Vehicle Registration Programs are designed for specific age groups as listed in the program description. Thus, required proof of household residency and age for all children is established by presenting the birth certificate.

REGISTRATION FAQ

•Resident fees apply to anyone living within Park District boundaries. Additional fees apply to individuals not residing within Park District boundaries for memberships, activities and special events.

•Registration is not accepted at classes. Only registered participants are allowed in the class—no substitutions.

•Individuals may only register themselves and individuals listed in their household account.

•When a class fills, a waiting list is formed. Every attempt is made to accommodate individuals on a wait list.

•Bring your Park District receipt to any program, event or trip to verify your enrollment.

•Mail-in payments accepted by check only. Make checks payable to: Tinley Park-Park District.

•The Park District reserves the right to cancel or postpone activities. See Cancellation Policy below.

•For programs in school locations, school activities have priority; this can cause cancellation of Park District programs.

•Complete both sides of the registration form located on page 77/78.

•Be sure to include program number for each program.

•The signature of patron or parent/guardian is required.

Cancellation Policy

The Park District reserves the right to cancel any program due to insufficient enrollment and issue refunds. The Park District reserves the right to change, adapt or reschedule any program variables, including instructors, dates, days, times, locations, etc.

Refund Policy Refunds must be requested prior to the second class of the session.

Refunds for one-day seminars, trips or special events must be requested at least one week prior to date of event. Refund amounts less than $10 are credited to the patron’s account and can be used for future registration purposes; a request for your full refund can be made at any time.

Behavior Policy

Tinley Park-Park District has an established behavior policy relative to inappropriate behavior.

Inappropriate behavior is defined as that which disrupts a recreation program/activity and which requires a class instructor/staff member to call for a stop of the behavior due to its effect on other participants or on the instructor's ability to conduct the program. Such behavior may call for immediate and/or permanent dismissal of the individual from the program. Additional rules may be developed for individual programs/ activities as deemed necessary by the staff.

Photos And Videos

Participants or their parent/guardian (if participant is under 18) permit the taking of photos, audio and video tapes during Park District activities for publication and use as the Park District deems necessary.

Volunteers

Volunteers coach youth athletes, assist recreation staff in preparing for events and provide support at numerous activities and events. In whatever capacity, volunteers add a special touch to the Park District. We appreciate you all beyond measure! Go to tinleyparkdistrict.org to complete a volunteer application.

The Tony Bettenhausen Recreation Center offers Park District residents a great opportunity to run, walk, play basketball or volleyball indoors. Childcare is available for participants in a Rec Center activity or those needing supervision for their child while they use track. This is the perfect place to get some exercise, have fun with friends, reduce stress or just take time out for you! Free wi-fi while in the building. Annual memberships are available. White Water Canyon Water Park is 5 acres of splashing, sliding water fun for the whole White Water Canyon family. Features include a zero-depth entry main pool, interactive kid’s play area, three slides, a three meter drop slide, diving board, lazy river, children’s Spray Ground and a Water Park giant sand play area. The full-service concession stand is perfect for quenching your thirst 8221 W. 171st Street or curbing your hunger after a day of swimming. Season passes go on sale March 1, 2021.

Canine Campus Dog Park

Canine Campus is a 10 acre off-leash dog park featuring separate fenced-in areas for small dogs (under 26 lb.) and large dogs. The affordable membership allows your dog to socialize and run free safe from street hazards. Vogt Visual Arts Center, once home to Tinley Park’s first mayor, Henry Vogt, opened as the VVAC in 1996 housing all of the Park District’s fine arts classes and an art gallery. The art gallery features monthly exhibits of works by various local and well-known artists. See page 39 for upcoming gallery schedule. Classes can be found on pages 35-38.

COVID-19 Information and Updates

Facility Guidelines

As Tinley Park-Park District prepared this brochure, the unprecedented restrictions caused by the COVID-19 pandemic affected program and event planning.

Face Covering Face coverings must be worn properly when entering and exiting the building, and in all common areas of our facilities including lobby, hallways and washrooms. Face covering must cover the nose and mouth completely. Once in the program or exercise area, check with staff/instructor for the latest requirement regarding face coverings. These rules are subject to change.

Decisions were made using the recommendations of the State of Illinois, the Illinois Department of Public Health and the Centers for Disease Control in place at that time.

To limit attendance, many special events now require advance registration, have multiple sessions and/or staggered start times. Class enrollment limits are reduced to stay within limitations in effect at the writing of this brochure.

