Fusion Surgeries May Not Alleviate Pain Recent Study Indicates that Surgery Can Leave Patients In More Pain “The study 1 provides clear evidence that for many patients, fusion surgeries designed to alleviate pain from degenerating discs don’t work,” says the study’s lead author Dr. Trang Nguyen, a researcher at the University of Cincinnati College of Medicine. Unfortunately, for most patients with bad backs, there is no easy solution, no magic bullet. Pain management experts — and some surgeons — say that patients need to scale back their expectations. With the right treatments, pain can be eased, but a complete cure is unlikely. These conclusions are coming because of a very recent study published in the medical journal Spine, and it clearly shows that in many cases surgery can even backfire, leaving patients in more pain. Researchers reviewed records from 1,450 patients in the Ohio Bureau of Workers’ Compensation database
who had diagnoses of disc degeneration, disc herniation or radiculopathy, a nerve condition that causes tingling and weakness of the limbs. Half of the patients had surgery to fuse two or more vertebrae in hopes of curing low back pain. The other half had no surgery, even though they had comparable diagnoses. After two years, just 26 percent of those who had surgery returned to work. That’s compared to 67 percent of patients who didn’t have surgery. The Real Cause of Back and Neck Pain The causes of back and neck pain can be narrowed to four major categories. The most common are herniated or bulging discs and degenerative disc disease. Herniated and bulging discs are located between the vertebrae (bones of the spine) and act as shock absorbers for the spine. The outer disc wall known as the annulus fibrosis surrounds a jelly-like center known as the nucleus pulposus. Through years of wear and tear, constant vibration or pounding, a slip and fall, coughing or sneezing, lifting and twisting or simply bending to pick up a piece a paper can cause a weakening of the disc wall and cause a bulge or tear and press on surrounding nerves. This is a condition in which part or all of the soft, gelatinous central portion, nucleus pulposus, of an Degenerative Disc intervertebral disc is forced through a weakened part, (annulus fibrosis), of the disc, resulting in back pain and nerve root irritation. Degenerative disc disease is degeneration of the discs and over time produces lowgrade inflammation and irritation and is a major cause of chronic low back pain. Because the discs in the spine do not have a dedicated blood supply, the discs must rely on a process called diffusion to receive their supply of water, nutrients, and oxygen. If the flow of these elements is disrupted, the intervertebral discs can degenerate. This is a state of dehydration.
stenosis, and it can also occur in young people who have had a spinal injury or other abnormal narrowing of the spinal canal. Spinal stenosis is also caused by scar tissue from a previous back or neck surgery. It most often occurs in the lumbar spine, but can also occur in the cervical and thoracic spine. When nerves in the lower back are compressed, they become inflamed and cause pain in the skin, buttocks, and legs. Facet Syndrome is when the very small amount of tissue Degenerative discs become more susceptible to injury from physical stress, day-to-day activities which can play a contributing role to serious conditions such as disc herniation, osteoarthritis and spinal stenosis. Sciatica is related to herniated or degenerative discs. This refers to pain along the length of the sciatic nerve, the largest nerve in the body. The sciatic nerve branches off the spine at the pelvis and travels down each leg. A problem at the root of this nerve, where the nerve exits the spine, may be felt along the entire length of the leg and can cause leg weakness. Sciatic pain can be severe and debilitating. Patients often misinterpret this as a problem with their legs, when the primary cause is the result of a bulging disc compressing a nerve at the base of the spine. Sometimes, with severe inflammation, pain will also be felt in the hips and buttocks.
Other Common Conditions are Spinal Stenosis and Facet Syndrome:
People over the age of 50 may experience spinal stenosis, which can be a part of the natural aging process. The spine changes with age and degenerates over time, which can—and will—cause pain. Diseases such as arthritis and scoliosis can cause spinal
between facet joints, the articular cartilage of the joint capsule, becomes irritated. Facet joints work as a pair in the back of each vertebra. They link the vertebrae above and below, allowing movement of the spine, and are the areas where the vertebrae move and actually touch one another. It can cause the same symptoms that are found with any other pinched nerve: pain, numbness, tingling, and a burning sensation in the lumbar spine. Pain will often show up just in the low back or the buttocks, and this might be referred to as facet syndrome. Facet syndrome in the neck might cause headaches or shoulder pain.

Injections, Continued Use of Pain Medications and Anti-inflammatories Are Not the Solution! Pain is your body telling you that something is wrong! Pain medications and other medications and treatments such as epidurals may provide only temporary relief, and studies show some injections may be no more effective than saline injections. The concern about the continued use of pain medications, anti-inflammatories and injections is that these simply mask the pain. Without pain the patient may continue to perform certain activities or movements that could cause damage to the injured area. The injured area could experience significant degeneration from repetitive injury and from the continued use of steroids. According to one school of thought, if you continue to exercise a joint that is not moving correctly, it may accelerate degeneration of the injured joint by up to 30%. Prolonged use of pain medications requires stronger doses over time and increases the risk of dependency and addiction. Prescriptions and over-the-counter pain relievers such as aspirin, or Ibuprofen can result in liver, kidney and gastrointestinal damage, fatigue, depression and other severe side-effects.

Because of back pain complaints there are many tests and treatments - some are painful- which include injections, surgery, fusion implants, and addictive medications. Yet, when seriously looking over all of the information, data, and statistics it is apparent that testing and treatments have sky-rocketed compared to the actual number of cases of back pain which proportionately have not increased. What could be driving the increase? Now clinical science is questioning the safety and effectiveness of some common
treatments for back pain which would include surgeries, especially fusions, injections, and drugs (opioids). Injections are often performed for treatment of back pain, however they offer only temporary relief, if any, and injections do not reduce the likelihood of surgery being recommended as the next step. Studies show that facet joint injections seem to be no more effective than saline injections. Pain medications are increasingly prescribed for chronic back pain and one category of chronic pain is from failed back surgery (FBS). Then the pain and prescriptions can continue for a lifetime. There are many questions about
the safety of the long-term use of pain medications. Spinal fusions can be very necessary for treatment of certain spine conditions such as vetebrae fractures, however the benefit is more under question for other conditions such as degenerative discs. Yet, even with the limited benefits the rate of fusions has accelerated. A recent major study1 concludes that there is a substantial increase in complex spinal surgeries (fusions) for no known reason. One author of the study, Dr. Deyo, has stated an opinion that this may be directly related to marketing by orthopedic manufacturers to surgeons. Surgery costs have sky-rocketed in the past decade and now passes the amount of money spent on cancer treatments. Get Out of Pain Fast!

ou may be thinking, “What is going to HAPPEN to me if I can’t find a solution to my back or neck pain?” The answer to your question starts out hopeful. I’ll just go to my doctor, and he will take care of me. He will know what to do. Then something very odd happens to you. You find out your primary care physician is not really well equipped to handle that job. He or she is very busy treating infections, diabetes, heart conditions and other health issues.
