[ letter from the editor ] Two things I’ve long admired about St. Louis are its incredible philanthropic community and its wealth of cultural offerings. You could go to cities a lot bigger and still not find a local opera company or an international film festival (not to mention a homegrown circus and the Chess Hall of Fame). Or, for instance, a Jewish Film Festival. That’s where I spent several evenings a couple of weeks ago. Not only were the movies carefully selected by a local committee, but you probably won’t find them anywhere else. I don’t suppose Netflix has a movie about the Polish priest who, unbeknownst to him, was born to Jewish parents or one about a young imam-in-training who pretends to be Jewish because when he finds friendship for the first time in his life, it is with a group of Orthodox boys. True, all these movies have some kind of Jewish content, whether they take place in Poland during the 1960s, Morocco during the 1930s or the U.S. today. But at their core they are about the human experience, and not limited to any ethnic group, just like, say, Girl With the Dragon Tattoo or Like Water for Chocolate. Which brings me back to philanthropy and culture: This festival is generously underwritten so that anyone can attend for $10 a movie. In fact, I ran into one of the sponsors between movies. She agreed when I said the films were wonderful, but also asked: I don’t know why more people in the community don’t come see them? —Dorothy F. Weiner Editor in Chief se

A better fence— well, higher, at least—will make better neighbors of Schnucks and residents of a subdivision adjacent to the new store under construction at Clarkson and Kehrs Mill roads. Ballwin city officials agreed to allow a height change in privacy fencing for the project to 10 feet from an 8-foot maximum, bringing to a close the disagreement between Schnucks and residents that began in 2009. Meanwhile, Schnucks is reworking engineering for a berm that also will help provide a buffer between the store lot and the neighborhood. Maybe the attitude of homeowners who allow teens to drink at parties on their premises is laissez-faire because, well, they did it when they were kids. Or they don’t worry because they think they can ‘keep an eye on them.’ Problems arise, of course, when intoxicated young people drive themselves home, perhaps taking friends with them. All too often, tragedy results. While not naïve enough to think they can put the kibosh on underage drinking, Chesterfield authorities are trying to introduce a chill factor—a $1,000 fine for the homeowner, even if they’re not present when minors are caught boozing it up. It’s more than a little creepy, but is it a crime? Cops here say they’ve identified the so-called ‘serial hugger’ who has been approaching women in public places and asking them for hugs. A woman shopping at Schnucks in Des Peres says a stranger approached her, said he knew her from somewhere, told her it was his birthday, then asked her for a hug. She complied, then called the cops after he left. Police in Ballwin, Manchester, St. Peters

and the metro-east have had similar complaints dating back a year or so. Back in the day, Spencer’s Grill in Kirkwood was one of the last holdouts during the lunch-counter protests of the mid-20th century, finally forced to integrate via the nonviolent action that had already succeeded in St. Louis. Now, another historical bit has surfaced that demonstrates the racial intolerance of several generations ago. The bylaws of the Osage Hills subdivision, ca. 1926, stipulate that non-whites may not occupy a house, nor enter unless they are servants. Of course, the federal Fair Housing Act of 1968 has since trumped the antiquated provision, but we’d bet that other neighborhoods throughout the metro have bylaws with similar offensive language. The document has historical significance; otherwise, to quote a city spokeswoman, it is “ridiculous.” Today, the neighborhood is racially diverse. In the entertainment business, the saying goes: any publicity is good publicity. Jumpin’ Jupiter, the cabaret-style nightclub in Maplewood across Manchester Road from Shop ’n Save, has been getting its share of not-so-positive attention of late. First, the business was nearly evicted for late rent payments. Then, Van Ella Productions, the former show producer, severed its relationship, alleging non-payment. Van Ella is the surname of burlesque performer Lola van Ella, and complaints had been made about her dancers standing outside the club to beckon would-be patrons inside. They wore corsets and stockings, and performers for the new show producer wear T-shirts outside while another gyrates in the front window on weekend nights. How edgy is too edgy for the ’Wood remains to be seen… or unseen.

The historic church at McKnight and Manchester roads has gone on to its higher reward—after a fashion. The Rock Hill Presbyterian Church wasn’t destroyed; it will be reincarnated. The pre-Civil War building is to be reassembled at Cedar Lake Cellars winery and entertainment venue in Wright City. Earlier this year, it looked as though the church would be moved a few hundred yards to Rock Hill Park to make way for a U-Gas station and convenience store. Fairfax House, which already has been moved once since the 1840s, has been removed from its original location facing Manchester Road and will be relocated elsewhere on the U-Gas property. The flying carpet has been grounded. Becky’s Tile & Carpet superstores, known for years via cheesy TV ads featuring the ‘Queen of Carpet’ riding an Arabian rug in front of—then flying through—the Gateway Arch, was in business for nearly 60 years when owner Becky Rothman announced a few weeks ago that the stores were closing. Rothman blamed the sagging economy for the move and asserted the company is doing everything it can to fill orders, if not fulfill customer expectations. Between two dozen and three dozen people will be out of work. Rothman’s father established the business in East St. Louis; when he passed away, his daughter took over when she was just 22. Town & Country’s loss of land in and around the Hwy. 141 project will be Chesterfield’s gain— and vice-versa, in another section. Meanwhile, Creve Coeur stands to gain some acreage from Chesterfield as well, all in the interest of clarifying city boundaries for emergency services of the three municipalities. About 107

acres will change hands, but there won’t be financial repercussions. The land, all vacant, presently features boundaries that zigzag according to property lines that no longer exist. The new city boundaries will follow the roadway, a project slated for completion in mid-July. The cities and the Missouri Department of Transportation have their fingers crossed that the process won’t need review by the county’s Boundary Commission. Stop the presses! The ones that might be printing up U. City stationery emblazoned with the new logo, that is. City officials voted 4-3 recently against using the logo that was unveiled in March at U. City’s Celebration of Diversity and has since been used on signs and banners. The kerfuffle is the result of some officials complaining that they weren’t kept in the loop, to turn a phrase. Other objections ranged from the colors chosen to the prominence of the could-be brand identity, Neighborhood to the World.

