Care and support for the elderly is based on classifications that contain standardized concepts. It contains nursing diagnoses, outcomes and interventions. The internationally accepted, reliable and valid NP-CDSS standard has been mapped with the Mikzo, the classification most commonly used in the long-term care of the elderly in the Netherlands.
Zorg en ondersteuning voor ouderen is gebaseerd op classificaties die gestandaardiseerde concepten bevatten. Het bevat verpleegkundige diagnoses, uitkomsten en interventies. De internationaal geaccepteerde, betrouwbare en valide NP-CDSS-standaard is gemapped met het Mikzo, de classificatie die in Nederland het meeste wordt ingezet in de langdurige zorg voor ouderen.

The NP-CDSS supports the nurse by containing SNL presenting knowledge (defined concepts for each phase of the Advanced Nursing Process.
The Mikzo classification uses the concepts of Nanda and Omaha System and their Standard Nursing language for each phase. All components of the Mikzo classification are mapped to Snomed CT and captured as a digital key. Additional to these concepts the PES (Problem –Etiology- Signals) is added to confirm the nursing diagnosis.

The NP-CDSS supports the nurse with possibilities for free text entries (besides standardized terminology) and open documentation (ie, specific judgments, rapports, protocol adaptations related to the patient situation, operationalization of unclassified diagnoses, goals, interventions, and outcomes and evaluations).
The Electronic Health Records that offer the Mikzo classification have the next free text entrees:
- A free text field for the nursing diagnosis and the PES (problem – etiology – signs and symptoms). To support the nurses a document is added with the most common signs and symptoms for each of the 31 topics.
- A free text field is added for the interventions.

De elektronische client dossiers die de Mikzo classificatie aanbieden, hebben de volgende vrije tekstingangen:

The NP-CDSS supports the nurse by containing relevant, holistic nursing documentation regarding patient information on physical aspects, psychosocial aspects, functional aspects, and environmental aspects.
The Mikzo classification has the Dutch Quality Framework for nursing home care as its starting point. There is a domain for the personal information, a domain for environmental aspects, a domain for aspect concerning wellbeing with three areas, a domain for mental ans physical risks with twelf areas and a domain for health issues with fifteen areas. For all areas there are five indicators to judge the possible demands for care.

The NP-CDSS supports the nurse by presenting prestructured, standardized, logical, and coherent (knowledge-based linkages between all phases of the nursing process: It links assessments with diagnoses, interventions and nursing-sensitive patient outcomes.
The Mikzo classification has the nursing process incorporated in the three steps that are mentioned under 1. The nursing-sensitive patient outcomes in each area are captured in five indicators. The indicators are based on the Nursing Outcome Classification, scientific information and actual protocols. The Mikzo classification is available in all major companies that offer an EHR.
Explanation
Twice a year the users of the Mikzo classification join an online meeting to discuss the Mikzo classification and its use in the EHR. The discussion is also about data driven care, auditing and cooperation.