The Park District will continue to adapt activities and events to adhere to the most current recommendations and guidelines. Providing a safe and healthy environment for all patrons and staff to enjoy our programs and events remains our priority. All changes and updates will be posted to our website and social media channels.

Virtual Programs When possible, a virtual format of classes will be offered.

Please look for the symbol above to determine if a virtual option is available. Additional virtual classes can be found in our Virtual Activities Brochure on our website. Look for the virtual program icon for classes that are either fully virtual, or will move to virtual if necessary.

Good Hygiene Frequent hand washing requirements include at the beginning and end of programs, and using the available hand sanitizer throughout the facilities.

Increased Cleaning and Sanitizing Since the start of the pandemic, Tinley Park-Park District has increased the frequency of cleaning and sanitizing our exercise equipment, facilities, program rooms and event locations.

Supplies Patrons should bring their own water bottles, towels, exercise mats, etc.

Chicago Blackhawks Clinic

Join us at Lewis Park for a one-day Ball Hockey Clinic! The camp provides a fun, skill-oriented experience for local youth hockey players and features the Blackhawks Fan Development Team with an inflatable rink. The clinic includes scrimmages with various age groups. Equipment supplied by the Park District.

Celebrate someone special in your life with a front yard birthday or graduation décor package! This is the perfect way to bring a smile to their face! After registering, you must email Erin Cortilet at erin.cortilet@tinleyparkdistrict.org to give the personalization for the sign (name) and arrange the party package and sign drop off. Orders must be made one week in advance. If you are ordering this for someone outside of your house, we require you to let them know we will be dropping off the party package. Party like it is 1776 - Tie Dye Style! The kit features red and blue dye with color setting ingredients already added. Just add water to the squeeze bottles and you are ready to create. Our four-legged friends can splash in any of our poochie pools. Other activities include a summer photo area, pet bandana decorating and more. Great Backyard Camping Kit

Our backyard camping kits are perfect for those family adventures and memory making moments just waiting to happen. Plan a camp out or late night in your own backyard (or purchase this kit filled with activities for the little ones to do at our Family Camp Out!).

Each kit includes crafts, glow sticks, items to explore your natural world and more in a reusable camp bag. Kit is priced per child and geared toward kids ages 3 and older. Pick-up available July 19-23 at the Bettenhausen Rec Center during open hours. These are great for vacation adventures too!

Family Camp Out

Bring your tent and sleeping bags for a fun filled night under the stars. Planned activities include a late night movie on our giant blimp screen, campfire complete with s’mores, camp crafts, family game area, star gazing and more! All campers enjoy a continental breakfast in the morning. Upon check-in you will be able to set-up your tent in the designated tent area. Indoor bathrooms will be utilized for this event. Camp Out rules will be sent to you before the event. Participants over 2 years of age must be included in the family size. Space is limited, so register early.

Drive-in Movie: Onward

NEW!

Join us for our first drive-in movie, Onward! Onward is the story of teenage elf brothers on a magical quest to spend one more day with their late father. Like all adventures, the journey is filled with cryptic maps, impossible obstacles and amazing discoveries.

When dear Mom finds her sons are missing, she teams up with the legendary manticore to bring them home. Social distancing and masks are required if you leave your vehicle for any reason. Gate opens at 7pm; movie starts at dusk (apprx 7:45pm).

5K

First, second and third place male and female runners in each age category win awards, based on chip timing. An award is also presented to the best overall male and female resident and non-resident runner. The first 550 registered participants receive unisex tech shirts. For the safety of our runners, spectators and staff, groups of 50 racers will start at staggered times, every 15 minutes beginning at 8:00 am.

Pre-registration closes Jul 1 at 3pm. Registration reopens at 7am on race day.

Packet pick-up is on Jul 3 from 8am to 2pm at the Recreation Center.

Starfish Swim School—Group Swim Lessons

Our Starfish Aquatics and Starfish Swim School program are unlike anything offered by other facilities. The program is designed for children 6 years of age and over. Students’ progress through the levels is at their own pace, mastering the skills in one level before advancing to the next. Each level of our Swim School class includes basic water safety and helping others in an emergency, in addition advancing to our Stroke School (Levels 5 & 6). During the first day of class, instructors assess abilities and place children in the appropriate group. Your child will focus on swim progressions and techniques that are essential for water safety. Beginning Junior Life Guarding Skills

This class is geared towards young teens interested in becoming a lifeguard. In this class, we introduce the skills taught in the lifeguard training program including CPR, First Aid, how to identify potential hazards in the aquatic environment and aquatic safety issues. On July 9, beginner junior lifeguards have a chance to shadow Park District lifeguards to gain a better understanding of the diligence and commitment required to be a successful lifeguard. Bring a towel and lock to store your things in the locker room. Fee includes a Junior Life Guard T-shirt.

