NEWS NEWS Gap in vaccination policy Private in-home caregivers unable to get vaccine

oday home healthcare and in-home support service providers are listed as eligible by the state to get vaccinated in the phase currently being administered in Ventura County.

Due to verification requirements and lack of clarity, however, local officials screening people with appointments may be turning away people who are at risk of exposure.

The state’s website describes who is eligible during Phase 1A and states that under Tier 2, recommendation B, individuals providing “home health care and in-home supportive services” can get vaccinated. But at least two privately employed in-home healthcare providers have been turned away when they showed up for their vaccine appointments in Ventura County.

On Friday, Feb. 5, Joan Dockery of Ojai went to her vaccine appointment at the Ventura County Fairgrounds. She provides private in-home healthcare and support services and registered through the county’s vaccine registration portal online.

“I got to one manager and was turned away,” said Dockery, adding, “Fortunately both my elderly clients have gotten theirs.”

But she pointed out another caregiver “who does the same thing I do was also turned away. So frustrating.”

She is 65 and doesn’t yet qualify due to her age, but she has been providing in-home healthcare and support services for over 40 years. Today she is taking care of two people, one with Alzheimer’s and another with dementia, and works seven days a week.

“I come from a large family, with 15 kids. My mother was a nurse, it’s just something that came kind of naturally.”

She started in this line of work in 1977, and has taken care of clients of all ages. Her two current clients “both love to converse.

I help with personal care, cooking, cleaning — they’re called the activities of daily living.”

She maintains CPR and First Aid certification and years ago was a Certified Nursing Assistant (CNA) and received a Home

Health Accreditation. She does basic nursing, checking blood pressure and testing the blood of diabetics. “There is a whole range of things that we do, whatever is needed to help with their care.”

Since the beginning of the pandemic she has worn a mask the whole time she is with a client and has been “very careful, social distancing, all of the things we are supposed to do. But they don’t wear a mask, it is too difficult for them.”

Dockery, like many in the field, does not work through an agency but is hired directly and privately by the family of the person she cares for.

Dockery could work through an agency, but she quickly learned that she could earn more when hired privately. She did work for a few years with Livingston Memorial, a Ventura County-based skilled nursing agency in the 1980s. “I enjoyed it, but knew I could make more money out on my own . . . since then I’ve been on private duty.”

She said once she got established it’s been mostly word of mouth and recommendations to get her subsequent jobs. “That’s how I get my jobs. It runs hot and cold. Sometimes I’m overwhelmed, working every day. Then I’ll go through a cold spot, not working for a couple of weeks or a month.”

There are no registration or certification requirements for private duty home healthcare providers. This means the number of workers like Dockery are hard to track. As the state prioritizes those most at risk of exposure to the coronavirus for vaccination, privately employed in-home healthcare providers are being left out.

To qualify for the vaccine, the state requires a formal paystub, a

badge and a letter from an employer on letterhead — items Dockery and many other private caregivers are unable to provide.

“They want you to have a letter on letterhead from an employer, which means a facility or an agency,” she explained. “[My roommate] works for an agency. She can go get [a vaccine] whenever she wants.”

Dockery knew she might be turned away. In her efforts to get vaccinated, the families she works for prepared a letter for her to show screeners, stating that she provided in-home healthcare. “I have got a letter from one employer, the second one just sent me one.” She also took a photo of herself with one of her clients, holding up the client’s ID showing their age. “I don’t know how else I can do it. I wish they would open it up to private duty.” But she guesses that if the state and county did that, “anybody could come in and say” they took care of people at home. “It’s a catch-22.”

As of press deadline the California Department of Public Health did not respond to numerous requests for information about whether privately employed in-home healthcare providers should be able to be vaccinated now.

I

t’s a small matter, but a detail dedicated recyclers want to get right. Should you remove the cap of a plastic bottle before recycling it? How about the lid of a metal container?

How about the lid of an aseptic package or drink box?

