The nurse on the line asks to come up, frustration in her voice, but I won’t give up hope.
We meet for the first time, I see the fear in your eyes. Under all this plastic and shield, I hold your hand in an attempt to make this more human and less cold. So many things begin to happen, all that you could have anticipated, all is much worse. Painful procedures, so much information, so many signatures, so much discomfort. All you ask for is water and the answer is “sorry, nothing by mouth right now,” you smile and comfort me by telling me “it’s ok.”
I won’t give up hope, I will give my all. Time for me to exit your room, I leave your bed facing the door, I teach you how to use devices you are unfamiliar with, I ask if you want to call your family, you hesitate because you don’t want them to worry; I wonder how to tell you that you should call them, this could be your last chance. I ask again and you agree to call, I hear your spouse crying as I exit the room. After peeling off all the layers, I stand outside your room and wave and smile at you under my mask in an attempt to show you that you are not alone. I call you to let you know that my shift is over and that I will be back in the morning, you shower me with blessings and tell me to stay safe until we meet again.
I hold your hand and feel life escaping your body and tell you to keep fighting, that we are in this battle with you and that we must meet again. The day brought much chaos and despair, but then your therapy begins to work and there’s a ray of hope in your fate. The ray of hope is quickly overshadowed by a long journey of much frustration and despair; we are doing all we can just as you wished because you want to see your family again. The last words you spoke to me were: “I want to go home.” Your life escaping, we try until the options have ran out. I guide your wife and daughter to the hallway where they must stand away from you and I watch them cry. My heart hurts and your daughter turns towards me and asks for a hug; I gently explain that I should not hug her to prevent contagion.

“Vitamin D is involved in regulating different proteins and immune modulators. It allows the immune system to function in a normal and healthy manner. Most people can get enough Vitamin D by spending just 20 minutes a day in the sunshine, between 10 a.m. and 3 p.m. That is when the UVB rays provide the best opportunity for Vitamin D exposure on the upper torso and arms without UVB-blocking sunscreen. Alternatives include getting Vitamin D through diet, including fatty fish like salmon or grass-fed red meat. But, Bloom says, sunlight is best.

Dr. Stanley Frochtzwajg, chief medical officer of Community Memorial Hospital, offers seven tips for overall health. • Nutrition: An Anti-inflammatory diet that is low in refined sugar, gluten and that limits dairy products. • Hydration: Water is needed for all processes in the body. • Sleep: Particularly deep REM sleep that is not disrupted by alcohol. • Exercise: 20-30 minutes of walking everyday, preferably outside. • Meditation/Relaxation: Meditation or a relaxing, meditative activity like drawing or gardening. • Social interaction: Conversation, physical hugging and sex are best, but virtual face-to-face connections can provide a similar dopamine response. • Spirituality: “It can be religion, philosophy or any aspect thereof. On ancient birthday of trees, the nontoxic methods of “tax” avoidance

by David Goldstein n the ancient world, one of the many ways kings taxed their subjects was based on the harvest of their subjects’ trees. According to a website sponsored by Ithaka, which describes itself as “a not-for-profit organization helping the academic community,” sometimes these taxes were comically complicated. For example, since likely crop yields in ancient Egypt depended on annual flooding of the Nile River, taxes required three tax collectors and two assessment periods. Before the floods, officials calculated taxes based on predicted flood heights. After each flood, three assessors refined the tax by holding cords, stretching the cords, and recording by the number of cord lengths the extent of cropland watered. These experts then calculated as a tax 10 percent of the expected harvest. Ancient commentary on biblical injunctions shows similar procedures elsewhere. The Jewish holiday of Tu BiShvat, linked to the lunar calendar but celebrated this year on Jan. 28, commemorates the “birthday” of the trees, a demarcation enacted partly for an ancient tax. Each tree, regardless of when it was planted, was counted as a year older on this date. The “tree year” was initially used for Levitical prohibitions on fruit from the first three years, but later used because each year, or partial year, of the tree’s harvest, 10 percent of crop yield from each tree was owed to the Temple in Jerusalem for distribution to the priestly class and the poor. 6— — January 28, 2021

