Welcome! With an aim to improve road safety for all, the Powered Wheelchair Mobility Scooter retailer training scheme was officially launched last year in the House of the Lords and has been successfully embraced by retailers. Turn to page 22 to get a taste of what to expect on the course and the feedback it has had so far. THIIS also asked some retailers to recommend their favourite walking aids for the Retailers’ Choice feature (p10). For a more comprehensive round-up of the best products on the market, from all-terrain rollators to snazzy walking sticks, turn to the Buyers’ Guide (p42). In Trade Talk (p17), Andrew Stevenson, a board member of the Wheelchair Alliance, discusses the group’s aim to improve wheelchair provision and help drive change in the sector. In this month’s Supplier Spotlight feature (p28) we hear from Greg Whelan, Product & Marketing Director at WellEll, who discusses the firm’s plans to widen its pressure area care offering. Meanwhile, in the latest article in the Let’s Get It Clear series (p36), Dr Barend ter Haar of BES Healthcare looks at why getting a client involved as much as possible with the prescription process of mobility equipment is vital. In addition, retailers looking to pass some savings on products to their customers should check out the latest Discount Club (p62) offerings. As ever, do get in touch if you have any news from your business to share with THIIS Magazine readers, whether it’s an important milestone or a new showroom.

Motability Scheme teams up with Team BRIT to support disabled drivers The Motability Scheme has joined up with Team BRIT to support its mission in providing a level playing field for disabled drivers. Around half of Team BRIT’s drivers are customers of the Motability Scheme which helps disabled people to get mobile by leasing a car, scooter or powered wheelchair. The partnership will also directly support the team’s youngest driver, 19-year-old Noah Cosby from Towcester. Three years ago, Noah was on his way towards a career as a motocross freestyler when he misjudged a jump, breaking his back in the fall. In 2023, he signed for Team BRIT, which has created innovative hand control technology to enable disabled drivers to compete on equal terms with able-bodied drivers. Last year, Noah obtained his race licence and competed in the Citroen C1 Series, learning more about the sport and honing his skills. Now, thanks to the partnership with the Motability Scheme, Noah will move up to the Britcar Trophy Championship, driving a BMW M240i with his teammate Asha Silva, also from Northampton.

A first-of-its-kind power mobility chair, designed to attempt a solo ascent of Mount Snowdon in 2024, has been unveiled by engineering firm Rock Engineering and disabled adventurer and war veteran Nick Wilson. The chair, dubbed ‘RockClimber’, was launched at Rock Engineering headquarter in Towcester, Northants. Through the challenge of conquering Snowdon unaided, wheelchair user Nick says that he wants to show in the most extreme way the everyday challenges that wheelchair users face. The project aims to demonstrate how inclusive engineering and technology can be used to improve accessibility in urban and rural environments, enabling people reliant on wheelchairs to have greater independence without the risk of falling or getting stuck. The next stages will see Rock Engineering and Nick start to test the capabilities of RockClimber, before tackling Mount Snowdon in May.

Walking CHOICE aids THE RETAILERS’

Wondering which walking aids are grabbing mobility companies’ attention? This month we asked retailers in the industry to highlight one product that is standing out for them…

Walking sticks from Classic Canes “One of our favourite and most popular walking aids is the range of Classic Canes walking sticks. “Classic Canes is a family-run business nestled in the heart of Somerset, England. Since their start in 1982, Classic Canes has been a global purveyor of exquisite walking sticks, folding canes, umbrellas, and seat sticks, reaching retailers across more than 40 countries. “The sticks are renowned for both style and quality, redefining the perception of walking aids. Their range of walking sticks is as far away from the standard grey NHS walking stick as you can get. “The collection boasts stylish, elegant, and vibrant walking sticks making a real statement. In fact so much so, that they are also seen as a fashion accessory as well as a walking aid. “At Ability Superstore, we proudly showcase Classic Canes’ diverse range, featuring a delightful array of canes that cater to many varied tastes including folding and nonfolding walking sticks, wooden canes and sticks with different shaped handles. “In fact, the range is so comprehensive that there’s a style to fit most tastes and our customers love them!”

