T

Stop living with foot pain

C

hildren can benefit enormously from early treatment of foot problems. Young people are very resilient, so if they are complaining of aches or pains in their feet or legs, it is often a sign of significant problems.

The majority of foot and leg problems Dr. Butler sees in children are related to mechanical problems in the feet. Most of these foot function problems are to be blamed on genetics; we inherit them from our parents.

If, as a parent, you have foot problems, there is a good chance that your children will develop the same problems. Diagnosing and treating these conditions early can greatly reduce or eliminate these conditions from becoming so severe later in life for your children.

Listening to the family history and performing a gait examination is an easy first step in determining whether a child is prone to developing family foot problems.

Treatment with the appropriate shoe gear and/or orthotics can be very beneficial to a person’s mobility and lifelong joint health.

Dr. Kent Butler, D.P.M., graduated with honours from the California College of Podiatric Medicine. He then completed a one-year surgical residency followed by a one-year biomechanics fellowship at the Pacific Coast Hospital in San Francisco. Specializing in biomechanics helps him to accurately assess a patient’s foot and leg function during the gait examination.

As a student, he worked four years making prescription orthotics for podiatrists. Here, he learned various fabrication techniques and modification skills to facilitate orthotic function. Dr. Butler is the only podiatrist who personally handcrafts every step of each custom orthotic for his patients.

Foot pain is not something you have to live with.

Acid Reflux By your friendly neighbourhood Heart Pharmacist

D

espite its caustic sounding name, acid reflux is not the newest rock band young people are listening to. Acid reflux is sometimes referred to as heartburn — a condition where your stomach contents rises up into your esophagus. Like loud music, heartburn can be painful and irritating. Luckily, for most of us, we simply take an antacid and the pain goes away. For others, the pain can be quite intense and return frequently. This month, we shall learn what acid reflux is and what it isn’t.

What normally happens when we eat or drink is the food travels down our esophagus and then into our stomach.

There is a tight band of muscles at the top of our stomach called the ‘lower esophageal sphincter’ (LES) which keeps all of your stomach contents where they should be.

Sometimes the muscles of the LES can relax, allowing the acidic contents of the stomach to come up into the esophagus.

This is referred to as acid reflux. The resulting irritation to the esophagus causes a burning sensation, which is usually felt just behind the breastbone and below the throat. The onset of this pain can be quite intense, but it usually goes away when a person takes antacids.

There is one thing that acid reflux is not: a medical condition called ‘gastroesophageal reflux disease’ (GERD). GERD can be thought of as a persistent form of acid reflux. The symptoms of GERD are the same as for acid reflux except that people who have GERD may also show symptoms of a dry cough or difficulty swallowing. GERD can also cause asthma-like symptoms. If you are having persistent acid reflux symptoms more than twice a week, it might be wise to bring these symptoms to the attention of your physician.

For many, non-drug options work very well. The first thing is to avoid foods or beverages that trigger your acid reflux. Some foods that can trigger heartburn are coffee, mint teas and spicy or fatty foods. If you must eat your trigger foods, take an antacid prior to, or just after the meal. If you smoke — stop. Try not to lie down for at least three hours after eating a meal.

If acid reflux is a problem at night, try raising the head of your bed. You only need to raise it by approximately six to eight inches, about the thickness of a large phonebook. Some people find it helpful to eat smaller meals more frequently, about five or six meals throughout the day.

One health tip that I feel is overlooked is to eat slowly. People tend to eat their meals

too fast; I feel this causes them to eat too much. This is an old weight loss tip that can be helpful for heartburn sufferers as well.

Consciously eat your meals.

As a parent, I am often at fault for this.

I fill my plate and stuff my face so fast that I am done before my little one is done. The next time you have a meal; take reasonablysized bites, put your fork/spoon down between bites and chew your mouthful at least 20 times. This delay in eating may seem odd at first, but it can be very helpful.

It allows your stomach to fill slowly and gives you the signal that you are full, before you eat too much and feel ‘stuffed’, which can lead to heartburn.