Today, many Jews celebrate the birthday of the trees by planting trees. Although figs, dates and pomegranates are the traditional tree crops of the holiday, I will plant a persimmon tree. Bare root tree planting is generally best at this time of year, and in the bare root stage, planting is easier. As noted by University of California Farm Advisor Dr. Ben Faber, “In the bare root stage, trees are still dormant, and you don’t need to be as delicate with them” as you would with evergreens, which typically suffer transplant shock. Although taxes no longer extract 10 percent of our harvest for a pharaoh, king or priestly class, and tithing to the hungry through organizations such as Food Forward or Food Share is voluntary, endemic to Ventura County are several other entities likely to extract a share of our crops. Gophers are prevalent, so one measure to protect your new tree is to protect roots with a mesh gopher barrier. I have dug holes deeper than needed for the root ball, lined the bottom of the hole with chicken wire shaped into a basket, and filled the basket partially with dirt before planting the tree at the desired depth. However, Dr. Faber warns, “You really need to line up the bottom of the tree ball with the bottom of the hole” because “if you dig it much deeper, the tree settles over time” and if the trunk stem is covered, the tree becomes vulnerable to disease. Instead, Faber recommends products such
as the Root Guard Speed Basket, which fits into the planting hole, preventing gopher attack. Moles are also eager to extract a portion of your crop. They are not really eating roots, but digging for worms, disturbing the root system. Both moles and gophers can be killed with spear- or harpoon-style traps, which are better than anticoagulant poison bait traps because the poison may harm non-targeted species, including predators eating poisoned animals. Ground squirrels eat roots, strip bark and also take fruits and nuts from trees. You can stop squirrels with metal guards around tree trunks. Some gardeners place poison gas devices inside burrows. Birds also try to take a share of crops. Screening or netting material tied around the tree is most effective. Sometimes, owl statues, fake snakes or cat statues can scare away birds, but only if these are repositioned frequently. Tax authorities of the ancient world no longer take a portion of your garden, so non-toxic methods of pest control can help you enjoy the full wealth of your harvest. n Ojai-based company saw a need develop as people were forced to stay home during the pandemic. n Tuesday, Jan. 26, Ventura County announced that a phone line for making vaccination appointments is live. The phone line is aimed at those with limited Internet access or language barriers. It is live as of Tuesday, according to county officials, but the county does not have any open appointment times because of limited vaccine availability. County public health officials said there is a vaccine shortage nationally, and that is impacting the availability of vaccines to the state and then at the county level. The county has administered 93% of the vaccines it has received and assured the public that vaccines are being given as promptly as possible when they are received. Available appointments will be updated every Monday. Dr. Robert Levin, health officer with Ventura County Public Health, also reported that the population of Ventura County makes up just 2% of the state’s population. He said the county so far has gotten “1.7% of the vaccine” allocated to California. He explained that, based on new data the county had provided to the state, the county’s allocation has been “recalculated” factoring in the number of people over 65, which increases the county’s allocation up to 2.2% of the state allocation. “This is good news,” he said.
While several Ventura County Supervisors suggested that the state was responsible for failing to provide enough vaccines to Ventura County, according to California’s Department of Public Health, the state does not take possession of the vaccine at any time in the process of vaccine distribution and it is the federal government that determines the weekly allocation of vaccines for each state. State allocations are announced each week by the federal government; local providers of the vaccine, in this case Ventura County, place weekly orders with the state. The state reviews that order and submits it to the federal government, which in turn submits the order to the manufacturer. The vaccines are delivered directly to local vaccine administration sites from the vaccine manufacturer or a third party distributor, and the number of doses received in each county is based on the calculation of that county’s vaccine eligible population as a percentage of the state’s population, and overall availability. Help write letters to seniors The Ventura County Area Agency on Aging (VCAAA) is seeking community volunteers to write letters to seniors in Ventura County. Over 30,000 people live alone in Ventura County and many suffer from isolation and depression, which may be heightened by the pandemic. Started in September with the Meals on Wheels program, the letter writing campaign, Letters of Support, is designed to build connections between older adults and those with disabilities to other community members. So far the campaign has sent over 9,000 letters offering care, support and connection. All letters are reviewed and mailed to the recipients by VCAAA. Vet tech program launched at Ventura College A new two-year program offering an Associate of Science degree in veterinary technology is being offered at Ventura College’s East Campus in Santa Paula starting in the Spring 2021 session. After the first year of the program,
students will be certified as a veterinary assistant, enabling them to seek entry-level positions in veterinary 1/26/21 11:13 AM technology. The program is launched in partnership with Ohana Pet Hospital of Ventura, which is helping the college to build a new teaching lab and raise funds to support the project. The program expects to receive accreditation from the American Veterinary Medical Association by the time the first students complete the two-year program, making graduates eligible to take the Veterinary Technician National Exam to become a Registered Veterinary Technician. There are currently no programs currently accredited by the AVMA in the tri-counties. Dr. Jill Muraoka Lim, a founding partner of Ohana Pet Hospital, said as there is “a local (and national) shortage of qualified and educated veterinary technicians and assistants, this program will help fill the staffing void that all veterinary hospitals face.” Program classes are full for the spring 2021 semester. Ventura College will host information sessions and make applications available in March 2021 for the 2021-22 school year. “Keep it simple and just move” A local trainer’s guide to finding fitness anytime, anywhere by Nancy D. Lackey Shaffer

