I am pleased to share with you The Weld Trust’s first ever annual report. Our mission is to promote excellence in health and education in Weld County. The Weld Trust provides grants to nonprofit organizations, schools, and government agencies located in Weld County or are exclusively for Weld County residents. In 2020, we awarded grants totaling more than $8.839 million using four funding priorities identified by our Board of Directors to 27 nonprofit organizations across Weld County. • indigent healthcare • chronic disease • behavioral health • education The Weld Trust also provided grants related to COVID-19 needs to 38 nonprofits and schools in Weld County. These grants allowed the critical services offered by these important agencies to continue through the pandemic. The COVID-19 grants totaled more than $286,000. In addition to the grants awarded, The Weld Trust is proud to sponsor events and fundraisers for nonprofit agencies whose work is focused within our mission. The Weld Trust supported 25 nonprofit events totaling $174,250 throughout the year. The Weld Trust is undergoing exciting changes in 2021. We are currently constructing our first strategic plan. Through this process, we have connected with many nonprofit partners, schools, government officials and key stakeholders to identify the funding priorities in the areas of health and education for The Weld Trust to focus on for the next three years. When the strategic plan is complete, I look forward to sharing it with you. At The Weld Trust we appreciate working together with the Weld County community to make a difference in your lives. Our History The Weld Trust,® formerly known as NCMC Inc., was formed in 1985. NCMC Inc. was created to assume the responsibility of the day-to-day operations of Greeley’s hospital, North Colorado Medical Center. In 2019, NCMC Inc. sold the buildings, land and equipment to Banner Health. Later that same year, the name was changed from NCMC Inc. to The Weld Trust.® Mission Statement: To promote excellence in health and education in Weld County.

Weld County opens Greeley Hospital.
Greeley Hospital moves to its current location on 16th Street in Greeley. The hospital name is changed to North Colorado Medical Center. NCMC Inc. is formed, a Colorado nonfor-profit corporation qualified under Section 501(c)(3). Responsibility for day-to-day operation of the hospital is transferred to NCMC Inc. together with the lease on the county land and facilities. Lutheran Health System is contracted to run the dayto-day operations of North Colorado Medical Center. Lutheran Health System merges with Good Samaritan Health System to form Banner Health. Banner purchases the land, buildings, and equipment from NCMC Inc. for $329.4 million. NCMC Inc. changed its name to The Weld Trust.® Its mission is to promote excellence in health and education in Weld County.

Banner Health Banner owns Greeley’s largest hospital, North Colorado Medical Center. In 2020, Banner provided $47.8 million in community benefit and care for underinsured and uninsured consumers. The Weld Trust values the importance of providing healthcare funding for those individuals who cannot otherwise afford it. Therefore, a substantial grant was provided for indigent healthcare and other health programs. 6
Catholic Charities of Weld County Catholic Charities provides the Hospital to Housing program at the Guadalupe Community Center. The program helps transition patients identified as homeless to permanent housing once their medical conditions have stabilized. The grant from The Weld Trust supported program costs.
Greeley-Weld Habitat for Humanity The Greeley-Weld Habitat for Humanity provides affordable housing opportunities for low-income families. Habitat homes have been built throughout Weld County including Greeley, Evans, Eaton, Milliken and Hudson. The Weld Trust grant went to underwrite the cost of building Habitat homes.
Lutheran Family Services Rocky Mountains Lutheran Family Services offers assistance to refugees and immigrants in Weld County through client advocacy in relationships with health service providers and transportation to medical and mental health appointments. The grant from The Weld Trust provided support for their program.
North Colorado Medical Center Foundation The NCMC Foundation provides the Med-Aid Prescription Assistance program, which offers prescription assistance to patients with an urgent medication need without an ability to afford it. The Weld Trust grant provided for this program, as well as toward scholarships in the medical field.
60+ Ride The 60+ Ride program provides volunteer driver transportation to health, wellness and social appointments in Weld County for persons aged 60 and over at no charge. The grant from The Weld Trust provided support to their program.
United Way of Weld County United Way of Weld County provides a wide array of human services including housing stability. The Weld Trust grant assisted with personnel and program costs associated with the Community Health Worker and Housing Navigation Center programs which work with Weld County’s most vulnerable households.
Weld Food Bank The Weld Food Bank provides a Mobile Food Pantry Program to supply nutritious food across Weld County. The Weld Trust grant allowed the Weld Food Bank to purchase a specially designed Mobile Food Pantry truck to assist with the outreach to rural Weld County communities and school districts.
Genesis Project of Northern Colorado* Genesis Project is a residential home for single moms and their children. Their participants have been exposed to one or more at-risk factors including addiction, abuse, poverty and homelessness. The Weld Trust provided housing for four single moms and their children in December 2020 resulting from broken pipes in their existing home.
Sunrise Community Health* The Weld Trust owns the land and building that Sunrise Community Health in Evans occupies. Each year, The Weld Trust donates the use of this 60,000 square foot facility to Sunrise Community Health which has a value of approximately $500,000.