Bill Wayson, a retired computer systems analyst living in Ventura, is one of the many local dedicated recyclers following the news and wanting to be sure about the rules for correct recycling so our recyclables will be as marketable as possible. By email, he asked me about the recycling rules regarding lids for a variety of containers.

To be sure of the latest rules, I contacted Daniel Marks, CEO of Berg Mill Supply Company, a company marketing much of the material collected from curbside and commercial programs in Ventura County. The answer for plastic bottles is the same as it has been for nearly a decade.

Marks explained, “Soda bottle lids are made from a different plastic than soda bottles, but grinding is one of the first steps at the plastic recycling factory. A simple sink/float tank separates the two types of plastic.” The polypropylene from lids floats, and the polyethylene terephthalate from the body of the bottle sinks.

With milk jugs, the lid and the container are similar types of plastic, so they can be processed together. Other containers may be made from different types of plastics, and in some cases, lids can be hard to separate, but recycling plastics other than bottles labeled 6— — February 11, 2021

#1 or #2 is difficult for many reasons in the current market.

For steel containers such as shaving cream the answer is different. Metal is not run through grinders prior to recycling; instead, it is crushed. Recycling is accomplished through melting the metal into a molten mass so it can be poured into new forms. Leaving the lid on in this case will result in the plastic being burned. When temperatures are high enough, pollution from burning can be minimized, but the purpose of recycling is the reduction of pollution in manufacturing processes, so leave the lids off steel containers.

Also, empty aerosol cans before recycling.

For the above two types of recycling, much of the recycling is done in the United States. Plastic from bottles, for example, is recycled at Pinnpack in Oxnard and Poly-Tainer in Simi Valley.

Similarly, domestic manufacturers made 82 million metric tons of steel in 2017, “enough to form a continuous steel beam that could circle the globe eight times” and “68 percent of that steel was made from scrap metal,” according to TheConversation.com, quoting 2018 statistics from the U.S. Geological Survey, Mineral Commodity Summaries.

In contrast, recycling of drink boxes and aseptic packages is accomplished through export. Separation of layers of plastic, metal and paper requires labor and water resources more available outside the United States. Ventura County sends these materials to India for recycling. This makes the lid question

more difficult to answer.

At first, Marks speculated the answer might be “no,” since he knew the lids would have no value but would require labor to separate. But then he checked with the buyer, who assured him the recyclers in India, out of concern for the environment, do separate and transport the plastic lids to recyclers.

Andrea Gomez, who lives in Ventura and is working on fulfilling her New Year’s resolution to be more “planet friendly” by recycling correctly, sent me an email asking if drink boxes and aseptic packages are even recycled. The answer is, with or without lids, these items are recycled only in areas of Ventura County collected by Harrison Industries or Athens Services. In other areas, lack of specialized separation equipment, the timing of commodity contracts, or other factors make both the lid and the container itself unrecyclable.

Bottom line on including lids: • Plastic Bottles — yes • Steel Containers — no • Aseptic packages and drink boxes — yes, if the containers are recycled in your area.

“Very dynamic times” CMH chief with COVID treated at home

n Jan. 14, 2021, the National Institutes of Health (NIH) stepped back from its statement last year that Ivermectin, an anti-parasitic drug approved for that use by the U.S. Food and Drug Administration, is not effective in treating COVID-19. The new statement said more data was needed to make that determination.

The NIH went on to state, “In general, the drug is well tolerated. It is currently being evaluated as a potential treatment for COVID-19.” An active clinical trial involving Ivermectin at Ventura Clinical Trials, overseen by Dr. Sabine Hazan, is one of those evaluations.

While studies continue, some doctors are using ivermectin with other medicines for the off-label use of treating COVID early outside of a hospital — and are reporting success.

But due to the lack of accepted standard of care for early treatment, many doctors don’t treat COVID-19 before it escalates, meaning many patients may find access to a potentially effective treatment blocked. Most people who test positive for COVID-19 are not given any early treatment recommendations; many who get sick will receive no treatment until they are admitted to the hospital.

