts families and workforce because of its affordability and livability. (Health Equity consideration).
- Explore ways to support the City of Willcox and other players in economic development efforts;
- NCCH applied for economic development grant (will know results summer 2024);
- Explore how Southeast Az Economic Development Group might be of assistance;
Provide services and support for health aging in place for our rapidly growing senior residents.
Health Equity Consideration, given the higher utilization of healthcare in this population and the steerage away from traditional provision of care models in distant rural locations.
- Explore provision of in person and/or telehealth specialty appointments for area residents with TMC One;
- Work with SEAGO and AAA to understand programming and assist in “getting out the word”.
Partner, advocate, educate and directly provide services to address the community’s growing behavioral health needs.
- Improve uptake of Concert health within NCCH Clinics;
- Continue growth of outpatient geri-psych program;
- Gain understanding of what services area organizations provide;
Improve access to primary care and dental services.
Fourth NP was recruited late 2023 for RHCs;
- Continue Physician Recruitment initiative;
- Encourage dental opportunities with our federally qualified health center, Chiricahua Community Health Centers.