WILDWOOD

Maybe we don’t get out much, or at least not on two wheels, but Wildwood’s recent bicycle recycling effort was a new spin for us. The city participated with the nonprofit St. Louis BWorks (Bicycle Works) to collect bicycles and parts that will be used to recondition those two-wheeled conveyances that came in missing spokes here or a pedal there. Several other bicycle recycling drives will be held, either with a corporate sponsor or other municipalities, to put needy kids in the rider’s seat. Youngsters get a chance to work with a group to solve problems, meanwhile getting a shot at their own bicycle and accessories— including, of course, a helmet.

Jeff, who is in his 30s and dealing with lifelong developmental disabilities, had never been able to find a good job or keep the low-paying positions he managed to get. “It was tough on him, especially at gatherings when the attorneys, engineers and CEOs in our family talked about work,” his mother recalls. Then Jeff joined Life Skills’ employment services program. Today, he is successfully employed. “Now when we get together, he talks about his job with the same pride and sense of purpose as everyone else,” his mom says. “I’ve never seen him so happy. ” Life Skills helps teens and adults with developmental disabilities such as autism, Down syndrome and cerebral palsy. “We teach them the skills they need to find and keep a job, make friends and function as independently as possible in their community,” says CEO Wendy Sullivan. Given the increase in autism diagnoses over the past decade, thousands of children with the disorder will enter adulthood during the next 15 years, she adds. Responding to the growing need for support and services, Life Skills has merged with TouchPoint Autism Services, effective July 1. “Together, we can better assist individuals and families facing autism and other disorders.” The new agency will reach more than 4,500 people in 95 counties statewide. When Life Skills started in 1964, there were few education, employment or even social opportunities for people with developmental disabilities. But gone are the days when they were confined to the margins of society, isolated in institutions. “No matter how severe their limitations, everyone can learn,” Sullivan says. Life Skills’ employment services program now has more than 275 clients working at 175 area businesses. “These aren’t ‘for show’ positions, but real jobs at competitive wages. Our clients don’t want favors. They hold their own alongside employees who don’t have disabilities. They participate fully and make a substantial contribution to businesses throughout St. Louis.” Training and other programs and services are expensive. “Life Skills receives some government funding, but it doesn’t cover costs or reach enough people,” Sullivan says. The organization’s major fundraiser, Par-Tee Golf Tournament and Dinner Auction, takes place July 22 and 23 at Meadowbrook Country Club. “This year we’re hoping to accommodate more guests by having the dinner and auction on Sunday and the tournament the next day,” she explains. Honorary chair is hockey Hall of Famer Bernie Federko; tournament chairs are Bob Budenholzer, Jeffrey Hunter and Mark Ianni. Dave Mungenast Lexus serves as Hole-in-One Sponsor. “We raised $350,000 last year and we’re hoping to beat that,” Sullivan says. Every dollar makes an impact: “The most valuable thing we do is open people’s eyes to what an individual with disabilities can actually accomplish. Everyone deserves the opportunity to take pride in their work and make a meaningful contribution to their community. Each human being has so much to offer. Life Skills gives them a chance to do it.”

Peter and the Starcatcher, a hit show he co-produced with Disney Theatrics, was nominated for nine Tonys and won five of the prestigious awards, including best performance by an actor and best sound, costumes, scenic design and lighting design. “It was a great night for the entire team—we’re still on cloud nine!” Lane says. Based on a best-selling novel by Dave Barry and St. Louis’ Ridley Pearson, the play is a prequel to Peter Pan. You can still see it at New York’s legendary Brooks Atkinson Theatre.

How’s this for a senior project: Hannah Akre, a recent MICDS graduate, established her very own nonprofit organization. Gloves Go Global collects unused laboratory gloves from science students at Washington University and ships them to understocked hospitals in South America, Haiti and Kenya. Wings of Hope and The Outreach Foundation help distribute the gloves. “My family and I often travel to Kenya on medical mission trips, so I’m aware that Third World facilities often can’t afford the basics,” says Akre, who will attend Colgate University this fall and plans to be a doctor. [ Special Centennial ] St. Louis Country Club will host the 2014 Curtis Cup Match, the U.S. Golf Association recently announced. The match features teams of women amateur golfers from the U.S. and Great Britain/Ireland in a match/ play championship. This will be the sixth USGA event held at the club, but the first since 1972. “St. Louis Country Club sees the event as a particularly appropriate way to celebrate the 100th anniversary of its renowned Charles Blair Macdonald-designed course,” says match chairman Bill Sedgwick.

Justitia, the Roman goddess of justice, is usually portrayed wearing a blindfold. Justice may be blind, but she’s not isolated from social change. As times evolve and customs shift, so does the practice of family law. “It used to be that 50 percent of all marriages ended in divorce; the past five years or so, it’s probably closer to 40 percent,” says attorney Margo Green of Green Cordonnier & House. Unfortunately, lower divorce rates don’t necessarily mean couples are learning to get along better. “Statistically, it means not as many people are getting married, a trend that’s gained traction since the recession started. They can’t afford marriage.” But just because people aren’t getting married doesn’t mean they’re not having children. In fact, more than half of births to American women under 30 now occur outside marriage. “It wasn’t all that long ago that having a child out of wedlock was considered a scandalous disgrace,” Green recalls. “Often, the parties involved didn’t even want to show their faces in my office; I used to meet them in a coffee shop! These days, even in a conservative town like St. Louis, a great many women are proud and perfectly comfortable being unmarried moms.” But custody, visitation and child support issues still need to be ironed out, even among single parents. “That’s why the number of paternity suits filed nationwide by single parents has risen from 12 to 22 percent in the last decade,” Green says. “Even if they don’t marry, parents still need to find common ground when it comes to their child’s security and well-being.” This is especially important because relationships between unmarried parents are more than twice as likely to dissolve than marriages, she adds. For better or worse, the pace of social change seems to be accelerating, aided and abetted by rapid technological innovation. “Whether that’s good or bad is ultimately for historians to decide,” Green says. She sees the divorce rate continuing to decline as fewer people marry. “I also think we’re entering the era of do-it-yourself divorces, especially among people who don’t have a substantial marital estate,” she says. “People will just download forms and fill them out.” The process will be fast and relatively inexpensive, but not necessarily in the best interests of both parties. “That’s why it will always make sense to consult an experienced family law attorney,” Green says. “My colleagues and I at GC&H are trained to zero in on increasingly complex financial, tax and other issues that don’t occur to most divorcing couples. New legislation has been passed regarding ignition interlock devices that will go into effect Oct. 1, 2013. These will include photo identification technology and global positioning features to track drivers who have received two intoxication-related traffic offenses within five years, or a 10-year DWI-related suspension. Fortunately, attorneys across the city generously give their time and expertise at legal nonprofits, so justice can be accessible to all St. Louisans.