Taurus H Basic from Topro Mobility “My choice for this category is this specialist walker from Topro. It goes well beyond being just a walking aid and can be part of a rehabilitation program to assist with upper body support and maybe assistance to rise from a seated position. “It can be either manual or powered in its operation and can carry accessories such as an oxygen bottle or infusion bag holder. “The central column support makes access easy for clinical staff and this may well be where this product excels most, in a clinical setting.” Julia Phipps Director at The Ability Superstore www.abilitysuperstore.com

Wheels in motion for change The Wheelchair Alliance plays a valuable role in representing wheelchair users and ensuring they get the right chair for them when they need it, as Andrew Stevenson explains… The Wheelchair Alliance is a community interest company that represents the community of wheelchair users in England. It was established in 2023, when a revamp and re-invigoration of the previous Wheelchair Working Group was requested by NHS England. Within this restructure, the British Healthcare Trade Association (BHTA) was asked to nominate an individual who would sit on the management board of the Wheelchair Alliance to represent the views and interests of member companies involved in the supply of products and services to the wheelchair sector. To avoid any commercial conflict of interest, I was

asked as President of the BHTA, to take up this voluntary position and did so in early 2023. The management board is made up of a diverse range of members, including wheelchair providers, representatives from the public, private and charity sectors and wheelchair users. Alongside the board, there are working groups being formed to drive other key topics forward, such as innovation and the use of new technology. As the alliance moves through this year other specialists within the wheelchair sector will be called upon to bring their skills and knowledge forwards, so that the main driver of the Alliance, which is ‘to

provide the right chair, at the right time, right now,’ can be achieved more times than currently. RESEARCH THAT DRIVES CHANGE In May 2022 the alliance produced its first research report, sponsored by the charity Motability, which oversees the Motability Scheme. The aim of the report was to acknowledge best practices, and discuss ways of improving oversight, data and equitable provision. This report produced some significant interest and engagement activity across the sector, and in December 2023,

thiis.co.uk | Trade Talk | 17

the alliance’s second report on the true value of increased investment in wheelchairs showed a societal return of up to £315 million in savings being released back into the NHS. The detail in these reports is the best the sector has seen in decades. The Wheelchair User Engagement Group is now some 145,000 membersstrong, and the aim is to expand this into a National Advisory Board with representation on the Alliance Board and Quality Framework Group. Plans are well advanced for an AllParty Parliamentary Group (APPG) to be formed for wheelchair provision, led by the Alliance and supported by Whizz Kidz, the UK charity for young wheelchair users, to further drive the alliance message into UK Government and stakeholder groups. JOIN THE WORKING GROUP Joining the Wheelchair Alliance Working Group provides members with direct links and access to the key areas that the alliance will be developing in 2024. The two-way flow of information, experience, and future planning is crucial to the health and development of the alliance. It is vital that the voices of wheelchair service providers are heard and their skills, experience and knowledge are represented in the ongoing development of wheelchair provision in the country. The link that the BHTA has with the Wheelchair Alliance is a strong and effective one. The voice of our members with their experience and skill in providing best-in-class products and services will now be heard. BHTA members account for over 40 per

18 | Trade Talk | thiis.co.uk

cent of wheelchair service provision in England and over 80 per cent of the product lines provided to wheelchair users. At the BHTA, members are currently working on quality and best practice frameworks, and its paper Lithium Battery Safety Guidance generated a lot of interested when it was distributed across the Wheelchair Alliance network. Its members have also been supporting an Innovation Special Interest Group which aims to fast-track innovation on products or services that better impact the user experience and value. The BHTA has also launched a Retailer Engagement Group to bring this key aspect of wheelchair provision into the alliance. These companies supply and support significant numbers of users and can greatly impact on the development of a best-in-class service across the country. The alliance needs to increase awareness of its work, to ensure that all wheelchair users and their families and care networks know that we are here to get their voice across. The recent reports produced by the Alliance have a 10-point Action Plan and these will be further expanded and developed in 2024. All this of course costs money, and funding of the alliance is a vital component that the BHTA is supporting through its members. The steps that I have seen taken in the last 18 months have been amazing. After years of neutrality, the voice of the wheelchair user is now at the forefront of the Wheelchair Alliance’s mission statement. If we as an industry are to see a more efficient, stronger, more innovative, professional and caring provision of products and services in future years, we have to work hand-in-hand with all stakeholders. We have the ability to substantially impact the future provision of wheelchairs to the most important piece of this chain – the user. The right chair at the right time, right now must be the norm. Companies working in the mobility and access sector can join the Wheelchair Alliance Working Group by making contact with me by calling 07720 936770 or emailing grosvenorhealthcare@gmail.com. www.wheelchair-alliance.co.uk

Did you know?

