Get your glow on and run a 5k! This is a LED timed 5k run/walk on a certified course with awards in six age groups. The event will finish with a DJ dance party and snacks in the dark. Finisher medals awarded. No dogs or strollers permitted.

Kids have a blast learning new games, sports or creative activities at ActivKids, our Before and After School recreation program for kids in grades K-5. Kids love ActivKids for the fun and friends and parents love knowing their children are safe, supervised and feel at home in the familiar setting of their own school. Locations

Dog Owners Ages 18+ People Training for Dogs

Dog owners receive the tools needed to raise a balanced and wellmannered dog. In just six weeks, dogs are taught to heel, sit, down, stay, come and stop unwanted behavior. Learn about nutrition, vaccinations and grooming. After completion, owners have option to repeat course with the same dog at no charge. Dogs must be four months old to attend. ActivTime keeps your children actively engaged in learning new activities each month. Monthly Payments Before School - Monday-Friday $166/month After School - Monday-Friday $196/month Before and After School - Monday-Friday $293/month Your monthly payment is calculated based on the days of the week your child is registered for and total school days during the year they attend. Payment amounts already exclude school holidays, breaks and nonattendance days. Total registration is divided into equal monthly payments. EZ pay form required. If your child is registered for less than 5 days per week, your monthly payment is less.

Nonrefundable registration fee of $25 due at time of registration. Before September 1, 2019

First month’s payment is due at registration. All subsequent payments are automatically withdrawn via EZ Pay on the first of each month beginning September 1, 2019 – May 1, 2020.

Flex Pass

Good for five Before or After School Care admissions $65. Flex Passes cannot be utilized for Monday, After School. Flex Passes can be used for either Before and/or After School ActivKids. To use the Flex Pass for After School ActivKids, call your child’s school office and site a minimum of 24 hours in advance to ensure enough staff and supplies. No call is necessary for Before School Care ActivKids. Same day usage is not allowed for After School Care.

Monday After School

Advanced registration is required $20/day. Registration required a minimum of 24 business hours prior to program date. Registration To register for these programs, complete the ActivKids registration forms and submit with payment. Forms are available online, at your school’s ActivKids site, or at the Fountain View or Simkus Recreation Centers. Registration must be within 24 hours of program start date. A minimum of two weeks' advance notice is required if your child needs special assistance from a WDSRA aide (1:1 support staff). More information can be found in the WDSRA section of this guide.

Need a more flexible camp schedule? Only pay for what you need when you register for a Flex Pass! Simply choose the exact dates your child will attend and register at Fountain View, Simkus or csparks.org. Registration should be completed at least two days in advance. Emergency Flex passes are available within two days for an additional $5. A limited number of flex passes are available each day and registration may close in advance, particularly on field trip days. Payment is due at the time of registration. Cancellations are not accepted within two days of the program day.

Hot Lunch

No time to pack a lunch? Kid-approved lunches are provided by Rocco Vinos Ristorante. Meals are offered on a weekly basis; daily options are not available. Menus are available online and at Simkus & Fountain View Recreation Center. Registration and entrée selections for Hot Lunch must be completed at least one week in advance.

Based on the hit film and the true story of Elmore City, OK which banned dancing for 90 years, Footloose is back! When big city teenager, Ren, moves to a small town, he finds that wide open spaces sometimes come with very narrow minds. As the town copes with tragedy, the influential Reverend Moore bans dancing and rock music. Pretty soon, Ren is the rebel with a cause, winning over the hearts and feet of the whole community. Level Placement If you are unsure of what level to register for, an assessment can be set up call 630-784-6105 to schedule a time. Youth Leagues Meet and Greet The meet and greet is a set time where players, parents and coaches meet each other and hand out any necessary information and in some cases, receive their uniforms. Parent Code of Conduct By registering your child you agree to comply with the Parent Code of Conduct which can be found on the home webpage of each youth sports league and is discussed by the coach at the preseason parent meeting. Placed on a Specific Team No, due to the nature of the program, we cannot accept any other coach, player, or carpool requests. However, one mutual friendship request is honored for players in an instructional league, Pinto (8U) baseball, or U8 soccer. Forms are available at csparks.org/youthsports-friendshiprequest/ Request deadlines are the same as registration deadlines (not early bird).

Playing up Playing up is not allowed for instructional soccer or tee ball. For all other leagues, playing up is allowed by one age or grade level but the following process must be adhered to: The player must register at his or her age or grade. A request to play up must be made to the program supervisor. The player must attend the evaluation day for the sport and must attend at the time designated for the requested league/level (i.e. if a player should be playing in the Mustang league but wants to play in the Major league, player must attend the Major player evaluation). Registration Deadlines Players placed on a waitlist are not guaranteed placement on a team. If you register before team formation, the player is moved off the waitlist if there are enough to form an additional team. However, if you register after team formation or if another team cannot be formed, placement will only be done when a team or teams are below the established maximum number of team participants. Players will not be added to any team one week prior to the first scheduled game.

Player Evaluation Players get rated based on each skill set for that particular sport. All players are welcome but new players and players who are playing up a level are required to attend. Registration Select a swim class based on student’s ability to swim, comfort level and age. Returning students select a class based on previous swim lesson report card. Registration ends 5 business days prior to the first day of class. Parent Attendance Parents are expected to stay in designated viewing area during the duration of student’s class in case of emergency where consent is required. Parent Tot 6 months-2 yrs

Parents assist with teaching their little swimmers comfort in water. Instruction includes: water adjustment, entering and exiting water, putting face in water, blowing bubbles, supported floating and jumping in with assistance. Swimmers not potty trained must wear swimsuit with swim diaper and tight fitting rubber/plastic pants.