In 1982, Joe Taylor watched his father win a landmark case in the Missouri Supreme Court, which mandated that under certain circumstances, businesses must provide a level of security for their patrons. It was a particularly personal victory; his father’s friend was raped in a St. Louis entertainment district after a business refused to walk her to the car. “I’ve grown up watching my father advocate for victims,” Taylor says. “When I decided to join him, I followed suit.” Today, father and son practice victim rights law at Taylor & Taylor, and Joe Taylor sits on the board of Crime Victim Advocacy Center of St. Louis. The organization provides a range of legal, social and mental health services to families and victims of violent crimes. “We try to use the resources of the community to help,” he says. “We offer free counseling and assist with filing for victim compensation claims, among other services.” CVAC does not have an on-staff attorney, so Taylor makes himself available for legal advice or, in some cases, actual representation. “Anyone can call me, and I will give them my time to figure out if there is anything we can do to fix the problem,” he says. “I like to help because a lot of these folks have already been through hell.” Many of CVAC’s clients are referred by the St. Louis Police Department. One senior citizen was tied to a chair while burglars ransacked her home and claimed to have killed her dog, Taylor says. After the police arrived, the woman was so distraught she refused to leave her home for medical attention. “She didn’t want to leave her dog, so we were able to get her community church involved to stay with the pet and board up the windows,” he says. Taylor says helping victims cope with the aftermath of violent crimes is essential. “Once the police step out, their job is over,” he says. “If people don’t have resources and the wherewithal to get psychiatric care to help understand and get back on their feet, that crime will plague them for the rest of their lives. This service helps focus them again.”

Deborah Benoit found her legal calling by chance. “I never expected to go into family law,” she says. “I tried it early in my career and found I really enjoyed it. You deal with people during stressful times, but I find it very rewarding because I’m able to help people get through it.” As a partner at Kruger & Benoit, she regularly handles domestic violence, child support, paternity cases and more. So when she heard about the work done at Legal Advocates for Abused Womenin the 1990s, she knew she had something to offer. LAAW works to protect women and children trapped in abusive relationships through crisis intervention, emotional and legal support, safety planning and advocacy. Since LAAW has only a few full-time attorneys available, Benoit’s skills are highly valued. “There’s a lot of work to be done regarding domestic violence,” she says. “It’s a natural fit for me as a woman lawyer with a practice in family law; I have the ability to help others in domestic violence situations.” The pro bono work is particularly important to Benoit, who says her legal education empowered her to give back. “Not everybody in my family was able to get an education. I felt fortunate to go to college and be as successful as I am,” she says. “As a result, I had a skill I could use to help other people.” Abusive relationships often go beyond domestic violence, Benoit says. Often, extenuating circumstances complicate already devastating issues. “I have represented people in cases that have involved not only troublesome family situations, but at the same time also terrible issues with children, or clients who are battling cancer,” she says. “It’s horrible when all those things happen at once. That’s why this job is so rewarding.”

Jordan Ault Legal Services of Eastern Missouri

What began as a chance for a young lawyer to log some hours in the courtroom turned into a lifelong passion for Jordan Ault of Husch Blackwell. “Many cases aren’t going to trial anymore, and courtroom experience for young associates is at a premium,” he says. “Pro bono was a great way for me to manage cases and get client experience. But after handling a few of these cases, it hits you how much you are able to really affect someone’s life for the better.” Ault began volunteering through Legal Services of Eastern Missouri in 2007. The organization helps thousands of clients each year tackle civil cases, such as health, welfare, immigration and housing. “I love my day job here at the firm, but there’s nothing like going home after helping a young single mother stay in her home.” Some clients simply need help comprehending the complex justice system. One of Ault’s cases involved an elderly woman whose identity was stolen. The constant calls from collection agencies stressed her to the point of hospitalization. “She was very proud that she’d always been able to pay her bills,” Ault remembers. “We didn’t have to go through the courts, but someone needed to explain how identity theft worked, and that she couldn’t fix it on her own. That’s when it hit me that the justice system doesn’t work if people don’t have access to advice.” No longer the first-year attorney with something to prove, Ault still works with LSEM clients. “Professionally, I got experiences very early in my career that led to opportunities I wouldn’t have had. There’s something almost addictive about it. I go through withdrawal if I don’t have a pro bono case going on,” he says, laughing.