?

Wheelchair services in England are not working as well as they could be due to inadequate funding and a lack of accurate wheelchair user data, according to the Wheelchair Alliance. Its research found that Government funds spent on wheelchair users equates to £125 per person per year, including the cost of a wheelchair, expert support as well as infrastructure costs. The alliance argues that this level of underfunding has remained the same for at least five years. If the costs of energy, equipment and staffing are all rising, this figure needs urgent review for wheelchair support to be protected. Adequate funding, the alliance states, also requires accurate wheelchair user data so families and carers can be confident that they will be treated equitably wherever they live. This means wheelchair services must offer regular data based on outcomes to ensure accountability and reduce regional differences.

Andrew Stevenson sits on the Management Board of the Wheelchair Alliance. He is President of the BHTA and also owns a specialist healthcare consultancy business, having worked for the last 25 years in board and senior management roles across the healthcare sector. Andrew also represents the BHTA on the NHS GS1 Barcode Working Group.

Putting road safety first

One of the latest PWMS Retailer Training Schemes in action at RDAC Solihull Driving Mobility centre

Driving Mobility’s training scheme is receiving a positive reaction from responsible powered wheelchair and mobility scooter retailers nationwide… With support from the Department for Transport (DfT), Driving Mobility is successfully providing its free training scheme for powered wheelchair and mobility scooter (PWMS) retailers and hirers which aims to improve road safety for all. The voluntary PWMS Retailer Training Scheme aims to improve road safety through an approved and consistent way of providing client assessments to ensure people with mobility difficulties are able to obtain the most appropriate mobility scooters or powerchairs. The scheme is operated by Driving Mobility, a charity which accredits a network of over 16 ‘fitness to drive’ and mobility equipment assessment

centres across the UK. Many with outreach facilities, these centres include independent charities and NHS departments which provide clinical guidance, advice and training so disabled and elderly people can gain or retain independence. With the use of powered mobility equipment increasing in the UK, Driving Mobility ensures products are matched more effectively with customer needs. With this first-ever DfT-supported training programme led by clinicians from Driving Mobility, retailers can combine new skills with their own product expertise, so consumers are able to drive more safely on well-suited products. The free training consists of online

modules and a practical session at one of the designated Driving Mobility centres across the UK.

WORKING IN PARTNERSHIP Through the training scheme, retailers and Driving Mobility centres are working closer together and building stronger relationships. This means when retailers have clients who would benefit from a more in-depth clinical assessment, they can be referred to a Driving Mobility Centre and then return to the retailer to purchase the bestsuited equipment. Hosted by Driving Mobility, the scheme was launched in the summer of 2023 at a House of Lords reception. The reception was attended by leading representatives within the mobility equipment retail sector, assessment centre managers, representatives from ShopMobility UK and figures in the public eye including Baroness Tanni Grey-Thomson; Margot Shatz, Department for Transport; and

Raymond Hodgkinson MBE. Since the launch there have been several well-attended training events held at Driving Mobility centres around the country. These have included Cornwall Mobility, Truro; RDAC, Solihull; East Anglian DriveAbility (EAD), Norfolk; and QEF Mobility Services, Carshalton. More dates are continually being added to the 2024 programme (see boxout, p26). RETAILER FEEDBACK At last October’s training session hosted by RDAC Solihull, retailers gained valuable insight including Sarah Caton, Director of Cowper’s Mobility Centre. Sarah said: “The PWMS Retailer Training Course was not only interesting, informative and a good opportunity to network with likeminded retailers, it was also pleasing

to see that the course content backed up everything we, as a responsible mobility company, strive to achieve.” Stacey Showering, Managing Director, Care Plus Mobility, reiterated Sarah’s comments by saying: “Great course and a real privilege to network with like-minded, committed professionals. The outcome of me applying the course content to my