Advanced Junior Life Guarding Skills

In the second level of Junior Life Guarding students review skills acquired in the beginner course and work towards refining and using these skills in simulated water rescues. Participants work on completing swim requirements needed to take any certified Lifeguard course.

Junior lifeguards again have the opportunity to shadow Park District Lifeguards. All participants must have completed the beginner program to be enrolled in this course.

Playground Splash

Spend time with your toddler on our zero depth playground filled with an enclosed slide, 2 racing slides, water cannons and water shower poles. At the sound of the bell get ready for a big bucket splash. The park is closed for morning programs during this time and the playground is only open to participants of Playground Splash. This is a self-directed play time for parent (or other responsible adult over the age of 18) and child. The creative play is up to you and your child. A lifeguard monitors this area. River Running

Walk or jog against the current in our 600 foot lazy river. This self-directed class is held when the Water Park is closed to recreational swimmers. This class is meant for exercise only; using the river current for resistance provides an aerobic and muscular workout. You'll find out that there is nothing lazy about this river! You may arrive and use the river any time during the one hour time frame. A lifeguard is posted for your safety. Water shoes are required; the river bottom has a rough surface. A drop-in fee of $5 per class is available depending on class size. Register early to guarantee your spot!

Tot Time Preschool

TOT TIME PRESCHOOL

This program is continuous beginning in September and ending in May. A child must be 3 or 4 years old on or before September 1, 2021.

All children must be toilet trained and able to take care of their own bathroom needs. There are three class options available: Traditional Preschool: The Tot Time Preschool Program includes a variety of developmentally appropriate activities including but not limited to: creative art experiences, singing and rhythm activities, stories, finger plays, role playing and body movement, free play, emerging writing and reading skills and activities related to everyday math and science discovery. *Kinder-Prep offers five days of learning to provide additional time and emphasis for writing, pre-reading, math and science.

STEAM Ahead in School: The Steam school day primarily engages students in developmentally appropriate hands-on activities with Science, Technology, Engineering, Art and Math exploration.

Forest School: Whatever the weather, students engage in interest-led play while they learn, explore and create in outdoor and indoor settings.

Emphasis is placed on the natural world through play, science, art, dramatic play and sensory experiences.

Sunrise to Sunset Camp

Supervised playtime is available at the McCarthy Park campsite! This campsite is for those who need extended hours before or after regular camp time. A daily snack is provided for the after-camp participants. To qualify, your child must be enrolled in regular Day Camp and Sunrise or Sunset Camp at McCarthy Park.

Family Discount and Payment Plans

A discount rate is offered for Day Camp, Tot Camps Firefly and Little Sprouts, Teen Camp, Sunrise and Sunset Camp if you register for both sessions at the same time (discount does not apply if you register for each session separately) if you have more than one child attending camp. Payment plan forms must be completed at the time of registration. A $10 nonrefundable service fee is charged for payment plans. Half of the entire fee is due at registration; payment in full is due June 1 for Session A or C, and July 1 for Session B.

Camp orientation has gone virtual! Meet the camp staff and have your questions answered! We will review camp policies and procedures, discuss drop-off/pick-up procedures and activities. A variety of sports are introduced, including floor hockey, soccer, basketball, volleyball and more. We also are playing smaller, team-style sports such as dodgeball, kickball, capture the flag and other school yard style sports! Acoustic Guitar

Private acoustic guitar lessons for all levels are offered through the Tinley Park-Park District. Each session includes eight, 30-minute private lessons. The instructor, Steve Lawler, has played for over 30 years and is certified by the Suzuki Association of the Americas. He has taught guitar using both Suzuki and traditional methods at local fine arts schools since 2002. Each student must have either a nylon or steel string guitar for class and for practice at home. Students are asked to purchase a music book (and a CD if using the Suzuki method) from a local or online music store.