> cholesterol drugs show promise after stroke We know cholesterol-lowering drugs, commonly called statins, can prevent heart attack and stroke caused by plugged arteries. New research at Washington University School of Medicine indicates an additional benefit. A study looked at 31 patients with ischemic strokes caused by a blood clot blocking blood flow to part of the brain. In 12 patients already taking statins to control cholesterol, blood flow returned to the blocked areas of the brain faster and more completely than in patients not taking statins. “Tissue plasminogen activator is the only drug we have to treat stroke, but it doesn’t work in every patient,” says Dr. Jin-Moo Lee, director of the cerebrovascular disease section in the

department of neurology. “But this study shows that tPA worked better in patients already on statins. The question now is whether we could give statins with tPA to increase effectiveness.” A new study at Columbia University is looking at statins given after a stroke, and Lee and his colleagues are watching carefully to see if the strategy is effective. They’re also monitoring if the timeframe in which statins are given has an impact on efficacy. “The results are encouraging,” Lee says. “To really test it, though, the statin must be given with the tPA. We hope a trial will be designed and launched in the future, based on the results from the Columbia study.”

obstructive sleep apnea, a condition in which people stop breathing many times a night, has a negative effect on the cardiovascular system. Now studies are linking sleep apnea to increased proliferation of melanoma cancer cells, increased tumor growth in mice—and higher incidence of cancer in humans. “The relationship with cancer makes sense, because we already know apnea can trigger immunological changes,” says Dr. Oscar Schwartz, medical director of the Sleep and EEG Center at Barnes-Jewish Hospital West County. Stress hormones are higher in normal people but much more often in sleep apnea patients, he explains. “When these substances are

irritate or injure the body.” A Wisconsin study showed the more severe the apnea, the higher the cancer risk, with non-obese patients at even higher risk. Getting apnea under control is important, and may involve wearing a dental appliance, undergoing surgery for an anatomical issue, or using C-Pap. “Research shows that people with untreated sleep apnea have up to three times the risk of dying as people without it,” Schwartz says. “So get treatment, even if you think it’s a nuisance.” And don’t make the mistake of thinking that your degree of snoring equals your degree of apnea, he adds. “Actually, the more you stop breathing, the less you snore.”

TB vaccine > new Saint Louis University Center for Vaccine Development is the only U.S. center currently conducting a clinical trial for a vaccine that prevents tuberculosis, the world’s second deadliest infectious disease. “Globally in 2007, there were 1.8 million deaths and nearly 9.3 million new cases of tuberculosis diagnosed, so a safe, effective vaccine is needed,” says Dr. Daniel Hoft, director of Infectious Diseases, Allergy and Immunology at Saint Louis University and a researcher on the study. “The vaccine we have now, BCG, has been around for 90 years, and prevents severe forms of TB and TB meningitis, especially in kids,” Hoft says. The World Health Organization recommends the vaccine be given at birth in countries where the disease is a serious problem. “But BCG doesn’t prevent the TB infection that persists in the body as a latent infection, allowing it to reactivate when the immune system is compromised.” The new vaccine is a form of BCG engineered to produce key antigens that allow the virus to escape from the cell, where the body can attack it. “We’re also looking at whether people susceptible to TB may need more than one vaccine; for instance, should we prime the body at birth with BCG and then come back later with a different type for a boost?” Hoft says. Phase I of the trial had mixed results. “Researchers are now analyzing data and looking at in vitro cell studies to see the effects of the vaccine at the cellular level and to determine design changes for future studies,” Hoft explains

> warfarin versus aspirin In a study of 2,300 heart failure patients, warfarin (Coumadin) did not significantly improve outcomes in patients more than those who took one aspirin a day. “This study looked at patients with a normal heart rhythm who were still at risk for blood clots,” says study co-author Dr. Douglas Mann, chief of the cardiovascular division at Washington University School of Medicine. Warfarin is still safely prescribed for patients with atrial fibrillation, he adds. “But in heart failure, the heart beats less vigorously, so blood tends to move slowly and become more stagnant.” Slowmoving blood tends to clot. “The good news here is doctors can give patients aspirin and be reassured that their heart failure won’t worsen or lead to an increased risk of death,” Mann says. Aspirin is low-cost, easily accessible, and doesn’t require the frequent blood testing needed for warfarin. “When we looked at outcomes separately, the study showed a significant decrease in stroke risk with warfarin, but also an increase in risk for bleeding,” he notes. “Warfarin is hard to adjust to keep people in the target zone for effective blood thinning without significant risk of bleeding, because the drug is affected by so many factors.” JUNE 27, 2012

Most Alzheimer’s treatment starts too late, but a new clinical trial structure and testing group may change all that. The Dominantly Inherited Alzheimer’s Network is made up of people whose families carry a gene mutation that gives them a 50 percent chance of developing early-onset Alzheimer’s disease. “Because this group makes up only 2 percent of Alzheimer’s cases, it has never been included in a clinical trial,” says Dr. Randall Bateman of Washington University. Now DIAN group members, some with the mutation and others without, will be the focus of the DIAN therapeutic trial unit, led by Bateman and conducted in centers across the world. “The DIAN, established in 2008, now has 400 participants under observation,” he says. “We’re measuring changes that may cause them to get Alzheimer’s in their 30s to 50s. This lets us find changes in their brain and cognition 15 to 20 years before onset of symptoms, giving us a unique opportunity for early intervention.” In the trial, slated to start early next year, 75 percent of participants will be on one of three active drugs; the rest will receive a placebo. The study will involve about 120 mutation carriers and some 80 to 90 non-carriers. The first biomarker studies will be completed within 12 to 18 months. Any medications demonstrating positive changes in biomarkers will be continued. The ones that don’t will be replaced with other drugs from the 14 nominated as worthy of testing, shortening the time to find effective preventive strategies. According to DIAN scientists, a six-month reduction in the treatment discovery timeline could translate into up to 2.5 million fewer cases of Alzheimer’s. A $4.2 million grant from the Alzheimer’s Association provides major support for the DIAN network and trials. “The study is built on the backbone of the DIAN observational study, so the data are sound,” says Maria Carillo, the association’s senior director of medical and scientific relations. The Alzheimer’s Association also provides DIAN with a web-based forum for information and sharing at alz.org.