clients will enhance their skills, fostering safety, confidence and hopefully a more enriched active lifestyle.” And Mary Eardley, of Mobility Britain, added: “As the only online retailer at the event, I would like to thank the Driving Mobility team and the other retailers, for making me feel so welcome, and for sharing their experiences and good advice. The training day has reinforced the importance of safety, particularly when selling remotely, and has positively impacted my practice.” Retailers attending the subsequent PWMS training at East Anglian Driveability (EAD) were equally as impressed, as Moddy Grazys, Mobility Aids Centre Peterborough, commented: “Having the simulation glasses helped me understand how different conditions

can affect how you ride a mobility scooter. It was very helpful to know that even in the Peterborough area we can refer customers to EAD.” Lisa Clarke and Linda Best of Felgains highlighted two key considerations they learnt: “The importance of getting clients to drive outside more during the assessment and how conditions such as a stroke can affect eyesight.” Overall, the course was described as “a real eye opener.” QUALITY TRAINING There are two modules that can be accessed via the online training platform Moodle. These two online modules can be undertaken at work, or from home, whichever is preferred. Following the successful completion

Mary Eardley, Mobility Britain

of these online modules, participants are then invited to attend their nearest Driving Mobility centre for half a day. Attendees undertake desk-based sessions and hands-on product training which aim to promote customer assessment good practice. Retailers and hirers who complete the course receive a certificate and eyecatching window sticker. These provide a clear indication that the business has completed training that is officially accredited by Driving Mobility and the Department for Transport. Their customers are then given confidence they are purchasing mobility scooters or powerchairs from a reputable retailer who has received clinically-led training for better assessment of their needs.

Mary Eardley, Mobility Britain, discussing how to supply the right powered wheelchairs

SUSTAINABLE SAFETY The retailer training aims to improve road safety for all – mobility scooter and powerchair users, pedestrians and vehicle drivers. The programme is supported by the Government which works in conjunction with Driving Mobility on many aspects of personal mobility and independence. In summary Richard Holden MP, Roads and Accessibility Minister, said: “It’s really important that we improve road safety for everyone, including owners of powered wheelchairs and mobility scooters. This scheme, supported by Government, will help to ensure that people with restricted

mobility can get around independently and confidently by training users, and I’m pleased to support it through our funding.” Edward Trewhella, Driving Mobility CEO, concludes: “Alongside the Department for Transport, we are aware that the way people are travelling is changing, especially with increased ownership of mobility scooters and powered wheelchairs. Hence our training scheme for retailers is timely and promises to enhance road safety for all, through more suitable mobility vehicle provision and greater awareness of the Highway Code.” The PWMS Retailer Training Scheme is free and can be booked here.

CAN YOU SHARE WITH OUR READERS WHAT YOUR ROLE AT WELLELL INVOLVES? “The role is split down into two equally important and complimentary areas. Firstly, the product and how we assess the market and its needs, relaying this back to our colleagues at Wellell Headquarters. Secondly, I focus on activity in terms of brand and product promotion throughout the UK, in both traditional and digital media and events.”

“The accelerated success of the Optima Turn mattress was also a highlight and how it has improved so many patient outcomes”

WHAT WERE THE HIGHLIGHTS FOR WELLELL IN THE UK IN 2023? “Collaborating with HQ and Subsidiary partners, to compile and film user stories from around the world on how our products have helped carers and patients to live well. “Another was exhibiting at a recordbreaking 13 shows in 2023, and the opportunity this provided in meeting so many new customers and reconnecting with long-standing clients. “The accelerated success of the Optima Turn mattress was also a highlight and how it has improved so many patient/user outcomes.” HAVE THERE BEEN ANY PROJECTS YOU HAVE BEEN MOST PROUD OF? “It’s hard to separate out projects in marketing as they are all so intrinsically linked together. The key area has been the User Story video, as you hear from those affected most, the difficulties that they face and how our products have made huge changes to peoples lives in terms of the patients, caregivers and the affected families of those people. “When you are privileged to make those changes to people’s everyday existence then it makes you feel humble and equally proud of what Wellell as an organisation achieves through its products on a daily basis.” CAN YOU DESCRIBE THE LATEST NEW LAUNCHES TO COME OUT FROM WELLELL AND SUMMARISE THEIR MAIN FEATURES? “For 2024, we’re widening our pressure area care offering. We have helped over 3,000,000 patients worldwide on their recovery journey, and the broader range will allow us to provide excellence that doesn’t have to cost more. With the current challenges faced in the market, particularly economically, we really value the opportunity to present products with uncompromising performance at an affordable price. “Our designs revolutionise patient comfort with advanced digital technologies that reduce the burden on staff and extend the product lifespan. By increasing the product range, we can present a solution for the unique