Lack of early treatment

An ever-growing contingent of doctors and medical researchers have been demanding that government officials raise a level of effort for early treatment approval to that seen for vaccine development, with the aim of developing a standard approach for early in-home treatment for those who test positive. But their calls have so far mostly fallen on deaf ears.

As a result, some physicians have chosen to forge ahead independently.

Hazan is one of a growing group of doctors across the country who are treating patients with success and speaking out.

She has so far treated hundreds of patients from across the U.S. who tested positive for COVID-19 and were too sick to meet the inclusion protocol of her active clinical trials or because the patients themselves wanted to receive the medicine. The clinical trials are double blind randomized - meaning participants may receive the medicines, or be given a placebo and no one will know until the trial concludes.

The Ventura County Reporter previously reported on Hazan’s work and her active FDA-authorized clinical trials for testing the efficacy of treatments and preventative measures for COVID-19 (“Trying to save humanity: Dr. Sabine Hazan and Progenobiome are working tirelessly to combat COVID19,” Kimberly Rivers, March 26, 2020). Early results in one of those ongoing trials took nearly six months to be peer reviewed and published (1). Hazan says the seriousness of this virus amplifies the importance of funding clinical trials and that local doctors should be supporting clinical trials in their region by informing and referring their patients.

CMH chief treated at home

Dr. Stanley Frochtzwajg, who is over 65, is the chief medical officer with Community Memorial Health System that operates Community Memorial Hospital in Ventura, Ojai Valley Hospital and numerous clinics across the county. He has been practicing medicine in Ventura County for over 40 years.

Last month, when he tested positive for coronavirus after feeling a few mild symptoms, he did what many of the general public would do: He stopped going to work, rested, waited and did some online research.

themselves,” he said with a small chuckle. But he says he strives to work out, eats an anti-inflammatory diet and “try to do all the right things. Nevertheless, after the first five days [after symptoms started] it felt like I was not getting better and I was concerned.”

Frochtzwajg studied the potential side effects of the medications listed in the protocol, including hydroxychloroquine and ivermectin. “I knew Dr. Hazan was deeply involved in this. I contacted her and said ‘I probably should have called you a few days ago.’” He said he had “weigh[ed] the risks and benefits of the treatment,” and that he would like to “go ahead” with the treatment “if she was willing to treat” him.

Overall he describes a fairly mild case of COVID-19. “Initially I was feeling some symptoms, then I tested positive and from there it was a two-week experience of fatigue, fever, mild cough, and I did lose my taste and my smell for one week,” said Frochtzwajg, speaking with the Ventura County Reporter on Tuesday, Feb. 2. He had tested positive just over two weeks prior. He also had “very little appetite,” during the course of his illness and one aspect of him having COVID that was “reinforced, not that I needed it, is how incredibly precious and valuable my wife is...she was like Florence Nightingale for me.”

He said he now was at about 60% of his normal energy level.

While ill he continued to attend hospital meetings virtually and publish his newsletter every Monday. “I was able to keep up on those kinds of things. I just realized my energy levels were unrecognizably very, very low.”

Confusion over study implications

One key aspect of the in-home treatment is a drug that has been used for decades to treat multiple conditions and illnesses, but which is not yet FDA-approved for COVID-19 treatment.

Hydroxychloroquine, long used to treat malaria, lupus and arthritis, recently became the center of a political hail storm around the federal government’s pandemic response. In March 2020, when it was initially touted as an effective treatment for COVID-19 — with, at the time, limited evidence on top of a slew of concerning side effects — it was easy to discredit the drug.

Many physicians, however, considered this a rush to judgment.

McCullough, in collaboration with physicians from Italy, first began work on an early treatment regimen for COVID in July 2020. He and others continue to ask why the U.S. government response to the pandemic fails to address all of the four pillars to combat an infectious pandemic. Those four pillars are: contagion spread, early treatment, late-term treatment and vaccination. (3) Even today, these questions and the call for an early treatment standard of care are being met with claims that it is “misinformation.”