Several studies show a higher prevalence of hearing loss in patients with macular degeneration, especially the dry form. “My own research indicates the risk of macular degeneration is more than two times higher in people with hearing loss, and that the risk is even greater in patients younger than 55,” says Dr. Jay Pepose of Pepose Vision Institute. Pepose hopes awareness about the relationship between hearing loss and macular degeneration prompts patients to visit the eye doctor more often. “We can perform a dilated retina exam to monitor changes in the macula, and also provide a monitoring device for home use.” Because people are more likely to see their eye doctor than an ENT specialist, his practice now offers free hearing screenings to adult clients. “These changes can happen so gradually that people often don’t notice,” he says. “Correcting vision and hearing problems can help people communicate and stay more tuned in to life.”

Area podiatrists have found a way to successfully control pain without the usual side effects of oral meds. Podiatrists John Holtzman, Seth Anderson, Kurt Kaufman, Tim Oldani, and Tony Lombardo have been compounding topical pain medications containing many of the same ingredients usually taken orally. “Each patient may need a different combination of anti-inflammatory or neuropathy medication, along with driving agents that help them penetrate the skin for better absorption,” Holtzman says. The podiatrists work with compounding specialist BELLEVUE PHARmACY to create the individualized treatments. He and his colleagues started using the compounded creams several years ago because so many patients reported side effects from their oral medications. “Using the same drugs in a cream can bypass those problems,” Holtzman says. Hundreds of patients have benefited from the approach, including those with nerve pain from chemotherapy or diabetes, joint pain from arthritis, and painful scars. “People who couldn’t sleep at night because of neuropathic foot pain have found relief, and oncologists are even referring patients to us,” Holtzman says.

Researchers at Washington University School of Medicine have restored normal blood sugar metabolism in diabetic mice by using nicotinamide mononucleotide, a naturally occurring compound. “We hope one day people will be able to take the compound like a daily vitamin to treat Type 2 diabetes,” says Dr. Shin-ichiro Imai, associate professor of developmental biology. NMN is one of several regulators in the body that promote healthy metabolism by the pancreas, liver, muscle and fat tissue, Imai explains. It reacts in the body to produce nicotinamide adenine dinucleotide NAD, a molecule that harvests energy from nutrients and converts it into a form cells can use. “When mice don’t make enough NMN, either because of aging or poor diet, the body slows production of NAD, leading to abnormal metabolic conditions like diabetes,” he says. “When we give diabetic mice NMN, blood sugars often return to normal—the effect is much greater than with any other known chemical.” Because humans also make NMN, the research on mice may one day be translated into new medications, Imai adds. His group is now conducting a long-term study in which the compound is dissolved in the drinking water of mice. “This work is the first step toward developing a nutriceutical that people could take to treat or prevent Type 2 diabetes,” he says.

Saint Louis University’s Center for Endometriosis at SSM St. Mary’s Health Center is offering new treatment options for women with endometriosis, a painful condition that can lead to infertility. “It affects 12 million women in the U.S., including 5 million teenagers,” says Dr. Patrick Yeung Jr., the Center’s director. The condition is often treated with birth control pills and pain medication, but that may not be the most effective method. Yeung works with a general surgeon, a psychiatrist and pain management doctor to diagnose each case. “One of our most important tools is a special type of transvaginal ultrasound that looks at the uterus, tubes and ovaries, and backward into the space between the vagina and the bowel where endometrial nodules can hide,” he explains. “We can see any nodules and how far they may have invaded the bowel. Then we can plan for a one-shot surgery to get rid of all the endometrial tissue.” The American College of Gynecology recommends the ultrasound test, but most doctors don’t use it, he adds. The Center is the first in our region to do laparoscopic excision of all identified lesions with a CO2laser, Yeung says. “Current medical thinking is that the lesions will just come back, but our experience differs: After we removed lesions in 20 teenagers, none were found to have endometriosis when we checked back with them two to five years later. If it’s gone, it can’t grow, and we’ve probably preserved fertility in these patients.”

The aesthetic industry is constantly looking for fillers that add natural-looking, long-lasting volume to facial contours. Two new products, Belotero and Voluma, promise to do just that. Dr. Dee Anna Glaser, professor of dermatology at Saint Louis university School of Medicine, has tested both. “They’re hyaluronic acid fillers, but with different characteristics,” Glaser says. Hyaluronic acid is naturally found in the skin but breaks down over time, Glaser explains. “Current versions are soft, pliable and natural-looking, but tend to be reabsorbed after a few months,” she says. So scientists have taken the basic gel and changed the cross-link structures of the molecules to prevent rapid degradation. Belotero received FDA approval late last year and will be available in the next month or two. “It can be used in delicate areas, such as around the mouth,” Glaser says. Voluma is still in study, but should be out soon. “It’s the first filler of its kind that targets the cheeks and midface volume.” In Canada, where Voluma is already approved, the filler has been shown to last from 18 months to two years, she adds.

Nurturing enlarges brains >

We instinctively know that nurturing is good for a child’s development, but now we have proof that it actually changes the brain. A study by child psychiatrists and neuroscientists at Washington University School of Medicine is the first to show that children nurtured by their mothers early in life have a 10 percent larger hippocampus than those who were not. The hippocampus is important in learning, memory and stress response. “The study involved children ages 7 to 10 who had participated in an earlier study on preschool depression,” says lead author and child psychiatrist Dr. Joan L. Luby In that study, both depressed and non-depressed preschoolers were observed interacting with a parent, usually the mother, who asked the child to delay opening a gift until the mother completed a task. The mother’s ability to support and nurture the child in this stressful circumstance was rated by observers. “When the children were tested years later, brain scans indicated that the ones without depression who were nurtured had a larger hippocampus than children whose mothers were not so nurturing,” Luby says. Depressed children had smaller volumes, consistent with studies on depressed adults. “What surprised us was that nurturing made such a big difference in mentally healthy children,” she adds. For parents, the message is clear: “Understand that how you treat your children is a powerful force in brain development and life success,” Luby says. “Focus on attentiveness and nurturing early in a child’s life, because those are the brain’s formative years.”