needs of even more patient profiles and the organisations that offer their services into the community and postacute sector.” WHAT KIND OF RESPONSE HAVE YOU HAD TO THESE FROM DEALERS? “The response from dealers has been very positive. Being able to provide quality products with effective pricing is always valuable. I think dealers have seen the value we can provide in both support for the product and in the pricing to ensure that everyone has access to our most quality products.” ASIDE FROM NEW PRODUCTS, WHAT PRODUCTS CONTINUE TO BE BESTSELLERS? CAN YOU DESCRIBE KEY FEATURES AND YOUR THOUGHTS ON WHY THEY ARE A SUCCESS? “At Wellell, we focus on pioneering pressure injury prevention. We offer fundamental solutions with advanced features at an affordable price, such as our Verso and Domus ranges, but we also excel in providing enhanced care to the more acute sector, with our Optima range. “Our Optima Turn turning mattress, one of our Optima Series products, provides automatic and timercontrolled turning up to 30 degrees supported by alternating pressure, multiple pressure relief therapy options and manually deflatable air cells for heel relief care, all easily accessible through the intuitive pump interface, streamlining patient handling and caregiver workload. “The success in our products comes from our proprietary designs

and technology. It’s difficult to find an alternative to our Optima and Domus products that can provide the same level of care and quality, so we trust that our product will be appreciated by users.” HOW DO YOU THINK TECHNOLOGY IS EVOLVING IN PRESSURE AREA CARE AT THE MOMENT? “This is an area that requires increased focus from the industry with changes in technology being one of the key areas for manufacturers to concentrate on. Pressure area care is an incredibly complicated area with many factors affecting the individuals “at risk” status. “This is where technology will need to help, at the stage where a pressure injury (PI) hasn’t yet developed. Herein lies the challenge as morbidity and mortality rates can be reduced if PUs are managed at a very early stage or even prevented completely. “The industry in general is working hard to design new ways to identify those at risk and I think that the next few years will see more and more smart technology emerging.”

“We value the opportunity to present products with uncompromising performance at an affordable price”

ARE YOU LOOKING TO EXPAND YOUR DEALER NETWORK, AND IF SO, WHAT KIND OF DEALERS ARE YOU LOOKING FOR? “We want everyone in the UK and Ireland to have seamless and efficient access to our products, so we’re always looking to include reputable dealers with a like-minded approach into our dealer network. Wellell were originally based in the south of England covering up to and including the Midlands, so we have a substantial incumbent network there. “For 2024, we’re looking to increase our footprint in the north of England, where local dealers will be able to access our products who may not have previously done so.” WHAT KIND OF SUPPORT DOES WELLELL OFFER FOR DEALERS WHO COME ONBOARD? “At Wellell UK, we’re committed to our customers’ success by building long-term partnerships and delivering industry-leading levels of customer satisfaction, providing value, trust and innovation. “Wellell are conveniently based in Worcester in the Midlands, and we have a fully outfitted training room. Dealers can use this training space to provide Wellell supported training to their customers, or alternatively, we are also able to come out to dealer locations to provide training on site. “We also have significant warehousing space, as well as access to product from Europe on a prompt basis, so dealers can expect full The Optima Turn in-situ, with the turn function being used by a care worker to assist in checking for early signs of pressure injury

support with meeting their orders on time and in full. This includes a guaranteed next working day delivery of in-stock items. “Our customer surveys show that we have a 95 per cent customer satisfaction rate, and I believe this tells you everything. Direct feedback from our customers is so important to us, and we encourage this to ensure that we’re providing the ultimate in both our products and our service.” CAN YOU SHARE ANY FURTHER PRODUCTS THAT WE CAN EXPECT FROM WELLELL IN THE NEXT YEAR? “We are working on a number of new products for 2024 and into 2025 as well as the final stages of our Apex to Wellell Global rebrand. The enhanced product range will soon be available to see in our new Wellell catalogue due out very soon.”

