TRINITY COLUMBARIUM AGREEMENT Trinity Episcopal Church 316 Adams Street Toledo, Ohio 43604 Trinity Episcopal Church (the “Church�) agrees to provide to Subscriber the interment of cremated remains in The Trinity Columbarium. The Subscriber agrees to pay to the Church $500 and to abide by the Additional Terms of this Agreement (and by any amendments to those terms that Trinity Church may make in the future).

TERMS OF AGREEMENT 1.

The Subscriber agrees that the Trinity Columbarium and all niches are at all times under the sole jurisdiction and control of the Church.

2.

Inscriptions on the plates of niches and memorial plaques are limited to the name, and years of birth and death of the deceased, and each inscription, plate, or plaque must first be approved by the Church, and shall be in accordance with any additional regulations the Church may adopt from time to time.

3.

The rights herein may not be assigned by the Subscriber without the prior written approval of the Church. The Church reserves the right to withhold approval of any assignment.

Assignees shall be bound by the terms of this Agreement (and by any amendments to those terms that Trinity Church may make in the future) and any other applicable rules and customs of the Church.

4.

Once a niche has been sealed, cremated remains shall not be disturbed or removed without the approval of the current Rector.

5.

The Church shall exercise reasonable care in maintaining The Columbarium. The Subscriber acknowledges that no liability shall lie against the Church, its Rector, Wardens, Vestry, members or employees arising out of the maintenance of The Columbarium or out of any loss or damages to urns, ashes, plates or plaques of any deceased person.

6.

The Subscriber acknowledges that The Columbarium and all undertakings and rights with respect to its maintenance and operation are under the jurisdiction of the Rector, Wardens and Vestry of the Church.

7.

No embellishment, ornament or memorial shall be placed within the Columbarium area without approval of the Rector, or in his or her absence, the Wardens of the Church.