On Nov. 19 before the Senate committee on Homeland Security and Governmental Affairs, McCullough and two other physicians provided sworn testimony that studies suggesting hydroxychloroquine and ivermectin were not effective against COVID failed to meet several basic aspects of what he and others were proposing. In those studies, the drugs were not given within the first 5 days of symptoms, and they were used alone rather than in conjunction with other medications. McCullough and his colleagues emphasized that any effective treatment of viral infections such as COVID required a multifaceted approach. In addition, those initial studies were based in a hospital setting — suggesting that study participants were already far sicker than patients for whom an early in-home protocol would be meant.

Timing, adaptation improve efficacy

The suggested treatment protocol ideally begins when someone first tests positive for COVID-19. Patients are started on a daily “nutraceutical bundle” of zinc sulfate (220 mg), Vitamin D (5000 IU), Vitamin C (3000 mg), and a flavonoid called quercetin (500 mg), found in several fruits and vegetables, which reduces viral replication. (2) All are available over the counter.

For those 50 years or older with one or more comorbidities, an intravenous infusion of monoclonal antibodies is suggested. In addition, the patient is immediately started on an array of anti-infective agents, which includes hydroxychloroquine, Ivermectin, azithromycin, doxycycline, and favipiravir. Some patients are started on a steroid like prednisone, and some are given an anticoagulant. (2) This treatment protocol is a roadmap for a standard of care to begin with, but doctors adapt it to the individual.

“I will tell you, it is very critical with illness to not ‘shotgun’ people, but rather to individualize care,” Frochtzwajg emphasized.

“The best caregivers and physicians do this routinely anyway with patients. I treat no two people the same because no two people are the same...different needs, genetics, variations in lifestyle, really require individualized care and that is precisely what Dr. Hazan did, she made a few modifications during my treatment regiment.”

When asked about the controversy around outpatient treatment protocols that are not FDA-approved, he responded, “We live in a very strange time with COVID and no way in God’s Earth prior to this last year would we ever have allowed a vaccine to be introduced after a few months of research. Vaccines typically take years before they’re released. But with COVID . . . we’ve made all sorts of variances in the way we treat people.”

“In fact at the hospital when we treat COVID patients we’ve tried a whole array of treatments, some of which we’ve continued and refined,” Frochtzwajg continued. “But we’ve also abandoned treatments that we henceforth did not feel played out in the way we expected. We are living in very dynamic times that force us to consider treatment regiments that we would have prior never considered without well-controlled double-blind studies. And yet we’re using all kinds of treatments including monoclonal antibodies, which are extremely controversial . . . because the urgency and the severity of the illness requires us to think differently.”

Changing the complexion of the pandemic

McCullough understands the strong pushback against moving forward with treatment protocols using drugs off-label.

“I think two tiers of doctors have emerged,” offered McCullough.

“One tier is engaging with patients, treating patients in that pre-hospital phase, markedly reducing hospitalization and deaths.

The other tier just don’t feel comfortable doing it.”

McCullough said hydroxychloroquine is “one of four [drugs] that is never used alone,” in the COVID early at home treatment protocol. “It’s been on the market for 65 years, available over the counter in many countries. The doctors who are familiar with it consider it safe and very effective. Those not familiar [with it] may not be so comfortable.”

McCullough and his wife were diagnosed with COVID late last year and were treated with hydroxychloroquine, and azythromycin, an antibiotic that works “inside cells.” “We underwent strict cardiac and pulmonary oxygenation monitoring. Followup swabs indicated we cleared the virus quickly and had the minimum time in quarantine and away from work,” said McCullough.

According to McCullough, doctors using this protocol and adapting it to their patients are already having a “huge impact on mortality” rates by keeping people out of the hospital. The protocol reduces the infection in the body, allowing a person to recover at home.

McCullough said timing is key — the protocol is most effective in the early stages of the disease. One issue complicating treatment is that most people think it’s just a mild cold.

“It starts out as the mildest illness,” he explained. “By the time they get sick they are already many days behind. If we treat [COVID when it’s] a mild illness it doesn’t become severe [for most people]. When someone gets a positive COVID test, [they should] get treated right away.” This is particularly important for those over 50, or who have serious existing medical issues.