Vitamin D Relieves Breast Cancer pain

By sequencing the genomes of ovarian cancer patients and their tumors, researchers are getting a better understanding of the mutations involved in ovarian cancer. “This knowledge is a powerful step to improving diagnosis and treatment,” says Elaine Mardis, co-director of Washington University Genome Institute. Researchers there and at other sites participating in The Cancer Genome Atlas project looked at ovarian tumors from 500 patients with serous adenocarcinoma, the most common form of ovarian cancer, responsible for 85 percent of deaths. They were able to compile a list of mutations that occur in ovarian tumors. “Mutations in a single gene, TP53, were present in more than 96 percent of the most common ovarian cancers,” Mardis says. She and her colleagues have been sequencing cancer genomes and comparing them to DNA in normal cells. “The most exciting part of cancer genomics is being able to look at patterns across different cancers and ask, What do I see that’s the same?” she says. In several hundred breast cancers studied, they found five or six mutated genes also considered leukemia genes. “It may be that an effective leukemia drug could be applied to those cancers.” The study showed different prognoses and some signatures that could stratify them into different probable outcomes, Mardis adds. Investigators found nearly 70 genes that can be targeted by existing medications. “Each tumor is different, but by looking at patterns, we can develop more specific treatment approaches.”

Patients on aromatase inhibitors, a class of hormone-lowering drugs for estrogenpositive breast cancer, frequently experience joint and muscle pain. These drugs are prescribed for five years or more to shrink breast tumors and fight recurrence. Though less toxic than chemotherapy, their side effects are still significant. But a new study at Washington University has demonstrated that a certain type of high-dose vitamin D supplement relieves the pain so therapy can continue. “The study has breakthrough, immediate clinical implications,” says lead author Dr. Antonella Rastelli, assistant professor of medicine. “Fifty percent of patients experience muscle pain severe enough to discontinue this important preventive treatment. We found that giving 400 IU’s of vitamin D per day, with a 50,000-unit dose once a week, relieved the pain.” The study used vitamin D2, a plant-based version, rather than D3 obtained from sun exposure. “The D2 clears the body faster, making it less toxic.” As an added benefit, the supplemental vitamin D also helped patients maintain blood calcium levels by preventing calcium loss from bones. Bone density tests done before the study and at six months showed no bone loss in participants on high-dose vitamin D. “We were the first group to do a double-blind placebo-controlled trial on this issue,” Rastelli says.

Sometime during their lives, 76 percent of all Americans suffer acute back pain. If it isn’t diagnosed and treated it can become chronic, affecting quality of life. Knowing that, the professionals at the Washington University Orthopedic Spine Center in Chesterfield provide comprehensive specialized care to patients with acute back and neck pain. “They need help as soon as possible, so we always arrange to see them within 48 hours,” says Dr. Heidi Prather, director of the Spine Center and a physiatrist in sports and musculoskeletal care. Prather heads up a multidisciplinary team of five physiatrists, two orthopedic surgeons, a nurse practitioner, a nurse educator, a physical therapy practice, and massage and manual medicine therapists. “The philosophy of the Center is that by diagnosing and treating acute pain earlier, we can can prevent it from becoming chronic,” she explains. Each treatment plan is individualized to fit the patient’s lifestyle and includes rehabilitation and education regarding body mechanics. “Because neck and back pain can recur, education is an important aspect of acute care,” she says. “It helps patients learn their role in the prevention and treatment process, so if the pain returns, they’ll know what to do.” Surgery is a last resort, but if indicated, the Center can handle it. “One of our goals is to report outcomes and track patients over time via their pain score, functional abilities and psychosocial aspects of their condition,” Prather says. Measurements are taken when treatment starts, then repeated in four to six weeks. “On average, patients’ pain scores improve by three points; their functional scores also improve, and their psychometric scores move down.” The likelihood of patients needing more treatment after four to six weeks was also reduced,” she adds. “As a nation, we spend a lot of money on back pain, in part because the mechanisms are multi-faceted,” Prather says. “With more than 1,000 patients in our outcomes database, we are on track to help our patients have less pain, improved function, and the ability to manage their pain if it recurs.”

Question: I am 34 and recently diagnosed with breast cancer. My doctor said the treatments may affect my fertility. I am concerned because I have always wanted to have children. Is egg freezing an option to help preserve my fertility?

Answer:

Given the great strides in treating cancer, women facing treatments that may affect fertility, such as chemotherapy and radiation, now have options. It is important to choose a fertility preservation program that will work closely with your cancer treatment team to determine the safest approach for you. As you mention, egg freezing, or oocyte cryopreservation is one option. Other options include embryo cryopreservation and ovarian tissue cryopreservation. Embryo cryopreservation is considered standard care as it is the option that has been around the longest. Thousands of healthy children have been born worldwide from frozen embryos. This can be a great option for women who have a partner who can provide sperm for fertilization of the oocytes prior to cryopreservation.

For women without a sperm source or who have objections to embryo freezing, oocyte freezing is an alternative. Because it is a newer technique, oocyte cryopreservation is considered experimental. However, Washington University has an Institutional Review Board-approved protocol for oocyte cryopreservation. A number of women have chosen this option and it is proven to work. Both embryo and oocyte cryopreservation include ovarian stimulation that require a woman to defer her cancer treatment for at least a week. For women with estrogensensitive cancers, specialized ovarian stimulation protocols keep circulating estrogen levels low. For women who do not have time to defer cancer treatment for ovarian stimulation or who cannot tolerate ovarian stimulation, there is ovarian tissue cryopreservation. Ovarian tissue cryopreservation is also considered experimental and data is limited, but Washington University has a protocol for this procedure that has been approved by our Institutional Review Board. Continuous advancements in fertility preservation are providing more options and helping young women with cancer work past their diagnosis and toward survival.