Among the many, many individuals struggling during the pandemic are gym owners and their customers. Without being able to access their preferred spot for CrossFit, yoga or lap swimming, many are finding it a challenge to stay fit. Gyms throughout Ventura County have gotten creative, offering classes online and outdoors — wonderful options for those who thrive on group energy, the guidance of a trained professional and a set schedule. But not everyone belongs to a gym; not every gym-goer is a class-goer; not every class-goer wants to log on for yet more screen time or mask up for an outdoor class. The longer COVID-19 restrictions last, the more vital it becomes for us to find ways to move our bodies, get our heart rates up and stay limber — without relying on a facility. Here’s a bit of good news: Getting in shape and/or staying there is more achievable than you think.

Nadine Stilwell of Ventura is a certified personal trainer and fitness nutrition specialist who has taught classes at local gyms and worked with clients of all fitness levels on an individual basis. She says that one good guideline to use when working out is that offered by the American Heart Association. “The American Heart Association recommends 30 minutes of moderate exercise five times a week, or 150 minutes,” Stilwell explains. “Moderate exercise is an activity that allows you to have a conversation but is too taxing to sing.” Think an easy jog or a brisk walk. According to Stilwell, this is a great goal to have no matter where you are in your fitness journey — new to exercise, trying to get back into shape or working to stay on track. The trick is to keep it consistent and achievable, and to record it. “What I think is a valuable tool for anybody is to log it somewhere. In a notebook, on a whiteboard, on the fridge . . . It helps to measure your success, motivate you and hold you accountable.” It’s also a handy tool for keeping track of those 150 minutes — useful if your pandemic schedule has you wondering what day, exactly, it is. As you start incorporating exercise into your weekly routine, you are likely to notice improvement. Maybe it’s getting harder to raise your heart rate, or you cover more distance in a 30-minute walk. Stilwell says that’s a signal that you should “maybe set the bar a little higher. Maybe you go for 35 minutes instead of just 30.” Another option: make those 30 minutes more intense by adding in squats, lunges or jumping jacks, or simply trying to 10 — — January 28, 2021

walk a little faster. At the same time, Stilwell thinks it’s important to not hold yourself to an impossible standard. The key is being flexible, and honoring where you’re at in your fitness journey. “Don’t make it so rigid that if you don’t achieve it you’re disappointed,” Stilwell says. “Disappointment is a limiting factor to your success. It’s okay if you can’t do 30 minutes a day! That’s just a goal to strive for.”
No gym or equipment required
After having access to a seemingly unlimited selection of weights, kettlebells, rowing machines, treadmills, elliptical steppers and other gym paraphernalia, it might seem impossible to get in a good workout at home. Not so, says Stilwell. “My methodology as a trainer is, I want to empower people to do this on their own,” she says. “I don’t want people to feel like they have to depend on a facility or a person or a piece of equipment.” She finds simplicity is key — exercises that are familiar, easy to do (she’s not a fan of overly complex moves), and rely on repetition or body weight to get the job done. Squats, lunges, pushups, sit-ups, jumping jacks and burpees are movements that can be done anywhere, and she herself uses them to stay in shape. And yes — walking can be a great (and low impact) piece of any fitness plan. “A brisk walk — I highly encourage you to do that!” Stilwell says. “Want to add something to that? Do some squats or lunges every couple of minutes.” “When I exercise, I don’t know what I’m going to do,” she
continues. “I just go out and start my walk.” She gets inspired to incorporate other movements based on her environment or how her body is feeling on a particular day. In addition to squats, she’s been known to perform pushups (done standing up against a wall or fence) and lunges while strolling around the neighborhood. Steep streets are great for cardio. Anyplace with steps will provide a great workout, too — walking or running up and down, doing step ups, lunging. While Stilwell is a big advocate of using body weight training to bring the burn, there’s a portable tool that she recommends. “Get yourself some exercise tubing,” she says. “It’s light, portable, convenient and economical. And it’s good for resistance.” Exercise tubing (also called resistance bands or resistance tubes) comes in a variety of resistance levels and is very versatile. Stilwell likes to take it with her on a walk, wrap it around a lamppost or other pole and do some rows, or stand on it to do bicep curls. Find other tube exercises in the instruction manual or with a simple online search.
Family fitness fun
Stilwell’s approach is designed to work for any body — including younger ones. As a mother of three herself, she’s had to find ways to incorporate exercise into her kids’ lives now that sports and PE have been curtailed. What’s worked best for her crew is working out as a family, and keeping it fun. “I really like to grab some dice and choose six exercises — easy exercises — to assign to each number of dots.” For example, one dot is for sit-ups, two dots is for squats, three dots