WHAT DO YOU SEE AS THE MAIN CHALLENGES FACING YOUR SECTOR? “With social care budgets being reduced the availability of funds for social care is a concern for those who require that service and those supplying into. Couple with that the issues facing staffing and recruitment, and an ageing population, means that the industry is going to be asked to offer cost-effective and creative solutions to help bridge the funding gap. “Herein lies the challenge of producing equipment that is cost effective in patient management, whilst reducing the workload on the caregivers in the acute, post-acute and community sectors.” ARE THERE ANY OTHER PLANS FOR WELLELL OVER THE NEXT 12 MONTHS? “Wellell are always planning exciting and interesting new innovations and approaches to the market. As part of an international business, Wellell UK will be building throughout 2024 on the 2023 successes in the UK and throughout our European regions by bringing the best of each region to benefit the whole organisation. “We have recently placed additional strategic individuals at European level to build our presence across the whole Pan European Region benefitting the UK and each of our subsidiaries. Wellell are investing heavily in the people and products that will ensure a successful and innovative 2024 and beyond.” uk.wellell.com

Appearance matters Client compliance has often been a challenge for many clinicians. In this month’s article we look at some of the reasons for this, with a number of case studies illustrating examples of when a solution that has been set up in the clinic with every best intention, often falls apart in the real world. How well can we put ourselves into the positions of our clients, and enhance their endorsement of the solution? Life is a real pain. Our clients do not sit in 90-90-90 text book positions, they are not symmetrical, their ‘comfortable’ lifestyle seated position can lead to increased skeletal deformity, they may not like the look of the equipment that we have prescribed which we know will benefit them, and their support staff haven’t been trained around the correct set-up and use of prescribed equipment, or, perhaps worse, the equipment is sitting in a corner unused. Over and above this, there are circumstances back home which can never be anticipated in the clinic or showroom, or even when an assessment has been made in a person’s home. What can be done to improve this? GAINING COMPLIANCE Having the client involved with the prescription process as much as

possible is vital. With the ‘help’ of the internet many clients may be very well informed, and may have arrived having done quite a bit of research, and think they know what they want – or they have been long-term equipment users and have researched suitable solutions from their peer groups. This at least provides a base to work from although it may present different challenges! The opposite is generally true, however. The majority of people who first meet a mobility challenge have no idea where to start. Through the NHS they may be pointed towards a wheelchair service, and then, depending on local policies, the individuals may or may not meet local criteria for equipment provision. At that point they are often unaware of personal wheelchair budgets, or of the possibility to top up their provision. At the recent UK Stroke Forum conference in Birmingham, one of the practicing physiotherapists whom I

met was unaware of these options, so if NHS staff are unaware, what chance is there for the ‘man on the street’? But appearance matters as well. Traditionally, many mobility devices used to be made principally of chrome covered steel. Things changed with

the advent of aluminium frames, and some choice of colours. But still most ancillary equipment is predominantly black, or at least the chosen NHS provision is black to enable matching and recycling options. After years of persuading, Bodypoint now uses teal-coloured thread as a highlight colour on a number of their positioning products, which gives that extra touch of refinement (Figure 1). Symmetric Designs offers different colours for the backs of their Apex head supports (Figure 2)1, which provides an uplifting personalised highlight. Another idea: for those with chest harnesses, how nice would it be if there were detachable covers, for example, to match the clothing choice of the day. Roho did offer a choice of colourful covers for their cushions at one time, but unfortunately these did not have a great take up, since this meant that Figure 3. Disney-approved wheel covers from Izzy Wheels

dealers would need to carry increased inventory, and black was always going to be the ‘safe’ option. PIMP MY RIDE ‘Pimping up’ one’s chair is a way to get a product looking less like a disability device, and there are various options available. Wheel covers, such as Izzy Wheels’, have been one option (Figure 3)2. As quoted from a report in Vogue: “Izzy Wheels is bridging the gap between fashion and disability, all while armed with creativity and an inclusive message that’s hard not to be inspired by”. Tarta has pioneered the opportunity for client choice from the start, by offering customers to have Tarta’s unique style of back supports supplied with customised finishes (Figure 4). Other personalisation options have been tried (Figure 5)! CAN ASSESSMENT TOOLS HELP? Pressure mapping can be a useful tool for gaining client understanding and acceptance. For a start, pressure mapping has been shown to provide better clinical outcomes. Data from a cushion study showed that on clinical judgement on its own, 21 per cent of clients developed pressure ulcers, whereas pressure mapping combined with clinical judgement and use of a pressure mapping device led to only nine per cent developed ulcers3. In another study, in an ICU, with no use of pressure mapping: out of 320 patients: 16 developed Stage