McCullough expressed concern that “the whole medical community is just not responsive to the idea of early treatment.” He said when someone gets a positive test result they should be given an info sheet of what to do, but instead they are basically just told “good luck...Our health care delivery system has a long way to go to be responsive.”

If a person’s doctor is not comfortable with the in-home treatment, McCullough recommends that they “ask for a referral to a doctor that would do it. They can engage with doctors [anywhere] over telemedicine.”

McCullough also recommended a “layer of early preventive nutraceuticals,” including zinc, Vitamin D, Vitamin C and quercetin, taken daily.

“None are curative,” he explained. “But it is true that those who are just a little deficient end up with a more severe case. I definitely recommend these daily through the pandemic.”

“There are no drugs that cure the illness,” aded McCullough, but having an adopted “sequenced multi-drug approach would change the entire complexion of the pandemic.”

McCullough is well aware of Hazan’s approach, saying “I strongly support it.” He says a national, and indeed global standard of care for early treatment is needed because the “vast majority of deaths occur in the hospital.” With doctors correctly using the early protocols, McCullough said they are finding an “85% reduction in hospitalizations.” (4) Each component of the suggested protocols are “currently available over the counter and have a proven safety track record.”

McCullough added that even with vaccines being rolled out, the administration is too slow to save the thousands that will continue to get sick and require hospitalization, without early treatment. “It is our only hope of reducing the risk of hospitalization and death.”

Frochtzwajg emphasized that everyone should continue to follow safety precautions. “I will be doing absolutely everything that we should all be doing, wearing my N95 [mask], wearing a face shield when working with patients — and I think that we’re all going to need to do that going forward to deal with this virus and the new higher contagious variant and variants. We’re not out of the woods yet by a long shot.”

“But it’s hard to convince people to do some of the basics,” said Frochtzwajg. “I was watching a rally the other day [on television], no one was wearing masks in Montana and I thought, we are not going to be over this for a very, very long time.”

In terms of the long road and continuing to be better equipped to combat the virus, Frochtzwajg said, “I think Dr. Hazan has the real answer, which is to fortify our intestinal microbiome so that we are much less susceptible . . . our immune system really originates in our microbiome and she is doing the incredible groundbreaking work that is elucidating that more and more. That is my greatest hope.”

Following Hazan’s protocol, Frochtzwajg says he was treated for 10 days. “I definitely started to show improvements in my fatigue, my fever resolved . . . Taste and smell returned about a week after starting treatment.”

“She was unbelievably attentive, concerned and supportive,” he said of Hazan. “It was truly amazing, one of the most compassionate and attentive physicians I have ever dealt with. I can’t speak highly enough of her attentive care or passion . . . I received four or five phone calls a day from her, checking in on me and my status.”

In addition, Frochtzwajg was self-monitoring oxygen saturation using an oximeter, and taking his temperature three to four times a day.

He did experience “mildly decreased oxygen” and says he was “fortunate” in that his levels never got “concernedly low.”

He had brought an oxygen tank home but he never needed to use it. The day he was interviewed he said his oxygen levels were “near normal.”

He has started to take walks around the block and hopes he’ll be back seeing patients this week. “Technically a person is no longer

“Ojai has a small community that facilitates new musicians,” she says. “I met a lot of people in the music industry.”

One of those people was Matt Sayles, of Ventucky String Band and Philville Records fame.

“I met Matt two years ago,” Jobus recalls. “The first time we put music together . . . we kind of jammed. He offered to do a recording for me.”

“Mountain Wife” was recorded in real time at Philville Studios West in Ventura. Sayles backs Jobus on rhythm guitar, with Bill Flores (pedal steel), Matt Cadenelli (percussion) and Terry Luna (bass) also lending a hand. It was recently released by Philville Records — although it was one of the first songs Jobus wrote after arriving in Ojai.

“I had a dream. I was singing the chorus over and over again,” she recalls. “I woke up early in the morning and thought, ‘I want to write that down.’”