The other way to help treat acid reflux is to reduce stomach acid content. The best treatment for mild and infrequent heartburn is calcium carbonate tablets (Tums, Rolaids, etc.). These products neutralize stomach acid and help reduce the burning feeling caused by stomach acid. They tend to work quite well and have few side effects.

Sodium bicarbonate products act similarly to calcium carbonate tablets.

Usually these products are effervescent (ie.

Bromoseltzer), and contain a lot of sodium that might be contra indicated for people with high blood pressure. Ask your physician or Heart pharmacist if these antacids would be right for you.

Another option for acid reflux relief is an alginic acid containing product (Gavison, etc). This is a natural product derived from brown seaweed. The alginic acid forms a layer of foam that floats on top of the stomach content. This helps protect the esophagus from the acidic content of the stomach, and thus stops any burning sensation.

If these two options do not help your heartburn, you can try the group of medicines referred to as the H2 blockers, better known as ‘ranitidine’ (Zantac) or ‘famotidine’ (Pepcid). These medications work by stopping your body from producing stomach acid. They are especially helpful if you know that a certain meal will cause heartburn.

They are more effective than antacids, but should be taken about one hour before meals. There are a few side effects with taking medications to help reduce stomach acid. The most common ones are digestive problems; bloating, gas, diarrhea or constipation. This is because your body needs some stomach acid to completely digest your food.

With prolonged use, these medications can lead to vitamin and mineral deficiencies. The most common deficiencies include vitamin B12, calcium, magnesium

and other minerals. If you are taking acid suppressing medications, you may consider taking a multivitamin. Make certain any mineral supplements you are taking are in the citrate form (ie. calcium citrate).

The citrate form of minerals are not as dependent on stomach acid for adequate absorption.

I hope this article has answered all of your burning questions about acid reflux. If you do suffer from acid reflux, first try the non-drug ideas I mentioned. These options will usually help relieve your symptoms.

Another worrisome issue is that the pain from a heart attack is similar to that caused by acid reflux. If chest pain appears suddenly, does not occur after eating, or happens after physical exertion, seek medical attention quickly. heartpharmacy.com

When Saanich Parks and Recreation created their current Older Adult Strategy, we heard from a large number of older adults about our recreation facilities. Some were regular users but many thought recreation centres were just for “sweat and spandex”, or were very intimidating. As such, one of the strategic priorities is to create Age Friendly Spaces and make our centres more welcoming for all ages, including older adults.

One way to achieve this is to add Welcome Ambassadors in our lobbies. Many people who are new to recreation are unfamiliar with our facilities and have questions about programs,

prices and passes. A friendly face and a person who has time to spend with them is what a Welcome Ambassador is all about.

Once implemented, a Welcome Ambassador will be available to answer questions about the centre, accompany you to the program space, walk you through the registration process and discuss local community resources to support your active lifestyle.

The inaugural Welcome Ambassador will be launched at Saanich Commonwealth Place this fall and we invite interested and engaging people to apply to fill this crucial volunteer role.

I

f it takes a community to raise a child — and it does — it must also take a community to have a great relationship, and dare I say — to die well.

Reviewing the demographics we face in British Columbia, with close to one million baby boomers living here, we have a rare opportunity to put a different face on dying and death. Given the raw numbers, home deaths will, by necessity,

become the norm. Are we ready for this once in a lifetime opportunity as neighbourhoods and communities?

With death branded as the grim reaper, seen as a failure by the medical system, and something to be avoided at all costs our basic mantra, why would any of us want to get involved as a caring neighbour or friend?

What would happen, though, if we rebranded death? How would we respond to death if it were viewed as an inspiring teacher?

Would we approach those dying differently if death were accepted as a natural and important aspect of living a full life? Could it be, perhaps, an

opportunity to say good-bye well, and a chance to leave some of our life’s wisdom behind with our family and friends?

As a neighbour, would I not be honoured to support the family next door during such an important time of life? Would I not be more than willing to provide a helping hand in creating a great farewell? Would I not be glad to share end of life planning tools with a neighbour struggling with the imminent loss of a loved one? Answer — yes!