Figure 4. Examples of customised Tarta back supports

2+ pressure ulcers. With the use of pressure mapping: out of 307 patients: only 1 developed a Stage 2+ pressure ulcer4. At a meeting some years ago, I was discussing the use of pressure mapping with a group of female therapists from a leading US spinal injuries clinic. Their comment was that their clients (mainly men) tended not to be very compliant, but as soon as they showed their clients data from their seating evaluations with a pressure mapping system, the clients were much more compliant, since they believed what they saw on a computer screen much more than what they were being told! In another situation, a wheelchair clinic had a client, a paraplegic, for whom they had supplied a top level ultralightweight manual chair, and top

level cushion, but he still came back repeatedly to clinic with tissue damage on his buttocks. In desperation, they placed a pressure mapping device under his cushion and set it to take regular readings day and night. When he came back to the clinic, they found that between eight and 11 each night there were no pressures showing, so they asked the client why. “Oh yeah. I go down to the pub every night, and leave my chair behind the bar, and sit on a bar stool with my mates.” I rest my case, m’lud. BEHAVIOURS BEYOND OUR CONTROL This case demonstrates an example of matters that we can not necessarily foresee or anticipate. Another couple of such cases were described by Filipe Correia during his autumn 2023 roadshow with BES Healthcare. One case was that of a young girl in a wheelchair who at meal times was sitting in a strange position with her head twisted away from the midline, and it was therefore difficult to feed her – and this was not how she normally sat in the chair. Eventually someone realised that there was a television always on in the background, almost as a kind of wallpaper. This was being ignored by all the adults, but not by the girl who was twisting her head to a position where she could watch the screen. Her wheelchair position was moved to enable a clear view of the TV, and the problem was solved! Another case was that of what was seen as a surly teenage lad for whom his updated chair and seating appeared to offer an excellent postural solution, but his mother was devastated as he had apparently suddenly started slouching in his seat and hiding his head in his lap. It turned out that the trigger for this change of behaviour had been entirely unrelated to the updated seating solution, but a trip to the orthodontist! As part of the required intervention, a brace had been fitted: the youngster was very self aware of this change and did not want people to see the brace. Gentle reassurance resolved the issue.

Figure 5. A wheelchair does not need to look like a mobility device

One needs to be aware that behaviour in the clinic may also not represent behaviour beyond the clinic. Many clients do strive to please their clinical adviser and probably sit up in their chairs and seating systems ‘correctly’ in the way they perceive their adviser wishes. While being pressuremapped, if they can see the screen, they may try to adjust their posture to give the picture they think is required. This can be a valuable education tool, to show what happens if they move away from the optimal posture. Conversely, a useful tip here is to go off and make a phone call or cup of coffee, and leave the client on their pressure mapping system for a while. The client will resort to the posture that they feel most comfortable with when on their own – legs crossed, arm hooked around the back of the chair, or whatever. Despite your best efforts in the clinic, this is what they will resort to when they leave, and so it’s useful to work with this posture while the client is in the clinic. APPEARANCE MATTERS What we’ve tried to address in this article is the importance of thinking outside the traditional box – client input and acceptance into solutions that can impact on short- and long-term acceptance. The principles of ‘Universal Design’ are important, and every effort should be made to avoid offering equipment that is assistive but where the design gives a visual ‘disability’ message. When a solution created in the clinic does not work, what are the reasons? Is it that it was not suitable for their normal day-to-day activities

for that client? Are there client-specific behavioural matters to be checked out? Or is simply that the clients, or more specifically their carers, have had little or no education or training in how the prescribed equipment should be used – an all too common a situation around the world5?
