Eating, Obesity and Diabetes Mental Health and Alcohol/Substance Abuse
Good Jobs and Healthy Economy
• Grow partnership with Diaper Bank of Southern Arizona. Diapers are required for day care programs – and often this is a limiting factor for some families.
• Join with Northern Cochise Community Hospital to evaluate the possibility of a collaborative effort with the school to introduce health careers to students
• Develop partnership with local school district for the Joint Technical Education District (JTED) certified nurse assistant program.
• Develop clinical externships for Cochise College medical assistant students.
Mental Health and Substance Abuse
• Expand Concert Health, a behavioral health partnership, to second primary care location
• With Northern Cochise Community Hospital, evaluate the feasibility of a geriatric psychiatric outpatient/telehealth program.
• Evaluate the possibility to medication assisted treatment (MAT) at primary care sites
• Collaborate with the local chapter of the National Alliance for Mental Illness to train frontline staff in mental health first aid (this is a carry over from prior implementation, project delayed due to pandemic)
Healthy Eating, Obesity and Diabetes
• Re-establish bariatric program (in-person & telehealth) that was put on hold during the pandemic. This will include adult and pediatric bariatric programming.
• Establish a Benson-based endocrinology program (inperson and telehealth) in collaboration with TMCOne.
• Implement chronic care management in primary sites to help patients manage conditions such as obesity and diabetes.
• Continued support of the Food Pantry and expand POWOW (Produce On Wheels Without Waste). Weekly recipe of the week in collaboration with Safeway to continue.
• Expand access to outpatient nutritional consults,