Adeo Adeo provides programming for survivors of brain injury which offers affordable, accessible housing along with 24/7 staff support. The grant from The Weld Trust assisted with housing costs for their clientele who are either homeless or at imminent risk of being homeless.
American Cancer Society The American Cancer Society Greeley office provides free lodging and transportation for Weld County residents to treatment, 24/7 access for cancer related questions, health grants to help rural patients access treatment, and local research grants. The Weld Trust grant supported their services and grants.
Dementia Friendly Communities of Northern Colorado Dementia Friendly offers services to individuals living with dementia and their families. They provide life-enrichment opportunities and dementia-friendly education including services via online support groups. The Weld Trust grant supported program costs to expand their presence in Weld County.
University of Northern Colorado Cancer Rehabilitation Institute The UNC Cancer Rehabilitation Institute provides cancer survivors exercise-based rehabilitation via individualized prescriptive exercise with a focus on improving the quality of life of cancer patients by alleviating treatment-related symptoms. The Weld Trust grant supported the Institute’s operating costs.

5 to Thrive Teen Health The 5 to Thrive program is for teens and run by teens. It is focused on reducing teen suicide, bringing an awareness and reducing the stigma around mental health for kids ages 12-18. They created a 5-part self-care model to help teens improve themselves. The Weld Trust grant provided support to their program.

North Range Behavioral Health North Range provides behavioral health and crisis services to individuals of all ages. They also provide suicide education and support services. The Weld Trust grant aided in the expansion of their crisis services in Southeast Weld County, as well as suicide prevention and counseling services county wide.
Sexual Assault Victim Advocate Center The Sexual Assault Victim Advocate Center provides crisis intervention, advocacy and counseling for those affected by sexual violence as well as prevention programs. The Weld Trust grant assisted with their direct services programs including a bilingual hotline, and an advocate for every individual and their family.
The Avery Center The Avery Center provides services for adults experiencing commercial sex trafficking. Their program works to provide financial literacy, education, and workforce development as they escape commercial sexual exploitation. The grant from The Weld Trust was used toward personnel and program costs.
Turning Point Center for Youth Turning Point provides programs for youth and families struggling with behavior, mental health, and substance abuse issues. Their programs include residential treatment and outpatient services. The Weld Trust grant enabled them to buildout the Weld County Outpatient Center to offer their full suite of services in Weld County.

Bright Futures Bright Futures offers the Careers in Healthcare Program which provides tuition assistance in the healthcare industry at Aims Community College, as well as have access to a healthcare career upon completion of the program. The grant from The Weld Trust supports up to 45 scholarships to students pursuing a degree or certificate in a healthcare related field.
Immigrant & Refugee Center of Northern Colorado The Immigrant and Refugee Center of Northern Colorado (IRCNOCO) provides education, community navigation and advocacy to immigrants and refugees. They are currently serving approximately 600 clients from 26 countries. The Weld Trust grant supported their educational programming raising general awareness of best health practices.
North Colorado Family Medicine North Colorado Family Medicine implemented two health and wellness programs, Lifestyle Medicine Patient Program and Lifestyle Medicine Symposium, for their patients as well as the community. The Weld Trust grant supported costs associated with these two programs. 9
Reach Out and Read Reach Out and Read Colorado works with pediatricians to distribute books in 32 different languages and prescribe reading to children aged 6 months to 5 years. Their primary focus is on children growing up in poverty. The grant from The Weld Trust was used to support their program for Weld County clients.
The Success Foundation The Success Foundation partners with the Greeley-Evans School District 6 to ensure students have access to the tools and opportunities needed for success. The grant from The Weld Trust supported their Career Explore Internship Program which is related to opportunities in the health science industry.
Weld County School District 6 Greeley-Evans School District 6 serves approximately 22,000 students. The Weld Trust grant supported students by funding four middle school and four high school Multi-Tiered Systems of Support counselors. These counselors support the socialemotional and academic needs for over 7,100 students.
Weld County School District RE-8 Weld School District RE-8 serves Fort Lupton and the surrounding areas. The Weld Trust grant supported the district with the funding for a Family and Community Outreach Specialist; as well as supplies for the expansion of resources to families experiencing homelessness and other acute needs.
Youth Empowerment Group The Youth Empowerment Group’s PowerUp Girls for Life Program provides education for 4th-8th graders on self-esteem, confidence, positive body image, social media use, bullying, coping with stress, and rejecting substances. The grant from The Weld Trust supported expenses related to the program.
COVID-19 Grants - Total Award: $286,433 The Weld Trust supported 38 nonprofit organizations and schools in 2020 located throughout Weld County with grants for COVID-19 related needs. These grants allowed organizations and schools to comply with the recommended guidelines during the pandemic crisis. Almost Home, Inc. Sponsorships - Total Amount: $174,250 Several organizations rely on fundraising events to support their programs and costs. The pandemic changed the fundraising activities for many nonprofit organizations. The Weld County nonprofit community proved to be resilient and discovered ways to host events even amidst this challenging time. The Weld Trust sponsored 25 events in 2020.