Learning to Live with Loss Grief is not time-limited.

The work of the grieving process is really about making adjustments and learning to live with the loss.

Grief is the road you travel.

When grieving, it’s important to listen to your instincts. If you feel sad, allow it. If you are lonely and want company, reach out. Grief is different for everyone.

By Marney Thompson, Director of Psychosocial Services, Victoria Hospice hen a loved one dies, the thoughts and emotions that come with that loss can be intense and confusing. As grief counsellors, we often encounter a question that, by contrast, is straight and to the point.

“Am I doing it right?”

The ‘it’ is grieving. For many, the way they’re grieving is natural and healthy.

However, the intensity of grief is often unfamiliar and disorienting for people. They don’t feel ‘normal’. Many ask how to get over it.

Grief is not time-limited. The work of the grieving process is really about making adjustments and learning to live with the loss.

Often, this is a tough realization for newly bereaved people. They’re in pain and distress and want to feel better. A part of them knows that they will always miss the person who has died. Grieving is about learning to redirect our energy and rediscovering what remains meaningful and worthwhile in life, even though that person has died.

A visual I often draw on is the ‘flu metaphor.’

Imagine a jar filled with pebbles, the jar representing your body and the

pebbles the symptoms. When you get the flu, the symptoms take up all of your space. As you get better, the jar empties.

Many people expect grief will resemble this. However, grief is an adaptation process.

The symptoms don’t disappear; you learn to live with them.

After loss, grief fills your jar, but as we grieve, we make adjustments to the loss — we grow. The jar becomes bigger, but the pebbles — representing the grief we carry — don’t disappear.

Once we have learned how to live with loss, we notice how our jar has gotten bigger, even though the grief is still there.

When grieving, it’s important to listen to your instincts. If you feel sad, allow it. If you are lonely and want company, reach out.

Grief is different for everyone.

Pay attention to the time and space your grief takes up. If you spend most of your time alone, feeling too full of grief to socialize, it may be helpful to check-in with your family physician or a grief counsellor.

Wireless technology and hearing aids

A

s technology advances, so, too, do the options for the hearing impaired. One innovative piece of gadgetry is sophisticated wireless hearing aids that are compatible with personal electronic devices.

Thanks to Bluetooth technology, hearing aid manufacturers are harnessing this science to expand on the abilities of hearing aids and to develop new products. In the past, in order to use an MP3 player or mobile phone, a person would need to remove traditional hearing aids to accommodate a pair of earbuds. Some hearing aids have had their limitations in filtering ambient noise.

Although Bluetooth-powered hearing devices are still in their infancy, Oticon, a global hearing technology company, has developed their own communication and entertainment solution called ConnectLine that enables people to connect hearing aids with different devices.

This transforms hearing aids into a personal wireless headset for listening to music, watching television or

video chatting.

Companies such as ReSound and Phonak offer Bluetooth-enabled devices. Their devices may be compatible with Android and iPhone models, some of which may only require an app or direct connectivity to the device. In fact, Apple has patented a specific Bluetooth connectivity with certain hearing aid manufacturers.

Others may require an accessory of some sort to make connections possible, especially when pairing with a TV or other audio device.

Thanks to cutting edge technology, assistive listening devices have improved considerably.

Individuals should speak with their hearing care professionals about their daily needs concerning hearing aids and explore the options in wireless hearing aid technology that can work seamlessly with their devices.

n today’s hectic world of juggling family, career, school, social commitments and everything else we have on our plates, I can promise you, death does not usually come at a convenient time.

It happens when it happens, whether or not you are prepared. You will now need to take the time, and often make the time, to deal with everything there is to do. And on top of that, you are also grieving.

It is important to note that when a death occurs, to take your time, surround yourself in support and let your community funeral provider assist with the necessary steps that need to be taken.

This emotion requires support and guidance from those you love and trust.

Sometimes overlooked are the members of your community who are also in need of support. Community support and participation is as important in death as it is in all oth-

er major life events where we celebrate and come together. This gives permission for friends, colleagues and acquaintances to participate in the tributes and share stories of who that person was to them.