She walked down to the Ventura River nearby, and a coyote approached.

“I just kept playing, and the coyote came up and laid on the ground,” Jobus recalls. She ended up finishing the entire song that morning.

Pre-COVID, Jobus was a regular on the small-venue circuit, performing at places such as MadeWest Brewing and Oak and Main. In 2019, she headed to Nashville, where she hoped to make her mark as a songwriter.

“I didn’t book any gigs,” she says. “The goal was to write for a publishing company.”

It was a prolific time: She expanded her catalog, and collaborated with several artists. Before anything got picked up, however, the pandemic hit and she decided to move west again. After spending a long spell with her mother in Texas, she came back to the area (she lives in Ventura currently) and decided to “just take a second to wait out this COVID stuff.”

In the meantime, she’s making a living at Topa Topa Brewing — which she loves.

“It’s so fun, and great for networking!” Jobus says.

“Every brewery has a different little vibe. I love Topa Topa because it has this sense of community.”

When she’s not filling to-go orders she’s continuing to write— several times a week — and considering her next move.

“Anything can happen”

Something she’s currently contemplating: streaming concerts, possibly in collaboration with Noah Berberea of Brother Earl and the Cousins. Berberea is in the process of creating a virtual concert series called the Dirty Couch, and he’s approached Jobus about working together.

“I love the idea of live recording, maybe getting some video footage,” she says.

Another plan: getting involved with national songwriting circles. She recently had a Zoom audition for one based in Nashville called Song Suffragettes.

The acoustic showcase features five emerging female country artists every week, and it’s been a great way for new talent to get noticed. According to the website (www.songsuffragettes.com), of the 200+ people who have performed, 12 singers have received record deals and more than 40 songwriters have received publishing deals.

“Lots of people in the industry go to these songwriter rounds,” Jobus says. “Anything can happen.”

The Song Suffragettes might end up bringing Jobus back to Nashville at some point. In the meantime, she’s finding joy in the here and now.

“Far off into the future — I just can’t think about that right now,” she says. “I have a great job I love, and I can’t wait to play live again. I’m just focusing on being happy.”

Men have perhaps 15 to 20 times the testosterone women do, and this seems to play a major role in how visually powered male sexuality is.

An episode of NPR’s “This American Life” featured Griffin Hansbury, who was born female and lived as a lesbian and then underwent sex reassignment surgery. Transitioning began with testosterone injections.

Before starting testosterone, Hansbury would see a woman on the subway and think: “She’s attractive.

I’d like to meet her. What’s that book she’s reading?” With the testosterone injections, even noticing nice ankles on a woman flooded Hansbury’s mind “with aggressive pornographic images. ... It was like ... a pornographic movie house in my mind. And I couldn’t turn it off.”

It’s important to note that Hansbury overdid it on the testosterone injections at first, giving himself two to seven times the testosterone of the average male. Men’s brains motivate them to stare at sexual eye candy in ways women’s brains do not. In brain imaging research by psychiatrist and neuroscientist Hans C. Breiter and his colleagues, heterosexual men viewing attractive female faces showed increased activation in the brain’s “reward regions”: areas involved in the anticipation of reward (also activated by food, drugs, and money).

The message to the man: “That’s rewarding! Go get it!”

The good news is we are not mental robots, slaves to our urges.

In most men, the brain’s “go get it!” message simply leads to more looking, not nooners with the hot-erellas of Instagram. Some women consider looking cheating, and they tell their partner he’s not allowed to eyeball other women. Chances are this doesn’t stop the looking; it just turns men into sneaks and liars and their partner into the enemy they have to keep secrets from. Consider that the guy set aside another strong evolved male preference — the longing for sexual variety — to be with you. In other words, being with you means a lot to him. Sure, he still spends time browsing in the online mall of naked and half-naked women, but browsing isn’t buying. Assess whether he seems to be a good person, a person of character, and a guy who consistently shows you he loves you and has your back. If so, your best bet might be staying out of his browser history and recognizing that a little eye-humping doesn’t mean there will be eye-penis coordination.