go off, everyone looks up, mesmerized,” says Susan Harvey, a pyrotechnician with J&M Displays. “It gives me chills to hear the crowd’s reaction and applause.” Harvey’s favorite moment of a fireworks show is after she pushes the final button to start the display and she’s able to wander among the crowd and see the results of her handiwork. This year, she will be part of the team that runs the huge fireworks displays at Fair Saint Louis on July 4, 6 and 7. “I’ve been shooting off fireworks for as long as I can remember,” Harvey says. She’s seen the industry grow, with shells releasing more complicated fireworks in a wider array of colors every year. Although safety has improved over the years, it’s still dangerous work. “We have to be careful of what is nearest to the mortar tubes when the show starts. The audience has to be a certain distance away, and we make sure our technicians are protected. The thing with fireworks is, no matter how careful you are, you can never be 100 percent sure it will go as planned.” Each night of Fair Saint Louis fireworks is a little different, says Bob Younger, chairman of the fireworks committee for the fair. The first night will feature more patriotic music, while on July 6 and 7, the score will be related to the performers earlier in the night. “I’m

most looking forward to the show on the Fourth, since it will have some of the biggest shells,” he says. Younger has visited Fair Saint Louis for more than 25 years, and now loves bringing his children to see the show. “When they were little, the booms scared them—but now that they’re used to the noise, they love fireworks as much as I do.” In addition to the downtown spectacle, Harvey also has coordinated other shows all around the metro area, including at Chesterfield, Innsbrook, Kirkwood and others. “For a huge show like the one at the riverfront, you design the display for an audience that’s looking from both the east and the west, and over a long length,” Harvey explains. “We do our best to design something that’s beautiful from many vantage points. For a smaller show like the one in Kirkwood, the audience is primarily in one direction, and we have to balance a good visual versus the restrictions of the space.” Murray Pounds, director of Kirkwood parks and recreation, says they can shoot shells as large as 6 inches. The show on July 4 will be set to a patriotic musical score developed in collaboration between J&M and the Kirkwood staff. “The visual elements will match the mood of the music,” Pounds says. “When there’s a big crescendo, the biggest explosions will go off!”

Longtime Harvest chef Nicholas Miller purchased the restaurant nearly two years ago from founder Steve Gontram. Harvest was among the first places here to create exciting meals focused heavily on locally grown, seasonal foods. Its reputation has always been strong, and that continues with the change in ownership. A recent meal confirmed that this place still has the whole package: great food, professional service and warm atmosphere—plus a big dose of environmental awareness (wind power, donated cooking oil). The dishes are anchored in Continental cuisine, but, odd as it may sound, many of them include Mexican touches. Like the Pork Trio served with red beans and rice and the chicken breast that combines French and Mexican with sides of tomato confit and chimichurri. Yet I found the unexpected pairings excellent and interesting. The menu is small, with about eight starters and

the same number of entrees. Our three appetizers illustrate the culinary range; we went from Foie Gras ($15), to Fettucini Carbonara ($11) to Tortilla Soup ($8). The pasta, with delicate house-made noodles, used cured and smoked chicken thigh in lieu of bacon. The meat was flavorful and smoky, the noodles delicious. Fresh fava beans dotted the plate, and a poached egg yolk sat on top of it all, an authentic touch, as eggs are often served over pasta in Italy. The soup, while addictively delicious, was oversalted, a problem we ran into more than once. It was thick, with plenty of whole black beans, and garnished with crunchy tortilla strips. The foie gras had good flavor and an interesting sauce of sweet strawberry-white balsamic puree. Entrees of Market Steak Poutine ($23), Mustard Pecan Crusted Trout ($25) and Berkshire Pork Trio ($26) were wonderful, if not uniformly perfect. The

pork plate included slices of tenderloin and pork belly, and a piece of porchetta (slow-roasted pork), with each being tender and flavorful. A mound of shredded, sauteed collard greens was the side, along with those deliciously buttered red beans and rice. The trout was outstanding, served skin-side up, with the mustard and pecans so well balanced that together they offered a third, more exciting flavor that was neither pecan-y nor mustard-y. And the skin added a little bit of juicy fat, like when you eat crisped chicken skin. The side of braised radishes was original and delicious. Harvest Market Steak Poutine ($23) was clearly made with superior beef—too bad it came to the table warm instead of hot. Pineapple Upside Down Cake was a small round tarte, deliciously sweet thanks to a pineapple ring on top of sponge cake. It had a sugary, buttery coating and caramel ice cream.

what people who are very different can teach each other. A destitute Sengalese man and a wealthy invalid in Paris strike up an unlikely friendship. We watch as the two, both broken in some way, help each other heal. It is funny, sad and uplifting, but not in a sappy way. It is the second most successful movie (in terms of viewers) in the history of French film, and it won best film awards at the 2011 Tokyo International Film Festival. St. Louis theaters fill with the sound of music in the summertime.