Adult Tap Dancing

Get those toes tapping for some fun and good old fashioned exercise! Adult tap class is for the beginner and for those who have danced before. Everyone learns brushes to time steps. In our ten-week summer dance program, we offer ballet, tap and jazz classes for students ages 3-12. Age requirement must be met by June 1, 2021. The instructor has the right to move children at her discretion. All students attend class in the proper uniform: girls should wear a plain black leotard made of lycra, cotton or nylon (no velvet material, skirts or prints on the leotard), white tights and white ballet shoes and/or black tap/jazz shoes; boys should wear a black t-shirt and shorts. The Community Theater program at the Tinley Park-Park District is a long standing program that welcomes seasoned and novice actors on stage each Spring and Fall. If you are new to the stage, let us help you develop your talents and the skills you need for the best of classic and contemporary theater. Auditions consist of a short interview and a reading of a brief monologue from the play. Please bring a list of your schedule conflicts as well as your clothing sizes to the audition. Theater Directing 101

Have you ever thought of learning the art of directing plays? Here’s your opportunity to begin Theatrical Director training. Build on your acting experiences, talents and skills in this concentrated program that expands your journey into this creative work. In addition to the weekly class, students act as Assistant Director for one of the future Covered Bridge Playhouse Theater (CBPT) productions, working with the Director during auditions, pre-production, rehearsals through to performances. Ladies Night Out - Wine Glass Painting

We are offering an open painting class to include oils, watercolors or acrylics where students may explore or pursue their areas of interest. Direct instruction and demos are included as needed for all students. Beginners and experienced artists are welcome to learn with us and have fun. This is a great class for beginners to experiment and learn different mediums, and experienced artists to transition to or incorporate new mediums into their work.

The Vogt Visual Arts Center (VVAC) is a facility of the Tinley ParkPark District. The VVAC was once home to the first Tinley Park Mayor, Henry Vogt. In 1996, it reopened as the VVAC, which hosts all of the park district’s fine arts classes and art gallery. The art Vogt Visual Arts Center gallery features a new exhibit each month with works from various local and well-known artists.

Field Rentals and Conditions

Several athletic fields throughout the Tinley Park-Park District are available for rentals. All field rental requests and paperwork must be submitted to the Athletic Supervisor, Ron Woodworth. If you have questions, please contact Ron at (708) 342-4208. Field conditions can also be found on our website at www.tinleyparkdistrict.org.

Looking to give your team a little more practice or just want to have some fun? Our batting cage offers opportunities for all ages.

Slow pitch to fast pitch baseball. Participants can even choose to forgo the pitching machine and give their arm a work out on the pitching mound. Rentals include enough space in your own private room to have team meetings, go over fielding assignments or even run small drills while others are working on their swing or pitching in the cage. Rental reservation requests require a minimum of 4 days advance notice. If you would like to request a rental, you must contact the Athletic Supervisor, Ron Woodworth, at (708) 342-4208. Rental requests are handled on a first comefirst served basis.

Coaching your child’s team offers the opportunity to directly teach the value of good sportsmanship, to learn about a sport along with your child and strengthen lifelong bonds with your child. We understand families are busy, so pair up with another parent to share coaching responsibilities! Not only does your child make new friends, but you do too! Fill out a Volunteer Coaching application on our website or at the Rec Center.

We offer voluntary certification courses making a head coach eligible for registration discounts.

Participants are only allowed to make one request per league. We try to fulfill a reasonable number of friendship requests, but none are guaranteed. A limited number of requests are granted in order to keep all teams evenly balanced. Siblings with the same last name and address are placed on the same team unless otherwise noted. Requests for specific coaches are not honored. Requests made after the registration deadline cannot be honored. The league is played in a community rink where youth can participate in an outdoor activity and enjoy the sport of ball hockey in a league setting. This camp gets kids excited about exercise! Campers learn fundamentals for improving flexibility, stretching, body positioning, developing core-strength, improving balance and other fitness concepts related to exercise and the jumping, running and throwing skills used in track and field. QuickStart Tennis is for children who are new to the game or who need additional instruction to rally using groundstrokes, volleys and serves. QuickStart Tennis takes a new approach to introducing kids to the game by using fun sized courts, nets, racquets and balls. The lighter low compression balls bounce lower, which helps develop proper techniques.

Olympic Taekwondo

MARTIAL ARTS

Students have the opportunity to improve concentration and self-esteem, achieve lifelong physical fitness and learn self-defense skills. A uniform fee is required at the first class. KH Kim Taekwondo offers belt testing twice a year at an additional fee. All colored belt students are required to have complete sparring gear.

Little Dragons

The Little Dragons class captivates the interest of our youngest students. Skills are developed through gentle instruction and appropriate games. This class focuses on developing balance, coordination and respect for the discipline of martial arts training. A uniform fee of $45 is required at the first class.