Terrific Tot Swim 3-4 yrs

Swimmers learn to enter/exit water with confidence. Instruction includes supported front/back floats, blowing bubbles, putting face in water, alternation of arms, front/back kicks, back glide with kicks and water safety. Swimmers not potty trained must wear swimsuit with swim diaper and tight fitting rubber/plastic pants.

Wave Classes 5-13 yrs

To confirm all students are placed in the correct class, within the first 15 minutes, students are evaluated then placed in their class for a secondary evaluation. During this time, students are moved to different parts of the pool with their teachers. During the second class all students receive another evaluation to confirm their correct placement. After the second class, students cannot switch classes until the following session. Classes are held in both the Therapy or Lap pool. It is common for students to complete multiple sessions in the Wave A curriculum before advancing. Consider this when registering for multiple classes. Register for the day and time that fits your needs and teacher will evaluate and place students in the appropriate class.

Wave A

The Wave A curriculum provides your child with the skills needed to survive in the water and provide the foundation for a life-long love of swimming. This curriculum includes Class 1-Water Exploration, Class 2-Primary Skills and Class 3-Stroke Readiness.

Wave B

The Wave B curriculum develops your child’s swimming techniques and provides the foundation needed for competitive swimming or recreational swimming for fitness. Wave A (or its equivalent) is a prerequisite for this program. Wave B includes Class 4-Stroke Development, Class 5-Stroke Refinement and Class 6-Stroke Proficiency.

Full payment for most programs is due at the time of registration. Payment may be made by cash, check or credit card (Visa/MC/AMEX/DISC). Make checks payable to the Carol Stream Park District. There is a $20 fee for returned checks. Activities with recurring or monthly payments must have an EZ Pay Agreement on file. Your payments are charged to your credit/debit card. For more information, please visit our Registration Services Team at Simkus or Fountain View Recreation Centers to complete an EZ Pay authorization form.

Photography Policy

Carol Stream Park occasionally takes photographs and videos of participants at our parks or facilities, in park district programs, classes or events for marketing purposes.

Waiting Lists

Once the maximum enrollment is met for a class, a waiting list is formed. If space becomes available or additional classes are created, you will be contacted. Payment is due when being waitlisted. Credits are issued after the first class for those who are not able to be registered.

Inclusion Services

Inclusion Services are designed to provide the least restrictive environment while providing the maximum opportunity to participate in regular park district programs. There is no charge for WDSRA’s support. To register for services, simply indicate that special accommodations are needed on the park district registration form. Two-week advance notice is requested for inclusion support.

All programs have minimum and maximum enrollments. If the minimum enrollment is not met one week prior to the program date, the Park District may have to cancel, postpone or consolidate the program with another program. If the program is cancelled, full credits are issued.

The Park District does not carry medical or accident insurance for program participants or park patrons. Review your personal health insurance plan to be certain that you and your family have proper coverage. We offer financial assistance/scholarships for qualified Carol Stream Park District residents. Confidential applications available at Fountain View Fitness Center and Simkus Recreation Center.

Smoking Policy

The District’s philosophy is to promote health and a clean air environment. The use of any and all tobacco products is prohibited in all park district buildings and facilities and at all parks and trails. Carol Stream Park District joins other park districts in Illinois with a tobacco-free policy. To read the full policy, go to csparks.org.

Refund and Withdrawal Policy

It is the District’s intent to provide quality recreation programs and services. However, the District recognizes that there may be occasions when the event/service doesn’t match a customer’s needs, or when the customer is unable to participate in an activity. For those occasions, the following withdrawal policy will apply. • Participant will have two options: o Have the original registration fee transferred to a gift certificate. There is no expiration date on the certificate. o Request a refund at time of withdrawal - all requests for refunds will be assessed a ‘Service Fee’ of 10% of the original, total registration fee - $5 minimum / $50 maximum.’ o Refunds under $5 may only be issued as a gift certificate; minimum value of gift certificate is $0.01. Exceptions in which a Withdrawal Service Fee will not be assessed: o Medical note exception is valid any time prior to the start of the last class and will not be subject to a ‘Service Fee’; a prorated refund or credit will be issued. o For monthly/quarterly payments, or memberships, there is no refund of the prior or current period; a cancellation of the future months’ charges will be allowed. o Forever Young Programs – Service Fee will apply after the payment deadline, and refunds may be issued prior to the event as long as Supervisor approval is given. No refunds after event. No Refunds are offered: • for annual memberships including but not limited to: Walking Track, Lap Swim, Coral Cove Water Park, or Bark Park • if participant did not attend all or any portion of the class/program • no refunds after 2nd meeting of class/program • for Summer Camp, ActivKids, or Preschool because participant’s vacation, illness, or any other absence • for returned (new or used) uniforms or costumes • for Enrollment, Administration, Registration, or Annual Fees • for Punch Passes (that have no expiration date) o includes Pool, Open Gymnastics, Open Baton • for “Private” Recreational Packages o Personal Fitness Packages, Customized/Private Swim Lesson Packages, Private Voice, Private Gymnastics Lessons • for trips – no refund unless customer provides a replacement; if replacement is found, Service Fee will still be assessed to the original participant • for Adult Athletic Leagues –if a team chooses to drop out Additional Notes: No prorated registration, enrollment, or administration fee for late registrants.

Meet Your Park Board of Commissioners The Carol Stream Park District Board of Commissioners consists of seven individuals who are elected by residents to represent the community’s best interests. The charge of the Board is to set policies that guide the Park District. Board members serve a four-year term and receive no compensation for the time they give. They come from a variety of backgrounds and share one important trait ― a commitment to serving the park and recreational needs of our community.

Regularly scheduled meetings of the Board are held on the second and/or the fourth Monday of each month according to a schedule adopted by the Board yearly.