First among equals, of course, is the oldest and biggest, The Muny. It just concluded its season opener, Thoroughly Modern Millie, and interestingly, The Muny’s new executive producer, Mike Isaacson was one of the producers of the show’s original Broadway run. Now playing is the Kander and Ebb hit Chicago, which has some of the most infectious and ingenious show tunes ever written. After that, Aladdin, adapted from the Disney animated feature that won an Oscar for its score by Alan Menken and Howard Ashman, takes center stage. The Muny’s show is only the third production of this new version, one that adds Menken and Ashman songs dropped from the film. St. Louis native and Muny favorite Ken Page returns as the Sultan. (Don’t be surprised if costumed, miniature versions of Aladdin and Princess Jasmine show up in the audience.) The Muny continues its string of first-time-at-The-Muny productions with Dreamgirls, the Tony- and Oscar-winning musical about a Supremes-like girl group. Though a first time for The Muny, it’s not a first for Jennifer Holliday. She’ll be reprising her role in the original Broadway production that made her a star and won her a Tony. Some have complained that The Muny keeps repeating the same shows. I don’t know if we have new man Isaacson to thank for the change, but we’ll see another first this year: Pirates! (or, Gilbert & Sullivan Plunder’d). The Muny explains that it’s a new version of The Pirates of Penzance, and claims that “the spirit and songs of the original have been revamped within a rousing and riotous Caribbean setting.” I wasn’t aware that Gilbert and Sullivan needed to be revamped, but I guess Caribbean pirates are hot at the box office these days. Then we’ve got two oft-seen favorites. Joseph and the Amazing Technicolor Dreamcoat, Andrew Lloyd Webber and Tim Rice’s first (and, to my mind, best) collaboration. And this time, Joseph will be played by Justin Guarini, runner-up on the first American Idol. Closing the season will be Rodgers and Hammerstein’s The King and I. St. Louis audiences love the classics, and the Muny responds to that love. If you’d like your theater filled not only with the sound of music, but also with cool air, head to the Kirkwood Civic Center and Stages St. Louis. There you’ll find The Sound of Music, the last Rodgers and Hammerstein musical, opening July 20. Until then, through July 1, you can enjoy Ain’t Misbehavin’, a revue of songs either written by or just popularized by Thomas ‘Fats’ Waller. I always find this show a joy and a delight, and at Stages it fits beautifully into their intimate theater. Stages has its show for young people also running through July 1. Like The Muny’s Aladdin, it’s a live version of a Disney animated feature, The Jungle Book. Because Stages’ family shows always draw large crowds, this production is in the new, larger Skip Viragh Center for the Arts at Chaminade. Stages keeps going into the fall, closing its season with a production of the Gershwin musical My One and Only that opens in Kirkwood Sept. 7. Several community groups also offer musicals on weekends, so it’s a good bet you can always find the sound of music somewhere in St. Louis this summer.

Lately, I have really been enjoying cider. As a beverage, it often gets overlooked. On a hot day, when I don’t feel like drinking wine or beer, cider just hits the spot. It comes in varying levels of sweetness. My ‘go-to’ lately has been Crispin Extra Dry.

Perusing the wine aisle at Schnucks the other day, it occurred to me that spending just a few more dollars can make a huge difference when it comes to similar wines. The best example is Chianti. Chianti is a larger region within Tuscany that produces sangiovese-based blends. Chianti Classico is the center of the larger Chianti region with arguably better soil yielding more complex wine. The Classico version is usually just a couple or few dollars more for a marked increase in quality. Riserva refers to oak aging. Which juice do they put into oak? Usually, they choose the best juice, which then becomes even more complex because of the flavors introduced by the oak. Riserva is again usually just a few dollars more. For an extra $5, the Chianti Classico Riserva is a far superior bottle to the Chianti.

The color of a wine relies primarily on the skins of the grape. Although oak treatment, age of a wine and acidity levels in the soil can contribute, the base color relies on the skins of the grape. Almost all grape juice is clear. If a winemaker immediately removes the juice from the skins the wine is a white. If a winemaker lets the skins soak for just a short period of time, it’s a rose. If the skins macerate for a longer period of time, the wine is a red. With red wines, the concentration of color depends upon the thickness of the grape skins. For example, pinot noir has very thin skins, so even after macerating the juice with the skins, the color can only reach a certain concentration level, as there is only so much pigmentation in those thin skins. Syrah, on the other hand, has very thick skins. the owner of the 3-acre property near the intersection of Clayton and Warson roads. The owners of 4 Tall Timbers Drive say the dream home would be perfect for another horse-loving family. They bought the property after a friend who was boarding their horses moved. They searched for new stables, but what they found instead was this picture-perfect new construction home in Ladue with enough space for two horses and access to the popular Ladue bridle trail—the best of both worlds for the longtime horse rider and his budding equestrian children. “We’ve even been known to ride the horses up the street to Sportsman’s for a beer and socializing,” he says. The owners and their four children are the first residents of this 5,000-square-foot house, built in 2007. The sprawling one and a half story home includes a first floor master suite and three upstairs bedrooms, as well as a 2,500-square-foot basement. “We spend the most time in the kitchen and hearth room. The island seats up to eight, which is great for quick food prep and clean up!” he says. The kitchen is equipped with a SubZero fridge and Viking six-burner range, three ovens, hood and microwave. There’s also a secret pantry: hidden behind normal cabinet doors is a 8-square-foot walk-in closet. “I’m the foodie of the family, and I love this kitchen. We almost always host our family get-togethers, too, which is one of the reasons we have two dishwashers!” Dual laundry rooms, one upstairs for the kids and another off the mud room by the back door, make family life here very convenient. The first floor offers views of the horses’ pasture from every room, including the master suite. In addition to the bedroom and luxurious bath, a sitting room large enough to be a nursery is attached, as well as his-and-hers closets. The master bath includes a multi-head shower and oversized whirlpool tub that sometimes doubles as an indoor waterpark for the children, all yonger than 10. There is also access to the patio, where the family can sit around an outdoor fireplace and enjoy the view. The horses love being so close to the family, too. “Sometimes, if I don’t get up early enough (in their opinion) to feed them, I’ll look up and they’re at the kitchen window,” the owner says. “And on Christmas morning, they peek in to see what Santa brought.”

The house needs a new roof and the Bald Cypress in front is dead. I’m sorry the Bald Cypress is dead, but I wish everything else in the front yard was. Please get rid of everything except the holly on the left. I’d like to add a long row of flowers in front of low-growing grasses. Many of the existing boxwoods could be reused to create a hedge to hide parked cars on the right. I’d like to recommend new, wider-paneled shutters with hinges and tie-backs. I would also add window boxes under several of the windows. The objective in doing this is to make the windows appear larger and add an easily installed layer of detail, interest and shadow to a rather dull façade. Next, I would install a new reddish-brown roof and paint the clapboard a warm beige to bring the major façade elements together in color. Minor details like new front doors and larger pier lanterns set directly onto the brick piers will help the house look updated. Getting rid of the odd little bay window roofs would remove an embarrassing detail that makes the bays seem like an afterthought. Good luck with your resale, ———Homework