vcreporter.com is for jumping jacks, etc. Roll the dice, do 10 reps (or however many works for you) of the exercise based on the number that comes up, and then roll again. Instead of reps, you could make each exercise a timed drill (30-60 seconds is a good amount). “Do that together as a family for 20 minutes,” Stilwell suggests, adding that letting the kids pick some or all of the exercises is another way to get them engaged (and doing movements they like). A similar game can be created with a deck of cards. Each suit is assigned an exercise; the number on the card is the number of reps. Keep going through the deck for 20 minutes. Face cards can be assigned a special number or exercise. If friendly competition is a motivator, give everyone their own set of dice or deck of cards and see how quickly they can get through a predetermined number of rolls or cards, or how many reps they can do in a timed drill. Whatever keeps it fun and interesting is a win. Tabatas are easy to do at home, too. A Tabata is a type of high intensity interval training (HIIT), originally developed in Japan by Izumi Tabata and Olympic speedskating coach Irisawa Koichi. A four-minute Tabata workout is divided into eight cycles: 20 seconds of exercise followed by 10 seconds of rest. “Most music platforms, including iTunes and Spotify, have Tabata programs you can search for,” Stilwell explains. “You’ll find playlists with Tabata songs and a voice coach who will tell you when to work and when to rest.” “Tabata is traditionally done HIIT-style,” she emphasizes, “but there’s no rulebook. You can apply it to your level of fitness. It is a fun way to have a coach tell you when to start or stop. You don’t think about timing yourself or setting up the music.” Dice, cards, prerecorded Tabatas — Stilwell appreciates all of these approaches because they can be done at home and adapted to any age or fitness level, and don’t require any fancy setup or equipment. For her, the less complicated, the better. “Keep it simple and just move!” she says.
Balance, core and stability
“I think balance is super important,” Stilwell says. It can help prevent falls, or make falls less damaging. Exercises that challenge our balance make us use our core to stay stable — which she thinks of as one way to “workout smarter rather than harder.” Stability balls, BOSU® Balance Trainers and core boards are devices designed to make you wobbly during a workout, and thus challenge balance and stability. But there are equipment-free ways to do the same, such as staying on the toes while stretching up, lifting one arm or leg during a plank, or doing rows, bicep curls or other weightlifting exercises on one leg. It’s wise to have something stable to hold onto (furniture, a wall) while Nadine Stilwell finding equilibrium. Sit-ups and bicycles are two familiar exercises which target the muscles in the abdomen. But a plank, done properly, is good for the arms, chest, back, glutes and abdomen — that’s a lot of core muscles to hit with one move! Whether it’s on your toes or on your knees, Stilwell says “it’s a very good, simple, core stability strengthening exercise that anybody can do.” She also likes bridges — lying on the back with knees bent and lifting the rear while feet stay on the floor — for stabilizing and strengthening the glutes. Stilwell calls them a “reciprocal move to the sitting position, so you’re counteracting the negative effects of sitting.” “When I work with clients, I always start with core and stability,” Stilwell says. “If you can improve your core
strength, that will help you on your walks, during Tabatas, or whatever you’re doing. No matter how conditioned you are, always incorporate core and balance movements.”
Tools of empowerment
There’s no lack of gyms offering online and outdoor options for exercise — and taking advantage of that is as simple as logging onto the website of your favorite location or trainer, or even revisiting the list we created shortly after stay-at-home orders were first announced (“COVID-19 Fit: Local fitness studios offering virtual classes” by Kateri Wozny, April 9, 2020). But an at-home workout is something anyone can do, either as an alternative or in addition to what’s available from local fitness centers. Because the longer we’re away from our traditional workout opportunities, the more important it is that we find ways to be — and stay — active. “I want everyone to keep it simple and keep their baseline fitness up,” she says. “This is also going to help reduce our anxiety, which is extremely important these days. All these things I’ve suggested here are just tools I have discovered for when we are home.” A final word of advice: “Do the type of movement that you want to do. “Open and raw” Healthcare workers coping under COVID

