I understand that if this request is approved, financial assistance will be contingent upon: successful completion of the NIMAA Program, in full; passing the National Healthcareer Association CCMA examination and obtaining CCMA certification; and, maintaining employment with TWCGME from the present time and for at least two (2) years following the date on which I obtain CCMA certification following completion of the NIMAA Program. The two (2) year period does not include any leave of absence period for any reason (including but not limited to FMLA) in excess of/other than regular use of PTO. If I fail to satisfy any of the above stated requirements, including by not remaining employed by TWC from the present and for at least two (2) years, as described, regardless of the reason for separation or non-employment, or if I voluntarily change my TWCGME employment status to part-time (working less than 20 hours per week) within the two year period, I understand and agree than I shall be responsible to repay to TWCGME the total amount of Financial Assistance paid by TWCGME to me or on my behalf in connection with the NIMAA Program. Any such reimbursement amount due by me may (but is not required to) be offset/ deducted by TWCGME from any remaining pay (to include the last paycheck) due to me, as applicable, to the fullest extent permitted by law, as determined by the TWCGME in its sole discretion. I hereby agree to and authorize such payroll offset/deduction. I also agree to pay any Financial Assistance reimbursement amounts due, including if applicable any amounts not satisfied by offset/deduction, on or before the last day or employment, if applicable, or within ten (10) days by demand from TWCGME. TWCGME reserces the right to take any and all lawful actionand pursue any and all remedies to recover any reimbursement amount due from me.