Early Taekwondo

This program helps kids develop conditioning, coordination, listening skills and self-confidence through creative activities.

ISKC’s youth karate classes put children in touch with their bodies and offer a good mix of strength training, cardiovascular and flexibility exercises. Children increase coordination, agility and poise, as well as learn lasting personal safety skills. They also benefit from the mental stimulation of learning the choreographed moves of the kata and the discipline of conforming to the class rules. Adult classes release tension, develop total body fitness, build stamina and increase overall productivity! In addition to self-defense skills, karate increases flexibility, cardiovascular endurance, muscle tone and improves core strength.

The Pit Stop–Teen Center

Looking for something to do, a place to hang out or meet new people? Drop in at the Bettenhausen Recreation Center’s “Pit Stop” Teen Center and see everything we have to offer! The Pit Stop is open on Tuesday and Thursday from 4:00-8:00 p.m. and Friday from 4:00-9:00 p.m. The drop-in center is open to teens in grades 6 through 12. Play pool, air hockey, foosball, PlayStation 4, Xbox One, Wii and much more. Check out our exer-gaming area blending fitness and fun in one! Watch for contests, special activity nights and more throughout the year, all included with your Rec Center membership or daily pass. There is so much to do—come in today! Pit Stop Rentals

The Pit Stop is available to rent! Looking for a place to hang out with your friends, throw a birthday party or just relax and have fun? The Pit Stop is the perfect venue! Rentals are available on Saturday and Sunday throughout the year.

Rentals are 2 hours in length. An adult 21 years and older must be present throughout the rental. Equipment includes ping-pong, billiards, Wii, Play Station 4, Xbox One, climbing wall, light wall, foosball and more! Pit Stop supervisors will be on hand to assist with equipment. A maximum of 25 people per rental allowed in the Pit Stop. Pit Stop rentals are for participants 10 years and older. For information and to book a rental, you must contact the Recreation Supervisor at (708) 342-4200. Rentals must be made a minimum of 3 weeks in advance. Availability is first come-first served.

Tacos and Tie Dye

Explore the effects of tie dye in this hands-on workshop as we experiment with a variety of tie dye techniques.

Participants should bring 4-5 pre-washed white colored items to tie dye such as shirts, pillow cases, socks, shorts, etc. In addition, please bring one dark colored shirt (black/navy). Please wear clothing that can get stained as this can be a messy adventure. Enjoy a dinner of walking tacos while we work—yum! Restorative Yoga

Restorative yoga helps to relieve the effects of stress by allowing the central nervous system to rest while the body is supported using props (bolsters, blankets, blocks, etc.). This promotes deep relaxation and a sense of calm that renews mind, body and spirit. Students hold restorative yoga postures for up to 5-8 minutes each, encouraging a release of deep-rooted muscular tension, stress and tightness. Open to students of all levels, no yoga experience necessary. Wear comfortable clothes, bring your yoga mat and blanket.

Senior Yoga

This safe and slower-moving class designed for senior citizens incorporates range of movement, alignment, stretching, awareness, balance, breathing and relaxation.

This class explores short Tai Chi routines organized by Tai Chi and medical experts for health purposes. Tai Chi is recognized for improving posture, balance, flexibility and respiration in addition to many other health benefits.

These easy to learn sets allow you to relax and enjoy the classic movements of Tai Chi drawn from the Yang, Chen and Sun Styles of Tai Chi. The class consists of warm-ups, instruction in a Tai Chi form and cool down movements.

Group Exercise and Pool Schedules

Tinley Fitness members can take a variety of group exercise and aquatic classes included in their membership. Class schedules are available at the front desk and online at www.tinleyparkdistrict.org.

Childcare Center

Childcare is available from 8am-12pm, Monday through Saturday; and from 4-8pm Monday through Friday. Unlimited childcare is $14 per month for the first child and $8 per month per additional child. Daily drop-in fee is $5 per child per day. Please see front desk for Childcare Center availability.

Birthday Parties

Your birthday party will make a splash at Tinley Fitness! You and your guests will have exclusive access to the pool and adjoining party room for two hours. Birthday party rentals are available Saturday and Sunday. Get more information and book your party at the Fitness Center front desk or by calling 708-342-4255.

Wellness Programs

Tinley Fitness members are able to participate in many additional Wellness Programs offered by our Personal Trainers.

South Suburban Special Recreation Association (SSSRA) is a therapeutic recreation program that is an extension of the Tinley Park-Park District. SSSRA is organized to provide individuals with special needs or disabilities the opportunity to be involved in year round recreation programs.