t takes a certain type of person to work in healthcare. Whether the person is called to nursing, anesthesia, primary care or surgery, they generally experience a call to help others. They expect to see illness, sometimes fatal. They expect to have ways to help and involve the family as a patient transitions to death with dignity. COVID-19 is changing all of that. “We are seeing a lot of anxiety, burn out, depression, a feeling of giving up. Hopelessness if you will,” said Dr. Javier Romero M.D., a surgeon and critical and intensive care physician at Ventura County Medical Center (VCMC). He is also the director of the surgical residency program, helping to train surgeons, at Community Memorial Hospital (CMH) in Ventura. He said it’s the nurses that feel the full force of this. They “get attached to patients, see them every day.” Romero explained that an added element healthcare workers are dealing with is the “uncertainty of the disease process.” Starting at the very beginning in March 2020, those caring for sick patients didn’t know exactly how to care for them. “It’s really important to tell the public, this is a novel virus,” Romero said. “Many of the treatment modalities are somewhat Tristan Jost (far left) and Kelly Searles, both respiratory therapists, experimental. We are seeing a very vicious disease. Patients, when they do succumb to it, really die by themselves. It’s hard.”
The stress of fighting COVID
Care providers on all fronts are being asked to cope with the stresses of fighting COVID. Romero said that it is important to shed light on the problem by talking about and normalizing these experiences and reactions. “Addressing it, saying it’s there to begin with, is the first step,” he said. “I want to emphasize how courageous Sonia is.” He referred to Sonia Nikolaus, a charge nurse in critical care at VCMC. She wrote an essay she shared on
Facebook, (printed in “Power to Speak” on page 4) that shows, through her eyes, what is happening. “Physicians only do a small part. I really appreciate what the nurses, respiratory therapists do. It really takes a whole team to take care of these folks. The true heroes are the folks who work around us.” The enormity of working under the pandemic is compelling caregivers to reach out to each other, share and listen. VCMC has an employee assistance program that provides a
psychologist for staff to talk with and have “open dialogue” about their experience. Romero has found that a “more public forum” helps make it “OK to feel this way.” As colleagues are “struggling” he’s been reminded of the idea of “listening to understand, not to respond. “The healthcare community is really trying to be compassionate, innovative and give top notch service and care, but this disease makes it really difficult,” Romero said. Normally, a patient’s family is a key part of managing care in the hospital. “This disease prohibits that,” Romero pointed out, and with all of the PPE, “that takes some of the humanity away from it. It’s very difficult for the healthcare community to render care.”

She emphasized that a lot of the normal care of nursing — helping people be more comfortable after a surgery or during an illness — is not available to patients with COVID. This is terrible for the person with COVID, and adds to the stress of the nurses, who feel powerless to offer relief. “The hardest part is seeing patients suffer. Usually we have medications we can give them [to help them] fall asleep, or to sedate them, to help them feel better. With COVID, we can’t do this. If you really want to go to the source, some growers, like Underwood Family Farms (with locations in Moorpark and Somis), will even allow you to go out to the fields to pick your own. These nutritious and delicious recipes courtesy of Underwood are made with ingredients grown right here in Ventura County and in season now. Bon appétit!
Blood Orange and Red Onion Salad
The blood orange is a hybrid of the tangerine and a pomelo and is known for its reddish flesh acquired from the presence of the powerful antioxidant anthocyanin. The rind is tougher and harder to peel, but the taste is sweeter. Blood oranges are at their peak right now and have been showing up at Underwood stands at local farmers markets, at the farm store and in the Healthy Harvest CSA box. 1/4 small red onion, very thinly sliced 1/4 cup rice wine vinegar Sea salt and freshly ground white pepper 4 blood oranges 2 tablespoons extra virgin olive oil 2 tablespoons torn basil leaves 1. Toss the red onion with the vinegar and season with salt and white pepper. Let stand at room temperature until softened, about 15 minutes. Drain. 2. Meanwhile, using a sharp knife, peel the oranges, removing all of the bitter white pith. Thinly slice the oranges crosswise, removing any pits. 3. Arrange the oranges on a platter and scatter the red onion on top. Drizzle with the olive oil and season with salt and white pepper. Garnish with the basil and serve.
Roasted Romanesco and Shrimp
This flavorful dish features roasted Romanesco, a beautiful, spring-green cauliflower varietal with intricately spiraling “buds.” Its texture is similar to cauliflower but the flavor is milder and a bit nutty. Romanesco is readily available this time of year, although traditional cauliflower or broccoli make decent substitutes. 1-2 pounds of Romanesco, cut into bite-size florets ¼ cup extra virgin olive oil 1 teaspoon whole coriander seeds 1 teaspoon whole cumin seeds ½ teaspoon chili powder 3 cloves of garlic, minced 1 pound of shrimp, shelled and deveined Dash of sea salt