Scholarships are available to SSSRA residents with disabilities who would like to participate in one or more programs, but cannot afford the fees charged.

INFORMATION

Tinley Park-Park District is home to over 40 beautiful park sites. Many of the parks offer pavilions which can be rented for your event.

Park Rental Procedures Park rentals are on a first come-first served basis.

Rentals must be completed in person at the Tony Bettenhausen Recreation Center. Rentals are available from May 1 through September 30 each year. Please note some sites may be unavailable because of normal Park District programming use or park renovations.

First Day of Reservations Third Monday of January Resident April 1 Non-Resident Following first day reservations, rentals may be requested at the Bettenhausen Rec Center in person during normal business hours. Renters must present a valid driver's license or state ID at time of rental.

Renters must be at least 21 years of age. Complete rules for renters and park amenities can be found on our website.

Host your next celebration with us!

The Tony Bettenhausen Recreation Center located at 8125 West 171st Street is the main facility of the Park District. We offer rooms of various sizes to meet your needs. Landmark Museum is a reproduction of the second school house in the Village of Tinley Park and later served as the first Village Hall. The Tinley Park Historical Society focuses on the research and preservation of the history of Tinley Park and its environs. The Historical Society is always looking for artifacts, photographs, letters, deeds, postcards, calendars, old phone books, newspapers, etc., related to the Tinley Park area. Landmark Wedding Chapel & Banquet Hall The Old Zion Lutheran church was built in 1884 and retains its old world charm and beautiful stained glass windows. The nondenominational chapel is air conditioned and can comfortably accommodate 150 guests. The facilities can be rented for weddings, baptisms, memorial services, etc.

Requests made after registration deadline are not honored. Coaches are not allowed to make requests. Coaches’ children are automatically placed on their team.

Waiver and Release

The Tinley Park-Park District is committed to conducting its recreation programs and activities in a safe manner and holds the safety of participants in high regard. The Tinley Park-Park District continually strives to reduce such risks and insists that all participants follow safety rules and instructions that are designed to protect the participants’ safety. However, participants and parents/guardians of minors registering for this program/activity must recognize that there is an inherent risk of injury when choosing to participate in recreational activities/programs. You are solely responsible for determining if you or your minor child/ward are physically fit and/or skilled for the activities contemplated by this agreement. It is always advisable, especially if the participant is pregnant, disabled in any way or recently suffered an illness, injury or impairment, to consult a physician before undertaking any physical activity. Warning of Risk Recreational activities/programs are intended to challenge and engage the physical, mental and emotional resources of each participant. Despite careful and proper preparation, instruction, medical advice, conditioning and equipment, there is still a risk of serious injury when participating in any recreational activity/program. Understandably, not all hazards and dangers can be foreseen. Depending on the particular activity, participants must understand that certain risks, dangers and horseplay, unsportsmanlike conduct, premises defects, inadequate or defective equipment, inadequate supervision, instruction or officiating, and all other circumstances inherent to indoor and outdoor recreational activities/programs exist. In this regard, it must be recognized that it is impossible for the Tinley Park-Park District to guarantee absolute safety. Waiver and Release of All Claims and Assumption of Risk. Please read this form carefully and be aware that in signing up and participating in this program/activity, you will be expressly assuming the risk and legal liability and waiving and releasing all claims for injuries, damages or loss which you or your minor child/ward might sustain as a result of participating in any and all activities connected with and associated with this program/activity (including transportation services/vehicle operation, when provided). I recognize and acknowledge that there are certain risks of physical injury to participants in this program/activity, and I voluntarily agree to assume the full risk of any and all injuries, damages or loss, regardless of severity, that my minor child/ward or I may sustain as a result of said participation. I further agree to waive and relinquish all claims I or my minor child/ward may have (or accrue to me or my child/ward) as a result of participating in this program/activity against the (Tinley Park-Park District), including its officials, agents, volunteers and employees (hereinafter collectively referred as “Tinley Park-Park District”). I do hereby fully release and forever discharge the Tinley Park-Park District from any and all claims for injuries, damages or loss that my minor child/ward or I may have or which may accrue to me or my minor child/ward and arising out of, connected with, or in any way associated with this program/activity. I have read and fully understand the above important information, warning of risk, assumption of risk and waiver and release of all claims. If registering on-line or via fax, your on-line or facsimile signature shall substitute for and have the same legal effect as an original form signature.
